5860acc1a9 x86: remove arch specific dma_supported implementation
And instead wire it up as method for all the dma_map_ops instances.

Note that this also means the arch specific check will be fully instead
of partially applied in the AMD iommu driver.

84d582d236 xen: Revert commits da72ff5bfc and 72a9b18629
Recent discussion (http://marc.info/?l=xen-devel&m=149192184523741)
established that commit 72a9b18629 ("xen: Remove event channel
notification through Xen PCI platform device") (and thus commit
da72ff5bfc ("partially revert "xen: Remove event channel
notification through Xen PCI platform device"")) are unnecessary and,
in fact, prevent HVM guests from booting on Xen releases prior to 4.0

Therefore we revert both of those commits.

The summary of that discussion is below:

  Here is the brief summary of the current situation:

  Before the offending commit (72a9b18629):

  1) INTx does not work because of the reset_watches path.
  2) The reset_watches path is only taken if you have Xen > 4.0
  3) The Linux Kernel by default will use vector inject if the hypervisor
     support. So even INTx does not work no body running the kernel with
     Xen > 4.0 would notice. Unless he explicitly disabled this feature
     either in the kernel or in Xen (and this can only be disabled by
     modifying the code, not user-supported way to do it).

  After the offending commit (+ partial revert):

  1) INTx is no longer support for HVM (only for PV guests).
  2) Any HVM guest The kernel will not boot on Xen < 4.0 which does
     not have vector injection support. Since the only other mode
     supported is INTx which.

  So based on this summary, I think before commit (72a9b18629) we were
  in much better position from a user point of view.

153654dbe5 x86/PCI: Implement pcibios_release_device to release IRQ from IOAPIC
The revert of 991de2e590 ("PCI, x86: Implement pcibios_alloc_irq()
and pcibios_free_irq()") causes a problem for IOAPIC hotplug. The
problem is that IRQs are allocated and freed in pci_enable_device()
and pci_disable_device(). But there are some drivers which don't call
pci_disable_device(), and they have good reasons not calling it, so
if they're using IOAPIC their IRQs won't have a chance to be released
from the IOAPIC. When this happens IOAPIC hot-removal fails with a
kernel stack dump and an error message like this:

It turns out that we can fix it in a different way without moving IRQ
allocation into pcibios_alloc_irq(), thus avoiding the regression of
991de2e590. We can keep the allocation and freeing of IRQs as is
within pci_enable_device()/pci_disable_device(), without breaking any
previous assumption of the rest of the system, keeping compatibility
with both the legacy and the modern drivers. We can accomplish this by
implementing the existing __weak hook of pcibios_release_device() thus
when a pci device is about to be deleted we get notified in the hook
and take the chance to release its IRQ, if any, from the IOAPIC.

Implement pcibios_release_device() for x86 to release any IRQ not released
by the driver.

 similar enough to the core DMA mapping code that with a few changes
 it was possible to remove the IB DMA mapping code entirely and
 switch the RDMA stack to use the core DMA mapping code.  This resulted
 in a nice set of cleanups, but touched the entire tree.  This branch
 will be submitted separately to Linus at the end of the merge window
 as per normal practice for tree wide changes like this.

c74fd80f2f xen: do not re-use pirq number cached in pci device msi msg data
Revert the main part of commit:
af42b8d12f ("xen: fix MSI setup and teardown for PV on HVM guests")

That commit introduced reading the pci device's msi message data to see
if a pirq was previously configured for the device's msi/msix, and re-use
that pirq.  At the time, that was the correct behavior.  However, a
later change to Qemu caused it to call into the Xen hypervisor to unmap
all pirqs for a pci device, when the pci device disables its MSI/MSIX
vectors; specifically the Qemu commit:
c976437c7dba9c7444fb41df45468968aaa326ad
("qemu-xen: free all the pirqs for msi/msix when driver unload")

Once Qemu added this pirq unmapping, it was no longer correct for the
kernel to re-use the pirq number cached in the pci device msi message
data.  All Qemu releases since 2.1.0 contain the patch that unmaps the
pirqs when the pci device disables its MSI/MSIX vectors.

This bug is causing failures to initialize multiple NVMe controllers
under Xen, because the NVMe driver sets up a single MSIX vector for
each controller (concurrently), and then after using that to talk to
the controller for some configuration data, it disables the single MSIX
vector and re-configures all the MSIX vectors it needs.  So the MSIX
setup code tries to re-use the cached pirq from the first vector
for each controller, but the hypervisor has already given away that
pirq to another controller, and its initialization fails.

5520b7e7d2 x86/boot/e820: Remove spurious asm/e820/api.h inclusions
A commonly used lowlevel x86 header, asm/pgtable.h, includes asm/e820/api.h
spuriously, without making direct use of it.

Removing it is not simple: over the years various .c code learned to rely
on this indirect inclusion.

Remove the unnecessary include - this should speed up the kernel build a bit,
as a large header is not included anymore in totally unrelated code.

Martin reported that the Supermicro X8DTH-i/6/iF/6F advertises incorrect
host bridge windows via _CRS:

  pci_root PNP0A08:00: host bridge window [io  0xf000-0xffff]
  pci_root PNP0A08:01: host bridge window [io  0xf000-0xffff]

Both bridges advertise the 0xf000-0xffff window, which cannot be correct.

Work around this by ignoring _CRS on this system.  The downside is that we
may not assign resources correctly to hot-added PCI devices (if they are
possible on this system).

c8b877a5e5 x86/pci/amd-bus: Convert to hotplug state machine
Install the callbacks via the state machine and let the core invoke
the callbacks on the already online CPUs.

The smp_call_function_single() is dropped because the ONLINE callback is
invoked on the target CPU since commit 1cf4f629d9 ("cpu/hotplug: Move
online calls to hotplugged cpu"). smp_call_function_single() invokes the
invoked function with interrupts disabled, but this calling convention is
not preserved as the MSR is not modified by anything else than this code.

72a9b18629 xen: Remove event channel notification through Xen PCI platform device
Ever since commit 254d1a3f02 ("xen/pv-on-hvm kexec: shutdown watches
from old kernel") using the INTx interrupt from Xen PCI platform
device for event channel notification would just lockup the guest
during bootup.  postcore_initcall now calls xs_reset_watches which
will eventually try to read a value from XenStore and will get stuck
on read_reply at XenBus forever since the platform driver is not
probed yet and its INTx interrupt handler is not registered yet. That
means that the guest can not be notified at this moment of any pending
event channels and none of the per-event handlers will ever be invoked
(including the XenStore one) and the reply will never be picked up by
the kernel.

The exact stack where things get stuck during xenbus_init:

Vector callbacks have always been the favourite event notification
mechanism since their introduction in commit 38e20b07ef ("x86/xen:
event channels delivery on HVM.") and the vector callback feature has
always been advertised for quite some time by Xen that's why INTx was
broken for several years now without impacting anyone.

Luckily this also means that event channel notification through INTx
is basically dead-code which can be safely removed without impacting
anybody since it has been effectively disabled for more than 4 years
with nobody complaining about it (at least as far as I'm aware of).

This commit removes event channel notification through Xen PCI
platform device.

Add set_dev_domain_options() to set PCI domain-specific options as devices
are added.  The first usage is to request exclusive userspace control of
PCIe hotplug indicators in VMD domains.

Devices in a VMD domain use PCIe hotplug Attention and Power Indicators in
a non-standard way; tell pciehp to ignore the indicators so userspace can
control them via the sysfs "attention" file.

To determine whether a bus is within a VMD domain, add a bool to the
pci_sysdata structure that the VMD driver sets during initialization.

ee6ee49fd0 x86/PCI: VMD: Synchronize with RCU freeing MSI IRQ descs
Fix a potential race when disabling MSI/MSI-X on a VMD domain device.  If
the VMD interrupt service is running, it may see a disabled IRQ.  We can
synchronize RCU just before freeing the MSI descriptor.  This is safe since
the irq_desc lock isn't held, and the descriptor is valid even though it is
disabled.  After vmd_msi_free(), though, the handler is reinitialized to
handle_bad_irq(), so we can't let the VMD ISR's list iteration see the
disabled IRQ after this.

6af7e4f772 PCI: Mark Haswell Power Control Unit as having non-compliant BARs
The Haswell Power Control Unit has a non-PCI register (CONFIG_TDP_NOMINAL)
where BAR 0 is supposed to be.  This is erratum HSE43 in the spec update
referenced below:

  The PCIe* Base Specification indicates that Configuration Space Headers
  have a base address register at offset 0x10.  Due to this erratum, the
  Power Control Unit's CONFIG_TDP_NOMINAL CSR (Bus 1; Device 30; Function
  3; Offset 0x10) is located where a base register is expected.

Mark the PCU as having non-compliant BARs so we don't try to probe any of
them.  There are no other BARs on this device.

Rename the quirk so it's not Broadwell-specific.

We can't initialize the list head on deletion as this causes the node to
point to itself, which causes an infinite loop if vmd_irq() happens to be

The list initialization was trying to fix a bug from multiple calls to

the interrupt is enabled.

this applies several uses of __ro_after_init to structures that are
only updated during __init, and const for some structures that are
never updated.  Additionally extends __init markings to some functions
that are only used during __init, and cleans up some missing C99 style

00085f1efa dma-mapping: use unsigned long for dma_attrs
The dma-mapping core and the implementations do not change the DMA
attributes passed by pointer.  Thus the pointer can point to const data.
However the attributes do not have to be a bitfield.  Instead unsigned
long will do fine:

1. This is just simpler.  Both in terms of reading the code and setting
   attributes.  Instead of initializing local attributes on the stack
   and passing pointer to it to dma_set_attr(), just set the bits.

2. It brings safeness and checking for const correctness because the
   attributes are passed by value.

eb008eb6f8 x86: Audit and remove any remaining unnecessary uses of module.h
Historically a lot of these existed because we did not have
a distinction between what was modular code and what was providing
support to modules via EXPORT_SYMBOL and friends.  That changed
when we forked out support for the latter into the export.h file.

This means we should be able to reduce the usage of module.h
in code that is obj-y Makefile or bool Kconfig.  In the case of
some of these which are modular, we can extend that to also include
files that are building basic support functionality but not related
to loading or registering the final module; such files also have
no need whatsoever for module.h

The advantage in removing such instances is that module.h itself
sources about 15 other headers; adding significantly to what we feed
cpp, and it can obscure what headers we are effectively using.

Since module.h was the source for init.h (for __init) and for
export.h (for EXPORT_SYMBOL) we consider each instance for the
presence of either and replace as needed.

In the case of crypto/glue_helper.c we delete a redundant instance
of MODULE_LICENSE in order to delete module.h -- the license info
is already present at the top of the file.

The uncore change warrants a mention too; it is uncore.c that uses
module.h and not uncore.h; hence the relocation done there.

power off specific Moorestown devices after the following commit:

... because the condition in mrfld_power_off_dev() is true for any platform
from the above list.

Remove duplicate power off certain devices on Intel Moorestown and rename

30ce035038 x86/PCI/VMD: Use untracked irq handler
There is no way to know which device in a VMD triggered an interrupt
without invoking every registered driver's actions. This uses the
untracked irq handler so that a less used device does not trigger
spurious interrupt.

We have been previously recommending users to enable "noirqdebug", but do
not want to force a system setting just to keep this domain functional.

bb27570525 x86/platform/intel_mid_pci: Rework IRQ0 workaround
On Intel Merrifield platform several PCI devices have a bogus configuration,
i.e. the IRQ0 had been assigned to few of them. These are PCI root bridge,
eMMC0, HS UART common registers, PWM, and HDMI. The actual interrupt line can
be allocated to one device exclusively, in our case to eMMC0, the rest should
cope without it and basically known drivers for them are not using interrupt
line at all.

Rework IRQ0 workaround, which was previously done to avoid conflict between
eMMC0 and HS UART common registers, to behave differently based on the device
in question, i.e. allocate interrupt line to eMMC0, but silently skip interrupt
allocation for the rest except HS UART common registers which are not used
anyway. With this rework IOSF MBI driver in particular would be used.

Commit b894157145 ("x86/PCI: Mark Broadwell-EP Home Agent & PCU as having
non-compliant BARs") marked Home Agent 0 & PCU has having non-compliant
BARs.  Home Agent 1 also has non-compliant BARs.

Mark Home Agent 1 as having non-compliant BARs so the PCI core doesn't
touch them.

The problem with these devices is documented in the Xeon v4 specification
update:

  BDF2          PCI BARs in the Home Agent Will Return Non-Zero Values
                During Enumeration

  Problem:      During system initialization the Operating System may access
                the standard PCI BARs (Base Address Registers).  Due to
                this erratum, accesses to the Home Agent BAR registers (Bus
                1; Device 18; Function 0,4; Offsets (0x14-0x24) will return
                non-zero values.

  Implication:  The operating system may issue a warning.  Intel has not
                observed any functional failures due to this erratum.

0c0e0736ac PCI: Set ROM shadow location in arch code, not in PCI core
IORESOURCE_ROM_SHADOW means there is a copy of a device's option ROM in
RAM.  The existence of such a copy and its location are arch-specific.
Previously the IORESOURCE_ROM_SHADOW flag was set in arch code, but the
0xC0000-0xDFFFF location was hard-coded into the PCI core.

If we're using a shadow copy in RAM, disable the ROM BAR and release the
address space it was consuming.  Move the location information from the PCI
core to the arch code that sets IORESOURCE_ROM_SHADOW.  Save the location
of the RAM copy in the struct resource for PCI_ROM_RESOURCE.

After this change, pci_map_rom() will call pci_assign_resource() and
pci_enable_rom() for these IORESOURCE_ROM_SHADOW resources, which we did
not do before.  This is safe because:

  - pci_assign_resource() will do nothing because the resource is marked
    IORESOURCE_PCI_FIXED, which means we can't move it, and

  - pci_enable_rom() will not turn on the ROM BAR's enable bit because the
    resource is marked IORESOURCE_ROM_SHADOW, which means it is in RAM
    rather than in PCI memory space.

Storing the location in the struct resource means "lspci" will show the
shadow location, not the value from the ROM BAR.

A shadow copy of an option ROM is placed by the BIOS as a fixed address.
Set IORESOURCE_PCI_FIXED to indicate that we can't move the shadow copy.
This prevents warnings like the following when we assign resources:

  BAR 6: [??? 0x00000000 flags 0x2] has bogus alignment

This warning is emitted by pdev_sort_resources(), which already ignores
IORESOURCE_PCI_FIXED resources.

The Home Agent and PCU PCI devices in Broadwell-EP have a non-BAR register
where a BAR should be.  We don't know what the side effects of sizing the
"BAR" would be, and we don't know what address space the "BAR" might appear
to describe.

Mark these devices as having non-compliant BARs so the PCI core doesn't
touch them.

6c777e8799 Revert "PCI, x86: Implement pcibios_alloc_irq() and pcibios_free_irq()"
991de2e590 ("PCI, x86: Implement pcibios_alloc_irq() and
pcibios_free_irq()") appeared in v4.3 and helps support IOAPIC hotplug.

Олег reported that the Elcus-1553 TA1-PCI driver worked in v4.2 but not
v4.3 and bisected it to 991de2e590.  Sunjin reported that the RocketRAID
272x driver worked in v4.2 but not v4.3.  In both cases booting with
"pci=routirq" is a workaround.

I think the problem is that after 991de2e590, we no longer call
pcibios_enable_irq() for upstream bridges.  Prior to 991de2e590, when a
driver called pci_enable_device(), we recursively called
pcibios_enable_irq() for upstream bridges via pci_enable_bridge().

After 991de2e590, we call pcibios_enable_irq() from pci_device_probe()
instead of the pci_enable_device() path, which does *not* call
pcibios_enable_irq() for upstream bridges.

Revert 991de2e590 to fix these driver regressions.

185a383ada x86/PCI: Add driver for Intel Volume Management Device (VMD)
The Intel Volume Management Device (VMD) is a Root Complex Integrated
Endpoint that acts as a host bridge to a secondary PCIe domain.  BIOS can
reassign one or more Root Ports to appear within a VMD domain instead of
the primary domain.  The immediate benefit is that additional PCIe domains
allow more than 256 buses in a system by letting bus numbers be reused
across different domains.

VMD domains do not define ACPI _SEG, so to avoid domain clashing with host
bridges defining this segment, VMD domains start at 0x10000, which is
greater than the highest possible 16-bit ACPI defined _SEG.

This driver enumerates and enables the domain using the root bus
configuration interface provided by the PCI subsystem.  The driver provides
configuration space accessor functions (pci_ops), bus and memory resources,
an MSI IRQ domain with irq_chip implementation, and DMA operations
necessary to use devices through the VMD endpoint's interface.

   1) Configuration Space: BAR 0 ("CFGBAR") of VMD provides the base
   address and size for configuration space register access to VMD-owned
   root ports.  It works similarly to MMCONFIG for extended configuration
   space.  Bus numbering is independent and does not conflict with the
   primary domain.

   2) MMIO Space: BARs 2 and 4 ("MEMBAR1" and "MEMBAR2") of VMD provide the
   base address, size, and type for MMIO register access.  These addresses
   are not translated by VMD hardware; they are simply reservations to be
   distributed to root ports' memory base/limit registers and subdivided
   among devices downstream.

   3) DMA: To interact appropriately with an IOMMU, the source ID DMA read
   and write requests are translated to the bus-device-function of the VMD
   endpoint.  Otherwise, DMA operates normally without VMD-specific address
   translation.

   4) Interrupts: Part of VMD's BAR 4 is reserved for VMD's MSI-X Table and
   PBA.  MSIs from VMD domain devices and ports are remapped to appear as
   if they were issued using one of VMD's MSI-X table entries.  Each MSI
   and MSI-X address of VMD-owned devices and ports has a special format
   where the address refers to specific entries in the VMD's MSI-X table.
   As with DMA, the interrupt source ID is translated to VMD's
   bus-device-function.

   The driver provides its own MSI and MSI-X configuration functions
   specific to how MSI messages are used within the VMD domain, and
   provides an irq_chip for independent IRQ allocation to relay interrupts
   from VMD's interrupt handler to the appropriate device driver's handler.

   5) Errors: PCIe error message are intercepted by the root ports normally
   (e.g., AER), except with VMD, system errors (i.e., firmware first) are
   disabled by default.  AER and hotplug interrupts are translated in the
   same way as endpoint interrupts.

   6) VMD does not support INTx interrupts or IO ports.  Devices or drivers
   requiring these features should either not be placed below VMD-owned
   root ports, or VMD should be disabled by BIOS for such endpoints.

d9c3d6ff22 x86/PCI: Allow DMA ops specific to a PCI domain
The Intel Volume Management Device (VMD) is a PCIe endpoint that acts as a
host bridge to another PCI domain.  When devices below the VMD perform DMA,
the VMD replaces their DMA source IDs with its own source ID.  Therefore,
those devices require special DMA ops.

Add interfaces to allow the VMD driver to set up dma_ops for the devices
below it.

Commit 4d6b4e69a2 ("x86/PCI/ACPI: Use common interface to support
PCI host bridge") converted x86 to use the common interface
acpi_pci_root_create, but the conversion missed on code piece in
arch/x86/pci/bus_numa.c, which causes regression on some legacy
AMD platforms as reported by Arthur Marsh <arthur.marsh@internode.on.net>.
The root causes is that acpi_pci_root_create() fails to insert
host bridge resources into iomem_resource/ioport_resource because
x86_pci_root_bus_resources() has already inserted those resources.
So change x86_pci_root_bus_resources() to not insert resources into
iomem_resource/ioport_resource.

8affb487d4 x86/PCI: Don't alloc pcibios-irq when MSI is enabled
The pcibios-irq and MSI both use dev->irq to store the IRQ number.  While
the MSI code checks for that and frees the pcibios-irq before overwriting
dev->irq, the pcibios_alloc_irq() function does not.

Usually this is not a problem, as the pcibios-irq is allocated before probe
time of the device and the MSI IRQ is allocted from the driver's probe
path.

But there are PCI devices handled by the core kernel and not by a standard
PCI driver, like the AMD IOMMU for example.  For the AMD IOMMU a normal PCI
device driver does not make sense, because a driver can be forcibly unbound
from its device, which is not a good idea for an IOMMU.

Nevertheless the PCI core code tries to match the PCI device implementing
the AMD IOMMU against drivers, and allocates/frees a pcibios IRQ every time
it tries out a new driver.  This overwrites the dev->irq field set by
pci_enable_msi() and sets it to 0 in the end (because the probe fails and
the pcibios-irq is freed again).

On suspend/resume this breaks the kernel, because the IRQ descriptor for
IRQ 0 is NULL.

Fix this by not allocating a pcibios-irq when MSI is already active.  This
also has the benefit, that a device claimed by the core kernel can not be
probed by a PCI driver later.

Revert dff22d2054 ("PCI: Call pci_read_bridge_bases() from core instead
of arch code").

Reading PCI bridge windows is not arch-specific in itself, but there is PCI
core code that doesn't work correctly if we read them too early.  For
example, Hannes found this case on an ARM Freescale i.mx6 board:

  pci_bus 0000:00: root bus resource [mem 0x01000000-0x01efffff]
  pci 0000:00:00.0: PCI bridge to [bus 01-ff]
  pci 0000:00:00.0: BAR 8: no space for [mem size 0x01000000] (mem window)
  pci 0000:01:00.0: BAR 2: failed to assign [mem size 0x00200000]
  pci 0000:01:00.0: BAR 1: failed to assign [mem size 0x00004000]
  pci 0000:01:00.0: BAR 0: failed to assign [mem size 0x00000100]

The 00:00.0 mem window needs to be at least 3MB: the 01:00.0 device needs
0x204100 of space, and mem windows are megabyte-aligned.

Bus sizing can increase a bridge window size, but never *decrease* it (see
d65245c329 ("PCI: don't shrink bridge resources")).  Prior to
dff22d2054, ARM didn't read bridge windows at all, so the "original size"
was zero, and we assigned a 3MB window.

After dff22d2054, we read the bridge windows before sizing the bus.  The
firmware programmed a 16MB window (size 0x01000000) in 00:00.0, and since
we never decrease the size, we kept 16MB even though we only needed 3MB.
But 16MB doesn't fit in the host bridge aperture, so we failed to assign
space for the window and the downstream devices.

I think this is a defect in the PCI core: we shouldn't rely on the firmware
to assign sensible windows.

Ray reported a similar problem, also on ARM, with Broadcom iProc.

Issues like this are too hard to fix right now, so revert dff22d2054.
