In the unlikely case when alloc_irq_table() is not able to return a
remap table then "ret" will be assigned with an error code. Later, the
code checks `index' and if it is negative (which it is because it is
initialized with `-1') and then then function properly aborts but
returns `-1' instead `-ENOMEM' what was intended.
In order to correct this, I assign -ENOMEM to index.

Before commit 0bb6e243d7 ("iommu/amd: Support IOMMU_DOMAIN_DMA type
allocation") amd_iommu_devtable_lock had a read_lock() user but now
there are none. In fact, after the mentioned commit we had only
write_lock() user of the lock. Since there is no reason to keep it as
writer lock, change its type to a spin_lock.
I *think* that we might even be able to remove the lock because all its
current user seem to have their own protection.

The irq_remap_table is allocated while the iommu_table_lock is held with
interrupts disabled.
>From looking at the call sites, all callers are in the early device
initialisation (apic_bsp_setup(), pci_enable_device(),
pci_enable_msi()) so make sense to drop the lock which also enables
interrupts and try to allocate that memory with GFP_KERNEL instead
GFP_ATOMIC.
Since during the allocation the iommu_table_lock is dropped, we need to
recheck if table exists after the lock has been reacquired. I *think*
that it is impossible that the "devid" entry appears in irq_lookup_table
while the lock is dropped since the same device can only be probed once.
However I check for both cases, just to be sure.

Setting the IRQ remap table for a specific devid (or its alias devid)
includes three steps. Those three steps are always repeated each time
this is done.
Introduce a new helper function, move those steps there and use that
function instead. The compiler can still decide if it is worth to
inline.

alloc_irq_table() has a special ioapic argument. If set then it will
pre-allocate / reserve the first 32 indexes. The argument is only once
true and it would make alloc_irq_table() a little simpler if we would
extract the special bits to the caller.
The caller of irq_remapping_alloc() is holding irq_domain_mutex so the
initialization of iommu->irte_ops->set_allocated() should not race
against other user.

The function get_irq_table() reads/writes irq_lookup_table while holding
the amd_iommu_devtable_lock. It also modifies
amd_iommu_dev_table[].data[2].
set_dte_entry() is using amd_iommu_dev_table[].data[0|1] (under the
domain->lock) so it should be okay. The access to the iommu is
serialized with its own (iommu's) lock.
So split out get_irq_table() out of amd_iommu_devtable_lock's lock.

domain_id_alloc() and domain_id_free() is used for id management. Those
two function share a bitmap (amd_iommu_pd_alloc_bitmap) and set/clear
bits based on id allocation. There is no need to share this with
amd_iommu_devtable_lock, it can use its own lock for this operation.

alloc_dev_data() adds new items to dev_data_list and search_dev_data()
is searching for items in this list. Both protect the access to the list
with a spinlock.
There is no need to navigate forth and back within the list and there is
also no deleting of a specific item. This qualifies the list to become a
lock less list and as part of this, the spinlock can be removed.
With this change the ordering of those items within the list is changed:
before the change new items were added to the end of the list, now they
are added to the front. I don't think it matters but wanted to mention
it.

find_dev_data() does not check whether the return value alloc_dev_data()
is NULL. This was okay once because the pointer was returned once as-is.
Since commit df3f7a6e8e ("iommu/amd: Use is_attach_deferred
call-back") the pointer may be used within find_dev_data() so a NULL
check is required.

The generic DMA-direct (CONFIG_DMA_DIRECT_OPS=y) implementation is now
functionally equivalent to the x86 nommu dma_map implementation, so
switch over to using it.
That includes switching from using x86_dma_supported in various IOMMU
drivers to use dma_direct_supported instead, which provides the same
functionality.

get_irq_table() previously acquired amd_iommu_devtable_lock which is not
a raw lock, and thus cannot be acquired from atomic context on
PREEMPT_RT. Many calls to modify_irte*() come from atomic context due to
the IRQ desc->lock, as does amd_iommu_update_ga() due to the preemption
disabling in vcpu_load/put().
The only difference between calling get_irq_table() and reading from
irq_lookup_table[] directly, other than the lock acquisition and
amd_iommu_rlookup_table[] check, is if the table entry is unpopulated,
which should never happen when looking up a devid that came from an
irq_2_irte struct, as get_irq_table() would have already been called on
that devid during irq_remapping_alloc().
The lock acquisition is not needed in these cases because entries in
irq_lookup_table[] never change once non-NULL -- nor would the
amd_iommu_devtable_lock usage in get_irq_table() provide meaningful
protection if they did, since it's released before using the looked up
table in the get_irq_table() caller.
Rename the old get_irq_table() to alloc_irq_table(), and create a new
lockless get_irq_table() to be used in non-allocating contexts that WARNs
if it doesn't find what it's looking for.

Currently, iommu_unmap, iommu_unmap_fast and iommu_map_sg return
size_t. However, some of the return values are error codes (< 0),
which can be misinterpreted as large size. Therefore, returning size 0
instead to signify failure to map/unmap.

search_dev_data() acquires a non-raw lock, which can't be done
from atomic context on PREEMPT_RT. There is no need to look at
dev_data because guest_mode should never be set if use_vapic is

pci_get_bus_and_slot() is restrictive such that it assumes domain=0 as
where a PCI device is present. This restricts the device drivers to be
reused for other domain numbers.
Getting ready to remove pci_get_bus_and_slot() function in favor of
pci_get_domain_bus_and_slot().
Hard-code the domain number as 0 for the AMD IOMMU driver.

The AMD IOMMU specification Rev 3.00 (December 2016) introduces a
new Enhanced PPR Handling Support (EPHSup) bit in the MMIO register
offset 0030h (IOMMU Extended Feature Register).
When EPHSup=1, the IOMMU hardware requires the PPR bit of the
device table entry (DTE) to be set in order to support PPR for a
particular endpoint device.

On an is_allocated() interrupt index, we ALIGN() the current index and
then increment it via the for loop, guaranteeing that it is no longer
aligned for alignments >1. We instead need to align the next index,
to guarantee forward progress, moving the increment-only to the case
where the index was found to be unallocated.

Since IOVA allocation failure is not unusual case we need to flush
CPUs' rcache in hope we will succeed in next round.
However, it is useful to decide whether we need rcache flush step because
of two reasons:
- Not scalability. On large system with ~100 CPUs iterating and flushing
rcache for each CPU becomes serious bottleneck so we may want to defer it.
- free_cpu_cached_iovas() does not care about max PFN we are interested in.
Thus we may flush our rcaches and still get no new IOVA like in the
commonly used scenario:

throws entries away for nothing and alloc_iova() fails again
4. Next alloc_iova_fast() call cannot take advantage of rcache since we
have just defeated caches. In this case we pick the slowest option
to proceed.
This patch reworks flushed_rcache local flag to be additional function
argument instead and control rcache flush step. Also, it updates all users
to do the flush as the last chance.

When SME memory encryption is active it will rely on SWIOTLB to handle
DMA for devices that cannot support the addressing requirements of
having the encryption mask set in the physical address. The IOMMU
currently disables SWIOTLB if it is not running in passthrough mode.
This is not desired as non-PCI devices attempting DMA may fail. Update
the code to check if SME is active and not disable SWIOTLB.

Now that the cached node optimisation can apply to all allocations, the
couple of users which were playing tricks with dma_32bit_pfn in order to
benefit from it can stop doing so. Conversely, there is also no need for
all the other users to explicitly calculate a 'real' 32-bit PFN, when
init_iova_domain() can happily do that itself from the page granularity.

With the upcoming reservation/management scheme, early activation will
assign a special vector. The final activation at request_irq() assigns a
real vector, which needs to be updated in the tables.
Split out the reconfiguration code in set_affinity and use it for
reactivation.

The irq_domain_ops.activate() callback has no return value and no way to
tell the function that the activation is early.
The upcoming changes to support a reservation scheme which allows to assign
interrupt vectors on x86 only when the interrupt is actually requested
requires:
- A return value, so activation can fail at request_irq() time
- Information that the activate invocation is early, i.e. before
request_irq().

Rename a few iommu cache-flush functions that start with
iommu_ to start with amd_iommu now. This is to prevent name
collisions with generic iommu code later on.

In get_domain(), 'domain' could be NULL before it's passed to dma_ops_domain()
to dereference. And the current code calling get_domain() can't deal with the
returned 'domain' well if its value is NULL.
So before dma_ops_domain() calling, check if 'domain' is NULL, If yes just return
ERR_PTR(-EBUSY) directly.

When iommu is pre_enabled in kdump kernel, if a device is set up with
guest translations (DTE.GV=1), then don't copy GCR3 table root pointer
but move the device over to an empty guest-cr3 table and handle the
faults in the PPR log (which answer them with INVALID). After all these
PPR faults are recoverable for the device and we should not allow the
device to change old-kernels data when we don't have to.

Add function copy_dev_tables to copy the old DEV table entries of the panicked
kernel to the new allocated device table. Since all iommus share the same device
table the copy only need be done one time. Here add a new global old_dev_tbl_cpy
to point to the newly allocated device table which the content of old device
table will be copied to. Besides, we also need to:
- Check whether all IOMMUs actually use the same device table with the same size
- Verify that the size of the old device table is the expected size.
- Reserve the old domain id occupied in 1st kernel to avoid touching the old
io-page tables. Then on-flight DMA can continue looking it up.
And also define MACRO DEV_DOMID_MASK to replace magic number 0xffffULL, it can be
reused in copy_dev_tables().

In AMD-Vi spec several bits of IO PTE fields and DTE fields are similar
so that both of them can share the same MACRO definition. However
defining them respectively can make code more read-able. Do it now.

IRTE[GALogIntr] bit should set when enabling guest_mode, which enables

The IOMMU is programmed with physical addresses for the various tables
and buffers that are used to communicate between the device and the
driver. When the driver allocates this memory it is encrypted. In order
for the IOMMU to access the memory as encrypted the encryption mask needs
to be included in these physical addresses during configuration.
The PTE entries created by the IOMMU should also include the encryption
mask so that when the device behind the IOMMU performs a DMA, the DMA
will be performed to encrypted memory.

Pass-through devices to VM guest can get updated IRQ affinity
information via irq_set_affinity() when not running in guest mode.
Currently, AMD IOMMU driver in GA mode ignores the updated information
if the pass-through device is setup to use vAPIC regardless of guest_mode.
This could cause invalid interrupt remapping.
Also, the guest_mode bit should be set and cleared only when
SVM updates posted-interrupt interrupt remapping information.

The counters are increased every time the TLB for a given
domain is flushed. We also store the current value of that
counter into newly added entries of the flush-queue, so that
we can tell whether this entry is already flushed.

The queue flushing is pretty inefficient when it flushes the
queues for all cpus at once. Further it flushes all domains
from all IOMMUs for all CPUs, which is overkill as well.
Rip it out to make room for something more efficient.

Currently if there is no room to add a command to the command buffer, the
driver performs a "completion wait" which only returns when all commands
on the queue have been processed. There is no need to wait for the entire
command queue to be executed before adding the next command.
Update the driver to perform the same udelay() loop that the "completion
wait" performs, but instead re-read the head pointer to determine if
sufficient space is available. The very first time it is found that there
is no space available, the udelay() will be skipped to immediately perform
the opportunistic read of the head pointer. If it is still found that
there is not sufficient space, then the udelay() will be performed.

As newer, higher speed devices are developed, perf data shows that the
amount of MMIO that is performed when submitting commands to the IOMMU
causes performance issues. Currently, the command submission path reads
the command buffer head and tail pointers and then writes the tail
pointer once the command is ready.
The tail pointer is only ever updated by the driver so it can be tracked
by the driver without having to read it from the hardware.
The head pointer is updated by the hardware, but can be read
opportunistically. Reading the head pointer only when it appears that
there might not be room in the command buffer and then re-checking the
available space reduces the number of times the head pointer has to be
read.

Misbehaving devices can cause an endless chain of
io-page-faults, flooding dmesg and making the system-log
unusable or even prevent the system from booting.
So ratelimit the error messages about io-page-faults on a
per-device basis.

variable, which is now declared as static.

The introduction of reserved regions has left a couple of rough edges
which we could do with sorting out sooner rather than later. Since we
are not yet addressing the potential dynamic aspect of software-managed
reservations and presenting them at arbitrary fixed addresses, it is
incongruous that we end up displaying hardware vs. software-managed MSI
regions to userspace differently, especially since ARM-based systems may
actually require one or the other, or even potentially both at once,
(which iommu-dma currently has no hope of dealing with at all). Let's
resolve the former user-visible inconsistency ASAP before the ABI has
been baked into a kernel release, in a way that also lays the groundwork
for the latter shortcoming to be addressed by follow-up patches.
For clarity, rename the software-managed type to IOMMU_RESV_SW_MSI, use
IOMMU_RESV_MSI to describe the hardware type, and document everything a
little bit. Since the x86 MSI remapping hardware falls squarely under
this meaning of IOMMU_RESV_MSI, apply that type to their regions as well,
so that we tell the same story to userspace across all platforms.
Secondly, as the various region types require quite different handling,
and it really makes little sense to ever try combining them, convert the
bitfield-esque #defines to a plain enum in the process before anyone
gets the wrong impression.

Bart Van Assche noted that the ib DMA mapping code was significantly
similar enough to the core DMA mapping code that with a few changes
it was possible to remove the IB DMA mapping code entirely and
switch the RDMA stack to use the core DMA mapping code. This resulted
in a nice set of cleanups, but touched the entire tree. This branch
will be submitted separately to Linus at the end of the merge window
as per normal practice for tree wide changes like this.

There is currently support for iommu sysfs bindings, but
those need to be implemented in the IOMMU drivers. Add a
more generic version of this by adding a struct device to
struct iommu_device and use that for the sysfs bindings.
Also convert the AMD and Intel IOMMU driver to make use of
it.

This struct represents one hardware iommu in the iommu core
code. For now it only has the iommu-ops associated with it,
but that will be extended soon.
The register/unregister interface is also added, as well as
making use of it in the Intel and AMD IOMMU drivers.

We introduce a new field to differentiate the reserved region
types and specialize the apply_resv_region implementation.
Legacy direct mapped regions have IOMMU_RESV_DIRECT type.
We introduce 2 new reserved memory types:
- IOMMU_RESV_MSI will characterize MSI regions that are mapped
- IOMMU_RESV_RESERVED characterize regions that cannot by mapped.

We want to extend the callbacks used for dm regions and
use them for reserved regions. Reserved regions can be
- directly mapped regions
- regions that cannot be iommu mapped (PCI host bridge windows, ...)
- MSI regions (because they belong to another address space or because
they are not translated by the IOMMU and need special handling)
So let's rename the struct and also the callbacks.

The generic command buffer entry is 128 bits (16 bytes), so the offset
of tail and head pointer should be 16 bytes aligned and increased with
0x10 per command.
When cmd buf is full, head = (tail + 0x10) % CMD_BUFFER_SIZE.
So when left space of cmd buf should be able to store only two
command, we should be issued one COMPLETE_WAIT additionally to wait
all older commands completed. Then the left space should be increased
after IOMMU fetching from cmd buf.
So left check value should be left <= 0x20 (two commands).

If acpihid_device_group() finds an existing group for the relevant
devid, it should be taking an additional reference on that group.
Otherwise, the caller of iommu_group_get_for_dev() will inadvertently
remove the reference taken by iommu_group_add_device(), and the group
will be freed prematurely if any device is removed.

The semaphore used by the AMD IOMMU to signal command
completion lived on the stack until now, which was safe as
the driver busy-waited on the semaphore with IRQs disabled,
so the stack can't go away under the driver.
But the recently introduced vmap-based stacks break this as
the physical address of the semaphore can't be determinded
easily anymore. The driver used the __pa() macro, but that
only works in the direct-mapping. The result were
Completion-Wait timeout errors seen by the IOMMU driver,
breaking system boot.
Since putting the semaphore on the stack is bad design
anyway, move the semaphore into 'struct amd_iommu'. It is
protected by the per-iommu lock and now in the direct
mapping again. This fixes the Completion-Wait timeout errors
and makes AMD IOMMU systems boot again with vmap-based
stacks enabled.

Introduce struct iommu_dev_data.use_vapic flag, which IOMMU driver
uses to determine if it should enable vAPIC support, by setting
the ga_mode bit in the device's interrupt remapping table entry.
Currently, it is enabled for all pass-through device if vAPIC mode
is enabled.

This patch implements irq_set_vcpu_affinity() function to set up interrupt
remapping table entry with vapic mode for pass-through devices.
In case requirements for vapic mode are not met, it falls back to set up
the IRTE in legacy mode.

This patch adds AMD IOMMU guest virtual APIC log (GALOG) handler.
When IOMMU hardware receives an interrupt targeting a blocking vcpu,
it creates an entry in the GALOG, and generates an interrupt to notify
the AMD IOMMU driver.
At this point, the driver processes the log entry, and notify the SVM
driver via the registered iommu_ga_log_notifier function.

This patch enables support for the new 128-bit IOMMU IRTE format,
which can be used for both legacy and vapic interrupt remapping modes.
It replaces the existing operations on IRTE, which can only support
the older 32-bit IRTE format, with calls to the new struct amd_irt_ops.
It also provides helper functions for setting up, accessing, and
updating interrupt remapping table entries in different mode.

Currently, IOMMU support two interrupt remapping table entry formats,
32-bit (legacy) and 128-bit (GA). The spec also implies that it might
support additional modes/formats in the future.
So, this patch introduces the new struct amd_irte_ops, which allows
the same code to work with different irte formats by providing hooks
for various operations on an interrupt remapping table entry.

The dma-mapping core and the implementations do not change the DMA
attributes passed by pointer. Thus the pointer can point to const data.
However the attributes do not have to be a bitfield. Instead unsigned
long will do fine:
1. This is just simpler. Both in terms of reading the code and setting
attributes. Instead of initializing local attributes on the stack
and passing pointer to it to dma_set_attr(), just set the bits.
2. It brings safeness and checking for const correctness because the
attributes are passed by value.

A two-level page-table can map up to 1GB of address space.
With the IOVA allocator now in use, the allocated addresses
are often more closely to 4G, which requires the address
space to be increased much more often. Avoid that by using a
three-level page-table by default.

Not doing so might cause IO-Page-Faults when a device uses
an alias request-id and the alias-dte is left in a lower
page-mode which does not cover the address allocated from
the iova-allocator.

Before a dma_ops_domain can be freed, we need to make sure
it is not longer referenced by the flush queue. So empty the
queue before a dma_ops_domain can be freed.

In case the queue doesn't fill up, we flush the TLB at least
10ms after the unmap happened to make sure that the TLB is
cleaned up.

With the flush queue the IOMMU TLBs will not be flushed at
every dma-ops unmap operation. The unmapped ranges will be
queued and flushed at once, when the queue is full. This
makes unmapping operations a lot faster (on average) and
restores the performance of the old address allocator.

Put the MSI-range, the HT-range and the MMIO ranges of PCI
devices into that range, so that these addresses are not
allocated for DMA.

The default domain for a device might also be
identity-mapped. In this case the kernel would crash when
unity mappings are defined for the device. Fix that by
making sure the domain is a dma_ops domain.

AMD has more drivers will use ACPI to platform bus driver later,
all those devices need iommu support, for example: eMMC driver.
For latest AMD eMMC controller, it will utilize sdhci-acpi.c driver,
which will rely on platform bus to match device and driver, where we
will set 'dev' of struct platform_device as map_sg parameter passing
to iommu driver for DMA request, so the iommu-ops are needed on the
platform bus.

Commit 61289cb ('iommu/amd: Remove old alias handling code')
removed the old alias handling code from the AMD IOMMU
driver because this is now handled by the IOMMU core code.
But this also removed the handling of PCI aliases, which is
not handled by the core code. This caused issues with PCI
devices that have hidden PCIe-to-PCI bridges that rewrite
the request-id.
Fix this bug by re-introducing some of the removed functions
from commit 61289cbaf6 and add a alias field
'struct iommu_dev_data'. This field carrys the return value
of the get_alias() function and uses that instead of the
amd_iommu_alias_table[] array in the code.

This patch creates a new function for finding or creating an IOMMU
group for acpihid(ACPI Hardware ID) device.
The acpihid devices with the same devid will be put into same group and

With the newly added ACPI-HID IVHD device entry support, this is no

get_device_id() returns an unsigned short device id. It never fails and
it never returns a negative so we can remove this condition.

Preallocate between 4 and 8 apertures when a device gets it
dma_mask. With more apertures we reduce the lock contention
of the domain lock significantly.
