This passes on the scsi_cmnd result field to users of passthrough
requests. Currently we abuse req->errors for this purpose, but that
field will go away in its current form.
Note that the old IDE code abuses the errors field in very creative
ways and stores all kinds of different values in it. I didn't dare
to touch this magic, so the abuses are brought forward 1:1.

And require all drivers that want to support BLOCK_PC to allocate it
as the first thing of their private data. To support this the legacy
IDE and BSG code is switched to set cmd_size on their queues to let
the block layer allocate the additional space.

Export fc_bsg_jobdone so drivers can use it directly instead of doing
the round-trip via struct fc_bsg_job::job_done() and use it in the
LLDDs. That way we can also unify the interfaces of fc_bsg_jobdone and
bsg_job_done.
As we've converted all LLDDs over to use fc_bsg_jobdone() directly, we
can remove the function pointer from struct fc_bsg_job as well.

Earlier IDC-TIME-EXTEND aen was a nop and ignored by driver.
We now have to handle the aen so that other protocol drivers can use time
extension during some loopback operations.

The patch changes a specifier used to output some variables. Instead of using
stack for each byte the '%*ph[CN]' allows to take a one pointer and prints
entire buffer as a hexadecimal string with the separator ':' or ''.

Avoid that the fcport structure gets leaked if
bsg_job->request->msgcode == FC_BSG_HST_ELS_NOLOGIN, the fcport
allocation succeeds and the !vha->flags.online branch is taken.
This was detected by Coverity. However, Coverity does not recognize
that all qla2x00_process_els() callers specify either
FC_BSG_RPT_ELS or FC_BSG_HST_ELS_NOLOGIN in the field
bsg_job->request->msgcode and that the value of that field is not
modified inside that function. This results in a false positive
report about a possible memory leak in an error path for
bsg_job->request->msgcode values other than the two mentioned
values. Make it easy for Coverity (and for humans) to recognize
that there is no fcport leak in the error path by changing the
bsg_job->request->msgcode == FC_BSG_HST_ELS_NOLOGIN test into
bsg_job->request->msgcode != FC_BSG_RPT_ELS.

If we need to do a chip reset because of a serious loopback error don't try to
reset the loopback mode on the port as the mailbox command will timeout.

Add the following error handling for loopback diagnostic mode with ISP83xx:
1. If we do not receive an MBA_DCBX_COMPLETE after our initial set port
configuration command, try to reset the port back into normal operation.
If that fails, take a FCoE dump and then reset the chip.
2. After completing the loopback diagnostic operation, if the reset of the port
back into normal operation fails then reset the port so we take a FCoE dump
and then reset the chip.
3. When we receive an IDC notification and the requested operation is loopback
extend the loop down timer so the link does not appear to down for an
extended period of time.

FC transport on receiving bsg_job submission failure, calls bsg_job->job_done()
and sets the bsg_job->reply->result the returned value. In contrast, when the
success code (0) is returned fc transport doesn't call bsg_job->job_done() and
doesn't populate bsg_job->reply->result.
