Per dwc3 2.50a spec, the is_devspec bit is used to distinguish the
Device Endpoint-Specific Event or Device-Specific Event (DEVT). If the
bit is 1, the event is represented Device-Specific Event, then use
[7:1] bits as Device Specific Event to marked the type. It has 7 bits,
and we can see the reserved8_31 variable name which means from 8 to 31
bits marked reserved, actually there are 24 bits not 25 bits between
that. And 1 + 7 + 24 = 32, the event size is 4 byes.
So in dwc3_event_type, the bit mask should be:
is_devspec [0] 1 bit
type [7:1] 7 bits
reserved8_31 [31:8] 24 bits

The __cpuinit type of throwaway sections might have made sense
some time ago when RAM was more constrained, but now the savings
do not offset the cost and complications. For example, the fix in
commit 5e427ec2d0 ("x86: Fix bit corruption at CPU resume time")

with improper use of the various __init prefixes.
After a discussion on LKML[1] it was decided that cpuinit should go
the way of devinit and be phased out. Once all the users are gone,
we can then finally remove the macros themselves from linux/init.h.
This removes all the remaining one-off uses of the __cpuinit macros

commit a66b2e (cpufreq: Preserve sysfs files across suspend/resume)
has unfortunately caused several things in the cpufreq subsystem to
break subtly after a suspend/resume cycle.
The intention of that patch was to retain the file permissions of the
cpufreq related sysfs files across suspend/resume. To achieve that,
the commit completely removed the calls to cpufreq_add_dev() and
__cpufreq_remove_dev() during suspend/resume transitions. But the
problem is that those functions do 2 kinds of things:
1. Low-level initialization/tear-down that are critical to the
correct functioning of cpufreq-core.
2. Kobject and sysfs related initialization/teardown.
Ideally we should have reorganized the code to cleanly separate these
two responsibilities, and skipped only the sysfs related parts during
suspend/resume. Since we skipped the entire callbacks instead (which
also included some CPU and cpufreq-specific critical components),
cpufreq subsystem started behaving erratically after suspend/resume.
So revert the commit to fix the regression. We'll revisit and address
the original goal of that commit separately, since it involves quite a
bit of careful code reorganization and appears to be non-trivial.
(While reverting the commit, note that another commit f51e1eb
(cpufreq: Fix cpufreq regression after suspend/resume) already
reverted part of the original set of changes. So revert only the
remaining ones).

In acpi_bus_device_attach(), if there is an ACPI device object
for the given handle and that device object has a scan handler
attached to it already, there's nothing more to do for that handle.
Moreover, if acpi_scan_attach_handler() is called then, it may
execute the .attach() callback of the ACPI scan handler already
attached to the device object and that may lead to interesting
breakage.
For this reason, make acpi_bus_device_attach() return success
immediately when the handle's device object has a scan handler

This is the remaining set of SCSI patches for the merge window. it's mostly
driver updates (scsi_debug, qla2xxx, storvsc, mp3sas). There are also several
bug fixes in fcoe, libfc, and megaraid_sas. We also have a couple of core

I.e. for letter/pillarboxing. For those cases we need to adjust the

commit 2dd24552ca
Author: Jesse Barnes <jbarnes@virtuousgeek.org>
Date: Thu Apr 25 12:55:01 2013 -0700
drm/i915: factor out GMCH panel fitting code and use for eDP v3
broke that by reordering the computation of the gmch pfit state with
the block of code that prepared the adjusted mode for it and told the
modeset core not to overwrite the adjusted mode with default settings.
We might want to switch around the core code to just fill in defaults,
but this code predates the pipe_config modeset rework. And in the old
crtc helpers we did not have a suitable spot to do this.

Part of the job of garbage collection is to add up however many sectors
of live data it finds in each bucket, but that doesn't work very well if
it doesn't reset GC_SECTORS_USED() when it starts. Whoops.
This wouldn't have broken anything horribly, but allocation tries to
preferentially reclaim buckets that are mostly empty and that's not
gonna work with an incorrect GC_SECTORS_USED() value.

The journal replay code starts by finding something that looks like a
valid journal entry, then it does a binary search over the unchecked
region of the journal for the journal entries with the highest sequence
numbers.
Trouble is, the logic was wrong - journal_read_bucket() returns true if
it found journal entries we need, but if the range of journal entries
we're looking for loops around the end of the journal - in that case
journal_read_bucket() could return true when it hadn't found the highest
sequence number we'd seen yet, and in that case the binary search did
the wrong thing. Whoops.

In the far-too-complicated closure code - closures can have destructors,
for probably dubious reasons; they get run after the closure is no
longer waiting on anything but before dropping the parent ref, intended
just for freeing whatever memory the closure is embedded in.
Trouble is, when remaining goes to 0 and we've got nothing more to run -
we also have to unlock the closure, setting remaining to -1. If there's
a destructor, that unlock isn't doing anything - nobody could be trying
to lock it if we're about to free it - but if the unlock _is needed...
that check for a destructor was racy. Argh.

The bits in BRIDGE_CAUSE are documented as RW0C - read, write 0 to
clear. If we read the register, mask off the watchdog bit, and
write it back, we're actually clearing every interrupt which wasn't
pending at the time we read the register - and that is racy.
Fix this to only write ~WATCHDOG_BIT to the register, which means
we write as zero only the watchdog bit.

Interrupt request doesn't use the right API: The TWD watchdog uses a per-cpu
interrupt (usually interrupt #30), and the GIC configuration should flag it as
such. With this setup, request_irq() should fail, and the right API is
request_percpu_irq(), together with enable_percpu_irq()/disable_percpu_irq().
Nothing ensures the userspace ioctl() will end-up kicking the watchdog on the
right CPU.
There are no users of this driver since a long time and it makes more sense to
get rid of it as nobody is looking to fix it.

A watchdog device may be stopped from userspace using WDIOC_SETOPTIONS
ioctl and flag WDIOS_DISABLECARD. If the device is closed after this
operation, watchdog_release() is called and status bits checked for
stopping it. Besides, if the device has not been unregistered a critical
message "watchdog did not stop!" is printed, although the ioctl may have
successfully stopped it already.
Without the patch a user application sample code like this will successfully
stop the watchdog, but the kernel will output the message
"watchdog did not stop!":
wd_fd = open("/dev/watchdog", O_RDWR);
flags = WDIOS_DISABLECARD;

disabled pfit) from the pipe to the lvds encoder's get_config
function.
Otherwise we get a pipe state mismatch if we use pipe B for a non-lvds
output and we've left the dither bit enabled behind us. This can
happen if the BIOS has set the bit (some seem to unconditionally do
that, even in the complete absence of an lvds port), but not enabled
pipe B at boot-up. Then we won't clear the pfit control register since
we can only touch that if the pfit is associated with our pipe in the
crtc configuration - we could trample over the pfit state of the other
pipe otherwise since it's shared. Once pipe B is enabled we notice
that the 6to8 dither bit is set and complain about the mismatch.
Note that testing indicates that we don't actually need to set this
bit when the pfit is disabled, dithering on 18bpp panels seems to work
regardless. But ripping that code out is not something for a bugfix
meant for -rc kernels.

Commit 24778be "spi: convert drivers to use bits_per_word_mask" removed
what appeared to be redundant code from many drivers. However, it
appears that in the spi-bitbang case, these functions are required by
the spi-bitbang core, even if they don't do anything. Restore them.
For 3.12, the spi-bitbang core should be adjusted not to require these
callbacks to exist if they don't need to do anything.
This is the equivalent of Michal Simek's patch "spi/xilinx: Revert
master->setup function removal", applied to other affected drivers.

dm-switch is a new target that maps IO to underlying block devices
efficiently when there is a large number of fixed-sized address regions
but there is no simple pattern to allow for a compact mapping
representation such as dm-stripe.
Though we have developed this target for a specific storage device, Dell
EqualLogic, we have made an effort to keep it as general purpose as
possible in the hope that others may benefit.

This patch removes "io_lock" and "map_lock" in struct mapped_device and
"holders" in struct dm_table and replaces these mechanisms with
sleepable-rcu.
Previously, the code would call "dm_get_live_table" and "dm_table_put" to
get and release table. Now, the code is changed to call "dm_get_live_table"
and "dm_put_live_table". dm_get_live_table locks sleepable-rcu and
dm_put_live_table unlocks it.
dm_get_live_table_fast/dm_put_live_table_fast can be used instead of
dm_get_live_table/dm_put_live_table. These *_fast functions use
non-sleepable RCU, so the caller must not block between them.
If the code changes active or inactive dm table, it must call
dm_sync_table before destroying the old table.

Fix a boundary condition that caused failure for certain device sizes.
The problem is reported at
http://code.google.com/p/cryptsetup/issues/detail?id=160
For certain device sizes the number of hashes at a specific level was
calculated incorrectly.
It happens for example for a device with data and metadata block size 4096
that has 16385 blocks and algorithm sha256.
The user can test if he is affected by this bug by running the
"veritysetup verify" command and also by activating the dm-verity kernel
driver and reading the whole block device. If it passes without an error,
then the user is not affected.
The condition for the bug is:
Split the total number of data blocks (data_block_bits) into bit strings,
each string has hash_per_block_bits bits. hash_per_block_bits is
rounddown(log2(metadata_block_size/hash_digest_size)). Equivalently, you
can say that you convert data_blocks_bits to 2^hash_per_block_bits base.
If there some zero bit string below the most significant bit string and at
least one bit below this zero bit string is set, then the bug happens.
The same bug exists in the userspace veritysetup tool, so you must use
fixed veritysetup too if you want to use devices that are affected by
this boundary condition.

When multipath needs to retry an ioctl the reference to the
current live table needs to be dropped. Otherwise a deadlock
occurs when all paths are down:
- dm_blk_ioctl takes a reference to the current table
and spins in multipath_ioctl().
- A new table is being loaded, but upon resume the process
hangs in dm_table_destroy() waiting for references to
drop to zero.
With this patch the reference to the old table is dropped
prior to retry, thereby avoiding the deadlock.

Intel Smart Connect is an Intel-specific ACPI interface for configuring
devices to wake up at regular intervals so they can pull down mail or other
internet updates, and then go to sleep again. If a user enables this in
Windows and then reboots into Linux, the device may wake up if it's put to
sleep. Since there's no Linux userland support for any of this, the machine
will then remain awake until something else puts it back to sleep.
I haven't figured out all that much about how this works (there's a bunch
of different ACPI calls available on the device), but this seems to be
enough to turn it off. We can add more features to this driver if anyone
ever cares about figuring out what the rest of the calls do or writing some
Linux userspace to implement the rest of it.

Intel Rapid Start Technology is a firmware-based suspend-to-disk
implementation. Once placed in S3, the device will wake once either a
timeout elapses or the battery reaches a critical level. It will then resume
to the firmware and copy the contents of RAM to a specialised partition, and
then power off the machine. If the user turns the machine back on the
firmware will copy the contents of the partition back to RAM and then resume
from S3 as normal.
This driver provides an interface for configuring the wakeup events and
timeout. It still requires firmware support and an appropriate suspend
partition.

out of order.
So, the patch will enable the wlan led function only if the wapf == 4.

drm/i915: Workaround incoherence with fence updates on Valleyview
Jon Bloomfield came up with a plausible explanation and cheap fix
(drm/i915: Fix incoherence with fence updates on Sandybridge+) for the
race condition, so lets run with it.
This is a candidate for stable as the old workaround incurs a
significant cost (calling wbinvd on all CPUs before performing the
register write) for some workloads as noted by Carsten Emde.

drm/i915: Workaround incoherence between fences and LLC across multiple CPUs
Thanks to further investigation by Jon Bloomfield, he realised that
the 64-bit register might be broken up by the hardware into two 32-bit
writes (a problem we have encountered elsewhere). This non-atomicity
would then cause an issue where a second thread would see an
intermediate register state (new high dword, old low dword), and this
register would randomly be used in preference to its own thread register.
This would cause the second thread to read from and write into a fairly
random tiled location. Breaking the operation into 3 explicit 32-bit
updates (first disable the fence, poke the upper bits, then poke the lower
bits and enable) ensures that, given proper serialisation between the
32-bit register write and the memory transfer, that the fence value is
always consistent.
Armed with this knowledge, we can explain how the previous workaround
work. The key to the corruption is that a second thread sees an
erroneous fence register that conflicts and overrides its own. By
serialising the fence update across all CPUs, we have a small window
where no GTT access is occurring and so hide the potential corruption.
This also leads to the conclusion that the earlier workaround was
incomplete.
v2: Be overly paranoid about the order in which fence updates become
visible to the GPU to make really sure that we turn the fence off before
doing the update, and then only switch the fence on afterwards.

Daniel noticed a problem where is we wrote to an object with ring A in
the middle of a very long running batch, then executed a quick batch on
ring B before a batch that reads from the same object, its obj->ring would
now point to ring B, but its last_write_seqno would be still relative to
ring A. This would allow for the user to read from the object before the
GPU had completed the write, as set_domain would only check that ring B
had passed the last_write_seqno.
To fix this simply (and inelegantly), we bump the last_write_seqno when
switching rings so that the last_write_seqno is always relative to the
current obj->ring.
This fixes igt/tests/gem_write_read_ring_switch.

This patch partially reverts commit 36ec8f8774 for
IvyBridge CPUs.
The original commit results in repeated 'Timed out waiting for forcewake old
ack to clear' messages on a Supermicro C7H61 board (BIOS version 2.00 and 2.00b)
with i7-3770K CPU. It ultimately results in a hangup if the system is highly
loaded. Reverting the commit for IvyBridge CPUs fixes the issue.
Issue a warning if the CPU is IvyBridge and mt forcewake is disabled, since
this condition can result in secondary issues.
v2: Only revert patch for Ivybridge CPUs

Pull networking updates from David Miller:
"This is a re-do of the net-next pull request for the current merge
window. The only difference from the one I made the other day is that
this has Eliezer's interface renames and the timeout handling changes
made based upon your feedback, as well as a few bug fixes that have
trickeled in.
Highlights:
1) Low latency device polling, eliminating the cost of interrupt
handling and context switches. Allows direct polling of a network
device from socket operations, such as recvmsg() and poll().
Currently ixgbe, mlx4, and bnx2x support this feature.
Full high level description, performance numbers, and design in
commit 0a4db187a9 ("Merge branch 'll_poll'")
From Eliezer Tamir.
2) With the routing cache removed, ip_check_mc_rcu() gets exercised
more than ever before in the case where we have lots of multicast
addresses. Use a hash table instead of a simple linked list, from
Eric Dumazet.
3) Add driver for Atheros CQA98xx 802.11ac wireless devices, from
Bartosz Markowski, Janusz Dziedzic, Kalle Valo, Marek Kwaczynski,
Marek Puzyniak, Michal Kazior, and Sujith Manoharan.
4) Support reporting the TUN device persist flag to userspace, from
Pavel Emelyanov.
5) Allow controlling network device VF link state using netlink, from
Rony Efraim.
6) Support GRE tunneling in openvswitch, from Pravin B Shelar.
7) Adjust SOCK_MIN_RCVBUF and SOCK_MIN_SNDBUF for modern times, from
Daniel Borkmann and Eric Dumazet.
8) Allow controlling of TCP quickack behavior on a per-route basis,
from Cong Wang.
9) Several bug fixes and improvements to vxlan from Stephen
Hemminger, Pravin B Shelar, and Mike Rapoport. In particular,
support receiving on multiple UDP ports.
10) Major cleanups, particular in the area of debugging and cookie
lifetime handline, to the SCTP protocol code. From Daniel
Borkmann.
11) Allow packets to cross network namespaces when traversing tunnel
devices. From Nicolas Dichtel.
12) Allow monitoring netlink traffic via AF_PACKET sockets, in a
manner akin to how we monitor real network traffic via ptype_all.
From Daniel Borkmann.
13) Several bug fixes and improvements for the new alx device driver,
from Johannes Berg.
14) Fix scalability issues in the netem packet scheduler's time queue,
by using an rbtree. From Eric Dumazet.
15) Several bug fixes in TCP loss recovery handling, from Yuchung
Cheng.
16) Add support for GSO segmentation of MPLS packets, from Simon
Horman.
17) Make network notifiers have a real data type for the opaque
pointer that's passed into them. Use this to properly handle
network device flag changes in arp_netdev_event(). From Jiri
Pirko and Timo Teräs.
18) Convert several drivers over to module_pci_driver(), from Peter
Huewe.
19) tcp_fixup_rcvbuf() can loop 500 times over loopback, just use a
O(1) calculation instead. From Eric Dumazet.
20) Support setting of explicit tunnel peer addresses in ipv6, just
like ipv4. From Nicolas Dichtel.
21) Protect x86 BPF JIT against spraying attacks, from Eric Dumazet.
22) Prevent a single high rate flow from overruning an individual cpu
during RX packet processing via selective flow shedding. From
Willem de Bruijn.
23) Don't use spinlocks in TCP md5 signing fast paths, from Eric
Dumazet.
24) Don't just drop GSO packets which are above the TBF scheduler's
burst limit, chop them up so they are in-bounds instead. Also
from Eric Dumazet.
25) VLAN offloads are missed when configured on top of a bridge, fix
from Vlad Yasevich.
26) Support IPV6 in ping sockets. From Lorenzo Colitti.
27) Receive flow steering targets should be updated at poll() time
too, from David Majnemer.
28) Fix several corner case regressions in PMTU/redirect handling due
to the routing cache removal, from Timo Teräs.
29) We have to be mindful of ipv4 mapped ipv6 sockets in

The moves themselves were generally async to graphics previously, with
the exception that if the "main" channel is used to synchronise a

Not really critical, and there's better ways of handling this, but they
are all rather invasive, so this is fine for now.

while trying to solve issues that became apparent while porting to the

The major problem was that the the previously mentioned patch took the
client mutex earlier than previously, but the pinning of new_bo can
can potentially cause a buffer move, which would result in attempting to
acquire the same mutex again.
This commit attempts to fix that "fix".

virtio net called virtqueue_enable_cq on RX path after napi_complete, so
with NAPI_STATE_SCHED clear - outside the implicit napi lock.
This violates the requirement to synchronize virtqueue_enable_cq wrt
virtqueue_add_buf. In particular, used event can move backwards,
causing us to lose interrupts.
In a debug build, this can trigger panic within START_USE.
Jason Wang reports that he can trigger the races artificially,
by adding udelay() in virtqueue_enable_cb() after virtio_mb().
However, we must call napi_complete to clear NAPI_STATE_SCHED before
polling the virtqueue for used buffers, otherwise napi_schedule_prep in
a callback will fail, causing us to lose RX events.
To fix, call virtqueue_enable_cb_prepare with NAPI_STATE_SCHED
set (under napi lock), later call virtqueue_poll with
NAPI_STATE_SCHED clear (outside the lock).

do_device_access() is a function that abstracts copying SG list from/to
ramdisk storage (fake_storep).
It must deal with the ranges exceeding actual fake_storep size, because
such ranges are valid if virtual_gb is set greater than zero, and they
should be treated as fake_storep is repeatedly mirrored up to virtual
size.
Unfortunately, it can't deal with the range which wraps around the end of
fake_storep. A wrap around range is copied by two
sg_copy_{from,to}_buffer() calls, but sg_copy_{from,to}_buffer() can't
copy from/to in the middle of SG list, therefore the second call can't
copy correctly.
This fixes it by using sg_pcopy_{from,to}_buffer() that can copy from/to
the middle of SG list.
This also simplifies the assignment of sdb->resid in
fill_from_dev_buffer(). Because fill_from_dev_buffer() is now only called
once per command execution cycle. So it is not necessary to take care to
decrease sdb->resid if fill_from_dev_buffer() is called more than once.

If something goes wrong during LUN scanning, e.g. a transport layer
failure occurs, then __scsi_remove_device() can get invoked by the
LUN scanning code for a SCSI device in state SDEV_CREATED_BLOCK and
before the SCSI device has been added to sysfs (is_visible == 0).
Make sure that even in this case the transition into state SDEV_DEL
occurs. This avoids that __scsi_remove_device() can get invoked a
second time by scsi_forget_host() if this last function is invoked
from another thread than the thread that performs LUN scanning.

In the function prot_verify_write(), kmap_atomic()/kunmap_atomic() for
data page and kmap_atomic()/kunmap_atomic() for protection information
page are not nested each other.
It worked perfectly before commit 3e4d3af501
("mm: stack based kmap_atomic()"). Because the kmap_atomic slot KM_IRQ0
was used for data page and the slot KM_IRQ1 was used for protection page.
But KM_types are gone and kmap_atomic() is using stack based implementation.
So two different kmap_atomic() usages must be strictly nested now.
This change ensures kmap_atomic() usage is strictly nested.

incremented in the loop for each data page. Finally 'daddr' reaches

Fix the issue by not incrementing 'daddr' in the loop and offsetting it

When Async scanning mode is enabled and device scanning is in progress,
devices should not be removed. But in actuality, devices are removed but
their transport layer entries are not removed. This causes error to add

So, in this patch, modified the code in such a way that device is not removed
when Async scanning mode is enabled and device scanning is in progress.

With some enclosures when LUN 0 is not created but LUN 1 or LUN X is created
then SCSI scan procedure calls target_alloc, slave_alloc call back functions
for LUN 0 and slave_destory() for same LUN 0.
In these kind of cases within slave_destroy, pointer to scsi_target in
_sas_device structure is set to NULL, following which when slave_alloc for LUN
1 is called then starget would not be set properly for this LUN. So,
scsi_target pointer pointing to NULL value would lead to a crash later in the
discovery procedure.
To solve this issue set the sas_device's scsi_target pointer to scsi_device's
scsi_target if it is NULL earlier in slave_alloc callback function.
