9baa3c34ac PCI: Remove DEFINE_PCI_DEVICE_TABLE macro use
We should prefer `struct pci_device_id` over `DEFINE_PCI_DEVICE_TABLE` to
meet kernel coding style guidelines.  This issue was reported by checkpatch.

A simplified version of the semantic patch that makes this change is as

57a7744e09 net: Replace u64_stats_fetch_begin_bh to u64_stats_fetch_begin_irq
Replace the bh safe variant with the hard irq safe variant.

We need a hard irq safe variant to deal with netpoll transmitting
packets from hard irq context, and we need it in most if not all of
the places using the bh safe variant.

Except on 32bit uni-processor the code is exactly the same so don't
bother with a bh variant, just have a hard irq safe variant that
everyone can use.

04698ef3a0 forcedeth: Use pci_enable_msix_range() instead of pci_enable_msix()
As result of deprecation of MSI-X/MSI enablement functions
pci_enable_msix() and pci_enable_msi_block() all drivers
using these two interfaces need to be updated to use the
new pci_enable_msi_range() and pci_enable_msix_range()
interfaces.

a81ab36bf5 drivers/net: delete non-required instances of include <linux/init.h>
None of these files are actually using any __init type directives
and hence don't need to include <linux/init.h>.   Most are just a
left over from __devinit and __cpuinit removal, or simply due to
code getting copied from one driver to the next.

This covers everything under drivers/net except for wireless, which
has been submitted separately.

0ab75ae81d ethernet: Fix FSF address in file headers
Several files refer to an old address for the Free Software Foundation
in the file header comment.  Resolve by replacing the address with
the URL <http://www.gnu.org/licenses/> so that we do not have to keep
updating the header comments anytime the address changes.

86d9be263a forcedeth: run loopback test only on chipsets that support it
The driver incorrectly run loopback test on chips that don't support it.
Loopback test is only supported by chips that has DEV_HAS_TEST_EXTENDED
flag and returns 4 (NV_TEST_COUNT_EXTENDED) as test count.

827da44c61 net: Explicitly initialize u64_stats_sync structures for lockdep
In order to enable lockdep on seqcount/seqlock structures, we
must explicitly initialize any locks.

The u64_stats_sync structure, uses a seqcount, and thus we need
to introduce a u64_stats_init() function and use it to initialize
the structure.

This unfortunately adds a lot of fairly trivial initialization code
to a number of drivers. But the benefit of ensuring correctness makes
this worth while.

Because these changes are required for lockdep to be enabled, and the
changes are quite trivial, I've not yet split this patch out into 30-some
separate patches, as I figured it would be better to get the various
maintainers thoughts on how to best merge this change along with
the seqcount lockdep enablement.

Removing some boilerplate by using module_pci_driver instead of calling
register and unregister in the otherwise empty init/exit functions.

The name of the pci_driver struct had to be changed in order to prevent
a build failure.

86a9bad3ab net: vlan: add protocol argument to packet tagging functions
Add a protocol argument to the VLAN packet tagging functions. In case of HW
tagging, we need that protocol available in the ndo_start_xmit functions,
so it is stored in a new field in the skb. The new field fits into a hole
(on 64 bit) and doesn't increase the sks's size.

f646968f8f net: vlan: rename NETIF_F_HW_VLAN_* feature flags to NETIF_F_HW_VLAN_CTAG_*
Rename the hardware VLAN acceleration features to include "CTAG" to indicate
that they only support CTAGs. Follow up patches will introduce 802.1ad
server provider tagging (STAGs) and require the distinction for hardware not
supporting acclerating both.

720a43efd3 drivers:net: Remove unnecessary OOM messages after netdev_alloc_skb
Emitting netdev_alloc_skb and netdev_alloc_skb_ip_align OOM
messages is unnecessary as there is already a dump_stack
after allocation failures.

Other trivial changes around these removals:

Convert a few comparisons of pointer to 0 to !pointer.
Change flow to remove unnecessary label.
Remove now unused variable.
Hoist assignment from if.

3f0a1b58ae forcedeth: prevent TX timeouts after reboot
This complements patch "net-forcedeth: fix TX timeout caused by TX
pause on down link" which ensures that a lock-up sequence is not sent
to the NIC. Present patch ensures that if a NIC is already locked-up,
the driver will recover from it when initializing the device.

It does the equivalent of the following recovery sequence:
 - write NVREG_TX_PAUSEFRAME_ENABLE_V1 to eth1's register
   NvRegTxPauseFrame
 - write NVREG_XMITCTL_START to eth1's register
   NvRegTransmitterControl
 - write 0 to eth1's register NvRegTransmitterControl
(this is at the heart of the "unbricking" sequence mentioned in patch
 "net-forcedeth: fix TX timeout caused by TX pause on down link")

Tested:
 - hardware is MCP55 device id 10de:0373 (rev a3), dual-port
 - reboot a kernel without any of patches mentioned
 - freeze the NIC (details on description for commit "net-forcedeth:
   fix TX timeout caused by TX pause on down link")
 - wait 5mn until ping hangs & TX timeout in dmesg
 - reboot on kernel with present patch
 - host is immediatly operational, no TX timeout

1ff39eb66b forcedeth: fix TX timeout caused by TX pause on down link
On some dual-port forcedeth devices such as MCP55 10de:0373 (rev a3),
when autoneg & TX pause are enabled while port is connected but
interface is down, the NIC will eventually freeze (TX timeouts,
network unreachable).

This patch ensures that TX pause is not configured in hardware when
interface is down. The TX pause request will be honored when interface
is later configured.

Tested:
 - hardware is MCP55 device id 10de:0373 (rev a3), dual-port
 - eth0 connected and UP, eth1 connected but DOWN
 - without this patch, following sequence would brick NIC:
      ifconfig eth0 down
      ifconfig eth1 up
      ifconfig eth1 down
      ethtool -A eth1 autoneg off rx on tx off
      ifconfig eth1 up
      ifconfig eth1 down
      ethtool -A eth1 autoneg on rx on tx on
      ifconfig eth1 up
      ifconfig eth1 down
      ifup eth0
      sleep 120  # or longer
      ethtool eth1
   Just in case, sequence to un-brick:
      ifconfig eth0 down
      ethtool -A eth1 autoneg off rx on tx off
      ifconfig eth1 up
      ifconfig eth1 down
      ifup eth0
 - with this patch: no TX timeout after "bricking" sequence above

Details:
 - The following register accesses have been identified as the ones
   causing the NIC to freeze in "bricking" sequence above:
    - write NVREG_TX_PAUSEFRAME_ENABLE_V1 to eth1's register NvRegTxPauseFrame
    - write NVREG_MISC1_PAUSE_TX | NVREG_MISC1_FORCE to eth1's register NvRegMisc1
    - write 0 to eth1's register NvRegTransmitterControl
   This is what this patch avoids.

7ce5d22219 net: use eth_hw_addr_random() and reset addr_assign_type
Use eth_hw_addr_random() instead of calling random_ether_addr()
to set addr_assign_type correctly to NET_ADDR_RANDOM.

Reset the state to NET_ADDR_PERM as soon as the MAC get
changed via .ndo_set_mac_address.

505a467b66 net-forcedeth: fix possible stats inaccuracies on 32b hosts
The software stats are updated from BH, this change ensures that 32b
UP hosts use appropriate protection.

Tested:
  - HW/SW stats consistent with pktgen (in particular tx=rx)
  - HW/SW stats consistent when tx/rx offloads disabled
  - no problem with+without lockdep (SMP 16-way)

8f5f69824f forcedeth: stats updated with a deferrable timer
Mark stats timer as deferrable: punctuality in waking the stats timer
callback doesn't matter much, as it is responsible only to avoid
integer wraparound.

We need at least 1 other timer to fire within 17s (fully loaded 1Gbps)
to avoid wrap-arounds. Desired period is still 10s.

f5d827aece forcedeth: implement ndo_get_stats64() API
This commit implements the ndo_get_stats64() API for forcedeth. Since
hardware stats are being updated from different contexts (process and
timer), this commit adds synchronization. For software stats, it
relies on the u64_stats_sync.h API.

Tested:
  - 16-way SMP x86_64 ->
    RX bytes:7244556582 (7.2 GB)  TX bytes:181904254 (181.9 MB)
  - pktgen + loopback: identical rx_bytes/tx_bytes and rx_packets/tx_packets

1ec4f2d38b forcedeth: allow to silence "TX timeout" debug messages
This adds a new module parameter "debug_tx_timeout" to silence most
debug messages in case of TX timeout. These messages don't provide a
signal/noise ratio high enough for production systems and, with ~30kB
logged each time, they tend to add to a cascade effect if the system
is already under stress (memory pressure, disk, etc.).

By default, the parameter is clear, meaning that only a single warning
will be reported.

674aee3b35 forcedeth: fix stats on hardware without extended stats support
This change makes sure that tx_packets/rx_bytes ifconfig counters are
updated even on NICs that don't provide hardware support for these
stats: they are now updated in software. For the sake of consistency,
we also now have tx_bytes updated in software (hardware counters
include ethernet CRC, and software doesn't account for it).

This reverts parts of:
 - "forcedeth: statistics optimization" (21828163b2)
 - "forcedeth: Improve stats counters" (0bdfea8ba8)
 - "forcedeth: remove unneeded stats updates" (4687f3f364)

Tested:
  pktgen + loopback (http://patchwork.ozlabs.org/patch/124698/)
  reports identical tx_packets/rx_packets and tx_bytes/rx_bytes.
