Explicitly exempt ZONE_DEVICE pages from kvm_is_reserved_pfn() and
instead manually handle ZONE_DEVICE on a case-by-case basis. For things
like page refcounts, KVM needs to treat ZONE_DEVICE pages like normal
pages, e.g. put pages grabbed via gup(). But for flows such as setting
A/D bits or shifting refcounts for transparent huge pages, KVM needs to
to avoid processing ZONE_DEVICE pages as the flows in question lack the
underlying machinery for proper handling of ZONE_DEVICE pages.
This fixes a hang reported by Adam Borowski[*] in dev_pagemap_cleanup()
when running a KVM guest backed with /dev/dax memory, as KVM straight up
doesn't put any references to ZONE_DEVICE pages acquired by gup().
Note, Dan Williams proposed an alternative solution of doing put_page()
on ZONE_DEVICE pages immediately after gup() in order to simplify the
auditing needed to ensure is_zone_device_page() is called if and only if
the backing device is pinned (via gup()). But that approach would break
kvm_vcpu_{un}map() as KVM requires the page to be pinned from map() 'til
unmap() when accessing guest memory, unlike KVM's secondary MMU, which
coordinates with mmu_notifier invalidations to avoid creating stale
page references, i.e. doesn't rely on pages being pinned.

fails, we need to decrease this count. By moving it earlier, we can push

Just like we do for WFE trapping, it can be useful to turn off
WFI trapping when the physical CPU is not oversubscribed (that
is, the vcpu is the only runnable process on this CPU) *and*
that we're using direct injection of interrupts.
The conditions are reevaluated on each vcpu_load(), ensuring that
we don't switch to this mode on a busy system.
On a GICv4 system, this has the effect of reducing the generation
of doorbell interrupts to zero when the right conditions are
met, which is a huge improvement over the current situation
(where the doorbells are screaming if the CPU ever hits a
blocking WFI).

In order to find out whether a vcpu is likely to be the target of
VLPIs (and to further optimize the way we deal with those), let's
track the number of VLPIs a vcpu can receive.
This gets implemented with an atomic variable that gets incremented
or decremented on map, unmap and move of a VLPI.

The timers are canceled from an preempt-notifier which is invoked with
disabled preemption which is not allowed on PREEMPT_RT.
The timer callback is short so in could be invoked in hard-IRQ context
on -RT.
Let the timer expire on hard-IRQ context even on -RT.

The page table pages corresponding to broken down large pages are zapped in
FIFO order, so that the large page can potentially be recovered, if it is
not longer being used for execution. This removes the performance penalty
for walking deeper EPT page tables.
By default, one large page will last about one hour once the guest
reaches a steady state.

It's possible that two LPIs locate in the same "byte_offset" but target
two different vcpus, where their pending status are indicated by two
different pending tables. In such a scenario, using last_byte_offset
optimization will lead KVM relying on the wrong pending table entry.
Let us use last_ptr instead, which can be treated as a byte index into
a pending table and also, can be vcpu specific.

When the VHE code was reworked, a lot of the vgic stuff was moved around,
but the GICv4 residency code did stay untouched, meaning that we come
in and out of residency on each flush/sync, which is obviously suboptimal.
To address this, let's move things around a bit:
- Residency entry (flush) moves to vcpu_load
- Residency exit (sync) moves to vcpu_put
- On blocking (entry to WFI), we "put"
- On unblocking (exit from WFI), we "load"
Because these can nest (load/block/put/load/unblock/put, for example),
we now have per-VPE tracking of the residency state.
Additionally, vgic_v4_put gains a "need doorbell" parameter, which only
gets set to true when blocking because of a WFI. This allows a finer
control of the doorbell, which now also gets disabled as soon as
it gets signaled.

Add a new helper, kvm_put_kvm_no_destroy(), to handle putting a borrowed
reference[*] to the VM when installing a new file descriptor fails. KVM
expects the refcount to remain valid in this case, as the in-progress
ioctl() has an explicit reference to the VM. The primary motiviation
for the helper is to document that the 'kvm' pointer is still valid
after putting the borrowed reference, e.g. to document that doing
mutex(&kvm->lock) immediately after putting a ref to kvm isn't broken.
[*] When exposing a new object to userspace via a file descriptor, e.g.
a new vcpu, KVM grabs a reference to itself (the VM) prior to making
the object visible to userspace to avoid prematurely freeing the VM
in the scenario where userspace immediately closes file descriptor.

Allow user space to inform the KVM host where in the physical memory
map the paravirtualized time structures should be located.
User space can set an attribute on the VCPU providing the IPA base
address of the stolen time structure for that VCPU. This must be
repeated for every VCPU in the VM.
The address is given in terms of the physical address visible to
the guest and must be 64 byte aligned. The guest will discover the
address via a hypercall.

Implement the service call for configuring a shared structure between a
VCPU and the hypervisor in which the hypervisor can write the time
stolen from the VCPU's execution time by other tasks on the host.
User space allocates memory which is placed at an IPA also chosen by user
space. The hypervisor then updates the shared structure using
kvm_put_guest() to ensure single copy atomicity of the 64-bit value
reporting the stolen time in nanoseconds.
Whenever stolen time is enabled by the guest, the stolen time counter is
reset.
The stolen time itself is retrieved from the sched_info structure
maintained by the Linux scheduler code. We enable SCHEDSTATS when
selecting KVM Kconfig to ensure this value is meaningful.

We currently intertwine the KVM PSCI implementation with the general
dispatch of hypercall handling, which makes perfect sense because PSCI
is the only category of hypercalls we support.
However, as we are about to support additional hypercalls, factor out
this functionality into a separate hypercall handler file.

In some scenarios, such as buggy guest or incorrect configuration of the
VMM and firmware description data, userspace will detect a memory access
to a portion of the IPA, which is not mapped to any MMIO region.
For this purpose, the appropriate action is to inject an external abort
to the guest. The kernel already has functionality to inject an
external abort, but we need to wire up a signal from user space that
lets user space tell the kernel to do this.
It turns out, we already have the set event functionality which we can
perfectly reuse for this.

For a long time, if a guest accessed memory outside of a memslot using
any of the load/store instructions in the architecture which doesn't
supply decoding information in the ESR_EL2 (the ISV bit is not set), the
kernel would print the following message and terminate the VM as a
result of returning -ENOSYS to userspace:
load/store instruction decoding not implemented
The reason behind this message is that KVM assumes that all accesses
outside a memslot is an MMIO access which should be handled by
userspace, and we originally expected to eventually implement some sort
of decoding of load/store instructions where the ISV bit was not set.
However, it turns out that many of the instructions which don't provide
decoding information on abort are not safe to use for MMIO accesses, and
the remaining few that would potentially make sense to use on MMIO
accesses, such as those with register writeback, are not used in
practice. It also turns out that fetching an instruction from guest
memory can be a pretty horrible affair, involving stopping all CPUs on
SMP systems, handling multiple corner cases of address translation in
software, and more. It doesn't appear likely that we'll ever implement
this in the kernel.
What is much more common is that a user has misconfigured his/her guest
and is actually not accessing an MMIO region, but just hitting some
random hole in the IPA space. In this scenario, the error message above
is almost misleading and has led to a great deal of confusion over the
years.
It is, nevertheless, ABI to userspace, and we therefore need to
introduce a new capability that userspace explicitly enables to change
behavior.
This patch introduces KVM_CAP_ARM_NISV_TO_USER (NISV meaning Non-ISV)
which does exactly that, and introduces a new exit reason to report the
event to userspace. User space can then emulate an exception to the
guest, restart the guest, suspend the guest, or take any other
appropriate action as per the policy of the running system.

The PMU emulation code uses the perf event sample period to trigger
the overflow detection. This works fine for the *first* overflow
handling, but results in a huge number of interrupts on the host,
unrelated to the number of interrupts handled in the guest (a x20
factor is pretty common for the cycle counter). On a slow system
(such as a SW model), this can result in the guest only making
forward progress at a glacial pace.
It turns out that the clue is in the name. The sample period is
exactly that: a period. And once the an overflow has occured,
the following period should be the full width of the associated
counter, instead of whatever the guest had initially programed.
Reset the sample period to the architected value in the overflow
handler, which now results in a number of host interrupts that is
much closer to the number of interrupts in the guest.

The current convention for KVM to request a chained event from the
host PMU is to set bit[0] in attr.config1 (PERF_ATTR_CFG1_KVM_PMU_CHAINED).
But as it turns out, this bit gets set *after* we create the kernel
event that backs our virtual counter, meaning that we never get
a 64bit counter.
Moving the setting to an earlier point solves the problem.

When a counter is disabled, its value is sampled before the event
is being disabled, and the value written back in the shadow register.
In that process, the value gets truncated to 32bit, which is adequate
for any counter but the cycle counter (defined as a 64bit counter).
This obviously results in a corrupted counter, and things like
"perf record -e cycles" not working at all when run in a guest...
A similar, but less critical bug exists in kvm_pmu_get_counter_value.
Make the truncation conditional on the counter not being the cycle

pages are created or destroyed. Clearing it will result in an

The first/last indexes are typically shared with a user app.
The app can change the 'last' index that the kernel uses
to store the next result. This change sanity checks the index
before using it for writing to a potentially arbitrary address.
This fixes CVE-2019-14821.

While parts of the VGIC support a large number of vcpus (we
bravely allow up to 512), other parts are more limited.
One of these limits is visible in the KVM_IRQ_LINE ioctl, which
only allows 256 vcpus to be signalled when using the CPU or PPI
types. Unfortunately, we've cornered ourselves badly by allocating
all the bits in the irq field.
Since the irq_type subfield (8 bit wide) is currently only taking
the values 0, 1 and 2 (and we have been careful not to allow anything
else), let's reduce this field to only 4 bits, and allocate the
remaining 4 bits to a vcpu2_index, which acts as a multiplier:
vcpu_id = 256 * vcpu2_index + vcpu_index
With that, and a new capability (KVM_CAP_ARM_IRQ_LINE_LAYOUT_2)
allowing this to be discovered, it becomes possible to inject
PPIs to up to 4096 vcpus. But please just don't.
Whilst we're there, add a clarification about the use of KVM_IRQ_LINE
on arm, which is not completely conditionned by KVM_CAP_IRQCHIP.

If the ap_list is longer than 256 entries, merge_final() in list_sort()
will call the comparison callback with the same element twice, causing
a deadlock in vgic_irq_cmp().

At the moment we use 2 IO devices per GICv3 redistributor: one
one for the RD_base frame and one for the SGI_base frame.
Instead we can use a single IO device per redistributor (the 2
frames are contiguous). This saves slots on the KVM_MMIO_BUS
which is currently limited to NR_IOBUS_DEVS (1000).
This change allows to instantiate up to 512 redistributors and may
speed the guest boot with a large number of VCPUs.

At the moment we initialise the target *mask* of a virtual IRQ to the
VCPU it belongs to, even though this mask is only defined for GICv2 and
quickly runs out of bits for many GICv3 guests.

Also for GICv3 guests the reporting of TARGET in the "vgic-state" debugfs
dump is wrong, due to this very same problem.
Because there is no requirement to create the VGIC device before the
VCPUs (and QEMU actually does it the other way round), we can't safely
initialise mpidr or targets in kvm_vgic_vcpu_init(). But since we touch
every private IRQ for each VCPU anyway later (in vgic_init()), we can
just move the initialisation of those fields into there, where we
definitely know the VGIC type.
On the way make sure we really have either a VGICv2 or a VGICv3 device,
since the existing code is just checking for "VGICv3 or not", silently
ignoring the uninitialised case.

If after an MMIO exit to userspace a VCPU is immediately run with an
immediate_exit request, such as when a signal is delivered or an MMIO
emulation completion is needed, then the VCPU completes the MMIO
emulation and immediately returns to userspace. As the exit_reason
does not get changed from KVM_EXIT_MMIO in these cases we have to
be careful not to complete the MMIO emulation again, when the VCPU is
eventually run again, because the emulation does an instruction skip
(and doing too many skips would be a waste of guest code :-) We need
to use additional VCPU state to track if the emulation is complete.
As luck would have it, we already have 'mmio_needed', which even
appears to be used in this way by other architectures already.

When a vpcu is about to block by calling kvm_vcpu_block, we call
back into the arch code to allow any form of synchronization that
may be required at this point (SVN stops the AVIC, ARM synchronises
the VMCR and enables GICv4 doorbells). But this synchronization
comes in quite late, as we've potentially waited for halt_poll_ns
to expire.
Instead, let's move kvm_arch_vcpu_blocking() to the beginning of
kvm_vcpu_block(), which on ARM has several benefits:
- VMCR gets synchronised early, meaning that any interrupt delivered
during the polling window will be evaluated with the correct guest
PMR
- GICv4 doorbells are enabled, which means that any guest interrupt
directly injected during that window will be immediately recognised
Tang Nianyao ran some tests on a GICv4 machine to evaluate such
change, and reported up to a 10% improvement for netperf:

Since commit 503a62862e ("KVM: arm/arm64: vgic: Rely on the GIC driver to
parse the firmware tables"), the vgic_v{2,3}_probe functions stopped using
a DT node. Commit 9097773245 ("KVM: arm/arm64: vgic-new: vgic_init:
implement kvm_vgic_hyp_init") changed the functions again, and now they
require exactly one argument, a struct gic_kvm_info populated by the GIC
driver. Unfortunately the comments regressed and state that a DT node is
used instead. Change the function comments to reflect the current
prototypes.

The LPI translation cache needs to be discarded when an ITS command
may affect the translation of an LPI (DISCARD, MAPC and MAPD with V=0)
or the routing of an LPI to a redistributor with disabled LPIs (MOVI,
MOVALL).
We decide to perform a full invalidation of the cache, irrespective
of the LPI that is affected. Commands are supposed to be rare enough
that it doesn't matter.

A HW mapped level sensitive interrupt asserted by a device will not be put
into the ap_list if it is disabled at the VGIC level. When it is enabled
again, it will be inserted into the ap_list and written to a list register
on guest entry regardless of the state of the device.
We could argue that this can also happen on real hardware, when the command
to enable the interrupt reached the GIC before the device had the chance to
de-assert the interrupt signal; however, we emulate the distributor and
redistributors in software and we can do better than that.

Since commit commit 328e566479 ("KVM: arm/arm64: vgic: Defer
touching GICH_VMCR to vcpu_load/put"), we leave ICH_VMCR_EL2 (or
its GICv2 equivalent) loaded as long as we can, only syncing it
back when we're scheduled out.
There is a small snag with that though: kvm_vgic_vcpu_pending_irq(),
which is indirectly called from kvm_vcpu_check_block(), needs to
evaluate the guest's view of ICC_PMR_EL1. At the point were we
call kvm_vcpu_check_block(), the vcpu is still loaded, and whatever
changes to PMR is not visible in memory until we do a vcpu_put().
Things go really south if the guest does the following:

Here, the hypervisor's view of PMR is zero, while the guest has enabled
its interrupts. kvm_vgic_vcpu_pending_irq() will then say that no
interrupts are pending (despite an interrupt being received) and we'll
block for no reason. If the guest doesn't have a periodic interrupt
firing once it has blocked, it will stay there forever.
To avoid this unfortuante situation, let's resync VMCR from
kvm_arch_vcpu_blocking(), ensuring that a following kvm_vcpu_check_block()
will observe the latest value of PMR.
This has been found by booting an arm64 Linux guest with the pseudo NMI
feature, and thus using interrupt priorities to mask interrupts instead
of the usual PSTATE masking.

When calling debugfs functions, there is no need to ever check the
return value. The function can work or not, but the code logic should
never do something different based on this.
Also, when doing this, change kvm_arch_create_vcpu_debugfs() to return
void instead of an integer, as we should not care at all about if this
function actually does anything or not.

After commit d73eb57b80 (KVM: Boost vCPUs that are delivering interrupts), a
five years old bug is exposed. Running ebizzy benchmark in three 80 vCPUs VMs
on one 80 pCPUs Skylake server, a lot of rcu_sched stall warning splatting
in the VMs after stress testing:

preempted_in_kernel is updated in preempt_notifier when involuntary preemption
ocurrs, it can be stale when the voluntarily preempted vCPUs are taken into
account by kvm_vcpu_on_spin() loop. This patch lets it just check preempted_in_kernel
for involuntary preemption.

We use "pmc->idx" and the "chained" bitmap to determine if the pmc is
chained, in kvm_pmu_pmc_is_chained(). But idx might be uninitialized
(and random) when we doing this decision, through a KVM_ARM_VCPU_INIT
ioctl -> kvm_pmu_vcpu_reset(). And the test_bit() against this random
idx will potentially hit a KASAN BUG [1].
In general, idx is the static property of a PMU counter that is not
expected to be modified across resets, as suggested by Julien. It
looks more reasonable if we can setup the PMU counter idx for a vcpu
in its creation time. Introduce a new function - kvm_pmu_vcpu_init()
for this basic setup. Oh, and the KASAN BUG will get fixed this way.

Inspired by commit 9cac38dd5d (KVM/s390: Set preempted flag during
vcpu wakeup and interrupt delivery), we want to also boost not just
lock holders but also vCPUs that are delivering interrupts. Most
smp_call_function_many calls are synchronous, so the IPI target vCPUs
are also good yield candidates. This patch introduces vcpu->ready to
boost vCPUs during wakeup and interrupt delivery time; unlike s390 we do
not reuse vcpu->preempted so that voluntarily preempted vCPUs are taken
into account by kvm_vcpu_on_spin, but vmx_vcpu_pi_put is not affected
(VT-d PI handles voluntary preemption separately, in pi_pre_block).
Testing on 80 HT 2 socket Xeon Skylake server, with 80 vCPUs VM 80GB RAM:

The PTE allocations in arm64 are identical to the generic ones modulo the
GFP flags.
Using the generic pte_alloc_one() functions ensures that the user page
tables are allocated with __GFP_ACCOUNT set.
The arm64 definition of PGALLOC_GFP is removed and replaced with
GFP_PGTABLE_USER for p[gum]d_alloc_one() for the user page tables and
GFP_PGTABLE_KERNEL for the kernel page tables. The KVM memory cache is now
using GFP_PGTABLE_USER.
The mappings created with create_pgd_mapping() are now using
GFP_PGTABLE_KERNEL.
The conversion to the generic version of pte_free_kernel() removes the NULL
check for pte.
The pte_free() version on arm64 is identical to the generic one and
can be simply dropped.

As part of setting up the host context, we populate its
MPIDR by using cpu_logical_map(). It turns out that contrary
to arm64, cpu_logical_map() on 32bit ARM doesn't return the
*full* MPIDR, but a truncated version.
This leaves the host MPIDR slightly corrupted after the first
run of a VM, since we won't correctly restore the MPIDR on
exit. Oops.
Since we cannot trust cpu_logical_map(), let's adopt a different
strategy. We move the initialization of the host CPU context as
part of the per-CPU initialization (which, in retrospect, makes
a lot of sense), and directly read the MPIDR from the HW. This
is guaranteed to work on both arm and arm64.

Currently, the {read,write}_sysreg_el*() accessors for accessing
particular ELs' sysregs in the presence of VHE rely on some local
hacks and define their system register encodings in a way that is
inconsistent with the core definitions in <asm/sysreg.h>.
As a result, it is necessary to add duplicate definitions for any
system register that already needs a definition in sysreg.h for
other reasons.
This is a bit of a maintenance headache, and the reasons for the
_el*() accessors working the way they do is a bit historical.
This patch gets rid of the shadow sysreg definitions in
<asm/kvm_hyp.h>, converts the _el*() accessors to use the core
__msr_s/__mrs_s interface, and converts all call sites to use the
standard sysreg #define names (i.e., upper case, with SYS_ prefix).
This patch will conflict heavily anyway, so the opportunity
to clean up some bad whitespace in the context of the changes is
taken.
The change exposes a few system registers that have no sysreg.h
definition, due to msr_s/mrs_s being used in place of msr/mrs:
additions are made in order to fill in the gaps.

KVM implements the firmware interface for mitigating cache speculation
vulnerabilities. Guests may use this interface to ensure mitigation is
active.
If we want to migrate such a guest to a host with a different support
level for those workarounds, migration might need to fail, to ensure that
critical guests don't loose their protection.
Introduce a way for userland to save and restore the workarounds state.
On restoring we do checks that make sure we don't downgrade our
mitigation level.

Recent commits added the explicit notion of "workaround not required" to
the state of the Spectre v2 (aka. BP_HARDENING) workaround, where we
just had "needed" and "unknown" before.
Export this knowledge to the rest of the kernel and enhance the existing
kvm_arm_harden_branch_predictor() to report this new state as well.
Export this new state to guests when they use KVM's firmware interface
emulation.

ARMv8 provides support for chained PMU counters, where an event type
of 0x001E is set for odd-numbered counters, the event counter will
increment by one for each overflow of the preceding even-numbered
counter. Let's emulate this in KVM by creating a 64 bit perf counter
when a user chains two emulated counters together.
For chained events we only support generating an overflow interrupt
on the high counter. We use the attributes of the low counter to
determine the attributes of the perf event.

We currently use pmc->bitmask to determine the width of the pmc - however
it's superfluous as the pmc index already describes if the pmc is a cycle
counter or event counter. The architecture clearly describes the widths of
these counters.
Let's remove the bitmask to simplify the code.

The perf event sample_period is currently set based upon the current
counter value, when PMXEVTYPER is written to and the perf event is created.
However the user may choose to write the type before the counter value in
which case sample_period will be set incorrectly. Let's instead decouple
event creation from PMXEVTYPER and (re)create the event in either
suitation.

Here is what I am guessing is going to be the last "big" SPDX update for
5.2. It contains all of the remaining GPLv2 and GPLv2+ updates that
were "easy" to determine by pattern matching. The ones after this are
going to be a bit more difficult and the people on the spdx list will be
discussing them on a case-by-case basis now.
Another 5000+ files are fixed up, so our overall totals are:
Files checked: 64545
Files with SPDX: 45529
Compared to the 5.1 kernel which was:
Files checked: 63848
Files with SPDX: 22576
This is a huge improvement.
Also, we deleted another 20000 lines of boilerplate license crud, always
nice to see in a diffstat.

The emulated ptimer needs to track the level changes, otherwise the
the interrupt will never get deasserted, resulting in the guest getting
stuck in an interrupt storm if it enables ptimer interrupts. This was
found with kvm-unit-tests; the ptimer tests hung as soon as interrupts
were enabled. Typical Linux guests don't have a problem as they prefer
using the virtual timer.

kvm_device->destroy() seems to be supposed to free its kvm_device
struct, but vgic_its_destroy() is not currently doing this,

One of the more common cases of allocation size calculations is finding
the size of a structure that has a zero-sized array at the end, along
with memory for some number of elements for that array. For example:

instance = kzalloc(sizeof(struct foo) + count * sizeof(struct boo), GFP_KERNEL);
Instead of leaving these open-coded and prone to type mistakes, we can
now use the new struct_size() helper:

Add a wrapper to invoke kvm_arch_check_processor_compat() so that the
boilerplate ugliness of checking virtualization support on all CPUs is
hidden from the arch specific code. x86's implementation in particular
is quite heinous, as it unnecessarily propagates the out-param pattern
into kvm_x86_ops.
While the x86 specific issue could be resolved solely by changing
kvm_x86_ops, make the change for all architectures as returning a value
directly is prettier and technically more robust, e.g. s390 doesn't set
the out param, which could lead to subtle breakage in the (highly
unlikely) scenario where the out-param was not pre-initialized by the
caller.
Opportunistically annotate svm_check_processor_compat() with __init.

KVM_CAP_MAX_VCPU_ID is currently always reporting KVM_MAX_VCPU_ID on all
architectures. However, on s390x, the amount of usable CPUs is determined
during runtime - it is depending on the features of the machine the code
is running on. Since we are using the vcpu_id as an index into the SCA
structures that are defined by the hardware (see e.g. the sca_add_vcpu()
function), it is not only the amount of CPUs that is limited by the hard-
ware, but also the range of IDs that we can use.
Thus KVM_CAP_MAX_VCPU_ID must be determined during runtime on s390x, too.
So the handling of KVM_CAP_MAX_VCPU_ID has to be moved from the common
code into the architecture specific code, and on s390x we have to return
the same value here as for KVM_CAP_MAX_VCPUS.
This problem has been discovered with the kvm_create_max_vcpus selftest.
With this change applied, the selftest now passes on s390x, too.

When assigning kvm irqfd we didn't check the irqchip mode but we allow
KVM_IRQFD to succeed with all the irqchip modes. However it does not
make much sense to create irqfd even without the kernel chips. Let's
provide a arch-dependent helper to check whether a specific irqfd is
allowed by the arch. At least for x86, it should make sense to check:
- when irqchip mode is NONE, all irqfds should be disallowed, and,
- when irqchip mode is SPLIT, irqfds that are with resamplefd should
be disallowed.
For either of the case, previously we'll silently ignore the irq or
the irq ack event if the irqchip mode is incorrect. However that can
cause misterious guest behaviors and it can be hard to triage. Let's
fail KVM_IRQFD even earlier to detect these incorrect configurations.

Commit ca84d1a24 (KVM: x86: Add clock sync request to hardware enable) mentioned
that "we always hold kvm_lock when hardware_enable is called. The one place that
doesn't need to worry about it is resume, as resuming a frozen CPU, the spinlock
won't be taken." However, commit 6706dae9 (virt/kvm: Replace spin_is_locked() with
lockdep) introduces a bug, it asserts when the lock is not held which is contrary
to the original goal.
This patch fixes it by WARN_ON when the lock is held.

KVM has helpers to handle the condition codes of trapped aarch32
instructions. These are marked __hyp_text and used from HYP, but they
aren't built by the 'hyp' Makefile, which has all the runes to avoid ASAN
and KCOV instrumentation.
Move this code to a new hyp/aarch32.c to avoid a hyp-panic when starting
an aarch32 guest on a host built with the ASAN/KCOV debug options.

kvm_dirty_bitmap_bytes() will return the size of the dirty bitmap of
the memslot rather than the size of bitmap passed over from the ioctl.
Here for KVM_CLEAR_DIRTY_LOG we should only copy exactly the size of
bitmap that covers kvm_clear_dirty_log.num_pages.

In KVM, specially for nested guests, there is a dominant pattern of:
=> map guest memory -> do_something -> unmap guest memory
In addition to all this unnecessarily noise in the code due to boiler plate
code, most of the time the mapping function does not properly handle memory
that is not backed by "struct page". This new guest mapping API encapsulate
most of this boiler plate code and also handles guest memory that is not
backed by "struct page".
The current implementation of this API is using memremap for memory that is
not backed by a "struct page" which would lead to a huge slow-down if it
was used for high-frequency mapping operations. The API does not have any
effect on current setups where guest memory is backed by a "struct page".
Further patches are going to also introduce a pfn-cache which would
significantly improve the performance of the memremap case.

If a memory slot's size is not a multiple of 64 pages (256K), then
the KVM_CLEAR_DIRTY_LOG API is unusable: clearing the final 64 pages
either requires the requested page range to go beyond memslot->npages,
or requires log->num_pages to be unaligned, and kvm_clear_dirty_log_protect
requires log->num_pages to be both in range and aligned.
To allow this case, allow log->num_pages not to be a multiple of 64 if
it ends exactly on the last page of the slot.

If a memory slot's size is not a multiple of 64 pages (256K), then
the KVM_CLEAR_DIRTY_LOG API is unusable: clearing the final 64 pages
either requires the requested page range to go beyond memslot->npages,
or requires log->num_pages to be unaligned, and kvm_clear_dirty_log_protect
requires log->num_pages to be both in range and aligned.
To allow this case, allow log->num_pages not to be a multiple of 64 if
it ends exactly on the last page of the slot.

When a P9 sPAPR VM boots, the CAS negotiation process determines which
interrupt mode to use (XICS legacy or XIVE native) and invokes a
machine reset to activate the chosen mode.
To be able to switch from one interrupt mode to another, we introduce
the capability to release a KVM device without destroying the VM. The
KVM device interface is extended with a new 'release' method which is
called when the file descriptor of the device is closed.
Once 'release' is called, the 'destroy' method will not be called
anymore as the device is removed from the device list of the VM.

Some KVM devices will want to handle special mappings related to the
underlying HW. For instance, the XIVE interrupt controller of the
POWER9 processor has MMIO pages for thread interrupt management and
for interrupt source control that need to be exposed to the guest when
the OS has the required support.

There are cases where halt polling is unwanted. For example when running
KVM on an over committed LPAR we rather want to give back the CPU to
neighbour LPARs instead of polling. Let us provide a callback that
allows architectures to disable polling.

When a VCPU never runs before a guest exists, but we set timer registers
up via ioctls, the associated hrtimer might never get cancelled.
Since we moved vcpu_load/put into the arch-specific implementations and
only have load/put for KVM_RUN, we won't ever have a scheduled hrtimer
for emulating a timer when modifying the timer state via an ioctl from
user space. All we need to do is make sure that we pick up the right
state when we load the timer state next time userspace calls KVM_RUN
again.
We also do not need to worry about this interacting with the bg_timer,
because if we were in WFI from the guest, and somehow ended up in a
kvm_arm_timer_set_reg, it means that:
1. the VCPU thread has received a signal,
2. we have called vcpu_load when being scheduled in again,
3. we have called vcpu_put when we returned to userspace for it to issue
another ioctl
And therefore will not have a bg_timer programmed and the event is
treated as a spurious wakeup from WFI if userspace decides to run the
vcpu again even if there are not virtual interrupts.
This fixes stray virtual timer interrupts triggered by an expiring
hrtimer, which happens after a failed live migration, for instance.

A failed KVM_ARM_VCPU_INIT should not set the vcpu target,
as the vcpu target is used by kvm_vcpu_initialized() to
determine if other vcpu ioctls may proceed. We need to set
the target before calling kvm_reset_vcpu(), but if that call
fails, we should then unset it and clear the feature bitmap
while we're at it.

With VHE different exception levels are used between the host (EL2) and
guest (EL1) with a shared exception level for userpace (EL0). We can take
advantage of this and use the PMU's exception level filtering to avoid
enabling/disabling counters in the world-switch code. Instead we just
modify the counter type to include or exclude EL0 at vcpu_{load,put} time.
We also ensure that trapped PMU system register writes do not re-enable
EL0 when reconfiguring the backing perf events.
This approach completely avoids blackout windows seen with !VHE.

The virt/arm core allocates a kvm_cpu_context_t percpu, at present this is
a typedef to kvm_cpu_context and is used to store host cpu context. The
kvm_cpu_context structure is also used elsewhere to hold vcpu context.
In order to use the percpu to hold additional future host information we
encapsulate kvm_cpu_context in a new structure and rename the typedef and
percpu to match.

When pointer authentication is supported, a guest may wish to use it.
This patch adds the necessary KVM infrastructure for this to work, with
a semi-lazy context switch of the pointer auth state.
Pointer authentication feature is only enabled when VHE is built
in the kernel and present in the CPU implementation so only VHE code
paths are modified.
When we schedule a vcpu, we disable guest usage of pointer
authentication instructions and accesses to the keys. While these are
disabled, we avoid context-switching the keys. When we trap the guest
trying to use pointer authentication functionality, we change to eagerly
context-switching the keys, and enable the feature. The next time the
vcpu is scheduled out/in, we start again. However the host key save is
optimized and implemented inside ptrauth instruction/register access
trap.
Pointer authentication consists of address authentication and generic
authentication, and CPUs in a system might have varied support for
either. Where support for either feature is not uniform, it is hidden
from guests via ID register emulation, as a result of the cpufeature
framework in the host.
Unfortunately, address authentication and generic authentication cannot
be trapped separately, as the architecture provides a single EL2 trap
covering both. If we wish to expose one without the other, we cannot
prevent a (badly-written) guest from intermittently using a feature
which is not uniformly supported (when scheduled on a physical CPU which
supports the relevant feature). Hence, this patch expects both type of
authentication to be present in a cpu.
This switch of key is done from guest enter/exit assembly as preparation
for the upcoming in-kernel pointer authentication support. Hence, these
key switching routines are not implemented in C code as they may cause
pointer authentication key signing error in some situations.

ARM64 standard pgtable functions are going to use pgtable_page_[ctor|dtor]
or pgtable_pmd_page_[ctor|dtor] constructs. At present KVM guest stage-2
PUD|PMD|PTE level page tabe pages are allocated with __get_free_page()
via mmu_memory_cache_alloc() but released with standard pud|pmd_free() or
pte_free_kernel(). These will fail once they start calling into pgtable_
[pmd]_page_dtor() for pages which never originally went through respective
constructor functions. Hence convert all stage-2 page table page release
functions to call buddy directly while freeing pages.

When disabling LPIs (for example on reset) at the redistributor
level, it is expected that LPIs that was pending in the CPU
interface are eventually retired.
Currently, this is not what is happening, and these LPIs will
stay in the ap_list, eventually being acknowledged by the vcpu
(which didn't quite expect this behaviour).
The fix is thus to retire these LPIs from the list of pending
interrupts as we disable LPIs.

Recently the generic timer test of kvm-unit-tests failed to complete
(stalled) when a physical timer is being used. This issue is caused
by incorrect update of CNTP_CVAL when CNTP_TVAL is being accessed,
introduced by 'Commit 84135d3d18 ("KVM: arm/arm64: consolidate arch
timer trap handlers")'. According to Arm ARM, the read/write behavior
of accesses to the TVAL registers is expected to be:
* READ: TimerValue = (CompareValue – (Counter - Offset)
* WRITE: CompareValue = ((Counter - Offset) + Sign(TimerValue)
This patch fixes the TVAL read/write code path according to the
specification.

Some aspects of vcpu configuration may be too complex to be
completed inside KVM_ARM_VCPU_INIT. Thus, there may be a
requirement for userspace to do some additional configuration
before various other ioctls will work in a consistent way.
In particular this will be the case for SVE, where userspace will
need to negotiate the set of vector lengths to be made available to
the guest before the vcpu becomes fully usable.
In order to provide an explicit way for userspace to confirm that
it has finished setting up a particular vcpu feature, this patch
adds a new ioctl KVM_ARM_VCPU_FINALIZE.
When userspace has opted into a feature that requires finalization,
typically by means of a feature flag passed to KVM_ARM_VCPU_INIT, a
matching call to KVM_ARM_VCPU_FINALIZE is now required before
KVM_RUN or KVM_GET_REG_LIST is allowed. Individual features may
impose additional restrictions where appropriate.
No existing vcpu features are affected by this, so current
userspace implementations will continue to work exactly as before,
with no need to issue KVM_ARM_VCPU_FINALIZE.
As implemented in this patch, KVM_ARM_VCPU_FINALIZE is currently a
placeholder: no finalizable features exist yet, so ioctl is not
required and will always yield EINVAL. Subsequent patches will add
the finalization logic to make use of this ioctl for SVE.
No functional change for existing userspace.

KVM's API requires thats ioctls must be issued from the same process
that created the VM. In other words, userspace can play games with a
VM's file descriptors, e.g. fork(), SCM_RIGHTS, etc..., but only the
creator can do anything useful. Explicitly reject device ioctls that
are issued by a process other than the VM's creator, and update KVM's
API documentation to extend its requirements to device ioctls.

We rely on the mmu_notifier call backs to handle the split/merge
of huge pages and thus we are guaranteed that, while creating a
block mapping, either the entire block is unmapped at stage2 or it
is missing permission.
However, we miss a case where the block mapping is split for dirty
logging case and then could later be made block mapping, if we cancel the
dirty logging. This not only creates inconsistent TLB entries for
the pages in the the block, but also leakes the table pages for
PMD level.
Handle this corner case for the huge mappings at stage2 by
unmapping the non-huge mapping for the block. This could potentially
release the upper level table. So we need to restart the table walk
once we unmap the range.

made the checks to skip huge mappings, stricter. However it introduced
a bug where we still use huge mappings, ignoring the flag to
use PTE mappings, by not reseting the vma_pagesize to PAGE_SIZE.
Also, the checks do not cover the PUD huge pages, that was
under review during the same period. This patch fixes both
the issues.

The normal interrupt flow is not to enable the vgic when no virtual
interrupt is to be injected (i.e. the LRs are empty). But when a guest
is likely to use GICv4 for LPIs, we absolutely need to switch it on
at all times. Otherwise, VLPIs only get delivered when there is something
in the LRs, which doesn't happen very often.
