Pull namespace updates from Eric Biederman:
 "Life has been busy and I have not gotten half as much done this round
  as I would have liked. I delayed it so that a minor conflict
  resolution with the mips tree could spend a little time in linux-next
  before I sent this pull request.

  This includes two long delayed user namespace changes from Kirill
  Tkhai. It also includes a very useful change from Serge Hallyn that
  allows the security capability attribute to be used inside of user
  namespaces. The practical effect of this is people can now untar
  tarballs and install rpms in user namespaces. It had been suggested to
  generalize this and encode some of the namespace information
  information in the xattr name. Upon close inspection that makes the
  things that should be hard easy and the things that should be easy
  more expensive.

  Then there is my bugfix/cleanup for signal injection that removes the
  magic encoding of the siginfo union member from the kernel internal
  si_code. The mips folks reported the case where I had used FPE_FIXME
  me is impossible so I have remove FPE_FIXME from mips, while at the
  same time including a return statement in that case to keep gcc from
  complaining about unitialized variables.

  I almost finished the work to get make copy_siginfo_to_user a trivial
  copy to user. The code is available at:

  But I did not have time/energy to get the code posted and reviewed
  before the merge window opened.

  I was able to see that the security excuse for just copying fields
  that we know are initialized doesn't work in practice there are buggy
  initializations that don't initialize the proper fields in siginfo. So
  we still sometimes copy unitialized data to userspace"

Slightly more changes than usual this time:
 
 	- KDump Kernel IOMMU take-over code for AMD IOMMU. The code now
 	  tries to preserve the mappings of the kernel so that master
 	  aborts for devices are avoided. Master aborts cause some
 	  devices to fail in the kdump kernel, so this code makes the
 	  dump more likely to succeed when AMD IOMMU is enabled.

 	  code is still optional, but AMD and Intel IOMMU drivers had

60f07c8ec5 s390/mm: fix race on mm->context.flush_mm
The order in __tlb_flush_mm_lazy is to flush TLB first and then clear
the mm->context.flush_mm bit. This can lead to missed flushes as the
bit can be set anytime, the order needs to be the other way aronud.

But this leads to a different race, __tlb_flush_mm_lazy may be called
on two CPUs concurrently. If mm->context.flush_mm is cleared first then
another CPU can bypass __tlb_flush_mm_lazy although the first CPU has
not done the flush yet. In a virtualized environment the time until the
flush is finally completed can be arbitrarily long.

Add a spinlock to serialize __tlb_flush_mm_lazy and use the function
in finish_arch_post_lock_switch as well.

b3e5dc45fd s390/mm: fix local TLB flushing vs. detach of an mm address space
The local TLB flushing code keeps an additional mask in the mm.context,
the cpu_attach_mask. At the time a global flush of an address space is
done the cpu_attach_mask is copied to the mm_cpumask in order to avoid
future global flushes in case the mm is used by a single CPU only after
the flush.

Trouble is that the reset of the mm_cpumask is racy against the detach
of an mm address space by switch_mm. The current order is first the
global TLB flush and then the copy of the cpu_attach_mask to the
mm_cpumask. The order needs to be the other way around.

e7fc5146cf s390/zcrypt: externalize test AP queue
Under certain specified conditions, the Test AP Queue (TAPQ)
subfunction of the Process Adjunct Processor Queue (PQAP) instruction
will be intercepted by a guest VM. The guest VM must have a means for
executing the intercepted instruction.

The vfio_ap driver will provide an interface to execute the
PQAP(TAPQ) instruction subfunction on behalf of a guest VM.
The code for executing the AP instructions currently resides in the
AP bus. This patch refactors the AP bus code to externalize access
to the PQAP(TAPQ) instruction subfunction to make it available to
the vfio_ap driver.

A 31-bit compat process can force a BUG_ON in crst_table_upgrade
with specific, invalid mmap calls, e.g.

fa41ba0d08 s390/mm: avoid empty zero pages for KVM guests to avoid postcopy hangs
Right now there is a potential hang situation for postcopy migrations,
if the guest is enabling storage keys on the target system during the
postcopy process.

For storage key virtualization, we have to forbid the empty zero page as
the storage key is a property of the physical page frame.  As we enable
storage key handling lazily we then drop all mappings for empty zero
pages for lazy refaulting later on.

This does not work with the postcopy migration, which relies on the
empty zero page never triggering a fault again in the future. The reason
is that postcopy migration will simply read a page on the target system
if that page is a known zero page to fault in an empty zero page.  At
the same time postcopy remembers that this page was already transferred
- so any future userfault on that page will NOT be retransmitted again
to avoid races.

If now the guest enters the storage key mode while in postcopy, we will
break this assumption of postcopy.

The solution is to disable the empty zero page for KVM guests early on
and not during storage key enablement. With this change, the postcopy
migration process is guaranteed to start after no zero pages are left.

As guest pages are very likely not empty zero pages anyway the memory
overhead is also pretty small.

While at it this also adds proper page table locking to the zero page
removal.

28b841b3a7 s390/dasd: Add discard support for FBA devices
The z/VM hypervisor provides virtual disks (VDISK) which are backed by
main memory of the hypervisor. Those devices are seen as DASD FBA disks
within the Linux guest.

Whenever data is written to such a device, memory is allocated
on-the-fly by z/VM accordingly. This memory, however, is not being freed
if data on the device is deleted by the guest OS.

In order to make memory usable after deletion again, add discard support
to the FBA discipline.

While at it, update comments regarding the DASD_FEATURE_* flags.

d66bf801e0 s390/uaccess: avoid mvcos jump label
If the kernel is compiled for z10 or later machines the uaccess
code inlines the mvcos instruction. The facility bit 27 which
indicates the availability of MVCOS has to be set. The have_mvcos
jump label will always be true.

Make the generation of the have_mvcos jump label conditional on
!CONFIG_HAVE_MARCH_Z10_FEATURES.

1bab1c02af KVM: s390: expose no-DAT to guest and migration support
The STFLE bit 147 indicates whether the ESSA no-DAT operation code is
valid, the bit is not normally provided to the host; the host is
instead provided with an SCLP bit that indicates whether guests can
support the feature.

This patch:
* enables the STFLE bit in the guest if the corresponding SCLP bit is
  present in the host.
* adds support for migrating the no-DAT bit in the PGSTEs
* fixes the software interpretation of the ESSA instruction that is
  used when migrating, both for the new operation code and for the old
  "set stable", as per specifications.

631aebfee8 KVM: s390: sthyi: remove invalid guest write access
handle_sthyi() always writes to guest memory if the sthyi function
code is zero in order to fault in the page that later is written to.

However a function code of zero does not necessarily mean that a write
to guest memory happens: if the KVM host is running as a second level
guest under z/VM 6.2 the sthyi instruction is indicated to be
available to the KVM host, however if the instruction is executed it
will always return with a return code that indicates "unsupported
function code".

In such a case handle_sthyi() must not write to guest memory. This
means that the prior write access to fault in the guest page may
result in invalid guest exceptions, and/or invalid data modification.

In order to be architecture compliant simply remove the write_guest()
call.

Given that the guest assumed a write access anyway, this fix does not
qualify for -stable. This just makes sure the sthyi handler is
architecture compliant.

8fa1696ea7 KVM: s390: Multiple Epoch Facility support
Allow for the enablement of MEF and the support for the extended
epoch in SIE and VSIE for the extended guest TOD-Clock.

A new interface is used for getting/setting a guest's extended TOD-Clock
that uses a single ioctl invocation, KVM_S390_VM_TOD_EXT.  Since the
host time is a moving target that might see an epoch switch or STP sync
checks we need an atomic ioctl and cannot use the exisiting two
interfaces. The old method of getting and setting the guest TOD-Clock is
still retained and is used when the old ioctls are called.

30d6e0a419 futex: Remove duplicated code and fix undefined behaviour
There is code duplicated over all architecture's headers for
futex_atomic_op_inuser. Namely op decoding, access_ok check for uaddr,
and comparison of the result.

Remove this duplication and leave up to the arches only the needed
assembly which is now in arch_futex_atomic_op_inuser.

This effectively distributes the Will Deacon's arm64 fix for undefined
behaviour reported by UBSAN to all architectures. The fix was done in
commit 5f16a046f8 (arm64: futex: Fix undefined behaviour with
FUTEX_OP_OPARG_SHIFT usage). Look there for an example dump.

And as suggested by Thomas, check for negative oparg too, because it was
also reported to cause undefined behaviour report.

Note that s390 removed access_ok check in d12a29703 ("s390/uaccess:
remove pointless access_ok() checks") as access_ok there returns true.
We introduce it back to the helper for the sake of simplicity (it gets
optimized away anyway).

7bf76f0169 s390/dasd: Change unsigned long long to unsigned long
Unsigned long long and unsigned long were different in size for 31-bit.
For 64-bit the size for both datatypes is 8 Bytes and since the support
for 31-bit is long gone we can clean up a little and change everything
to unsigned long.
Change get_phys_clock() along the way to accept unsigned long as well so
that the DASD code can be consistent.

857b8de967 KVM: s390: sthyi: fix specification exception detection
sthyi should only generate a specification exception if the function
code is zero and the response buffer is not on a 4k boundary.

The current code would also test for unknown function codes if the
response buffer, that is currently only defined for function code 0,
is not on a 4k boundary and incorrectly inject a specification
exception instead of returning with condition code 3 and return code 4
(unsupported function code).

Fix this by moving the boundary check.

4a4eefcd0e KVM: s390: sthyi: fix sthyi inline assembly
The sthyi inline assembly misses register r3 within the clobber
list. The sthyi instruction will always write a return code to
register "R2+1", which in this case would be r3. Due to that we may
have register corruption and see host crashes or data corruption
depending on how gcc decided to allocate and use registers during
compile time.

99baac21e4 mm: fix MADV_[FREE|DONTNEED] TLB flush miss problem
Nadav reported parallel MADV_DONTNEED on same range has a stale TLB
problem and Mel fixed it[1] and found same problem on MADV_FREE[2].

Quote from Mel Gorman:
 "The race in question is CPU 0 running madv_free and updating some PTEs
  while CPU 1 is also running madv_free and looking at the same PTEs.
  CPU 1 may have writable TLB entries for a page but fail the pte_dirty
  check (because CPU 0 has updated it already) and potentially fail to
  flush.

  Hence, when madv_free on CPU 1 returns, there are still potentially
  writable TLB entries and the underlying PTE is still present so that a
  subsequent write does not necessarily propagate the dirty bit to the
  underlying PTE any more. Reclaim at some unknown time at the future
  may then see that the PTE is still clean and discard the page even
  though a write has happened in the meantime. I think this is possible
  but I could have missed some protection in madv_free that prevents it
  happening."

This patch aims for solving both problems all at once and is ready for
other problem with KSM, MADV_FREE and soft-dirty story[3].

TLB batch API(tlb_[gather|finish]_mmu] uses [inc|dec]_tlb_flush_pending
and mmu_tlb_flush_pending so that when tlb_finish_mmu is called, we can
catch there are parallel threads going on.  In that case, forcefully,
flush TLB to prevent for user to access memory via stale TLB entry
although it fail to gather page table entry.

I confirmed this patch works with [4] test program Nadav gave so this
patch supersedes "mm: Always flush VMA ranges affected by zap_page_range
v2" in current mmotm.

NOTE:

This patch modifies arch-specific TLB gathering interface(x86, ia64,
s390, sh, um).  It seems most of architecture are straightforward but
s390 need to be careful because tlb_flush_mmu works only if
mm->context.flush_mm is set to non-zero which happens only a pte entry
really is cleared by ptep_get_and_clear and friends.  However, this
problem never changes the pte entries but need to flush to prevent
memory access from stale tlb.

This patch is a preparatory patch for solving race problems caused by
TLB batch.  For that, we will increase/decrease TLB flush pending count
of mm_struct whenever tlb_[gather|finish]_mmu is called.

Before making it simple, this patch separates architecture specific part
and rename it to arch_tlb_[gather|finish]_mmu and generic part just
calls it.

It shouldn't change any behavior.

3f4298427a s390/vmcp: make use of contiguous memory allocator
If memory is fragmented it is unlikely that large order memory
allocations succeed. This has been an issue with the vmcp device
driver since a long time, since it requires large physical contiguous
memory ares for large responses.

To hopefully resolve this issue make use of the contiguous memory
allocator (cma). This patch adds a vmcp specific vmcp cma area with a
default size of 4MB. The size can be changed either via the
VMCP_CMA_SIZE config option at compile time or with the "vmcp_cma"
kernel parameter (e.g. "vmcp_cma=16m").

For any vmcp response buffers larger than 16k memory from the cma area
will be allocated. If such an allocation fails, there is a fallback to
the buddy allocator.

cd4386a931 s390/cpcmd,vmcp: avoid GFP_DMA allocations
According to the CP Programming Services manual Diagnose Code 8
"Virtual Console Function" can be used in all addressing modes. Also
the input and output buffers do not have a limitation which specifies
they need to be below the 2GB line.

This is true at least since z/VM 5.4.

Therefore remove the sam31/64 instructions and allow for simple
GFP_KERNEL allocations. This makes it easier to allocate a 1MB page
if the user requested such a large return buffer.

34ad7cdc1b s390/mm: prevent memory offline for memory blocks with cma areas
Memory blocks that contain areas for the contiguous memory allocator
(cma) should not be allowed to go offline. Otherwise this would render
cma completely useless.
This might make sense on other architectures where memory might be
taken offline due to hardware errors, but not on architectures which
support memory hotplug for load balancing.

199b5763d3 KVM: add spinlock optimization framework
If a vcpu exits due to request a user mode spinlock, then
the spinlock-holder may be preempted in user mode or kernel mode.
(Note that not all architectures trap spin loops in user mode,
only AMD x86 and ARM/ARM64 currently do).

But if a vcpu exits in kernel mode, then the holder must be
preempted in kernel mode, so we should choose a vcpu in kernel mode
as a more likely candidate for the lock holder.

This introduces kvm_arch_vcpu_in_kernel() to decide whether the
vcpu is in kernel-mode when it's preempted.  kvm_vcpu_on_spin's
new argument says the same of the spinning VCPU.

b0a0c2566f bpf, s390: fix jit branch offset related to ldimm64
While testing some other work that required JIT modifications, I
run into test_bpf causing a hang when JIT enabled on s390. The
problematic test case was the one from ddc665a4bb (bpf, arm64:
fix jit branch offset related to ldimm64), and turns out that we
do have a similar issue on s390 as well. In bpf_jit_prog() we
update next instruction address after returning from bpf_jit_insn()
with an insn_count. bpf_jit_insn() returns either -1 in case of
error (e.g. unsupported insn), 1 or 2. The latter is only the
case for ldimm64 due to spanning 2 insns, however, next address
is only set to i + 1 not taking actual insn_count into account,
thus fix is to use insn_count instead of 1. bpf_jit_enable in
mode 2 provides also disasm on s390:

unwittingly pass MSG_ZEROCOPY. Continue to ignore this flag unless a
socket opts in to zerocopy.

Introduce socket option SO_ZEROCOPY to enable MSG_ZEROCOPY processing.
Processes can also query this socket option to detect kernel support
for the feature. Older kernels will return ENOPROTOOPT.

3cb8f11c5d s390: use generic uapi/asm/swab.h
clang doesn't like s390 specific inline assembler constraints. These
are present in our arch specific uapi/asm/swab.h which again is
required by some ebpf test cases.

For current compiler versions the generic swab.h already makes use of
gcc's builtin functions. Therefore we can simply remove our own header
file and use the generic one.

This will generate worse code if used with compilers before gcc 4.8,
which has no __builtin_bswap16(); or before gcc v4.4, which has no
__builtin_bswap[32|64](). For these cases a C implementation fallback
would be used which generates more code, but is still correct (170KB
extra code for gcc 4.3 with performance_defconfig).

However given that we need (and want) to get rid of the inline
assemblies anyway in order to be able to use clang, the above is just
a minor drawback if old gcc compilers are used.

With current compilers there is close to zero difference, except for
three btrfs bit functions which generate more out-of-line code. The
generated code looks still correct and also uses the s390 specific
byteswap instructions.

bccf90d6e0 PCI: Add a generic weak pcibios_fixup_bus()
Multiple architectures define this as an empty function, and I'm adding
another one as part of the RISC-V port.  Add a __weak version of
pcibios_fixup_bus() and delete the now-obselete ones in a handful of
ports.

The only functional change should be that microblaze used to export
pcibios_fixup_bus().  None of the other architectures exports this, so I
just dropped it.

The z14 machine introduces new mode of the next-instruction-access-intent
NIAI instruction. With NIAI-8 it is possible to pin a cache-line on a
CPU for a small amount of time, NIAI-7 releases the cache-line again.
Finally NIAI-4 can be used to prevent the CPU to speculatively access
memory beyond the compare-and-swap instruction to get the lock.

Use these instruction in the spinlock code.

6e2ef5e4f6 s390/time: add support for the TOD clock epoch extension
The TOD epoch extension adds 8 epoch bits to the TOD clock to provide
a continuous clock after 2042/09/17. The store-clock-extended (STCKE)
instruction will store the epoch index in the first byte of the
16 bytes stored by the instruction. The read_boot_clock64 and the
read_presistent_clock64 functions need to take the additional bits
into account to give the correct result after 2042/09/17.

The clock-comparator register will stay 64 bit wide. The comparison
of the clock-comparator with the TOD clock is limited to bytes
1 to 8 of the extended TOD format. To deal with the overflow problem
due to an epoch change the clock-comparator sign control in CR0 can
be used to switch the comparison of the 64-bit TOD clock with the
clock-comparator to a signed comparison.

The decision between the signed vs. unsigned clock-comparator
comparisons is done at boot time. Only if the TOD clock is in the
second half of a 142 year epoch the signed comparison is used.
This solves the epoch overflow issue as long as the machine is
booted at least once in an epoch.

c67da7c7c5 s390/mm: introduce defines to reflect the hardware mmu
Add various defines like e.g. _REGION1_SHIFT to reflect the hardware
mmu. We have quite a bit code that does not make use of the Linux
memory management primitives but directly modifies page, segment and
region values.

Most of this is open-coded like e.g. "1UL << 53". In order to clean
this up introduce a couple of new defines. The existing Linux memory
management defines are changed, so the mapping to the hardware
implementation is reflected.

c9b5ad546e s390/mm: tag normal pages vs pages used in page tables
The ESSA instruction has a new option that allows to tag pages that
are not used as a page table. Without the tag the hypervisor has to
assume that any guest page could be used in a page table inside the
guest. This forces the hypervisor to flush all guest TLB entries
whenever a host page table entry is invalidated. With the tag
the host can skip the TLB flush if the page is tagged as normal page.

cc731525f2 signal: Remove kernel interal si_code magic
struct siginfo is a union and the kernel since 2.4 has been hiding a union
tag in the high 16bits of si_code using the values:
__SI_KILL
__SI_TIMER
__SI_POLL
__SI_FAULT
__SI_CHLD
__SI_RT
__SI_MESGQ
__SI_SYS

While this looks plausible on the surface, in practice this situation has
not worked well.

- Injected positive signals are not copied to user space properly
  unless they have these magic high bits set.

- Injected positive signals are not reported properly by signalfd
  unless they have these magic high bits set.

- These kernel internal values leaked to userspace via ptrace_peek_siginfo

- It was possible to inject these kernel internal values and cause the
  the kernel to misbehave.

- Kernel developers got confused and expected these kernel internal values
  in userspace in kernel self tests.

- Kernel developers got confused and set si_code to __SI_FAULT which
  is SI_USER in userspace which causes userspace to think an ordinary user
  sent the signal and that it was not kernel generated.

- The values make it impossible to reorganize the code to transform
  siginfo_copy_to_user into a plain copy_to_user.  As si_code must
  be massaged before being passed to userspace.

So remove these kernel internal si codes and make the kernel code simpler
and more maintainable.

To replace these kernel internal magic si_codes introduce the helper
function siginfo_layout, that takes a signal number and an si_code and
computes which union member of siginfo is being used.  Have
siginfo_layout return an enumeration so that gcc will have enough
information to warn if a switch statement does not handle all of union
members.

A couple of architectures have a messed up ABI that defines signal
specific duplications of SI_USER which causes more special cases in
siginfo_layout than I would like.  The good news is only problem
architectures pay the cost.

Update all of the code that used the previous magic __SI_ values to
use the new SIL_ values and to call siginfo_layout to get those
values.  Escept where not all of the cases are handled remove the
defaults in the switch statements so that if a new case is missed in
the future the lack will show up at compile time.

Modify the code that copies siginfo si_code to userspace to just copy
the value and not cast si_code to a short first.  The high bits are no
longer used to hold a magic union member.

Fixup the siginfo header files to stop including the __SI_ values in
their constants and for the headers that were missing it to properly
update the number of si_codes for each signal type.

The fixes to copy_siginfo_from_user32 implementations has the
interesting property that several of them perviously should never have
worked as the __SI_ values they depended up where kernel internal.
With that dependency gone those implementations should work much
better.

The idea of not passing the __SI_ values out to userspace and then
not reinserting them has been tested with criu and criu worked without
changes.

325cdacd03 debug: Fix WARN_ON_ONCE() for modules
Mike Galbraith reported a situation where a WARN_ON_ONCE() call in DRM
code turned into an oops.  As it turns out, WARN_ON_ONCE() seems to be
completely broken when called from a module.

The bug was introduced with the following commit:

  19d436268d ("debug: Add _ONCE() logic to report_bug()")

That commit changed WARN_ON_ONCE() to move its 'once' logic into the bug
trap handler.  It requires a writable bug table so that the BUGFLAG_DONE
bit can be written to the flags to indicate the first warning has
occurred.

The bug table was made writable for vmlinux, which relies on
vmlinux.lds.S and vmlinux.lds.h for laying out the sections.  However,
it wasn't made writable for modules, which rely on the ELF section
header flags.

97ca7bfc19 s390/mm: set change and reference bit on lazy key enablement
When we enable storage keys for a guest lazily, we reset the ACC and F
values. That is correct assuming that these are 0 on a clear reset and
the guest obviously has not used any key setting instruction.

We also zero out the change and reference bit. This is not correct as
the architecture prefers over-indication instead of under-indication
for the keyless->keyed transition.

This patch fixes the behaviour and always sets guest change and guest
reference for all guest storage keys on the keyless -> keyed switch.

This patch allocates extra pages for these vmcoreinfo_XXX variables, one
advantage is that it enhances some safety of vmcoreinfo, because
vmcoreinfo now is kept far away from other kernel data structures.

a73dc5370e s390: reduce ELF_ET_DYN_BASE
Now that explicitly executed loaders are loaded in the mmap region, we
have more freedom to decide where we position PIE binaries in the
address space to avoid possible collisions with mmap or stack regions.

For 64-bit, align to 4GB to allow runtimes to use the entire 32-bit
address space for 32-bit pointers.  On 32-bit use 4MB, which is the
traditional x86 minimum load location, likely to avoid historically
requiring a 4MB page table entry when only a portion of the first 4MB
would be used (since the NULL address is avoided).  For s390 the
position could be 0x10000, but that is needlessly close to the NULL
address.

9386fac34c mm/hugetlb: allow architectures to override huge_pte_clear()
When unmapping a hugepage range, huge_pte_clear() is used to clear the
page table entries that are marked as not present.  huge_pte_clear()
internally just ends up calling pte_clear() which does not correctly
deal with hugepages consisting of contiguous page table entries.

Add a size argument to address this issue and allow architectures to
override huge_pte_clear() by wrapping it in a #ifndef block.

Update s390 implementation with the size parameter as well.

Note that the change only affects huge_pte_clear() - the other generic
hugetlb functions don't need any change.

7868a2087e mm/hugetlb: add size parameter to huge_pte_offset()
A poisoned or migrated hugepage is stored as a swap entry in the page
tables.  On architectures that support hugepages consisting of
contiguous page table entries (such as on arm64) this leads to ambiguity
in determining the page table entry to return in huge_pte_offset() when
a poisoned entry is encountered.

Let's remove the ambiguity by adding a size parameter to convey
additional information about the requested address.  Also fixup the
definition/usage of huge_pte_offset() throughout the tree.

e1073d1e79 mm/hugetlb: clean up ARCH_HAS_GIGANTIC_PAGE
This moves the #ifdef in C code to a Kconfig dependency.  Also we move
the gigantic_page_supported() function to be arch specific.

This allows architectures to conditionally enable runtime allocation of
gigantic huge page.  Architectures like ppc64 supports different
gigantic huge page size (16G and 1G) based on the translation mode
selected.  This provides an opportunity for ppc64 to enable runtime
allocation only w.r.t 1G hugepage.

No functional change in this patch.

3d79a728f9 mm, memory_hotplug: replace for_device by want_memblock in arch_add_memory
arch_add_memory gets for_device argument which then controls whether we
want to create memblocks for created memory sections.  Simplify the
logic by telling whether we want memblocks directly rather than going
through pointless negation.  This also makes the api easier to
understand because it is clear what we want rather than nothing telling
for_device which can mean anything.

This shouldn't introduce any functional change.

f1dd2cd13c mm, memory_hotplug: do not associate hotadded memory to zones until online
The current memory hotplug implementation relies on having all the
struct pages associate with a zone/node during the physical hotplug
phase (arch_add_memory->__add_pages->__add_section->__add_zone).  In the
vast majority of cases this means that they are added to ZONE_NORMAL.
This has been so since 9d99aaa31f ("[PATCH] x86_64: Support memory
hotadd without sparsemem") and it wasn't a big deal back then because
movable onlining didn't exist yet.

Much later memory hotplug wanted to (ab)use ZONE_MOVABLE for movable
onlining 511c2aba8f ("mm, memory-hotplug: dynamic configure movable
memory and portion memory") and then things got more complicated.
Rather than reconsidering the zone association which was no longer
needed (because the memory hotplug already depended on SPARSEMEM) a
convoluted semantic of zone shifting has been developed.  Only the
currently last memblock or the one adjacent to the zone_movable can be
onlined movable.  This essentially means that the online type changes as
the new memblocks are added.

This is an awkward semantic because an udev event is sent as soon as the
block is onlined and an udev handler might want to online it based on
some policy (e.g.  association with a node) but it will inherently race
with new blocks showing up.

This patch changes the physical online phase to not associate pages with
any zone at all.  All the pages are just marked reserved and wait for
the onlining phase to be associated with the zone as per the online
request.  There are only two requirements

	- existing ZONE_NORMAL and ZONE_MOVABLE cannot overlap

	- ZONE_NORMAL precedes ZONE_MOVABLE in physical addresses

the latter one is not an inherent requirement and can be changed in the
future.  It preserves the current behavior and made the code slightly
simpler.  This is subject to change in future.

This means that the same physical online steps as above will lead to the
following state: Normal Movable

Implementation:
The current move_pfn_range is reimplemented to check the above
requirements (allow_online_pfn_range) and then updates the respective
zone (move_pfn_range_to_zone), the pgdat and links all the pages in the
pfn range with the zone/node.  __add_pages is updated to not require the
zone and only initializes sections in the range.  This allowed to
simplify the arch_add_memory code (s390 could get rid of quite some of
code).

devm_memremap_pages is the only user of arch_add_memory which relies on
the zone association because it only hooks into the memory hotplug only
half way.  It uses it to associate the new memory with ZONE_DEVICE but
doesn't allow it to be {on,off}lined via sysfs.  This means that this
particular code path has to call move_pfn_range_to_zone explicitly.

The original zone shifting code is kept in place and will be removed in
the follow up patch for an easier review.

Please note that this patch also changes the original behavior when
offlining a memory block adjacent to another zone (Normal vs.  Movable)
used to allow to change its movable type.  This will be handled later.

1b862aecfb mm, memory_hotplug: get rid of is_zone_device_section
Device memory hotplug hooks into regular memory hotplug only half way.
It needs memory sections to track struct pages but there is no
need/desire to associate those sections with memory blocks and export
them to the userspace via sysfs because they cannot be onlined anyway.

This is currently expressed by for_device argument to arch_add_memory
which then makes sure to associate the given memory range with
ZONE_DEVICE.  register_new_memory then relies on is_zone_device_section
to distinguish special memory hotplug from the regular one.  While this
works now, later patches in this series want to move __add_zone outside
of arch_add_memory path so we have to come up with something else.

Add want_memblock down the __add_pages path and use it to control
whether the section->memblock association should be done.
arch_add_memory then just trivially want memblock for everything but
for_device hotplug.

remove_memory_section doesn't need is_zone_device_section either.  We
can simply skip all the memblock specific cleanup if there is no
memblock for the given section.

This shouldn't introduce any functional change.

6474924e2b arch: remove unused macro/function thread_saved_pc()
The only user of thread_saved_pc() in non-arch-specific code was removed
in commit 8243d55977 ("sched/core: Remove pointless printout in
sched_show_task()").  Remove the implementations as well.

Some architectures use thread_saved_pc() in their arch-specific code.
Leave their thread_saved_pc() intact.

d52cd2076e KVM: s390: Inject machine check into the nested guest
With vsie feature enabled, kvm can support nested guests (guest-3).
So inject machine check to the guest-2 if it happens when the nested
guest is running. And guest-2 will detect the machine check belongs
to guest-3 and reinject it into guest-3.
The host (guest-1) tries to inject the machine check to the picked
destination vcpu if it's a floating machine check.

4d62fcc0b6 KVM: s390: Inject machine check into the guest
If the exit flag of SIE indicates that a machine check has happened
during guest's running and needs to be injected, inject it to the guest
accordingly.
But some machine checks, e.g. Channel Report Pending (CRW), refer to
host conditions only (the guest's channel devices are not managed by
the kernel directly) and are therefore not injected into the guest.
External Damage (ED) is also not reinjected into the guest because ETR
conditions are gone in Linux and STP conditions are not enabled in the
guest, and ED contains only these 8 ETR and STP conditions.
In general, instruction-processing damage, system recovery,
storage error, service-processor damage and channel subsystem damage
will be reinjected into the guest, and the remain (System damage,
timing-facility damage, warning, ED and CRW) will be handled on the host.

312e8462ab s390/pci: recognize name clashes with uids
When uid checking is enabled firmware guarantees uniqueness of the uids
and we use them for device enumeration. Tests have shown that uid checking
can be toggled at runtime. This is unfortunate since it can lead to name
clashes.

Recognize these name clashes by allocating bits in zpci_domain even for
firmware provided ids.

In contrast to other hotplug events PEC 0x306 isn't about a single
but multiple devices. Also there's no information on what happened
to these devices. We correctly handled hotplug that way but failed
to handle hot-unplug. This patch addresses that and implements
hot-unplug of multiple devices via PEC 306.

DMA tables are freed in zpci_dma_exit_device regardless of the return
code of zpci_unregister_ioat. This could lead to a use after free. On
the other hand during function hot-unplug, zpci_unregister_ioat will
always fail since the function is already gone.

So let zpci_unregister_ioat report success when the function is gone
but don't cleanup the dma table when a function could still have it
in access.

4dfbd3efe3 s390/pci: improve error handling during interrupt deregistration
When we ask a function to stop creating interrupts this may fail
due to the function being already gone (e.g. after hot-unplug).

Consequently we don't free associated resources like summary bits
and bit vectors used for irq processing. This could lead to
situations where we ran out of these resources and fail to setup
new interrupts.

The fix is to just ignore the errors in cases where we can be
sure no new interrupts are generated.

When a machine check happens in the guest, related mcck info (mcic,
external damage code, ...) is stored in the vcpu's lowcore on the host.
Then the machine check handler's low-level part is executed, followed
by the high-level part.

If the high-level part's execution is interrupted by a new machine check
happening on the same vcpu on the host, the mcck info in the lowcore is
overwritten with the new machine check's data.

If the high-level part's execution is scheduled to a different cpu,
the mcck info in the lowcore is uncertain.

Therefore, for both cases, the further reinjection to the guest will use
the wrong data.
Let's backup the mcck info in the lowcore to the sie page
for further reinjection, so that the right data will be used.

Add new member into struct sie_page to store related machine check's
info of mcic, failing storage address and external damage code.

For real-space designation asces the asce origin part is only a token.
The asce token origin must not be used to generate an effective
address for storage references. This however is erroneously done
within kvm_s390_shadow_tables().

Furthermore within the same function the wrong parts of virtual
addresses are used to generate a corresponding real address
(e.g. the region second index is used as region first index).

Both of the above can result in incorrect address translations. Only
for real space designations with a token origin of zero and addresses
below one megabyte the translation was correct.

Furthermore replace a "!asce.r" statement with a "!*fake" statement to
make it more obvious that a specific condition has nothing to do with
the architecture, but with the fake handling of real space designations.

28b5ba2aa0 net: introduce SO_PEERGROUPS getsockopt
This adds the new getsockopt(2) option SO_PEERGROUPS on SOL_SOCKET to
retrieve the auxiliary groups of the remote peer. It is designed to
naturally extend SO_PEERCRED. That is, the underlying data is from the
same credentials. Regarding its syntax, it is based on SO_PEERSEC. That
is, if the provided buffer is too small, ERANGE is returned and @optlen
is updated. Otherwise, the information is copied, @optlen is set to the
actual size, and 0 is returned.

While SO_PEERCRED (and thus `struct ucred') already returns the primary
group, it lacks the auxiliary group vector. However, nearly all access
controls (including kernel side VFS and SYSVIPC, but also user-space
polkit, DBus, ...) consider the entire set of groups, rather than just
the primary group. But this is currently not possible with pure
SO_PEERCRED. Instead, user-space has to work around this and query the
system database for the auxiliary groups of a UID retrieved via
SO_PEERCRED.

Unfortunately, there is no race-free way to query the auxiliary groups
of the PID/UID retrieved via SO_PEERCRED. Hence, the current user-space
solution is to use getgrouplist(3p), which itself falls back to NSS and
whatever is configured in nsswitch.conf(3). This effectively checks
which groups we *would* assign to the user if it logged in *now*. On
normal systems it is as easy as reading /etc/group, but with NSS it can
resort to quering network databases (eg., LDAP), using IPC or network
communication.

Long story short: Whenever we want to use auxiliary groups for access
checks on IPC, we need further IPC to talk to the user/group databases,
rather than just relying on SO_PEERCRED and the incoming socket. This
is unfortunate, and might even result in dead-locks if the database
query uses the same IPC as the original request.

So far, those recursions / dead-locks have been avoided by using
primitive IPC for all crucial NSS modules. However, we want to avoid
re-inventing the wheel for each NSS module that might be involved in
user/group queries. Hence, we would preferably make DBus (and other IPC
that supports access-management based on groups) work without resorting
to the user/group database. This new SO_PEERGROUPS ioctl would allow us
to make dbus-daemon work without ever calling into NSS.

1be7107fbe mm: larger stack guard gap, between vmas
Stack guard page is a useful feature to reduce a risk of stack smashing
into a different mapping. We have been using a single page gap which
is sufficient to prevent having stack adjacent to a different mapping.
But this seems to be insufficient in the light of the stack usage in
userspace. E.g. glibc uses as large as 64kB alloca() in many commonly
used functions. Others use constructs liks gid_t buffer[NGROUPS_MAX]
which is 256kB or stack strings with MAX_ARG_STRLEN.

This will become especially dangerous for suid binaries and the default
no limit for the stack size limit because those applications can be
tricked to consume a large portion of the stack and a single glibc call
could jump over the guard page. These attacks are not theoretical,
unfortunatelly.

Make those attacks less probable by increasing the stack guard gap
to 1MB (on systems with 4k pages; but make it depend on the page size
because systems with larger base pages might cap stack allocations in
the PAGE_SIZE units) which should cover larger alloca() and VLA stack
allocations. It is obviously not a full fix because the problem is
somehow inherent, but it should reduce attack space a lot.

One could argue that the gap size should be configurable from userspace,
but that can be done later when somebody finds that the new 1MB is wrong
for some special case applications.  For now, add a kernel command line
option (stack_guard_gap) to specify the stack gap size (in page units).

Implementation wise, first delete all the old code for stack guard page:
because although we could get away with accounting one extra page in a
stack vma, accounting a larger gap can break userspace - case in point,
a program run with "ulimit -S -v 20000" failed when the 1MB gap was
counted for RLIMIT_AS; similar problems could come with RLIMIT_MLOCK
and strict non-overcommit mode.

Instead of keeping gap inside the stack vma, maintain the stack guard
gap as a gap between vmas: using vm_start_gap() in place of vm_start
(or vm_end_gap() in place of vm_end if VM_GROWSUP) in just those few
places which need to respect the gap - mainly arch_get_unmapped_area(),
and and the vma tree's subtree_gap support for that.

The two commits did not take into account that behavior of standby
memory changes fundamentally if the re-IPL method is changed from
Load Clear to Load Normal.

In case of the old re-IPL clear method all memory that was initially
in standby state will be put into standby state again within the
re-IPL process. Or in other words: memory that was brought online
before a re-IPL will be offline again after a reboot.

Given that we use different re-IPL methods depending on the hypervisor
and CCW-type vs SCSI re-IPL it is not easy to tell in advance when and
why memory will stay online or will be offline after a re-IPL.
This does also have other side effects, since memory that is online
from the beginning will be in ZONE_NORMAL by default vs ZONE_MOVABLE
for memory that is offline.

Therefore, before the change, a user could online and offline memory
easily since standby memory was always in ZONE_NORMAL.  After the
change, and a re-IPL, this depended on which memory parts were online
before the re-IPL.

From a usability point of view the current behavior is more than
suboptimal. Therefore revert these changes until we have a better
solution and get back to a consistent behavior. The bad thing about
this is that the time required for a re-IPL will be significantly
increased for configurations with several 100GB or 1TB of memory.

23fefe119c s390/kvm: avoid global config of vm.alloc_pgste=1
The system control vm.alloc_pgste is used to control the size of the
page tables, either 2K or 4K. The idea is that a KVM host sets the
vm.alloc_pgste control to 1 which causes *all* new processes to run
with 4K page tables. For a non-kvm system the control should stay off
to save on memory used for page tables.

Trouble is that distributions choose to set the control globally to
be able to run KVM guests. This wastes memory on non-KVM systems.

Introduce the PT_S390_PGSTE ELF segment type to "mark" the qemu
executable with it. All executables with this (empty) segment in
its ELF phdr array will be started with 4K page tables. Any executable
without PT_S390_PGSTE will run with the default 2K page tables.

This removes the need to set vm.alloc_pgste=1 for a KVM host and
minimizes the waste of memory for page tables.

To avoid confusion rename the PSW_AS_* enums to PSW_BITS_AS_*.

In addition also rename the PSW_AMODE_* enums, so they also follow the

60c497014e s390/mm: use correct address space when enabling DAT
Right now the kernel uses the primary address space until finally the
switch to the correct home address space will be done when the idle
PSW will be loaded within psw_idle().

Correct this and simply use the home address space when DAT is enabled
for the first time.

This doesn't really fix a bug, but fixes odd behavior.

ead1dec8ed s390/ipl: revert Load Normal semantics for LPAR CCW-type re-IPL
This reverts the two commits

7afbeb6df2 ("s390/ipl: always use load normal for CCW-type re-IPL")
0f7451ff3a ("s390/ipl: use load normal for LPAR re-ipl")

The two commits did not take into account that behavior of standby
memory changes fundamentally if the re-IPL method is changed from
Load Clear to Load Normal.

In case of the old re-IPL clear method all memory that was initially
in standby state will be put into standby state again within the
re-IPL process. Or in other words: memory that was brought online
before a re-IPL will be offline again after a reboot.

Given that we use different re-IPL methods depending on the hypervisor
and CCW-type vs SCSI re-IPL it is not easy to tell in advance when and
why memory will stay online or will be offline after a re-IPL.
This does also have other side effects, since memory that is online
from the beginning will be in ZONE_NORMAL by default vs ZONE_MOVABLE
for memory that is offline.

Therefore, before the change, a user could online and offline memory
easily since standby memory was always in ZONE_NORMAL.  After the
change, and a re-IPL, this depended on which memory parts were online
before the re-IPL.

From a usability point of view the current behavior is more than
suboptimal. Therefore revert these changes until we have a better
solution and get back to a consistent behavior. The bad thing about
this is that the time required for a re-IPL will be significantly
increased for configurations with several 100GB or 1TB of memory.

2b7b9817c2 s390/dumpstack: remove raw stack dump
Remove raw stack dumps that are printed before call traces in case of
a warning, or the 'l' sysrq trigger (show a stack backtrace for all
active CPUs).

Besides that a raw stack dump should not be shown for the 'l' sysrq
trigger the value of the dump is close to zero. That's also why we
don't print it in case of a panic since ages anymore. That this is
still printed on warnings is just a leftover. So get rid of this
completely.

7e9710af23 s390: provide default ioremap and iounmap declaration
Move the CONFIG_PCI device so that ioremap and iounmap are always
available. This looks safe as there's nothing PCI specific in the
implementation of these functions.

I have designs to use these functions in scatterlist.c where they'd likely
never be called without CONFIG_PCI set, but this is needed to compile
such changes.

c39457ff1f s390/perf: fix null string in perf list pmu command
Command 'perf list pmu' displays events which contain
an invalid string "(null)=xxx", where xxx is the pmu event
name, for example:
   cpum_cf/AES_BLOCKED_CYCLES,(null)=AES_BLOCKED_CYCLES/
This is not correct, the invalid string should not be
displayed at all.

It is caused by an obsolete term in the
sysfs attribute file for each s390 CPUMF counter event.
Reading from the sysfs file also displays the event
name.

Fix this by omitting the event name.  This patch makes
s390 CPUMF sysfs files consistent with other plattforms.

This is an interface change between user and kernel
but does not break anything. Reading from a counter event
sysfs file should only list terms mentioned in the
/sys/bus/event_source/devices/<cpumf>/format directory.
Name is not listed.

f96c6f72bc s390/mm: remove incorrect _REGION3_ENTRY_ORIGIN define
_REGION3_ENTRY_ORIGIN defines a wrong mask which can be used to
extract a segment table origin from a region 3 table entry. It removes
only the lower 11 instead of 12 bits from a region 3 table entry.
Luckily this bit is currently always zero, so nothing bad happened yet.

In order to avoid future bugs just remove the region 3 specific mask
and use the correct generic _REGION_ENTRY_ORIGIN mask.

8aa8680aa3 s390: Remove 'message security assist' from the list of vital facilities
The code in arch/s390/crypto checks for the availability of the
'message security assist' facility on its own, either by using
module_cpu_feature_match(MSA, ...) or by checking the facility
bit during cpacf_query(). Thus setting the MSA facility bit in
gen_facilities.c as hard requirement is not necessary. We can
remove it here, so that the kernel can also run on systems that
do not provide the MSA facility yet (like the emulated environment
of QEMU, for example).

c4684f98d3 s390/crypto: fix aes/paes Kconfig dependeny
The s390_paes and the s390_aes kernel module used just one
config symbol CONFIG_CRYPTO_AES. As paes has a dependency
to PKEY and this requires ZCRYPT the aes module also had
a dependency to the zcrypt device driver which is not true.
Fixed by introducing a new config symbol CONFIG_CRYPTO_PAES
which has dependencies to PKEY and ZCRYPT. Removed the
dependency for the aes module to ZCRYPT.

2a842acab1 block: introduce new block status code type
Currently we use nornal Linux errno values in the block layer, and while
we accept any error a few have overloaded magic meanings.  This patch
instead introduces a new  blk_status_t value that holds block layer specific
status codes and explicitly explains their meaning.  Helpers to convert from
and to the previous special meanings are provided for now, but I suspect
we want to get rid of them in the long run - those drivers that have a
errno input (e.g. networking) usually get errnos that don't know about
the special block layer overloads, and similarly returning them to userspace
will usually return somethings that strictly speaking isn't correct
for file system operations, but that's left as an exercise for later.

For now the set of errors is a very limited set that closely corresponds
to the previous overloaded errno values, but there is some low hanging
fruite to improve it.

blk_status_t (ab)uses the sparse __bitwise annotations to allow for sparse
typechecking, so that we can easily catch places passing the wrong values.

47b2c3fff4 security/keys: add CONFIG_KEYS_COMPAT to Kconfig
CONFIG_KEYS_COMPAT is defined in arch-specific Kconfigs and is missing for
several 64-bit architectures : mips, parisc, tile.

At the moment and for those architectures, calling in 32-bit userspace the
keyctl syscall would return an ENOSYS error.

This patch moves the CONFIG_KEYS_COMPAT option to security/keys/Kconfig, to
make sure the compatibility wrapper is registered by default for any 64-bit
architecture as long as it is configured with CONFIG_COMPAT.

A first step in vcpu->requests encapsulation.  Additionally, we now
use READ_ONCE() when accessing vcpu->requests, which ensures we
always load vcpu->requests when it's accessed.  This is important as
other threads can change it any time.  Also, READ_ONCE() documents
that vcpu->requests is used with other threads, likely requiring
memory barriers, which it does.

base, rather than requiring each arch to remember to start from 8.
That suggestion, along with Radim Krcmar's recent VCPU request flag
addition, snowballed into defining something of an arch VCPU request

(Looks like x86 is running out of arch VCPU request bits.  Maybe
 someday we'll need to extend to 64.)

5d5dbc4ef2 s390: Prevent hotplug rwsem recursion
The text patching functions which are invoked from jump_label and kprobes
code are protected against cpu hotplug at the call sites.

Use stop_machine_cpuslocked() to avoid recursion on the cpu hotplug
rwsem. stop_machine_cpuslocked() contains a lockdep assertion to catch any
unprotected callers.

2337e879e8 s390/kernel: Use stop_machine_cpuslocked()
stp_work_fn() holds get_online_cpus() while invoking stop_machine().

stop_machine() invokes get_online_cpus() as well. This is correct, but
prevents the conversion of the hotplug locking to a percpu rwsem.

Use stop_machine_cpuslocked() to avoid the nested call. Convert
*_online_cpus() to the new interfaces while at it.

c0e7bb38c0 s390/kvm: do not rely on the ILC on kvm host protection fauls
For most cases a protection exception in the host (e.g. copy
on write or dirty tracking) on the sie instruction will indicate
an instruction length of 4. Turns out that there are some corner
cases (e.g. runtime instrumentation) where this is not necessarily
true and the ILC is unpredictable.

Let's replace our 4 byte rewind_pad with 3 byte nops to prepare for
all possible ILCs.

refcount_t type and corresponding API should be
used instead of atomic_t when the variable is used as
a reference counter. This allows to avoid accidental
refcounter overflows that might lead to use-after-free

d04a4c76f7 s390: move _text symbol to address higher than zero
The perf tool assumes that kernel symbols are never present at address
zero. In fact it assumes if functions that map symbols to addresses
return zero, that the symbol was not found.

Given that s390's _text symbol historically is located at address zero
this yields at least a couple of false errors and warnings in one of
perf's test cases about not present symbols ("perf test 1").

To fix this simply move the _text symbol to address 0x200, just behind
the initial psw and channel program located at the beginning of the
kernel image. This is now hard coded within the linker script.

I tried a nicer solution which moves the initial psw and channel
program into an own section. However that would move the symbols
within the "real" head.text section to different addresses, since the
".org" statements within head.S are relative to the head.text
section. If there is a new section in front, everything else will be
moved. Alternatively I could have adjusted all ".org" statements. But
this current solution seems to be the easiest one, since nobody really
cares where the _text symbol is actually located.

f5c8b96010 s390/uaccess: use sane length for __strncpy_from_user()
The average string that is copied from user space to kernel space is
rather short. E.g. booting a system involves about 50.000
strncpy_from_user() calls where the NULL terminated string has an
average size of 27 bytes.

By default our s390 specific strncpy_from_user() implementation
however copies up to 4096 bytes, which is a waste of cpu cycles and
cache lines. Therefore reduce the default length to L1_CACHE_BYTES
(256 bytes), which also reduces the average execution time of
strncpy_from_user() by 30-40%.

Alternatively we could have switched to the generic
strncpy_from_user() implementation, however it turned out that that
variant would be slower than the now optimized s390 variant.
