1f50cc1705 KVM: PPC: Book3S HV: Fix H_CEDE return code for nested guests
The h_cede_tm kvm-unit-test currently fails when run inside an L1 guest
via the guest/nested hypervisor.

  FAIL h_cede_tm (2 tests, 1 unexpected failures)

While the test relates to transactional memory instructions, the actual
failure is due to the return code of the H_CEDE hypercall, which is
reported as 224 instead of 0. This happens even when no TM instructions
are issued.

224 is the value placed in r3 to execute a hypercall for H_CEDE, and r3
is where the caller expects the return code to be placed upon return.

In the case of guest running under a nested hypervisor, issuing H_CEDE
causes a return from H_ENTER_NESTED. In this case H_CEDE is
specially-handled immediately rather than later in
kvmppc_pseries_do_hcall() as with most other hcalls, but we forget to
set the return code for the caller, hence why kvm-unit-test sees the
224 return code and reports an error.

Guest kernels generally don't check the return value of H_CEDE, so
that likely explains why this hasn't caused issues outside of
kvm-unit-tests so far.

Fix this by setting r3 to 0 after we finish processing the H_CEDE.

1dff3064c7 KVM: PPC: Book3S HV: Treat TM-related invalid form instructions on P9 like the valid ones
On P9 DD2.2 due to a CPU defect some TM instructions need to be emulated by
KVM. This is handled at first by the hardware raising a softpatch interrupt
when certain TM instructions that need KVM assistance are executed in the
guest. Althought some TM instructions per Power ISA are invalid forms they
can raise a softpatch interrupt too. For instance, 'tresume.' instruction
as defined in the ISA must have bit 31 set (1), but an instruction that
matches 'tresume.' PO and XO opcode fields but has bit 31 not set (0), like
0x7cfe9ddc, also raises a softpatch interrupt. Similarly for 'treclaim.'
and 'trechkpt.' instructions with bit 31 = 0, i.e. 0x7c00075c and
0x7c0007dc, respectively. Hence, if a code like the following is executed
in the guest it will raise a softpatch interrupt just like a 'tresume.'

and then treats the executed instruction as a 'nop'.

However the POWER9 User's Manual, in section "4.6.10 Book II Invalid
Forms", informs that for TM instructions bit 31 is in fact ignored, thus
for the TM-related invalid forms ignoring bit 31 and handling them like the
valid forms is an acceptable way to handle them. POWER8 behaves the same
way too.

This commit changes the handling of the cases here described by treating
the TM-related invalid forms that can generate a softpatch interrupt
just like their valid forms (w/ bit 31 = 1) instead of as a 'nop' and by
gently reporting any other unrecognized case to the host and treating it as
illegal instruction instead of throwing a trace and treating it as a 'nop'.

afd313564c KVM: PPC: Book3S HV: Use RADIX_PTE_INDEX_SIZE in Radix MMU code
In kvmppc_unmap_free_pte() in book3s_64_mmu_radix.c, we use the
non-constant value PTE_INDEX_SIZE to clear a PTE page.

We can instead use the constant RADIX_PTE_INDEX_SIZE, because we know
this code will only be running when the Radix MMU is active.

Note that we already use RADIX_PTE_INDEX_SIZE for the allocation of
kvm_pte_cache.

cd758a9b57 KVM: PPC: Book3S HV: Use __gfn_to_pfn_memslot in HPT page fault handler
This makes the same changes in the page fault handler for HPT guests
that commits 31c8b0d069 ("KVM: PPC: Book3S HV: Use __gfn_to_pfn_memslot()
in page fault handler", 2018-03-01), 71d29f43b6 ("KVM: PPC: Book3S HV:
Don't use compound_order to determine host mapping size", 2018-09-11)
and 6579804c43 ("KVM: PPC: Book3S HV: Avoid crash from THP collapse
during radix page fault", 2018-10-04) made for the page fault handler
for radix guests.

In summary, where we used to call get_user_pages_fast() and then do
special handling for VM_PFNMAP vmas, we now call __get_user_pages_fast()
and then __gfn_to_pfn_memslot() if that fails, followed by reading the
Linux PTE to get the host PFN, host page size and mapping attributes.

This also brings in the change from SetPageDirty() to set_page_dirty_lock()
which was done for the radix page fault handler in commit c3856aeb29
("KVM: PPC: Book3S HV: Fix handling of large pages in radix page fault
handler", 2018-02-23).

4d39576259 KVM: Remove unnecessary asm/kvm_host.h includes
Remove includes of asm/kvm_host.h from files that already include
linux/kvm_host.h to make it more obvious that there is no ordering issue
between the two headers.  linux/kvm_host.h includes asm/kvm_host.h to
pick up architecture specific settings, and this will never change, i.e.
including asm/kvm_host.h after linux/kvm_host.h may seem problematic,
but in practice is simply redundant.

0577d1abe7 KVM: Terminate memslot walks via used_slots
Refactor memslot handling to treat the number of used slots as the de
facto size of the memslot array, e.g. return NULL from id_to_memslot()
when an invalid index is provided instead of relying on npages==0 to
detect an invalid memslot.  Rework the sorting and walking of memslots
in advance of dynamically sizing memslots to aid bisection and debug,
e.g. with luck, a bug in the refactoring will bisect here and/or hit a
WARN instead of randomly corrupting memory.

Alternatively, a global null/invalid memslot could be returned, i.e. so
callers of id_to_memslot() don't have to explicitly check for a NULL
memslot, but that approach runs the risk of introducing difficult-to-
debug issues, e.g. if the global null slot is modified.  Constifying
the return from id_to_memslot() to combat such issues is possible, but
would require a massive refactoring of arch specific code and would
still be susceptible to casting shenanigans.

Add function comments to update_memslots() and search_memslots() to
explicitly (and loudly) state how memslots are sorted.

Opportunistically stuff @hva with a non-canonical value when deleting a
private memslot on x86 to detect bogus usage of the freed slot.

No functional change intended.

2a49f61dfc KVM: Ensure validity of memslot with respect to kvm_get_dirty_log()
Rework kvm_get_dirty_log() so that it "returns" the associated memslot
on success.  A future patch will rework memslot handling such that
id_to_memslot() can return NULL, returning the memslot makes it more
obvious that the validity of the memslot has been verified, i.e.
precludes the need to add validity checks in the arch code that are
technically unnecessary.

To maintain ordering in s390, move the call to kvm_arch_sync_dirty_log()
from s390's kvm_vm_ioctl_get_dirty_log() to the new kvm_get_dirty_log().
This is a nop for PPC, the only other arch that doesn't select
KVM_GENERIC_DIRTYLOG_READ_PROTECT, as its sync_dirty_log() is empty.

Ideally, moving the sync_dirty_log() call would be done in a separate
patch, but it can't be done in a follow-on patch because that would
temporarily break s390's ordering.  Making the move in a preparatory
patch would be functionally correct, but would create an odd scenario
where the moved sync_dirty_log() would operate on a "different" memslot
due to consuming the result of a different id_to_memslot().  The
memslot couldn't actually be different as slots_lock is held, but the
code is confusing enough as it is, i.e. moving sync_dirty_log() in this
patch is the lesser of all evils.

0dff084607 KVM: Provide common implementation for generic dirty log functions
Move the implementations of KVM_GET_DIRTY_LOG and KVM_CLEAR_DIRTY_LOG
for CONFIG_KVM_GENERIC_DIRTYLOG_READ_PROTECT into common KVM code.
The arch specific implemenations are extremely similar, differing
only in whether the dirty log needs to be sync'd from hardware (x86)
and how the TLBs are flushed.  Add new arch hooks to handle sync
and TLB flush; the sync will also be used for non-generic dirty log
support in a future patch (s390).

The ulterior motive for providing a common implementation is to
eliminate the dependency between arch and common code with respect to
the memslot referenced by the dirty log, i.e. to make it obvious in the
code that the validity of the memslot is guaranteed, as a future patch
will rework memslot handling such that id_to_memslot() can return NULL.

to allow arch specific code to free arch specific resources in the old
memslot without having to cast away the attribute.  Freeing resources in

kvm_free_memslot() by eliminating the last usage of its @dont param.

Remove kvm_arch_create_memslot() now that all arch implementations are
effectively nops.  Removing kvm_arch_create_memslot() eliminates the
possibility for arch specific code to allocate memory prior to setting
a memslot, which sets the stage for simplifying kvm_free_memslot().

Allocate the rmap array during kvm_arch_prepare_memory_region() to pave
the way for removing kvm_arch_create_memslot() altogether.  Moving PPC's

 "Saner fs_parser.c guts and data structures. The system-wide registry
  of syntax types (string/enum/int32/oct32/.../etc.) is gone and so is
  the horror switch() in fs_parse() that would have to grow another case
  every time something got added to that system-wide registry.

  New syntax types can be added by filesystems easily now, and their
  namespace is that of functions - not of system-wide enum members. IOW,
  they can be shared or kept private and if some turn out to be widely
  useful, we can make them common library helpers, etc., without having
  to do anything whatsoever to fs_parse() itself.

  And we already get that kind of requests - the thing that finally
  pushed me into doing that was "oh, and let's add one for timeouts -
  things like 15s or 2h". If some filesystem really wants that, let them
  do it. Without somebody having to play gatekeeper for the variants
  blessed by direct support in fs_parse(), TYVM.

  Quite a bit of boilerplate is gone. And IMO the data structures make a

d4bf905307 powerpc: Fix CONFIG_TRACE_IRQFLAGS with CONFIG_VMAP_STACK
When CONFIG_PROVE_LOCKING is selected together with (now default)
CONFIG_VMAP_STACK, kernel enter deadlock during boot.

At the point of checking whether interrupts are enabled or not, the
value of MSR saved on stack is read using the physical address of the
stack. But at this point, when using VMAP stack the DATA MMU
translation has already been re-enabled, leading to deadlock.

Don't use the physical address of the stack when
CONFIG_VMAP_STACK is set.

9dc086f1e9 powerpc/futex: Fix incorrect user access blocking
The early versions of our kernel user access prevention (KUAP) were
written by Russell and Christophe, and didn't have separate
read/write access.

At some point I picked up the series and added the read/write access,
but I failed to update the usages in futex.h to correctly allow read
and write.

However we didn't notice because of another bug which was causing the
low-level code to always enable read and write. That bug was fixed
recently in commit 1d8f739b07 ("powerpc/kuap: Fix set direction in
allow/prevent_user_access()").

futex_atomic_cmpxchg_inatomic() is passed the user address as %3 and
does:

  1:     lwarx   %1,  0, %3
         cmpw    0,  %1, %4
         bne-    3f
  2:     stwcx.  %5,  0, %3

Which clearly loads and stores from/to %3. The logic in
arch_futex_atomic_op_inuser() is similar, so fix both of them to use
allow_read_write_user().

Without this fix, and with PPC_KUAP_DEBUG=y, we see eg:

782e067dba virtio-blk: remove VIRTIO_BLK_F_SCSI support
Since the need for a special flag to support SCSI passthrough on a
block device was added in May 2017 the SCSI passthrough support in
virtio-blk has been disabled.  It has always been a bad idea
(just ask the original author..) and we have virtio-scsi for proper
passthrough.  The feature also never made it into the virtio 1.0
or later specifications.

0ed1325967 mm/mmu_gather: invalidate TLB correctly on batch allocation failure and flush
Architectures for which we have hardware walkers of Linux page table
should flush TLB on mmu gather batch allocation failures and batch flush.
Some architectures like POWER supports multiple translation modes (hash
and radix) and in the case of POWER only radix translation mode needs the
above TLBI.  This is because for hash translation mode kernel wants to
avoid this extra flush since there are no hardware walkers of linux page
table.  With radix translation, the hardware also walks linux page table
and with that, kernel needs to make sure to TLB invalidate page walk cache
before page table pages are freed.

TLB caches for RCU_TABLE_FREE")

The changes to sparc are to make sure we keep the old behavior since we
are now removing HAVE_RCU_TABLE_NO_INVALIDATE.  The default value for
tlb_needs_table_invalidate is to always force an invalidate and sparc can
avoid the table invalidate.  Hence we define tlb_needs_table_invalidate to
false for sparc architecture.

12e4d53f3f powerpc/mmu_gather: enable RCU_TABLE_FREE even for !SMP case
Patch series "Fixup page directory freeing", v4.

This is a repost of patch series from Peter with the arch specific changes
except ppc64 dropped.  ppc64 changes are added here because we are redoing
the patch series on top of ppc64 changes.  This makes it easy to backport
these changes.  Only the first 2 patches need to be backported to stable.

The thing is, on anything SMP, freeing page directories should observe the
exact same order as normal page freeing:

 1) unhook page/directory
 2) TLB invalidate
 3) free page/directory

Without this, any concurrent page-table walk could end up with a
Use-after-Free.  This is esp.  trivial for anything that has software
page-table walkers (HAVE_FAST_GUP / software TLB fill) or the hardware
caches partial page-walks (ie.  caches page directories).

Even on UP this might give issues since mmu_gather is preemptible these
days.  An interrupt or preempted task accessing user pages might stumble
into the free page if the hardware caches page directories.

This patch series fixes ppc64 and add generic MMU_GATHER changes to
support the conversion of other architectures.  I haven't added patches
w.r.t other architecture because they are yet to be acked.

This patch (of 9):

A followup patch is going to make sure we correctly invalidate page walk
cache before we free page table pages.  In order to keep things simple
enable RCU_TABLE_FREE even for !SMP so that we don't have to fixup the
!SMP case differently in the followup patch

!SMP case is right now broken for radix translation w.r.t page walk
cache flush.  We can get interrupted in between page table free and
that would imply we have page walk cache entries pointing to tables
which got freed already.  Michael said "both our platforms that run on
Power9 force SMP on in Kconfig, so the !SMP case is unlikely to be a
problem for anyone in practice, unless they've hacked their kernel to
build it !SMP."

those of user space.  For this it needs to know when it has reached a
'leaf' entry in the page tables.  This information is provided by the
p?d_leaf() functions/macros.

For powerpc p?d_is_leaf() functions already exist.  Export them using the
new p?d_leaf() name.

5f2fb52fac kbuild: rename hostprogs-y/always to hostprogs/always-y
In old days, the "host-progs" syntax was used for specifying host
programs. It was renamed to the current "hostprogs-y" in 2004.

It is typically useful in scripts/Makefile because it allows Kbuild to
selectively compile host programs based on the kernel configuration.

This commit renames like follows:

  always       ->  always-y
  hostprogs-y  ->  hostprogs

So, scripts/Makefile will look like this:

  always-$(CONFIG_BUILD_BIN2C) += ...
  always-$(CONFIG_KALLSYMS)    += ...
      ...
  hostprogs := $(always-y) $(always-m)

I think this makes more sense because a host program is always a host
program, irrespective of the kernel configuration. We want to specify
which ones to compile by CONFIG options, so always-y will be handier.

The "always", "hostprogs-y", "hostprogs-m" will be kept for backward
compatibility for a while.

In order to provide a clearer, more symmetric API for pinning and
unpinning DMA pages.  This way, pin_user_pages*() calls match up with

commit f382fb0bce ("block: remove legacy IO schedulers").

The IOSCHED_DEADLINE was replaced by MQ_IOSCHED_DEADLINE and it will be
now enabled by default (along with MQ_IOSCHED_KYBER).

5e84dd547b powerpc/configs/skiroot: Enable some more hardening options
Enable more hardening options.

Note BUG_ON_DATA_CORRUPTION selects DEBUG_LIST and is essentially just
a synonym for it.

DEBUG_SG, DEBUG_NOTIFIERS, DEBUG_LIST, DEBUG_CREDENTIALS and
SCHED_STACK_END_CHECK should all be low overhead and just add a few
extra checks.

SLAB_FREELIST_RANDOM, and SLUB_DEBUG_ON will add some overhead to the
SLAB allocator, but nothing that should be meaningful for skiroot.

Unselecting SLAB_MERGE_DEFAULT causes the SLAB to use more memory, but
the skiroot kernel shouldn't be memory constrained on any of our
systems, all it does is run a small bootloader.

Disabling merging has some security/robustness benefit as it means a
user-after-free or overflow will be limited to the objects in that
slab, rather than potentially affecting objects from unrelated slabs
that have been merged.

Note also that slab merging is disabled anyway by enabling
SLUB_DEBUG_ON, because of the SLAB_NEVER_MERGE mask.

3554c12d83 powerpc/configs/skiroot: Disable xmon default & enable reboot on panic
If the skiroot kernel crashes we don't want it sitting at an xmon
prompt forever. Instead it's more helpful to reboot and bring the
boot loader back up, and if the crash was transient we can then boot
successfully.

Similarly if we panic we should reboot, with a short timeout in case
someone is watching the console.

It also enables SECURITY_LOCKDOWN_LSM with _EARLY and
LOCK_DOWN_KERNEL_FORCE_INTEGRITY options enabled. This still allows
xmon to be used in read-only mode.

MODULE_SIG is selected by lockdown, so it is still enabled.

Because we're setting LOCK_DOWN_KERNELFORCE_INTEGRITY=y we also need
to enable KEXEC_FILE=y so that kexec continues to work.

028fb6ded7 powerpc/configs/skiroot: Drop HID_LOGITECH
Commit bdd08fff49 ("HID: logitech: Add depends on LEDS_CLASS to
Logitech Kconfig entry") made HID_LOGITECH depend on LEDS_CLASS which
we do not enable, meaning we are not actually enabling those drivers
any more.

The Kconfig help text suggests USB HID compliant Logictech devices
will continue to work without HID_LOGITECH, so just drop it.

The QLGE driver moved to staging in commit 955315b0dc ("qlge: Move

It sounds like the device is obsolete, so drop the driver.

43e76cd368 powerpc: Do not consider weak unresolved symbol relocations as bad
Commit 8580ac9404 ("bpf: Process in-kernel BTF") introduced two weak
symbols that may be unresolved at link time which result in an absolute
relocation to 0. relocs_check.sh emits the following warning:

"WARNING: 2 bad relocations
c000000001a41478 R_PPC64_ADDR64    _binary__btf_vmlinux_bin_start
c000000001a41480 R_PPC64_ADDR64    _binary__btf_vmlinux_bin_end"

whereas those relocations are legitimate even for a relocatable kernel
compiled with -pie option.

relocs_check.sh already excluded some weak unresolved symbols explicitly:
remove those hardcoded symbols and add some logic that parses the symbols
using nm, retrieves all the weak unresolved symbols and excludes those from
the list of the potential bad relocations.
