On NAND flash devices, UBI reserves some physical erase blocks (PEB) for
bad block handling. Today, the number of reserved PEB can only be set as a
percentage of the total number of PEB in each MTD partition. For example, for a
NAND flash with 128KiB PEB, 2 MTD partition of 20MiB (mtd0) and 100MiB (mtd1)
and 2% reserved PEB:
- the UBI device on mtd0 will have 2 PEB reserved
- the UBI device on mtd1 will have 16 PEB reserved
The problem with this behaviour is that NAND flash manufacturers give a
minimum number of valid block (NVB) during the endurance life of the
device, e.g.:

From this number we can deduce the maximum number of bad PEB that a device will
contain during its endurance life: a 128MiB NAND flash (1024 PEB) will not have
less than 20 bad blocks during the flash endurance life.
But the manufacturer doesn't tell where those bad block will appear. He doesn't
say either if they will be equally disposed on the whole device (and I'm pretty
sure they won't). So, according to the datasheets, we should reserve the
maximum number of bad PEB for each UBI device (worst case scenario: 20 bad
blocks appears on the smallest MTD partition).
So this patch make UBI use the whole MTD device size to calculate the maximum
bad expected eraseblocks.
The Kconfig option is in per1024 blocks, thus it can have a default value of 20
which is *very* common for NAND devices.

The existing mechanism of reserving PEBs for bad PEB handling has two
flaws:
- It is calculated as a percentage of good PEBs instead of total PEBs.
- There's no limit on the amount of PEBs UBI reserves for future bad
eraseblock handling.
This patch changes the mechanism to overcome these flaws.
The desired level of PEBs reserved for bad PEB handling (beb_rsvd_level)
is set to the maximum expected bad eraseblocks (bad_peb_limit) minus the
existing number of bad eraseblocks (bad_peb_count).
The actual amount of PEBs reserved for bad PEB handling is usually set
to the desired level (but in some circumstances may be lower than the
desired level, e.g. when attaching to a device that has too few
available PEBs to satisfy the desired level).
In the case where the device has too many bad PEBs (above the expected
limit), then the desired level, and the actual amount of PEBs reserved
are set to zero. No PEBs will be set aside for future bad eraseblock
handling - even if some PEBs are made available (e.g. by shrinking a
volume).
If another PEB goes bad, and there are available PEBs, then the
eraseblock will be marked bad (consuming one available PEB). But if
there are no available PEBs, ubi will go into readonly mode.

Introduce 'ubi->bad_peb_limit', which specifies an upper limit of PEBs
UBI expects to go bad. Currently, it is initialized to a fixed percentage
of total PEBs in the UBI device (configurable via CONFIG_MTD_UBI_BEB_LIMIT).
The 'bad_peb_limit' is intended to be used for calculating the amount of PEBs
UBI needs to reserve for bad eraseblock handling.

Currently, there are several locations where an attempt to reserve more
PEBs for bad PEB handling is made, with the same code being duplicated.
Harmonize it by introducing 'ubi_update_reserved()'.
Also, improve the debug message issued, making it more descriptive.

The actual value (1%) is too low for actual NAND devices, a huge
majority of device has 2% maximum bad blocks (SLC or MLC).
(Actually it's 20 blocks on a 1024 blocks device, 40/2048...)

Commit "62f38455 UBI: modify ubi_wl_flush function to clear work queue for a lnum"
takes the 'work_sem' semaphore in write mode for the entire loop, which is not
very good because it will block other workers for potentially long time. We do
not need to have it in write mode - read mode is enough, and we do not need to
hole it over the entire loop. So this patch turns changes the locking: takes
'work_sem' in read mode and pushes it down to the loop.

Commit "aa44d1d UBI: remove Kconfig debugging option" broke UBI and it
refuses to initialize if debugfs (CONFIG_DEBUG_FS) is disabled. I incorrectly
assumed that debugfs files creation function will return success if debugfs
is disabled, but they actually return -ENODEV. This patch fixes the issue.

This patch modifies ubi_wl_flush to force the erasure of
particular volume id / logical eraseblock number pairs. Previous functionality
is preserved when passing UBI_ALL for both values. The locations where ubi_wl_flush
were called are appropriately changed: ubi_leb_erase only flushes for the
erased LEB, and ubi_create_volume forces only flushing for its volume id.
External code can call this new feature via the new function ubi_flush() added
to kapi.c, which simply passes through to ubi_wl_flush().
This was tested by disabling the call to do_work in ubi thread, which results
in the work queue remaining unless explicitly called to remove. UBIFS was
changed to call ubifs_leb_change 50 times for four different LEBs. Then the
new function was called to clear the queue: passing wrong volume ids / lnum,
correct ones, and finally UBI_ALL for both to ensure it was finally all
cleard. The work queue was dumped each time and the selective removal
of the particular LEB numbers was observed. Extra checks were enabled and
ubifs's integck was also run. Finally, the drive was repeatedly filled and
emptied to ensure that the queue was cleared normally.

This is part of a multipart patch to allow UBI to force the erasure of
particular logical eraseblock numbers. In this patch, the volume id and LEB
number are added to ubi_work data structure, and both are also passed as a
parameter to schedule erase to set it appropriately. Whenever ubi_wl_put_peb
is called, the lnum is also passed to be forwarded to schedule erase. Later,
a new ubi_sync_lnum will be added to execute immediately all work related to
that lnum.
This was tested by outputting the vol_id and lnum during the schedule of
erasure. The ubi thread was disabled and two ubifs drives on separate
partitions repeated changed a small number of LEBs. The ubi module was readded,
and all the erased LEBs, corresponding to the volumes, were added to the
schedule erase queue.

This patch adds the volume id to struct ubi_ainf_peb when scanning the LEBs at
startup. PEBs now added to the erase queue will know their original LEB number
and volume id, if available, and will be -1 otherwise (for instance, if the VID
header is unreadable).
This was tested by creating an ubi device with 3 volumes and disabiling the
ubi_thread's do_work functionality. The different ubi volumes were formatted
to ubifs and had files created and erased. The ubi modules was reloaded and
the list of LEB's added to the erased list was outputted, confirming the
volume ids and LEB numbers were appropriate.

Explicitly provide the first internal volume ID value in the comment for
UBI_INTERNAL_VOL_START. This allows developers who, when adding features
related to volume ids and observe unexpected very large volume ids, to grep
for the observed value in the source code and find out immediately that it is
expected behaviour.

Rename the constant to UBI_UNKNOWN, for the same reason that we are going
to add nother attaching method and re-use the same data structures, so the
"SCAN" in the name becomes incorrect. I've also removed the "_EC" part because

Richard removed the "dtype" hint, but few commentaries were left and this patch
removes them. I've also added a better description about the "dtype" field in
the ubi-user.h for people who may ever wonder what was that dtype thing about.

makes it emit a warning before retrying. This should allow users notice issues
earlier.

that most people produce bug reports which are difficult to understand.

Many users of debugfs copy the implementation of default_open() when
they want to support a custom read/write function op. This leads to a
proliferation of the default_open() implementation across the entire
tree.

This patch renames all MTD functions by adding a "_" prefix:
mtd->erase -> mtd->_erase
mtd->read_oob -> mtd->_read_oob
...
The reason is that we are re-working the MTD API and from now on it is
an error to use MTD function pointers directly - we have a corresponding
API call for every pointer. By adding a leading "_" we achieve the following:
1. Make sure we convert every direct pointer users
2. A leading "_" suggests that this interface is internal and it becomes
less likely that people will use them directly
3. Make sure all the out-of-tree modules stop compiling and the owners
spot the big API change and amend them.

While looking at a problem reported by UBI around the PEB moving area I
noticed that the 'MOVE_CANCEL_BITFLIPS' is a bit inconsistent name and
'MOVE_TARGET_BITFLIPS' better - let's rename it.

Remove the pre-allocated 'peb_buf2' buffer because we do not really need it.
The only reason UBI has it is to check that the data were written correctly.
But we do not have to have 2 buffers for this and waste RAM - we can just
compare CRC checksums instead. This reduces UBI memory consumption.

The 'find_wl_entry()' function expects the maximum difference as the second
argument, not the maximum absolute value. So the "unknown" eraseblock picking
was incorrect, as Shmulik Ladkani spotted. This patch fixes the issue.

The "max" parameter of 'find_wl_entry()' was documented incorrectly and
it actually means the maximum possible difference from the smallest erase
counter. Rename it to "diff" instead, and amend the documentation.

Two bad things can happen in ubi_scan():
1. If kmem_cache_create() fails we jump to out_si and call
ubi_scan_destroy_si() which calls kmem_cache_destroy().
But si->scan_leb_slab is NULL.
2. If process_eb() fails we jump to out_vidh, call
kmem_cache_destroy() and ubi_scan_destroy_si() which calls
again kmem_cache_destroy().

Currently it's possible to create a volume without a name. E.g:
ubimkvol -n 32 -s 2MiB -t static /dev/ubi0 -N ""
After that vtbl_check() will always fail because it does not permit
empty strings.

Patch ab50ff6847 broke UBI debugging messages:
before that commit when UBI debugging was enabled, users saw few useful
debugging messages after attaching an MTD device. However, that patch turned
'dbg_msg()' into 'pr_debug()', so to enable the debugging messages users have
to enable them first via /sys/kernel/debug/dynamic_debug/control, which is
very impractical.
This commit makes 'dbg_msg()' to use 'printk()' instead of 'pr_debug()', just
as it was before the breakage.

This patch is part of a patch-set which changes the MTD interface
from 'mtd->func()' form to 'mtd_func()' form. We need this because
we want to add common code to to all drivers in the mtd core level,
which is impossible with the current interface when MTD clients
call driver functions like 'read()' or 'write()' directly.
At this point we just introduce a new inline wrapper function, but
later some of them are expected to gain more code. E.g., the input
parameters check should be moved to the wrappers rather than be
duplicated at many drivers.
This particular patch introduced the 'mtd_erase()' interface. The
following patches add all the other interfaces one by one.

When we fail to erase a PEB, we free the corresponding erase entry object,
but then re-schedule this object if the error code was something like -EAGAIN.
Obviously, it is a bug to use the object after we have freed it.

When UBI debugging is disabled, we have debugging messages defined as:
if (0)
pr_debug()
But pr_debug macro defines data structures with debugging data and makes
the linux binary larger, even though we have "if (0)".

Fix up trivial conflicts in fs/xfs/linux-2.6/xfs_super.c due to the new
shrinker callout for the inode cache, that clashed with the xfs code to
start the periodic workers later.
Btrfs needs to be able to control how filemap_write_and_wait_range() is called
in fsync to make it less of a painful operation, so push down taking i_mutex and
the calling of filemap_write_and_wait() down into the ->fsync() handlers. Some
file systems can drop taking the i_mutex altogether it seems, like ext3 and
ocfs2. For correctness sake I just pushed everything down in all cases to make
sure that we keep the current behavior the same for everybody, and then each
individual fs maintainer can make up their mind about what to do from there.

This patch fixes an oops in the error path of 'ubi_attach_mtd_dev()'. If
anything after 'uif_init()' fails, we get an oops in 'cancel_pending()'. The
reason is that 'uif_close()' drops the last reference count for 'ubi->dev' and
whole 'struct ubi_device' is freed. And then
'ubi_wl_close()'->'cancel_pending()' tries to access the 'ubi' pointer and
problems begin.
Note, in 'ubi_detach_mtd_dev()' function we get a device reference to
work-around this issue. Do the same in the error path of
'ubi_attach_mtd_dev()'.

Current layout does not allow us to add inline functions to debug.h which use
the 'struct ubi_device' object, because it is undefined there. Move
'#include "debug.h"' in "ubi.h" down so to make 'struct ubi_device" be defined.
Additionally, this makes it possible to remove a bunch of forward declarations
in "debug.h". This is a preparation to the next patch.

We'll need the 'struct ubi_device *ubi' pointer in every debugging function (to
access the ->dbg field), so add this pointer to all the functions implementing
UBI debugging test modes like 'ubi_dbg_is_bitflip()' etc.

This patch introduces debugfs support to UBI. All the UBI stuff is kept in the
"ubi" debugfs directory, which contains per-UBI device "ubi/ubiX"
sub-directories, containing debugging files. This file also creates
"ubi/ubiX/chk_gen" and "ubi/ubiX/chk_io" knobs for switching general and I/O
extra checks on and off. And it removes the 'debug_chks' UBI module parameters.

There was an attempt to standartize various "__attribute__" and
other macros in order to have potentially portable and more
consistent code, see commit 82ddcb0405.
Note, that commit refers Rober Love's blog post, but the URL
is broken, the valid URL is:
http://blog.rlove.org/2005/10/with-little-help-from-your-compiler.html
Moreover, nowadays checkpatch.pl warns about using
__attribute__((packed)):
"WARNING: __packed is preferred over __attribute__((packed))"
It is not a big deal for UBI to use __packed, so let's do it.

Rename the ioctl which sets volume properties from 'UBI_IOCSETPROP' to
'UBI_IOCSETVOLPROP' to reflect the fact that this ioctl is about volume
properties, not device properties. This is also consistent with the
other volume ioctl name - 'UBI_IOCVOLUP'.
The main motivation for the re-name, however, is that we are going
to introduce the per-UBI device "set properties" ioctl, so we need
good and logical naming.
At the same time, re-name the "set volume properties request" data
structure from 'struct ubi_set_prop_req' to
'struct ubi_set_vol_prop_req'.
And re-name 'UBI_PROP_DIRECT_WRITE' to 'UBI_VOL_PROP_DIRECT_WRITE'.

This patch makes the UBI control device (/dev/ubi_ctrl) non-seekable.
The seek operation does is not applicable to this file, so it is
cleaner to explicitly return error (which the added 'no_llseek()')
does than trying to change the position (which the removed
'default_llseek()' does).
This is an API break, but the only known user of this interface is
mtd-utils which does not need the seeking functionality. And any app
which relies on this is broken, but I'm not aware of such apps.

All UBI needs is to make sure we stacktraces when UBI debugging
is enabled. It is enough to select KALLSYMS for this, KALLSYMS_ALL
is not necessary.
And the current Kconfig line we have:
select KALLSYMS_ALL if KALLSYMS && DEBUG_KERNEL
is just too complex to be sane and right. But this "if" part there
is needed to prevent "unmet direct dependency" warnings, because
KALLSYMS_ALL depends on KALLSYMS and DEBUG_KERNEL, so we cannot
just select KALLSYMS_ALL.
Anyway, this feels messy, and we do not seem to really need KALLSYMS_ALL,
so select KALLSYMS instead.

'do_sync_erase()' has to check whether we are in R/O mode before
erasing the PEB. This patch adds the check and while on it, adds an
assertion which validates the 'pnum' argument, as well as removes
a check which is always true because it has already been done
few lines before.

Similarly to the debugging checks and message, make the test modes
be dynamically selected via the "debug_tsts" module parameter or
via the "/sys/module/ubi/parameters/debug_tsts" sysfs file. This
is consistent with UBIFS as well.
And now, since all the Kconfig knobs became dynamic, we can remove
the Kconfig.debug file completely.

This patch adds a possibility to dynamically switch UBI self-checks
on and off, instead of toggling them compile-time from the configuration
menu. This is much more flexible, and consistent with UBIFS, and this
also simplifies UBI Kconfig menu and the code.
This patch introduces two levels of self-checks - general, which
includes all self-checks which are relatively fast, and I/O, which
includes write-verify checks and erase-verify checks, which are
relatively slow and involve flash I/O.

This additional little macro is used to print a bit more messages
while scanning the media. However, we have the 'dbg_bld()' macro

This patch kills the 'ubi->dbg_peb_buf' debugging buffer and the
associated mutex, because all users of this buffer are now gone.
We are killing this buffer because we are going to switch to
dynamic debugging control, just like in UBIFS, which means that
CONFIG_MTD_UBI_DEBUG_PARANOID will be removed. In this case we'd
end up always allocating 'ubi->dbg_peb_buf', which is rather large
(128KiB or more), and this would be wasteful. Thus, we are just
killing it.

Provide the LEB offset information in the UBI device information data
structure. This piece of information is required by UBIFS to find out
what are the LEB offsets which are aligned to the max. write size.
If LEB offset not aligned to max. write size, then UBIFS has to take
this into account to write more optimally.

When the read operation fails, UBI tries to re-read several times in
a hope that one of the subsequent reads may succeed. However, currently
UBI re-reads only if MTD failed to read all data, but does not re-reads
if all the data were read, but with an integrity error (-EBADMSB). This
patch makes UBI to always re-try reading.
This should be useful for reading NAND pages with unstable bits -
re-reading may help to get correct data.

During scanning UBI allocates one struct ubi_scan_leb object for each PEB,
so it can end up allocating thousands of them. Use slab cache to reduce
memory consumption for these 48-byte objects, because currently used
'kmalloc()' ends up allocating 64 bytes per object, instead of 48.

This change affects only the debugging code. Namely, use mtd->read()
function instead of ubi_io_read() to avoid bit-flips injection
(ubi_dbg_is_bitflip()) which we do not want on the debugging path.

When reading data from the flash, corrupt the buffer we are about to
read to. The idea is to fix the following possible situation:
1. The buffer contains data from previous operation, e.g., read from
another PEB previously. The data looks like expected, e.g., if we
just do not read anything and return - the caller would not
notice this. E.g., if we are reading a VID header, the buffer may
contain a valid VID header from another PEB.
2. The driver is buggy and returns use success or -EBADMSG or
-EUCLEAN, but it does not actually put any data to the buffer.
This may confuse UBI or upper layers - they may think the buffer
contains valid data while in fact it is just old data.
Thus, try to reveal such buggy MTD drivers with simple debugging
code which fills the read buffer with 0x12 constant.

Unfortunately, this commit breaks UBIFS backward compatibility and
makes new UBIFS refuse older UBIFS-formatted media:
UBIFS error: validate_sb: min. I/O unit mismatch: 8 in superblock, 64 real
Thus, we have to revert this patch and work on a better solution.

Wrong macro was used in calculating the data offset: UBI_EC_HDR_SIZE instead of
UBI_VID_HDR_SIZE. The data offset should be VID header offset + VID header size
(aligned to the minimum I/O unit).
This was not a bug only because currently UBI_EC_HDR_SIZE and UBI_VID_HDR_SIZE
have the same value of 64 bytes.

In 'nor_erase_prepare()' we want to make sure the UBI headers are
corrupted. But it is possible that one of the headers just contains
all 0xFFs, which is also OK, because UBI will erase it in case of
a power cut.

Fix up trivial conflict in drivers/mtd/Kconfig as per DavidW.
Previously we used mtd->writesize field to set UBI's minimal
I/O unit size. This sometimes caused UBIFS recovery issues
when mounting an uncleanly unmounted UBIFS partition on NOR
flash since mtd->writesize is 1 byte for NOR flash. The
MTD CFI driver however often performs writing multiple
bytes in one programming operation using the chip's write
buffer. We have to use the size of this write buffer as
a minimal I/O unit size for UBI on NOR flash to fix the
observed UBIFS recovery issues.

My new shiny code for corrupted PEB detection has NOR specific bug.
We tread PEB as corrupted and preserve it, if
1. EC header is OK.
2. VID header is corrupted.
3. data area is not "all 0xFFs"
In case of NOR we have 'nor_erase_prepare()' quirk, which invalidates
the headers before erasing the PEB. And we invalidate first the VID
header, and then the EC header. So if a power cut happens after we have
invalidated the VID header, but before we have invalidated the EC
header, we end up with a PEB which satisfies the above 3 conditions,
and the scanning code will treat it as corrupted, and will print
scary warnings, wrongly.
This patch fixes the issue by firt invalidating the EC header, then
invalidating the VID header. In case of power cut inbetween, we still
just lose the EC header, and UBI can deal with this situation gracefully.

If we get a bit-flip of ECC error while reading the data area, do not add it to
corrupted list, because it is possible that this is just unstable PEB with
corruptions caused by unclean reboots.
This patch also improves commentaries.

1, not -EINVAL; Also, the caller ('process_eb()') should not add the PEB to the

While scanning the flash we read all VID headers and store some important
information in 'struct ubi_scan_leb'. Store also the 'copy_flag' value there
as it is needed when comparing LEBs. We do not increase memory consumption
because this is just one bit and we have plenty of spare bits in
'struct ubi_scan_leb' (sizeof(struct ubi_scan_leb) is 48 both with and
without this patch).

Currently UBI erases all corrupted eraseblocks, irrespectively of the nature
of corruption: corruption due to power cuts and non-power cut corruption.
The former case is OK, but the latter is not, because UBI may destroy
potentially important data.
With this patch, during scanning, when UBI hits a PEB with corrupted VID
header, it checks whether this PEB contains only 0xFF data. If yes, it is
safe to erase this PEB and it is put to the 'erase' list. If not, this may
be important data and it is better to avoid erasing this PEB. Instead,
UBI puts it to the corr list and moves out of the pool of available PEB.
IOW, UBI preserves this PEB.
Such corrupted PEB lessen the amount of available PEBs. So the more of them
we accumulate, the less PEBs are available. The maximum amount of non-power
cut corrupted PEBs is 8.
This patch is a response to UBIFS problem where reporter
(Matthew L. Creech <mlcreech@gmail.com>) observes that UBIFS index points
to an unmapped LEB. The theory is that corresponding PEB somehow got
corrupted and UBI wiped it. This patch (actually a series of patches)
tries to make sure such PEBs are preserved - this would make it is easier
to analyze the corruption.

Start using the 'corr' list and add there PEBs which look truly corrupted,
which means they have corrupted VID header and the data which follows the
corrupted header does not contain all 0xFF bytes.
At the moment, this does not change UBI functionality much because these
PEBs will be erase when scanning finishes. But the plan is to teach UBI
preserving them.

Currently UBI maintains 2 lists of PEBs during scanning:
1. 'erase' list - PEBs which have no corruptions but should be erased
2. 'corr' list - PEBs which have some corruptions and should be erased
But we do not really need 2 lists for PEBs which should be erased after
scanning is done - this is redundant. So this patch makes sure all PEBs
which are corrupted are moved to the head of the 'erase' list. We add
them to the head to make sure they are erased first and we get rid of
corruption ASAP.
However, we do not remove the 'corr' list and realted functions, because
the plan is to use this list for other purposes. Namely, we plan to
put eraseblocks with corruption which does not look like it was caused
by unclean power cut. Then we'll preserve thes PEBs in order to avoid
killing potentially valuable user data.
This patch also amends PEBs accounting, because it was closely tight to
the 'erase'/'corr' lists separation.

Rename local variable 'ec_corr' into 'ec_err' to make the code a little bit
more readable. 'ec_err' is more appropriate because it sounds more like 'error
when EC was read' and it looks more logical because we use it together with
'err'. Just a minor nicification which should improve the rather complex
scanning code.

Currently UBI has one small flaw - when we read EC or VID header, but find only
0xFF bytes, we return UBI_IO_FF and do not report whether we had bit-flips or
not. In case of the VID header, the scanning code adds this PEB to the free list,
even though there were bit-flips.
Imagine the following situation: we start writing VID header to a PEB and have a
power cut, so the PEB becomes unstable. When we scan and read the PEB, we get
a bit-flip. Currently, UBI would just ignore this and treat the PEB as free. This
patch changes UBI behavior and now UBI will schedule this PEB for erasure.

The 'UBI_IO_PEB_EMPTY' and 'UBI_IO_PEB_FREE' are essentially the same
and mean that there are only 0xFF bytes instead of headers. Simplify
UBI a little by turning them into a single 'UBI_IO_FF' error code.
Also, stop maintaining commentaries in 'ubi_io_read_vid_hdr()' which are
almost identical to commentaries in 'ubi_io_read_ec_hdr()'.

Rename UBI_IO_BAD_HDR_READ into UBI_IO_BAD_HDR_EBADMSG which is presumably more
self-documenting and readable. Indeed, the '_READ' suffix does not tell much and
even confuses, while '_EBADMSG' tells about uncorrectable ECC error, because we
use -EBADMSG all over the place to represent ECC errors.

Cleanup the Kconfig for UBI by using menuconfig to enable/disable the entire
driver. Remove the dependency checks for MTD_UBI and MTD_UBI_DEBUG by
wrapping the options in if/endif blocks and remove any redundant checks.
Remove all default n since that is the Kconfig default. Change menu "Additional
UBI debugging messages" into a comment to remove one menu level.

All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.
The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.
New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time. Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.
The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.
Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

Kernel panic - not syncing: Fatal exception
The problem is that 'ubi_wl_scrub_peb()' does not expect that PEBs may
be in the erroneous tree, which is a bug. This patch fixes the bug
and adds corresponding check to 'ubi_wl_scrub_peb()'. Now it will simply
ignore erroneous PEBs, instead of causing an oops.

Commit 0798cea8c2 "UBI: improve corrupted flash handling"
broke delet-compatible volumes handling - it introduced a limit of 8 eraseblocks which
may be corrupted. And delete-compatible eraseblocks are added to the "corrupted" list,
so if we'd have a large delete-compatible volume, UBI would refuse it.
The fix is to add delete-compatible volumes to the erase list instead. Indeed, they are
corrupted, we just have to erase them.

list_for_each_entry uses its first argument to move from one element to the
next, so modifying it can break the iteration. The variable re1 is already
used within the loop as a temporary variable, and is not live here.

Currently, when UBI attaches an MTD device and cannot reserve all 1% (by
default) of PEBs for bad eraseblocks handling, it prints a warning. However,
Matthew L. Creech <mlcreech@gmail.com> is not very happy to see this warning,
because he did reserve enough of PEB at the beginning, but with time some
PEBs became bad. The warning is not necessary in this case.
This patch makes UBI print the warning
o if this is a new image
o of this is used image and the amount of reserved PEBs is only 10% (or less)
of the size of the reserved PEB pool.

When a delete-compatible volume is found, it is first added to the
'corr' list, which contains "corrupted" PEBs which should be erased,
and then it is added to the used volumes tree. However, the second
step should not be done. This does not cause problems in practice,
because we never access delete-compattible volumes, but it is still
not the right thing to do.

ECC errors are quite typical errors on NAND, so it is worth improving
the UBI message and print something like

This patch improves the way UBI handles corrupted flash, or flash
containing garbage or non-UBI data, which is the same from UBI POW.
Namely, we do the following:
* if 5% or more PEBs are corrupted, refuse the flash
* if less than 5% PEBs are corrupted, do not refuse the flash
and format these PEBs
* if less than 8 PEBs are corrupted, format them silently, otherwise
print a warning message.

This is just a preparation patch which introduces several
'struct ubi_scan_info' fields which count eraseblocks of different
types. This will be used later on to decide whether it is safe to

This patch introduces the %UBI_IO_BAD_HDR_READ return code for
the I/O level function. We will use this code in order to distinguish
between "corrupted header possibly because this is non-ubi data" and
"corrupted header possibly because of real data corruption and ECC error".
So far this patch does not introduce any functional change, just a
preparation.

We do not really need 2 separate error codes for indicating bad VID
and bad EC headers (UBI_IO_BAD_EC_HDR, UBI_IO_BAD_VID_HDR), it is
enough to have only one UBI_IO_BAD_HDR return code.
This patch does not introduce any functional change, only some
code simplification.

UBI can be built into the kernel or be compiled as a kernel module.
Further on the command line one can specify MTD devices to be attach to
UBI while loading. In the current implementation the UBI driver refuses
to load if one of the MTD devices cannot be attached.
Consider:
1) UBI compiled into the kernel and
2) a MTD device specified on the command line and
3) this MTD device contains bogus data (for whatever reason).
During init UBI tries to attach the MTD device is this fails the whole
UBI subsystem isn't initialized. Later the userspace cannot attach any
MTD to UBI because UBI isn't loaded.
This patch keeps the current behaviour: if UBI is compiled as a module
and a MTD device cannot be attached the UBI module cannot be loaded,
but changes it for the UBI-is-built-into-the-kernel usecase.
If UBI is builtin, a not attachable MTD device doen't stop UBI from
initializing. This slightly modifies the behaviour if multiple MTD
devices are specified on the command line. Now every MTD device is
probed and, if possible, attached, i.e. a faulty MTD device doesn't
stop the others from being attached.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Passing the attribute to the low level IO functions allows all kinds
of cleanups, by sharing low level IO code without requiring
an own function for every piece of data.
Also drivers can extend the attributes with own data fields
and use that in the low level function.
This makes the class attributes the same as sysdev_class attributes
and plain attributes.
This will allow further cleanups in drivers.

Add an extra debugging check function which validates writes.
After every write it reads the data back, compares it with the
original data, and complains if they mismatch.
Useful for debugging. No-op if extra debugging checks are disabled.

UBI debugging functions were a little bit over-engineered and
returned more error codes than needed, and the callers had to
do useless checks. Simplify the return codes.
Impact: only debugging code is affected, which means that for
non-developers this is a no-op patch.

In the error path of 'ubi_attach_mtd_dev()' we have a tricky situation:
we have to release things differently depending on at which point
the failure happening. Namely, if @ubi->dev is not initialized, we have
to free everything ourselves. But if it was, we should not free the @ubi
object, because it will be freed in the 'dev_release()' function. And
we did not get this situation right.
This patch introduces additional argument to the 'uif_init()' function.
On exit, this argument indicates whether the final 'free(ubi)' will
happen in 'dev_release()' or not. So the caller always knows how to
properly release the resources.
Impact: all memory is now correctly released when UBI fails to attach
an MTD device.

The @mtd_devs and @mtd_dev_param variables are used only during the
initialization, and all functions that use the variables have
the __init prefix. This means we can safely mark the variables
as __initdata, which is a tiny optimization.
Impact: tiny RAM consumption optimization when UBI is used as a kernel
module.

When truncating an UBI volume, UBI should allocates a PEB-sized

The in kernel copy of a volume's update marker is not initialised from the
volume table. This means that volumes where an update was unfinnished will
not be treated as "forbidden to use". This is basically that the update
functionality was broken.

ubiupdatevol -t does the following:
- ubi_start_update()
- set_update_marker()
- for all LEBs ubi_eba_unmap_leb()
- clear_update_marker()
- ubi_wl_flush()
ubi_wl_flush() physically erases all PEB, once it returns all PEBs are
empty. clear_update_marker() has the update marker written after return.
If there is a power cut between the last two functions then the UBI
volume has no longer the "update" marker set and may have some valid
LEBs while some of them may be gone.
If that volume in question happens to be a UBIFS volume, then mount
will fail with

The patch waits for the wl worker to finish prior clearing the "update"
marker on flash. The two new LEB which are scheduled for erasing after
clear_update_marker() should not matter because they are only visible to
UBI.

Add an 'ubi_open_volume_path(path, mode)' function which works like
'open_bdev_exclusive(path, mode, ...)' where path is the special file
representing the UBI volume, typically /dev/ubi0_0.
This is needed to teach UBIFS being able to mount UBI character devices.

Commit 32bc482028 did not fully fix
the backward compatibility issues. We still fail to properly handle
situations when the first PEB contains non-zero image sequence
number, but one of the following PEBs contains zero image sequence
number. For example, this may happen if we mount a new image with
an old kernel, and then try to mount it in the new kernel.
This patch should fix the issue.

More testing of NOR flash against power cuts showed that sometimes
eraseblocks may be unwritable, and we cannot really invalidate
them before erasure. But in this case the eraseblock probably
contains garbage anyway, and we do not have to invalidate the
headers. This assumption might be not true, but this is at least
what I have observed. So if we cannot invalidate the headers,
we make sure that the PEB does not contain valid VID header.
If this is true, everything is fine, otherwise we panic.
Useful for debugging problems, compiled in only if UBI debugging
is enabled. This patch also makes the UBI writing function dump
the flash if it fails to write.

There was a bug report recently where UBI prints:
UBI error: ubi_attach_mtd_dev: failed to attach by scanning, error -22
error messages and refuses to attach a PEB. It turned out to be a
buggy flash driver which returned garbage to almost every UBI read.
This patch makes UBI print a better message in such cases. Namely,
if UBI finds 8 or more corrupted PEBs, it prints a warning and
lists the corrupted PEBs.

In case of NOR flash, UBI zeroes EC and VID headers' magic,
in order to detect interrupted erasures. It first zeroes out
the EC magic, then VID magic. However, if a power cut happens
in between, we'll end up with a corrupted EC header and a valid
VID header, in which case UBI accepts the PEB, but prints a
warning. This patch makes sure we first zero out the VID
magic, then the EC magic, not vice versa. This is just a
small amendment to prevent warning messages.

If the EC header is corrupted, but the VID header is OK, UBI accepts the
PEB and treats it as "used". However, generally this should not happen.
Print a warning if this happens.

If we fail in 'ubi_eba_init_scan()', we free
'ubi->volumes[i]->eba_tbl' in there, but also later free it
in 'free_internal_volumes()'. Fix this by assigning NULL
to 'ubi->volumes[i]->eba_tbl' after it is freed.

This commit fixes NOR flash recovery issues observed with Spansion
S29GL512N NOR.
When NOR erases, it first fills PEBs with zeroes, then sets all bytes
to 0xFF. Filling with zeroes starts from the end of the PEB. And when
power is cut, this results in PEBs containing correct EC and VID headers
but corrupted with zeros at the end. This confuses UBI and it mistakinly
accepts these PEBs and associate them with LEBs.

Move the image seq. number handling from I/O level to the scanning
lever, where it really belongs to. Move the @image_seq_set variable
to the @struct ubi_scan_info structure, which exists only during
scanning.

An image sequence number is added to the UBI erase-counter header
to be able determine if the root file system contains a mixture
of old and new images (because the flashing failed to complete).
A change to nolo is also needed for this to take effect.

The 'paranoid_check_empty()' is bogus because, which is easilly
seen on NOR flash, which has long erase cycles, and which may
easilly end-up with half-erased eraseblocks. In this case the
paranoid check fails. I is just wrong to assume that PEBs which
do not have EC headers always contain all 0xFF. Such assumption
should not be made on the I/O level, which is quite low.
Thus, just kill the check.

The UBIFS WL worker may encounter read errors and there is logic
which makes a decision whether we should do one of:
1. cancel the operation and move the PEB with the read errors to
the 'erroneous' list;
2. switch to R/O mode.
ATM, only -EIO errors trigger 1., other errors trigger 2. The idea
is that if we know we encountered an I/O error, do 1. Otherwise,
we do not know how to react, and do 2., just in case. E.g., if
the underlying driver became crazy because of a bug, we do not
want to harm any data, and switch to R/O mode.
This patch does 2 things:
1. Makes sure reads from the source PEB always cause 1. This is
more consistent with other reads which come from the upper
layers and never cause R/O.
2. Teaches UBI to do 1. also on -EBADMSG, UBI_IO_BAD_VID_HDR,
-ENOMEM, and -ETIMEOUT. But this is only when reading the
target PEB.
This preblems were hunted by Adrian Hunter.

UBI volume notifications are intended to create the API to get clients
notified about volume creation/deletion, renaming and re-sizing. A
client can subscribe to these notifications using 'ubi_volume_register()'
and cancel the subscription using 'ubi_volume_unregister()'. When UBI
volumes change, a blocking notifier is called. Clients also can request
"added" events on all volumes that existed before client subscribed
to the notifications.
If we use notifications instead of calling functions like 'ubi_gluebi_xxx()',
we can make the MTD emulation layer to be more flexible: build it as a
separate module and load/unload it on demand.

This patch improves UBI errors handling. ATM UBI switches to
R/O mode when the WL worker fails to read the source PEB.
This means that the upper layers (e.g., UBIFS) has no
chances to unmap the erroneous PEB and fix the error.
This patch changes this behaviour and makes UBI put PEBs
like this into a separate RB-tree, thus preventing the
WL worker from hitting the same read errors again and
again.
But there is a 10% limit on a maximum amount of PEBs like this.
If there are too much of them, UBI switches to R/O mode.
Additionally, this patch teaches UBI not to panic and
switch to R/O mode if after a PEB has been copied, the
target LEB cannot be read back. Instead, now UBI cancels
the operation and schedules the target PEB for torturing.
The error paths has been tested by ingecting errors
into 'ubi_eba_copy_leb()'.

This patch fixes the error path in the WL worker - in same cases
UBI oopses when 'goto out_error' happens and e1 or e2 are NULL.
This patch also cleans up the error paths a little. And I have
tested nearly all error paths in the WL worker.

This patch fixes a minor problem where we may fail to wake
upe the UBI background thread. This is not fatal at all,
it may just result at sligtly worse performace for a short
period of time, just because the thread will be woken up
when real I/O on the UBI starts.
Anywey, the issue is the race condition between
'ubi_attach_mtd_dev()' and 'ubi_thread()'. If we do not
serialize them, the 'wake_up_process()' call may be done
before 'ubi_thread()' went seep, but after it checked
'ubi->thread_enabled'.

The @vol->upd_marker should be protected by the @ubi->device_mutex,
otherwise 'paranoid_check_volume()' complains sometimes because
vol->upd_marker is 1 while vtbl_rec->upd_marker is 0.

If a volume paranoid check fails, do not return an error
code to the caller, but just print error messages and go
forward. The primary reason for this is that it is difficult
to recover and cancel the operation at that stage.

When paranoid checs are enabled, the 'io_paral' test from the
'mtd-utils' package fails. The symptoms are:
UBI error: paranoid_check_all_ff: flash region at PEB 3973:512, length 15872 does not contain all 0xFF bytes
UBI error: paranoid_check_all_ff: paranoid check failed for PEB 3973
UBI: hex dump of the 512-16384 region
It turned out to be a bug in the checking function. Suppose there
are 2 tasks - A and B. Task A is the wear-levelling working
('wear_leveling_worker()'). It is reading the VID header to find
which LEB this PEB belongs to. Say, task A is reading header
of PEB X. Suppose PEB X is unmapped, and has no VID header.
Task B is trying to write to PEB X.
Task A: in 'ubi_io_read_vid_hdr()': reads the VID header from PEB X.
The read data contain all 0xFF bytes.
Task B: writes VID header and some data to PEB X
Task A: assumes PEB X is empty, calls 'paranoid_check_all_ff()', which
fails.
The solution for this problem is to make 'paranoid_check_all_ff()'
re-read the VID header, re-check it, and only if it is not there,
check the rest. This now implemented by the 'paranoid_check_empty()'
function.

The @mult_mutex does not serve any purpose. We already have
@volumes_mutex and it is enough. The @volume mutex is pushed
down to the 'ubi_rename_volumes()', because we want first
to open all volumes in the exclusive mode, and then lock the
mutex, just like all other ioctl's (remove, re-size, etc) do.

GregKH asked to fix UBI which has fake device release method. Indeed,
we have to free UBI device description object from the release method,
because otherwise we'll oops is someone opens a UBI device sysfs file,
then the device is removed, and he reads the file. With this fix, he
will get -ENODEV instead of an oops.

UBI ioctl's do not work when running 64-bit kernel and 32-bit
user-land. Fix this by adding the compat_ioctl method.
Also, UBI serializes all ioctls, so more than one ioctl at a time
is not a problem. Amd UBI does not seem to depend on anything else,
so use unlocked_ioctl instead of ioctl (no BKL needed).

Some ioctl's in UBI are enabled only when debugging is switched
on. There is not particular reason for this, just noone needed
them. However, some people need the now for their user-space
development. Thus, allow these ioctl's even if UBI debugging
is disabled.

UBI has 2 RB-trees to implement PEB protection, which is too
much for simply prevent PEB from being moved for some time.
This patch implements this using lists. The benefits:
1. No need to allocate protection entry on each PEB get.
2. No need to maintain balanced trees and walk them.

MTD internal API presently uses 32-bit values to represent
device size. This patch updates them to 64-bits but leaves
the external API unchanged. Extending the external API
is a separate issue for several reasons. First, no one
needs it at the moment. Secondly, whether the implementation
is done with IOCTLs, sysfs or both is still debated. Thirdly
external API changes require the internal API to be accepted
first.
Note that although the MTD API will be able to support 64-bit
device sizes, existing drivers do not and are not required
to do so, although NAND base has been updated.
In general, changing from 32-bit to 64-bit values cause little
or no changes to the majority of the code with the following
exceptions:
- printk message formats
- division and modulus of 64-bit values
- NAND base support
- 32-bit local variables used by mtdpart and mtdconcat
- naughtily assuming one structure maps to another
in MEMERASE ioctl

When a PEB is moved and a write error happens, UBI switches
to R/O mode, which is wrong, because we just copy the data
and may select a different PEB and re-try this. This patch
fixes WL worker's behavior.

We cannot call 'ubi_wl_get_peb()' with @ubi->buf_mutex locked,
because 'ubi_wl_get_peb()' may force erasure, which, in turn,
may call 'torture_peb()' which also locks the @ubi->buf_mutex
and deadlocks.

The 'ubi_io_read_vid_hdr()' and 'ubi_io_read_ec_hdr()' function
have the 'verbose' argument which controls whether they should
print a warning if the VID/EC header was not found or was corrupted.
Some callers require the headers to be OK, and pass 1. Some allow
a corrupted/not present header, and pass 0.

And UBI_IO_DEBUG is 1 if CONFIG_MTD_UBI_DEBUG_MSG_BLD is true. So in
this case the warning is printed all the time. This confuses people.
Thus, do not print the messages as warnings if UBI_IO_DEBUG is true,
but print them as debugging messages instead.

'ubi_io_read_data()' may return EBADMSG in case of an ECC error,
and we should not panic because of this. We have CRC32 checksum
and may check the data. So just ignore the EBADMSG error.
This patch also fixes a minor spelling error at the same time.

In case of error, the function add_volume returns an ERR pointer. The
result of IS_ERR, which is supposed to be used in a test as it is, is
here checked to be less than zero, which seems odd. We suggest to
replace this test by a simple IS_ERR test.

start it anyway, because otherwise we see tonns of kernel debugging

Before UBI got into mainline, there was a slight flash format
change - we did not have sequence number support, then added it.
We have carried full support of those ancient images till this
moment. Now the support is removed, well, not fully removed.
Now UBI will support only _clean_ old images, which were cleanly
detached last time (just before kernel upgrade). This is most
likely the case.
But we will not support unclean ancient images. Surprisingly,
this allows us to remove a big chunk of legacy code.
And the same should be true for downgrading: clean images should
downgrade fine, but unclean ones will not.

If bit-flips happen often, UBI prints to many messages. Lessen
the amount by only printing the messages when the PEB has been
scrubbed. Also, print torturing messages.

leb_read_unlock() may be called simultaniously by several tasks.
The would race at the following code:
up_read(&le->mutex);
if (free)
kfree(le);
And it is possible that one task frees 'le' before the other tasks
do 'up_read()'. Fix this by doing up_read and free inside the
'ubi->ltree' lock. Below it the oops we had because of this:

Normally UBI volumes are freed in the release function of
the struct device object. However, on error path they may
have to be freed before the struct device objects have been
initialized.

ubi_free_volume() function sets ubi->volumes[] to NULL, so
ubi_eba_close() is useless, it does not free what has to be freed.
So zap it and free vol->eba_tbl at the volume release function.

Instead of correctly pad the buffer wich we are writing to the
eraseblock during update, we used weird construct:
memset(buf + len, 0xFF, len - len);
Fix this.

Print information about logicale eraseblock size, sub-page
size and so on at early stage, befor an attempt to attach
the MTD device was made. This is more convenient to do so
because the attempt to attach may fail, and the information
is never printed then.

(a + b) / (c + d) != a / c + b / d. The old code errornously
assumed this incorrect formuld. Instead, just sum all erase
counters in a 64-bit variable and divide to the number of EBs
at the end.

The new trend in linux is not to store headers which define
on-media format in the include/ directory, but instead, store
them locally. This is because these headers "do not define any
kernel<->userspace interface".
Do so for UBI as well.

In C, signed 1-bit bitfields can only take the values 0 and -1, only 0 and 1
are ever assigned in current code. Make them unsigned bitfields.

The problem: NAND flashes have different amount of initial bad physical
eraseblocks (marked as bad by the manufacturer). For example, for 256MiB
Samsung OneNAND flash there might be from 0 to 40 bad initial eraseblocks,
which is about 2%. When UBI is used as the base system, one needs to know
the exact amount of good physical eraseblocks, because this number is
needed to create the UBI image which is put to the devices during
production. But this number is not know, which forces us to use the
minimum number of good physical eraseblocks. And UBI additionally
reserves some percentage of physical eraseblocks for bad block handling
(default is 1%), so we have 1-3% of PEBs reserved at the end, depending
on the amount of initial bad PEBs. But it is desired to always have
1% (or more, depending on the configuration).
Solution: this patch adds an "auto-resize" flag to the volume table.
The volume which has the "auto-resize" flag will automatically be re-sized
(enlarged) on the first UBI initialization. UBI clears the flag when
the volume is re-sized. Only one volume may have the "auto-resize" flag.
So, the production UBI image may have one volume with "auto-resize"
flag set, and its size is automatically adjusted on the first boot
of the device.

This slab cache is not really needed since the number of objects
is low and the constructor does not make much sense because we
allocate oblects when doint I/O, which is way slower then allocation.

When creating a new volume, do not forget to increment the
vol_count variable.
Also, users are not interested in internal volumes, so do not show
them in the volumes_count sysfs file.

UBI allows to specify MTD device name or number when the module is being
loaded. When parsing MTD device identity string, it first tries to treat
it as device NAME, and if that fails, it treats it as device number.
Make it vice-versa as this is more logical and makes less troubles when
you have an MTD device named "1" and try to load mtd1 which has different
name. This is especially easy to hit when gluebi is enabled.

because we cammot really use volumes_mutex - it cases reverse
locking problems with mtd_tbl_mutex when gluebi is used -
thanks to lockdep.

This is one more step on the way to "removable" UBI devices. It
adds reference counting for UBI devices. Every time a volume on
this device is opened - the device's refcount is increased. It
is also increased if someone is reading any sysfs file of this
UBI device or of one of its volumes.

This patch is a preparation to make UBI devices dynamic. It
adds an UBI control device which has dynamically allocated
major number and registers itself as "ubi_ctrl". It does not
do anything so far. The idea is that this device will allow
to attach/detach MTD devices from userspace.
This is symilar to what the Linux device mapper has.
The next things to do are:
* Fix UBI, because it now assumes UBI devices cannot go away
* Implement control device ioctls which will attach/detach MTD
devices

The flush function should finish all the pending jobs. But if
somebody else is doing a work, this function should wait and let
it finish.
This patche uses rw semaphore for synchronization purpose - it
just looks quite convinient.

When the WL worker is moving an LEB, the volume might go away
occasionally. UBI does not handle these situations correctly.
This patch introduces a new mutex which serializes wear-levelling
worker and the the 'ubi_wl_put_peb()' function. Now, if one puts
an LEB, and its PEB is being moved, it will wait on the mutex.
And because we unmap all LEBs when removing volumes, this will make
the volume remove function to wait while the LEB movement
finishes.
Below is an example of an oops which should be fixed by this patch:

Add ref_count field to UBI volumes and remove weired "vol->removed"
field. This way things are better understandable and we do not have

If we fail halfway through sysfs file creation, we may just call
sysfs remove function and it will delete all the files we created.
For non-existing files it will also be OK - the remove functions
just return -ENOENT.

Error path in volume creation is bogus. First of, it ovverrides the
'err' variable and returns zero to the caller. Second, ubi_assert()

When a volume is opened, get its kref via get_device() call.
And put the reference when closing the volume. With this, we
may have a bit saner volume delete.
