KVM: s390: vsie: Fix region 1 ASCE sanity shadow address checks
In case we have a region 1 the following calculation
(31 + ((gmap->asce & _ASCE_TYPE_MASK) >> 2)*11)
results in 64. As shifts beyond the size are undefined the compiler is
free to use instructions like sllg. sllg will only use 6 bits of the
shift value (here 64) resulting in no shift at all. That means that ALL
addresses will be rejected.
The can result in endless loops, e.g. when prefix cannot get mapped.

mm: allow VM_FAULT_RETRY for multiple times
The idea comes from a discussion between Linus and Andrea [1]. Before this patch we only allow a page fault to retry once. We achieved this by clearing the FAULT_FLAG_ALLOW_RETRY flag when doing handle_mm_fault() the second time. This was majorly used to avoid unexpected starvation of the system by looping over forever to handle the page fault on a single page. However that should hardly happen, and after all for each code path to return a VM_FAULT_RETRY we'll first wait for a condition (during which time we should possibly yield the cpu) to happen before VM_FAULT_RETRY is really returned. This patch removes the restriction by keeping the FAULT_FLAG_ALLOW_RETRY flag when we receive VM_FAULT_RETRY. It means that the page fault handler now can retry the page fault for multiple times if necessary without the need to generate another page fault event. Meanwhile we still keep the FAULT_FLAG_TRIED flag so page fault handler can still identify whether a page fault is the first attempt or not. Then we'll have these combinations of fault flags (only considering ALLOW_RETRY flag and TRIED flag): - ALLOW_RETRY and !TRIED: this means the page fault allows to retry, and this is the first try - ALLOW_RETRY and TRIED: this means the page fault allows to retry, and this is not the first try - !ALLOW_RETRY and !TRIED: this means the page fault does not allow to retry at all - !ALLOW_RETRY and TRIED: this is forbidden and should never be used In existing code we have multiple places that has taken special care of the first condition above by checking against (fault_flags & FAULT_FLAG_ALLOW_RETRY). This patch introduces a simple helper to detect the first retry of a page fault by checking against both (fault_flags & FAULT_FLAG_ALLOW_RETRY) and !(fault_flag & FAULT_FLAG_TRIED) because now even the 2nd try will have the ALLOW_RETRY set, then use that helper in all existing special paths. One example is in __lock_page_or_retry(), now we'll drop the mmap_sem only in the first attempt of page fault and we'll keep it in follow up retries, so old locking behavior will be retained. This will be a nice enhancement for current code [2] at the same time a supporting material for the future userfaultfd-writeprotect work, since in that work there will always be an explicit userfault writeprotect retry for protected pages, and if that cannot resolve the page fault (e.g., when userfaultfd-writeprotect is used in conjunction with swapped pages) then we'll possibly need a 3rd retry of the page fault. It might also benefit other potential users who will have similar requirement like userfault write-protection. GUP code is not touched yet and will be covered in follow up patch. mm: introduce FAULT_FLAG_DEFAULT
Although there're tons of arch-specific page fault handlers, most of them are still sharing the same initial value of the page fault flags. Say, merely all of the page fault handlers would allow the fault to be retried, and they also allow the fault to respond to SIGKILL. Let's define a default value for the fault flags to replace those initial page fault flags that were copied over. With this, it'll be far easier to introduce new fault flag that can be used by all the architectures instead of touching all the archs. mm: introduce fault_signal_pending()
For most architectures, we've got a quick path to detect fatal signal after a handle_mm_fault(). Introduce a helper for that quick path. It cleans the current codes a bit so we don't need to duplicate the same check across archs. More importantly, this will be an unified place that we handle the signal immediately right after an interrupted page fault, so it'll be much easier for us if we want to change the behavior of handling signals later on for all the archs. Note that currently only part of the archs are using this new helper, because some archs have their own way to handle signals. In the follow up patches, we'll try to apply this helper to all the rest of archs. Another note is that the "regs" parameter in the new helper is not used yet. It'll be used very soon. Now we kept it in this patch only to avoid touching all the archs again in the follow up patches. s390/gmap: return proper error code on ksm unsharing
If a signal is pending we might return -ENOMEM instead of -EINTR.
We should propagate the proper error during KSM unsharing.
unmerge_ksm_pages returns -ERESTARTSYS on signal_pending. This gets
translated by entry.S to -EINTR. It is important to get this error
code so that userspace can retry.
To make this clearer we also add -EINTR to the documentation of the
PV_ENABLE call, which calls unmerge_ksm_pages.

KVM: s390/mm: Make pages accessible before destroying the guest
Before we destroy the secure configuration, we better make all pages accessible again. This also happens during reboot, where we reboot into a non-secure guest that then can go again into secure mode. As this "new" secure guest will have a new ID we cannot reuse the old page state. KVM: s390: protvirt: Secure memory is not mergeable
KSM will not work on secure pages, because when the kernel reads a secure page, it will be encrypted and hence no two pages will look the same. Let's mark the guest pages as unmergeable when we transition to secure mode. s390/kasan: add KASAN_VMALLOC support
Add KASAN_VMALLOC support which now enables vmalloc memory area access checks as well as enables usage of VMAP_STACK under kasan. KASAN_VMALLOC changes the way vmalloc and modules areas shadow memory is handled. With this new approach only top level page tables are pre-populated and lower levels are filled dynamically upon memory allocation. s390/kasan: support memcpy_real with TRACE_IRQFLAGS
Currently if the kernel is built with CONFIG_TRACE_IRQFLAGS and KASAN and used as crash kernel it crashes itself due to trace_hardirqs_off/trace_hardirqs_on being called with DAT off. This happens because trace_hardirqs_off/trace_hardirqs_on are instrumented and kasan code tries to perform access to shadow memory to validate memory accesses. Kasan shadow memory is populated with vmemmap, so all accesses require DAT on. memcpy_real could be called with DAT on or off (with kasan enabled DAT is set even before early code is executed). Make sure that trace_hardirqs_off/trace_hardirqs_on are called with DAT on and only actual __memcpy_real is called with DAT off. Also annotate __memcpy_real and _memcpy_real with __no_sanitize_address to avoid further problems due to switching DAT off. The problem is that we were putting the NUL terminator too far: buf[sizeof(buf) - 1] = '\0'; If the user input isn't NUL terminated and they haven't initialized the whole buffer then it leads to an info leak. mm: treewide: clarify pgtable_page_{ctor,dtor}() naming
The naming of pgtable_page_{ctor,dtor}() seems to have confused a few people, and until recently arm64 used these erroneously/pointlessly for other levels of page table. To make it incredibly clear that these only apply to the PTE level, and to align with the naming of pgtable_pmd_page_{ctor,dtor}(), let's rename them to pgtable_pte_page_{ctor,dtor}(). pagewalk: separate function pointers from iterator data

s390/kasan: add kdump support
If kasan enabled kernel is used as crash kernel it crashes itself with program check loop during kdump execution. The reason for that is that kasan shadow memory backed by pages beyond OLDMEM_SIZE. s390/cmma: reuse kstrtobool for option value parsing
"cmma" option setup already recognises some textual values. Yet kstrtobool is a more common way to parse boolean values, reuse it to unify option value parsing behavior and simplify code a bit. While at it, __setup value parsing callbacks are expected to return 1 when an option is recognized, and returning any other value won't trigger any error message currently, so simply return 1. mm/memory_hotplug: allow arch_remove_memory() without CONFIG_MEMORY_HOTREMOVE
We want to improve error handling while adding memory by allowing to use arch_remove_memory() and __remove_pages() even if CONFIG_MEMORY_HOTREMOVE is not set to e.g., implement something like: arch_add_memory() rc = do_something(); if (rc) { arch_remove_memory(); } We won't get rid of CONFIG_MEMORY_HOTREMOVE for now, as it will require quite some dependencies for memory offlining. mm, kprobes: generalize and rename notify_page_fault() as kprobe_page_fault()
Architectures which support kprobes have very similar boilerplate around
calling kprobe_fault_handler(). Use a helper function in kprobes.h to
unify them, based on the x86 code.
This changes the behaviour for other architectures when preemption is
enabled. Previously, they would have disabled preemption while calling
the kprobe handler. However, preemption would be disabled if this fault
was due to a kprobe, so we know the fault was not due to a kprobe
handler and can simply return failure.

The force_sig function is built for delivering synchronous signals such as SIGSEGV where the userspace application caused a synchronous fault (such as a page fault) and the kernel responded with a signal. Because the name force_sig does not make this clear, and because the force_sig takes a task parameter the function force_sig has been abused for sending other kinds of signals over the years. s390 updates for the 5.3 merge window
- Improve stop_machine wait logic: replace cpu_relax_yield call in generic stop_machine function with a weak stop_machine_yield function. This is overridden on s390, which yields the current cpu to the neighbouring cpu after a couple of retries, instead of blindly giving up the cpu to the hipervisor. This significantly improves stop_machine performance on s390 in overcommitted scenarios. s390/mm: force swiotlb for protected virtualization
On s390, protected virtualization guests have to use bounced I/O buffers. That requires some plumbing. Let us make sure, any device that uses DMA API with direct ops correctly is spared from the problems, that a hypervisor attempting I/O to a non-shared page would bring. randomize_stack_top() checks for current task flag PF_RANDOMIZE in order
to use stack randomization and PF_RANDOMIZE is set when
ADDR_NO_RANDOMIZE is unset, so no need to check for ADDR_NO_RANDOMIZE
in stack_maxrandom_size.

s390/jump_label: replace stop_machine with smp_call_function
The use of stop_machine to replace the mask bits of the jump label branch is a very heavy-weight operation. This is in fact not necessary, the mask of the branch can simply be updated, followed by a signal processor to all the other CPUs to force them to pick up the modified instruction. signal: Remove the task parameter from force_sig_fault
As synchronous exceptions really only make sense against the current task (otherwise how are you synchronous) remove the task parameter from from force_sig_fault to make it explicit that is what is going on. The two known exceptions that deliver a synchronous exception to a stopped ptraced task have already been changed to force_sig_fault_to_task. If get_fault_type() is not inlined, the compiler cannot deduce that
all the possible paths in the 'switch' statement are covered.
Of course, we could mark get_fault_type() as __always_inline to get
back the original behavior, but I do not think it sensible to force
inlining just for the purpose of suppressing the warning. Since this
is just a matter of warning, I want to keep as much room for compiler
optimization as possible.
I added unreachable() to teach the compiler that the 'default' label
is unreachable.
I got rid of the 'inline' marker. Even without the 'inline' hint,
the compiler inlines functions based on its inlining heuristic.
