In the Linux kernel traditionally pfns are represented by an unsigned long.
However a few bits of the SGI IP27 platform code that were ported from
IRIX are using pfn_t for historic reasons. This is conflicting with
KVM's use of pfn_t.

The include is unecessary and will when building the IP35 result in
recursive header inclusion spaghetti.

Some of the were relying into smp.h being dragged in by another header
which of course is fragile. <asm/cpu-info.h> uses smp_processor_id()
only in macros and including smp.h there leads to an include loop, so
don't change cpu-info.h.

There was already a define for NMI_OFFSET in asm/sn/addr.h, which now
clashes with linux/hardirq.h. Rename the one in sn/addr.h to fix IP27
builds..
