5154e9f126 usb: gadget: r8a66597-udc: Make BUSWAIT configurable through platform data
BUSWAIT is a 4-bit-wide value that controls the number of access waits
from the CPU to on-chip USB module. b'0000 inserts 0 wait (2 access
cycles) and b'1111 inserts 15 waits (17 access cycles, hardware
initial value), respectively.

BUSWAIT value depends on peripheral clock frequency supplied to on-chip
of each CPU, hence should be configurable through platform data.

Note that this patch assumes that b'0000 (0 wait, 2 access cycles) is
rerely used and considered as invalid. If valid 'buswait' data is not
provided by platform, initial b'1111 (15 waits, 17 access cycles) will
be applied as a safe default.

352c2dc8b0 usb: gadget: udc-core: add "new-style" registration interface
udc_start() should only trigger the internal state machine and make
minimal house keeping. Before that call udc-core calls the bind()
callback and after the callback the pullup().

udc_stop() is simillar, udc-core calls pullup(), unbind() and finally
udc_stop().

bdb64d7272 usb: gadget: add SuperSpeed support to the Gadget Framework
SuperSpeed USB has defined a new descriptor, called
the Binary Device Object Store (BOS) Descriptor. It
has also changed a bit the definition of SET_FEATURE
and GET_STATUS requests to add USB3-specific details.

This patch implements both changes to the Composite
Gadget Framework.

a59d6b91cb usb: gadget: add streams support to the gadget framework
This patch defines necessary fields to support
streaming for USB3.0.

It implements a new function, called
usb_ep_autoconfig_ss(), to be used instead of the
existing usb_ep_autoconfig() when working in
SuperSpeed mode and there is a need to search for
an endpoint according to the number of required
streams.

72c973dd2b usb: gadget: add usb_endpoint_descriptor to struct usb_ep
Change usb_ep_enable() prototype to use endpoint
descriptor from usb_ep.

This optimization spares the FDs from saving the
endpoint chosen descriptor. This optimization is
not full though. To fully exploit this change, one
needs to update all the UDCs as well since in the
current implementation each of them saves the
endpoint descriptor in it's internal (and extended)
endpoint structure.

2ccea03a8f usb: gadget: introduce UDC Class
this class will be used to abstract away several of the duplicated
operations scattered among the USB gadget controller drivers.

Later, we can add an atomic notifier to tell interested drivers about
what's happening with the controller. Notifications such as suspend,
resume, enumerated, etc. will be useful, at a minimum, for implementing
usb charger detection.

As part of the converting process usb_gadget_probe_driver() is no longer
part of each udc but pushed into the ->stap() callback. The same for his
couterpart.

The core is currently set explicit to 'n'. It will be changed to 'y' once
all users are converted since it provides functions which clash with
other drivers.

e73a9891b3 usb: renesas_usbhs: add DMAEngine support
USB DMA was installed on "normal DMAC" when SH7724 or older SuperH,
but the "USB-DMAC" was prepared on recent SuperH.
These 2 DMAC have a little bit different behavior.

This patch add DMAEngine code for "normal DMAC",
but it is still using PIO fifo.
The DMA fifo will be formally supported in the future.

You can enable DMA fifo by local fixup
usbhs_fifo_pio_push_handler -> usbhs_fifo_dma_push_handler
usbhs_fifo_pio_pop_handler  -> usbhs_fifo_dma_pop_handler
on usbhsg_ep_enable.

This DMAEngine was tested by g_file_storage on SH7724 Ecovec board

1b9ba00017 usb: gadget: composite: Allow function drivers to pause control transfers
Some USB function drivers (e.g. f_mass_storage.c) need to delay or defer the
data/status stages of standard control requests like SET_CONFIGURATION or
SET_INTERFACE till they are done with their bookkeeping and are actually ready
for accepting new commands to their interface.

They can now achieve this functionality by returning USB_GADGET_DELAYED_STATUS
in their setup handlers (e.g. set_alt()). The composite framework will then
defer completion of the control transfer by not completing the data/status stages.

This ensures that the host does not send new packets to the interface till the
function driver is ready to take them.

When the function driver that requested for USB_GADGET_DELAYED_STATUS is done
with its bookkeeping, it should signal the composite framework to continue with
the data/status stages of the control transfer. It can do so by invoking
the new API usb_composite_setup_continue(). This is where the control transfer's
data/status stages are completed and host can initiate new transfers.

The DELAYED_STATUS mechanism is currently only supported if the expected data phase
is 0 bytes (i.e. w_length == 0). Since SET_CONFIGURATION and SET_INTERFACE are the
only cases that will use this mechanism, this is not a limitation.

d860852e08 USB: OTG: msm: Implement charger detection
Implement good battery algorithm defined in the battery charging V1.2 spec
for detecting different charging ports.  USB hardware is put into low power
mode when connected to a dedicated charging port.  vbus_draw and set_power
methods are implemented for determining the allowed current from Host in
different states (un-configured/suspend/configured).

The charger block is implemented using vendor specific registers and the
PHY used in MSM8960(28nm PHY) different from older targets like MSM8x60
and MSM7x30(45nm PHY).  The PHY vendor and product id registers are not
implemented in the above chipsets.  Hence PHY type is passed via platform
data.

3df0045325 usb: fix building musb drivers
Commit 3dacdf11 "usb: factor out state_string() on otg drivers"
broke building musb drivers since there is already another
otg_state_string() function in musb drivers, but with different
prototype. Fix musb drivers to use common otg_state_string(), too.

Also provide a nop for otg_state_string() if CONFIG_USB_OTG_UTILS
is not defined.

c430131a02 USB: EHCI: Support controllers with big endian capability regs
The two first HC capability registers (CAPLENGTH and HCIVERSION)
are defined as one 8-bit and one 16-bit register. Most HC
implementations have selected to treat these registers as part
of a 32-bit register, giving the same layout for both big and
small endian systems.

This patch adds a new quirk, big_endian_capbase, to support
controllers with big endian register interfaces that treat
HCIVERSION and CAPLENGTH as individual registers.

As pointed out by Arnd Bergmann, in include/linux/usb/ehci_def.h, struct
ehci_caps is defined with __attribute__((packed)) for no good reason,
and this triggers undefined behaviour when using ARM's readl() on
pointers to elements of this structure:

38a2f37258 usbnet: Fix up 'FLAG_POINTTOPOINT' and 'FLAG_MULTI_PACKET' overlaps.
USB tethering does not work anymore since 2.6.39-rc2, but it's okay in
-rc1. The root cause is the new added mask code 'FLAG_POINTTOPOINT'
overlaps 'FLAG_MULTI_PACKET'  in  include/linux/usb/usbnet.h, this
causes logic issue in  rx_process(). This patch cleans up the overlap.

Fix 41 occurrences of this type of Section mismatch warning
in g_mass_storage, g_serial, g_cdc, g_multi, g_nokia, g_ether, g_ffs:
(the 75 number reported earlier contained some duplicates.)

WARNING: drivers/usb/gadget/g_mass_storage.o(.text+0x687a): Section mismatch in reference from the function fsg_bind() to the function .devinit.text:usb_ep_autoconfig()
The function fsg_bind() references
the function __devinit usb_ep_autoconfig().
This is often because fsg_bind lacks a __devinit
annotation or the annotation of usb_ep_autoconfig is wrong.

Also remove __devinit from usb_ep_autoconfig_reset() to prevent
possible section mismatch problems with it.

4d42d417be rndis_host: Poll status before control channel where necessary
Some RNDIS devices don't respond on the control channel until polled
on the status channel.  In particular, this was reported to be the
case for the 2Wire HomePortal 1000SW and for some Windows Mobile
devices.

c261344d3c usbnet: use eth%d name for known ethernet devices
The documentation for the USB ethernet devices suggests that
only some devices are supposed to use usb0 as the network interface
name instead of eth0. The logic used there, and documented in
Kconfig for CDC is that eth0 will be used when the mac address
is a globally assigned one, but usb0 is used for the locally
managed range that is typically used on point-to-point links.

Unfortunately, this has caused a lot of pain on the smsc95xx
device that is used on the popular pandaboard without an
EEPROM to store the MAC address, which causes the driver to
call random_ether_address().

Obviously, there should be a proper MAC addressed assigned to
the device, and discussions are ongoing about how to solve
this, but this patch at least makes sure that the default
interface naming gets a little saner and matches what the
user can expect based on the documentation, including for
new devices.

The approach taken here is to flag whether a device might be a
point-to-point link with the new FLAG_POINTTOPOINT setting in
the usbnet driver_info. A driver can set both FLAG_POINTTOPOINT
and FLAG_ETHER if it is not sure (e.g. cdc_ether), or just one
of the two.  The usbnet framework only looks at the MAC address
for device naming if both flags are set, otherwise it trusts the
flag.

7a635ea989 net/usb: Ethernet quirks for the LG-VL600 4G modem
This adds a driver for the CDC Ethernet part of this modem.  The
device's ID is blacklisted in cdc_ether.c and is white-listed in
this new driver because of the quirks needed to make it useful.
The modem's firmware exposes a CDC ACM port for modem control and a
CDC Ethernet port for network data.  The descriptors look fine but
both ports actually are some sort of multiplexers requiring non-
standard headers added/removed from every packet or they get
ignored.  All information is based on a usb traffic log from a
Windows machine.

On the Verizon 4G network I've seen speeds up to 1.1MB/s so far with
this driver, a speed-o-meter site reports 16.2Mbps/10.5Mbps.
Userspace scripts are required to talk to the CDC ACM port.

500132a0f2 USB: Add support for SuperSpeed isoc endpoints
Use the Mult and bMaxBurst values from the endpoint companion
descriptor to calculate the max length of an isoc transfer.

Add USB_SS_MULT macro to access Mult field of bmAttributes, at
Sarah's suggestion.

This patch should be queued for the 2.6.36 and 2.6.37 stable trees, since
those were the first kernels to have isochronous support for SuperSpeed
devices.

131dec344d USB: Remove bogus USB_PORT_STAT_SUPER_SPEED symbol.
USB_PORT_STAT_SUPER_SPEED is a made up symbol that the USB core used to
track whether USB ports had a SuperSpeed device attached.  This is a
linux-internal symbol that was used when SuperSpeed and non-SuperSpeed
devices would show up under the same xHCI roothub.  This particular
port status is never returned by external USB 3.0 hubs.  (Instead they
have a USB_PORT_STAT_SPEED_5GBPS that uses a completely different speed
mask.)

Now that the xHCI driver registers two roothubs, USB 3.0 devices will only
show up under USB 3.0 hubs.  Rip out USB_PORT_STAT_SUPER_SPEED and replace
it with calls to hub_is_superspeed().

c563543784 usb: Make core allocate resources per PCI-device.
Introduce the notion of a PCI device that may be associated with more than
one USB host controller driver (struct usb_hcd).  This patch is the start
of the work to separate the xHCI host controller into two roothubs: a USB
3.0 roothub with SuperSpeed-only ports, and a USB 2.0 roothub with
HS/FS/LS ports.

One usb_hcd structure is designated to be the "primary HCD", and a pointer
is added to the usb_hcd structure to keep track of that.  A new function
call, usb_hcd_is_primary_hcd() is added to check whether the USB hcd is
marked as the primary HCD (or if it is not part of a roothub pair).  To
allow the USB core and xHCI driver to access either roothub in a pair, a
"shared_hcd" pointer is added to the usb_hcd structure.

Add a new function, usb_create_shared_hcd(), that does roothub allocation
for paired roothubs.  It will act just like usb_create_hcd() did if the
primary_hcd pointer argument is NULL.  If it is passed a non-NULL
primary_hcd pointer, it sets usb_hcd->shared_hcd and usb_hcd->primary_hcd
fields.  It will also skip the bandwidth_mutex allocation, and set the
secondary hcd's bandwidth_mutex pointer to the primary HCD's mutex.

IRQs are only allocated once for the primary roothub.

Introduce a new usb_hcd driver flag that indicates the host controller
driver wants to create two roothubs.  If the HCD_SHARED flag is set, then
the USB core PCI probe methods will allocate a second roothub, and make
sure that second roothub gets freed during rmmod and in initialization
error paths.

When usb_hc_died() is called with the primary HCD, make sure that any
roothubs that share that host controller are also marked as being dead.

The xHCI driver essentially has both a USB 2.0 and a USB 3.0 roothub.  So
setting the HCD_USB3 bits in the hcd->driver->flags is a bit misleading.
Add a new field to usb_hcd, bcdUSB.  Store the result of
hcd->driver->flags & HCD_MASK in it.  Later, when we have the xHCI driver
register the two roothubs, we'll set the usb_hcd->bcdUSB field to HCD_USB2
for the USB 2.0 roothub, and HCD_USB3 for the USB 3.0 roothub.

Update the USB core to deal with USB 3.0 hubs.  These hubs have a slightly
different hub descriptor than USB 2.0 hubs, with a fixed (rather than
variable length) size.  Change the USB core's hub descriptor to have a
union for the last fields that differ.  Change the host controller drivers
that access those last fields (DeviceRemovable and PortPowerCtrlMask) to
use the union.

Translate the new version of the hub port status field into the old
version that khubd understands.  (Note: we need to fix it to translate the
roothub's port status once we stop converting it to USB 2.0 hub status
internally.)

Add new code to handle link state change status.  Send out new control
messages that are needed for USB 3.0 hubs, like Set Hub Depth.

This patch is a modified version of the original patch submitted by John
Youn.  It's updated to reflect the removal of the "bitmap" #define, and
change the hub descriptor accesses of a couple new host controller
drivers.

da13051cc7 USB: Remove bitmap #define from hcd.h
Using a #define to redefine a common variable name is a bad thing,
especially when the #define is in a header.  include/linux/usb/hcd.h
redefined bitmap to DeviceRemovable to avoid typing a long field in the
hub descriptor.  This has unintended side effects for files like
drivers/usb/core/devio.c that include that file, since another header
included after hcd.h has different variables named bitmap.

Remove the bitmap #define and replace instances of it in the host
controller code.  Cleanup the spaces around function calls and square
brackets while we're at it.

79ad3b5add usb: host: Add EHCI driver for NVIDIA Tegra SoCs
The Tegra 2 SoC has 3 EHCI compatible USB controllers. This patch adds
the necessary glue to allow the ehci-hcd driver to work on Tegra 2
SoCs.

The platform data is used to configure board-specific phy settings and
to configure the operating mode, as one of the ports may be used as a otg
port. For additional power saving, the driver supports powering down the
phy on bus suspend when it is used, for example, to connect an internal
device that use an out-of-band remote wakeup mechanism (e.g. a gpio).

9b37596a2e USB: move usbcore away from hcd->state
The hcd->state variable is a disaster.  It's not clearly owned by
either usbcore or the host controller drivers, and they both change it
from time to time, potentially stepping on each other's toes.  It's
not protected by any locks.  And there's no mechanism to prevent it
from going through an invalid transition.

This patch (as1451) takes a first step toward fixing these problems.
As it turns out, usbcore uses hcd->state for essentially only two
things: checking whether the controller's root hub is running and
checking whether the controller has died.  Therefore the patch adds
two new atomic bitflags to the hcd structure, to store these pieces of
information.  The new flags are used only by usbcore, and a private
spinlock prevents invalid combinations (a dead controller's root hub
cannot be running).

The patch does not change the places where usbcore sets hcd->state,
since HCDs may depend on them.  Furthermore, there is one place in
usb_hcd_irq() where usbcore still must use hcd->state: An HCD's
interrupt handler can implicitly indicate that the controller died by
setting hcd->state to HC_STATE_HALT.  Nevertheless, the new code is a
big improvement over the current code.

The patch makes one other change.  The hcd_bus_suspend() and
hcd_bus_resume() routines now check first whether the host controller
has died; if it has then they return immediately without calling the
HCD's bus_suspend or bus_resume methods.

This fixes the major problem reported in Bugzilla #29902: The system
fails to suspend after a host controller dies during system resume.

969e3033ae USB: serial drivers need to use larger bulk-in buffers
When a driver doesn't know how much data a device is going to send,
the buffer size should be at least as big as the endpoint's maxpacket
value.  The serial drivers don't follow this rule; many of them
request only 256-byte bulk-in buffers.  As a result, they suffer
overflow errors if a high-speed device wants to send a lot of data,
because high-speed bulk endpoints are required to have a maxpacket
size of 512.

This patch (as1450) fixes the problem by using the driver's
bulk_in_size value as a minimum, always allocating buffers no smaller
than the endpoint's maxpacket size.

We don't actually need this and it causes problems for internal use of
this functionality. Currently there is a single use of the FILE * pointer.
That is the serial core which uses it to check tty_hung_up_p. However if
that is true then IO_ERROR is also already set so the check may be removed.

2694a48d90 USB: HCD: Add driver hooks for (un)?map_urb_for_dma
Provide optional hooks for the host controller driver to override the
default DMA mapping and unmapping routines.  In general, these shouldn't
be necessary unless the host controller has special DMA requirements,
such as alignment contraints.  If these are not specified, the
general usb_hcd_(un)?map_urb_for_dma functions will be used instead.
Also, pass the status to unmap_urb_for_dma so it can know whether the
DMA buffer has been overwritten.

Finally, add a flag to be used by these implementations if they
allocated a temporary buffer so it can be freed properly when unmapping.

d14fc1a74e USB: serial: handle Data Carrier Detect changes
Alan's commit 335f8514f2 introduced
.carrier_raised function in several drivers.  That also means
tty_port_block_til_ready can now suspend the process trying to open the serial
port when Carrier Detect is low and put it into tty_port.open_wait queue.  We
need to wake up the process when Carrier Detect goes high and trigger TTY
hangup when CD goes low.

Some of the devices do not report modem status line changes, or at least we
don't understand the status message, so for those we remove .carrier_raised
again.

0029227f1b xHCI: synchronize irq in xhci_suspend()
Synchronize the interrupts instead of free them in xhci_suspend(). This will
prevent a double free when the host is suspended and then the card removed.

Set the flag hcd->msix_enabled when using MSI-X, and check the flag in
suspend_common(). MSI-X synchronization will be handled by xhci_suspend(),
and MSI/INTx will be synchronized in suspend_common().

This patch should be queued for the 2.6.37 stable tree.

352a337832 USB: otg: fix link breakage, when the NOP USB Xceiver is a module
If the NOP USB OTG transceiver driver is built as a module, the otg.h
header declares external functions, but if they are referenced from the
kernel proper, as, e.g., in the OMAP3 case, where the omap3evm board is
calling the usb_nop_xceiv_register() function, linkage breaks. This patch
fixes this problem.

496dda704b usb: musb: host: unmap the buffer for PIO data transfers
The USB stack maps the buffer for DMA if the controller supports DMA.
MUSB controller can perform DMA as well as PIO transfers.
The buffer needs to be unmapped before CPU can perform
PIO data transfers.

Export unmap_urb_for_dma() so that drivers can perform
the DMA unmapping in a sane way.

ad1a8102f9 USB: gadget: composite: Better string override handling
The iManufatcurer, iProduct and iSerialNumber composite module
parameters were only used when the gadget driver registers
strings for manufacturer, product and serial number.  If the
gadget never bothered to set corresponding fields in USB device
descriptors those module parameters are ignored.

This commit makes the parameters work even if the strings ID
have not been assigned.  It also changes the way IDs are
overridden -- what IDs are overridden is now saved in
usb_composite_dev structure -- which makes it unnecessary to
modify the string tables the way previous code did.

The commit also adds a iProduct and iManufatcurer fields to the
usb_composite_device structure.  If they are set, appropriate
strings are reserved and added to device descriptor.  This makes
it unnecessary for gadget drivers to maintain code for setting
those.  If iProduct is not set it defaults to

f0ae849df1 usb: Add Intel Langwell USB OTG Transceiver Driver
This adds support for the USB transceiver driver in the Langwell chipset used
on the Intel MID platforms. It folds up the original patch set which includes
basic support for the device, PHY low power mode (Please notice that there is
a limitation, after we drive VBus down, 2ms delay is required from SCU FW to
sync up OTGSC register with USBCFG register), software timers (the hardware
timers do not work in low power mode), HNP, SRP.

Dan Rosenberg noted that various drivers return the struct with uncleared
fields. Instead of spending forever trying to stomp all the drivers that

ff2f078743 USB: fix race between root-hub wakeup & controller suspend
This patch (as1395) adds code to hcd_pci_suspend() for handling wakeup
races.  This is another general race pattern, similar to the "open
vs. unregister" race we're all familiar with.  Here, the race is
between suspending a device and receiving a wakeup request from one of
the device's suspended children.

In particular, if a root-hub wakeup is requested at about the same
time as the corresponding USB controller is suspended, and if the
controller is enabled for wakeup, then the controller should either
fail to suspend or else wake right back up again.

During system sleep this won't happen very much, especially since host
controllers generally aren't enabled for wakeup during sleep.  However
it is definitely an issue for runtime PM.  Something like this will be
needed to prevent the controller from autosuspending while waiting for
a root-hub resume to take place.  (That is, in fact, the common case,
for which there is an extra test.)

method used by PCI-based host controller drivers.  ehci-hcd in
particular needs to know whether or not to enable wakeup when

available through device_may_wakeup(), when support is added for
runtime suspend this will no longer be true.

541c7d432f USB: convert usb_hcd bitfields into atomic flags
This patch (as1393) converts several of the single-bit fields in
struct usb_hcd to atomic flags.  This is for safety's sake; not all
CPUs can update bitfield values atomically, and these flags are used
in multiple contexts.

The flag fields that are set only during registration or removal can
remain as they are, since non-atomic accesses at those times will not
cause any problems.

(Strictly speaking, the authorized_default flag should become atomic
as well.  I didn't bother with it because it gets changed only via
sysfs.  It can be done later, if anyone wants.)

usb_string_ids_tab() and usb_string_ids_n() functions added to
the composite framework.  The first accepts an array of
usb_string object and for each registeres a string id and the
second registeres a given number of ids and returns the first.

This may simplify string ids registration since gadgets and
composite functions won't have to call usb_string_id() several
times and each time check for errer status -- all this will be
done with a single call.

6d88e67925 USB: don't stop root-hub status polls too soon
This patch (as1390) fixes a problem that crops up when a UHCI host
controller is unbound from uhci-hcd while there are still some active
URBs.  The URBs have to be unlinked when the root hub is unregistered,
and uhci-hcd relies upon root-hub status polls as part of its
unlinking procedure.  But usb_hcd_poll_rh_status() won't make those
status calls if hcd->rh_registered is clear, and the flag is cleared
_before_ the unregistration takes place.

Since hcd->rh_registered is used for other things and needs to be
cleared early, the solution is to add a new flag (rh_pollable) and use
it instead.  It gets cleared _after_ the root hub is unregistered.

Now that the status polls don't end too soon, we have to make sure
they also don't occur too late -- after the root hub's usb_device
structure or the HCD's private structures are deallocated.  Therefore
the patch adds usb_get_device() and usb_put_device() calls to protect
the root hub structure, and it adds an extra del_timer_sync() to
prevent the root-hub timer from causing an unexpected status poll.

This additional complexity would not be needed if the HCD framework
had provided separate stop() and release() callbacks instead of just
stop().  This lack could be fixed at some future time (although it
would require changes to every host controller driver); when that
happens this patch won't be needed any more.

ddf8abd259 USB: f_fs: the FunctionFS driver
The FunctionFS is a USB composite function that can be used
with the composite framework to create an USB gadget.

>From kernel point of view it is just a composite function with
some unique behaviour.  It may be added to an USB
configuration only after the user space driver has registered
by writing descriptors and strings (the user space program has
to provide the same information that kernel level composite
functions provide when they are added to the configuration).

>From user space point of view it is a file system which when
mounted provide an "ep0" file.  User space driver need to
write descriptors and strings to that file.  It does not need
to worry about endpoints, interfaces or strings numbers but
simply provide descriptors such as if the function was the
only one (endpoints and strings numbers starting from one and
interface numbers starting from core).  The FunctionFS changes
numbers of those as needed also handling situation when
numbers differ in different configurations.

When descriptors and strings are written "ep#" files appear
(one for each declared endpoint) which handle communication on
a single endpoint.  Again, FunctionFS takes care of the real
numbers and changing of the configuration (which means that
"ep1" file may be really mapped to (say) endpoint 3 (and when
configuration changes to (say) endpoint 2)).  "ep0" is used
for receiving events and handling setup requests.

When all files are closed the function disables itself.

Remove multi-urb write from the generic driver and simplify the
prepare_write_buffer prototype:

The default implementation simply fills dest with data from port write
fifo but drivers can override it if they need to process the outgoing
data (e.g. add headers).

Turn ftdi_sio into a generic fifo-based driver, which lowers CPU usage
significantly for small writes while retaining maximum throughput.

27c7acf220 USB: serial: reimplement generic fifo-based writes
Reimplement fifo-based writes in the generic driver using a multiple
pre-allocated urb scheme.

In contrast to multi-urb writes, no allocations (of urbs or buffers) are
made during run-time and there is less pressure on the host stack
queues as currently only two urbs are used (implementation is generic
and can handle more than two urbs as well, though).

Initial tests using ftdi_sio show that the implementation achieves the
same (maximum) throughput at high baudrates as multi-urb writes. The CPU
usage is much lower than for multi-urb writes for small write requests
and only slightly higher for large (e.g. 2k) requests (due to extra copy
via fifo?).

Also outperforms multi-urb writes for small write requests on an
embedded arm-9 system, where multi-urb writes are CPU-bound at high
baudrates (perf reveals that a lot of time is spent in the host stack
enqueue function -- could perhaps be a bug as well).

Keeping the original write_urb, buffer and flag for now as there are
other drivers depending on them.

eab1cafc3b USB: Support for allocating USB 3.0 streams.
Bulk endpoint streams were added in the USB 3.0 specification.  Streams
allow a device driver to overload a bulk endpoint so that multiple
transfers can be queued at once.

The device then decides which transfer it wants to work on first, and can
queue part of a transfer before it switches to a new stream.  All this
switching is invisible to the device driver, which just gets a completion
for the URB.  Drivers that use streams must be able to handle URBs
completing in a different order than they were submitted to the endpoint.

This requires adding new API to set up xHCI data structures to support
multiple queues ("stream rings") per endpoint.  Drivers will allocate a
number of stream IDs before enqueueing URBs to the bulk endpoints of the
device, and free the stream IDs in their disconnect function.  See
Documentation/usb/bulk-streams.txt for details.

The new mass storage device class, USB Attached SCSI Protocol (UASP), uses
these streams API.

This allows for drivers to manipulate (e.g. add headers) to bulk out
data before it is sent.

This adds a new function pointer to usb_serial_driver:

The function is generic and can be used with either kfifo-based or
multi-urb writes:

If *dest is NULL the implementation should allocate dest.
If src is NULL the implementation should use the port write fifo.

If not set, a generic implementation is used which simply uses memcpy or
kfifo_out.

Use dynamic transfer buffer sizes since it is more efficient to let the
host controller do the partitioning to fit endpoint size. This way we
also do not use more than one urb per write request.

Replace max_in_flight_urbs with multi_urb_write flag in struct
usb_serial_driver to enable multi-urb writes.

Use MAX_TX_URBS=40 and a max buffer size of PAGE_SIZE to prevent DoS
attacks.

Allow drivers to define custom bulk in/out buffer sizes in struct
usb_serial_driver. If not set, fall back to the default buffer size
which matches the endpoint size.

Three drivers are currently freeing the pre-allocated buffers and
allocating larger ones to achieve this at port probe (ftdi_sio) or even
at port open (ipaq and iuu_phoenix), which needless to say is suboptimal.

This is needed for devices which try to hide unused interfaces by only
lowering config->bNumInterfaces, and which can't handle if you try to talk
to the "hidden" interfaces.

This patch (as1348) removes the bogus
USB_PORT_FEAT_{HIGHSPEED,SUPERSPEED} symbols from ch11.h.  No such
features are defined by the USB spec.  (There is a PORT_LOWSPEED
feature, but the spec doesn't mention it except to say that host
software should never use it.)  The speed indicators are port
statuses, not port features.

As a temporary workaround for the xhci-hcd driver, a fictional
USB_PORT_STAT_SUPER_SPEED symbol is added.

5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Some of the board might use external Vbus power supply on musb
interface which would require to program ULPI_BUSCONTROL register.

Adding 'extvbus' flag which can be set from such boards which will
be checked at musb driver files before programming ULPI_BUSCONTROL.

e9a20171df USB: otg: add notifier support
The notifier will be used to communicate usb events
to other drivers like the charger chip.

This can be used as source of information to kick
usb charger detection as described by the USB
Battery Charging Specification 1.1 and/or to
pass bMaxPower field of selected usb_configuration
to charger chip in order to use that information
as input current on the charging profile
setup.

Use the definitions from linux/usb/audio.h all over the ALSA USB audio
driver and add some missing definitions there as well.

Use the endpoint attribute macros from linux/usb/ch9 and remove the own
things from sound/usb/usbaudio.h.

Now things are also nicely prefixed which makes understanding the code
easier.

28e1b77308 ALSA: usbaudio: parse USB descriptors with structs
In preparation of support for v2.0 audio class, use the structs from
linux/usb/audio.h and add some new ones to describe the fields that are
actually parsed by the descriptor decoders.

Also, factor out code from usb_create_streams(). This makes it easier to
adopt the new iteration logic needed for v2.0.

60b8675592 usbnet: Convert dev(dbg|err|warn|info) macros to netdev_<level>
These macros are too similar to the dev_<level> equivalents
but take a usbnet * argument.  Convert them to the recently
introduced netdev_<level> macros and remove the old macros.

The old macros had "\n" appended to the format string.
Add the "\n" to the converted uses.

Some existing uses of the dev<foo> macros in cdc_eem.c
probably mistakenly had trailing "\n".  No "\n" added there.

Fix net1080 this/other log message inversion.

This adds support for OTG transceivers directly connected to the ULPI
interface. In particular, the following details are added

- a struct for low level io functions (read/write)
- a priv field to be used as 'viewport' by low level access functions
- an (*init) and (*shutdown) callbacks, along with static inline helpers
- a (*set_vbus) callback to switch the port power on and off
- a flags field for per-transceiver settings
- some defines for the flags bitmask to configure platform specific
  details

5242658d1b USB gadget: Handle endpoint requests at the function level
Control requests targeted at an endpoint (that is sent to EP0 but
specifying the target endpoint address in wIndex) are dispatched to the
current configuration's setup callback, requiring all gadget drivers to
dispatch the requests to the correct function driver.

To avoid this, record which endpoints are used by each function in the
composite driver SET CONFIGURATION handler and dispatch requests
targeted at endpoints to the correct function.
