mach/timex.h is only supposed to contain a definition for
CLOCK_TICK_RATE. Remove additional includes, and move private
definitions to a private header file.

Remove includes of asm/hardware.h in addition to asm/arch/hardware.h.
Then, since asm/hardware.h only exists to include asm/arch/hardware.h,
update everything to directly include asm/arch/hardware.h and remove
asm/hardware.h.

Move the xtime write mode seqlock into timer_tick(), so it only
surrounds the call to do_timer().
This avoids a deadlock in update_process_times() ...
hrtimer_get_softirq_time() which tries to get a read mode seqlock
on xtime, thereby preventing booting.
