Merge tag 'v4.20-rc3' into for-4.21/block

Merge in -rc3 to resolve a few conflicts, but also to get a few
important fixes that have gone into mainline since the block
4.21 branch was forked off (most notably the SCSI queue issue,

a3ecf48248 scsi: myrs: only build on little-endian platforms
Reading throught the new driver, I noticed that this cannot work on
big-endian CPUs, and the old DAC960 had exactly the same behavior.

To document this for the future, add a Kconfig dependency that prevents it
from being included in big-endian kernels.  Since the hardware is really
old and we never had a working driver on it for big-endian platforms,
it's unlikely to make a difference to users.

This is mostly updates of the usual drivers: UFS, esp_scsi, NCR5380,
 qla2xxx, lpfc, libsas, hisi_sas.  In addition there's a set of mostly
 small updates to the target subsystem a set of conversions to the
 generic DMA API, which do have some potential for issues in the older
 drivers but we'll handle those as case by case fixes. A new myrs for
 the DAC960/mylex raid controllers to replace the block based DAC960
 which is also being removed by Jens in this merge window. Plus the
 usual slew of trivial changes.

53dce332db scsi: esp_scsi: De-duplicate PIO routines
As a temporary measure, the code to implement PIO transfers was
duplicated in zorro_esp and mac_esp. Now that it has stabilized move the
common code into the core driver but don't build it unless needed.

This replaces the inline assembler with more portable writesb() calls.
Optimizing the m68k writesb() implementation is a separate patch.

The NCR QUAD 720 SCSI driver depends on microchannel support, but
microchannel support was removed from the kernel with commit bb8187d35f
("MCA: delete all remaining traces of microchannel bus support.") over 6
years ago.

So zap the Q720 SCSI driver as well.

d5038a13ec scsi: core: switch to scsi-mq by default
It has been more than one year since we tried to change the default from
legacy to multi queue in SCSI with commit c279bd9e406 ("scsi: default to
scsi-mq"). But due to issues with suspend/resume and performance problems
it had been reverted again with commit cbe7dfa26e ("Revert "scsi: default
to scsi-mq"").

In the meantime there have been a substantial amount of performance
improvements and suspend/resume got fixed as well, thus we can re-enable
scsi-mq without a significant performance penalty.

3109e5ae03 scsi: zorro_esp: New driver for Amiga Zorro NCR53C9x boards
New combined SCSI driver for all ESP based Zorro SCSI boards for m68k Amiga.

Code largely based on board specific parts of the old drivers (blz1230.c,
blz2060.c, cyberstorm.c, cyberstormII.c, fastlane.c which were removed after
the 2.6 kernel series for lack of maintenance) with contributions by Tuomas
Vainikka (TCQ bug tests and workaround) and Finn Thain (TCQ bugfix by use of
PIO in extended message in transfer).

New Kconfig option and Makefile entries for new Amiga Zorro ESP SCSI driver
included in this patch.

Use DMA transfers wherever possible, with board-specific DMA set-up functions
copied from the old driver code. Three byte reselection messages do appear to
cause DMA timeouts. So wire up a PIO transfer routine for these
instead. esp_reselect_with_tag explicitly sets
esp->cmd_block_dma as target address for the message bytes but PIO
requires a virtual address.  Substiute kernel virtual address
esp->cmd_block in PIO transfer call if DMA address is esp->cmd_block_dma
and phase is message in.

PIO code taken from mac_esp.c where the reselection timeout issue was debugged
and fixed first, with minor macro and function rename.

c091fbe9a2 block: fix CDROM dependency on BLK_DEV
After the cdrom cleanup, I get randconfig warnings for some configurations:

warning: (BLK_DEV_IDECD && BLK_DEV_SR) selects CDROM which has unmet direct dependencies (BLK_DEV)

This adds an explicit BLK_DEV dependency for both drivers. The other
drivers that select 'CDROM' already have this and don't need a change.

2a750166a5 block: Rework drivers/cdrom/Makefile
Instead of referring from inside drivers/cdrom/Makefile to all the
drivers that use this driver, let these drivers select the cdrom
driver. This change makes the cdrom build code follow the approach
that is used for most other drivers, namely refer from the higher
layers to the lower layer instead of from the lower layer to the
higher layers.

cbe7dfa26e Revert "scsi: default to scsi-mq"
Defaulting to scsi-mq in 4.13-rc has shown various regressions
on setups that we didn't previously consider.  Fixes for them are
in progress, but too invasive to make it in this cycle.  So for
now revert the commit that defaults to blk-mq for SCSI.  For 4.14
we'll plan to try again with these fixes.

This reverts commit 5c279bd9e4.

7d7080335f scsi: lpfc: Finalize Kconfig options for nvme
Reviewing the result of what was just added for Kconfig, we made a poor
choice. It worked well for full kernel builds, but not so much for how
it would be deployed on a distro.

Here's the final result:
- lpfc will compile in NVME initiator and/or NVME target support based
  on whether the kernel has the corresponding subsystem support.
  Kconfig is not used to drive this specifically for lpfc.
- There is a module parameter, lpfc_enable_fc4_type, that indicates
  whether the ports will do FCP-only or FCP & NVME (NVME-only not yet
  possible due to dependency on fc transport). As FCP & NVME divvys up
  exchange resources, and given NVME will not be often initially, the
  default is changed to FCP only.

We only need this code to support scsi, ide, cciss and virtio.  And at
least for virtio it's a deprecated feature to start with.

This should shrink the kernel size for embedded device that only use,
say eMMC a bit.
