Introduce xen_dma_map_page, xen_dma_unmap_page,
xen_dma_sync_single_for_cpu and xen_dma_sync_single_for_device.
They have empty implementations on x86 and ia64 but they call the
corresponding platform dma_ops function on arm and arm64.

All the callers of irq_create_of_mapping() pass the contents of a struct
of_phandle_args structure to the function. Since all the callers already
have an of_phandle_args pointer, why not pass it directly to
irq_create_of_mapping()?

This patch makes the kernel use of_phandle_args everywhere. This in
itself isn't a big deal, but it makes some follow-on patches simpler.

The OF irq handling code has been overloading the term 'map' to refer to
both parsing the data in the device tree and mapping it to the internal
linux irq system. This is probably because the device tree does have the
concept of an 'interrupt-map' function for translating interrupt
references from one node to another, but 'map' is still confusing when
the primary purpose of some of the functions are to parse the DT data.
This patch renames all the of_irq_map_* functions to of_irq_parse_*
which makes it clear that there is a difference between the parsing
phase and the mapping phase. Kernel code can make use of just the
parsing or just the mapping support as needed by the subsystem.
The patch was generated mechanically with a handful of sed commands.

In latest UEFI spec(by now it is 2.4) memory error definition
for CPER (UEFI 2.4 Appendix N Common Platform Error Record)
adds some new fields. These fields help people to locate
memory error to an actual DIMM location.

This H/W error log driver (a.k.a eMCA driver) is implemented based on

After errors are captured, more detailed platform specific information
can be got via this new enhanced H/W error log driver. Most notably we
can track memory errors back to the DIMM slot silk screen label.

As Intel rolling out more SoC's after Moorestown, we need to
re-structure the code in a way that is backward compatible and easy to
expand. This patch implements a flexible way to support multiple boards
and devices.
This patch does not add any new functional support. It just refactors
the existing code to increase the modularity and decrease the code
duplication for supporting multiple soc's and boards.
Currently intel-mid.c has both board and soc related code in one file.
This patch moves the board related code to new files and let linker
script to create SFI devite table following this:
1. Move the SFI device specific code to
arch/x86/platform/intel-mid/device-libs/platform_<device>.*
A new device file is added for every supported device. This code will
get conditionally compiled by using corresponding device driver
CONFIG option.
2. Move the device_ids location to .x86_intel_mid_dev.init section by
using new sfi_device() macro.

When Intel mid uses SFI table to enumerate devices, it requires an extra
device table with further information about how to probe such devices.
This patch creates a section where the device table will stay if
CONFIG_X86_INTEL_MID is selected.

SFI device_id[] table parsing code is duplicated in every SFI
device handler. This patch removes this code duplication, by
adding a seperate function get_device_id() to parse through the
device table. Also this patch moves the SPI, I2C, IPC info code from
sfi_parse_devs() to respective device handlers.

mrst is used as common name to represent all intel_mid type
soc's. But moorsetwon is just one of the intel_mid soc. So
renamed them to use intel_mid.
This patch mainly renames the variables and related
functions that uses *mrst* prefix with *intel_mid*.
To ensure that there are no functional changes, I have compared
the objdump of related files before and after rename and found
the only difference is symbol and name changes.

Also, renamed the corresponding header files and made changes
to the driver files that included these header files.
To ensure that there are no functional changes, I have compared

There's been reports of high NMI handler overhead, highlighted by
such kernel messages:

The solution to that is to avoid having to call copy_from_user_nmi() for

Since we already limit the max basic block size, we can easily
pre-allocate a piece of memory to copy the entire thing into in one
go.

We use jump label to enable pv-spinlock. With the changes in (442e0973e9
Merge branch 'x86/jumplabel'), the jump label behaviour has changed
that would result in eventual hang of the VM since we would end up in a
situation where slow path locks would halt the vcpus but we will not be
able to wakeup the vcpu by lock releaser using unlock kick.
Similar problem in Xen and more detailed description is available in
a945928ea2 (xen: Do not enable spinlocks before jump_label_init()
has executed)
This patch splits kvm_spinlock_init to separate jump label changes with
pvops patching and also make jump label enabling after jump_label_init().

Page pinning is not mandatory in kvm async page fault processing since
after async page fault event is delivered to a guest it accesses page once
again and does its own GUP. Drop the FOLL_GET flag in GUP in async_pf
code, and do some simplifying in check/clear processing.

I have a randconfig here which has enabled only
CONFIG_MICROCODE=y
CONFIG_MICROCODE_OLD_INTERFACE=y
with both
# CONFIG_MICROCODE_INTEL is not set
# CONFIG_MICROCODE_AMD is not set
off. Which makes building the microcode functionality a little
pointless. Don't do that in such cases then.

The gfn_to_index function relies on huge page defines which either may
not make sense on systems that don't support huge pages or are defined
in an unconvenient way for other architectures. Since this is
x86-specific, move the function to arch/x86/include/asm/kvm_host.h.

Hyper-V supports a mechanism for retrieving the local APIC frequency.
Use this and bypass the calibration code in the kernel . This would
allow us to boot the Linux kernel as a "modern VM" on Hyper-V where
many of the legacy devices (such as PIT) are not emulated.

Due to the way kernel is initialized under Xen is possible that the
ring1 selector used by the kernel for the boot cpu end up to be copied
to userspace leading to segmentation fault in the userspace.
Xen code in the kernel initialize no-boot cpus with correct selectors (ds
and es set to __USER_DS) but the boot one keep the ring1 (passed by Xen).
On task context switch (switch_to) we assume that ds, es and cs already
point to __USER_DS and __KERNEL_CSso these selector are not changed.
If processor is an Intel that support sysenter instruction sysenter/sysexit
is used so ds and es are not restored switching back from kernel to
userspace. In the case the selectors point to a ring1 instead of __USER_DS
the userspace code will crash on first memory access attempt (to be
precise Xen on the emulated iret used to do sysexit will detect and set ds
and es to zero which lead to GPF anyway).
Now if an userspace process call kernel using sysenter and get rescheduled
(for me it happen on a specific init calling wait4) could happen that the
ring1 selector is set to ds and es.
This is quite hard to detect cause after a while these selectors are fixed
(__USER_DS seems sticky).
Bisecting the code commit 7076aada10 appears
to be the first one that have this issue.

Use xen_alloc_coherent_pages and xen_free_coherent_pages to allocate or
free coherent pages.
We need to be careful handling the pointer returned by
xen_alloc_coherent_pages, because on ARM the pointer is not equal to
phys_to_virt(*dma_handle). In fact virt_to_phys only works for kernel
direct mapped RAM memory.
In ARM case the pointer could be an ioremap address, therefore passing
it to virt_to_phys would give you another physical address that doesn't
correspond to it.
Make xen_create_contiguous_region take a phys_addr_t as start parameter to
avoid the virt_to_phys calls which would be incorrect.

Previously we coalesced windows by expanding the first overlapping one and
making the second invalid. But we never look at the expanded first window
again, so we fail to notice other windows that overlap it. For example, we
coalesced these:

into these, which still overlap:

72f857950f broke shadow on EPT. This patch reverts it and fixes PAE
on nEPT (which reverted commit fixed) in other way.
Shadow on EPT is now broken because while L1 builds shadow page table
for L2 (which is PAE while L2 is in real mode) it never loads L2's
GUEST_PDPTR[0-3]. They do not need to be loaded because without nested
virtualization HW does this during guest entry if EPT is disabled,
but in our case L0 emulates L2's vmentry while EPT is enables, so we
cannot rely on vmcs12->guest_pdptr[0-3] to contain up-to-date values
and need to re-read PDPTEs from L2 memory. This is what kvm_set_cr3()
is doing, but by clearing cache bits during L2 vmentry we drop values
that kvm_set_cr3() read from memory.
So why the same code does not work for PAE on nEPT? kvm_set_cr3()
reads pdptes into vcpu->arch.walk_mmu->pdptrs[]. walk_mmu points to
vcpu->arch.nested_mmu while nested guest is running, but ept_load_pdptrs()
uses vcpu->arch.mmu which contain incorrect values. Fix that by using
walk_mmu in ept_(load|save)_pdptrs.

HAVE_ARCH_DEVTREE_FIXUPS appears to always be needed except for sparc,
but it is only used for /proc/device-teee and sparc does not enable
/proc/device-tree. So this option is redundant. Remove the option and
always enable it. This has the side effect of fixing /proc/device-tree
on arches such as arm64 which failed to define this option.

xen_swiotlb_alloc_coherent needs to allocate a coherent buffer for cpu
and devices. On native x86 is sufficient to call __get_free_pages in
order to get a coherent buffer, while on ARM (and potentially ARM64) we
need to call the native dma_ops->alloc implementation.
Introduce xen_alloc_coherent_pages to abstract the arch specific buffer
allocation.
Similarly introduce xen_free_coherent_pages to free a coherent buffer:
on x86 is simply a call to free_pages while on ARM and ARM64 is
arm_dma_ops.free.

The AMD_GART driver was made EXPERT/EMBEDDED a long time
ago to avoid unbootable 64bit systems with 32bit only devices.
This was before swiotlb was there, which does the job
of this fallback today. SWIOTLB is always on, so systems
should always boot.
The drawback is that every system has to compile that
driver in (it cannot be a module).
Also:
- Newer AMD CPUs (the APUs) don't seem to have AMD_GART support
at all anymore.
- Newer AMD platforms have a much better real IOMMU
- The AMD GART driver was never very good (lots of overhead,
e.g. in flushing due to some workarounds) and it's doubtful it's
really better than SWIOTLB.
- On older K8 systems it didn't even work with all chipsets.
- The 32bit device bounce buffer case should be rare/
non performance critical these days anyways.
- On non AMD systems it is not needed at all.
So drop the EXPERT dependency on AMD_GART and remove the
default y. The driver can be still compiled in, just
it's an explicit decision now, and people who don't want
it can unselect it.
I also clarified the description a bit.
This allows to save ~8K text on most modern x86-64 systems.

Commit ebd97be635 ('PCI: remove ARCH_SUPPORTS_MSI kconfig option')
removed the ARCH_SUPPORTS_MSI option which architectures could select
to indicate that they support MSI. Now, all architectures are supposed
to build fine when MSI support is enabled: instead of having the
architecture tell *when* MSI support can be used, it's up to the
architecture code to ensure that MSI support can be enabled.
On x86, commit ebd97be635 removed the following line:
select ARCH_SUPPORTS_MSI if (X86_LOCAL_APIC && X86_IO_APIC)
Which meant that MSI support was only available when the local APIC
and I/O APIC were enabled. While this is always true on SMP or x86-64,
it is not necessarily the case on i386 !SMP.
The below patch makes sure that the local APIC and I/O APIC support is
always enabled when MSI support is enabled. To do so, it:
* Ensures the X86_UP_APIC option is not visible when PCI_MSI is
enabled. This is the option that allows, on UP machines, to enable
or not the APIC support. It is already not visible on SMP systems,
or x86-64 systems, for example. We're simply also making it
invisible on i386 MSI systems.
* Ensures that the X86_LOCAL_APIC and X86_IO_APIC options are 'y'
when PCI_MSI is enabled.
Notice that this change requires a change in drivers/iommu/Kconfig to
avoid a recursive Kconfig dependencey. The AMD_IOMMU option selects
PCI_MSI, but was depending on X86_IO_APIC. This dependency is no
longer needed: as soon as PCI_MSI is selected, the presence of
X86_IO_APIC is guaranteed. Moreover, the AMD_IOMMU already depended on
X86_64, which already guaranteed that X86_IO_APIC was enabled, so this
dependency was anyway redundant.

In the PEBS handler report the transaction flags using the new
generic transaction flags facility. Most of them come from
the "tsx_tuning" field in PEBSv2, but the abort code is derived
from the RAX register reported in the PEBS record.

Currently the cap_user_time_zero capability has different tests than
cap_user_time; even though they expose the exact same data.
Switch from CONSTANT && NONSTOP to sched_clock_stable to also deal
with multi cabinet machines and drop the tsc_disabled() check.. non of
this will work sanely without tsc anyway.

This patch restores the capability to enter KDB (and KGDB) from
the UV NMI handler. This is needed because the UV system
console is not capable of sending the 'break' signal to the
serial console port. It is also useful when the kernel is hung
in such a way that it isn't responding to normal external I/O,
so sending 'g' to sysreq-trigger does not work either.
Another benefit of the external NMI command is that all the cpus
receive the NMI signal at roughly the same time so they are more
closely aligned timewise.
It utilizes the newly added kgdb_nmicallin function to gain
entry to KGDB/KDB by the master. The slaves still enter via the
standard kgdb_nmicallback function. It also uses the new
'send_ready' pointer to tell KGDB/KDB to signal the slaves when
to proceed into the KGDB slave loop.
It is enabled when the nmi action is set to "kdb" and the kernel
is built with CONFIG_KDB enabled. Note that if kgdb is
connected that interface will be used instead.

The new_cr3 MMU callback has been a wrapper for mmu_free_roots since commit
e676505 (KVM: MMU: Force cr3 reload with two dimensional paging on mov
cr3 emulation, 2012-07-08).
The commit message mentioned that "mmu_free_roots() is somewhat of an overkill,
but fixing that is more complicated and will be done after this minimal fix".
One year has passed, and no one really felt the need to do a different fix.
Wrap the call with a kvm_mmu_new_cr3 function for clarity, but remove the
callback.

A guest can still attempt to save and restore XSAVE states even if they
have been masked in CPUID leaf 0Dh. This usually is not visible to
the guest, but is still wrong: "Any attempt to set a reserved bit (as
determined by the contents of EAX and EDX after executing CPUID with
EAX=0DH, ECX= 0H) in XCR0 for a given processor will result in a #GP
exception".
The patch also performs the same checks as __kvm_set_xcr in KVM_SET_XSAVE.
This catches migration from newer to older kernel/processor before the
guest starts running.

XSAVE entries that KVM does not support are reported by
KVM_GET_SUPPORTED_CPUID for leaf 0Dh index 0 if the host supports them;
they should be left out unless there is also hypervisor support for them.
Sub-leafs are correctly handled in supported_xcr0_bit, fix index 0
to match.

IORESOURCE_BUSY is used to mark temporary driver mem-resources
instead of global regions. This suppresses warnings if regions
overlap with a region marked as BUSY.
This was always the case for VESA/VGA/EFI framebuffer regions so
do the same for simplefb regions. The reason we do this is to
allow device handover to real GPU drivers like
i915/radeon/nouveau which get the same regions via PCI BARs.
Maybe at some point we will be able to unregister platform
devices properly during the handover. In this case the simplefb
region would get removed before the new region is created.
However, this is currently not the case and would require rather
huge changes in remove_conflicting_framebuffers(). Add the BUSY
marker now and try to eventually rewrite the handover for a next release.

x86-64 runs irq_exit() under the irq stack. So it can afford
to run softirqs in hardirq exit without the need to switch
the stacks. The hardirq stack is good enough for that.
Now x86-64 runs softirqs in the hardirq stack anyway, so what we
mostly skip is some needless per cpu refcounting updates there.
x86-32 is not concerned because it only runs the irq handler on
the irq stack.

common code: disable irqs (to avoid races with the pending check),
check if there are softirqs pending, then execute __do_softirq() on
a specific stack.
Consolidate the common parts such that archs only worry about the
stack switch.

cpu_hotplug_driver_lock() serializes CPU online/offline operations
when ARCH_CPU_PROBE_RELEASE is set. This lock interface is no longer
necessary with the following reason:
- lock_device_hotplug() now protects CPU online/offline operations,
including the probe & release interfaces enabled by
ARCH_CPU_PROBE_RELEASE. The use of cpu_hotplug_driver_lock() is
redundant.
- cpu_hotplug_driver_lock() is only valid when ARCH_CPU_PROBE_RELEASE
is defined, which is misleading and is only enabled on powerpc.
This patch removes the cpu_hotplug_driver_lock() interface. As
a result, ARCH_CPU_PROBE_RELEASE only enables / disables the cpu
probe & release interface as intended. There is no functional change
in this patch.

The problem in efi_main was that the idt was cleared before the
interrupts were disabled.
The UEFI spec states that interrupts aren't used so this shouldn't be
too much of a problem. Peripherals however don't necessarily know about
this and thus might cause interrupts to happen anyway. Even if
ExitBootServices() has been called.
This means there is a risk of an interrupt being triggered while the IDT
register is nullified and the interrupt bit hasn't been cleared,
allowing for a triple fault.
This patch disables the interrupt flag, while leaving the existing IDT
in place. The CPU won't care about the IDT at all as long as the
interrupt bit is off, so it's safe to leave it in place as nothing will
ever happen to it.

This patch fixes a problem with EFI memory maps larger than 128 entries
when booting using the EFI stub, which results in overflowing e820_map
in boot_params and an eventual halt when checking the map size in
sanitize_e820_map().
If the number of map entries is greater than what can fit in e820_map,
add the extra entries to the setup_data list using type SETUP_E820_EXT.
These extra entries are then picked up when the setup_data list is
parsed in parse_e820_ext().

If #PF happens during delivery of an exception into L2 and L1 also do
not have the page mapped in its shadow page table then L0 needs to
generate vmexit to L2 with original event in IDT_VECTORING_INFO, but
current code combines both exception and generates #DF instead. Fix that
by providing nVMX specific function to handle page faults during page
table walk that handles this case correctly.

If an exception causes vmexit directly it should not be reported in
IDT_VECTORING_INFO during the exit. For that we need to be able to
distinguish between exception that is injected into nested VM and one that
is reinjected because its delivery failed. Fortunately we already have
mechanism to do so for nested SVM, so here we just use correct function
to requeue exceptions and make sure that reinjected exception is not
moved to IDT_VECTORING_INFO during vmexit emulation and not re-checked
for interception during delivery.

EXIT_REASON_VMLAUNCH/EXIT_REASON_VMRESUME exit does not mean that nested
VM will actually run during next entry. Move setting nested_run_pending
closer to vmentry emulation code and move its clearing close to vmexit to
minimize amount of code that will erroneously run with nested_run_pending
set.

As the new x86 CPU bootup printout format code maintainer, I am

Yuanhan reported a serious throughput regression in his pigz
benchmark. Using the ftrace patch I found that several idle
paths need more TLC before we can switch the generic
need_resched() over to preempt_need_resched.
The preemption paths benefit most from preempt_need_resched and
do indeed use it; all other need_resched() users don't really
care that much so reverting need_resched() back to
tif_need_resched() is the simple and safe solution.

On AMD family 14h, applying microcode patch on the a core (core0)
would also affect the other core (core1) in the same compute
unit. The driver would skip applying the patch on core1, but it
still need to update kernel structures to reflect the proper
patch level.
The current logic is not updating the struct
ucode_cpu_info.cpu_sig.rev of the skipped core. This causes the
/sys/devices/system/cpu/cpu1/microcode/version to report
incorrect patch level as shown below:

When building on x86, the final image building step always emits stats
to stderr, even though this information is neither a warning nor an error:

Validating automated builds would be cleaner if stderr did not have to
filter out these lines. Instead, change how tools/build is called, and
make the zoffset header unconditional, and write to a specified file
instead of to stdout, which can then be used for statistics, leaving
stderr open for legitimate warnings and errors, like the output from
die().

In current implementation for reboot type CF9 and CF9_COND,
warm and cold reset are not differentiated, and both are
performed by writing 0x06 to port 0xCF9.
This commit will differentiate warm and cold reset:
For warm reset, write 0x06 to port 0xCF9;
For cold reset, write 0x0E to port 0xCF9.
[ hpa: This meaning of "cold" and "warm" reset is different from other
reboot types use, where "warm" means "bypass BIOS POST". It is also
not entirely clear that it actually solves any actual problem. However,
it would seem fairly harmless to offer this additional option.
Also note that we do not mask bit 3 in the "warm reset" case. This
preserves the behavior on existing systems, including ones quirked
to use CF9. It seems reasonable that on any system where the
warm/cold distinction actually matters that bit 3 would be read as
zero. ]

Current ACPI tables in initrd is limited to 10, that is too small.
64 should be good enough as we have 35 sigs and could have several
SSDT.
Two problems in current code prevent us from increasing limit:
1. The cpio file info array is put in stack, as every element is 32
bytes, could run out of stack if we have that array size to 64.
We can move it out from stack, make it global and put it into the
__initdata section.
2. early_ioremap() only can remap 256k one time. Current code maps
10 tables at a time. If we increased that limit, the whole size
could be more than 256k, so early_ioremap() would fail with that.
We can map chunks one by one during copying, instead of mapping
all of them together.

On hosts with more than 168 GB of memory, a 32-bit guest may attempt
to grant map an MFN that is error cannot lookup in its mapping of the
m2p table. There is an m2p lookup as part of m2p_add_override() and
m2p_remove_override(). The lookup falls off the end of the mapped
portion of the m2p and (because the mapping is at the highest virtual
address) wraps around and the lookup causes a fault on what appears to
be a user space address.
do_page_fault() (thinking it's a fault to a userspace address), tries
to lock mm->mmap_sem. If the gntdev device is used for the grant map,
m2p_add_override() is called from from gnttab_mmap() with mm->mmap_sem
already locked. do_page_fault() then deadlocks.
The deadlock would most commonly occur when a 64-bit guest is started
and xenconsoled attempts to grant map its console ring.
Introduce mfn_to_pfn_no_overrides() which checks the MFN is within the
mapped portion of the m2p table before accessing the table and use
this in m2p_add_override(), m2p_remove_override(), and mfn_to_pfn()
(which already had the correct range check).
All faults caused by accessing the non-existant parts of the m2p are
thus within the kernel address space and exception_fixup() is called
without trying to lock mm->mmap_sem.
This means that for MFNs that are outside the mapped range of the m2p
then mfn_to_pfn() will always look in the m2p overrides. This is
correct because it must be a foreign MFN (and the PFN in the m2p in
this case is only relevant for the other domain).

Convert x86 to use a per-cpu preemption count. The reason for doing so
is that accessing per-cpu variables is a lot cheaper than accessing
thread_info variables.
We still need to save/restore the actual preemption count due to
PREEMPT_ACTIVE so we place the per-cpu __preempt_count variable in the
same cache-line as the other hot __switch_to() variables such as
current_task.
NOTE: this save/restore is required even for !PREEMPT kernels as
cond_resched() also relies on preempt_count's PREEMPT_ACTIVE to ignore
task_struct::state.
Also rename thread_info::preempt_count to ensure nobody is
'accidentally' still poking at it.

Rewrite the preempt_count macros in order to extract the 3 basic
preempt_count value modifiers:
__preempt_count_add()
__preempt_count_sub()
and the new:
__preempt_count_dec_and_test()
And since we're at it anyway, replace the unconventional
$op_preempt_count names with the more conventional preempt_count_$op.
Since these basic operators are equivalent to the previous _notrace()
variants, do away with the _notrace() versions.

Mike reported that commit 7d1a9417 ("x86: Use generic idle loop")
regressed several workloads and caused excessive reschedule
interrupts.
The patch in question failed to notice that the x86 code had an
inverted sense of the polling state versus the new generic code (x86:
default polling, generic: default !polling).
Fix the two prominent x86 mwait based idle drivers and introduce a few
new generic polling helpers (fixing the wrong smp_mb__after_clear_bit
usage).
Also switch the idle routines to using tif_need_resched() which is an
immediate TIF_NEED_RESCHED test as opposed to need_resched which will
end up being slightly different.

Linus suggested using asm goto to get rid of the typical SETcc + TEST
instruction pair -- which also clobbers an extra register -- for our
typical modify_and_test() functions.
Because asm goto doesn't allow output fields it has to include an
unconditinal memory clobber when it changes a memory variable to force
a reload.
Luckily all atomic ops already imply a compiler barrier to go along
with their memory barrier semantics.

and returns the loaded data through parameters, rather than taking
an x86 specific setup_header structure. For ARM, this will be used

Make efi_free() safely callable with size of 0, similar to free() being
callable with NULL pointers, and do nothing in that case.
Remove size checks that this makes redundant. This also avoids some
size checks in the ARM EFI stub code that will be added as well.

Move the open-coded conversion to a shared function for
use by all architectures. Change the allocation to prefer
a high address for ARM, as this is required to avoid conflicts
with reserved regions in low memory. We don't know the specifics
of these regions until after we process the command line and
device tree.

Rename relocate_kernel() to efi_relocate_kernel(), and take
parameters rather than x86 specific structure. Add max_addr
argument as for ARM we have some address constraints that we
need to enforce when relocating the kernel. Add alloc_size
parameter for use by ARM64 which uses an uncompressed kernel,
and needs to allocate space for BSS.

Add system table pointer argument to shared EFI stub related functions
so they no longer use a global system table pointer as they did when part
of eboot.c. For the ARM EFI stub this allows us to avoid global
variables completely and thereby not have to deal with GOT fixups.
Not having the EFI stub fixup its GOT, which is shared with the
decompressor, simplifies the relocating of the zImage to a
bootable address.

The x86/AMD64 EFI stubs must use a call wrapper to convert between
the Linux and EFI ABIs, so void pointers are sufficient. For ARM,
the ABIs are compatible, so we can directly invoke the function
pointers. The functions that are used by the ARM stub are updated
to match the EFI definitions.
Also add some EFI types used by EFI functions.

Commit d7c53c9e enabled ARCH_CPU_PROBE_RELEASE on x86 in order to
serialize CPU online/offline operations. Although it is the config
option to enable CPU hotplug test interfaces, probe & release, it is
also the option to enable cpu_hotplug_driver_lock() as well. Therefore,
this option had to be enabled on x86 with dummy arch_cpu_probe() and
arch_cpu_release().
Since then, lock_device_hotplug() was introduced to serialize CPU

lock_device_hotplug[_sysfs]() serializes CPU & Memory online/offline
and hotplug operations. However, this lock is not held in the debug
interfaces below that initiate CPU online/offline operations.
- _debug_hotplug_cpu(), cpu0 hotplug test interface enabled by
CONFIG_DEBUG_HOTPLUG_CPU0.
- cpu_probe_store() and cpu_release_store(), cpu hotplug test interface
enabled by CONFIG_ARCH_CPU_PROBE_RELEASE.
This patch changes the above interfaces to hold lock_device_hotplug().

_debug_hotplug_cpu() is a debug interface that puts cpu0 offline during
boot-up when CONFIG_DEBUG_HOTPLUG_CPU0 is set. After cpu0 is put offline
in this interface, however, /sys/devices/system/cpu/cpu0/online still
shows 1 (online).
This patch fixes _debug_hotplug_cpu() to update dev->offline when CPU
online/offline operation succeeded.

xen_init_spinlocks() currently calls static_key_slow_inc() before
jump_label_init() is invoked. When CONFIG_JUMP_LABEL is set (which usually is
the case) the effect of this static_key_slow_inc() is deferred until after
jump_label_init(). This is different from when CONFIG_JUMP_LABEL is not set, in
which case the key is set immediately. Thus, depending on the value of config
option, we may observe different behavior.
In addition, when we come to __jump_label_transform() from jump_label_init(),
the key (paravirt_ticketlocks_enabled) is already enabled. On processors where
ideal_nop is not the same as default_nop this will cause a BUG() since it is
expected that before a key is enabled the latter is replaced by the former
during initialization.
To address this problem we need to move
static_key_slow_inc(¶virt_ticketlocks_enabled) so that it is called
after jump_label_init(). We also need to make sure that this is done before
other cpus start to boot. early_initcall appears to be a good place to do so.
(Note that we cannot move whole xen_init_spinlocks() there since pv_lock_ops
need to be set before alternative_instructions() runs.)

Fiddling with CR3 for L2 is L1's job. It may set its own, different
identity map or simple leave it alone if unrestricted guest mode is
enabled. This also fixes reading back the current CR3 on L2 exits for
reporting it to L1.

kvm_set_cr0 performs checks on the state transition that may prevent
loading L1's cr0. For now we rely on the hardware to catch invalid
states loaded by L1 into its VMCS. Still, consistency checks on the host
state part of the VMCS on guest entry will have to be improved later on.

If a system has hung and it no longer responds to external
events, this patch adds the capability of doing a standard kdump
and system reboot then triggered by the system NMI command.
It is enabled when the nmi action is changed to "kdump" and the
kernel is built with CONFIG_KEXEC enabled.

The standard NMI handler dumps the states of all the cpus. This
includes a full register dump and stack trace. This can be way
more information than what is needed. This patch adds a
"summary" dump that is basically a form of the "ps" command. It
includes the symbolic IP address as well as the command field
and basic process information.
It is enabled when the nmi action is changed to "ips".

The current UV NMI handler has not been updated for the changes
in the system NMI handler and the perf operations. The UV NMI
handler reads an MMR in the UV Hub to check to see if the NMI
event was caused by the external 'system NMI' that the operator
can initiate on the System Mgmt Controller.
The problem arises when the perf tools are running, causing
millions of perf events per second on very large CPU count
systems. Previously this was okay because the perf NMI handler
ran at a higher priority on the NMI call chain and if the NMI
was a perf event, it would stop calling other NMI handlers
remaining on the NMI call chain.
Now the system NMI handler calls all the handlers on the NMI
call chain including the UV NMI handler. This causes the UV NMI
handler to read the MMRs at the same millions per second rate.
This can lead to significant performance loss and possible
system failures. It also can cause thousands of 'Dazed and
Confused' messages being sent to the system console. This
effectively makes perf tools unusable on UV systems.
To avoid this excessive overhead when perf tools are running,
this code has been optimized to minimize reading of the MMRs as
much as possible, by moving to the NMI_UNKNOWN notifier chain.
This chain is called only when all the users on the standard
NMI_LOCAL call chain have been called and none of them have
claimed this NMI.
There is an exception where the NMI_LOCAL notifier chain is
used. When the perf tools are in use, it's possible that the UV
NMI was captured by some other NMI handler and then either
ignored or mistakenly processed as a perf event. We set a
per_cpu ('ping') flag for those CPUs that ignored the initial
NMI, and then send them an IPI NMI signal. The NMI_LOCAL
handler on each cpu does not need to read the MMR, but instead
checks the in memory flag indicating it was pinged. There are
two module variables, 'ping_count' indicating how many requested
NMI events occurred, and 'ping_misses' indicating how many stray
NMI events. These most likely are perf events so it shows the
overhead of the perf NMI interrupts and how many MMR reads were avoided.
This patch also minimizes the reads of the MMRs by having the
first cpu entering the NMI handler on each node set a per HUB
in-memory atomic value. (Having a per HUB value avoids sending
lock traffic over NumaLink.) Both types of UV NMIs from the SMI
layer are supported.

This patch moves the UV NMI support from the x2apic file to a
new separate uv_nmi.c file in preparation for the next sequence
of patches. It prevents upcoming bloat of the x2apic file, and
has the added benefit of putting the upcoming /sys/module
parameters under the name 'uv_nmi' instead of 'x2apic_uv_x',
which was obscure.

Solve the problems around the broken definition of perf_event_mmap_page::
cap_usr_time and cap_usr_rdpmc fields which used to overlap, partially
fixed by:
860f085b74 ("perf: Fix broken union in 'struct perf_event_mmap_page'")
The problem with the fix (merged in v3.12-rc1 and not yet released
officially), noticed by Vince Weaver is that the new behavior is
not detectable by new user-space, and that due to the reuse of the
field names it's easy to mis-compile a binary if old headers are used
on a new kernel or new headers are used on an old kernel.
To solve all that make this change explicit, detectable and self-contained,
by iterating the ABI the following way:
- Always clear bit 0, and rename it to usrpage->cap_bit0, to at least not
confuse old user-space binaries. RDPMC will be marked as unavailable
to old binaries but that's within the ABI, this is a capability bit.
- Rename bit 1 to ->cap_bit0_is_deprecated and always set it to 1, so new
libraries can reliably detect that bit 0 is deprecated and perma-zero
without having to check the kernel version.
- Use bits 2, 3, 4 for the newly defined, correct functionality:
cap_user_rdpmc : 1, /* The RDPMC instruction can be used to read counts */
cap_user_time : 1, /* The time_* fields are used */
cap_user_time_zero : 1, /* The time_zero field is used */
- Rename all the bitfield names in perf_event.h to be different from the
old names, to make sure it's not possible to mis-compile it

The 'size' field can then be used in the future to add new fields and it
will act as a natural ABI version indicator as well.
Also adjust tools/perf/ userspace for the new definitions, noticed by
Adrian Hunter.

Due to the workaround described in commit 916f676f8 ("x86, efi: Retain
boot service code until after switching to virtual mode") EFI Boot
Service regions are mapped for a period during boot. Unfortunately, with
the limited size of the i386 direct kernel map it's possible that some
of the Boot Service regions will not be directly accessible, which
causes them to be ioremap()'d, triggering the above warning as the
regions are marked as E820_RAM in the e820 memmap.
There are currently only two situations where we need to map EFI Boot
Service regions,
1. To workaround the firmware bug described in 916f676f8
2. To access the ACPI BGRT image
but since we haven't seen an i386 implementation that requires either,
this simple fix should suffice for now.

Page tables in a read-only memory slot will currently cause a triple
fault because the page walker uses gfn_to_hva and it fails on such a slot.
OVMF uses such a page table; however, real hardware seems to be fine with
that as long as the accessed/dirty bits are set. Save whether the slot
is readonly, and later check it when updating the accessed and dirty bits.

x86 chips with LPSS (low power subsystem) such as Lynxpoint and
Baytrail have SoC like peripheral support and controllable pins.
At the moment, Baytrail needs the pinctrl-baytrail driver to let
peripherals control their gpio resources, but more pincontrol
functions such as pin muxing and grouping are possible to add
later.

On Intel SNB (SNB, SNB-EP), the event MEM_LOAD_UOPS_MISS_RETIRED
supports PEBS. It was missing for the SNB PEBS event constraint
table thereby preventing any measurement with PEBS for it.
This patch adds the event to the PEBS table for SNB.
WARNING: it should be noted that this event like a few others
are subject to the erratum BT241 for Xeon E5 (SNB-EP). As such,
the event may undercount when used with PEBS unless the
workaround is implemented. But without this patch and just the
workaround, the kernel would not allow precise sampling on this

The x86 fault handler bails in the middle of error handling when the
task has a fatal signal pending. For a subsequent patch this is a
problem in OOM situations because it relies on pagefault_out_of_memory()
being called even when the task has been killed, to perform proper
per-task OOM state unwinding.
Shortcutting the fault like this is a rather minor optimization that
saves a few instructions in rare cases. Just remove it for
user-triggered faults.
Use the opportunity to split the fault retry handling from actual fault
errors and add locking documentation that reads suprisingly similar to
ARM's.

Unlike global OOM handling, memory cgroup code will invoke the OOM killer
in any OOM situation because it has no way of telling faults occuring in
kernel context - which could be handled more gracefully - from
user-triggered faults.
Pass a flag that identifies faults originating in user space from the
architecture-specific fault handlers to generic code so that memcg OOM
handling can be improved.

Clean up the weird CP interrupt exception code by keeping a CP mask.
Andi suggested this implementation but weirdly didn't actually
implement it himself, do so now because it removes the conditional in
the interrupt handler and avoids the assumption its only on cnt2.

Add TSX event aliases, and export them from the kernel to perf.
These are used by perf stat -T and to allow
more user friendly access to events. The events are designed to
be fairly generic and may also apply to other architectures
implementing HTM. They all cover common situations that
happens during tuning of transactional code.
For Haswell we have to separate the HLE and RTM events,
as they are separate in the PMU.
This adds the following events:
tx-start Count start transaction (used by perf stat -T)
tx-commit Count commit of transaction
tx-abort Count all aborts
tx-conflict Count aborts due to conflict with another CPU.
tx-capacity Count capacity aborts (transaction too large)
Then matching el-* events for HLE
cycles-t Transactional cycles (used by perf stat -T)
* also exists on POWER8
cycles-ct Transactional cycles commited (used by perf stat -T)
* according to Michael Ellerman POWER8 has a cycles-transactional-committed,
* perf stat -T handles both cases
Note for useful abort profiling often precise has to be set,
as Haswell can only report the point inside the transaction
with precise=2.
For some classes of aborts, like conflicts, this is not needed,
as it makes more sense to look at the complete critical section.
This gives a clean set of generalized events to examine transaction
success and aborts. Haswell has additional events for TSX, but those
are more specialized for very specific situations.

Use the existing weight reporting facility to report the transaction
abort cost, that is the number of cycles wasted in aborts.
Haswell reports this in the PEBS record.
This was in fact the original user for weight.
This is a very useful sort key to concentrate on the most
costly aborts and a good metric for TSX tuning.

With checkpointed counters there can be a situation where the counter
is overflowing, aborts the transaction, is set back to a non overflowing
checkpoint, causes interupt. The interrupt doesn't see the overflow
because it has been checkpointed. This is then a spurious PMI, typically with
a ugly NMI message. It can also lead to excessive aborts.
Avoid this problem by:
- Using the full counter width for counting counters (earlier patch)
- Forbid sampling for checkpointed counters. It's not too useful anyways,
checkpointing is mainly for counting. The check is approximate
(to still handle KVM), but should catch the majority of cases.
- On a PMI always set back checkpointed counters to zero.

There was a bug in the handling of SNB-EP/IVB-EP uncore PCI
fixed counters, e.g., IMC.
It would cause erratic values to be returned for the IMC
clockticks event. This was due to a bogus hwc->config value
which was then written to PCI config space.
The erratic values can be seen via:
$ perf stat -a -C 0 -e uncore_imc_0/clockticks/ -I 1000 sleep 10
The fixed counter has most fields marked as reserved with
hw reset values of 0. Yet the kernel was defaulting to a
hwc->config = ~0 and that was causing the issues.
This patch sets the hwc->config values for fixed uncore event
to 0. Now, the values of IMC clockticks is correct.

The IvyBridge event CYCLE_ACTIVITY:CYCLES_LDM_PENDING can only
be measured on counters 0-3 when HT is off. When HT is on, you
only have counters 0-3.

Currently hugepage migration works well only for pmd-based hugepages
(mainly due to lack of testing,) so we had better not enable migration of
other levels of hugepages until we are ready for it.
Some users of hugepage migration (mbind, move_pages, and migrate_pages) do
page table walk and check pud/pmd_huge() there, so they are safe. But the
other users (softoffline and memory hotremove) don't do this, so without
this patch they can try to migrate unexpected types of hugepages.
To prevent this, we introduce hugepage_migration_support() as an
architecture dependent check of whether hugepage are implemented on a pmd
basis or not. And on some architecture multiple sizes of hugepages are
available, so hugepage_migration_support() also checks hugepage size.

The previous patch doing vmstats for TLB flushes ("mm: vmstats: tlb flush
counters") effectively missed UP since arch/x86/mm/tlb.c is only compiled
for SMP.
UP systems do not do remote TLB flushes, so compile those counters out on
UP.
arch/x86/kernel/cpu/mtrr/generic.c calls __flush_tlb() directly. This is
probably an optimization since both the mtrr code and __flush_tlb() write
cr4. It would probably be safe to make that a flush_tlb_all() (and then
get these statistics), but the mtrr code is ancient and I'm hesitant to
touch it other than to just stick in the counters.

I was investigating some TLB flush scaling issues and realized that we do
not have any good methods for figuring out how many TLB flushes we are
doing.
It would be nice to be able to do these in generic code, but the
arch-independent calls don't explicitly specify whether we actually need
to do remote flushes or not. In the end, we really need to know if we
actually _did_ global vs. local invalidations, so that leaves us with few
options other than to muck with the counters from arch-specific code.

The 64bit __copy_{from,to}_user_inatomic always called
copy_from_user_generic, but skipped the special optimizations for 1/2/4/8
byte accesses.
This especially hurts the futex call, which accesses the 4 byte futex
user value with a complicated fast string operation in a function call,
instead of a single movl.
Use __copy_{from,to}_user for _inatomic instead to get the same
optimizations. The only problem was the might_fault() in those functions.
So move that to new wrapper and call __copy_{f,t}_user_nocheck()
from *_inatomic directly.
32bit already did this correctly by duplicating the code.

entire flattened tree into the random pool at boot. May not be
significant, but shouldn't hurt either.
