98fe3633c5 x86/mm/hotplug: Fix BUG_ON() after hot-remove by not freeing PUD
Since commit:

  af2cf278ef ("x86/mm/hotplug: Don't remove PGD entries in remove_pagetable()")

we no longer free PUDs so that we do not have to synchronize
all PGDs on hot-remove/vfree().

But the new 5-level page table patchset reverted that for 4-level
page tables, in the following commit:

  f2a6a70501: ("x86: Convert the rest of the code to support p4d_t")

This patch restores the damage and disables free_pud() if we are in the
4-level page table case, thus avoiding BUG_ON() after hot-remove.

fc5f9d5f15 x86/mm: Fix boot crash caused by incorrect loop count calculation in sync_global_pgds()
Jeff Moyer reported that on his system with two memory regions 0~64G and
1T~1T+192G, and kernel option "memmap=192G!1024G" added, enabling KASLR
will make the system hang intermittently during boot. While adding 'nokaslr'
won't.

The back trace is:

 Oops: 0000 [#1] SMP

 RIP: memcpy_erms()
 [ .... ]
 Call Trace:
  pmem_rw_page()

  generic_file_read_iter()
  blkdev_read_iter()
  __vfs_read()
  vfs_read()
  SyS_read()
  entry_SYSCALL_64_fastpath()

This crash happens because the for loop count calculation in sync_global_pgds()
is not correct. When a mapping area crosses PGD entries, we should
calculate the starting address of region which next PGD covers and assign
it to next for loop count, but not add PGDIR_SIZE directly. The old
code works right only if the mapping area is an exact multiple of PGDIR_SIZE,
otherwize the end region could be skipped so that it can't be synchronized
to all other processes from kernel PGD init_mm.pgd.

In Jeff's system, emulated pmem area [1024G, 1216G) is smaller than
PGDIR_SIZE. While 'nokaslr' works because PAGE_OFFSET is 1T aligned, it
makes this area be mapped inside one PGD entry. With KASLR enabled,
this area could cross two PGD entries, then the next PGD entry won't
be synced to all other processes. That is why we saw empty PGD.

Fix it.

e6ab9c4d43 x86/mm/64: Fix crash in remove_pagetable()
remove_pagetable() does page walk using p*d_page_vaddr() plus cast.
It's not canonical approach -- we usually use p*d_offset() for that.

It works fine as long as all page table levels are present. We broke the
invariant by introducing folded p4d page table level.

As result, remove_pagetable() interprets PMD as PUD and it leads to
crash:

	BUG: unable to handle kernel paging request at ffff880300000000
	IP: memchr_inv+0x60/0x110
	PGD 317d067

	PUD 3180067
	PMD 33f102067
	PTE 8000000300000060

Let's fix this by using p*d_offset() instead of p*d_page_vaddr() for
page walk.

2959a5f726 mm: add arch-independent testcases for RODATA
This patch makes arch-independent testcases for RODATA. Both x86 and
x86_64 already have testcases for RODATA, But they are arch-specific
because using inline assembly directly.

And cacheflush.h is not a suitable location for rodata-test related
things. Since they were in cacheflush.h, If someone change the state of
CONFIG_DEBUG_RODATA_TEST, It cause overhead of kernel build.

To solve the above issues, write arch-independent testcases and move it
to shared location.

ddffe98d16 mm/memory_hotplug: set magic number to page->freelist instead of page->lru.next
To identify that pages of page table are allocated from bootmem
allocator, magic number sets to page->lru.next.

But page->lru list is initialized in reserve_bootmem_region(). So when
calling free_pagetable(), the function cannot find the magic number of
pages. And free_pagetable() frees the pages by free_reserved_page() not
put_page_bootmem().

But if the pages are allocated from bootmem allocator and used as page
table, the pages have private flag. So before freeing the pages, we
should clear the private flag by put_page_bootmem().

Before applying the commit 7bfec6f47b ("mm, page_alloc: check multiple
page fields with a single branch"), we could find the following visible
issue:

  BUG: Bad page state in process kworker/u1024:1
  page:ffffea103cfd8040 count:0 mapcount:0 mappi

  [...] ret_from_fork+0x22/0x40

And the issue still silently occurs.

Until freeing the pages of page table allocated from bootmem allocator,
the page->freelist is never used. So the patch sets magic number to
page->freelist instead of page->lru.next.

09821ff1d5 x86/boot/e820: Prefix the E820_* type names with "E820_TYPE_"
So there's a number of constants that start with "E820" but which

with 'enum e820_type' values:

	E820MAP
	E820NR
	E820_X_MAX
	E820MAX

To better differentiate the 'enum e820_type' values prefix them
with E820_TYPE_.

af2cf278ef x86/mm/hotplug: Don't remove PGD entries in remove_pagetable()
So when memory hotplug removes a piece of physical memory from pagetable
mappings, it also frees the underlying PGD entry.

This complicates PGD management, so don't do this. We can keep the
PGD mapped and the PUD table all clear - it's only a single 4K page
per 512 GB of memory hotplugged.

4b599fedb7 x86/mm: Audit and remove any unnecessary uses of module.h
Historically a lot of these existed because we did not have
a distinction between what was modular code and what was providing
support to modules via EXPORT_SYMBOL and friends. That changed
when we forked out support for the latter into the export.h file.

This means we should be able to reduce the usage of module.h
in code that is obj-y Makefile or bool Kconfig. The advantage
in doing so is that module.h itself sources about 15 other headers;
adding significantly to what we feed cpp, and it can obscure what
headers we are effectively using.

Since module.h was the source for init.h (for __init) and for
export.h (for EXPORT_SYMBOL) we consider each obj-y/bool instance
for the presence of either and replace accordingly where needed.

Note that some bool/obj-y instances remain since module.h is
the header for some exception table entry stuff, and for things
like __init_or_module (code that is tossed when MODULES=n).

dcb32d9913 x86/mm: Use pte_none() to test for empty PTE
The page table manipulation code seems to have grown a couple of
sites that are looking for empty PTEs. Just in case one of these
entries got a stray bit set, use pte_none() instead of checking
for a zero pte_val().

The use pte_same() makes me a bit nervous. If we were doing a
pte_same() check against two cleared entries and one of them had
a stray bit set, it might fail the pte_same() check. But, I
don't think we ever _do_ pte_same() for cleared entries. It is
almost entirely used for checking for races in fault-in paths.

faa379332f x86/mm: Add PUD VA support for physical mapping
Minor change that allows early boot physical mapping of PUD level virtual

cf4fb15b31 x86/boot: Split out kernel_ident_mapping_init()
In order to support on-demand page table creation when moving the
kernel for KASLR, we need to use kernel_ident_mapping_init() in the
decompression code.

This splits it out into its own file for use outside of init_64.c.
Additionally, checking for __pa/__va defines is added since they
need to be overridden in the decompression code.

43c75f933b x86/mm: Streamline and restore probe_memory_block_size()
The cumulative effect of the following two commits:

  bdee237c03 ("x86: mm: Use 2GB memory block size on large-memory x86-64 systems")
  982792c782 ("x86, mm: probe memory block size for generic x86 64bit")

... is some pretty convoluted code.

The first commit also removed code for the UV case without stated reason,
which might lead to unexpected change in behavior.

This commit has no other (intended) functional change; just seeks to simplify
and make the code more understandable, beyond restoring the UV behavior.

The whole section with the "tail size" doesn't seem to be
reachable, since both the >= 64GB and < 64GB case return, so it
was removed.

4b94ffdc41 x86, mm: introduce vmem_altmap to augment vmemmap_populate()
In support of providing struct page for large persistent memory
capacities, use struct vmem_altmap to change the default policy for
allocating memory for the memmap array. The default vmemmap_populate()
allocates page table storage area from the page allocator. Given
persistent memory capacities relative to DRAM it may not be feasible to
store the memmap in 'System Memory'. Instead vmem_altmap represents
pre-allocated "device pages" to satisfy vmemmap_alloc_block_buf()
requests.

Warn on any residual W+X mappings after setting NX
if DEBUG_WX is enabled. Introduce a separate
X86_PTDUMP_CORE config that enables the code for
dumping the page tables without enabling the debugfs

033fbae988 mm: ZONE_DEVICE for "device memory"
While pmem is usable as a block device or via DAX mappings to userspace
there are several usage scenarios that can not target pmem due to its
lack of struct page coverage. In preparation for "hot plugging" pmem
into the vmemmap add ZONE_DEVICE as a new zone to tag these pages
separately from the ones that are subject to standard page allocations.
Importantly "device memory" can be removed at will by userspace
unbinding the driver of the device.

Having a separate zone prevents allocation and otherwise marks these
pages that are distinct from typical uniform memory. Device memory has
different lifetime and performance characteristics than RAM. However,
since we have run out of ZONES_SHIFT bits this functionality currently
depends on sacrificing ZONE_DMA.

a023748d53 Merge branch 'x86-mm-for-linus' of git://git.kernel.org/pub/scm/linux/kernel/git/tip/tip
Pull x86 mm tree changes from Ingo Molnar:
 "The biggest change is full PAT support from Jürgen Gross:

     The x86 architecture offers via the PAT (Page Attribute Table) a
     way to specify different caching modes in page table entries. The
     PAT MSR contains 8 entries each specifying one of 6 possible cache
     modes. A pte references one of those entries via 3 bits:
     _PAGE_PAT, _PAGE_PWT and _PAGE_PCD.

     The Linux kernel currently supports only 4 different cache modes.
     The PAT MSR is set up in a way that the setting of _PAGE_PAT in a
     pte doesn't matter: the top 4 entries in the PAT MSR are the same
     as the 4 lower entries.

     This results in the kernel not supporting e.g. write-through mode.
     Especially this cache mode would speed up drivers of video cards
     which now have to use uncached accesses.

     OTOH some old processors (Pentium) don't support PAT correctly and
     the Xen hypervisor has been using a different PAT MSR configuration
     for some time now and can't change that as this setting is part of
     the ABI.

     This patch set abstracts the cache mode from the pte and introduces
     tables to translate between cache mode and pte bits (the default
     cache mode "write back" is hard-wired to PAT entry 0). The tables
     are statically initialized with values being compatible to old
     processors and current usage. As soon as the PAT MSR is changed
     (or - in case of Xen - is read at boot time) the tables are changed
     accordingly. Requests of mappings with special cache modes are
     always possible now, in case they are not supported there will be a
     fallback to a compatible but slower mode.

     Summing it up, this patch set adds the following features:

      - capability to support WT and WP cache modes on processors with
        full PAT support

      - processors with no or uncorrect PAT support are still working as
        today, even if WT or WP cache mode are selected by drivers for
        some pages

      - reduction of Xen special handling regarding cache mode

  Another change is a boot speedup on ridiculously large RAM systems,
  plus other smaller fixes"

bdee237c03 x86: mm: Use 2GB memory block size on large-memory x86-64 systems
On large-memory x86-64 systems of 64GB or more with memory hot-plug
enabled, use a 2GB memory block size. Eg with 64GB memory, this reduces
the number of directories in /sys/devices/system/memory from 512 to 32,
making it more manageable, and reducing the creation time accordingly.

This caveat is that the memory can't be offlined (for hotplug or
otherwise) with the finer default 128MB granularity, but this is
unimportant due to the high memory densities generally used with such
large-memory systems, where eg a single DIMM is the order of 16GB.

f955371ca9 x86: remove the Xen-specific _PAGE_IOMAP PTE flag
The _PAGE_IO_MAP PTE flag was only used by Xen PV guests to mark PTEs
that were used to map I/O regions that are 1:1 in the p2m. This
allowed Xen to obtain the correct PFN when converting the MFNs read
from a PTE back to their PFN.

Xen guests no longer use _PAGE_IOMAP for this. Instead mfn_to_pfn()
returns the correct PFN by using a combination of the m2p and p2m to
determine if an MFN corresponds to a 1:1 mapping in the the p2m.

Remove _PAGE_IOMAP, replacing it with _PAGE_UNUSED2 to allow for
future uses of the PTE flag.

At first, sync_global_pgds() checks whether target PGD is none
or not. And if PGD is none, the PGD is skipped. But when
hot-removing memory, PGD may be none since PGD may be cleared by
free_pud_table(). So when sync_global_pgds() is called after
hot-removing memory, sync_global_pgds() should not skip PGD even
if the PGD is none. And sync_global_pgds() must clear PGD
entries of all processes MM.

Currently sync_global_pgds() does not clear PGD entries of all
processes MM when hot-removing memory. So when hot adding
memory which is same memory range as removed memory after
hot-removing memory, following call traces are shown:

The patch-set fixes the issue.

This patch (of 2):

remove_pagetable() gets start argument and passes the argument
to sync_global_pgds(). In this case, the argument must not be
modified. If the argument is modified and passed to
sync_global_pgds(), sync_global_pgds() does not correctly
synchronize PGD to PGD entries of all processes MM since
synchronized range of memory [start, end] is wrong.

Unfortunately the start argument is modified in
remove_pagetable(). So this patch fixes the issue.
