Remove mmiowb() from the kernel memory barrier API and instead, for

The only relatively recent changes have been addressing review
comments on the documentation, which is in a much better shape thanks
to the efforts of Ben and Ingo.
I was initially planning to split this into two pull requests so that
you could run the coccinelle script yourself, however it's been plain

mmiowb() is now implied by spin_unlock() on architectures that require
it, so there is no reason to call it from driver code. This patch was
generated using coccinelle:
@mmiowb@
@@
- mmiowb();
and invoked as:
$ for d in drivers include/linux/qed sound; do \
spatch --include-headers --sp-file mmiowb.cocci --dir $d --in-place; done
NOTE: mmiowb() has only ever guaranteed ordering in conjunction with
spin_unlock(). However, pairing each mmiowb() removal in this patch with
the corresponding call to spin_unlock() is not at all trivial, so there
is a small chance that this change may regress any drivers incorrectly
relying on mmiowb() to order MMIO writes between CPUs using lock-free
synchronisation. If you've ended up bisecting to this commit, you can
reintroduce the mmiowb() calls using wmb() instead, which should restore
the old behaviour on all architectures other than some esoteric ia64
systems.

Currently variable data0 is not being initialized so a garbage value is
being passed to vxge_hw_vpath_fw_api and this value is being written to
the rts_access_steer_data0 register. There are other occurrances where
data0 is being initialized to zero (e.g. in function
vxge_hw_upgrade_read_version) so I think it makes sense to ensure data0
is initialized likewise to 0.

Neterion was standalone for several years, then acquired by Exar and
shutdown in 11 months without ever making any new Exar branded adapters.
Users would probably think of them as Neterion and not Exar (as there
have been no follow-on adapters and the vast majority ever sold were
under the Neterion name).
6c541b4595 ("net: ethernet: Sort Kconfig sourcing alphabetically")
sorted Kconfig sourcing based on directory names, but in a couple cases,
the menu item text is quite different from the directory name and is not
sorted correctly:
drivers/net/ethernet/neterion/Kconfig => "Exar devices"
To address that and clear up any confusion about the name, "Exar" was
changed to "Neterion (Exar)" and the relevant entries in the Makefile
and Kconfig were reordered to match the alphabetical organization.

Please do not apply this to mainline directly, instead please re-run the
coccinelle script shown below and apply its output.
For several reasons, it is desirable to use {READ,WRITE}_ONCE() in
preference to ACCESS_ONCE(), and new code is expected to use one of the
former. So far, there's been no reason to change most existing uses of
ACCESS_ONCE(), as these aren't harmful, and changing them results in
churn.
However, for some features, the read/write distinction is critical to
correct operation. To distinguish these cases, separate read/write
accessors must be used. This patch migrates (most) remaining
ACCESS_ONCE() instances to {READ,WRITE}_ONCE(), using the following
coccinelle script:
----
// Convert trivial ACCESS_ONCE() uses to equivalent READ_ONCE() and
// WRITE_ONCE()

We could add another #ifdef here, but it's nicer to avoid those warnings
for good by converting the existing #ifdef to if(IS_ENABLED()), which has
the same effect but provides better compile-time coverage in general,
and lets the compiler understand better when the function is intentionally
unused.
