758bca385a atm: idt77252: fix null-ptr-dereference

this one is similar to the phy_data allocation fix in uPD98402, the
driver allocate the idt77105_priv and store to dev_data but later
dereference using dev->dev_data, which will cause null-ptr-dereference.

fix this issue by changing dev_data to phy_data so that PRIV(dev) can
work correctly.

f35954a396 atm: uPD98402: fix incorrect allocation

dev->dev_data is set in zatm.c, calling zatm_start() will overwrite this
dev->dev_data in uPD98402_start() and a subsequent PRIV(dev)->lock
(i.e dev->phy_data->lock) will result in a null-ptr-dereference.

I believe this is a typo and what it actually want to do is to allocate
phy_data instead of dev_data.

f8f6190094 atm: lanai: dont run lanai_dev_close if not open

lanai_dev_open() can fail. When it fail, lanai->base is unmapped and the
pci device is disabled. The caller, lanai_init_one(), then tries to run
atm_dev_deregister(). This will subsequently call lanai_dev_close() and
use the already released MMIO area.

To fix this issue, set the lanai->base to NULL if open fail,
and test the flag in lanai_dev_close().

6f6e459475 atm: eni: dont release is never initialized

label err_eni_release is reachable when eni_start() fail.
In eni_start() it calls dev->phy->start() in the last step, if start()
fail we don't need to call phy->stop(), if start() is never called, we
neither need to call phy->stop(), otherwise null-ptr-deref will happen.

In order to fix this issue, don't call phy->stop() in label err_eni_release

6dceaa9f56 atm: nicstar: Unmap DMA on send error
The `skb' is mapped for DMA in ns_send() but does not unmap DMA in case
push_scqe() fails to submit the `skb'. The memory of the `skb' is
released so only the DMA mapping is leaking.

Unmap the DMA mapping in case push_scqe() failed.

7028977965 atm: atmtcp: Constify atmtcp_v_dev_ops
The only usage of atmtcp_v_dev_ops is to pass its address to
atm_dev_register() which takes a pointer to const, and comparing its
address to another address, which does not modify it. Make it const to
allow the compiler to put it in read-only memory.

In queue_skb(), skb->data is mapped to streaming DMA on line 850:
  dma_map_single(..., skb->data, ...);

Then skb->data is accessed on lines 862 and 863:

These accesses may cause data inconsistency between CPU cache and

51875dad43 atm: fix atm_dev refcnt leaks in atmtcp_remove_persistent
atmtcp_remove_persistent() invokes atm_dev_lookup(), which returns a
reference of atm_dev with increased refcount or NULL if fails.

The refcount leaks issues occur in two error handling paths. If
dev_data->persist is zero or PRIV(dev)->vcc isn't NULL, the function
returns 0 without decreasing the refcount kept by a local variable,
resulting in refcount leaks.

Fix the issue by adding atm_dev_put() before returning 0 both when
dev_data->persist is zero or PRIV(dev)->vcc isn't NULL.

3f649ab728 treewide: Remove uninitialized_var() usage
Using uninitialized_var() is dangerous as it papers over real bugs[1]
(or can in the future), and suppresses unrelated compiler warnings
(e.g. "unused variable"). If the compiler thinks it is uninitialized,
either simply initialize the variable or make compiler changes.

In preparation for removing[2] the[3] macro[4], remove all remaining
needless uses with the following script:

decreasing, but there are still more than 2400 instances.

This commit finishes the conversion. While I touched the lines,
I also fixed the indentation.

There are a variety of indentation styles found.

fa865ba183 firestream: fix memory leaks
In fs_open(), 'vcc' is allocated through kmalloc() and assigned to
'atm_vcc->dev_data.' In the following execution, if an error occurs, e.g.,
there is no more free channel, an error code EBUSY or ENOMEM will be
returned. However, 'vcc' is not deallocated, leading to memory leaks. Note
that, in normal cases where fs_open() returns 0, 'vcc' will be deallocated
in fs_close(). But, if fs_open() fails, there is no guarantee that
fs_close() will be invoked.

To fix this issue, deallocate 'vcc' before the error code is returned.
