A long time ago, in v2.4, VM_RESERVED kept swapout process off VMA,
currently it lost original meaning but still has some effects:

This patch removes reserved_vm counter from mm_struct. Seems like nobody
cares about it, it does not exported into userspace directly, it only

Thus VM_RESERVED can be replaced with VM_IO or pair VM_DONTEXPAND | VM_DONTDUMP.
remap_pfn_range() and io_remap_pfn_range() set VM_IO|VM_DONTEXPAND|VM_DONTDUMP.
remap_vmalloc_range() set VM_DONTEXPAND | VM_DONTDUMP.

ARMv7 platforms.
The goal in implementing this architecture is to exploit the hardware
as much as possible. That means use as little as possible of PV
operations (so no PV MMU) - and use existing PV drivers for I/Os
(network, block, console, etc). This is similar to how PVHVM guests
operate in X86 platform nowadays - except that on ARM there is no need
for QEMU. The end result is that we share a lot of the generic Xen
drivers and infrastructure.

Register pfn_is_ram helper speed up reading /proc/vmcore in the kdump
kernel. See commit message of 997c136f51 ("fs/proc/vmcore.c: add hook
to read_from_oldmem() to check for non-ram pages") for details.
It makes use of a new hvmop HVMOP_get_mem_type which was introduced in
xen 4.2 (23298:26413986e6e0) and backported to 4.1.1.
The new function is currently only enabled for reading /proc/vmcore.
Later it will be used also for the kexec kernel. Since that requires
more changes in the generic kernel make it static for the time being.

In order to add xen EFI frambebuffer video support, it is required to add
xen-efi's new video type (XEN_VGATYPE_EFI_LFB) case and handle it in the
function xen_init_vga and set the video type to VIDEO_TYPE_EFI to enable
efi video mode.

The xen c/s 25873 allows the hypervisor to retrieve the NUMLOCK flag.
With this patch, the Linux kernel can get the state according to the
data in the BIOS.

The hypervisor is in charge of allocating the proper "NUMA" memory
and dealing with the CPU scheduler to keep them bound to the proper
NUMA node. The PV guests (and PVHVM) have no inkling of where they
run and do not need to know that right now. In the future we will
need to inject NUMA configuration data (if a guest spans two or more
NUMA nodes) so that the kernel can make the right choices. But those
patches are not yet present.
In the meantime, disable the NUMA capability in the PV guest, which
also fixes a bootup issue. Andre says:
"we see Dom0 crashes due to the kernel detecting the NUMA topology not
by ACPI, but directly from the northbridge (CONFIG_AMD_NUMA).
This will detect the actual NUMA config of the physical machine, but
will crash about the mismatch with Dom0's virtual memory. Variation of
the theme: Dom0 sees what it's not supposed to see.
This happens with the said config option enabled and on a machine where
this scanning is still enabled (K8 and Fam10h, not Bulldozer class)

As the initial domain we are able to search/map certain regions
of memory to harvest configuration data. For all low-level we
use ACPI tables - for interrupts we use exclusively ACPI _PRT
(so DSDT) and MADT for INT_SRC_OVR.
The SMP MP table is not used at all. As a matter of fact we do
not even support machines that only have SMP MP but no ACPI tables.
Lets follow how Moorestown does it and just disable searching
for BIOS SMP tables.
This also fixes an issue on HP Proliant BL680c G5 and DL380 G6:

which is that ioremap would end up mapping 0xff using _PAGE_IOMAP
(which is what early_ioremap sticks as a flag) - which meant
we would get MFN 0xFF (pte ff461, which is OK), and then it would
also map 0x100 (b/c ioremap tries to get page aligned request, and
it was trying to map 0xf4fa0 + PAGE_SIZE - so it mapped the next page)
as _PAGE_IOMAP. Since 0x100 is actually a RAM page, and the _PAGE_IOMAP
bypasses the P2M lookup we would happily set the PTE to 1000461.
Xen would deny the request since we do not have access to the
Machine Frame Number (MFN) of 0x100. The P2M[0x100] is for example
0x80140.

With this patch we provide the functionality to initialize the
Xen-SWIOTLB late in the bootup cycle - specifically for
Xen PCI-frontend. We still will work if the user had
supplied 'iommu=soft' on the Linux command line.
Note: We cannot depend on after_bootmem to automatically
determine whether this is early or not. This is because
when PCI IOMMUs are initialized it is after after_bootmem but
before a lot of "other" subsystems are initialized.

If the caller passes a valid kmap_op to m2p_add_override, we use
kmap_op->dev_bus_addr to store the original mfn, but dev_bus_addr is
part of the interface with Xen and if we are batching the hypercalls it
might not have been written by the hypervisor yet. That means that later
on Xen will write to it and we'll think that the original mfn is
actually what Xen has written to it.
Rather than "stealing" struct members from kmap_op, keep using
page->index to store the original mfn and add another parameter to
m2p_remove_override to get the corresponding kmap_op instead.
It is now responsibility of the caller to keep track of which kmap_op
corresponds to a particular page in the m2p_override (gntdev, the only
user of this interface that passes a valid kmap_op, is already doing that).

At this stage x86_init.paging.pagetable_setup_done is only used in the
XEN case. Move its content in the x86_init.paging.pagetable_init setup

Callers of xen_remap_domain_range() need to know if the remap failed
because frame is currently paged out. So they can retry the remap
later on. Return -ENOENT in this case.
This assumes that the error codes returned by Xen are a subset of
those used by the kernel. It is unclear if this is defined as part of
the hypercall ABI.

While TLB_FLUSH_ALL gets passed as 'end' argument to
flush_tlb_others(), the Xen code was made to check its 'start'
parameter. That may give a incorrect op.cmd to MMUEXT_INVLPG_MULTI
instead of MMUEXT_TLB_FLUSH_MULTI. Then it causes some page can not
be flushed from TLB.
This patch fixed this issue.

We would traverse the full P2M top directory (from 0->MAX_DOMAIN_PAGES
inclusive) when trying to figure out whether we can re-use some of the
P2M middle leafs.
Which meant that if the kernel was compiled with MAX_DOMAIN_PAGES=512
we would try to use the 512th entry. Fortunately for us the p2m_top_index
has a check for this:

If the P2M revectoring would fail, we would try to continue on by
cleaning the PMD for L1 (PTE) page-tables. The xen_cleanhighmap
is greedy and erases the PMD on both boundaries. Since the P2M
array can share the PMD, we would wipe out part of the __ka
that is still used in the P2M tree to point to P2M leafs.
This fixes it by bypassing the revectoring and continuing on.
If the revector fails, a nice WARN is printed so we can still
troubleshoot this.

We end up having the P2M array (that is the one that was
grafted on the P2M tree) filled with IDENTITY_FRAME or
INVALID_P2M_ENTRY) entries. The patch titled
"xen/p2m: Reuse existing P2M leafs if they are filled with 1:1 PFNs or INVALID."
recycles said slots and replaces the P2M tree leaf's with
&mfn_list[xx] with p2m_identity or p2m_missing.
And re-uses the P2M array sections for other P2M tree leaf's.
For the above mentioned bootup excerpt, the PFNs at
0x20000->0x20200 are going to be IDENTITY based:
P2M[0][256][0] -> P2M[0][257][0] get turned in IDENTITY_FRAME.
We can re-use that and replace P2M[0][256] to point to p2m_identity.
The "old" page (the grafted P2M array provided by Xen) that was at
P2M[0][256] gets put somewhere else. Specifically at P2M[6][358],
b/c when we populate back:
Populating 1acb8a-1f20e9 pfn range: 283999 pages added
we fill P2M[6][358][0] (and P2M[6][358], P2M[6][359], ...) with
the new MFNs.
That is all OK, except when we revector we assume that the PFN
count would be the same in the grafted P2M array and in the
newly allocated. Since that is no longer the case, as we have
holes in the P2M that point to p2m_missing or p2m_identity we
have to take that into account.

We call memblock_reserve for [start of mfn list] -> [PMD aligned end
of mfn list] instead of <start of mfn list> -> <page aligned end of mfn list].
This has the disastrous effect that if at bootup the end of mfn_list is
not PMD aligned we end up returning to memblock parts of the region
past the mfn_list array. And those parts are the PTE tables with
the disastrous effect of seeing this at bootup:

P2M tree was using) is partially disassembled. The cleanup_highmap
has removed the PMD entries from 0-16MB and anything past _brk_end
up to the max_pfn_mapped (which is the end of the ramdisk).
The xen_remove_p2m_tree and code around has ripped out the __ka for
the old P2M array.
Here we continue on doing it to where the Xen page-tables were.
It is safe to do it, as the page-tables are addressed using __va.
For good measure we delete anything that is within MODULES_VADDR
and up to the end of the PMD.
At this point the __ka only contains PMD entries for the start
of the kernel up to __brk.

The 'xen_revector_p2m_tree()' function allocates a new P2M tree
copies the contents of the old one in it, and returns the new one.
At this stage, the __ka address space (which is what the old
P2M tree was using) is partially disassembled. The cleanup_highmap
has removed the PMD entries from 0-16MB and anything past _brk_end
up to the max_pfn_mapped (which is the end of the ramdisk).
We have revectored the P2M tree (and the one for save/restore as well)
to use new shiny __va address to new MFNs. The xen_start_info
has been taken care of already in 'xen_setup_kernel_pagetable()' and
xen_start_info->shared_info in 'xen_setup_shared_info()', so
we are free to roam and delete PMD entries - which is exactly what
we are going to do. We rip out the __ka for the old P2M array.

Since the __va and __ka are 1:1 up to MODULES_VADDR and
cleanup_highmap rids __ka of the ramdisk mapping, what
we want to do is similar - get rid of the P2M in the __ka
address space. There are two ways of fixing this:
1) All P2M lookups instead of using the __ka address would
use the __va address. This means we can safely erase from
__ka space the PMD pointers that point to the PFNs for
P2M array and be OK.
2). Allocate a new array, copy the existing P2M into it,
revector the P2M tree to use that, and return the old
P2M to the memory allocate. This has the advantage that
it sets the stage for using XEN_ELF_NOTE_INIT_P2M
feature. That feature allows us to set the exact virtual
address space we want for the P2M - and allows us to
boot as initial domain on large machines.
So we pick option 2).
This patch only lays the groundwork in the P2M code. The patch
that modifies the MMU is called "xen/mmu: Copy and revector the P2M tree."

us with all the initial memory mapped that we need to function.
The initial memory mapped is up to the bootstack, which means
we can reference using __ka up to 4.f):

Which is that the level2_kernel_pgt (__ka virtual addresses)
and level2_ident_pgt (__va virtual address) contain the same
PMD entries. So if you modify a PTE in __ka, it will be reflected
in __va (and vice-versa).

is that we tried to make the the memblock_reserve more selective so
that it would be clear what region is reserved. Sadly we ran
in the problem wherein on a 64-bit hypervisor with a 32-bit
initial domain, the pt_base has the cr3 value which is not
neccessarily where the pagetable starts! As Jan put it: "
Actually, the adjustment turns out to be correct: The page
tables for a 32-on-64 dom0 get allocated in the order "first L1",
"first L2", "first L3", so the offset to the page table base is
indeed 2. When reading xen/include/public/xen.h's comment
very strictly, this is not a violation (since there nothing is said
that the first thing in the page table space is pointed to by
pt_base; I admit that this seems to be implied though, namely
do I think that it is implied that the page table space is the
range [pt_base, pt_base + nt_pt_frames), whereas that
range here indeed is [pt_base - 2, pt_base - 2 + nt_pt_frames),
which - without a priori knowledge - the kernel would have
difficulty to figure out)." - so lets just fall back to the
easy way and reserve the whole region.

This patch removes the "return -ENOSYS" for auto_translated_physmap
guests from privcmd_mmap, thus it allows ARM guests to issue privcmd
mmap calls. However privcmd mmap calls are still going to fail for HVM
and hybrid guests on x86 because the xen_remap_domain_mfn_range
implementation is currently PV only.

When we are finished with return PFNs to the hypervisor, then
populate it back, and also mark the E820 MMIO and E820 gaps
as IDENTITY_FRAMEs, we then call P2M to set areas that can
be used for ballooning. We were off by one, and ended up
over-writting a P2M entry that most likely was an IDENTITY_FRAME.
For example:

We still patch SMP instructions to UP variants if we boot with a
single CPU, but not at any other time. In particular, not if we
unplug CPUs to return to a single cpu.
Paul McKenney points out:
mean offline overhead is 6251/48=130.2 milliseconds.
If I remove the alternatives_smp_switch() from the offline
path [...] the mean offline overhead is 550/42=13.1 milliseconds
Basically, we're never going to get those 120ms back, and the
code is pretty messy.
We get rid of:
1) The "smp-alt-once" boot option. It's actually "smp-alt-boot", the
documentation is wrong. It's now the default.
2) The skip_smp_alternatives flag used by suspend.
3) arch_disable_nonboot_cpus_begin() and arch_disable_nonboot_cpus_end()
which were only used to set this one flag.

If a PV guest is booted the native SWIOTLB should not be
turned on. It does not help us (we don't have any PCI devices)
and it eats 64MB of good memory. In the case of PV guests
with PCI devices we need the Xen-SWIOTLB one.

Its pretty easy:
1). We only check to see if we need Xen SWIOTLB for PV guests.
2). If swiotlb=force or iommu=soft is set, then Xen SWIOTLB will
be enabled.
3). If it is an initial domain, then Xen SWIOTLB will be enabled.
4). Native SWIOTLB must be disabled for PV guests.

If a 64-bit hypervisor is booted with a 32-bit initial domain,
the hypervisor deals with the initial domain as "compat" and
does some extra adjustments (like pagetables are 4 bytes instead
of 8). It also adjusts the xen_start_info->pt_base incorrectly.

If P2M leaf is completly packed with INVALID_P2M_ENTRY or with
1:1 PFNs (so IDENTITY_FRAME type PFNs), we can swap the P2M leaf
with either a p2m_missing or p2m_identity respectively. The old
page (which was created via extend_brk or was grafted on from the
mfn_list) can be re-used for setting new PFNs.
This also means we can remove git commit:

and make the amount that is required to be reserved much smaller.

Currently kexec in a PVonHVM guest fails with a triple fault because the
new kernel overwrites the shared info page. The exact failure depends on
the size of the kernel image. This patch moves the pfn from RAM into
MMIO space before the kexec boot.
The pfn containing the shared_info is located somewhere in RAM. This
will cause trouble if the current kernel is doing a kexec boot into a
new kernel. The new kernel (and its startup code) can not know where the
pfn is, so it can not reserve the page. The hypervisor will continue to
update the pfn, and as a result memory corruption occours in the new
kernel.
One way to work around this issue is to allocate a page in the
xen-platform pci device's BAR memory range. But pci init is done very
late and the shared_info page is already in use very early to read the
pvclock. So moving the pfn from RAM to MMIO is racy because some code
paths on other vcpus could access the pfn during the small window when
the old pfn is moved to the new pfn. There is even a small window were
the old pfn is not backed by a mfn, and during that time all reads
return -1.
Because it is not known upfront where the MMIO region is located it can
not be used right from the start in xen_hvm_init_shared_info.
To minimise trouble the move of the pfn is done shortly before kexec.
This does not eliminate the race because all vcpus are still online when
the syscore_ops will be called. But hopefully there is no work pending
at this point in time. Also the syscore_op is run last which reduces the
risk further.

init_hvm_pv_info does not fail, make it a void function.
remove arguments from init_hvm_pv_info because they are not used by the

When switching tasks in a Xen PV guest, avoid updating the TLS
descriptors if they haven't changed. This improves the speed of
context switches by almost 10% as much of the time the descriptors are
the same or only one is different.
The descriptors written into the GDT by Xen are modified from the
values passed in the update_descriptor hypercall so we keep shadow
copies of the three TLS descriptors to compare against.

When constructing the initial page tables, if the MFN for a usable PFN
is missing in the p2m then that frame is initially ballooned out. In
this case, zero the PTE (as in decrease_reservation() in
drivers/xen/balloon.c).
This is obviously safe instead of having an valid PTE with an MFN of
INVALID_P2M_ENTRY (~0).

In xen_set_pte() if batching is unavailable (because the caller is in
an interrupt context such as handling a page fault) it would fall back
to using native_set_pte() and trapping and emulating the PTE write.
On 32-bit guests this requires two traps for each PTE write (one for
each dword of the PTE). Instead, do one mmu_update hypercall
directly.
During construction of the initial page tables, continue to use
native_set_pte() because most of the PTEs being set are in writable
and unpinned pages (see phys_pmd_init() in arch/x86/mm/init_64.c) and
using a hypercall for this is very expensive.
This significantly improves page fault performance in 32-bit PV
guests.

When MCA error occurs, it would be handled by Xen hypervisor first,
and then the error information would be sent to initial domain for logging.
This patch gets error information from Xen hypervisor and convert
Xen format error into Linux format mcelog. This logic is basically
self-contained, not touching other kernel components.
By using tools like mcelog tool users could read specific error information,
like what they did under native Linux.

x86 has no flush_tlb_range support in instruction level. Currently the
flush_tlb_range just implemented by flushing all page table. That is not
the best solution for all scenarios. In fact, if we just use 'invlpg' to
flush few lines from TLB, we can get the performance gain from later
remain TLB lines accessing.
But the 'invlpg' instruction costs much of time. Its execution time can
compete with cr3 rewriting, and even a bit more on SNB CPU.
So, on a 512 4KB TLB entries CPU, the balance points is at:
(512 - X) * 100ns(assumed TLB refill cost) =
X(TLB flush entries) * 100ns(assumed invlpg cost)
Here, X is 256, that is 1/2 of 512 entries.
But with the mysterious CPU pre-fetcher and page miss handler Unit, the
assumed TLB refill cost is far lower then 100ns in sequential access. And
2 HT siblings in one core makes the memory access more faster if they are
accessing the same memory. So, in the patch, I just do the change when
the target entries is less than 1/16 of whole active tlb entries.
Actually, I have no data support for the percentage '1/16', so any
suggestions are welcomed.
As to hugetlb, guess due to smaller page table, and smaller active TLB
entries, I didn't see benefit via my benchmark, so no optimizing now.
My micro benchmark show in ideal scenarios, the performance improves 70
percent in reading. And in worst scenario, the reading/writing
performance is similar with unpatched 3.4-rc4 kernel.

- When booting as PVHVM we would try to use PV console - but would not validate
the parameters causing us to crash during restore b/c we re-use the wrong event
channel.
- When booting on machines with SR-IOV PCI bridge we didn't check for the bridge
and tried to use it.
- Under AMD machines would advertise the APERFMPERF resulting in needless amount
of MSRs from the guest.
- A global value (xen_released_pages) was not subtracted at bootup when pages
were added back in. This resulted in the balloon worker having the wrong
account of how many pages were truly released.
- Fix dead-lock when xen-blkfront is run in the same domain as xen-blkback.

When the frontend and the backend reside on the same domain, even if we
add pages to the m2p_override, these pages will never be returned by
mfn_to_pfn because the check "get_phys_to_machine(pfn) != mfn" will

The explanation is in the comment within the code:
We need to do this because the pages shared by the frontend
(xen-blkfront) can be already locked (lock_page, called by
do_read_cache_page); when the userspace backend tries to use them
with direct_IO, mfn_to_pfn returns the pfn of the frontend, so
do_blockdev_direct_IO is going to try to lock the same pages
again resulting in a deadlock.

Internally the xen-blkback uses m2p_add_override to swizzle (temporarily)
a 'struct page' to have a different MFN (so that it can point to another
guest). It also can easily find out whether another pfn corresponding
to the mfn exists in the m2p, and can set the FOREIGN bit
in the p2m, making sure that mfn_to_pfn returns the pfn of the backend.
This allows the backend to perform direct_IO on these pages, but as a
side effect prevents the frontend from using get_user_pages_fast on
them while they are being shared with the backend.

There were paravirt_ops hooks for the full register set variant of
{rd,wr}msr_safe which are actually not used by anyone anymore. Remove
them to make the code cleaner and avoid silent breakages when the pvops
members were uninitialized. This has been boot-tested natively and under
Xen with PVOPS enabled and disabled on one machine.

ipi_call_lock/unlock() lock resp. unlock call_function.lock. This lock
protects only the call_function data structure itself, but it's
completely unrelated to cpu_online_mask. The mask to which the IPIs
are sent is calculated before call_function.lock is taken in
smp_call_function_many(), so the locking around set_cpu_online() is
pointless and can be removed.

Xen PV kernels allow access to the APERF/MPERF registers to read the
effective frequency. Access to the MSRs is however redirected to the
currently scheduled physical CPU, making consecutive read and
compares unreliable. In addition each rdmsr traps into the hypervisor.
So to avoid bogus readouts and expensive traps, disable the kernel
internal feature flag for APERF/MPERF if running under Xen.
This will
a) remove the aperfmperf flag from /proc/cpuinfo
b) not mislead the power scheduler (arch/x86/kernel/cpu/sched.c) to
use the feature to improve scheduling (by default disabled)
c) not mislead the cpufreq driver to use the MSRs
This does not cover userland programs which access the MSRs via the
device file interface, but this will be addressed separately.

Stub out MSR methods that aren't actually needed. This fixes a crash
as Xen Dom0 on AMD Trinity systems. A bigger patch should be added to
remove the paravirt machinery completely for the methods which
apparently have no users!

We did not take into account that xen_released_pages would be
used outside the initial E820 parsing code. As such we would
did not subtract from xen_released_pages the count of pages
that we had populated back (instead we just did a simple
extra_pages = released - populated).
The balloon driver uses xen_released_pages to set the initial
current_pages count. If this is wrong (too low) then when a new
(higher) target is set, the balloon driver will request too many pages
from Xen."
This fixes errors such as:

where the free_memory should be 128.

In xen_memory_setup(), if a page that is being released has a VA
mapping this must also be updated. Otherwise, the page will be not
released completely -- it will still be referenced in Xen and won't be
freed util the mapping is removed and this prevents it from being
reallocated at a different PFN.
This was already being done for the ISA memory region in
xen_ident_map_ISA() but on many systems this was omitting a few pages
as many systems marked a few pages below the ISA memory region as
reserved in the e820 map.
This fixes errors such as:

When the Xen hypervisor boots a PV kernel it hands it two pieces
of information: nr_pages and a made up E820 entry.
The nr_pages value defines the range from zero to nr_pages of PFNs
which have a valid Machine Frame Number (MFN) underneath it. The
E820 mirrors that (with the VGA hole):

With that overlaying the nr_pages directly on the E820 does not
work as there are gaps and non-RAM regions that won't be used
by the memory allocator. The 'xen_release_chunk' helps with that
by punching holes in the P2M (PFN to MFN lookup tree) for those
regions and tells us that:
Freeing 20000-20200 pfn range: 512 pages freed
Freeing 40000-40200 pfn range: 512 pages freed
Freeing bad80-badf4 pfn range: 116 pages freed
Freeing badf6-bae7f pfn range: 137 pages freed
Freeing bb000-100000 pfn range: 282624 pages freed
Released 283999 pages of unused memory
Those 283999 pages are subtracted from the nr_pages and are returned
to the hypervisor. The end result is that the initial domain
boots with 1GB less memory as the nr_pages has been subtracted by
the amount of pages residing within the PCI hole. It can balloon up
to that if desired using 'xl mem-set 0 8092', but the balloon driver
is not always compiled in for the initial domain.
This patch, implements the populate hypercall (XENMEM_populate_physmap)
which increases the the domain with the same amount of pages that
were released.
The other solution (that did not work) was to transplant the MFN in
the P2M tree - the ones that were going to be freed were put in
the E820_RAM regions past the nr_pages. But the modifications to the
M2P array (the other side of creating PTEs) were not carried away.
As the hypervisor is the only one capable of modifying that and the
only two hypercalls that would do this are: the update_va_mapping
(which won't work, as during initial bootup only PFNs up to nr_pages
are mapped in the guest) or via the populate hypercall.
The end result is that the kernel can now boot with the
nr_pages without having to subtract the 283999 pages.
On a 8GB machine, with various dom0_mem= parameters this is what we get:

The accessing PCI configuration space with the PCI BIOS32 service does
not work in PV guests.
On systems without MMCONFIG or where the BIOS hasn't marked the
MMCONFIG region as reserved in the e820 map, the BIOS service is
probed (even though direct access is preferred) and this hangs.

which is due to the fact we are trying to access a PFN that is not
accessible to us. The reason (at least in this case) was that
PGD[256] is set to __HYPERVISOR_VIRT_START which was setup (by the
hypervisor) to point to a read-only linear map of the MFN->PFN array.
During our parsing we would get the MFN (a valid one), try to look
it up in the MFN->PFN tree and find it invalid and return ~0 as PFN.
Then pte_mfn_to_pfn would happilly feed that in, attach the flags
and return it back to the caller. 'ptdump_show' bitshifts it and
gets and invalid value that it tries to dereference.
Instead of doing all of that, we detect the ~0 case and just
return !_PAGE_PRESENT.
This bug has been in existence .. at least until 2.6.37 (yikes!)

On x86_64 on AMD machines where the first APIC_ID is not zero, we get:

BIOS bug: APIC version mismatch, boot CPU: 0, CPU 1: version 10
which means that when the ACPI processor driver loads and
tries to parse the _Pxx states it fails to do as, as it
ends up calling acpi_get_cpuid which does this:

And the bootup CPU, has not been found so it fails and returns -1
for the first CPU - which then subsequently in the loop that
"acpi_processor_get_info" does results in returning an error, which
means that "acpi_processor_add" failing and per_cpu(processor)
is never set (and is NULL).
That means that when xen-acpi-processor tries to load (much much
later on) and parse the P-states it gets -ENODEV from
acpi_processor_register_performance() (which tries to read
the per_cpu(processor)) and fails to parse the data.

In xen_restore_fl_direct(), xen_force_evtchn_callback() was being
called even if no events were pending. This resulted in (depending on
workload) about a 100 times as many xen_version hypercalls as
necessary.
Fix this by correcting the sense of the conditional jump.
This seems to give a significant performance benefit for some
workloads.
There is some subtle tricksy "..since the check here is trying to
check both pending and masked in a single cmpw, but I think this is
correct. It will call check_events now only when the combined
mask+pending word is 0x0001 (aka unmasked, pending)." (Ian)

With this patch, we cap the amount of vCPUS that the initial domain
can run, to exactly what dom0_max_vcpus=X has specified.
In the future, if there is a hypercall that will allow a running

There are exactly four users of __monitor and __mwait:
- cstate.c (which allows acpi_processor_ffh_cstate_enter to be called
when the cpuidle API drivers are used. However patch
"cpuidle: replace xen access to x86 pm_idle and default_idle"
provides a mechanism to disable the cpuidle and use safe_halt.
- smpboot (which allows mwait_play_dead to be called). However
safe_halt is always used so we skip that.
- intel_idle (same deal as above).
- acpi_pad.c. This the one that we do not want to run as we
will hit the below crash.
Why do we want to expose MWAIT_LEAF in the first place?
We want it for the xen-acpi-processor driver - which uploads
C-states to the hypervisor. If MWAIT_LEAF is set, the cstate.c
sets the proper address in the C-states so that the hypervisor
can benefit from using the MWAIT functionality. And that is
the sole reason for using it.
Without this patch, if a module performs mwait or monitor we
get this:

end up hitting a problem "that the next element is wrong because
we should be able to cope with that. The problem is that the
next->next pointer would be set LIST_POISON1. " (Stefano's

During early bootup we can't use alloc_page, so to allocate
leaf pages in the P2M we need to use extend_brk. For that
we are utilizing the early_alloc_p2m and early_alloc_p2m_middle
functions to do the job for us. This function follows the
same logic as set_phys_to_machine.

At the start of the function we were checking for idx != 0
and bailing out. And later calling extend_brk if idx != 0.
That is unnecessary so remove that checks.

For identity cases we want to call reserve_brk only on the boundary
conditions of the middle P2M (so P2M[x][y][0] = extend_brk). This is
to work around identify regions (PCI spaces, gaps in E820) which are not
aligned on 2MB regions.
However for the case were we want to allocate P2M middle leafs at the
early bootup stage, irregardless of this alignment check we need some
means of doing that. For that we provide the new argument.

The reason we are dying is b/c the call acpi_get_override_irq() is used,
which returns the polarity and trigger for the IRQs. That function calls
mp_find_ioapics to get the 'struct ioapic' structure - which along with the
mp_irq[x] is used to figure out the default values and the polarity/trigger
overrides. Since the mp_find_ioapics now returns -1 [b/c the IOAPIC is filled
with 0xffffffff], the acpi_get_override_irq() stops trying to lookup in the
mp_irq[x] the proper INT_SRV_OVR and we can't install the SCI interrupt.
The proper fix for this is going in v3.5 and adds an x86_io_apic_ops
struct so that platforms can override it. But for v3.4 lets carry this
work-around. This patch does that by providing a slightly different variant
of the fake IOAPIC entries.

A few limitations have been identified in the current dma-mapping
design and its implementations for various architectures. There exist
more than one function for allocating and freeing the buffers:
currently these 3 are used dma_{alloc, free}_coherent,
dma_{alloc,free}_writecombine, dma_{alloc,free}_noncoherent.
For most of the systems these calls are almost equivalent and can be
interchanged. For others, especially the truly non-coherent ones
(like ARM), the difference can be easily noticed in overall driver
performance. Sadly not all architectures provide implementations for
all of them, so the drivers might need to be adapted and cannot be
easily shared between different architectures. The provided patches
unify all these functions and hide the differences under the already

The patches in the current set introduce a new alloc/free methods
(with support for memory attributes) in dma_map_ops structure, which
will later replace dma_alloc_coherent and dma_alloc_writecombine
functions."
People finally started piping up with support for merging this, so I'm
merging it as the last of the pending stuff from the merge window.
Looks like pohmelfs is going to wait for 3.5 and more external support
for merging.

By using the functionality provided by "[CPUFREQ]: provide
disable_cpuidle() function to disable the API."
Under the Xen hypervisor we do not want the initial domain to exercise
the cpufreq scaling drivers. This is b/c the Xen hypervisor is
in charge of doing this as well and we can end up with both the
Linux kernel and the hypervisor trying to change the P-states
leading to weird performance issues.

For the hypervisor to take advantage of the MWAIT support it needs
to extract from the ACPI _CST the register address. But the
hypervisor does not have the support to parse DSDT so it relies on
the initial domain (dom0) to parse the ACPI Power Management information
and push it up to the hypervisor. The pushing of the data is done
by the processor_harveset_xen module which parses the information that
the ACPI parser has graciously exposed in 'struct acpi_processor'.
For the ACPI parser to also expose the Cx states for MWAIT, we need
to expose the MWAIT capability (leaf 1). Furthermore we also need to
expose the MWAIT_LEAF capability (leaf 5) for cstate.c to properly
function.
The hypervisor could expose these flags when it traps the XEN_EMULATE_PREFIX
operations, but it can't do it since it needs to be backwards compatible.
Instead we choose to use the native CPUID to figure out if the MWAIT
capability exists and use the XEN_SET_PDC query hypercall to figure out
if the hypervisor wants us to expose the MWAIT_LEAF capability or not.
Note: The XEN_SET_PDC query was implemented in c/s 23783:
"ACPI: add _PDC input override mechanism".
With this in place, instead of

Note: The cpu_idle which would be calling the mwait variants for idling

default_idle (which called safe_halt, which called xen_safe_halt).

Using of PAT to change pages from WB to WC works quite nicely.
Changing it back to WB - not so much. The crux of the matter is
that the code that does this (__page_change_att_set_clr) has only
limited information so when it tries to the change it gets
the "raw" unfiltered information instead of the properly filtered one -
and the "raw" one tell it that PSE bit is on (while infact it
is not). As a result when the PTE is set to be WB from WC, we get
tons of:

this band-aid solution so that at least users don't get
the page corruption (the pages that are WC don't get changed to WB
and end up being recycled for filesystem or other things causing
mysterious crashes).
The negative impact of this patch is that users of WC flag
(which are InfiniBand, radeon, nouveau drivers) won't be able
to set that flag - so they are going to see performance degradation.
But stability is more important here.

percpu_xxx funcs are duplicated with this_cpu_xxx funcs, so replace them

I don't know much of xen code. But, since the code is in x86 architecture,

If NR_CPUS < 256 then arch_spinlock_t is only 16 bits wide but struct
xen_spinlock is 32 bits. When a spin lock is contended and
xl->spinners is modified the two bytes immediately after the spin lock
would be corrupted.
This is a regression caused by 84eb950db1
(x86, ticketlock: Clean up types and accessors) which reduced the size
of arch_spinlock_t.
Fix this by making xl->spinners a u8 if NR_CPUS < 256. A
BUILD_BUG_ON() is also added to check the sizes of the two structures
are compatible.
In many cases this was not noticable as there would often be padding
bytes after the lock (e.g., if any of CONFIG_GENERIC_LOCKBREAK,
CONFIG_DEBUG_SPINLOCK, or CONFIG_DEBUG_LOCK_ALLOC were enabled).
The bnx2 driver is affected. In struct bnx2, phy_lock and
indirect_lock may have no padding after them. Contention on phy_lock
would corrupt indirect_lock making it appear locked and the driver
would deadlock.

clamped the total amount of RAM to the current maximum reservation. This is
correct for dom0 but is not correct for guest domains. In order to boot a guest
"pre-ballooned" (e.g. with memory=1G but maxmem=2G) in order to allow for
future memory expansion the guest must derive max_pfn from the e820 provided by
the toolstack and not the current maximum reservation (which can reflect only
the current maximum, not the guest lifetime max). The existing algorithm
already behaves this correctly if we do not artificially limit the maximum
number of pages for the guest case.
For a guest booted with maxmem=512, memory=128 this results in:

With this change "xl mem-set <domain> 512M" will successfully increase the
guest RAM (by reducing the balloon).

memblock_init() initializes arrays for regions and memblock itself;
however, all these can be done with struct initializers and
memblock_init() can be removed. This patch kills memblock_init() and
initializes memblock with struct initializer.
The only difference is that the first dummy entries don't have .nid
set to MAX_NUMNODES initially. This doesn't cause any behavior
difference.

The idea behind commit d91ee5863b ("cpuidle: replace xen access to x86
pm_idle and default_idle") was to have one call - disable_cpuidle()
which would make pm_idle not be molested by other code. It disallows
cpuidle_idle_call to be set to pm_idle (which is excellent).
But in the select_idle_routine() and idle_setup(), the pm_idle can still
be set to either: amd_e400_idle, mwait_idle or default_idle. This
depends on some CPU flags (MWAIT) and in AMD case on the type of CPU.
In case of mwait_idle we can hit some instances where the hypervisor
(Amazon EC2 specifically) sets the MWAIT and we get:

In the case of amd_e400_idle we don't get so spectacular crashes, but we
do end up making an MSR which is trapped in the hypervisor, and then
follow it up with a yield hypercall. Meaning we end up going to
hypervisor twice instead of just once.
The previous behavior before v3.0 was that pm_idle was set to
default_idle regardless of select_idle_routine/idle_setup.
We want to do that, but only for one specific case: Xen. This patch
does that.

Receiver-side copying of packets is based on this implementation, it gives
better performance and better CPU accounting. It totally supports three types:
full-page, sub-page and transitive grants.
However this patch does not cover sub-page and transitive grants, it mainly
focus on Full-page part and implements grant table V2 interfaces corresponding
to what already exists in grant table V1, such as: grant table V2
initialization, mapping, releasing and exported interfaces.
Each guest can only supports one type of grant table type, every entry in grant
table should be the same version. It is necessary to set V1 or V2 version before
initializing the grant table.
Grant table exported interfaces of V2 are same with those of V1, Xen is
responsible to judge what grant table version guests are using in every grant
operation.
V2 fulfills the same role of V1, and it is totally backwards compitable with V1.
If dom0 support grant table V2, the guests runing on it can run with either V1
or V2.

This patch introduces new structures of grant table V2, grant table V2 is an
extension from V1. Grant table is shared between guest and Xen, and Xen is
responsible to do corresponding work for grant operations, such as: figure
out guest's grant table version, perform different actions based on
different grant table version, etc. Although full-page structure of V2
is different from V1, it play the same role as V1.

When mapping a foreign page with xenbus_map_ring_valloc() with the
GNTTABOP_map_grant_ref hypercall, set the GNTMAP_contains_pte flag and
pass a pointer to the PTE (in init_mm).
After the page is mapped, the usual fault mechanism can be used to
update additional MMs. This allows the vmalloc_sync_all() to be
removed from alloc_vm_area().

In xen_memory_setup() all reserved regions and gaps are set to an
identity (1-1) p2m mapping. If an available page has a PFN within one
of these 1-1 mappings it will become inaccessible (as it MFN is lost)
so release them before setting up the mapping.
This can make an additional 256 MiB or more of RAM available
(depending on the size of the reserved regions in the memory map) if
the initial pages overlap with reserved regions.
The 1:1 p2m mappings are also extended to cover partial pages. This
fixes an issue with (for example) systems with a BIOS that puts the
DMI tables in a reserved region that begins on a non-page boundary.

Allow the extra memory (used by the balloon driver) to be in multiple
regions (typically two regions, one for low memory and one for high
memory). This allows the balloon driver to increase the number of
available low pages (if the initial number if pages is small).
As a side effect, the algorithm for building the e820 memory map is
simpler and more obviously correct as the map supplied by the
hypervisor is (almost) used as is (in particular, all reserved regions
and gaps are preserved). Only RAM regions are altered and RAM regions
above max_pfn + extra_pages are marked as unused (the region is split
in two if necessary).

Allow the xen balloon driver to populate its list of extra pages from
more than one region of memory. This will allow platforms to provide
(for example) a region of low memory and a region of high memory.
The maximum possible number of extra regions is 128 (== E820MAX) which
is quite large so xen_extra_mem is placed in __initdata. This is safe
as both xen_memory_setup() and balloon_init() are in __init.
The balloon regions themselves are not altered (i.e., there is still
only the one region).

In xen_memory_setup() pages that occur in gaps in the memory map are
released back to Xen. This reduces the domain's current page count in
the hypervisor. The Xen balloon driver does not correctly decrease
its initial current_pages count to reflect this. If 'delta' pages are
released and the target is adjusted the resulting reservation is
always 'delta' less than the requested target.
This affects dom0 if the initial allocation of pages overlaps the PCI
memory region but won't affect most domU guests that have been setup
with pseudo-physical memory maps that don't have gaps.
Fix this by accouting for the released pages when starting the balloon
driver.
If the domain's targets are managed by xapi, the domain may eventually
run out of memory and die because xapi currently gets its target
calculations wrong and whenever it is restarted it always reduces the
target by 'delta'.

If we want to use granted pages for AIO, changing the mappings of a user
vma and the corresponding p2m is not enough, we also need to update the
kernel mappings accordingly.
Currently this is only needed for pages that are created for user usages
through /dev/xen/gntdev. As in, pages that have been in use by the
kernel and use the P2M will not need this special mapping.
However there are no guarantees that in the future the kernel won't
start accessing pages through the 1:1 even for internal usage.
In order to avoid the complexity of dealing with highmem, we allocated
the pages lowmem.
We issue a HYPERVISOR_grant_table_op right away in
m2p_add_override and we remove the mappings using another
HYPERVISOR_grant_table_op in m2p_remove_override.
Considering that m2p_add_override and m2p_remove_override are called
once per page we use multicalls and hypercall batching.
Use the kmap_op pointer directly as argument to do the mapping as it is
guaranteed to be present up until the unmapping is done.
Before issuing any unmapping multicalls, we need to make sure that the
mapping has already being done, because we need the kmap->handle to be
set correctly.

overflow here, only the original calculation going through the order

The reason being that the last E820 entry is created using the
'extra_pages' (which is based on how many pages have been freed).
The mentioned git commit sets the initial value of 'extra_pages'
using a hypercall which returns the number of pages (if dom0_mem
has been used) or -1 otherwise. If the later we return with
MAX_DOMAIN_PAGES as basis for calculation:
return min(max_pages, MAX_DOMAIN_PAGES);
and use it:
extra_limit = xen_get_max_pages();
if (extra_limit >= max_pfn)
extra_pages = extra_limit - max_pfn;
else
extra_pages = 0;
which means we end up with extra_pages = 128GB in PFNs (33554432)
- 8GB in PFNs (2097152, on this specific box, can be larger or smaller),
and then we add that value to the E820 making it:
Xen: 00000000ff000000 - 0000000100000000 (reserved)
Xen: 0000000100000000 - 000000133f2e2000 (usable)
which is clearly wrong. It should look as so:
Xen: 00000000ff000000 - 0000000100000000 (reserved)
Xen: 0000000100000000 - 000000027fbda000 (usable)
Naturally this problem does not present itself if dom0_mem=max:X
is used.

PV spinlocks cannot possibly work with the current code because they are
enabled after pvops patching has already been done, and because PV
spinlocks use a different data structure than native spinlocks so we
cannot switch between them dynamically. A spinlock that has been taken
once by the native code (__ticket_spin_lock) cannot be taken by
__xen_spin_lock even after it has been released.

We have hit a couple of customer bugs where they would like to
use those parameters to run an UP kernel - but both of those
options turn of important sources of interrupt information so
we end up not being able to boot. The correct way is to
pass in 'dom0_max_vcpus=1' on the Xen hypervisor line and
the kernel will patch itself to be a UP kernel.

If vmalloc page_fault happens inside of interrupt handler with interrupts
disabled then on exit path from exception handler when there is no pending
interrupts, the following code (arch/x86/xen/xen-asm_32.S:112):

will enable interrupts even if they has been previously disabled according to

Solution is in setting XEN_vcpu_info_mask only when it should be set
according to

but not clearing it if there isn't any pending events.

Use the domain's maximum reservation to limit the amount of extra RAM
for the memory balloon. This reduces the size of the pages tables and
the amount of reserved low memory (which defaults to about 1/32 of the
total RAM).
On a system with 8 GiB of RAM with the domain limited to 1 GiB the
kernel reports:
Before:
Memory: 627792k/4472000k available
After:
Memory: 549740k/11132224k available
A increase of about 76 MiB (~1.5% of the unused 7 GiB). The reserved
low memory is also reduced from 253 MiB to 32 MiB. The total
additional usable RAM is 329 MiB.
For dom0, this requires at patch to Xen ('x86: use 'dom0_mem' to limit
the number of pages for dom0') (c/s 23790)

The tracing code used sched_clock() to get tracing timestamps, which
ends up calling xen_clocksource_read(). xen_clocksource_read() must
disable preemption, but if preemption tracing is enabled, this results
in infinite recursion.
I've only noticed this when boot-time tracing tests are enabled, but it
seems like a generic bug. It looks like it would also affect
kvm_clocksource_read().

The order-based approach is not only less efficient (requiring a shift
and a compare, typical generated code looking like this
mov eax, [machine_to_phys_order]
mov ecx, eax
shr ebx, cl
test ebx, ebx
jnz ...
whereas a direct check requires just a compare, like in
cmp ebx, [machine_to_phys_nr]
jae ...
), but also slightly dangerous in the 32-on-64 case - the element
address calculation can wrap if the next power of two boundary is
sufficiently far away from the actual upper limit of the table, and
hence can result in user space addresses being accessed (with it being
unknown what may actually be mapped there).
Additionally, the elimination of the mistaken use of fls() here (should
have been __fls()) fixes a latent issue on x86-64 that would trigger
if the code was run on a system with memory extending beyond the 44-bit
boundary.

Three places in the kernel assume that the only long mode CPL 3
selector is __USER_CS. This is not true on Xen -- Xen's sysretq
changes cs to the magic value 0xe033.
Two of the places are corner cases, but as of "x86-64: Improve
vsyscall emulation CS and RIP handling"
(c9712944b2), vsyscalls will segfault
if called with Xen's extra CS selector. This causes a panic when
older init builds die.
It seems impossible to make Xen use __USER_CS reliably without
taking a performance hit on every system call, so this fixes the
tests instead with a new paravirt op. It's a little ugly because
ptrace.h can't include paravirt.h.

When a Xen Dom0 kernel boots on a hypervisor, it gets access
to the raw-hardware ACPI tables. While it parses the idle tables
for the hypervisor's beneift, it uses HLT for its own idle.
Rather than have xen scribble on pm_idle and access default_idle,
have it simply disable_cpuidle() so acpi_idle will not load and
architecture default HLT will be used.

Make sure the fastpath code is inlined. Batch the page permission change

Other than sanity check and debug message, the x86 specific version of
memblock reserve/free functions are simple wrappers around the generic
versions - memblock_reserve/free().
This patch adds debug messages with caller identification to the
generic versions and replaces x86 specific ones and kills them.
arch/x86/include/asm/memblock.h and arch/x86/mm/memblock.c are empty
after this change and removed.

The earlier attempts (24bdb0b62c)
at fixing this problem caused other problems to surface (PV guests
with no PCI passthrough would have SWIOTLB turned on - which meant
64MB of precious contingous DMA32 memory being eaten up per guest).
The problem was: "on xen we add an extra memory region at the end of
the e820, and on this particular machine this extra memory region
would start below 4g and cross over the 4g boundary:
[0xfee01000-0x192655000)
Unfortunately e820_end_of_low_ram_pfn does not expect an
e820 layout like that so it returns 4g, therefore initial_memory_mapping
will map [0 - 0x100000000), that is a memory range that includes some
reserved memory regions."
The memory range was the IOAPIC regions, and with the 1-1 mapping
turned on, it would map them as RAM, not as MMIO regions. This caused
the hypervisor to complain. Fortunately this is experienced only under
the initial domain so we guard for it.

The MAXSMP config option requires CPUMASK_OFFSTACK, which in turn
requires we init the memory for the maps while we bring up the cpus.
MAXSMP also increases NR_CPUS to 4096. This increase in size exposed an
issue in the argument construction for multicalls from
xen_flush_tlb_others. The args should only need space for the actual
number of cpus.
Also in 2.6.39 it exposes a bootup problem.

We only need to set max_pfn_mapped to the last pfn mapped on x86_64 to
make sure that cleanup_highmap doesn't remove important mappings at
_end.
We don't need to do this on x86_32 because cleanup_highmap is not called
on x86_32. Besides lowering max_pfn_mapped on x86_32 has the unwanted
side effect of limiting the amount of memory available for the 1:1
kernel pagetable allocation.
This patch reverts the x86_32 part of the original patch.

We no longer support HIGHPTE allocations, so ptes should always be
within the kernel's direct map, and don't need pagetable walks
to convert to machine addresses.

xen_set_pte_at and xen_clear_pte are essentially identical to
xen_set_pte, so just make them all common.
When batched set_pte and pte_clear are the same, but the unbatch operation
must be different: they need to update the two halves of the pte in
different order.

In principle update_va_mapping is a good match for set_pte_at, since
it gets the address being mapped, which allows Xen to use its linear
pagetable mapping.
However that assumes that the pmd for the address is attached to the
current pagetable, which may not be true for a given user address space
because the kernel pmd is not shared (at least on 32-bit guests).
Normally the kernel will automatically sync a missing part of the
pagetable with the init_mm pagetable transparently via faults, but that
fails when a missing address is passed to Xen.
And while the linear pagetable mapping is very useful for 32-bit Xen
(as it avoids an explicit domain mapping), 32-bit Xen is deprecated.
64-bit Xen has all memory mapped all the time, so it makes no real
difference.
The upshot is that we should use mmu_update, since it can operate on
non-current pagetables or detached pagetables.

Xen can work out when we're doing IO mappings for itself, so we don't
need to do anything special, and the extra tests just clog things up.

.. when applicable. We need to track in the p2m_mfn and
p2m_mfn_p the MFNs and pointers, respectivly, for the P2M entries
that are allocated for the identity mappings. Without this,
a PV domain with an E820 that triggers the 1-1 mapping to kick in,
won't be able to be restored as the P2M won't have the identity
mappings.

git commit 24bdb0b62c (xen: do not create
the extra e820 region at an addr lower than 4G) does not take into
account that ifdef CONFIG_X86_32 instead of e820_end_of_low_ram_pfn()
find_low_pfn_range() is called (both calls are from arch/x86/kernel/setup.c).
find_low_pfn_range() behaves correctly and does not require change in
xen_extra_mem_start initialization. Additionally, if xen_extra_mem_start
is initialized in the same way as ifdef CONFIG_X86_64 then memory hotplug
support for Xen balloon driver (under development) is broken.

When we parse the raw E820, the Xen hypervisor can set "E820_RAM"
to "E820_UNUSABLE" if the mem=X argument is used. As such we
should _not_ consider the E820_UNUSABLE as an 1-1 identity
mapping, but instead use the same case as for E820_RAM.

Introduce a new x86_init hook called pagetable_reserve that at the end
of init_memory_mapping is used to reserve a range of memory addresses for
the kernel pagetable pages we used and free the other ones.
On native it just calls memblock_x86_reserve_range while on xen it also
takes care of setting the spare memory previously allocated
for kernel pagetable pages from RO to RW, so that it can be used for
other purposes.
A detailed explanation of the reason why this hook is needed follows.

at some point init_memory_mapping is going to reach the pagetable pages
area and map those pages too (mapping them as normal memory that falls
in the range of addresses passed to init_memory_mapping as argument).
Some of those pages are already pagetable pages (they are in the range
pgt_buf_start-pgt_buf_end) therefore they are going to be mapped RO and
everything is fine.
Some of these pages are not pagetable pages yet (they fall in the range
pgt_buf_end-pgt_buf_top; for example the page at pgt_buf_end) so they
are going to be mapped RW. When these pages become pagetable pages and
are hooked into the pagetable, xen will find that the guest has already
a RW mapping of them somewhere and fail the operation.
The reason Xen requires pagetables to be RO is that the hypervisor needs
to verify that the pagetables are valid before using them. The validation
operations are called "pinning" (more details in arch/x86/xen/mmu.c).
In order to fix the issue we mark all the pages in the entire range
pgt_buf_start-pgt_buf_top as RO, however when the pagetable allocation
is completed only the range pgt_buf_start-pgt_buf_end is reserved by
init_memory_mapping. Hence the kernel is going to crash as soon as one
of the pages in the range pgt_buf_end-pgt_buf_top is reused (b/c those
ranges are RO).
For this reason we need a hook to reserve the kernel pagetable pages we
used and free the other ones so that they can be reused for other
purposes.
On native it just means calling memblock_x86_reserve_range, on Xen it
also means marking RW the pagetable pages that we allocated before but
that haven't been used before.
Another way to fix this is without using the hook is by adding a 'if
(xen_pv_domain)' in the 'init_memory_mapping' code and calling the Xen
counterpart, but that is just nasty.

mask_rw_pte is currently checking if a pfn is a pagetable page if it
falls in the range pgt_buf_start - pgt_buf_end but that is incorrect
because pgt_buf_end is a moving target: pgt_buf_top is the real
boundary.

the init_memory_mapping has completed (in a perfect world we would
call this function from init_memory_mapping, but lets ignore that).
Because we are called _after_ init_memory_mapping the pgt_buf_[start,
end,top] have all changed to new values (b/c another init_memory_mapping
is called). Hence, the first time we enter this function, we save
away the pgt_buf_start value and update the pgt_buf_[end,top].
When we detect that the "old" pgt_buf_start through pgt_buf_end
PFNs have been reserved (so memblock_x86_reserve_range has been called),
we immediately set out to RW the "old" pgt_buf_end through pgt_buf_top.
And then we update those "old" pgt_buf_[end|top] with the new ones
so that we can redo this on the next pagetable.
