2703eaaf4e scsi: qla2xxx: Use tabs to indent code
Most but not all code in the qla2xxx driver uses tabs for indentation.
Make the qla2xxx code easier to read by using tabs consistently for
indentation. This patch improves conformance with the Linux kernel coding
style.

a6a6d0589a scsi: scsi_transport_fc: nvme: display FC-NVMe port roles
Currently the FC-NVMe driver is leverating the SCSI FC transport class to
access the remote ports. Which means that all FC-NVMe remote ports will be
visible to the fc transport layer, but due to missing definitions the port
roles will always be 'unknown'.  This patch adds the missing definitions to
the fc transport class to that the port roles are correctly displayed.

e81d1bcbde scsi: qla2xxx: Further limit FLASH region write access from SysFS
Recent ISPs have larger and more complex flash-write semantics
(secure-access and signing). The BSG interfaces support these semantics for
all ISPs and is exclusively used by QLogic user-space tools. Limit
flash-write operations to ISPs <= 25xx.

5cbdae10bf scsi: qla2xxx: Fix incorrect region-size setting in optrom SYSFS routines
Commit e6f77540c0 ("scsi: qla2xxx: Fix an integer overflow in sysfs
code") incorrectly set 'optrom_region_size' to 'start+size', which can
overflow option-rom boundaries when 'start' is non-zero.  Continue setting
optrom_region_size to the proper adjusted value of 'size'.

6b1f44466d scsi: qla2xxx: Increase the max_sgl_segments to 1024
This patch increases max_sgl_segments value from 128 to the maximum
supported which is 1024. Increasing max_sgl_segments will allow the driver
to support larger I/O sizes

Driver maintains state machine for processing and completing switch
commands. This patch resets FCF_ASYNC_{SENT|ACTIVE} flag to indicate if the
previous command is active or sent, in order for next GPSC command to
advance the state machine.

740e29358e scsi: qla2xxx: Set the SCSI command result before calling the command done
This patch tries to address race condition between abort handler and
completion handler. When scsi command result is set by both abort and
completion handler, scsi_done() is only called after refcount on SRB
structure goes to zero. The abort handler sets this result prematurely even
when the refcount is non-zero value. Fix this by setting SCSI cmd->result
before scsi_done() is called.

statement because it thinks the logical not should be applied to the whole
statement, not just the left side:

This particular instance was already fixed by commit 0bfe7d3cae ("scsi:
qla2xxx: Simplify conditional check") upstream but it was reintroduced by
commit 3695310e37 ("scsi: qla2xxx: Update flash read/write routine") in
the 5.2/scsi-queue.

4705f10e82 scsi: qla2xxx: Fix FC-AL connection target discovery
Commit 7f147f9bfd ("scsi: qla2xxx: Fix N2N target discovery with Local
loop") fixed N2N target discovery for local loop.  However, same code is
used for FC-AL discovery as well. Added check to make sure we are bypassing
area and domain check only in N2N topology for target discovery.

There are special %pad and %pap format strings in Linux that we could use
here, but since the driver already does 64-bit arithmetic on the values,
using a plain 'u64' seems more consistent here.

Note: A possible related issue may be that the driver possibly checks the
wrong kind of overflow: when an IOMMU is in use, buffers that cross a
32-bit boundary in physical addresses would still be mapped into dma
addresses within the low 4GB space, so I suspect that we actually want to
check sg_dma_address() instead of sg_phys() here.

db0f166e9a scsi: qla2xxx: Fix panic in qla_dfs_tgt_counters_show
When trying to display tgt_counters in the debugfs, a panic can result.

There is no null check for qpair after it is assigned in the for-loop.
Unless vha->hw->queue_pair_map array is completely filled with entries, the
system will panic dereferencing a null pointer.

f3e0269517 scsi: qla2xxx: Avoid PCI IRQ affinity mapping when multiqueue is not supported
This patch fixes warning seen when BLK-MQ is enabled and hardware does not
support MQ. This will result into driver requesting MSIx vectors which are
equal or less than pre_desc via PCI IRQ Affinity infrastructure.

f233e8c000 scsi: qla2xxx: Move debug messages before sending srb preventing panic
When sending an srb with qla2x00_start_sp, the sp can complete and be freed
by the time we log the debug message saying we sent it. This can cause a
panic if sp gets reused quickly or when running a kernel that poisons freed
memory.

This was partially fixed by (not every case was addressed):

dcbf8f8087 scsi: qla2xxx: Restore FAWWPN of Physical Port only for loop down
When loop was made down explicitly due to cable pull, then for N2N toplogy,
if FAWWPN BIT is enabled by user, then it would restore some default
(garbage) value for Physical port WWPN, so this show garbage WWPN for the
port. Fix is, to restore physical port WWPN, if it is fabric
configuration. When loop is explicitly made down, and FAWWPN feature is
enabled, then driver need to restore original flashed WWPN.

50b812755e scsi: qla2xxx: Fix DMA error when the DIF sg buffer crosses 4GB boundary
When SGE buffer containing DIF information crosses 4G boundary, it results
in DMA error. This patch fixes this issue by calculating SGE buffer size
and if it crosses 4G boundary, driver will split it into multiple SGE
buffers to avoid DMA error.

When SCSI-MQ is enabled, in some case system would present
nr_possible_cpus() which is greater than requested vectors by the
driver. This results into driver being able to get larger number of MSI-X
vectors than actual online CPUs.  Driver then uses
pci_alloc_irq_vectors_affinity() to assign 1:1 mapping and affinity for
each MSI-x vector to CPUs. When the command is submitted using MSI-x
vector, assigned to offline CPU, it results in an ABTS and system
hang. This hang is result of a driver not being able to process interrupt
on a vector assigned to an Off-line CPUs

This patch fixes this issue by setting irq_offset value for the
blk_mq_pci_map_queues() to use only those CPUs which has CPU mask affinity
assigned and are online. By using the irq_offset value, driver will allow
online cpumask to decide which vectors are used in blk_mq_pci_map_queues().

17b18eaa6f scsi: qla2xxx: deadlock by configfs_depend_item
The intent of invoking configfs_depend_item in commit 7474f52a82
("tcm_qla2xxx: Perform configfs depend/undepend for base_tpg")
was to prevent a physical Fibre Channel port removal when
virtual (NPIV) ports announced through that physical port are active.
The change does not work as expected: it makes enabled physical port
dependent on target configfs subsystem (the port's parent), something
the configfs guarantees anyway.

Besides, scheduling work in a worker thread and waiting for the work's
completion is not really a valid workaround for the requirement not to call
configfs_depend_item from a configfs callback: the call occasionally
deadlocks.

Thus, removing configfs_depend_item calls does not break anything and fixes
the deadlock problem.

fallback to target/$fabric vs .fabric_name comparison if .fabric_alias
isn't initialised.  iscsi_target_mod is the only fabric module to set
.fabric_alias . All other fabric modules rely on .fabric_name matching and
can drop the duplicate string.

All fabrics return a const string. In all cases *except* iSCSI the
get_fabric_name() string matches fabric_ops.name.

Both fabric_ops.get_fabric_name() and fabric_ops.name are user-facing, with
the former being used for PR/ALUA state and the latter for ConfigFS
(config/target/$name), so we unfortunately need to keep both strings around
for now.  Replace the useless .get_fabric_name() accessor function with a
const string fabric_name member variable.

This patch prevents user space mailbox request from doing chip reset if the
mailbox timed out. The chip reset is only reserved for the DPC thread to
ensure all mailbox requests are flushed properly. The DPC thread is

Current code relies on switch to provide a unique combination of WWPN +
NPORTID to tract an FC port.  This patch tries to detect a case where switch
data base can get corrupted where multiple WWPNs can have the same Nport ID.
The 1st Nport ID on the list will be kept while the duplicate Nport ID will be
discarded.

d594db0187 scsi: qla2xxx: Fix premature command free
When qla2xxx and Target Core gets out of sync during command cleanup, qla2xxx
will not free command until it is out of firmware's hand and Target Core has
called the release on the command.

This patch adds synchronization using cmd_lock and release flag.  If the
release flag is set, then qla2xxx will free up the command using
qlt_free_cmd() otherwise transport_generic_free_cmd() will be responsible for
relase of the command.

On PLOGI complete + RSCN received, driver tries to handle RSCN but failed to
reset the session back to the beginning to restart the login process. Instead
the session was left in the Plogi complete without moving forward.  This patch
will push the session state back to the delete state and restart the
connection.

Add ability to allow each physical port to control operating mode.  Current
code forces all ports to behave in one mode (i.e. initiator, target or
dual). This patch allows user to select the operating mode for each port.

- Driver must be loaded in dual mode to allow resource allocation

- In addition user can make adjustment to exchange resources using following

0e324e949e scsi: qla2xxx: Fix race condition for resource cleanup
For Loop topology + Initiator, FW is in control of PLOGI/PRLI.  When link is
reset, driver will try to cleanup the session by doing an Implicit Logout.
Instead, the code is doing an Explicit Logout.  The explicit logout interferes
with FW state machine in trying to reconnect.  The implicit logout was meant
for FW to flush commands.  In loop, it is not needed because FW will auto
flush.

ABTS response for 20s. On a timeout, second ABTS can go out with another 20s

585def9b2f scsi: qla2xxx: Move ABTS code behind qpair
Current abort code defaults to legacy single queue where hardware_lock is used
to protect command search.  This patch moves this code behind the QPair where
the qp_lock_ptr will reference the appropriate lock for either legacy/single
queue or MQ.

d4f7a16aec scsi: qla2xxx: Remove ASYNC GIDPN switch command
Using GPNFT/GNNFT command will be able to cover switch database with less
number of scans. This patch removes Get NportID with provided WWPN/GIDPN
switch command. By making this change, in large fabric with lots of remote
port or NPIV ports with noisy SAN, the number of GIDPN commands issued by a
port when it detects large number of remote ports going away or coming back,
can overwhelmn the switch and it can becomde unresponsive. In a case where the
fabric has not change, GIDPN is not required.

0aca77843e scsi: qla2xxx: Reduce holding sess_lock to prevent CPU lock-up
- Reduce sess_lock holding to prevent CPU Lock up. sess_lock was held across
  fc_port registration and deletion.  These calls can be blocked by upper
  layer. Sess_lock is also being accessed by interrupt thread.

- Reduce number of loops in processing work_list to prevent kernel complaint
  of CPU lockup or holding sess_lock.

6a62946892 scsi: qla2xxx: Move {get|rel}_sp to base_qpair struct
Currently, qla2x00_[get_sp|rel_sp] routines does {get|release} of srb
resource/srb_mempool directly from qla_hw_data.  qla2x00_start_sp() is used to
issue management commands through the default Request Q 0 & Response Q 0 or
base_qpair. This patch moves access of these resources through
base_qpair. Instead of having knowledge of specific Q number and lock to
rsp/req queue, this change will key off the qpair that is assigned to the srb
resource. This lays the ground work for other routines to see this resource
through the qpair.

8b4673ba3a scsi: qla2xxx: Add support for ZIO6 interrupt threshold
Add sysfs support to control zio6 interrupt threshold. Using this sysfs hook
user can set when to generate interrupts. This value will be used to tell
firmware to generate interrupt at a certain interval.  If the number of
exchanges/commands fall below defined setting, then the interrupt will be
generated immediately by the firmware.

By default ZIO6 will coalesce interrupts to a specified interval
regardless of low traffic or high traffic.

6b0431d6fa scsi: qla2xxx: Fix out of order Termination and ABTS response
Following changes are added by this patch

- Prevent ABTS Response from getting in front of Termination of exchange.
  Firmware requires driver to cleanup exchanges before ABTS response can be
  sent. This reduces ABTS response error which triggers extra command
  re-termination and re-sending of ABTS response.

- Add bits in driver and tracks CTIO/ATIO attribute bits for proper command
  Termination. A copy of the ATTR bits will be kept in the ABTS task
  management command as a back up copy, if an ABTS response encounters an
  error.

0691094ff3 scsi: qla2xxx: Add logic to detect ABTS hang and response completion
ABTS error completion can trigger an exchange cleanup from the driver and
another ABTS response will be generated.  This retry of ABTS response can
cause loop between driver trying to send ABTS and firmware returning error.

This patch fixes this issue by adding logic to check for unresolved exchanges
and clean up before ABTS is retried.  This patch also addes the fix to use the
same qpair as the ABTS completion for the terminatation of exchange as well as
retry of ABTS response.

cd4ed6b470 scsi: qla2xxx: Move rport registration out of internal work_list
Currently, the rport registration is being called from a single work element
that is used to process QLA internal "work_list".  This work_list is meant for
quick and simple task (ie no sleep).  The Rport registration process sometime
can be delayed by upper layer.  This causes back pressure with the internal
queue where other jobs are unable to move forward.

This patch will schedule the registration process with a new work element
(fc_port.reg_work).  While the RPort is being registered, the current state of
the fcport will not move forward until the registration is done.  If the state
of the fabric has changed, a new field/next_disc_state will record the next
action on whether to 'DELETE' or 'Reverify the session/ADISC'.

8777e4314d scsi: qla2xxx: Migrate NVME N2N handling into state machine
This patch fixes regression introduced for the N2N support for FC-NVMe. For
FC-NVMe with N2N connection, instead of FW initiating the Login, Driver
starts Login process.  This patch migrates that new process from a
standalone path into existing session management state machine. With this
state change now driver will not wait for pull NPort ID from FW.
