The external interrupt interception can only occur in rare cases, e.g.
when the PSW of the interrupt handler has a bad value. The old handler
for this interception simply ignored these events (except for increasing
the exit_external_interrupt counter), but for proper operation we either
have to inject the interrupts manually or we should drop to userspace in
case of errors.

When copy_from_user() fails, this code returns the number of bytes
remaining instead of a negative error code. The positive number is
returned to the user but otherwise it is harmless.

This patch enables the IBS facility when a single VCPU is running.
The facility is dynamically turned on/off as soon as other VCPUs
enter/leave the stopped state.
When this facility is operating, some instructions can be executed
faster for single-cpu guests.

This patch introduces two new functions to set/clear the CPUSTAT_STOPPED bit and
makes use of it at all applicable places. These functions prepare the additional
execution of code when starting/stopping a vcpu.
The CPUSTAT_STOPPED bit should not be touched outside of these functions.

The s390 architecture has a special protection mechanism that can
be used to prevent write access to the vital data in the low-core
memory area. This patch adds a new helper function that can be used
to check for such write accesses and in case of protection, it also
sets up the exception data accordingly.

When the guest executes the MVPG instruction with DAT disabled,
and the source or destination page is not mapped in the host,
the so-called partial execution interception occurs. We need to
handle this event by setting up a mapping for the corresponding
user pages.

When a guest is single-stepped, we want to disable timer interrupts. Otherwise,
the guest will continuously execute the external interrupt handler and make
debugging of code where timer interrupts are enabled almost impossible.
The delivery of timer interrupts can be enforced in such sections by setting a
breakpoint and continuing execution.
In order to disable timer interrupts, they are disabled in the control register
of the guest just before SIE entry and are suppressed in the interrupt
check/delivery methods.

This patch adds support to debug the guest using the PER facility on s390.
Single-stepping, hardware breakpoints and hardware watchpoints are supported. In
order to use the PER facility of the guest without it noticing it, the control
registers of the guest have to be patched and access to them has to be
intercepted(stctl, stctg, lctl, lctlg).
All PER program interrupts have to be intercepted and only the relevant PER
interrupts for the guest have to be given back. Special care has to be taken
about repeated exits on the same hardware breakpoint. The intervention of the
host in the guests PER configuration is not fully transparent. PER instruction
nullification can not be used by the guest and too many storage alteration
events may be reported to the guest (if it is activated for special address
ranges only) when the host concurrently debugging it.

When a program interrupt was to be delivered until now, no program interrupt
parameters were stored in the low-core of the target vcpu.
This patch enables the delivery of those program interrupt parameters, takes
care of concurrent PER events which can be injected in addition to any program
interrupt and uses the correct instruction length code (depending on the
interception code) for the injection of program interrupts.

Whenever a program interrupt is intercepted, some parameters are stored in the
sie control block. These parameters have to be extracted in order to be
reinjected correctly. This patch also takes care of intercepted PER events which
can occurr in addition to any program interrupt.

Convert handle_tpi() to new guest access functions.
The code now sets up a structure which is copied with a single call to
guest space instead of issuing several separate guest access calls.
This is necessary since the to be copied data may cross a page boundary.
If a protection exception happens while accessing any of the pages, the
instruction is suppressed and may not have modified any memory contents.

The new guest memory access function write_guest() and read_guest() can be
used to access guest memory in an architecture compliant way.
These functions will look at the vcpu's PSW and select the correct address
space for memory access and also perform correct address wrap around.
In case DAT is turned on, page tables will be walked otherwise access will
happen to real or absolute memory.
Any access exception will be recognized and exception data will be stored
in the vcpu's kvm_vcpu_arch.pgm member. Subsequently an exception can be
injected if necessary.
Missing are:
- key protection checks
- access register mode support
- program event recording support
This patch also adds write_guest_real(), read_guest_real(),
write_guest_absolute() and read_guest_absolute() guest functions which can
be used to access real and absolute storage. These functions currently do
not perform any access checks, since there is no use case (yet?).

put_guest_lc, read_guest_lc and write_guest_lc are guest access
functions which shall only be used to access the lowcore of a vcpu.
These functions should be used for e.g. interrupt handlers where no
guest memory access protection facilities, like key or low address
protection, are applicable.
At a later point guest vcpu lowcore access should happen via pinned
prefix pages, so that these pages can be accessed directly via the
kernel mapping. All of these *_lc functions can be removed then.

Add a 'struct kvm_s390_pgm_info pgm' member to kvm_vcpu_arch. This
structure will be used if during instruction emulation in the context
of a vcpu exception data needs to be stored somewhere.

Add kvm_s390_logical_to_effective() helper which converts a guest vcpu's
logical storage address to a guest vcpu effective address by applying the
rules of the vcpu's addressing mode defined by PSW bits 31 and 32
(extendended and basic addressing mode).
Depending on the vcpu's addressing mode the upper 40 bits (24 bit addressing
mode), 33 bits (31 bit addressing mode) or no bits (64 bit addressing mode)
will be zeroed and the remaining bits will be returned.

When userspace reset the guest without notifying kvm, the CMMA state
of the pages might be unused, resulting in guest data corruption.
To avoid this, CMMA must be enabled only if userspace understands
the implications.
CMMA must be enabled before vCPU creation. It can't be switched off
once enabled. All subsequently created vCPUs will be enabled for
CMMA according to the CMMA state of the VM.

page_table_reset_pgste() already does a complete page table walk to
reset the pgste. Enhance it to initialize the storage keys to
PAGE_DEFAULT_KEY if requested by the caller. This will be used
for lazy storage key handling. Also provide an empty stub for
!CONFIG_PGSTE
Lets adopt the current code (diag 308) to not clear the keys.

When kvm_get_vcpu() returned NULL for the destination CPU in
__sigp_emergency() or __sigp_external_call(), the memory for the
"inti" structure was not released anymore. This patch fixes this
issue by moving the check for !dst_vcpu before the kzalloc() call.

We allocate a page for the 2k sca, so lets use the space to improve
hit rate of some internal cpu caches. No need to change the freeing

Since commit 7c470539c9
(s390/kvm: avoid automatic sie reentry) we will run through the C code
of KVM on host interrupts instead of just reentering the guest. This
will result in additional ucontrol exits (at least HZ per second). Let
handle a 0 intercept in the kernel and dont return to userspace,
even if in ucontrol mode.

The BUG_ON in kvm-s390.c is unreachable, as we get the vcpu per common code,
which itself does this from the private_data field of the file descriptor,
and there is no KVM_UNCREATE_VCPU.
The __{set,unset}_cpu_idle BUG_ONs are not triggerable because the vcpu
creation code already checks against KVM_MAX_VCPUS.

When SIGP SET_PREFIX is called with an illegal CPU id, it must return
the condition code 3 ("not operational") instead of 1. Also fixed the
order in which the checks are done - CC3 has a higher priority than CC1.
And while we're at it, this patch also get rid of the floating interrupt
lock here by using kvm_get_vcpu() to get the local_int struct of the
destination CPU.

Commit "kvm: Record the preemption status of vcpus using preempt notifiers"
caused a performance regression on s390. It turned out that in the case that
if a former sleeping cpu, that was woken up, this cpu is not a yield candidate
since it gave up the cpu voluntarily. To retain this candiate its preempted
flag is set during wakeup and interrupt delivery time.
Significant performance measurement work and code analysis to solve this
issue was provided by Mao Chuan Li and his team in Beijing.

This patch enables Collaborative Memory Management (CMM) for kvm
on s390. CMM allows the guest to inform the host about page usage
(see arch/s390/mm/cmm.c). The host uses this information to avoid
swapping in unused pages in the page fault handler. Further, a CPU
provided list of unused invalid pages is processed to reclaim swap
space of not yet accessed unused pages.

This patch enables async page faults for s390 kvm guests.
It provides the userspace API to enable and disable_wait this feature.
The disable_wait will enforce that the feature is off by waiting on it.
Also it includes the diagnose code, called by the guest to enable async page faults.
The async page faults will use an already existing guest interface for this
purpose, as described in "CP Programming Services (SC24-6084)".

Userspace can flood the kernel with interrupts as of now, so let's
limit the number of pending floating interrupts injected via either
the floating interrupt controller or the KVM_S390_INTERRUPT ioctl.
We can have up to 4*64k pending subchannels + 8 adapter interrupts,
as well as up to ASYNC_PF_PER_VCPU*KVM_MAX_VCPUS pfault done interrupts.
There are also sclp and machine checks. This gives us
(4*65536+8+64*64+1+1) = 266250 interrupts.

(KVM: s390: Removed SIE_INTERCEPT_UCONTROL) lost a hunk when we
reworked our patch queue to rework the async_fp code. We now
ignore faults on the sie instruction (guest accesses non-existing
memory) instead of sending a fault into the guest. This leads to
hang situations with the old virtio transport that checks for
descriptor memory after guest memory. Instead of bailing out this
code now goes wild...
Lets re-add the check.

This patch enables transactional execution for KVM guests
on s390 systems zec12 or later.
We rework the allocation of the page containing the sie_block
to also back the Interception Transaction Diagnostic Block.
If available the TE facilities will be enabled.
Setting bit 73 and 50 in vfacilities bitmask reveals the HW
facilities Transactional Memory and Constraint Transactional
Memory respectively to the KVM guest.
Furthermore, the patch restores the Program-Interruption TDB
from the Interception TDB in case a program interception has
occurred and the ITDB has a valid format.

The diagnose 500 subcode 3 contains the 32 bit subchannel id in bits 32-63
(counting from the left). As for other I/O instructions, bits 0-31 should be
ignored and thus not be passed to kvm_io_bus_write_cookie().
This fixes a bug where the guest passed non-zero bits 0-31 which the
host tried to interpret, leading to ioeventfd notification failures.

When SIGP RESTART detected an illegal CPU address, there is no need to
drop to userspace, we can return CC3 to the guest directly instead.
Also renamed __sigp_restart() to sigp_check_callable() (since this
is a better description of what the function is really doing) and
moved a string specific to RESTART to the calling place instead, so
that this function gets usable by other SIGP orders, too.

The SIGP order STOP_AND_STORE_STATUS is defined to stop a CPU and store
its status. However, we only stored the status if the CPU was still
running, so make sure that the status is now also stored if the CPU was
already stopped. This fixes the problem that the CPU information was
not stored correctly in kdump files, rendering them unreadable.

The EQBS and SQBS instructions do not set CC3 for invalid channels, but
should throw an operation exception instead when not available. Thus they
should not be handled by the handle_io_inst() wrapper but drop to userspace

The TPI handler currently uses vcpu->run->s.regs.crs[6] to get the current
value of CR6. I think this is wrong, because vcpu->run->s.regs.crs is
only updated when kvm_arch_vcpu_ioctl_run() drops back to userspace.
So let's change the TPI handler to use vcpu->arch.sie_block->gcr[6] instead.

is probably the most interesting change from a user point of view.
On the x86 side there are nested virtualization improvements and a
few bugfixes. ARM got transparent huge page support, improved
overcommit, and support for big endian guests.
Finally, there is a new interface to connect KVM with VFIO. This
helps with devices that use NoSnoop PCI transactions, letting the
driver in the guest execute WBINVD instructions. This includes
some nVidia cards on Windows, that fail to start without these
patches and the corresponding userspace changes.

The FPC_VALID_MASK has been used to check the validity of the value
to be loaded into the floating-point-control register. With the
introduction of the floating-point extension facility and the
decimal-floating-point additional bits have been defined which need
to be checked in a non straight forward way. So far these bits have
been ignored which can cause an incorrect results for decimal-
floating-point operations, e.g. an incorrect rounding mode to be
set after signal return.
The static check with the FPC_VALID_MASK is replaced with a trial
load of the floating-point-control value, see test_fp_ctl.
In addition an information leak with the padding word between the
floating-point-control word and the floating-point registers in
the s390_fp_regs is fixed.

The result of the store-clock-fast (STCKF) instruction is a bit fuzzy.
It can happen that the value stored on one CPU is smaller than the value
stored on another CPU, although the order of the stores is the other
way around. This can cause deltas of get_tod_clock() values to become
negative when they should not be.
We need to be more careful with store-clock-fast, this patch partially
reverts git commit e4b7b4238e666682555461fa52eecd74652f36bb "time:
always use stckf instead of stck if available". The get_tod_clock()
function now uses the store-clock-extended (STCKE) instruction.
get_tod_clock_fast() can be used if the fuzziness of store-clock-fast
is acceptable e.g. for wait loops local to a CPU.

The kvm->srcu lock has to be held while accessing the memory of
guests and during certain other actions. This patch now adds
the locks to the __vcpu_run function so that all affected code
is protected now (and additionally to the KVM_S390_STORE_STATUS
ioctl, which can be called out-of-band and needs a separate lock).

The patch renames the array holding the HW facility bitmaps.
This allows to interprete the variable as set of virtual
machine specific "virtual" facilities. The basic idea is
to make virtual facilities externally managable in future.
An availability test for virtual facilites has been added
as well.

Fix the test within handle_pfmf() if the host has the NQ key-setting
facility installed.
Right now the code would incorrectly generate a program check in the
guest if the NQ control bit for a pfmf request was set and if the host
has the NQ key-setting facility installed.

This is called right after the memslots is updated, i.e. when the result
of update_memslots() gets installed in install_new_memslots(). Since
the memslots needs to be updated twice when we delete or move a memslot,
kvm_arch_commit_memory_region() does not correspond to this exactly.
In the following patch, x86 will use this new API to check if the mmio
generation has reached its maximum value, in which case mmio sptes need
to be flushed out.

Saves repeating "(void __force *)__uptr" but it's less clear. Using
the output of PTR_RET() to determine the error rather than just
testing IS_ERR() is odd.
For example, I *assume* __gptr_to_uptr() never returns NULL? Because
the __ret would be 0 for the old code. The new version is clearer, IMHO:
it would try to get_user() on that address.
If you hate this variant, I can just s/PTR_RET/PTR_ERR_OR_ZERO/ instead.

LCTL and LCTLG are also privileged instructions, thus there is no need for
treating them separately from the other instructions in priv.c. So this
patch moves these two instructions to priv.c, adds a check for supervisor
state and simplifies the "handle_eb" instruction decoding by merging the
two eb_handlers jump tables from intercept.c and priv.c into one table only.

We need more fine-grained control about the point in time when we check
for privileged instructions, since the exceptions that can happen during
an instruction have a well-defined priority. For example, for the PFMF
instruction, the check for PGM_PRIVILEGED_OP must happen after the check
for PGM_OPERATION since the latter has a higher precedence - thus the
check for privileged operation must not be done in kvm_s390_handle_b9()
already.

The handle_epsw() function calculated the first register in the wrong way,
so that it always used r0 by mistake. Now the code uses the common helper

Lets use the common waitqueue for kvm cpus on s390. By itself it is
just a cleanup, but it should also improve the accuracy of diag 0x44
which is implemented via kvm_vcpu_on_spin. kvm_vcpu_on_spin has
an explicit check for waiting on the waitqueue to optimize the
yielding.

This patch enables kvm to give large pages to the guest. The heavy
lifting is done by the hardware, the host only has to take care
of the PFMF instruction, which is also part of EDAT-1.
We also support the non-quiescing key setting facility if the host
supports it, to behave similar to the interpretation of sske.

The guest prefix pages must be mapped writeable all the time
while SIE is running, otherwise the guest might see random
behaviour. (pinned at the pte level) Turns out that mlocking is
not enough, the page table entry (not the page) might change or
become r/o. This patch uses the gmap notifiers to kick guest
cpus out of SIE.

Lets provide functions to prevent KVM from reentering SIE and
to kick cpus out of SIE. We cannot use the common kvm_vcpu_kick code,
since we need to kick out guests in places that hold architecture
specific locks (e.g. pgste lock) which might be necessary on the
other cpus - so no waiting possible.
So lets provide a bit in a private field of the sie control block
that acts as a gate keeper, after we claimed we are in SIE.
Please note that we do not reuse prog0c, since we want to access
that bit without atomic ops.

To model the standby memory with memory_region_add_subregion
and friends, the guest would have one or more regions of ram.
Remove the check allowing only one memory slot and the check
requiring the real address of memory slot starts at zero.

kvm_s390_inject_program_int() and friends may fail if no memory is available.
This must be reported to the calling functions, so that this gets passed
down to user space which should fix the situation.
Alternatively we end up with guest state corruption.
So fix this and enforce return value checking by adding a __must_check
annotation to all of these function prototypes.

Being unable to parse the 5- and 8-line if statements I had to split them
to be able to make any sense of them and verify that they match the
architecture.
So change the code since I guess that other people will also have a hard
time parsing such long conditional statements with line breaks.
Introduce a common is_valid_psw() function which does all the checks needed.
In case of lpsw (64 bit psw -> 128 bit psw conversion) it will do some not
needed additional checks, since a couple of bits can't be set anyway, but
that doesn't hurt.

When converting a 64 bit psw to a 128 bit psw the addressing mode bit of
the "addr" part of the 64 bit psw must be moved to the basic addressing
mode bit of the "mask" part of the 128 bit psw.
In addition the addressing mode bit must be cleared when moved to the "addr"
part of the 128 bit psw.
Otherwise an invalid psw would be generated if the orginal psw was in the
31 bit addressing mode.

When checking for validity the lpsw/lpswe handler check that only
the lower 20 bits instead of 24 bits have a non-zero value.
There handling valid psws as invalid ones.

Some memslot updates dont affect the gmap implementation,
e.g. setting/unsetting dirty tracking. Since a gmap update
will cause tlb flushes and segment table invalidations we
want to avoid that.

The code can be significantly shortened. There is no functional change,
except that for large (> PAGE_SIZE) copies the guest translation would
be done more frequently.
However, there is not a single user which does this currently. If one
gets added later on this functionality can be added easily again.

The put_guest_u*/get_guest_u* are nothing but wrappers for the regular
put_user/get_user uaccess functions. The only difference is that before
accessing user space the guest address must be translated to a user space
address.
Change the order of arguments for the guest access functions so they
match their uaccess parts. Also remove the u* suffix, so we simply
have put_guest/get_guest which will automatically use the right size
dependent on pointer type of the destination/source that now must be
correct.
In result the same behaviour as put_user/get_user except that accesses
must be aligned.

Let's change to the paradigm that every return code from guest memory
access functions that is not zero translates to -EFAULT and do not
explictly compare.
Explictly comparing the return value with -EFAULT has already shown to
be a bit fragile. In addition this is closer to the handling of
copy_to/from_user functions, which imho is in general a good idea.
Also shorten the return code handling in interrupt.c a bit.

When out-of-memory the tprot code incorrectly injected a program check
for the guest which reported an addressing exception even if the guest
address was valid.
Let's use the new gmap_translate() which translates a guest address to
a user space address whithout the chance of running into an out-of-memory
situation.
Also make it more explicit that for -EFAULT we won't find a vma.

Guest access functions like copy_to/from_guest() call __guestaddr_to_user()
which in turn call gmap_fault() in order to translate a guest address to a
user space address.
In error case __guest_addr_to_user() returns either -EFAULT or -ENOMEM.
The copy_to/from_guest functions just pass these return values down to the
callers.
The -ENOMEM case however is problematic since there are several places
which access guest memory like:
rc = copy_to_guest(...);
if (rc == -EFAULT)
error_handling();
So in case of -ENOMEM the code assumes that the guest memory access
succeeded even though it failed.
This can cause guest data or state corruption.
If __guestaddr_to_user() returns -ENOMEM the meaning is that a valid user
space mapping exists, but there was not enough memory available when trying
to build the guest mapping. In other words an out-of-memory situation
occured.
For normal user space accesses an out-of-memory situation causes the page
fault handler to map -ENOMEM to -EFAULT (see fixup code in do_no_context()).
We need to do exactly the same for the kvm gaccess functions.
So __guestaddr_to_user() should just map all error codes to -EFAULT.

X86 does not use this any more. The remaining user, s390's !user_alloc
check, can be simply removed since KVM_SET_MEMORY_REGION ioctl is no
longer supported.

The s390 architecture is unique in respect to dirty page detection,
it uses the change bit in the per-page storage key to track page
modifications. All other architectures track dirty bits by means
of page table entries. This property of s390 has caused numerous
problems in the past, e.g. see git commit ef5d437f71
"mm: fix XFS oops due to dirty pages without buffers on s390".
To avoid future issues in regard to per-page dirty bits convert
s390 to a fault based software dirty bit detection mechanism. All
user page table entries which are marked as clean will be hardware
read-only, even if the pte is supposed to be writable. A write by
the user process will trigger a protection fault which will cause
the user pte to be marked as dirty and the hardware read-only bit
is removed.
With this change the dirty bit in the storage key is irrelevant
for Linux as a host, but the storage key is still required for
KVM guests. The effect is that page_test_and_clear_dirty and the
related code can be removed. The referenced bit in the storage
key is still used by the page_test_and_clear_young primitive to
provide page age information.
For page cache pages of mappings with mapping_cap_account_dirty
there will not be any change in behavior as the dirty bit tracking
already uses read-only ptes to control the amount of dirty pages.
Only for swap cache pages and pages of mappings without
mapping_cap_account_dirty there can be additional protection faults.
To avoid an excessive number of additional faults the mk_pte
primitive checks for PageDirty if the pgprot value allows for writes
and pre-dirties the pte. That avoids all additional faults for
tmpfs and shmem pages until these pages are added to the swap cache.

There are two ways to express an interruption subclass:
- As a bitmask, as used in cr6.
- As a number, as used in the I/O interruption word.
Unfortunately, we have treated the I/O interruption word as if it
contained the bitmask as well, which went unnoticed so far as
- (not-yet-released) qemu made the same mistake, and
- Linux guest kernels don't check the isc value in the I/O interruption
word for subchannel interrupts.
Make sure that we treat the I/O interruption word correctly.

Instructions with long displacement have a signed displacement.
Currently the sign bit is interpreted as 2^20: Lets fix it by doing the
sign extension from 20bit to 32bit and then use it as a signed variable
in the addition (see kvm_s390_get_base_disp_rsy).
Furthermore, there are lots of "int" in that code. This is problematic,
because shifting on a signed integer is undefined/implementation defined
if the bit value happens to be negative.
Fortunately the promotion rules will make the right hand side unsigned
anyway, so there is no real problem right now.
Let's convert them anyway to unsigned where appropriate to avoid
problems if the code is changed or copy/pasted later on.

On store status we need to copy the current state of registers
into a save area. Currently we might save stale versions:
The sie state descriptor doesnt have fields for guest ACRS,FPRS,
those registers are simply stored in the host registers. The host
program must copy these away if needed. We do that in vcpu_put/load.
If we now do a store status in KVM code between vcpu_put/load, the
saved values are not up-to-date. Lets collect the ACRS/FPRS before
saving them.
This also fixes some strange problems with hotplug and virtio-ccw,
since the low level machine check handler (on hotplug a machine check
will happen) will revalidate all registers with the content of the
save area.

Converting a 64 Bit TOD format value to nanoseconds means that the value
must be divided by 4.096. In order to achieve that we multiply with 125
and divide by 512.
When used within sched_clock() this triggers an overflow after appr.
417 days. Resulting in a sched_clock() return value that is much smaller
than previously and therefore may cause all sort of weird things in
subsystems that rely on a monotonic sched_clock() behaviour.
To fix this implement a tod_to_ns() helper function which converts TOD
values without overflow and call this function from both places that
open coded the conversion: sched_clock() and kvm_s390_handle_wait().

is correct in itself. Unfortunately, there is a BUG ON that (correctly)
checks for preemptible to cover the call to rcu later on.

This check might trigger depending on the kernel config.
Lets make sure that no preemption happens during kvm_guest_enter.
We can enable preemption again after the call to

Add a new capability, KVM_CAP_S390_CSS_SUPPORT, which will pass
intercepts for channel I/O instructions to userspace. Only I/O
instructions interacting with I/O interrupts need to be handled
in-kernel:
- TEST PENDING INTERRUPTION (tpi) dequeues and stores pending
interrupts entirely in-kernel.
- TEST SUBCHANNEL (tsch) dequeues pending interrupts in-kernel
and exits via KVM_EXIT_S390_TSCH to userspace for subchannel-
related processing.

Explicitely catch all channel I/O related instructions intercepts
in the kernel and set condition code 3 for them.
This paves the way for properly handling these instructions later
on.
Note: This is not architecture compliant (the previous code wasn't
either) since setting cc 3 is not the correct thing to do for some
of these instructions. For Linux guests, however, it still has the
intended effect of stopping css probing.

Add support for injecting machine checks (only repressible
conditions for now).
This is a bit more involved than I/O interrupts, for these reasons:
- Machine checks come in both floating and cpu varieties.
- We don't have a bit for machine checks enabling, but have to use
a roundabout approach with trapping PSW changing instructions and
watching for opened machine checks.

Add support for handling I/O interrupts (standard, subchannel-related
ones and rudimentary adapter interrupts).
The subchannel-identifying parameters are encoded into the interrupt
type.
I/O interrupts are floating, so they can't be injected on a specific
vcpu.

Switching to or from guest context is done on ioctl context.
So by the time we call kvm_guest_enter() or kvm_guest_exit()
we know we are not running the idle task.
As a result, we can directly account the cputime using
vtime_account_system().
There are two good reasons to do this:
* We avoid some useless checks on guest switch. It optimizes
a bit this fast path.
* In the case of CONFIG_IRQ_TIME_ACCOUNTING, calling vtime_account()
checks for irq time to account. This is pointless since we know
we are not in an irq on guest switch. This is wasting cpu cycles
for no good reason. vtime_account_system() OTOH is a no-op in
this config option.
* We can remove the irq disable/enable around kvm guest switch in s390.
A further optimization may consist in introducing a vtime_account_guest()
that directly calls account_guest_time().

Newer kernels (linux-next with the transparent huge page patches)
use rrbm if the feature is announced via feature bit 66.
RRBM will cause intercepts, so KVM does not handle it right now,
causing an illegal instruction in the guest.
The easy solution is to disable the feature bit for the guest.

Change return code handling of the stsi() function:
In case function code 0 was specified the return value is the
current configuration level (already shifted). That way all
the code that actually copied the stsi_0() function can go
away.
Otherwise the return value is 0 (success) or negative to
indicate an error (currently only -EOPNOTSUPP).
Also stsi() is no longer an inline function. The function is
not performance critical, but every caller would generate an
exception table entry for this function.

Remove the file name from the comment at top of many files. In most
cases the file name was wrong anyway, so it's rather pointless.
Also unify the IBM copyright statement. We did have a lot of sightly
different statements and wanted to change them one after another
whenever a file gets touched. However that never happened. Instead
people start to take the old/"wrong" statements to use as a template
for new files.
So unify all of them in one go.

If sigp sense doesn't have any status bits to report, it should set
cc 0 and leave the register as-is.
Since we know about the external call pending bit, we should report
it if it is set as well.

If an invalid parameter is passed or the addressed cpu is in an
incorrect state sigp set prefix will store a status.
This status must only have bits set as defined by the architecture.
The current kvm implementation missed to clear bits and also did
not set the intended status bit ("and" instead of "or" operation).

Only if the sensed cpu is not running a status is stored, which
is reflected by condition code 1. If the cpu is running, condition
code 0 should be returned.
Just the opposite of what the code is doing.

The initial cpu reset sets the cpu in the stopped state.
Several places check for the cpu state (e.g. sigp set prefix) and
not setting the STOPPED state triggered errors with newer guest
kernels after reboot.

field accessible from userspace. This is needed in order to

Currently qemu/kvm on s390 uses a guest mapping that does not
allow the guest backing page table to be write-protected to
support older systems. On those older systems a host write
protection fault will be delivered to the guest.
Newer systems allow to write-protect the guest backing memory
and let the fault be delivered to the host, thus allowing COW.
Use a capability bit to tell qemu if that is possible.

This patch implements the directed yield hypercall found on other
System z hypervisors. It delegates execution time to the virtual cpu
specified in the instruction's parameter.
Useful to avoid long spinlock waits in the guest.

The kvm_vcpu_kick function performs roughly the same funcitonality on
most all architectures, so we shouldn't have separate copies.
PowerPC keeps a pointer to interchanging waitqueues on the vcpu_arch
structure and to accomodate this special need a
__KVM_HAVE_ARCH_VCPU_GET_WQ define and accompanying function
kvm_arch_vcpu_wq have been defined. For all other architectures this
is a generic inline that just returns &vcpu->wq;

On reboot the guest sends in smp_send_stop() a sigp stop to all CPUs
except for current CPU. Then the guest switches to the IPL cpu by
sending a restart to the IPL CPU, followed by a sigp stop to the
current cpu. Since restart is handled by userspace it's possible that
the restart is delivered before the old stop. This means that the IPL
CPU isn't restarted and we have no running CPUs. So let's make sure
that there is no stop action pending when we do the restart.

In handle_stop() handle the stop bit before doing the store status as
described for "Stop and Store Status" in the Principles of Operation.
We have to give up the local_int.lock before calling kvm store status
since it calls gmap_fault() which might sleep. Since local_int.lock
only protects local_int.* and not guest memory we can give up the lock.

This patch allows the user to fault in pages on a virtual cpus
address space for user controlled virtual machines. Typically this
is superfluous because userspace can just create a mapping and
let the kernel's page fault logic take are of it. There is one
exception: SIE won't start if the lowcore is not present. Normally
the kernel takes care of this [handle_validity() in
arch/s390/kvm/intercept.c] but since the kernel does not handle
intercepts for user controlled virtual machines, userspace needs to
be able to handle this condition.

This patch disables in-kernel handling of SIE intercepts for user
controlled virtual machines. All intercepts are passed to userspace
via KVM_EXIT_SIE exit reason just like SIE intercepts that cannot be
handled in-kernel for regular KVM guests.

via the mapping of the vcpu file descriptor. In order to do so,
a new arch callback named kvm_arch_vcpu_fault is introduced for all

This patch introduces a new exit reason in the kvm_run structure
named KVM_EXIT_S390_UCONTROL. This exit indicates, that a virtual cpu
has regognized a fault on the host page table. The idea is that
userspace can handle this fault by mapping memory at the fault
location into the cpu's address space and then continue to run the
virtual cpu.

This patch introduces two ioctls for virtual cpus, that are only
valid for kernel virtual machines that are controlled by userspace.
Each virtual cpu has its individual address space in this mode of
operation, and each address space is backed by the gmap
implementation just like the address space for regular KVM guests.
KVM_S390_UCAS_MAP allows to map a part of the user's virtual address
space to the vcpu. Starting offset and length in both the user and
the vcpu address space need to be aligned to 1M.
KVM_S390_UCAS_UNMAP can be used to unmap a range of memory from a
virtual cpu in a similar way.

This patch introduces a new config option for user controlled kernel
virtual machines. It introduces a parameter to KVM_CREATE_VM that
allows to set bits that alter the capabilities of the newly created
virtual machine.
The parameter is passed to kvm_arch_init_vm for all architectures.
The only valid modifier bit for now is KVM_VM_S390_UCONTROL.
This requires CAP_SYS_ADMIN privileges and creates a user controlled
virtual machine on s390 architectures.

KVM on s390 always had a sync mmu. Any mapping change in userspace
mapping was always reflected immediately in the guest mapping.

There is a potential host deadlock in the tprot intercept handling.
We must not hold the mmap semaphore while resolving the guest
address. If userspace is remapping, then the memory detection in
the guest is broken anyway so we can safely separate the
address translation from walking the vmas.

CPUSTAT_RUNNING was implemented signifying that a vcpu is not stopped.
This is not, however, what the architecture says: RUNNING should be
set when the host is acting on the behalf of the guest operating
system.
CPUSTAT_RUNNING has been changed to be set in kvm_arch_vcpu_load()
and to be unset in kvm_arch_vcpu_put().
For signifying stopped state of a vcpu, a host-controlled bit has
been used and is set/unset basically on the reverse as the old
CPUSTAT_RUNNING bit (including pushing it down into stop handling
proper in handle_stop()).

KVM common code does vcpu_load prior to calling our arch ioctls and
vcpu_put after we're done here. Via the kvm_arch_vcpu_load/put
callbacks we do load the fpu and access register state into the
processor, which saves us moving the state on every SIE exit the
kernel handles. However this breaks register setting from userspace,
because of the following sequence:
1a. vcpu load stores userspace register content
1b. vcpu load loads guest register content
2. kvm_arch_vcpu_ioctl_set_fpu/sregs updates saved guest register content
3a. vcpu put stores the guest registers and overwrites the new content
3b. vcpu put loads the userspace register set again
This patch loads the new guest register state into the cpu, so that the correct
(new) set of guest registers will be stored in step 3a.

spaces for kvm guest images) changed kvm on s390 to use a separate
address space for kvm guests. We can now put KVM guests anywhere
in the user address mode with a size up to 8PB - as long as the
memory is 1MB-aligned. This change was done without KVM extension
capability bit.
The change was added after 3.0, but we still have a chance to add
a feature bit before 3.1 (keeping the releases in a sane state).
We use number 71 to avoid collisions with other pending kvm patches
as requested by Alexander Graf.

In some cases (preemption, page fault) there is the possibility that
this address space switch is lost.
The typical symptom was a huge amount of validity intercepts or
random guest addressing exceptions.
Fix this by doing the switch in sie_loop and sie_exit and saving the
address space in the gmap structure itself. Also use the preempt
notifier.

This patch removes the mmu reload logic for kvm on s390. Via Martin's
new gmap interface, we can safely add or remove memory slots while
guest CPUs are in-flight. Thus, the mmu reload logic is not needed
anymore.

This patch removes kvm-s390 internal assumption of a linear mapping
of guest address space to user space. Previously, guest memory was
translated to user addresses using a fixed offset (gmsor). The new
code uses gmap_fault to resolve guest addresses.

This patch switches kvm from using (Qemu's) user address space to
Martin's gmap address space. This way QEMU does not have to use a
linker script in order to fit large guests at low addresses in its
address space.

The entry to / exit from sie has subtle dependencies to the first level
interrupt handler. Move the sie assembler code to entry64.S and replace
the SIE_HOOK callback with a test and the new _TIF_SIE bit.
In addition this patch fixes several problems in regard to the check for
the_TIF_EXIT_SIE bits. The old code checked the TIF bits before executing
the interrupt handler and it only modified the instruction address if it
pointed directly to the sie instruction. In both cases it could miss
a TIF bit that normally would cause an exit from the guest and would
reenter the guest context.

When running a kvm guest we can get intercepts for tprot, if the host
page table is read-only or not populated. This patch implements the
most common case (linux memory detection).
This also allows host copy on write for guest memory on newer systems.

virtio has been so far used only in the context of virtualization,
and the virtio Kconfig was sourced directly by the relevant arch
Kconfigs when VIRTUALIZATION was selected.
Now that we start using virtio for inter-processor communications,
we need to source the virtio Kconfig outside of the virtualization
scope too.
Moreover, some architectures might use virtio for both virtualization
and inter-processor communications, so directly sourcing virtio
might yield unexpected results due to conflicting selections.
The simple solution offered by this patch is to always source virtio's
Kconfig in drivers/Kconfig, and remove it from the appropriate arch
Kconfigs. Additionally, a virtio menu entry has been added so virtio
drivers don't show up in the general drivers menu.
This way anyone can use virtio, though it's arguably less accessible
(and neat!) for virtualization users now.
Note: some architectures (mips and sh) seem to have a VIRTUALIZATION
menu merely for sourcing virtio's Kconfig, so that menu is removed too.

commit 9ff4cfb3fc ([S390] kvm-390: Let
kernel exit SIE instruction on work) fixed a problem of commit
commit cd3b70f5d4 ([S390] virtualization
aware cpu measurement) but uncovered another one.
If a kvm guest accesses guest real memory that doesnt exist, the
page fault handler calls the sie hook, which then rewrites
the return psw from sie_inst to either sie_exit or sie_reenter.
On return, the page fault handler will then detect the wrong access
as a kernel fault causing a kernel oops in sie_reenter or sie_exit.
We have to add these two addresses to the exception table to allow
graceful exits.

This patch fixes the sie exit on interrupts. The low level
interrupt handler returns to the PSW address in pt_regs and not
to the PSW address in the lowcore.
Without this fix a cpu bound guest might never leave guest state
since the host interrupt handler would blindly return to the
SIE instruction, even on need_resched and friends.

IA64 support forces us to abstract the allocation of the kvm structure.
But instead of mixing this up with arch-specific initialization and
doing the same on destruction, split both steps. This allows to move
generic destruction calls into generic code.
It also fixes error clean-up on failures of kvm_create_vm for IA64.

Newer (guest) kernels use sigp sense running in their spinlock
implementation to check if the other cpu is running before yielding
the processor. This revealed some wrong guest settings, causing
unnecessary exits for every sigp sense running.

This config option enables or disables three single instructions
which aren't expensive. This is too fine grained.
Besided that everybody who uses kvm would enable it anyway in order
to debug performance problems.

Use the SPP instruction to set a tag on entry to / exit of the virtual
machine context. This allows the cpu measurement facility to distinguish
the samples from the host and the different guests.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Use asm offsets to make sure the offset defines to struct _lowcore and
its layout don't get out of sync.
Also add a BUILD_BUG_ON() which checks that the size of the structure
is sane.
And while being at it change those sites which use odd casts to access
the current lowcore. These should use S390_lowcore instead.

This is mostly useless since no distro ships 31 bit kernels together
with a 64 bit dumper.

kvm_handle_sie_intercept uses a jump table to get the intercept handler
for a SIE intercept. Static code analysis revealed a potential problem:
the intercept_funcs jump table was defined to contain (0x48 >> 2) entries,
but we only checked for code > 0x48 which would cause an off-by-one
array overflow if code == 0x48.
Use the compiler and ARRAY_SIZE to automatically set the limits.

What it is: vhost net is a character device that can be used to reduce
the number of system calls involved in virtio networking.
Existing virtio net code is used in the guest without modification.
There's similarity with vringfd, with some differences and reduced scope
- uses eventfd for signalling
- structures can be moved around in memory at any time (good for
migration, bug work-arounds in userspace)
- write logging is supported (good for migration)
- support memory table and not just an offset (needed for kvm)
common virtio related code has been put in a separate file vhost.c and
can be made into a separate module if/when more backends appear. I used
Rusty's lguest.c as the source for developing this part : this supplied
me with witty comments I wouldn't be able to write myself.
What it is not: vhost net is not a bus, and not a generic new system
call. No assumptions are made on how guest performs hypercalls.
Userspace hypervisors are supported as well as kvm.
How it works: Basically, we connect virtio frontend (configured by
userspace) to a backend. The backend could be a network device, or a tap
device. Backend is also configured by userspace, including vlan/mac
etc.
Status: This works for me, and I haven't see any crashes.
Compared to userspace, people reported improved latency (as I save up to
4 system calls per packet), as well as better bandwidth and CPU
utilization.
Features that I plan to look at in the future:
- mergeable buffers
- zero copy
- scalability tuning: figure out the best threading model to use
Note on RCU usage (this is also documented in vhost.h, near
private_pointer which is the value protected by this variant of RCU):
what is happening is that the rcu_dereference() is being used in a
workqueue item. The role of rcu_read_lock() is taken on by the start of
execution of the workqueue item, of rcu_read_unlock() by the end of
execution of the workqueue item, and of synchronize_rcu() by
flush_workqueue()/flush_work(). In the future we might need to apply
some gcc attribute or sparse annotation to the function passed to
INIT_WORK(). Paul's ack below is for this RCU usage.

Introduce user_mode to replace the two variables switch_amode and
s390_noexec. There are three valid combinations of the old values:
1) switch_amode == 0 && s390_noexec == 0
2) switch_amode == 1 && s390_noexec == 0
3) switch_amode == 1 && s390_noexec == 1
They get replaced by
1) user_mode == HOME_SPACE_MODE
2) user_mode == PRIMARY_SPACE_MODE
3) user_mode == SECONDARY_SPACE_MODE
The new kernel parameter user_mode=[primary,secondary,home] lets
you choose the address space mode the user space processes should
use. In addition the CONFIG_S390_SWITCH_AMODE config option
is removed.

This patch corrects the checking of the new address for the prefix register.
On s390, the prefix register is used to address the cpu's lowcore (address
0...8k). This check is supposed to verify that the memory is readable and
present.
copy_from_guest is a helper function, that can be used to read from guest
memory. It applies prefixing, adds the start address of the guest memory in
user, and then calls copy_from_user. Previous code was obviously broken for
two reasons:
- prefixing should not be applied here. The current prefix register is
going to be updated soon, and the address we're looking for will be
0..8k after we've updated the register
- we're adding the guest origin (gmsor) twice: once in subject code
and once in copy_from_guest
With kuli, we did not hit this problem because (a) we were lucky with
previous prefix register content, and (b) our guest memory was mmaped
very low into user address space.

This patch moves s390 processor status word into the base kvm_run
struct and keeps it up-to date on all userspace exits.
The userspace ABI is broken by this, however there are no applications
in the wild using this. A capability check is provided so users can
verify the updated API exists.

X86 CPUs need to have some magic happening to enable the virtualization
extensions on them. This magic can result in unpleasant results for
users, like blocking other VMMs from working (vmx) or using invalid TLB
entries (svm).
Currently KVM activates virtualization when the respective kernel module
is loaded. This blocks us from autoloading KVM modules without breaking
other VMMs.
To circumvent this problem at least a bit, this patch introduces on
demand activation of virtualization. This means, that instead
virtualization is enabled on creation of the first virtual machine
and disabled on destruction of the last one.
So using this, KVM can be easily autoloaded, while keeping other
hypervisors usable.

This patch relocates the variables kvm-s390 uses to track guest mem addr/size.
As discussed dropping the variables at struct kvm_arch level allows to use the
common vcpu->request based mechanism to reload guest memory if e.g. changes
via set_memory_region.
The kick mechanism introduced in this series is used to ensure running vcpus
leave guest state to catch the update.

If signal pending is true we exit without updating kvm_run, userspace
currently just does nothing and jumps to kvm_run again.
Since we did not set an exit_reason we might end up with a random one
(whatever was the last exit). Therefore it was possible to e.g. jump to
the psw position the last real interruption set.
Setting the INTR exit reason ensures that no old psw data is swapped
in on reentry.

To ensure vcpu's come out of guest context in certain cases this patch adds a
s390 specific way to kick them out of guest context. Currently it kicks them
out to rerun the vcpu_run path in the s390 code, but the mechanism itself is
expandable and with a new flag we could also add e.g. kicks to userspace etc.

There are two waitqueues in kvm for wait handling:
vcpu->wq for virt/kvm/kvm_main.c and
vpcu->arch.local_int.wq for the s390 specific wait code.
the wait handling in kvm_s390_handle_wait was broken by using different
wait_queues for add_wait queue and remove_wait_queue.
There are two options to fix the problem:
o move all the s390 specific code to vcpu->wq and remove
vcpu->arch.local_int.wq
o move all the s390 specific code to vcpu->arch.local_int.wq
This patch chooses the 2nd variant for two reasons:
o s390 does not use kvm_vcpu_block but implements its own enabled wait
handling.
Having a separate wait_queue make it clear, that our wait mechanism is
different
o the patch is much smaller

2.6.31-rc introduced an architecture level set checker based on facility
bits. e.g. if the kernel is compiled to run only on z9, several facility
bits are checked very early and the kernel refuses to boot if a z9 specific
facility is missing.
Until now kvm on s390 did not implement the store facility extended (STFLE)
instruction. A 2.6.31-rc kernel that was compiled for z9 or higher did not
boot in kvm. This patch implements stfle.
This patch should go in before 2.6.31.

This check verifies that the guest we're trying to run in KVM_RUN
has some memory assigned to it. It enters an endless exception
loop if this is not the case.

This patch adds a sanity check for the content of the guest
prefix register content before faulting in the cpu lowcore
that it refers to. The guest might end up in an endless loop
where SIE complains about missing lowcore with incorrect
content of the prefix register without this fix.

This patch makes sure we do unlink a vcpu's sie control block
from the system control area in kvm_arch_vcpu_destroy. This
prevents illegal accesses to the sie control block from other
virtual cpus after free.

This patch reworks the s390 clock comparator wakeup to hrtimer. The clock
comparator is a per-cpu value that is compared against the TOD clock. If
ckc <= TOD an external interrupt 1004 is triggered. Since the clock comparator
and the TOD clock have a much higher resolution than jiffies we should use
hrtimers to trigger the wakeup. This speeds up guest nanosleep for small
values.
Since hrtimers callbacks run in hard-irq context, I added a tasklet to do
the actual work with enabled interrupts.

This patch fixes an incorrectness in the kvm backend for s390.
In case virtual cpus are being created before the corresponding
memory slot is being registered, we need to update the sie
control blocks for the virtual cpus.

In consideration of the s390 memslot constraints locking was changed
to trylock. These locks should never be held, as vcpu's can't run without
the single memslot we just assign when running this code. To ensure this
never deadlocks in case other code changes the code uses trylocks and bail
out if it can't get all locks.
Additionally most of the discussed special conditions for s390 like
only one memslot and no user_alloc are now checked for validity in
kvm_arch_set_memory_region.

Currently we use the cpuid (via STIDP instruction) to recognize LPAR,
z/VM and KVM.
The architecture states, that bit 0-7 of STIDP returns all zero, and
if STIDP is executed in a virtual machine, the VM operating system
will replace bits 0-7 with FF.
KVM should not use FE to distinguish z/VM from KVM for interested
guests. The proper way to detect the hypervisor is the STSI (Store
System Information) instruction, which return information about the
hypervisors via function code 3, selector1=2, selector2=2.
This patch changes the detection routine of Linux to use STSI instead
of STIDP. This detection is earlier than bootmem, we have to use a
static buffer. Since STSI expects a 4kb block (4kb aligned) this
patch also changes the init.data alignment for s390. As this section
will be freed during boot, this should be no problem.
Patch is tested with LPAR, z/VM, KVM on LPAR, and KVM under z/VM.

This patch fixes the SET PREFIX interrupt if triggered by userspace.
Until now, it was not necessary, but life migration will need it. In
addition, it helped me creating SMP support for my kvm_crashme tool
(lets kvm execute random guest memory content).

The kernel handles some priviledged instruction exits. While I was
unable to trigger such an exit from guest userspace, the code should
check for supervisor state before emulating a priviledged instruction.
I also renamed kvm_s390_handle_priv to kvm_s390_handle_b2. After all
there are non priviledged b2 instructions like stck (store clock).

KVM on s390 does not support the ESA/390 architecture. We refuse to
change the architecture mode and print a warning. This patch removes
the printk for several reasons:
o A malicious guest can flood host dmesg
o The old message had no newline
o there is no connection between the message and the failing guest
This patch simply removes the printk. We already set the condition
code to 3 - the guest knows that something went wrong.

This rips out the support for KVM_DEBUG_GUEST and introduces a new IOCTL
instead: KVM_SET_GUEST_DEBUG. The IOCTL payload consists of a generic
part, controlling the "main switch" and the single-step feature. The
arch specific part adds an x86 interface for intercepting both types of
debug exceptions separately and re-injecting them when the host was not
interested. Moveover, the foundation for guest debugging via debug
registers is layed.
To signal breakpoint events properly back to userland, an arch-specific
data block is now returned along KVM_EXIT_DEBUG. For x86, the arch block
contains the PC, the debug exception, and relevant debug registers to
tell debug events properly apart.
The availability of this new interface is signaled by
KVM_CAP_SET_GUEST_DEBUG. Empty stubs for not yet supported archs are
provided.
Note that both SVM and VTX are supported, but only the latter was tested
yet. Based on the experience with all those VTX corner case, I would be
fairly surprised if SVM will work out of the box.

kvm_arch_sync_events is introduced to quiet down all other events may happen
contemporary with VM destroy process, like IRQ handler and work struct for
assigned device.
For kvm_arch_sync_events is called at the very beginning of kvm_destroy_vm(), so
the state of KVM here is legal and can provide a environment to quiet down other
events.

The s390 backend of kvm never calls kvm_vcpu_uninit. This causes
a memory leak of vcpu->run pages.
Lets call kvm_vcpu_uninit in kvm_arch_vcpu_destroy to free
the vcpu->run.

Currently it is impossible to unload the kvm module on s390.
This patch fixes kvm_arch_destroy_vm to release all cpus.
This make it possible to unload the module.
In addition we stop messing with the module refcount in arch code.

We can get an exit for instructions starting with 0xae, even if the guest is
in userspace. Lets make sure, that the signal processor handler is only called
in guest supervisor mode. Otherwise, send a program check.

The z10 system supports large pages, kvm-s390 doesnt.
Make sure that we dont advertise large pages to avoid the guest crashing as
soon as the guest kernel activates DAT.

The lctl(g) instructions require a specific alignment for the parameters.
The architecture requires a specification program check if these alignments
are not used. Enforcing this alignment also removes a possible host BUG,
since the get_guest functions check for proper alignment and emits a BUG.

The current interrupt handling on s390 misbehaves on an error case. On s390
each cpu has the prefix area (lowcore) for interrupt delivery. This memory
must always be available. If we fail to access the prefix area for a guest
on interrupt delivery the configuration is completely unusable. There is no
point in sending another program interrupt to an inaccessible lowcore.
Furthermore, we should not bug the host kernel, because this can be triggered
by userspace. I think the guest kernel itself can not trigger the problem, as
SET PREFIX and SIGNAL PROCESSOR SET PREFIX both check that the memory is
available and sane. As this is a userspace bug (e.g. setting the wrong guest
offset, unmapping guest memory) we should kill the userspace process instead
of BUGing the host kernel.
In the long term we probably should notify the userspace process about this
problem.

While doing some tests with our lcrash implementation I have seen a
naming conflict with prefix_info in kvm_host.h vs. addrconf.h
To avoid future conflicts lets rename private definitions in
asm/kvm_host.h by adding the kvm_s390 prefix.

Some machines do not accept 16EB as guest storage limit. Lets change the
default for the guest storage limit to a sane value. We also should set
the guest_origin to what userspace thinks it is. This allows guests
starting at an address != 0.

The call to add_timer was issued before local_int.lock was taken and before
timer_due was set to 0. If the timer expires before the lock is being taken,
the timer function will set timer_due to 1 and exit before the vcpu falls
asleep. Depending on other external events, the vcpu might sleep forever.
This fix pulls setting timer_due to the beginning of the function before
add_timer, which ensures correct behavior.

If the guest accesses non-existing memory, the sie64a function returns
-EFAULT. We must check the return value and send a program check to the
guest if the sie instruction faulted, otherwise the guest will loop at
the faulting code.
