330e2d61cd xhci: Avoid USB autosuspend when resuming USB2 ports.
When USB bus host controller root hub resumes from autosuspend,
it immediately tries to enter auto-suspend, but there can be a
scenario when root hub is resuming its usb2 ports, in that particular
case USB host controller auto suspend fails since it is busy
to resuming its usb2 ports.

This makes multiple failed cycles of auto-suspend until all usb2
ports of host controller root hub do not resume.

This patch uses USB core framework usb_hcd_start_port_resume,
usb_hcd_end_port_resume API's in order to  autoresume/autosuspend
root hub properly.

e1c3c7e54e usb: xhci: tegra: Firmware header is little endian
The XUSB firmware header is in little endian byte order, so make the
fields __le32 and __le16 instead of u32 and u16 to avoid warnings from
sparse when the fields are used with the endian-aware __le32_to_cpu()

2815ef7fe4 xhci-pci: allow host runtime PM as default for Intel Alpine and Titan Ridge
The xhci controller on Alpine and Titan Ridge keeps the whole thunderbolt
awake if the host controller is not allowed tp sleep.
This is the case even if no USB devices are connected to the host.

Because of this bigger impact, allow runtime pm as default for these xhci
controllers in the driver.

f8f80be501 xhci: Use soft retry to recover faster from transaction errors
Use soft retry to recover from a USB Transaction Errors that are caused by
temporary error conditions. The USB device is not aware that the xHC
has halted the endpoint, and will be waiting for another retry

A Soft Retry perform additional retries and recover from an error which has
caused the xHC to halt an endpoint.

Soft retry has some limitations:
Soft Retry attempts shall not be performed on Isoch endpoints
Soft Retry attempts shall not be performed if the device is behind a TT in
a HS Hub

Software shall limit the number of unsuccessful Soft Retry attempts to
prevent an infinite loop.

For more details on Soft retry see xhci specs  4.6.8.1

46216506ce usb: host: fotg2: Fix potential NULL dereference
There is code in the .remove() hook to handle the drvdata
being NULL, for good reasons: it is never set, so it will
always be NULL. As I moved code around, static checkers
start complaining.

Instead of this, make sure to always set it on successful
probe so we can always dereference it on the remove path.

Use the platform_device_[set|get]_drvdata() since this is
a platform device.

0a3b53305c usb: xhci: fix interrupt transfer error happened on MTK platforms
The MTK xHCI controller use some reserved bytes in endpoint context for
bandwidth scheduling, so need keep them in xhci_endpoint_copy();

The issue is introduced by:
commit f5249461b5 ("xhci: Clear the host side toggle manually when
endpoint is soft reset")
It resets endpoints and will drop bandwidth scheduling parameters used
by interrupt or isochronous endpoints on MTK xHCI controller.

87f88dfcde USB: OHCI: Remove USB bus reset delay from OHCI handover code
Paul pointed out that the 50-ms sleep during OHCI initialization takes
up a large fraction of a system's boot time.  Things get worse when
there are two OHCI controllers present, each requiring 50 ms.

However, there really is no need to send a 50-ms reset signal out all
the root-hub ports during initialization.  The ports themselves will
be disabled, and the only way to enable a port is to reset it.
Therefore all attached USB devices will receive a proper reset in any
case.  The controller reset does not need to be long enough to reset
those other devices, so the 50-ms delay isn't necessary.

Without the delay, there is no remaining incentive for skipping the
reset when the controller is already in the RESET state.  This patch
removes the test, issuing the command unconditionally, and removes the
following delay.

ffa8a31b5b usb: host: fotg2: add silicon clock handling
When used in a system with software-controlled silicon clocks,
the FOTG210 needs to grab, prepare and enable the clock.

This is needed on for example the Cortina Gemini, where the
platform will by default gate off the clock unless the
peripheral (in this case the USB driver) grabs and enables
the clock.

If there is no clock available on the platform, we live
without it. Make sure to percolate probe deferrals.

222471f764 usb: host: xhci-plat: Iterate over parent nodes for finding quirks
In xhci_plat_probe() both sysdev and pdev->dev are being used
for finding quirks. There are some drivers(like dwc3 host.c)
which adds quirks(like usb3-lpm-capable) into pdev and the logic
present in xhci_plat_probe() checks for quirks in either sysdev
or pdev for finding the quirks. Because of this logic, some of
the quirks are getting missed(usb3-lpm-capable quirk added by dwc3
host.c driver is getting missed).This patch fixes this by iterating
over all the available parents for finding the quirks. In this way
all the quirks which are present in child or parent are correctly
updated.

4937213ba7 xhci: Fix use after free for URB cancellation on a reallocated endpoint
Make sure the cancelled URB is on the current endpoint ring.

If the endpoint ring has been reallocated since the URB was enqueued
then the URB may contain TD and TRB pointers to a already freed ring.
In this the case return the URB without touching any of the freed ring
structure data.

Don't try to stop the ring. It would be useless.

This can occur if endpoint is not flushed before it is dropped and
re-added, which is the case in usb_set_interface() as xhci does
things in an odd order.

pm_runtime_put_sync() gets called everytime in xhci_dbc_stop().

If dbc is not started, this makes the runtime PM counter incorrectly

d850c16583 xhci: Fix kernel oops in trace_xhci_free_virt_device
commit 44a182b9d1 ("xhci: Fix use-after-free in xhci_free_virt_device")
set dev->udev pointer to NULL in xhci_free_dev(), it will cause kernel
panic in trace_xhci_free_virt_device. This patch reimplement the trace
function trace_xhci_free_virt_device, remove dev->udev dereference and
added more useful parameters to show in the trace function,it also makes
sure dev->udev is not NULL before calling trace_xhci_free_virt_device.
This issue happened when xhci-hcd trace is enabled and USB devices hot
plug test. Original use-after-free patch went to stable so this needs so
be applied there as well.

229bc19fd7 xhci: Fix perceived dead host due to runtime suspend race with event handler
Don't rely on event interrupt (EINT) bit alone to detect pending port
change in resume. If no change event is detected the host may be suspended
again, oterwise roothubs are resumed.

There is a lag in xHC setting EINT. If we don't notice the pending change
in resume, and the controller is runtime suspeded again, it causes the
event handler to assume host is dead as it will fail to read xHC registers
once PCI puts the controller to D3 state.

The EINT lag is described in a additional note in xhci specs 4.19.2:

"Due to internal xHC scheduling and system delays, there will be a lag
between a change bit being set and the Port Status Change Event that it
generated being written to the Event Ring. If SW reads the PORTSC and
sees a change bit set, there is no guarantee that the corresponding Port
Status Change Event has already been written into the Event Ring."

590b5b7d86 treewide: kzalloc_node() -> kcalloc_node()
The kzalloc_node() function has a 2-factor argument form, kcalloc_node(). This
patch replaces cases of:

        kzalloc_node(a * b, gfp, node)

with:
        kcalloc_node(a * b, gfp, node)

as well as handling cases of:

        kzalloc_node(a * b * c, gfp, node)

with:

        kzalloc_node(array3_size(a, b, c), gfp, node)

as it's slightly less ugly than:

        kcalloc_node(array_size(a, b), c, gfp, node)

This does, however, attempt to ignore constant size factors like:

        kzalloc_node(4 * 1024, gfp, node)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

6da2ec5605 treewide: kmalloc() -> kmalloc_array()
The kmalloc() function has a 2-factor argument form, kmalloc_array(). This
patch replaces cases of:

        kmalloc(a * b, gfp)

with:
        kmalloc_array(a * b, gfp)

as well as handling cases of:

        kmalloc(a * b * c, gfp)

with:

        kmalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kmalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kmalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

The tools/ directory was manually excluded, since it has its own
implementation of kmalloc().

12de0a35c9 xhci: Add quirk to zero 64bit registers on Renesas PCIe controllers
Some Renesas controllers get into a weird state if they are reset while
programmed with 64bit addresses (they will preserve the top half of the
address in internal, non visible registers).

You end up with half the address coming from the kernel, and the other
half coming from the firmware.

Also, changing the programming leads to extra accesses even if the
controller is supposed to be halted. The controller ends up with a fatal
fault, and is then ripe for being properly reset. On the flip side,
this is completely unsafe if the defvice isn't behind an IOMMU, so
we have to make sure that this is the case. Can you say "broken"?

This is an alternative method to the one introduced in 8466489ef5
("xhci: Reset Renesas uPD72020x USB controller for 32-bit DMA issue"),
which will subsequently be removed.

36b6857932 xhci: Allow more than 32 quirks
We now have 32 different quirks, and the field that holds them
is full. Let's bump it up to the next stage so that we can handle
some more... The type is now an unsigned long long, which is 64bit
on most architectures.

We take this opportunity to change the quirks from using (1 << x)
to BIT_ULL(x).

The USB Host Controller driver 'ohci-at91.c' reads a Special Function
Register - OHCI Interrupt Configuration Register (AT91_SFR_OHCIICR)
for bits SUSPEND_A/B/C. These bits are defined in sama5d2 alone, so
sfr register mapping is done with compatible string "atmel,sama5d2-sfr".
This gives a kernel warning 'failed to find sfr node' with non sama5d2
cpus which is removed here, thus leaving it up to having a proper DTS.

58c38116c6 usb: xhci: tegra: Add support for managing powergates
The Tegra XHCI controller requires that the XUSBA (for superspeed) and
XUSBC (for host) power-domains are enabled. Commit 8df127456f
("soc/tegra: pmc: Enable XUSB partitions on boot") was added to force
on these power-domains if the XHCI driver is enabled while proper
power-domain support is added, to ensure the device did not hang on
boot. However, rather than forcing on these power-domains in the PMC
driver we can use the legacy Tegra powergate APIs to turn on these
power-domains during the probe of the Tegra XHCI driver.

In the near future we plan to move the Tegra XHCI driver to use the
generic PM domain framework for power-domains and so to prepare for
this only use the legacy Tegra powergate API if there is not PM
domain associated with device (ie. dev.pm_domain is NULL). Please
note that in the future the superspeed and host resets will be handled
by the generic PM domain provider and so these are only these are only
needed in the case where there is no generic PM domain.

f468b55cb7 usb: xhci: tegra: Prepare for adding runtime PM support
When adding runtime PM support to the Tegra XHCI driver, it is desirable
to move the function calls to enable the clocks, regulators and PHY from
the tegra_xusb_probe into the runtime PM handlers. Currently, the
clocks, regulators and PHY are all enabled before we call
usb_create_hcd() in tegra_xusb_probe(), however, we cannot call
pm_runtime_get_sync() at this point because the platform device data is
not yet initialised. Fortunately, the function usb_create_hcd() can be
called before we enable the clocks, regulators and PHY and so prepare
for adding runtime PM support, by moving the call to usb_create_hcd()
before we enable the hardware.

Showing the content of the port status and control register for
each port (PORTSC)

Portxx is numbered starting from 1 for historical reasons to better
match port numbering shown by lsusb and other places.

Ports in debugfs are in the order XHC controller has them,
In most cases USB2 ports come first, followed by USB3 ports.
i.e. USB2 ports are port01-portxx, and USB3 portxx-portmax.

eaefcf246b xhci: change xhci_test_and_clear_bit() to use new port structure
Don't use pointers to port array and port index as function parameters
in xhci_test_and_clear_bit(), just use a pointer to the right port
structure.

xhci_test_and_clear_bit() was the last port_array user in

bcaa9d5c59 xhci: Create new structures to store xhci port information
Current way of having one array telling only the port speed,
and then two separate arrays with mmio addresses for usb2 and usb3 ports
requeres helper functions to transate  hw to hcd, and hcd to hw port
numbers, and is hard to expand.

Instead create a structure describing a port, including the mmio address,
the port hardware index, hcd port index, and a pointer to the roothub
it belongs to.

Create one array containing all port structures in the same order the
hardware controller sees them. Then add an array of port pointers to
each xhci hub structure pointing to the ports that belonging to the
roothub.

This way we can easily convert hw indexed port events to usb core
hcd port numbers, and vice versa usb core hub hcd port numbers
to hw index and mmio address.

Other benefit is that we can easily find the parent hcd and xhci
structure of a port structure. This is useful in debugfs where
we can give one port structure pointer as parameter and get both
the correct mmio address and xhci lock needed to set some port
parameter.

2278446e2b xhci: Fix USB3 NULL pointer dereference at logical disconnect.
Hub driver will try to disable a USB3 device twice at logical disconnect,
racing with xhci_free_dev() callback from the first port disable.

This can be triggered with "udisksctl power-off --block-device <disk>"
or by writing "1" to the "remove" sysfs file for a USB3 device
in 4.17-rc4.

USB3 devices don't have a similar disabled link state as USB2 devices,
and use a U3 suspended link state instead. In this state the port
is still enabled and connected.

hub_port_connect() first disconnects the device, then later it notices
that device is still enabled (due to U3 states) it will try to disable
the port again (set to U3).

The xhci_free_dev() called during device disable is async, so checking
for existing xhci->devs[i] when setting link state to U3 the second time
was successful, even if device was being freed.

The regression was caused by, and whole thing revealed by,
Commit 44a182b9d1 ("xhci: Fix use-after-free in xhci_free_virt_device")
which sets xhci->devs[i]->udev to NULL before xhci_virt_dev() returned.
and causes a NULL pointer dereference the second time we try to set U3.

Fix this by checking xhci->devs[i]->udev exists before setting link state.

The original patch went to stable so this fix needs to be applied there as
well.

	What you can't see just from reading the patch is that in both
	cases (ehci->itd_pool and ehci->sitd_pool) there are two
	allocation paths -- the two branches of an "if" statement -- and
	only one of the paths calls dma_pool_[z]alloc.  However, the
	memset is needed for both paths, and so it can't be eliminated.
	Given that it must be present, there's no advantage to calling
	dma_pool_zalloc rather than dma_pool_alloc.

KASAN found a use-after-free in xhci_free_virt_device+0x33b/0x38e
where xhci_free_virt_device() sets slot id to 0 if udev exists:
if (dev->udev && dev->udev->slot_id)
	dev->udev->slot_id = 0;

dev->udev will be true even if udev is freed because dev->udev is
not set to NULL.

set dev->udev pointer to NULL in xhci_free_dev()

The original patch went to stable so this fix needs to be applied
there as well.

ee30ec0cba usb: phy: Add Kconfig entry for Tegra PHY driver
Tegra's EHCI driver has a build dependency on Tegra's PHY driver and
currently Tegra's PHY driver is built only when Tegra's EHCI driver is
built. Add own Kconfig entry for the Tegra's PHY driver so that drivers
other than ehci-tegra (like ChipIdea UDC) could work with ehci-tegra
driver being disabled in kernels config by allowing user to manually
select the PHY driver.

143470368e usb: tegra: Move utmi-pads reset from ehci-tegra to tegra-phy
UTMI pads are shared by USB controllers and reset of UTMI pads is shared
with the reset of USB1 controller. Currently reset of UTMI pads is done by
the EHCI driver and ChipIdea UDC works because EHCI driver always happen
to be probed first. Move reset controls from ehci-tegra to tegra-phy in
order to resolve the problem.

The Dell Inspiron 5775 is a Raven Ridge. The Enable Slot command timed
out when a USB device gets plugged:

 metag, mn10300, score, and tile, including the associated device drivers.
 
 I have been working with the (former) maintainers for each one to ensure
 that my interpretation was right and the code is definitely unused in
 mainline kernels. Many had fond memories of working on the respective
 ports to start with and getting them included in upstream, but also saw
 no point in keeping the port alive without any users.

 different, they all suffered the same fate: There was one company
 in charge of an SoC line, a CPU microarchitecture and a software
 ecosystem, which was more costly than licensing newer off-the-shelf
 CPU cores from a third party (typically ARM, MIPS, or RISC-V). It seems
 that all the SoC product lines are still around, but have not used the
 custom CPU architectures for several years at this point. In contrast,
 CPU instruction sets that remain popular and have actively maintained
 kernel ports tend to all be used across multiple licensees.
 
 The removal came out of a discussion that is now documented at
 https://lwn.net/Articles/748074/. Unlike the original plans, I'm not
 marking any ports as deprecated but remove them all at once after I made

da2827a298 usb: isp1362: remove blackfin arch glue
The blackfin architecture is getting removed, and this is the last
remaining architecture specific setting, so the various hacks
can be removed now.

From all I can tell, there are no remaining in-tree users of the
driver, but it could be used by out-of-tree platform ports.
I've marked the driver as 'depends on COMPILE_TEST', short of
removing it outright.

It was originally written for some ARM PXA machines using the same
chip, but that platform never really worked and the code has been
removed a long time ago.

53d473fc1e usb: host: Remove the deprecated ATH79 USB host config options
The options USB_EHCI_ATH79 and USB_OHCI_ATH79 only enable the
generic EHCI and OHCI platform drivers, and have been marked as
deprecated since 2012.

These can be safely removed if we make sure that USB_EHCI_ROOT_HUB_TT
still get enabled for the EHCI driver. This is now done be selecting
this option when the EHCI platform driver is enabled on the ATH79
platform.

fa31b3cb2a xhci: Add Intel extended cap / otg phy mux handling
The xHCI controller on various Intel SoCs has an extended cap mmio-range
which contains registers to control the muxing to the xHCI (host mode)
or the dwc3 (device mode) and vbus-detection for the otg usb-phy.

Having a role-sw driver included in the xHCI code (under drivers/usb/host)
is not desirable. So this commit adds a simple handler for this extended
capability, which creates a platform device with the caps mmio region as
resource, this allows us to write a separate platform role-sw driver for
the role-switch.

Note this commit adds a call to the new xhci_ext_cap_init() function
to xhci_pci_probe(), it is added here because xhci_ext_cap_init() must
be called only once. If in the future we also want to handle ext-caps
on non pci xHCI HCDs from xhci_ext_cap_init() a call to it should also
be added to other bus probe paths.

Modify xhci_find_next_ext_cap(base, offset, id) to return the next
capability offset if 0 is passed for id. Otherwise it will behave as
previously and return the offset of the next capability with matching id

capability id 0 is not used by xHCI (reserved)

This is useful when we want to loop through all capabilities.

xhci driver displays the supported xHC USB revision in a message during
driver load:

"Host supports USB 3.1 Enhanced SuperSpeed"

Get the USB minor revision number from the xhci protocol capability.
This will show the correct supported revisions for new USB 3.2 and later
hosts

Don't rely on the SBRN (serial bus revision number) register, it's often
showing 0x30 (USB3.0) for hosts that support USB 3.1

f5249461b5 xhci: Clear the host side toggle manually when endpoint is soft reset
Some devices use a clear endpoint halt request as a soft reset, even if
the endpoint is not halted. This will clear the toggle and sequence on the
device side.

xHCI however refuses to reset a non-halted endpoint, so instead
we need to issue a configure endpoint command on xHCI to clear its host
side toggle and sequence, and get it in sync with the device side.

This is a respin of a old patch that was reverted as it had a stale
endpoint context dequeue value which caused regression.

15febf5eed xhci: refactor xhci_urb_enqueue a bit with minor changes
make the local ep_state variable a pointer to the actual ring ep_state.
This allows us to read fresh ep_state values every time, will be useful
later.

Also move the streams check out from bulk only case. Even if only
bulk tranfers can use streams we shouldn't continue if those flags
are set. Main reason for this change is really code readability and
grouping functionality

The default stop endpoint completion handler will give back cancelled
URBs, and clean, or move past those canceller TRBs on the ring.

This is not always the preferred action.

If the stop endpoint command issuer is waiting for a completion
skip the default handler and just call the completion.

a400efe455 xhci: zero usb device slot_id member when disabling and freeing a xhci slot
set udev->slot_id to zero when disabling and freeing the xhci slot.
Prevents usb core from calling xhci with a stale slot id.

xHC controller may be reset during resume to recover from some error.
All slots are unusable as they are disabled and freed.
xhci driver starts slot enumeration again from 1 in the order they are
enabled. In the worst case a stale udev->slot_id for one device matches
a newly enabled slot_id for a different device, causing us to
perform a action on the wrong device.

4e88d4c083 usb: add a flag to skip PHY initialization to struct usb_hcd
The USB HCD core driver parses the device-tree node for "phys" and
"usb-phys" properties. It also manages the power state of these PHYs
automatically.
However, drivers may opt-out of this behavior by setting "phy" or
"usb_phy" in struct usb_hcd to a non-null value. An example where this
is required is the "Qualcomm USB2 controller", implemented by the
chipidea driver. The hardware requires that the PHY is only powered on
after the "reset completed" event from the controller is received.

A follow-up patch will allow the USB HCD core driver to manage more than
one PHY. Add a new "skip_phy_initialization" bitflag to struct usb_hcd
so drivers can opt-out of any PHY management provided by the USB HCD
core driver.

This also updates the existing drivers so they use the new flag if they
want to opt out of the PHY management provided by the USB HCD core
driver. This means that for these drivers the new "multiple PHY"
handling (which will be added in a follow-up patch) will be disabled as
well.

This patch reverts the commit 835e4241e7 ("usb: host: xhci-plat:
enable clk in resume timing") because this driver also has runtime PM
and the commit 560869100b ("clk: renesas: cpg-mssr: Restore module
clocks during resume") will restore the clock on R-Car H3 environment.

If the xhci_plat_suspend() disables the clk, the system cannot enable
the clk in resume like the following behavior:

< In resume >
 - genpd_resume_noirq() runs and enable the clk (enable_count = 1)
 - cpg_mssr_resume_noirq() restores the clk register.
  -- Since the clk was disabled in suspend, cpg_mssr_resume_noirq()
     will disable the clk and keep the enable_count.
 - Even if xhci_plat_resume() calls clk_prepare_enable(), since
   the enable_count is 1, the clk will be not enabled.

After this patch is applied, the cpg-mssr driver will save the clk
as enable, so the clk will be enabled in resume.

usb: Move USB_UHCI_BIG_ENDIAN_* out of USB_SUPPORT
Move the Kconfig symbols USB_UHCI_BIG_ENDIAN_MMIO and
USB_UHCI_BIG_ENDIAN_DESC out of drivers/usb/host/Kconfig, which is
conditional upon USB && USB_SUPPORT, so that it can be freely selected
by platform Kconfig symbols in architecture code.

For example once the MIPS_GENERIC platform selects are fixed in commit
2e6522c565 ("MIPS: Fix typo BIG_ENDIAN to CPU_BIG_ENDIAN"), the MIPS
32r6_defconfig warns like so:

46408ea558 usb: ohci: Proper handling of ed_rm_list to handle race condition between usb_kill_urb() and finish_unlinks()
There is a race condition between finish_unlinks->finish_urb() function
and usb_kill_urb() in ohci controller case. The finish_urb calls
spin_unlock(&ohci->lock) before usb_hcd_giveback_urb() function call,
then if during this time, usb_kill_urb is called for another endpoint,
then new ed will be added to ed_rm_list at beginning for unlink, and
ed_rm_list will point to newly added.

When finish_urb() is completed in finish_unlinks() and ed->td_list
becomes empty as in below code (in finish_unlinks() function):

The *last = ed->ed_next will make ed_rm_list to point to ed->ed_next
and previously added ed by usb_kill_urb will be left unreferenced by
ed_rm_list. This causes usb_kill_urb() hang forever waiting for
finish_unlink to remove added ed from ed_rm_list.

The main reason for hang in this race condtion is addition and removal
of ed from ed_rm_list in the beginning during usb_kill_urb and later
last* is modified in finish_unlinks().

As suggested by Alan Stern, the solution for proper handling of
ohci->ed_rm_list is to remove ed from the ed_rm_list before finishing
any URBs. Then at the end, we can add ed back to the list if necessary.

This properly handle the updated ohci->ed_rm_list in usb_kill_urb().

91b119359c usb: host: ehci: always enable interrupt for qtd completion at test mode
At former code, the SETUP stage does not enable interrupt
for qtd completion, it relies on IAA watchdog to complete
interrupt, then the transcation would be considered timeout
if the flag need_io_watchdog is cleared by platform code.

In this commit, we always add enable interrupt for qtd completion,
then the qtd completion can be notified by hardware interrupt.

b2685bdacd ohci-hcd: Fix race condition caused by ohci_urb_enqueue() and io_watchdog_func()
Running io_watchdog_func() while ohci_urb_enqueue() is running can
cause a race condition where ohci->prev_frame_no is corrupted and the
watchdog can mis-detect following error:

  ohci-platform 664a0800.usb: frame counter not updating; disabled
  ohci-platform 664a0800.usb: HC died; cleaning up

Specifically, following scenario causes a race condition:

  1. ohci_urb_enqueue() calls spin_lock_irqsave(&ohci->lock, flags)
     and enters the critical section
  2. ohci_urb_enqueue() calls timer_pending(&ohci->io_watchdog) and it
     returns false
  3. ohci_urb_enqueue() sets ohci->prev_frame_no to a frame number
     read by ohci_frame_no(ohci)
  4. ohci_urb_enqueue() schedules io_watchdog_func() with mod_timer()
  5. ohci_urb_enqueue() calls spin_unlock_irqrestore(&ohci->lock,
     flags) and exits the critical section
  6. Later, ohci_urb_enqueue() is called
  7. ohci_urb_enqueue() calls spin_lock_irqsave(&ohci->lock, flags)
     and enters the critical section
  8. The timer scheduled on step 4 expires and io_watchdog_func() runs
  9. io_watchdog_func() calls spin_lock_irqsave(&ohci->lock, flags)
     and waits on it because ohci_urb_enqueue() is already in the
     critical section on step 7
 10. ohci_urb_enqueue() calls timer_pending(&ohci->io_watchdog) and it
     returns false
 11. ohci_urb_enqueue() sets ohci->prev_frame_no to new frame number
     read by ohci_frame_no(ohci) because the frame number proceeded
     between step 3 and 6
 12. ohci_urb_enqueue() schedules io_watchdog_func() with mod_timer()
 13. ohci_urb_enqueue() calls spin_unlock_irqrestore(&ohci->lock,
     flags) and exits the critical section, then wake up
     io_watchdog_func() which is waiting on step 9
 14. io_watchdog_func() enters the critical section
 15. io_watchdog_func() calls ohci_frame_no(ohci) and set frame_no
     variable to the frame number
 16. io_watchdog_func() compares frame_no and ohci->prev_frame_no

On step 16, because this calling of io_watchdog_func() is scheduled on
step 4, the frame number set in ohci->prev_frame_no is expected to the
number set on step 3.  However, ohci->prev_frame_no is overwritten on
step 11.  Because step 16 is executed soon after step 11, the frame
number might not proceed, so ohci->prev_frame_no must equals to
frame_no.

To address above scenario, this patch introduces a special sentinel
value IO_WATCHDOG_OFF and set this value to ohci->prev_frame_no when
the watchdog is not pending or running.  When ohci_urb_enqueue()
schedules the watchdog (step 4 and 12 above), it compares
ohci->prev_frame_no to IO_WATCHDOG_OFF so that ohci->prev_frame_no is
not overwritten while io_watchdog_func() is running.

In function xhci_stop, xhci_debugfs_exit called before xhci_mem_cleanup.
xhci_debugfs_exit removed the xhci debugfs root nodes, xhci_mem_cleanup
called function xhci_free_virt_devices_depth_first which in turn called
function xhci_debugfs_remove_slot.
Function xhci_debugfs_remove_slot removed the nodes for devices, the nodes
folders are sub folder of xhci debugfs.

It is unreasonable to remove xhci debugfs root folder before
xhci debugfs sub folder. Function xhci_mem_cleanup should be called
before function xhci_debugfs_exit.

d916767172 xhci: Fix xhci debugfs devices node disappearance after hibernation
During system resume from hibernation, xhci host is reset, all the
nodes in devices folder are removed in xhci_mem_cleanup function.
Later nodes in /sys/kernel/debug/usb/xhci/* are created again in
function xhci_run, but the nodes already exist, so the nodes still
keep the old ones, finally device nodes in xhci debugfs folder
/sys/kernel/debug/usb/xhci/*/devices/* are disappeared.

This fix removed xhci debugfs nodes before the nodes are re-created,
so all the nodes in xhci debugfs can be re-created successfully.

fa2dfd0ec2 xhci: Fix NULL pointer in xhci debugfs
Commit dde634057d ("xhci: Fix use-after-free in xhci debugfs") causes a
null pointer dereference while fixing xhci-debugfs usage of ring pointers
that were freed during hibernate.

The fix passed addresses to ring pointers instead, but forgot to do this
change for the xhci_ring_trb_show function.

The address of the ring pointer passed to xhci-debugfs was of a temporary
ring pointer "new_ring" instead of the actual ring "ring" pointer. The
temporary new_ring pointer will be set to NULL later causing the NULL
pointer dereference.

This issue was seen when reading xhci related files in debugfs:

1208d8a84f xhci: Don't print a warning when setting link state for disabled ports
When disabling a USB3 port the hub driver will set the port link state to
U3 to prevent "ejected" or "safely removed" devices that are still
physically connected from immediately re-enumerating.

If the device was really unplugged, then error messages were printed
as the hub tries to set the U3 link state for a port that is no longer
enabled.

xhci-hcd ee000000.usb: Cannot set link state.
usb usb8-port1: cannot disable (err = -32)

Don't print error message in xhci-hub if hub tries to set port link state
for a disabled port. Return -ENODEV instead which also silences hub driver.

bde0716d1f xhci: workaround for AMD Promontory disabled ports wakeup
For AMD Promontory xHCI host, although you can disable USB ports in
BIOS settings, those ports will be enabled anyway after you remove a
device on that port and re-plug it in again. It's a known limitation of
the chip. As a workaround we can clear the PORT_WAKE_BITS.

The Aspeed SoCs use uhci-platform. With the new dynamic clock
control framework, the corresponding IP block clock must be
properly enabled.

This is a simplified variant of what ehci-platform does, it

a2ecc4df9f usb: xhci-mtk: supports remote wakeup for mt2712 with two xHCI IPs
The old way of usb wakeup only supports platform with single xHCI IP,
such as mt8173, but mt2712 has two xHCI IPs, so rebuild its flow and
supports the new glue layer of usb wakeup on mt2712 which is different
from mt8173.
Due to there is a hardware bug with the LINE STATE wakeup mode on
mt8173 which causes wakeup failure by low speed devices, and also
because IP SLEEP mode can cover all functions of LINE STATE mode,
it is unused in fact, and will not support it later, so remove it at
the same time.

The issue is caused by the xhci ring structures being reallocated
when the system is resumed, but pointers to the old structures
being retained in the debugfs files "private" field:

052f71e25a xhci: Fix xhci debugfs NULL pointer dereference in resume from hibernate
Free the virt_device and its debugfs_private member together.

When resuming from hibernate the .free_dev callback unconditionally
freed the debugfs_private member, but could leave virt_device intact.

This triggered a NULL pointer dereference after resume when usbmuxd
sent a USBDEVFS_SETCONFIGURATION ioctl to a device, trying to add a
endpoint debugfs entry to a already freed debugfs_private pointer.

aa15d3d257 USB: remove the URB_NO_FSBR flag
The URB_NO_FSBR flag has never really been used.  It was introduced as
a potential way for UHCI to minimize PCI bus usage (by not attempting
full-speed bulk and control transfers more than once per frame), but
the flag was not set by any drivers.

There's no point in keeping it around.  This patch simplifies the API
by removing it.  Unfortunately, it does have to be kept as part of the
usbfs ABI, but at least we can document in
include/uapi/linux/usbdevice_fs.h that it doesn't do anything.

Avoid null pointer dereference if some function is walking through the
devs array accessing members of a new virt_dev that is mid allocation.

Add the virt_dev to xhci->devs[i] _after_ the virt_device and all its
members are properly allocated.

issue found by KASAN: null-ptr-deref in xhci_find_slot_id_by_port

"Quick analysis suggests that xhci_alloc_virt_device() is not mutex
protected. If so, there is a time frame where xhci->devs[slot_id] is set
but not fully initialized. Specifically, xhci->devs[i]->udev can be NULL."

ab725cbec3 usb: xhci: allow imod-interval to be configurable
The xHCI driver currently has the IMOD set to 160, which
translates to an IMOD interval of 40,000ns (160 * 250)ns

Commit 0cbd4b34cd ("xhci: mediatek: support MTK xHCI host controller")
introduced a QUIRK for the MTK platform to adjust this interval to 20,
which translates to an IMOD interval of 5,000ns (20 * 250)ns. This is
due to the fact that the MTK controller IMOD interval is 8 times
as much as defined in xHCI spec.

Instead of adding more quirk bits for additional platforms, this patch
introduces the ability for vendors to set the IMOD_INTERVAL as is
optimal for their platform. By using device_property_read_u32() on
"imod-interval-ns", the IMOD INTERVAL can be specified in nano seconds.
If no interval is specified, the default of 40,000ns (IMOD=160) will be
used.

No bounds checking has been implemented due to the fact that a vendor
may have violated the spec and would need to specify a value outside of
the max 8,000 IRQs/second limit specified in the xHCI spec.

dfba2174dc usb: xhci: Add DbC support in xHCI driver
xHCI compatible USB host controllers(i.e. super-speed USB3 controllers)
can be implemented with the Debug Capability(DbC). It presents a debug
device which is fully compliant with the USB framework and provides the
equivalent of a very high performance full-duplex serial link. The debug
capability operation model and registers interface are defined in 7.6.8
of the xHCI specification, revision 1.1.

The DbC debug device shares a root port with the xHCI host. By default,
the debug capability is disabled and the root port is assigned to xHCI.
When the DbC is enabled, the root port will be assigned to the DbC debug
device, and the xHCI sees nothing on this port. This implementation uses
a sysfs node named <dbc> under the xHCI device to manage the enabling
and disabling of the debug capability.

When the debug capability is enabled, it will present a debug device
through the debug port. This debug device is fully compliant with the
USB3 framework, and it can be enumerated by a debug host on the other
end of the USB link. As soon as the debug device is configured, a TTY
serial device named /dev/ttyDBC0 will be created.

The problem can occur if xhci_plat_remove() is called shortly after
xhci_plat_probe(). While xhci_free_virt_devices_depth_first been
called before the device has been setup and get real_port initialized.
The problem occurred on Hikey960 and was reproduced by Guenter Roeck
on Kevin with chromeos-4.4.

e4ec40ec4b xhci: Don't show incorrect WARN message about events for empty rings
xHC can generate two events for a short transfer if the short TRB and
last TRB in the TD are not the same TRB.

The driver will handle the TD after the first short event, and remove
it from its internal list. Driver then incorrectly prints a warning
for the second event:

"WARN Event TRB for slot x ep y with no TDs queued"

Fix this by not printing a warning if we get a event on a empty list
if the previous event was a short event.

1d5a31582e usb: host: fix incorrect updating of offset
The variable temp is incorrectly being updated, instead it should
be offset otherwise the loop just reads the same capability value
and loops forever.  Thanks to Alan Stern for pointing out the

e99e88a9d2 treewide: setup_timer() -> timer_setup()
This converts all remaining cases of the old setup_timer() API into using
timer_setup(), where the callback argument is the structure already
holding the struct timer_list. These should have no behavioral changes,
since they just change which pointer is passed into the callback with
the same available pointers after conversion. It handles the following
examples, in addition to some other variations.

Casting from unsigned long:

ef1362d9c0 USB: host: ohci: Remove redundant license text
Now that the SPDX tag is in all USB files, that identifies the license
in a specific and legally-defined manner.  So the extra GPL text wording
can be removed as it is no longer needed at all.

This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text.  And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.

No copyright headers or other non-license-description text was removed.
