b67a95f2ab powerpc/xive: Skip ioremap() of ESB pages for LSI interrupts
The PCI INTx interrupts and other LSI interrupts are handled differently
under a sPAPR platform. When the interrupt source characteristics are
queried, the hypervisor returns an H_INT_ESB flag to inform the OS
that it should be using the H_INT_ESB hcall for interrupt management
and not loads and stores on the interrupt ESB pages.

A default -1 value is returned for the addresses of the ESB pages. The
driver ignores this condition today and performs a bogus IO mapping.
Recent changes and the DEBUG_VM configuration option make the bug

1ca3dec2b2 powerpc/xive: Prevent page fault issues in the machine crash handler
When the machine crash handler is invoked, all interrupts are masked
but interrupts which have not been started yet do not have an ESB page
mapped in the Linux address space. This crashes the 'crash kexec'
sequence on sPAPR guests.

To fix, force the mapping of the ESB page when an interrupt is being
mapped in the Linux IRQ number space. This is done by setting the
initial state of the interrupt to OFF which is not necessarily the
case on PowerNV.

855d9140a3 powerpc/xmon: Fix output of XIVE IPI
When dumping the XIVE state of an CPU IPI, xmon does not check if the
CPU is started or not which can cause an error. Add a check for that
and change the output to be on one line just as the XIVE interrupts of
the machine.

5896163f7f powerpc/xmon: Improve output of XIVE interrupts
When looping on the list of interrupts, add the current value of the
PQ bits with a load on the ESB page. This has the side effect of
faulting the ESB page of all interrupts.

6ccb4ac2bf powerpc/xive: Fix bogus error code returned by OPAL
There's a bug in skiboot that causes the OPAL_XIVE_ALLOCATE_IRQ call
to return the 32-bit value 0xffffffff when OPAL has run out of IRQs.
Unfortunatelty, OPAL return values are signed 64-bit entities and
errors are supposed to be negative. If that happens, the linux code
confusingly treats 0xffffffff as a valid IRQ number and panics at some
point.

A fix was recently merged in skiboot:

e97391ae2bb5 ("xive: fix return value of opal_xive_allocate_irq()")

but we need a workaround anyway to support older skiboots already
in the field.

Internally convert 0xffffffff to OPAL_RESOURCE which is the usual error
returned upon resource exhaustion.

2ad7a27dea KVM: PPC: Book3S: Enable XIVE native capability only if OPAL has required functions
There are some POWER9 machines where the OPAL firmware does not support
the OPAL_XIVE_GET_QUEUE_STATE and OPAL_XIVE_SET_QUEUE_STATE calls.
The impact of this is that a guest using XIVE natively will not be able
to be migrated successfully.  On the source side, the get_attr operation
on the KVM native device for the KVM_DEV_XIVE_GRP_EQ_CONFIG attribute
will fail; on the destination side, the set_attr operation for the same
attribute will fail.

This adds tests for the existence of the OPAL get/set queue state
functions, and if they are not supported, the XIVE-native KVM device
is not created and the KVM_CAP_PPC_IRQ_XIVE capability returns false.
Userspace can then either provide a software emulation of XIVE, or
else tell the guest that it does not have a XIVE controller available
to it.

b4868ff55d powerpc/xive: Fix dump of XIVE interrupt under pseries
The xmon 'dxi' command calls OPAL to query the XIVE configuration of a
interrupt. This can only be done on baremetal (PowerNV) and it will
crash a pseries machine.

Introduce a new XIVE get_irq_config() operation which implements a
different query depending on the platform, PowerNV or pseries, and
modify xmon to use a top level wrapper.

da15c03b04 powerpc/xive: Implement get_irqchip_state method for XIVE to fix shutdown race
Testing has revealed the existence of a race condition where a XIVE
interrupt being shut down can be in one of the XIVE interrupt queues
(of which there are up to 8 per CPU, one for each priority) at the
point where free_irq() is called.  If this happens, can return an
interrupt number which has been shut down.  This can lead to various
symptoms:

- irq_to_desc(irq) can be NULL.  In this case, no end-of-interrupt
  function gets called, resulting in the CPU's elevated interrupt
  priority (numerically lowered CPPR) never gets reset.  That then
  means that the CPU stops processing interrupts, causing device
  timeouts and other errors in various device drivers.

- The irq descriptor or related data structures can be in the process
  of being freed as the interrupt code is using them.  This typically
  leads to crashes due to bad pointer dereferences.

This race is basically what commit 62e0468650 ("genirq: Add optional
hardware synchronization for shutdown", 2019-06-28) is intended to
fix, given a get_irqchip_state() method for the interrupt controller
being used.  It works by polling the interrupt controller when an
interrupt is being freed until the controller says it is not pending.

With XIVE, the PQ bits of the interrupt source indicate the state of
the interrupt source, and in particular the P bit goes from 0 to 1 at
the point where the hardware writes an entry into the interrupt queue
that this interrupt is directed towards.  Normally, the code will then
process the interrupt and do an end-of-interrupt (EOI) operation which
will reset PQ to 00 (assuming another interrupt hasn't been generated
in the meantime).  However, there are situations where the code resets
P even though a queue entry exists (for example, by setting PQ to 01,
which disables the interrupt source), and also situations where the
code leaves P at 1 after removing the queue entry (for example, this
is done for escalation interrupts so they cannot fire again until
they are explicitly re-enabled).

The code already has a 'saved_p' flag for the interrupt source which
indicates that a queue entry exists, although it isn't maintained
consistently.  This patch adds a 'stale_p' flag to indicate that
P has been left at 1 after processing a queue entry, and adds code
to set and clear saved_p and stale_p as necessary to maintain a
consistent indication of whether a queue entry may or may not exist.

With this, we can implement xive_get_irqchip_state() by looking at
stale_p, saved_p and the ESB PQ bits for the interrupt.

There is some additional code to handle escalation interrupts
properly; because they are enabled and disabled in KVM assembly code,
which does not have access to the xive_irq_data struct for the
escalation interrupt.  Hence, stale_p may be incorrect when the
escalation interrupt is freed in kvmppc_xive_{,native_}cleanup_vcpu().
Fortunately, we can fix it up by looking at vcpu->arch.xive_esc_on,
with some careful attention to barriers in order to ensure the correct
result if xive_esc_irq() races with kvmppc_xive_cleanup_vcpu().

Finally, this adds code to make noise on the console (pr_crit and
WARN_ON(1)) if we find an interrupt queue entry for an interrupt
which does not have a descriptor.  While this won't catch the race
reliably, if it does get triggered it will be an indication that
the race is occurring and needs to be debugged.

xive_find_target_in_mask() has the following for(;;) loop which has a
bug when @first == cpumask_first(@mask) and condition 1 fails to hold
for every CPU in @mask. In this case we loop forever in the for-loop.

This is because, when @first == cpumask_first(@mask), we never hit the
condition 2 (cpu == first) since prior to this check, we would have
executed "cpu = cpumask_next(cpu, mask)" which will set the value of
@cpu to a value greater than @first or to nr_cpus_ids. When this is
coupled with the fact that condition 1 is not met, we will never exit
this loop.

This was discovered by the hard-lockup detector while running LTP test
concurrently with SMT switch tests.

To fix this, move the check for condition 2 after the check for
condition 3, so that we are able to break out of the loop soon after
iterating through all the CPUs in the @mask in the problem case. Use
do..while() to achieve this.

a3bf9fbdad powerpc/pseries: Fix xive=off command line
On POWER9, if the hypervisor supports XIVE exploitation mode, the
guest OS will unconditionally requests for the XIVE interrupt mode
even if XIVE was deactivated with the kernel command line xive=off.
Later on, when the spapr XIVE init code handles xive=off, it disables
XIVE and tries to fall back on the legacy mode XICS.

This discrepency causes a kernel panic because the hypervisor is
configured to provide the XIVE interrupt mode to the guest :

Look for xive=off during prom_init and don't ask for XIVE in this
case. One exception though: if the host only supports XIVE, we still
want to boot so we ignore xive=off.

supports XIVE.

39e9af3de5 KVM: PPC: Book3S HV: XIVE: Add a TIMA mapping
Each thread has an associated Thread Interrupt Management context
composed of a set of registers. These registers let the thread handle
priority management and interrupt acknowledgment. The most important
are :

    - Interrupt Pending Buffer     (IPB)
    - Current Processor Priority   (CPPR)
    - Notification Source Register (NSR)

They are exposed to software in four different pages each proposing a
view with a different privilege. The first page is for the physical
thread context and the second for the hypervisor. Only the third
(operating system) and the fourth (user level) are exposed the guest.

A custom VM fault handler will populate the VMA with the appropriate
pages, which should only be the OS page for now.

23ad1a2700 powerpc: Add -Werror at arch/powerpc level
Back when I added -Werror in commit ba55bd7436 ("powerpc: Add
configurable -Werror for arch/powerpc") I did it by adding it to most
of the arch Makefiles.

At the time we excluded math-emu, because apparently it didn't build
cleanly. But that seems to have been fixed somewhere in the interim.

So move the -Werror addition to the top-level of the arch, this saves
us from repeating it in every Makefile and means we won't forget to
add it to any new sub-dirs.

'default n' is the default value for any bool or tristate Kconfig
setting so there is no need to write it explicitly.

Also since commit f467c5640c ("kconfig: only write '# CONFIG_FOO
is not set' for visible symbols") the Kconfig behavior is the same
regardless of 'default n' being present or not:

    One side effect of (and the main motivation for) this change is making
    the following two definitions behave exactly the same:

                default n

    With this change, neither of these will generate a
    '# CONFIG_FOO is not set' line (assuming FOO isn't selected/implied).
    That might make it clearer to people that a bare 'default n' is
    redundant.

8ac9e5bfd8 powerpc/xive: Use xive_cpu->chip_id instead of looking it up again
Function xive_native_get_ipi() might use chip_id without it being
initialized, if the CPU node is not found, as reported by smatch:

  error: uninitialized symbol 'chip_id'

As suggested by Cédric, we can use xc->chip_id instead of consulting
the device tree for chip id, which is safe since xive_prepare_cpu()
should have initialized ->chip_id by the time xive_native_get_ipi() is
called.

patch replaces cases of:

8a792262f3 powerpc/xive: Remove (almost) unused macros
The GETFIELD and SETFIELD macros in xive-regs.h aren't used except for
a single instance of GETFIELD, so replace that and remove them.

These macros are also defined in vas.h, so either those should be
eventually replaced or the macros moved into bitops.h.

282498d65f powerpc/xive: prepare all hcalls to support long busy delays
This is not the case for the moment, but future releases of pHyp might
need to introduce some synchronisation routines under the hood which
would make the XIVE hcalls longer to complete.

As this was done for H_INT_RESET, let's wrap the other hcalls in a
loop catching the H_LONG_BUSY_* codes.

b32e56e5a8 powerpc/xive: Fix trying to "push" an already active pool VP
When setting up a CPU, we "push" (activate) a pool VP for it.

However it's an error to do so if it already has an active
pool VP.

This happens when doing soft CPU hotplug on powernv since we
don't tear down the CPU on unplug. The HW flags the error which
gets captured by the diagnostics.

Fix this by making sure to "pull" out any already active pool
first.

8e036c8d30 powerpc/xive: Use hw CPU ids when configuring the CPU queues
The CPU event notification queues on sPAPR should be configured using
a hardware CPU identifier.

The problem did not show up on the Power Hypervisor because pHyp
supports 8 threads per core which keeps CPU number contiguous. This is
not the case on all sPAPR virtual machines, some use SMT=1.

Also improve error logging by adding the CPU number.

bf4159da47 KVM: PPC: Book3S HV: Enable use of the new XIVE "single escalation" feature
That feature, provided by Power9 DD2.0 and later, when supported
by newer OPAL versions, allows us to sacrifice a queue (priority 7)
in favor of merging all the escalation interrupts of the queues
of a single VP into a single interrupt.

This reduces the number of host interrupts used up by KVM guests
especially when those guests use multiple priorities.

It will also enable a future change to control the masking of the
escalation interrupts more precisely to avoid spurious ones.

b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

cc56939802 powerpc/xive: Clear XIVE internal structures when a CPU is removed
Commit eac1e731b5 ("powerpc/xive: guest exploitation of the XIVE
interrupt controller") introduced support for the XIVE exploitation
mode of the P9 interrupt controller on the pseries platform.

At that time, support for CPU removal was not complete on PowerVM and
CPU hot unplug remained untested. It appears that some cleanups of the
XIVE internal structures are required before releasing the CPU,
without which the kernel crashes in a RTAS call doing the CPU
isolation.

These changes fix the crash by deconfiguring the IPI interrupt source
and clearing the event queues of the CPU when it is removed.

9b130ad5bb treewide: make "nr_cpu_ids" unsigned
First, number of CPUs can't be negative number.

Second, different signnnedness leads to suboptimal code in the following
cases:

1)
	kmalloc(nr_cpu_ids * sizeof(X));

"int" has to be sign extended to size_t.

2)
	while (loff_t *pos < nr_cpu_ids)

MOVSXD is 1 byte longed than the same MOV.

Other cases exist as well. Basically compiler is told that nr_cpu_ids
can't be negative which can't be deduced if it is "int".

Code savings on allyesconfig kernel: -3KB

5f121292f0 powerpc/xive: improve debugging macros
Having the CPU identifier in the debug logs is helpful when tracking
issues. Also add some more logging and fix a compile issue in
