Enable device-qualifier quirk for Elan Touchscreen, which often fails to
handle requests for the device_descriptor.
Note that the device sometimes do respond properly with a Request Error

When this happens any further descriptor requests also fails, for
example:

[ 1530.945588] usb 2-7: unable to read config index 0 descriptor/start: -71

The driver's handling of DMA buffers for non-aligned transfers
was kind of nuts. For IN transfers, it left the URB DMA buffer
mapped until the transfer completed, then synced it, copied the
data from the bounce buffer, then synced it again.
Instead of that, just call usb_hcd_unmap_urb_for_dma() to unmap
the buffer before starting the transfer. Then no syncing is
required when doing the copy. This should also allow handling of
other types of mappings besides just dma_map_single() ones.
Also reduce the size of the bounce buffer allocation for Isoc
endpoints to 3K, since that's the largest possible transfer size.

Because we have not enough memory to have each TX FIFO of size at least
3072 bytes (the maximum single packet size with 3 transactions per
microframe), we create four FIFOs of lenght 1024, and four of length
3072 bytes, and assing them to endpoints dynamically according to
maxpacket size value of given endpoint.
Up to now there were initialized 16 TX FIFOs, but we use only 8 IN
endpoints, so we can split available memory for 8 FIFOs to have more
memory for each one.
It needed to do some small modifications in few places in code, because
there was assumption that TX FIFO numbers assigned to endpoints are the
same as the endpoint numbers, which is not true since we have dynamic
FIFO assigning.

This patch adds quirks for Entrega Technologies (later Xircom PortGear) USB-
SCSI converters. They use Shuttle Technology EUSB-01/EUSB-S1 chips. The
US_FL_SCM_MULT_TARG quirk is needed to allow multiple devices on the SCSI
chain to be accessed. Without it only the (single) device with SCSI ID 0
can be used.
The standalone converter sold by Entrega had model number U1-SC25. Xircom
acquired Entrega and re-branded the product line PortGear. The PortGear USB
to SCSI Converter (model PGSCSI) is internally identical to the Entrega
product, but later models may use a different USB ID. The Entrega-branded
units have USB ID 1645:0007, as does my Xircom PGSCSI, but the Windows and
Macintosh drivers also support 085A:0028.
Entrega also sold the "Mac USB Dock", which provides two USB ports, a Mac
(8-pin mini-DIN) serial port and a SCSI port. It appears to the computer as
a four-port hub, USB-serial, and USB-SCSI converters. The USB-SCSI part may
have initially used the same ID as the standalone U1-SC25 (1645:0007), but
later production used 085A:0026.
My Xircom PortGear PGSCSI has bcdDevice=0x0100. Units with bcdDevice=0x0133
probably also exist.
This patch adds quirks for 1645:0007, 085A:0026 and 085A:0028. The Windows
driver INF file also mentions 085A:0032 "PortStation SCSI Module", but I
couldn't find any mention of that actually existing in the wild; perhaps it
was cancelled before release?

The Ariston Technologies iConnect 025 and iConnect 050 (also known as e.g.

Microsystems chips. Only the connectors differ; both have the same USB ID.

I don't have one of these, but based on the other entries for Shuttle/
SCM-based converters this patch is very likely correct. I used 0x0000 and

Old code in ehci-hcd tries to expedite disabling endpoints after the
controller has stopped, by destroying the endpoint's associated QH
without first unlinking the QH. This was necessary back when the
driver wasn't so careful about keeping track of the controller's
state.
But now we are careful about it, and the driver knows that when the
controller isn't running, no unlinking delay is needed. Furthermore,
skipping the unlink step will trigger a BUG() in qh_destroy() when the
preceding QH is released, because the link pointer will be non-NULL.
Removing the lines that skip the unlinking step and go directly to
QH_STATE_IDLE fixes the problem.

This patch introduces virtual endpoint address mapping. It separates
function logic form physical endpoint addresses making it more hardware
independent.
Following modifications changes user space API, so to enable them user
have to switch on the FUNCTIONFS_VIRTUAL_ADDR flag in descriptors.
Endpoints are now refered using virtual endpoint addresses chosen by
user in endpoint descpriptors. This applies to each context when endpoint
address can be used:
- when accessing endpoint files in FunctionFS filesystemi (in file name),
- in setup requests directed to specific endpoint (in wIndex field),
- in descriptors returned by FUNCTIONFS_ENDPOINT_DESC ioctl.
In endpoint file names the endpoint address number is formatted as
double-digit hexadecimal value ("ep%02x") which has few advantages -
it is easy to parse, allows to easly recognize endpoint direction basing
on its name (IN endpoint number starts with digit 8, and OUT with 0)
which can be useful for debugging purpose, and it makes easier to introduce
further features allowing to use each endpoint number in both directions
to have more endpoints available for function if hardware supports this
(for example we could have ep01 which is endpoint 1 with OUT direction,
and ep81 which is endpoint 1 with IN direction).
Physical endpoint address can be still obtained using ioctl named
FUNCTIONFS_ENDPOINT_REVMAP, but now it's not neccesary to handle
USB transactions properly.

The SCSI specification requires that the second Command Data Byte
should contain the LUN value in its high-order bits if the recipient
device reports SCSI level 2 or below. Nevertheless, some USB
mass-storage devices use those bits for other purposes in
vendor-specific commands. Currently Linux has no way to send such
commands, because the SCSI stack always overwrites the LUN bits.
Testing shows that Windows 7 and XP do not store the LUN bits in the
CDB when sending commands to a USB device. This doesn't matter if the
device uses the Bulk-Only or UAS transports (which virtually all
modern USB mass-storage devices do), as these have a separate
mechanism for sending the LUN value.
Therefore this patch introduces a flag in the Scsi_Host structure to
inform the SCSI midlayer that a transport does not require the LUN
bits to be stored in the CDB, and it makes usb-storage set this flag
for all devices using the Bulk-Only transport. (UAS is handled by a
separate driver, but it doesn't really matter because no SCSI-2 or
lower device is at all likely to use UAS.)
The patch also cleans up the code responsible for storing the LUN
value by adding a bitflag to the scsi_device structure. The test for
whether to stick the LUN value in the CDB can be made when the device
is probed, and stored for future use rather than being made over and
over in the fast path.

The zte_ev driver is based on code (once) distributed by ZTE that still
appears to originally have been reverse-engineered and bolted onto the
generic driver.
A closer analysis of the zte_ev setup code reveals that it consists of
standard CDC requests (SET/GET_LINE_CODING and SET_CONTROL_LINE_STATE)
but unfortunately fails to get some of those right. In particular, as
reported by Lei Liu, it fails to lower DTR/RTS on close. It also appears
that the control requests lack the interface argument.
Since line control is already handled properly by the option driver, and
the SET/GET_LINE_CODING requests appears to be redundant (amounts to a
SET 9600 8N1) let's remove the redundant zte_ev driver.
Also move the remaining ZTE PIDs to the generic option modem driver.

Some late fixes for dwc3 so we have something more stable
on v3.17-final.
Most bugs have been there for quite a while and nobody
noticed, except for TRB completion when multiple TRBs
are started.
Patches were tested on AM437x SK and J6 EVM and are passing
my tests.

Resuming from hibernate (S4) will restart and re-initialize xHC.
The device contexts are freed and will be re-allocated later during device reset.
Usb core will disable link pm in device resume before device reset, which will
try to change the max exit latency, accessing the device contexts before they are re-allocated.
There is no need to zero (disable) the max exit latency when disabling hw lpm
for a freshly re-initialized xHC. So check that device context exists before
doing anything. The max exit latency will be set again after device reset when usb core
enables the link pm.

The xhci driver will OOPS on resume from S2/S3 if dma_alloc_coherent()
is out of memory. This is a result of two things:
1. xhci_mem_cleanup() in xhci-mem.c free's xhci->lpm_command if
it's not NULL, but doesn't set it to NULL after the free.
2. xhci_mem_cleanup() is called twice on resume, once for normal
restart and once from xhci_mem_init() if dma_alloc_coherent() fails,
resulting in a free of xhci->lpm_command that has already been freed.
The fix is to set xhci->lpm_command to NULL after freeing it.

The Iomega Jaz USB Adapter is a SCSI-USB converter cable. The hardware
seems to be identical to e.g. the Microtech XpressSCSI, using a Shuttle/
SCM chip set. However its firmware restricts it to only work with Jaz

On connecting the cable a message like this appears four times in the log:
reset full speed USB device number 4 using uhci_hcd
That's non-fatal but the US_FL_SINGLE_LUN quirk fixes it.

There are a large numbers of issues with ASM1051 devices in uas mode:
1) They do not support REPORT SUPPORTED OPERATION CODES
2) They use out of spec 8 byte status iu-s when they have no sense data,
switching to normal 16 byte status iu-s when they do have sense data.
3) They hang / crash when combined with some disks, e.g. a Crucial M500 ssd.
4) They hang / crash when stressed (through e.g. sg_reset --bus) with disks
with which then normally do work (once 1 & 2 are worked around).
Where as in BOT mode they appear to work fine, so the best way forward with
these devices is to just blacklist them for uas usage.
Unfortunately this is easier said then done. as older versions of the ASM1053
(which works fine) use the same usb-id as the ASM1051.
When connected over USB-3 the 2 can be told apart by the number of streams
they support. So this patch adds some less then pretty code to disable uas for
the ASM1051. When connected over USB-2, simply disable uas alltogether for
devices with the shared usb-id.

When the driver is removed s3c_hsotg_phy_disable is called three times
instead of once. This results in decreasing of the phy reference counter
below zero and thus consecutive inserts of the module fails.
This patch removes calls to s3c_hsotg_phy_disable from s3c_hsotg_remove
and s3c_hsotg_udc_stop.
s3c_hsotg_udc_stop is called from udc-core.c only after
usb_gadget_disconnect, which in turn calls s3c_hsotg_pullup, which
already calls s3c_hsotg_phy_disable.
s3c_hsotg_remove must be called only after udc_stop, so there is no
point in disabling phy once again there.

Commit 71c731a (usb: host: xhci: Fix Compliance Mode
on SN65LVP3502CP Hardware) implemented a workaround
for a known issue with Texas Instruments' USB 3.0
redriver IC but it left a condition where any xHCI
host would be taken out of reset if port was placed
in compliance mode and there was no device connected
to the port.
That condition would trigger a fake connection to a
non-existent device so that usbcore would trigger a
warm reset of the port, thus taking the link out of
reset.
This has the side-effect of preventing any xHCI host
connected to a Linux machine from starting and running
the USB 3.0 Electrical Compliance Suite because the
port will mysteriously taken out of compliance mode
and, thus, xHCI won't step through the necessary
compliance patterns for link validation.
This patch fixes the issue by just adding a missing
check for XHCI_COMP_MODE_QUIRK inside
xhci_hub_report_usb3_link_state() when PORT_CAS isn't
set.
This patch should be backported to all kernels containing
commit 71c731a.

To handle class requests received on ep0, the driver needs to access the
length and direction of the request after the setup stage. It currently
stores them in a v4l2 event during the setup stage, and then copies them
from the event structure to the driver internal state structure when the
event is dequeued.

USB-serial fixes for v3.17-rc4
These updates add back some PIDs that were lost in a recent revert and add a
couple of new ones. Included is also an update to how the sierra driver binds
its interfaces in order to avoid binding CDC interfaces.

Probe is testing if the current interface provides two bulk endpoints.
While this achieves the goal of only binding to the correct interface,
we already know we can find the device on interface number 1. Stop
checking the endpoints and just return successfully when interface
number 1 is probed.

Currently, we disable pm_runtime before all register
accesses are done, this is dangerous and might lead
to abort exceptions due to the driver trying to access
a register which is clocked by a clock which was long
gated.
Fix that by moving pm_runtime_put_sync() and pm_runtime_disable()
as the last thing we do before returning from our ->remove()

The PHY configuration is stored in an opaque "config" field, but when
allocating the structure, its proper size needs to be known. In the case
of UTMI, the proper structure is tegra_utmip_config of which a local
variable already exists, so we can use that to obtain the size from.

Up to now, when endpoint addresses in descriptors were non-consecutive,
there were created redundant files, which could cause problems in kernel,
when user tried to read/write to them. It was result of fact that maximum
endpoint address was taken as total number of endpoints in function.
This patch adds endpoint descriptors counting and storing their addresses
in eps_addrmap to verify their cohesion in each speed.
Endpoint address map would be also useful for further features, just like
vitual endpoint address mapping.

This patch fixes the condition of is_done in usbhsf_dma_push_done().
This function will be called after a transmission finished by DMAC.
So, the function should check if the transmission packet is short packet

by the pio handler if the "*is_done" is not set. Otherwize, the
transaction will not finish if a gadget driver sets the "zero" flag

The UAC2 function driver currently responds to all packets at all times
with wMaxPacketSize packets. That results in way too fast audio
playback as the function driver (which is in fact supposed to define
the audio stream pace) delivers as fast as it can.
Fix this by sizing each packet correctly with the following steps:
a) Set the packet's size by dividing the nominal data rate by the
playback endpoint's interval.
b) If there is a residual value from the calculation in a), add
it to a accumulator to keep track of it across packets.
c) If the accumulator has gathered at least the number of bytes
that are needed for one sample frame, increase the packet size.
This way, the packet size calculation will get rid of any kind of
imprecision that would otherwise occur with a simple division over
time.
Some of the variables that are needed while processing each packet
are pre-computed for performance reasons.

With packet sizes other than 512, payloads in the packets may wrap
around the ALSA dma buffer partially, which leads to memory corruption
and audible clicks and pops in the audio stream at the moment, because
there is no boundary check before the memcpy().
In preparation to an implementation for smaller and dynamically sized
packets, we have to address such cases, and copy the payload in two
steps conditionally.
The 'src' and 'dst' approach doesn't work here anymore, as different
behavior is necessary in playback and capture cases. Thus, this patch
open-codes the routine now.

commit f226708(usb: gadget: composite: dequeue cdev->req before free it in
composite_dev_cleanup) fixed a bug: free the usb request(i.e. cdev->req) but
does not dequeue it beforehand. This fix is not proper enough because it
dequeues the request after free its data buffer, considering the hardware can
access the buffer's memory anytime before the request's complettion rountine
runs, and usb_ep_dequeue always call the complettion rountine before it returns,
so the best way is to dequeue the request before free its buffer.

The known example uses interface numbers 12 and 13 for the ECM

It seems cleaner to restrict this driver to the ff/ff/ff
vendor specific interfaces rather than increasing the already
long interface number blacklist. This should be more future
proof if Sierra adds more class functions using interface

These are simple non-queued interrupt IN and interrupt OUT transfers. Currently,
only gadget zero is capable of executing the interrupt EP test cases. However,
extending the same to other gadgets is extremely simple and can be done
on-demand.
The two new tests added are
- Test 25: To verify Interrupt OUT transfer
- Test 26: To verify Interrupt IN transfer
Since the default value of wMaxPacketSize is set as 1024, so interrupt
IN transfers must be specified with the size parameter = multiple of

the transfer. See [RUN 4] for sample logs

The uSCSI from Newer Technology is a SCSI-USB converter with USB ID 06ca:2003.
Like several other SCSI-USB products, it's a Shuttle Technology OEM device.
Without a suitable entry in unusual-devs.h, the converter can only access the
(single) device with SCSI ID 0. Copying the entry for device 04e6:0002 allows
it to work with devices with other SCSI IDs too.
There are currently six entries for Shuttle-developed SCSI-USB devices in
unusual-devs.h (grep for euscsi):
04e6:0002 Shuttle eUSCSI Bridge USB_SC_DEVICE, USB_PR_DEVICE

This patch fixes a potential security issue in the whiteheat USB driver
which might allow a local attacker to cause kernel memory corrpution. This
is due to an unchecked memcpy into a fixed size buffer (of 64 bytes). On
EHCI and XHCI busses it's possible to craft responses greater than 64
bytes leading a buffer overflow.

Eliminate static struct *agdev_g from f_uac2.c.
It is used for freeing its memory, but the same address can be found
by calling container_of in afunc_unbind(). This implies eliminating
uac2_unbind_config(). The audio_config_driver in audio.c does not have
its unbind method any more. It has been used only when uac2 is used,

When we manually need to move the TR dequeue pointer we need to set the
correct cycle bit as well. Previously we used the trb pointer from the
last event received as a base, but this was changed in
commit 1f81b6d22a ("usb: xhci: Prefer endpoint context dequeue pointer")
to use the dequeue pointer from the endpoint context instead
It turns out some Asmedia controllers advance the dequeue pointer
stored in the endpoint context past the event triggering TRB, and
this messed up the way the cycle bit was calculated.
Instead of adding a quirk or complicating the already hard to follow cycle bit
code, the whole cycle bit calculation is now simplified and adapted to handle
event and endpoint context dequeue pointer differences.

dev_err() itself shows the port number, reduce the port number shown
in the error message, essentially reverting to the state before the
commit 5ee0f803cc.

After dbgp_bind()-dbgp_unbind() cycle happens, static variable dbgp
contains pointers to already deallocated memory (dbgp.serial and dbgp.req).
If the next dbgp_bind() fails, for example in usb_ep_alloc_request(),
dbgp_bind() calls dbgp_unbind() on failure path,
and dbgp_unbind() frees dbgp.serial that still stores a pointer
to already deallocated memory.
The patch sets pointers to NULL in dbgp_unbind().

This reverts commit 73228a0538 ("USB: option,zte_ev: move most ZTE
CDMA devices to zte_ev").
Move the IDs of the devices that were previously driven by the option
driver back to that driver.
As several users have reported, the zte_ev driver is causing random
disconnects as well as reconnect failures.
A closer analysis of the zte_ev setup code reveals that it consists of
standard CDC requests (SET/GET_LINE_CODING and SET_CONTROL_LINE_STATE)
but unfortunately fails to get some of those right. In particular, as
reported by Liu Lei, it fails to lower DTR/RTS on close. It also appears
that the control requests lack the interface argument.
Note that the zte_ev driver is based on code (once) distributed by ZTE
that still appears to originally have been reverse-engineered and bolted
onto the generic driver.
Since line control is already handled properly by the option driver, and
the SET/GET_LINE_CODING requests appears to be redundant (amounts to a
SET 9600 8N1), this is a first step in ultimately removing the redundant
zte_ev driver.
Note that AC2726 had already been moved back to option, and that some
IDs were in the device table of both drivers prior to the commit being
reverted.

Streams on the EJ168 do not work as they should. I've spend 2 days trying
to get them to work, but without success.
The first problem is that when ever you ring the stream-ring doorbell, the
controller starts executing trbs at the beginning of the first ring segment,
event if it ended somewhere else previously. This can be worked around by
allowing enqueing only one td (not a problem with how streams are typically
used) and then resetting our copies of the enqueueing en dequeueing pointers
on a td completion to match what the controller seems to be doing.
This way things seem to start working with uas and instead of being able
to complete only the very first scsi command, the scsi core can probe the disk.
But then things break later on when td-s get enqueued with more then one
trb. The controller does seem to increase its dequeue pointer while executing
a stream-ring (data transfer events I inserted for debugging do trigger).
However execution seems to stop at the final normal trb of a multi trb td,
even if there is a data transfer event inserted after the final trb.
The first problem alone is a serious deviation from the spec, and esp.
dealing with cancellation would have been very tricky if not outright
impossible, but the second problem simply is a deal breaker altogether,
so this patch simply disables streams.
Note this will cause the usb-storage + uas driver pair to automatically switch
to using usb-storage instead of uas on these devices, essentially reverting
to the 3.14 and earlier behavior when uas was marked CONFIG_BROKEN.

Some jmicron uas chipsets act up (they disconnect from the bus) when sending
more then 32 commands to them at once.
Rather then building an ever growing list with usb-id based quirks for
devices using this chipset, simply reduce the qdepth to 32 when connected
over usb-2. 32 should be plenty to keep things close to maximum
possible throughput on usb-2.

This patch adds a usb quirk to support devices with interupt endpoints
and bInterval values expressed as microframes. The quirk causes the
parse endpoint function to modify the reported bInterval to a standards
conforming value.
There is currently code in the endpoint parser that checks for
bIntervals that are outside of the valid range (1-16 for USB 2+ high
speed and super speed interupt endpoints). In this case, the code assumes
the bInterval is being reported in 1ms frames. As well, the correction
is only applied if the original bInterval value is out of the 1-16 range.
With this quirk applied to the device, the bInterval will be
accurately adjusted from microframes to an exponent.

This patch adds an extra check to ohci-hcd's I/O watchdog routine. If
the controller stops updating the frame counter, we will assume it is
dead. But there has to be an exception: Some controllers stop the
frame counter when no ports are connected. Check to make sure there
is at least one active port before deciding the controller is dead.
(This test may appear racy, but it isn't. Enabling a newly connected
port takes several milliseconds, during which time the frame counter
must advance.)

Some OHCI controllers have a bug: They fail to add completed TDs to
the done queue. Examining this queue is the only method ohci-hcd has
for telling when a transfer is complete; failure to add a TD can
result in an URB that never completes and cannot be unlinked.
This patch adds a watchdog routine to ohci-hcd. The routine
periodically scans the active ED and TD lists, looking for TDs which
are finished but not on the done queue. When one is found, and it is
certain that the controller hardware will never add the TD to the done
queue, the watchdog routine manually puts the TD on the done list so
that it can be handled normally.
The watchdog routine also checks for a condition indicating the
controller has died. If the done queue is non-empty but the
HccaDoneHead pointer hasn't been updated for a few hundred
milliseconds, we assume the controller will never update it and
therefore is dead.

URBs for a particular endpoint should complete sequentially. That is,
we shouldn't call the completion handler for one URB until the handler
for the previous URB has returned.
When the OHCI watchdog routine is added, there will be two paths for
completing URBs: interrupt handler and watchdog routine. Their
activities have to be synchronized so that completions don't occur in
multiple threads concurrently.
For that purpose, this patch creates an ohci_work() routine which will
be responsible for calling process_done_list() and finish_unlinks(),
the two routines that detect when an URB is complete. Everything will
funnel through ohci_work(), and it will be careful not to run in more
than one thread at a time.

This patch changes the way ohci-hcd handles the TD done list. In
addition to relying on the TD pointers stored by the controller
hardware, we need to handle TDs that the hardware has forgotten about.
This means the list has to exist even while the dl_done_list() routine
isn't running. That function essentially gets split in two:
update_done_list() reads the TD pointers stored by the hardware and
adds the TDs to the done list, and process_done_list() scans through
the list to handle URB completions. When we detect a TD that the
hardware forgot about, we will be able to add it to the done list
manually and then process it normally.
Since the list is really a queue, and because there can be a lot of
TDs, keep the existing singly linked implementation. To insure that
URBs are given back in order of submission, whenever a TD is added to
the done list, all the preceding TDs for the same endpoint must be
added as well (going back to the first one that isn't already on the
done list).
The done list manipulations must all be protected by the private
lock. The scope of the lock is expanded in preparation for the
watchdog routine to be added in a later patch.
We have to be more careful about giving back unlinked URBs. Since TDs
may be added to the done list by the watchdog routine and not in
response to a controller interrupt, we have to check explicitly to
make sure all the URB's TDs that were added to the done list have been
processed before giving back the URB.

When an URB is unlinked from a dead controller, ohci-hcd gives back
the URB with no regard for cleaning up the internal data structures.
This won't play nicely with the upcoming changes to the TD done
list.
Therefore make ohci_urb_dequeue() call finish_unlinks(), which uses
td_done() to do a proper cleanup, rather than calling finish_urb()
directly. Also, remove the checks that urb_priv is non-NULL; the
driver guarantees that urb_priv will never be NULL for a valid URB.

Problem Summary: Problem has been observed generally with PM states
where VBUS goes off during suspend. There are some SS USB devices which
take longer time for link training compared to many others. Such
devices fail to reconnect with same old address which was associated
with it before suspend.
When system resumes, at some point of time (dpm_run_callback->
usb_dev_resume->usb_resume->usb_resume_both->usb_resume_device->
usb_port_resume) SW reads hub status. If device is present,
then it finishes port resume and re-enumerates device with same
address. If device is not present then, SW thinks that device was
removed during suspend and therefore does logical disconnection
and removes all the resource allocated for this device.
Now, if I put sufficient delay just before root hub status read in
usb_resume_device then, SW sees always that device is present. In normal
course(without any delay) SW sees that no device is present and then SW
removes all resource associated with the device at this port. In the
latter case, after sometime, device says that hey I am here, now host
enumerates it, but with new address.
Problem had been reproduced when I connect verbatim USB3.0 hard disc
with my STiH407 XHCI host running with 3.10 kernel.
I see that similar problem has been reported here.
https://bugzilla.kernel.org/show_bug.cgi?id=53211
Reading above it seems that bug was not in 3.6.6 and was present in 3.8
and again it was not present for some in 3.12.6, while it was present
for few others. I tested with 3.13-FC19 running at i686 desktop, problem
was still there. However, I was failed to reproduce it with 3.16-RC4
running at same i686 machine. I would say it is just a random
observation. Problem for few devices is always there, as I am unable to
find a proper fix for the issue.
So, now question is what should be the amount of delay so that host is
always able to recognize suspended device after resume.
XHCI specs 4.19.4 says that when Link training is successful, port sets
CSC bit to 1. So if SW reads port status before successful link
training, then it will not find device to be present. USB Analyzer log
with such buggy devices show that in some cases device switch on the
RX termination after long delay of host enabling the VBUS. In few other
cases it has been seen that device fails to negotiate link training in
first attempt. It has been reported till now that few devices take as
long as 2000 ms to train the link after host enabling its VBUS and
RX termination. This patch implements a 2000 ms timeout for CSC bit to set
ie for link training. If in a case link trains before timeout, loop will
exit earlier.
This patch implements above delay, but only for SS device and when
persist is enabled.
So, for the good device overhead is almost none. While for the bad
devices penalty could be the time which it take for link training.
But, If a device was connected before suspend, and was removed
while system was asleep, then the penalty would be the timeout ie
2000 ms.
Results:
Verbatim USB SS hard disk connected with STiH407 USB host running 3.10
Kernel resumes in 461 msecs without this patch, but hard disk is
assigned a new device address. Same system resumes in 790 msecs with
this patch, but with old device address.

When using USB 3.0 pen drive with the [AMD] FCH USB XHCI Controller

I found the commit number 41e7e056cd
(USB: Allow USB 3.0 ports to be disabled.) causes the bug. After doing
some experiments, the bug can be fixed by avoiding executing the function
hub_usb3_port_disable(). Because the port status with [AMD] FCH USB
XHCI Controlleris [1022:7814] is already in RxDetect
(I tried printing out the port status before setting to Disabled state),
it's reasonable to check the port status before really executing

delays enumeration noticably. There should be no reason to reset a device

OTG3 and EH Compliance Plan 1.0 talks about Super Speed OTG Verification
system (SS-OVS) which consists of an excersizer and analyzer.
USB Compliance Suite from Lecroy or Ellisys can act as such SS-OVS for
Link Layer Validation (LVS).
Some modifications are needed for an embedded Linux USB host to pass all
these tests. Most of these tests require just Link to be in U0. They do
not work with default Linux USB stack since, default stack does port
reset and then starts sending setup packet, which is not expected by
Link Layer Validation (LVS) device of Lecroy Compliance Suit. Then,
There are many Link Layer Tests which need host to generate specific
traffic.
This patch supports specific traffic generation cases. As of now all the
host Lecroy Link Layer-USBIF tests (except TD7.26) passes
with this patch for single run using Lecroy USB Compliance Suite
Version 1.98 Build 239 and Lecroy USB Protocol Analyzer version 4.80
Build 1603. Therefore patch seems to be a good candidate for inclusion.
Further modification can be done on top of it.
lvstest driver will not bind to any device by default. It can bind
manually to a super speed USB host controller root hub. Therefore, regular
hub driver must be unbound before this driver is bound. For example, if
2-0:1.0 is the xhci root hub, then execute following to unbind hub driver.
echo 2-0:1.0 > /sys/bus/usb/drivers/hub/unbind
Then write Linux Foundation's vendor ID which is used by root hubs and
SS root hub's device ID into new_id file. Writing IDs into new_id file
will also bind the lvs driver with any available SS root hub interfaces.
echo "1D6B 3" > /sys/bus/usb/drivers/lvs/new_id
Now connect LVS device with root hub port.
Test case specific traffic can be generated as follows whenever needed:
1. To issue "Get Device descriptor" command for TD.7.06:
echo > /sys/bus/usb/devices/2-0\:1.0/get_dev_desc
2. To set U1 timeout to 127 for TD.7.18
echo 127 > /sys/bus/usb/devices/2-0\:1.0/u1_timeout
3. To set U2 timeout to 0 for TD.7.18
echo 0 > /sys/bus/usb/devices/2-0\:1.0/u2_timeout
4. To issue "Hot Reset" for TD.7.29
echo > /sys/bus/usb/devices/2-0\:1.0/hot_reset
5. To issue "U3 Entry" for TD.7.35
echo > /sys/bus/usb/devices/2-0\:1.0/u3_entry
6. To issue "U3 Exit" for TD.7.36
echo > /sys/bus/usb/devices/2-0\:1.0/u3_exit

This patch fixes a bug in ohci-hcd. When an URB is unlinked, the
corresponding Endpoint Descriptor is added to the ed_rm_list and taken
off the hardware schedule. Once the ED is no longer visible to the
hardware, finish_unlinks() handles the URBs that were unlinked or have
completed. If any URBs remain attached to the ED, the ED is added
back to the hardware schedule -- but only if the controller is
running.
This fails when a controller dies. A non-empty ED does not get added
back to the hardware schedule and does not remain on the ed_rm_list;
ohci-hcd loses track of it. The remaining URBs cannot be unlinked,
which causes the USB stack to hang.
The patch changes finish_unlinks() so that non-empty EDs remain on
the ed_rm_list if the controller isn't running. This requires moving
some of the existing code around, to avoid modifying the ED's hardware
fields more than once.

There are 2 methods for ZLP (zero-length packet) generation:
1) In software
2) Automatic generation by device controller
1) is implemented in UDC driver and it attaches ZLP to IN packet if
descriptor->size < wLength
2) can be enabled/disabled by setting ZLT bit in the QH
When gadget ffs is connected to ubuntu host, the host sends
get descriptor request and wLength in setup packet is 255 while the
size of descriptor which will be sent by gadget in IN packet is
64 byte. So the composite driver sets req->zero = 1.
In UDC driver following code will be executed then
if (hwreq->req.zero && hwreq->req.length
&& (hwreq->req.length % hwep->ep.maxpacket == 0))
add_td_to_list(hwep, hwreq, 0);
Case-A:
So in case of ubuntu host, UDC driver will attach a ZLP to the IN packet.
ubuntu host will request 255 byte in IN request, gadget will send 64 byte
with ZLP and host will come to know that there is no more data.
But hold on, by default ZLT=0 for endpoint 0 so hardware also tries to
automatically generate the ZLP which blocks enumeration for ~6 seconds due
to endpoint 0 STALL, NAKs are sent to host for any requests (OUT/PING)
Case-B:
In case when gadget ffs is connected to Apple device, Apple device sends
setup packet with wLength=64. So descriptor->size = 64 and wLength=64
therefore req->zero = 0 and UDC driver will not attach any ZLP to the
IN packet. Apple device requests 64 bytes, gets 64 bytes and doesn't
further request for IN data. But ZLT=0 by default for endpoint 0 so
hardware tries to automatically generate the ZLP which blocks enumeration
for ~6 seconds due to endpoint 0 STALL, NAKs are sent to host for any
requests (OUT/PING)
According to USB2.0 specs:
8.5.3.2 Variable-length Data Stage
A control pipe may have a variable-length data phase in which the
host requests more data than is contained in the specified data
structure. When all of the data structure is returned to the host,
the function should indicate that the Data stage is ended by
returning a packet that is shorter than the MaxPacketSize for the
pipe. If the data structure is an exact multiple of wMaxPacketSize
for the pipe, the function will return a zero-length packet to indicate
the end of the Data stage.
In Case-A mentioned above:
If we disable software ZLP generation & ZLT=0 for endpoint 0 OR if software
ZLP generation is not disabled but we set ZLT=1 for endpoint 0 then
enumeration doesn't block for 6 seconds.
In Case-B mentioned above:
If we disable software ZLP generation & ZLT=0 for endpoint then enumeration
still blocks due to ZLP automatically generated by hardware and host not needing
it. But if we keep software ZLP generation enabled but we set ZLT=1 for
endpoint 0 then enumeration doesn't block for 6 seconds.
So the proper solution for this issue seems to disable automatic ZLP generation
by hardware (i.e by setting ZLT=1 for endpoint 0) and let software (UDC driver)
handle the ZLP generation based on req->zero field.
