130c93fd10 arm64: efi: don't restore TTBR0 if active_mm points at init_mm
init_mm isn't a normal mm: it has swapper_pg_dir as its pgd (which
contains kernel mappings) and is used as the active_mm for the idle
thread.

When restoring the pgd after an EFI call, we write current->active_mm
into TTBR0. If the current task is actually the idle thread (e.g. when
initialising the EFI RTC before entering userspace), then the TLB can
erroneously populate itself with junk global entries as a result of
speculative table walks.

When we do eventually return to userspace, the task can end up hitting
these junk mappings leading to lockups, corruption or crashes.

This patch fixes the problem in the same way as the CPU suspend code by
ensuring that we never switch to the init_mm in efi_set_pgd and instead
point TTBR0 at the zero page. A check is also added to cpu_switch_mm to
BUG if we get passed swapper_pg_dir.

da9c177de8 arm64: enforce x1|x2|x3 == 0 upon kernel entry as per boot protocol
According to the arm64 boot protocol, registers x1 to x3 should be
zero upon kernel entry, and non-zero values are reserved for future
use. This future use is going to be problematic if we never enforce
the current rules, so start enforcing them now, by emitting a warning
if non-zero values are detected.

6f4d57fa70 arm64: remove __calc_phys_offset
This removes the function __calc_phys_offset and all open coded
virtual to physical address translations using the offset kept
in x28.

Instead, just use absolute or PC-relative symbol references as
appropriate when referring to virtual or physical addresses,
respectively.

8b0a95753a arm64: merge __enable_mmu and __turn_mmu_on
Enabling of the MMU is split into two functions, with an align and
a branch in the middle. On arm64, the entire kernel Image is ID mapped
so this is really not necessary, and we can just merge it into a
single function.

Also replaces an open coded adrp/add reference to __enable_mmu pair
with adr_l.

a44ef51799 arm64: remove processor_id
The global processor_id is assigned the MIDR_EL1 value of the boot
CPU in the early init code, but is never referenced afterwards.

As the relevance of the MIDR_EL1 value of the boot CPU is debatable
anyway, especially under big.LITTLE, let's remove it before anyone
starts using it.

The adrp instruction is mostly used in combination with either
an add, a ldr or a str instruction with the low bits of the
referenced symbol in the 12-bit immediate of the followup
instruction.

Introduce the macros adr_l, ldr_l and str_l that encapsulate
these common patterns.

a591ede4cd arm64: Get rid of struct cpu_table
struct cpu_table is an artifact left from the (very) early days of
the arm64 port, and its only real use is to allow the most beautiful
"AArch64 Processor" string to be displayed at boot time.

Really? Yes, really.

Let's get rid of it. In order to avoid another BogoMips-gate, the
aforementioned string is preserved.

8fff105e13 arm64: perf: reject groups spanning multiple HW PMUs
The perf core implicitly rejects events spanning multiple HW PMUs, as in
these cases the event->ctx will differ. However this validation is
performed after pmu::event_init() is called in perf_init_event(), and
thus pmu::event_init() may be called with a group leader from a
different HW PMU.

The ARM64 PMU driver does not take this fact into account, and when
validating groups assumes that it can call to_arm_pmu(event->pmu) for
any HW event. When the event in question is from another HW PMU this is
wrong, and results in dereferencing garbage.

This patch updates the ARM64 PMU driver to first test for and reject
events from other PMUs, moving the to_arm_pmu and related logic after
this test. Fixes a crash triggered by perf_fuzzer on Linux-4.0-rc2, with
a CCI PMU present:

06f75a1f62 ARM, arm64: kvm: get rid of the bounce page
The HYP init bounce page is a runtime construct that ensures that the
HYP init code does not cross a page boundary. However, this is something
we can do perfectly well at build time, by aligning the code appropriately.

For arm64, we just align to 4 KB, and enforce that the code size is less
than 4 KB, regardless of the chosen page size.

For ARM, the whole code is less than 256 bytes, so we tweak the linker
script to align at a power of 2 upper bound of the code size

Note that this also fixes a benign off-by-one error in the original bounce
page code, where a bounce page would be allocated unnecessarily if the code
was exactly 1 page in size.

On ARM, it also fixes an issue with very large kernels reported by Arnd
Bergmann, where stub sections with linker emitted veneers could erroneously
trigger the size/alignment ASSERT() in the linker script.

4a97abd443 arm64/crypto: issue aese/aesmc instructions in pairs
This changes the AES core transform implementations to issue aese/aesmc
(and aesd/aesimc) in pairs. This enables a micro-architectural optimization
in recent Cortex-A5x cores that improves performance by 50-90%.

Measured performance in cycles per byte (Cortex-A57):

  before        3.64            1.34            1.32
  after         1.95            0.85            0.93

Note that this results in a ~5% performance decrease for older cores.

b63dbef93f arm64: fixmap: check idx is definitely valid
Fixmap indices are in the interval (FIX_HOLE, __end_of_fixed_addresses),
but in __set_fixmap we only check idx <= __end_of_fixed_addresses, and
therefore indices <= FIX_HOLE are erroneously accepted. If called with
such an idx, __set_fixmap may corrupt page tables outside of the fixmap
region.

This patch ensures that we validate the idx against both endpoints of
the interval.

19fc577579 arm64: fixmap: make FIX_TEXT_POKE0 permanent
The FIX_TEXT_POKE0 is currently at the end of the temporary fixmap
slots, despite the fact that it can be used at any point during runtime
(e.g. for poking the text of loaded modules), and thus should be a
permanent fixmap slot (as is the case on arm and x86).

This patch moves FIX_TEXT_POKE0 into the set of permanent fixmap slots.

e6a2e1b6c2 arm64: mm: unexport set_memory_ro and set_memory_rw
This effectively unexports set_memory_ro and set_memory_rw functions from
commit 11d91a770f ("arm64: Add CONFIG_DEBUG_SET_MODULE_RONX support").

No module user of those is in mainline kernel and we explicitly do not want
modules to use these functions, as they i.e. RO-protect eBPF (interpreted and
JIT'ed) images from malicious modifications/bugs.

Outside of eBPF scope, I believe also other set_memory_* functions should
be unexported on arm64 due to non-existant mainline module user. Laura
mentioned that they have some uses for modules doing set_memory_*, but
none that are in mainline and it's unclear if they would ever get there.

a8fcd8b192 arm64: Enable CONFIG_COMPAT also for 64k page size
With binutils 2.25 the default alignment for 32bit arm sections changed to
have everything 64k aligned. Armv7 binaries built with this binutils version
run successfully on an arm64 system.

Since effectively there is now the chance to run armv7 code on arm64 even
with 64k page size, it doesn't make sense to block people from enabling
CONFIG_COMPAT on those configurations.

ad08fd494b arm64: Adjust EFI libstub object include logic
Commit f4f75ad5 ("efi: efistub: Convert into static library")
introduced a static library for EFI stub, libstub.

The EFI libstub directory is referenced by the kernel build system via
a obj subdirectory rule in:
drivers/firmware/efi/Makefile

Unfortunately, arm64 also references the EFI libstub via:
libs-$(CONFIG_EFI_STUB) += drivers/firmware/efi/libstub/

If we're unlucky, the kernel build system can enter libstub via two
simultaneous threads resulting in build failures such as:

667f3fd395 arm64: log CPU boot modes
We currently don't log the boot mode for arm64 as we do for arm, and
without KVM the user is provided with no indication as to which mode(s)
CPUs were booted in, which can seriously hinder debugging in some cases.

Add logging to the boot path once all CPUs are up. Where CPUs are
mismatched in violation of the boot protocol, WARN and set a taint (as
we do for CPU other CPU feature mismatches) given that the
firmware/bootloader is buggy and should be fixed.

424a383824 arm64: fix hyp mode mismatch detection
Commit 828e9834e9 ("arm64: head: create a new function for setting
the boot_cpu_mode flag") added BOOT_CPU_MODE_EL1, a nonzero value
replacing uses of zero. However it failed to update __boot_cpu_mode
appropriately.

A CPU booted at EL2 writes BOOT_CPU_MODE_EL2 to __boot_cpu_mode[0], and
a CPU booted at EL1 writes BOOT_CPU_MODE_EL1 to __boot_cpu_mode[1].
Later is_hyp_mode_mismatched() determines there to be a mismatch if
__boot_cpu_mode[0] != __boot_cpu_mode[1].

If all CPUs are booted at EL1, __boot_cpu_mode[0] will be set to
BOOT_CPU_MODE_EL1, but __boot_cpu_mode[1] will retain its initial value
of zero, and is_hyp_mode_mismatched will erroneously determine that the
boot modes are mismatched. This hasn't been a problem so far, but later
patches which will make use of is_hyp_mode_mismatched() expect it to
work correctly.

This patch initialises __boot_cpu_mode[1] to BOOT_CPU_MODE_EL1, fixing
the erroneous mismatch detection when all CPUs are booted at EL1.

137650aad9 arm64: apply alternatives for !SMP kernels
Currently we only perform alternative patching for kernels built with
CONFIG_SMP, as we call apply_alternatives_all() in smp.c, which is only
built for CONFIG_SMP. Thus !SMP kernels may not have necessary
alternatives patched in.

This patch ensures that we call apply_alternatives_all() once all CPUs
are booted, even for !SMP kernels, by having the smp_init_cpus() stub
call this for !SMP kernels via up_late_init. A new wrapper,
do_post_cpus_up_work, is added so we can hook other calls here later
(e.g. boot mode logging).

1baa82f480 arm64: Implement cpu_relax as yield
ARM64 has the yield nop hint which has the intended semantics of
cpu_relax. Implement.

The immediate application is ARM CPU emulators. An emulator can take
advantage of the yield hint to de-prioritise an emulated CPU in favor
of other emulation tasks. QEMU A64 SMP emulation has yield awareness,
and sees a significant boot time performance increase with this change.

285994a62c arm64: Invalidate the TLB corresponding to intermediate page table levels
The ARM architecture allows the caching of intermediate page table
levels and page table freeing requires a sequence like:

	pmd_clear()
	TLB invalidation
	pte page freeing

With commit 5e5f6dc105 (arm64: mm: enable HAVE_RCU_TABLE_FREE logic),
the page table freeing batching was moved from tlb_remove_page() to
tlb_remove_table(). The former takes care of TLB invalidation as this is
also shared with pte clearing and page cache page freeing. The latter,
however, does not invalidate the TLBs for intermediate page table levels
as it probably relies on the architecture code to do it if required.
When the mm->mm_users < 2, tlb_remove_table() does not do any batching
and page table pages are freed before tlb_finish_mmu() which performs
the actual TLB invalidation.

This patch introduces __tlb_flush_pgtable() for arm64 and calls it from
the {pte,pmd,pud}_free_tlb() directly without relying on deferred page
table freeing.

35307b9a5f arm/arm64: KVM: Implement Stage-2 page aging
Until now, KVM/arm didn't care much for page aging (who was swapping
anyway?), and simply provided empty hooks to the core KVM code. With
server-type systems now being available, things are quite different.

This patch implements very simple support for page aging, by clearing
the Access flag in the Stage-2 page tables. On access fault, the current
fault handling will write the PTE or PMD again, putting the Access flag
back on.

It should be possible to implement a much faster handling for Access
faults, but that's left for a later patch.

With this in place, performance in VMs is degraded much more gracefully.

174178fed3 KVM: arm/arm64: add irqfd support
This patch enables irqfd on arm/arm64.

Both irqfd and resamplefd are supported. Injection is implemented
in vgic.c without routing.

This patch enables CONFIG_HAVE_KVM_EVENTFD and CONFIG_HAVE_KVM_IRQFD.

KVM_CAP_IRQFD is now advertised. KVM_CAP_IRQFD_RESAMPLE capability
automatically is advertised as soon as CONFIG_HAVE_KVM_IRQFD is set.

Irqfd injection is restricted to SPI. The rationale behind not
supporting PPI irqfd injection is that any device using a PPI would
be a private-to-the-CPU device (timer for instance), so its state
would have to be context-switched along with the VCPU and would
require in-kernel wiring anyhow. It is not a relevant use case for
irqfds.

We can definitely decide at run-time whether to use the GIC and timers
or not, and the extra code and data structures that we allocate space
for is really negligable with this config option, so I don't think it's
worth the extra complexity of always having to define stub static
inlines.  The !CONFIG_KVM_ARM_VGIC/TIMER case is pretty much an untested
code path anyway, so we're better off just getting rid of it.

04b8dc85bf arm64: KVM: Do not use pgd_index to index stage-2 pgd
The kernel's pgd_index macro is designed to index a normal, page
sized array. KVM is a bit diffferent, as we can use concatenated
pages to have a bigger address space (for example 40bit IPA with
4kB pages gives us an 8kB PGD.

In the above case, the use of pgd_index will always return an index
inside the first 4kB, which makes a guest that has memory above
0x8000000000 rather unhappy, as it spins forever in a page fault,
whist the host happilly corrupts the lower pgd.

The obvious fix is to get our own kvm_pgd_index that does the right
thing(tm).

Tested on X-Gene with a hacked kvmtool that put memory at a stupidly
high address.

A possible approach for this is to split the compound page using
split_page() at allocation time, and change the teardown path to
free one page at a time.  It turns out that alloc_pages_exact() and
free_pages_exact() does exactly that.

While we're at it, the PGD allocation code is reworked to reduce
duplication.

This has been tested on an X-Gene platform with a 4kB/48bit-VA host
kernel, and kvmtool hacked to place memory in the second page of
the hardware PGD (PUD for the host kernel). Also regression-tested
on a Cubietruck (Cortex-A7).

8b5f5a073f arm64: Don't use is_module_addr in setting page attributes
The set_memory_* functions currently only support module
addresses. The addresses are validated using is_module_addr.
That function is special though and relies on internal state
in the module subsystem to work properly. At the time of
module initialization and calling set_memory_*, it's too early
for is_module_addr to work properly so it always returns
false. Rather than be subject to the whims of the module state,
just bounds check against the module virtual address range.

af4819af8d arm64: cpuidle: add asm/proc-fns.h inclusion
ARM64 CPUidle driver requires the cpu_do_idle function so that it can
be used to enter the shallowest idle state, and it is declared in
asm/proc-fns.h.

The current ARM64 CPUidle driver does not include asm/proc-fns.h
explicitly and it has so far relied on implicit inclusion from other
header files.

Owing to some header dependencies reshuffling this currently triggers
build failures when CONFIG_ARM64_64K_PAGES=y:

9d42d48a34 arm64: compat Fix siginfo_t -> compat_siginfo_t conversion on big endian
The native (64-bit) sigval_t union contains sival_int (32-bit) and
sival_ptr (64-bit). When a compat application invokes a syscall that
takes a sigval_t value (as part of a larger structure, e.g.
compat_sys_mq_notify, compat_sys_timer_create), the compat_sigval_t
union is converted to the native sigval_t with sival_int overlapping
with either the least or the most significant half of sival_ptr,
depending on endianness. When the corresponding signal is delivered to a
compat application, on big endian the current (compat_uptr_t)sival_ptr
cast always returns 0 since sival_int corresponds to the top part of
sival_ptr. This patch fixes copy_siginfo_to_user32() so that sival_int
is copied to the compat_siginfo_t structure.

a1e50a8225 arm64: Increase the swiotlb buffer size 64MB
With commit 3690951fc6 (arm64: Use swiotlb late initialisation), the
swiotlb buffer size is limited to MAX_ORDER_NR_PAGES. However, there are
platforms with 32-bit only devices that require bounce buffering via
swiotlb. This patch changes the swiotlb initialisation to an early 64MB
memblock allocation. In order to get the swiotlb buffer correctly
allocated (via memblock_virt_alloc_low_nopanic), this patch also defines
ARCH_LOW_ADDRESS_LIMIT to the maximum physical address capable of 32-bit
DMA.

This is triggered by the use of virt_to_page() on a module address,
which ends to pointing to Nowhereland if you're lucky, or corrupt
your precious data if not.

This patch fixes the logic by mimicking what is done on arm:
- If we're patching a module and CONFIG_DEBUG_SET_MODULE_RONX is set,
  use vmalloc_to_page().
- If we're patching the kernel and CONFIG_DEBUG_RODATA is set,
  use virt_to_page().
- Otherwise, use the provided address, as we can write to it directly.

Tested on 4.0-rc1 as a KVM guest.

0eee0fbd41 arm64: crypto: increase AES interleave to 4x
This patch increases the interleave factor for parallel AES modes
to 4x. This improves performance on Cortex-A57 by ~35%. This is
due to the 3-cycle latency of AES instructions on the A57's
relatively deep pipeline (compared to Cortex-A53 where the AES
instruction latency is only 2 cycles).

At the same time, disable inline expansion of the core AES functions,
as the performance benefit of this feature is negligible.

  Measured on AMD Seattle (using tcrypt.ko mode=500 sec=1):

6910fa16db arm64: enable PTE type bit in the mask for pte_modify
Caught during Trinity testing. The pte_modify does not allow
modification for PTE type bit. This cause the test to hang
the system. It is found that the PTE can't transit from an
inaccessible page (b00) to a valid page (b11) because the mask
does not allow it. This happens when a big block of mmaped
memory is set the PROT_NONE, then the a small piece is broken
off and set to PROT_WRITE | PROT_READ cause a huge page split.

e1b6b6ce55 arm64: vdso: minor ABI fix for clock_getres
The vdso implementation of clock_getres currently returns 0 (success)
whenever a null timespec is provided by the caller, regardless of the
clock id supplied.

This behavior is incorrect.  It should fall back to syscall when an
unrecognized clock id is passed, even when the timespec argument is
null.  This ensures that clock_getres always returns an error for
invalid clock ids.

cb9e3c292d mm: vmalloc: pass additional vm_flags to __vmalloc_node_range()
For instrumenting global variables KASan will shadow memory backing memory
for modules.  So on module loading we will need to allocate memory for
shadow and map it at address in shadow that corresponds to the address
allocated in module_alloc().

__vmalloc_node_range() could be used for this purpose, except it puts a
guard hole after allocated area.  Guard hole in shadow memory should be a
problem because at some future point we might need to have a shadow memory
at address occupied by guard hole.  So we could fail to allocate shadow
for module_alloc().

Now we have VM_NO_GUARD flag disabling guard page, so we need to pass into
__vmalloc_node_range().  Add new parameter 'vm_flags' to
__vmalloc_node_range() function.

b9085bcbf5 Fairly small update, but there are some interesting new features.
Common: Optional support for adding a small amount of polling on each HLT
 instruction executed in the guest (or equivalent for other architectures).
 This can improve latency up to 50% on some scenarios (e.g. O_DSYNC writes
 or TCP_RR netperf tests).  This also has to be enabled manually for now,
 but the plan is to auto-tune this in the future.
 
 ARM/ARM64: the highlights are support for GICv3 emulation and dirty page
 tracking
 
 s390: several optimizations and bugfixes.  Also a first: a feature
 exposed by KVM (UUID and long guest name in /proc/sysinfo) before
 it is available in IBM's hypervisor! :)
 
 MIPS: Bugfixes.
 
 x86: Support for PML (page modification logging, a new feature in
 Broadwell Xeons that speeds up dirty page tracking), nested virtualization
 improvements (nested APICv---a nice optimization), usual round of emulation
 fixes.  There is also a new option to reduce latency of the TSC deadline
 timer in the guest; this needs to be tuned manually.

f56141e3e2 all arches, signal: move restart_block to struct task_struct
If an attacker can cause a controlled kernel stack overflow, overwriting
the restart block is a very juicy exploit target.  This is because the
restart_block is held in the same memory allocation as the kernel stack.

Moving the restart block to struct task_struct prevents this exploit by
making the restart_block harder to locate.

Note that there are other fields in thread_info that are also easy
targets, at least on some architectures.

It's also a decent simplification, since the restart code is more or less
identical on all architectures.

61f77eda9b mm/hugetlb: reduce arch dependent code around follow_huge_*
Currently we have many duplicates in definitions around
follow_huge_addr(), follow_huge_pmd(), and follow_huge_pud(), so this
patch tries to remove the m.  The basic idea is to put the default
implementation for these functions in mm/hugetlb.c as weak symbols
(regardless of CONFIG_ARCH_WANT_GENERAL_HUGETL B), and to implement
arch-specific code only when the arch needs it.

For follow_huge_addr(), only powerpc and ia64 have their own
implementation, and in all other architectures this function just returns
ERR_PTR(-EINVAL).  So this patch sets returning ERR_PTR(-EINVAL) as
default.

As for follow_huge_(pmd|pud)(), if (pmd|pud)_huge() is implemented to
always return 0 in your architecture (like in ia64 or sparc,) it's never
called (the callsite is optimized away) no matter how implemented it is.
So in such architectures, we don't need arch-specific implementation.

In some architecture (like mips, s390 and tile,) their current
arch-specific follow_huge_(pmd|pud)() are effectively identical with the
common code, so this patch lets these architecture use the common code.

One exception is metag, where pmd_huge() could return non-zero but it
expects follow_huge_pmd() to always return NULL.  This means that we need
arch-specific implementation which returns NULL.  This behavior looks
strange to me (because non-zero pmd_huge() implies that the architecture
supports PMD-based hugepage, so follow_huge_pmd() can/should return some
relevant value,) but that's beyond this cleanup patch, so let's keep it.

Justification of non-trivial changes:
- in s390, follow_huge_pmd() checks !MACHINE_HAS_HPAGE at first, and this
  patch removes the check. This is OK because we can assume MACHINE_HAS_HPAGE
  is true when follow_huge_pmd() can be called (note that pmd_huge() has
  the same check and always returns 0 for !MACHINE_HAS_HPAGE.)
- in s390 and mips, we use HPAGE_MASK instead of PMD_MASK as done in common
  code. This patch forces these archs use PMD_MASK, but it's OK because
  they are identical in both archs.
  In s390, both of HPAGE_SHIFT and PMD_SHIFT are 20.
  In mips, HPAGE_SHIFT is defined as (PAGE_SHIFT + PAGE_SHIFT - 3) and
  PMD_SHIFT is define as (PAGE_SHIFT + PAGE_SHIFT + PTE_ORDER - 3), but
  PTE_ORDER is always 0, so these are identical.

That laconic "some MM" is mainly the removal of remap_file_pages(),
which is a big simplification of the VM, and which gets rid of a *lot*
of random cruft and special cases because we no longer support the
non-linear mappings that it used.

From a user interface perspective, nothing has changed, because the
remap_file_pages() syscall still exists, it's just done by emulating the
old behavior by creating a lot of individual small mappings instead of
one non-linear one.

The emulation is slower than the old "native" non-linear mappings, but
nobody really uses or cares about remap_file_pages(), and simplifying
the VM is a big advantage.

f781951299 kvm: add halt_poll_ns module parameter
This patch introduces a new module parameter for the KVM module; when it
is present, KVM attempts a bit of polling on every HLT before scheduling
itself out via kvm_vcpu_block.

This parameter helps a lot for latency-bound workloads---in particular
I tested it with O_DSYNC writes with a battery-backed disk in the host.
In this case, writes are fast (because the data doesn't have to go all
the way to the platters) but they cannot be merged by either the host or
the guest.  KVM's performance here is usually around 30% of bare metal,
or 50% if you use cache=directsync or cache=writethrough (these
parameters avoid that the guest sends pointless flush requests, and
at the same time they are not slow because of the battery-backed cache).
The bad performance happens because on every halt the host CPU decides
to halt itself too.  When the interrupt comes, the vCPU thread is then
migrated to a new physical CPU, and in general the latency is horrible
because the vCPU thread has to be scheduled back in.

With this patch performance reaches 60-65% of bare metal and, more
important, 99% of what you get if you use idle=poll in the guest.  This
means that the tunable gets rid of this particular bottleneck, and more
work can be done to improve performance in the kernel or QEMU.

Of course there is some price to pay; every time an otherwise idle vCPUs
is interrupted by an interrupt, it will poll unnecessarily and thus
impose a little load on the host.  The above results were obtained with
a mostly random value of the parameter (500000), and the load was around
1.5-2.5% CPU usage on one of the host's core for each idle guest vCPU.

The patch also adds a new stat, /sys/kernel/debug/kvm/halt_successful_poll,
that can be used to tune the parameter.  It counts how many HLT
instructions received an interrupt during the polling period; each
successful poll avoids that Linux schedules the VCPU thread out and back
in, and may also avoid a likely trip to C1 and back for the physical CPU.

While the VM is idle, a Linux 4 VCPU VM halts around 10 times per second.
Of these halts, almost all are failed polls.  During the benchmark,
instead, basically all halts end within the polling period, except a more
or less constant stream of 50 per second coming from vCPUs that are not
running the benchmark.  The wasted time is thus very low.  Things may
be slightly different for Windows VMs, which have a ~10 ms timer tick.

The effect is also visible on Marcelo's recently-introduced latency
test for the TSC deadline timer.  Though of course a non-RT kernel has
awful latency bounds, the latency of the timer is around 8000-10000 clock
cycles compared to 20000-120000 without setting halt_poll_ns.  For the TSC
deadline timer, thus, the effect is both a smaller average latency and
a smaller variance.

When handling a fault in stage-2, we need to resync I$ and D$, just
to be sure we don't leave any old cache line behind.

That's very good, except that we do so using the *user* address.
Under heavy load (swapping like crazy), we may end up in a situation
where the page gets mapped in stage-2 while being unmapped from
userspace by another CPU.

At that point, the DC/IC instructions can generate a fault, which
we handle with kvm->mmu_lock held. The box quickly deadlocks, user
is unhappy.

Instead, perform this invalidation through the kernel mapping,
which is guaranteed to be present. The box is much happier, and so
am I.

to that page. Let's also assume that the host uses a cache-coherent
IO subsystem. Let's finally assume that the host is under memory
pressure and starts to swap things out.

Before this "uncached" page is evicted, we need to make sure
we invalidate potential speculated, clean cache lines that are
sitting there, or the IO subsystem is going to swap out the
cached view, loosing the data that has been written directly
into memory.

Trying to emulate the behaviour of set/way cache ops is fairly
pointless, as there are too many ways we can end-up missing stuff.
Also, there is some system caches out there that simply ignore
set/way operations.

So instead of trying to implement them, let's convert it to VA ops,
and use them as a way to re-enable the trapping of VM ops. That way,
we can detect the point when the MMU/caches are turned off, and do
a full VM flush (which is what the guest was trying to do anyway).

This allows a 32bit zImage to boot on the APM thingy, and will
probably help bootloaders in general.

6917c857e3 arm64: Avoid breakage caused by .altmacro in fpsimd save/restore macros
Alternate macro mode is not a property of a macro definition, but a
gas runtime state that alters the way macros are expanded for ever
after (until .noaltmacro is seen).

This means that subsequent assembly code that calls other macros can
break if fpsimdmacros.h is included.

Since these instruction sequences are simple (if dull -- but in a
good way), this patch solves the problem by simply expanding the
.irp loops.  The pre-existing fpsimd_{save,restore} macros weren't
rolled with .irp anyway and the sequences affected are short, so
this change restores consistency at little cost.

a1c76574f3 arm64: mm: use *_sect to check for section maps
The {pgd,pud,pmd}_bad family of macros have slightly fuzzy
cross-architecture semantics, and seem to imply a populated entry that
is not a next-level table, rather than a particular type of entry (e.g.
a section map).

In arm64 code, for those cases where we care about whether an entry is a
section mapping, we can instead use the {pud,pmd}_sect macros to
explicitly check for this case. This helps to document precisely what we
care about, making the code easier to read, and allows for future
relaxation of the *_bad macros to check for other "bad" entries.

To that end this patch updates the table dumping and initial table setup
to check for section mappings with {pud,pmd}_sect, and adds/restores
BUG_ON(*_bad((*p)) checks after we've handled the *_sect and *_none
cases so as to catch remaining "bad" cases.

In the fault handling code, show_pte is left with *_bad checks as it
only cares about whether it can walk the next level table, and this path
is used for both kernel and userspace fault handling. The former case
will be followed by a die() where we'll report the address that
triggered the fault, which can be useful context for debugging.

a3bba370c2 arm64: drop unnecessary cache+tlb maintenance
In paging_init, we call flush_cache_all, but this is backed by Set/Way
operations which may not achieve anything in the presence of cache line
migration and/or system caches. If the caches are already in an
inconsistent state at this point, there is nothing we can do (short of
flushing the entire physical address space by VA) to empty architected
and system caches. As such, flush_cache_all only serves to mask other
potential bugs. Hence, this patch removes the boot-time call to
flush_cache_all.

Immediately after the cache maintenance we flush the TLBs, but this is
also unnecessary. Before enabling the MMU, the TLBs are invalidated, and
thus are initially clean. When changing the contents of active tables
(e.g. in fixup_executable() for DEBUG_RODATA) we perform the required
TLB maintenance following the update, and therefore no additional
maintenance is required to ensure the new table entries are in effect.
Since activating the MMU we will not have modified system register
fields permitted to be cached in a TLB, and therefore do not need
maintenance for any cached system register fields. Hence, the TLB flush
is unnecessary.

Shortly after the unnecessary TLB flush, we update TTBR0 to point to an
empty zero page rather than the idmap, and flush the TLBs. This
maintenance is necessary to remove the global idmap entries from the
TLBs (as they would conflict with userspace mappings), and is retained.

af3cfdbf56 arm64: kernel: remove ARM64_CPU_SUSPEND config option
ARM64_CPU_SUSPEND config option was introduced to make code providing
context save/restore selectable only on platforms requiring power
management capabilities.

Currently ARM64_CPU_SUSPEND depends on the PM_SLEEP config option which
in turn is set by the SUSPEND config option.

The introduction of CPU_IDLE for arm64 requires that code configured
by ARM64_CPU_SUSPEND (context save/restore) should be compiled in
in order to enable the CPU idle driver to rely on CPU operations
carrying out context save/restore.

The ARM64_CPUIDLE config option (ARM64 generic idle driver) is therefore
forced to select ARM64_CPU_SUSPEND, even if there may be (ie PM_SLEEP)
failed dependencies, which is not a clean way of handling the kernel
configuration option.

For these reasons, this patch removes the ARM64_CPU_SUSPEND config option
and makes the context save/restore dependent on CPU_PM, which is selected
whenever either SUSPEND or CPU_IDLE are configured, cleaning up dependencies
in the process.

This way, code previously configured through ARM64_CPU_SUSPEND is
compiled in whenever a power management subsystem requires it to be
present in the kernel (SUSPEND || CPU_IDLE), which is the behaviour
expected on ARM64 kernels.

The cpu_suspend and cpu_init_idle CPU operations are added only if
CPU_IDLE is selected, since they are CPU_IDLE specific methods and
should be grouped and defined accordingly.

PSCI CPU operations are updated to reflect the introduced changes.

e0b2180010 arm64: dts: add baud rate to Juno stdout-path
Without explicit command-line parameters, the Juno UART ends up running
at 57600 baud in the kernel, which is at odds with the 115200 baud used
by the rest of the firmware. Since commit 7914a7c565 now lets us
fix this by specifying default options in stdout-path, do so.

33b36543df arm64: uapi: expose our struct ucontext to the uapi headers
arm64 defines its own ucontext structure which is incompatible with the
struct defined (and exposed to userspace by) the asm-generic headers.

glibc carries its own struct definition that is compatible with the
arm64 definition, but we should expose our format in the uapi headers in
case other libraries want to make use of the ucontext pushed as part of
an arm64 sigframe.

This patch moves the arm64 asm/ucontext.h to the uapi headers, along
with the necessary #include of linux/types.h.

2d888f48e0 arm64: Emulate SETEND for AArch32 tasks
Emulate deprecated 'setend' instruction for AArch32 bit tasks.

	setend [le/be] - Sets the endianness of EL0

On systems with CPUs which support mixed endian at EL0, the hardware
support for the instruction can be enabled by setting the SCTLR_EL1.SED
bit. Like the other emulated instructions it is controlled by an entry in
/proc/sys/abi/. For more information see :
	Documentation/arm64/legacy_instructions.txt

The instruction is emulated by setting/clearing the SPSR_EL1.E bit, which
will be reflected in the PSTATE.E in AArch32 context.

This patch also restores the native endianness for the execution of signal
handlers, since the process could have changed the endianness.

Note: All CPUs on the system must have mixed endian support at EL0. Once the
handler is registered, hotplugging a CPU which doesn't support mixed endian,
could lead to unexpected results/behavior in applications.

736d474f0f arm64: Consolidate hotplug notifier for instruction emulation
As of now each insn_emulation has a cpu hotplug notifier that
enables/disables the CPU feature bit for the functionality. This
patch re-arranges the code, such that there is only one notifier
that runs through the list of registered emulation hooks and runs
their corresponding set_hw_mode.

We do nothing when a CPU is dying as we will set the appropriate bits
as it comes back online based on the state of the hooks.

9f71ac961b arm64: Fix SCTLR_EL1 initialisation
We initialise the SCTLR_EL1 value by read-modify-writeback
of the desired bits, leaving the other bits (including reserved
bits(RESx)) untouched. However, sometimes the boot monitor could
leave garbage values in the RESx bits which could have different
implications. This patch makes sure that all the bits, including
the RESx bits, are set to the proper state, except for the
'endianness' control bits, EE(25) & E0E(24)- which are set early
in the el2_setup.

Updated the state of the Bit[6] in the comment to RES0 in the
comment.

aa03c428e6 arm64: Fix overlapping VA allocations
PCI IO space was intended to be 16MiB, at 32MiB below MODULES_VADDR, but
commit d1e6dc91b5 ("arm64: Add architectural support for PCI")
extended this to cover the full 32MiB. The final 8KiB of this 32MiB is
also allocated for the fixmap, allowing for potential clashes between
the two.

This change was masked by assumptions in mem_init and the page table
dumping code, which assumed the I/O space to be 16MiB long through
seaparte hard-coded definitions.

This patch changes the definition of the PCI I/O space allocation to
live in asm/memory.h, along with the other VA space allocations. As the
fixmap allocation depends on the number of fixmap entries, this is moved
below the PCI I/O space allocation. Both the fixmap and PCI I/O space
are guarded with 2MB of padding. Sites assuming the I/O space was 16MiB
are moved over use new PCI_IO_{START,END} definitions, which will keep
in sync with the size of the IO space (now restored to 16MiB).

As a useful side effect, the use of the new PCI_IO_{START,END}
definitions prevents a build issue in the dumping code due to a (now
redundant) missing include of io.h for PCI_IOBASE.

da141706ae arm64: add better page protections to arm64
Add page protections for arm64 similar to those in arm.
This is for security reasons to prevent certain classes
of exploits. The current method:

- Map all memory as either RWX or RW. We round to the nearest
  section to avoid creating page tables before everything is mapped
- Once everything is mapped, if either end of the RWX section should
  not be X, we split the PMD and remap as necessary
- When initmem is to be freed, we change the permissions back to
  RW (using stop machine if necessary to flush the TLB)
- If CONFIG_DEBUG_RODATA is set, the read only sections are set
  read only.

With all of the GICv3 code in place now we allow userland to ask the
kernel for using a virtual GICv3 in the guest.
Also we provide the necessary support for guests setting the memory
addresses for the virtual distributor and redistributors.
This requires some userland code to make use of that feature and
explicitly ask for a virtual GICv3.
Document that KVM_CREATE_IRQCHIP only works for GICv2, but is
considered legacy and using KVM_CREATE_DEVICE is preferred.

While the generation of a (virtual) inter-processor interrupt (SGI)
on a GICv2 works by writing to a MMIO register, GICv3 uses the system
register ICC_SGI1R_EL1 to trigger them.
Add a trap handler function that calls the new SGI register handler
in the GICv3 code. As ICC_SRE_EL1.SRE at this point is still always 0,
this will not trap yet, but will only be used later when all the data
structures have been initialized properly.

a0675c25d6 arm/arm64: KVM: add virtual GICv3 distributor emulation
With everything separated and prepared, we implement a model of a
GICv3 distributor and redistributors by using the existing framework
to provide handler functions for each register group.

Currently we limit the emulation to a model enforcing a single
security state, with SRE==1 (forcing system register access) and
ARE==1 (allowing more than 8 VCPUs).

We share some of the functions provided for GICv2 emulation, but take
the different ways of addressing (v)CPUs into account.
Save and restore is currently not implemented.

Similar to the split-off of the GICv2 specific code, the new emulation
code goes into a new file (vgic-v3-emul.c).

For a GICv2 there is always only one (v)CPU involved: the one that
does the access. On a GICv3 the access to a CPU redistributor is
memory-mapped, but not banked, so the (v)CPU affected is determined by
looking at the MMIO address region being accessed.
To allow passing the affected CPU into the accessors later, extend
struct kvm_exit_mmio to add an opaque private pointer parameter.
The current GICv2 emulation just does not use it.

2f5fa41a7a arm/arm64: KVM: make the value of ICC_SRE_EL1 a per-VM variable
ICC_SRE_EL1 is a system register allowing msr/mrs accesses to the
GIC CPU interface for EL1 (guests). Currently we force it to 0, but
for proper GICv3 support we have to allow guests to use it (depending
on their selected virtual GIC model).
So add ICC_SRE_EL1 to the list of saved/restored registers on a
world switch, but actually disallow a guest to change it by only
restoring a fixed, once-initialized value.
This value depends on the GIC model userland has chosen for a guest.

limited by the used GIC model. GICv3 will lift this limit, but we
still need to observe it for guests using GICv2.

Store and check the value in struct kvm_arch, but keep it down to
8 for now.

4429fc64b9 arm/arm64: KVM: rework MPIDR assignment and add accessors
The virtual MPIDR registers (containing topology information) for the
guest are currently mapped linearily to the vcpu_id. Improve this
mapping for arm64 by using three levels to not artificially limit the
number of vCPUs.
To help this, change and rename the kvm_vcpu_get_mpidr() function to
mask off the non-affinity bits in the MPIDR register.
Also add an accessor to later allow easier access to a vCPU with a
given MPIDR. Use this new accessor in the PSCI emulation.

c7c52e4829 arm64: Add dtb files to archclean rule
As dts files have been reorganised under vendor subdirs, dtb files
cannot be removed with "make distclean" now. Thus, this patch moves
dtb files under archclean rule and removes unnecessary entries.

6083fe74b7 arm64: respect mem= for EFI
When booting with EFI, we acquire the EFI memory map after parsing the
early params. This unfortuantely renders the option useless as we call
memblock_enforce_memory_limit (which uses memblock_remove_range behind
the scenes) before we've added any memblocks. We end up removing
nothing, then adding all of memory later when efi_init calls
reserve_regions.

Instead, we can log the limit and apply this later when we do the rest
of the memblock work in memblock_init, which should work regardless of
the presence of EFI. At the same time we may as well move the early
parameter into arm64's mm/init.c, close to arm64_memblock_init.

Any memory which must be mapped (e.g. for use by EFI runtime services)
must be mapped explicitly reather than relying on the linear mapping,
which may be truncated as a result of a mem= option passed on the kernel
command line.

7fe5d2b1da arm64/efi: handle potential failure to remap memory map
When remapping the UEFI memory map using ioremap_cache(), we
have to deal with potential failure. Note that, even if the
common case is for ioremap_cache() to return the existing linear
mapping of the memory map, we cannot rely on that to be always the
case, e.g., in the presence of a mem= kernel parameter.

At the same time, remove a stale comment and move the memmap code
together.

0145058c3d arm64: partially revert "ARM: 8167/1: extend the reserved memory for initrd to be page aligned"
This patch partially reverts commit 421520ba98
(only the arm64 part). There is no guarantee that the boot-loader places other
images like dtb in a different page than initrd start/end, especially when the
kernel is built with 64KB pages. When this happens, such pages must not be
freed. The free_reserved_area() already takes care of rounding up "start" and
rounding down "end" to avoid freeing partially used pages.

ddeeefe2df arm64/efi: efistub: Apply __init annotation
This ensures all stub component are freed when the kernel proper is
done booting, by prefixing the names of all ELF sections that have
the SHF_ALLOC attribute with ".init". This approach ensures that even
implicitly emitted allocated data (like initializer values and string
literals) are covered.

At the same time, remove some __init annotations in the stub that have
now become redundant, and add the __init annotation to handle_kernel_image
which will now trigger a section mismatch warning without it.

d67703a8a6 arm64: kill off the libgcc dependency
The arm64 kernel builds fine without the libgcc. Actually it should not
be used at all in the kernel. The following are the reasons indicated
by Russell King:

  Although libgcc is part of the compiler, libgcc is built with the
  expectation that it will be running in userland - it expects to link
  to a libc.  That's why you can't build libgcc without having the glibc
  headers around.

  Meanwhile, having the kernel build the compiler support functions that
  it needs ensures that (a) we know what compiler support functions are
  being used, (b) we know the implementation of those support functions
  are sane for use in the kernel, (c) we can build them with appropriate
  compiler flags for best performance, and (d) we remove an unnecessary
  dependency on the build toolchain.

056bb5f51c arm64: kvm: decode ESR_ELx.EC when reporting exceptions
To aid the developer when something triggers an unexpected exception,
decode the ESR_ELx.EC field when logging an ESR_ELx value using the
newly introduced esr_get_class_string. This doesn't tell the developer
the specifics of the exception encoded in the remaining IL and ISS bits,
but it can be helpful to distinguish between exception classes (e.g.
SError and a data abort) without having to manually decode the field,
which can be tiresome.
