Benoit Taine 9baa3c34ac PCI: Remove DEFINE_PCI_DEVICE_TABLE macro use
We should prefer `struct pci_device_id` over `DEFINE_PCI_DEVICE_TABLE` to
meet kernel coding style guidelines. This issue was reported by checkpatch.

A simplified version of the semantic patch that makes this change is as
follows (http://coccinelle.lip6.fr/):

// <smpl>

@@
identifier i;
declarer name DEFINE_PCI_DEVICE_TABLE;
initializer z;
@@

- DEFINE_PCI_DEVICE_TABLE(i)
+ const struct pci_device_id i[]
= z;

// </smpl>

* Hyper-V Synthetic Video Frame Buffer Driver
*
* This is the driver for the Hyper-V Synthetic Video, which supports
* screen resolution up to Full HD 1920x1080 with 32 bit color on Windows
* Server 2012, and 1600x1200 with 16 bit color on Windows Server 2008 R2
* or earlier.
*
* It also solves the double mouse cursor issue of the emulated video mode.
*
* The default screen resolution is 1152x864, which may be changed by a
* kernel parameter:
* video=hyperv_fb:<width>x<height>
* For example: video=hyperv_fb:1280x1024
*
* Portrait orientation is also supported:
* For example: video=hyperv_fb:864x1152
