afd13a1f2b [SCSI] isci: update afe (analog-front-end) recipe for C1
C1 silicon requires updates to the phy tuning recipe and also support
for user provided cable selects (per-phy) for short, medium, and long
cables. Default to 'short' awaiting support for selecting the cable via
oem parameters.

0eecee413d [SCSI] scsi_transport_fc: Clear Devloss Callback Done flag in fc_remote_port_rolechg
This patch fixes a bug where devloss is not called on fc_host teardown.
The issue is seen if the LLDD uses rport_rolechg to add the target role
to an rport.

When an rport goes away, the LLDD will call fc_remote_port_delete, which
will start the devloss timer. If the timer expires, the transport will
call the devloss callback and set the FC_RPORT_DEVLOSS_CALLBK_DONE flag.
However, the rport structure is not deleted, it is retained to store the
SCSI id mappings for the rport in case it comes back. In the scenario
where it does come back, and the driver calls fc_remote_port_add, but does
not indicate the "target" role for the rport - the create will clear the
structure, but forgets to clear FC_RPORT_DEVLOSS_CALLBK_DONE flag (which
is cleared if it's added with the target role). The secondary call, of
fc_remote_port_rolechg to add the target role also does not clear the flag.
Thus, the next time the rport goes away, the resulting devloss timer
expiration will not call the driver callback as the flag is still set.

This patch adds the FC_RPORT_DEVLOSS_CALLBK_DONE flags to the list of
those that are cleared upon reuse of the rport structure.

f4b1952a45 [media] dvb-core: fix a regression with MythTV
As reported by Lawrence[1], MythTV 0.24.1 does the wrong thing
with a DVBv5 call: it fills the delivery system with
SYS_UNDEFINED, expecting that the DVB core would work with that.

This used to work by accident, as the DVB core were missing the
check for the supported delivery systems. Yet, fixing it
is easy, so let's add a logic to handle this case, to
provide backward compatibility.

ec8013bedd dm: do not forward ioctls from logical volumes to the underlying device
A logical volume can map to just part of underlying physical volume.
In this case, it must be treated like a partition.

0bfc96cb77 block: fail SCSI passthrough ioctls on partition devices
Linux allows executing the SG_IO ioctl on a partition or LVM volume, and
will pass the command to the underlying block device. This is
well-known, but it is also a large security problem when (via Unix
permissions, ACLs, SELinux or a combination thereof) a program or user
needs to be granted access only to part of the disk.

This patch lets partitions forward a small set of harmless ioctls;
others are logged with printk so that we can see which ioctls are
actually sent. In my tests only CDROM_GET_CAPABILITY actually occurred.
Of course it was being sent to a (partition on a) hard disk, so it would
have failed with ENOTTY and the patch isn't changing anything in
practice. Still, I'm treating it specially to avoid spamming the logs.

In principle, this restriction should include programs running with
CAP_SYS_RAWIO. If for example I let a program access /dev/sda2 and
/dev/sdb, it still should not be able to read/write outside the
boundaries of /dev/sda2 independent of the capabilities. However, for
now programs with CAP_SYS_RAWIO will still be allowed to send the
ioctls. Their actions will still be logged.

This patch does not affect the non-libata IDE driver. That driver
however already tests for bd != bd->bd_contains before issuing some
ioctl; it could be restricted further to forbid these ioctls even for
programs running with CAP_SYS_ADMIN/CAP_SYS_RAWIO.

f408c985ce GPIO: sa1100: implement proper gpiolib gpio_to_irq conversion
The existing gpio_to_irq() implementation on sa1100 only translates
validly for internal GPIOs. Since this sub-arch enables GPIOLIB

Get rid of the private gpio_to_irq() implementation, replacing it
with the .to_irq method in the sa1100 gpio chip instead.

dfd00c4c8f rt2800pci: fix spurious interrupts generation
Same devices can generate interrupt without properly setting bit in
INT_SOURCE_CSR register (spurious interrupt), what will cause IRQ line
will be disabled by interrupts controller driver.

We discovered that clearing INT_MASK_CSR stops such behaviour. We
previously first read that register, and then clear all know interrupt
sources bits and do not touch reserved bits. After this patch, we write
to all register content (I believe writing to reserved bits on that
register will not cause any problems, I tested that on my rt2800pci
device).

This fix very bad performance problem, practically making device
unusable (since worked without interrupts), reported in:
https://bugzilla.redhat.com/show_bug.cgi?id=658451

We previously tried to workaround that issue in commit
4ba7d99978 "rt2800pci: handle spurious
interrupts", but it was reverted in commit
82e5fc2a34
as thing, that will prevent to detect real spurious interrupts.

92c1ff1fa7 ipw2x00: signedness bug handling frame length
This is basically just a cleanup. Large positive numbers get counted as
negative but then get implicitly cast to positive again for the checks
that matter.

This does make a small difference in ipw_handle_promiscuous_rx() when we
test "if (unlikely((len + IPW_RX_FRAME_SIZE) > skb_tailroom(rxb->skb)))"
It should return there, but we don't return until a couple lines later
when we test "if (len > IPW_RX_BUF_SIZE - sizeof(struct ipw_rt_hdr)) {".
The difference is that in the second test the sizeof() means that there
is an implied cast to unsigned.

At the moment we cannot allocate more than stolen memory size for framebuffers.
To get around that issues we discard modes that doesn't fit. This is a temporary

The big WARN() has nothing to do with the culprit - which is that

099469502f Merge branch 'akpm' (aka "Andrew's patch-bomb, take two")
Andrew explains:

 - various misc stuff

 - Most of the rest of MM: memcg, threaded hugepages, others.

 - cpumask

 - kexec

 - kdump

 - some direct-io performance tweaking

 - radix-tree optimisations

 - new selftests code

A note on this: often people will develop a new userspace-visible
feature and will develop userspace code to exercise/test that
feature. Then they merge the patch and the selftest code dies.
Sometimes we paste it into the changelog. Sometimes the code gets
   thrown into Documentation/(!).

This saddens me. So this patch creates a bare-bones framework which
will henceforth allow me to ask people to include their test apps in
the kernel tree so we can keep them alive. Then when people enhance
or fix the feature, I can ask them to update the test app too.

The infrastruture is terribly trivial at present - let's see how it
   evolves.

 - checkpoint/restart feature work.

A note on this: this is a project by various mad Russians to perform
c/r mainly from userspace, with various oddball helper code added
into the kernel where the need is demonstrated.

So rather than some large central lump of code, what we have is
little bits and pieces popping up in various places which either
expose something new or which permit something which is normally
   kernel-private to be modified.

The overall project is an ongoing thing. I've judged that the size
and scope of the thing means that we're more likely to be successful
with it if we integrate the support into mainline piecemeal rather
than allowing it all to develop out-of-tree.

However I'm less confident than the developers that it will all
eventually work! So what I'm asking them to do is to wrap each piece
of new code inside CONFIG_CHECKPOINT_RESTORE. So if it all
eventually comes to tears and the project as a whole fails, it should
be a simple matter to go through and delete all trace of it.

This lot pretty much wraps up the -rc1 merge for me.

f5138e4221 kdump: add udev events for memory online/offline
Currently no udev events for memory hotplug "online" and "offline" are
generated:

  # udevadm monitor
  # echo offline > /sys/devices/system/memory/memory4/state
  ==> No event

When kdump is loaded, kexec detects the current memory configuration and
stores it in the pre-allocated ELF core header. Therefore, for kdump it
is necessary to reload the kdump kernel with kexec when the memory
configuration changes (e.g. for online/offline hotplug memory).

In order to do this automatically, udev rules should be used. This kernel
patch adds udev events for "online" and "offline". Together with this
kernel patch, the following udev rules for online/offline have to be added
to "/etc/udev/rules.d/98-kexec.rules":

  SUBSYSTEM=="memory", ACTION=="online", PROGRAM="/etc/init.d/kdump restart"
  SUBSYSTEM=="memory", ACTION=="offline", PROGRAM="/etc/init.d/kdump restart"

9c4886e5e6 gianfar: Fix invalid TX frames returned on error queue when time stamping
When TX time stamping for PTP messages is enabled on a socket, a time
stamp is returned on the socket error queue to the user space application
after the frame was transmitted. The transmitted frame is also returned on
the error queue so that an application knows to which frame the time stamp
belongs.

In the current implementation the TxFCB is immediately followed by the
frame. Since the eTSEC inserts the TX time stamp 8 bytes after the TxFCB,
parts of the frame have been overwritten and an invalid frame was returned
on the socket error queue.

This patch fixes the described problem by adding additional 16 padding
bytes between the TxFCB and the frame for all messages sent from a time
stamping enabled socket (other sockets are not affected).

Introduce function mdiobus_alloc_size() as an alternative to mdiobus_alloc().
Most callers of mdiobus_alloc() also allocate a private data structure, and

69116f279a module_param: avoid bool abuse, add bint for special cases.
For historical reasons, we allow module_param(bool) to take an int (or
an unsigned int). That's going away.

A few drivers really want an int: they set it to -1 and a parameter
will set it to 0 or 1. This sucks: reading them from sysfs will give
'Y' for both -1 and 1, but if we change it to an int, then the users
might be broken (if they did "param" instead of "param=1").

Use a new 'bint' parser for them.

(ntfs has a different problem: it needs an int for debug_msgs because
it's also exposed via sysctl.)

61bd5e5683 brcmsmac: fix reading of PCI sprom contents
It appears that you can only read the sprom contents with aligned 16-bit
reads: anything else causes at least some versions of the broadcom
chipset to abort the PCI transaction, returning 0xff.

This apparently doesn't trigger very often, because most setups don't
use an external srom chip, and the OTP sprom loading doesn't have this
issue. But at least the current 11" Macbook Air does trigger it, and
wireless communications were broken as a result.

0db13fc2ab mmc: fix a deadlock between system suspend and MMC block IO
Performing MMC block IO with simultaneous STR can lead to a deadlock: the
mmc_pm_notify() function claims the host and then calls bus .remove()
method, which lands in mmc_blk_remove(), which calls mmc_blk_remove_req()
then it goes to -> mmc_cleanup_queue() -> kthread_stop(), which waits for
the mmc-block thread to stop. If the mmc-block thread at that time is
processing block requests, it will also try to claim the host in
mmc_blk_issue_rq() and block there. This patch fixes the problem by
calling .remove() before claiming the host.

3abc1e8042 mmc: sdhci: restore the enabled dma when do reset all
If dma is enabled, it'll be cleared when reset all is performed, this can
be observed on some platforms, such as P2041 which has a version 2.3

e562966dba virtio: balloon: Add freeze, restore handlers to support S4
Handling balloon hibernate / restore is tricky. If the balloon was
inflated before going into the hibernation state, upon resume, the host
will not have any memory of that. Any pages that were passed on to the
host earlier would most likely be invalid, and the host will have to
re-balloon to the previous value to get in the pre-hibernate state.

So the only sane thing for the guest to do here is to discard all the
pages that were put in the balloon. When to discard the pages is the
next question.

One solution is to deflate the balloon just before writing the image to
the disk (in the freeze() PM callback). However, asking for pages from
the host just to discard them immediately after seems wasteful of
resources. Hence, it makes sense to do this by just fudging our
counters soon after wakeup. This means we don't deflate the balloon
before sleep, and also don't put unnecessary pressure on the host.

This also helps in the thaw case: if the freeze fails for whatever
reason, the balloon should continue to remain in the inflated state.
This was tested by issuing 'swapoff -a' and trying to go into the S4
state. That fails, and the balloon stays inflated, as expected. Both
the host and the guest are happy.

Finally, in the restore() callback, we empty the list of pages that were
previously given off to the host, add the appropriate number of pages to
the totalram_pages counter, reset the num_pages counter to 0, and
all is fine.

As a last step, delete the vqs on the freeze callback to prepare for
hibernation, and re-create them in the restore and thaw callbacks to
resume normal operation.

The kthread doesn't race with any operations here, since it's frozen
before the freeze() call and is thawed after the thaw() and restore()
callbacks, so we're safe with that.

4678d6f970 virtio_blk: fix config handler race
Fix a theoretical race related to config work
handler: a config interrupt might happen
after we flush config work but before we
reset the device. It will then cause the
config work to run during or after reset.

Two problems with this:
- if this runs after device is gone we will get use after free
- access of config while reset is in progress is racy
(as layout is changing).

As a solution
1. flush after reset when we know there will be no more interrupts
2. add a flag to disable config access before reset

ee7cd8981e virtio: expose added descriptors immediately.
A virtio driver does virtqueue_add_buf() multiple times before finally
calling virtqueue_kick(); previously we only exposed the added buffers
in the virtqueue_kick() call. This means we don't need a memory
barrier in virtqueue_add_buf(), but it reduces concurrency as the
device (ie. host) can't see the buffers until the kick.

In the unusual (but now possible) case where a driver does add_buf()
and get_buf() without doing a kick, we do need to insert one before
our counter wraps. Otherwise we could wrap num_added, and later on
not realize that we have passed the marker where we should have
kicked.

6601056a96 mmc: Remove redundant spi driver bus initialization
In ancient times it was necessary to manually initialize the bus field of an
spi_driver to spi_bus_type. These days this is done in spi_driver_register(),
so we can drop the manual assignment.

The patch was generated using the following coccinelle semantic patch:
// <smpl>

597dd9d79c mmc: core: Use delayed work in clock gating framework
Current clock gating framework disables the MCI clock as soon as the
request is completed and enables it when a request arrives. This aggressive
clock gating framework, when enabled, cause following issues:

When there are back-to-back requests from the Queue layer, we unnecessarily
end up disabling and enabling the clocks between these requests since 8MCLK
clock cycles is a very short duration compared to the time delay between
back to back requests reaching the MMC layer. This overhead can effect the
overall performance depending on how long the clock enable and disable
calls take which is platform dependent. For example on some platforms we
can have clock control not on the local processor, but on a different
subsystem and the time taken to perform the clock enable/disable can add
significant overhead.

Also if the host controller driver decides to disable the host clock too
when mmc_set_ios function is called with ios.clock=0, it adds additional
delay and it is highly possible that the next request had already arrived
and unnecessarily blocked in enabling the clocks. This is seen frequently
when the processor is executing at high speeds and in multi-core platforms
thus reduces the overall throughput compared to if clock gating is
disabled.

Fix this by delaying turning off the clocks by posting request on
delayed workqueue. Also cancel the unscheduled pending work, if any,
when there is access to card.

sysfs entry is provided to tune the delay as needed, default
value set to 200ms.

052d81da6e mmc: sdio: Fix to support any block size optimally
This patch allows any block size to be set on the SDIO link,
and still have an arbitrary sized packet (adjusted in size by
using sdio_align_size) transferred in an optimal way
(preferably one transfer).

Previously if the block size was larger than the default of
512 bytes and the transfer size was exactly one block size
(possibly thanks to using sdio_align_size to get an optimal
transfer size), it was sent as a number of byte transfers instead
of one block transfer. Also if the number of blocks was
(max_blocks * N) + 1, the tranfer would be conducted with a number
of blocks and finished off with a number of byte transfers.

When doing this change it was also possible to break out the quirk
for broken byte mode in a much cleaner way, and collect the logic of
when to do byte or block transfer in one function instead of two.

72f0d453d8 UBI: fix debugging messages
Patch ab50ff6847 broke UBI debugging messages:
before that commit when UBI debugging was enabled, users saw few useful
debugging messages after attaching an MTD device. However, that patch turned
'dbg_msg()' into 'pr_debug()', so to enable the debugging messages users have
to enable them first via /sys/kernel/debug/dynamic_debug/control, which is
very impractical.

This commit makes 'dbg_msg()' to use 'printk()' instead of 'pr_debug()', just
as it was before the breakage.

b1bd055d39 block: Introduce blk_set_stacking_limits function
Stacking driver queue limits are typically bounded exclusively by the
capabilities of the low level devices, not by the stacking driver
itself.

This patch introduces blk_set_stacking_limits() which has more liberal
metrics than the default queue limits function. This allows us to
inherit topology parameters from bottom devices without manually
tweaking the default limits in each driver prior to calling the stacking
function.

Since there is now a clear distinction between stacking and low-level
devices, blk_set_default_limits() has been modified to carry the more
conservative values that we used to manually set in
blk_queue_make_request().

ebe967492c mb86a20s: Add a few more register settings at the init seq
Some time ago, Manoel sent us a patch adding more stuff
to the init sequence. However, his patch were also doing
non-related stuff, by changing the init logic without
any good reason. So, it was asked for him to submit a
patch with just the data that has changed, in order to
allow us to better analyze it.

As he didn't what it was requested, I finally found some
time to dig into his init sequence and add it here.

Basically, new stuff is added there. There are a few changes:

1) The removal of the extra (duplicated) logic that puts
   the chip into the serial mode;
2) Some Viterbi VBER measurement init data was changed from
0x00 to 0xff for layer A, to match what was done for
   layers B and C.

None of those caused any regressions and both make sense
on my eyes.

The other parameters additions actually increased the
tuning quality for some channels. Yet, some channels that
were previously discovered with scan disappered, while
others appeared instead. This were tested in Brasilia,
with an external antena.

At the overall, it is now a little better. So, better to
add these, and then try to figure out a configuration that
would get even better scanning results.

d07b901f9d [media] af9005, af9015: use symbolic names for USB id table indices
The af9005_properties and af9015_properties tables make use of USB ids
from the USB id tables with hardcoded indices, as in
"&af9015_usb_table[30]". Adding new entries before the end breaks
such references, so everyone has had to carefully tiptoe to only add
entries at the end of the list.

In the spirit of "dw2102: use symbolic names for dw2102_table
indices", use C99-style initializers with symbolic names for each
index to avoid this. In the new regime, properties tables referring
to the USB ids have names like "&af9015_usb_table[CINERGY_T_STICK_RC]"
that do not change meaning when items in the USB id table are
reordered.

7a6f6c29d2 [media] cx231xx: use URB_NO_TRANSFER_DMA_MAP on URBs allocated with usb_alloc_urb()
URBs allocated with usb_alloc_urb() are allocated from DMA-coherent
areas, and therefore it is not necessary to call dma_map_single() on
such buffers. Worst, on ARM, calling dma_map_single() on a
DMA-coherent buffer will trigger a BUG_ON() in
arch/arm/mm/dma-mapping.c.

Therefore, we mark all URBs allocated with usb_alloc_urb() with the
URB_NO_TRANSFER_DMA_MAP transfer_flags, so that the USB core does not
do dma_map_single()/dma_unmap_single() on those buffers.

This is similar to 882787ff8f for the
gspca driver, and has already been discussed on the linux-media list

If, for some reason, the driver needs to wait for a close()
to happen, not all memories will be freed, because the free
logic were in the wrong place.

Also, v4l2_device_unregister() seems to be called too early,
as devices are still using it.

Finally, even with the device disconnected, there is one
USB function call that will still try to talk with it.

90bf3aab42 [media] cx231xx-input: stop polling if the device got removed.
If the device got removed, stops polling it. Also, un-registers
it at input/evdev, as it won't work anymore. We can't free the
IR structure yet, as the ir_remove method will be called later.
