ab8edab132 vhost-scsi: Take configfs group dependency during VHOST_SCSI_SET_ENDPOINT
This patch addresses a bug where individual vhost-scsi configfs endpoint
groups can be removed from below while active exports to QEMU userspace
still exist, resulting in an OOPs.

It adds a configfs_depend_item() in vhost_scsi_set_endpoint() to obtain
an explicit dependency on se_tpg->tpg_group in order to prevent individual
vhost-scsi WWPN endpoints from being released via normal configfs methods
while an QEMU ioctl reference still exists.

Also, add matching configfs_undepend_item() in vhost_scsi_clear_endpoint()
to release the dependency, once QEMU's reference to the individual group
at /sys/kernel/config/target/vhost/$WWPN/$TPGT is released.

This reworks vhost core dropping unnecessary RCU uses in favor of VQ mutexes
 which are used on fast path anyway.  This fixes worst-case latency for users
 which change the memory mappings a lot.
 Memory allocation for vhost-net now supports fallback on vmalloc (same as for
 vhost-scsi) this makes it possible to create the device on systems where memory
 is very fragmented, with slightly lower performance.

9f977ef7b6 vhost-scsi: Include prot_bytes into expected data transfer length
This patch updates vhost_scsi_get_tag() to accept the combined
expected data transfer length + T10 PI bytes as the value passed
into target_submit_cmd().

This is required now that target-core logic in commit 14ef9200
expects to subtract se_cmd->prot_length from se_cmd->data_length.

    vhost: replace rcu with mutex
replaced rcu sync for memory accesses with VQ mutex locl/unlock.
This is correct since all accesses are under VQ mutex, but incomplete:
we still do useless rcu lock/unlock operations, someone might copy this
code into some other context where this won't be right.
This use of RCU is also non standard and hard to understand.
Let's copy the pointer to each VQ structure, this way
the access rules become straight-forward, and there's
no need for RCU anymore.

All memory accesses are done under some VQ mutex.
So lock/unlock all VQs is a faster equivalent of synchronize_rcu()
for memory access changes.
Some guests cause a lot of these changes, so it's helpful
to make them faster.

23cc5a991c vhost-net: extend device allocation to vmalloc
Michael Mueller provided a patch to reduce the size of
vhost-net structure as some allocations could fail under
memory pressure/fragmentation. We are still left with
high order allocations though.

This patch is handling the problem at the core level, allowing
vhost structures to use vmalloc() if kmalloc() failed.

As vmalloc() adds overhead on a critical network path, add __GFP_REPEAT
to kzalloc() flags to do this fallback only when really needed.

People are still looking at cleaner ways to handle the problem
at the API level, probably passing in multiple iovecs.
This hack seems consistent with approaches
taken since then by drivers/vhost/scsi.c and net/core/dev.c

95e7c4341b vhost/scsi: Enable T10 PI IOV -> SGL memory mapping
This patch updates vhost_scsi_handle_vq() to check for the existance
of virtio_scsi_cmd_req_pi comparing vq->iov[0].iov_len in order to
calculate seperate data + protection SGLs from data_num.

Also update tcm_vhost_submission_work() to pass the pre-allocated
cmd->tvc_prot_sgl[] memory into target_submit_cmd_map_sgls(), and
update vhost_scsi_get_tag() parameters to accept scsi_tag, lun, and
task_attr.

e31885dd90 vhost/scsi: Add T10 PI IOV -> SGL memory mapping logic
This patch adds vhost_scsi_map_iov_to_prot() to perform the mapping of
T10 data integrity memory between virtio iov + struct scatterlist using
get_user_pages_fast() following existing code.

As with vhost_scsi_map_iov_to_sgl(), this does sanity checks against the
total prot_sgl_count vs. pre-allocated SGLs, and loops across protection
iovs using vhost_scsi_map_to_sgl() to perform the actual memory mapping.

Also update tcm_vhost_release_cmd() to release associated tvc_prot_sgl[]
struct page.

5a01d08217 vhost/scsi: Move sanity check into vhost_scsi_map_iov_to_sgl
Move the overflow check for sgl_count > TCM_VHOST_PREALLOC_SGLS into
vhost_scsi_map_iov_to_sgl() so that it's based on the total number
of SGLs for all IOVs, instead of single IOVs.

Also, rename TCM_VHOST_PREALLOC_PAGES -> TCM_VHOST_PREALLOC_UPAGES
to better describe pointers to user-space pages.

e70beee783 target: Pass in transport supported PI at session initialization
In order to support local WRITE_INSERT + READ_STRIP operations for
non PI enabled fabrics, the fabric driver needs to be able signal
what protection offload operations are supported.

This is done at session initialization time so the modes can be
signaled by individual se_wwn + se_portal_group endpoints, as well
as optionally across different transports on the same endpoint.

For iser-target, set TARGET_PROT_ALL if the underlying ib_device
has already signaled PI offload support, and allow this to be
exposed via a new iscsit_transport->iscsit_get_sup_prot_ops()
callback.

For loopback, set TARGET_PROT_ALL to signal SCSI initiator mode
operation.

For all other drivers, set TARGET_PROT_NORMAL to disable fabric
level PI.

131e6abc67 target: Add TFO->abort_task for aborted task resources release
Now that TASK_ABORTED status is not generated for all cases by
TMR ABORT_TASK + LUN_RESET, a new TFO->abort_task() caller is
necessary in order to give fabric drivers a chance to unmap
hardware / software resources before the se_cmd descriptor is
released via the normal TFO->release_cmd() codepath.

This patch adds TFO->aborted_task() in core_tmr_abort_task()
in place of the original transport_send_task_abort(), and
also updates all fabric drivers to implement this caller.

The fabric drivers that include changes to perform cleanup
via ->aborted_task() are:

  - iscsi-target
  - iser-target
  - srpt
  - tcm_qla2xxx

The fabric drivers that currently set ->aborted_task() to
NOPs are:

  - loopback
  - tcm_fc
  - usb-gadget
  - sbp-target
  - vhost-scsi

For the latter five, there appears to be no additional cleanup
required before invoking TFO->release_cmd() to release the
se_cmd descriptor.

d8316f3991 vhost: fix total length when packets are too short
When mergeable buffers are disabled, and the
incoming packet is too large for the rx buffer,
get_rx_bufs returns success.

This was intentional in order for make recvmsg
truncate the packet and then handle_rx would
detect err != sock_len and drop it.

Unfortunately we pass the original sock_len to
recvmsg - which means we use parts of iov not fully
validated.

Fix this up by detecting this overrun and doing packet drop
immediately.

b0c057ca7e vhost: fix a theoretical race in device cleanup
vhost_zerocopy_callback accesses VQ right after it drops a ubuf
reference.  In theory, this could race with device removal which waits
on the ubuf kref, and crash on use after free.

Do all accesses within rcu read side critical section, and synchronize
on release.

Since callbacks are always invoked from bh, synchronize_rcu_bh seems
enough and will help release complete a bit faster.

0ad8b480d6 vhost: fix ref cnt checking deadlock
vhost checked the counter within the refcnt before decrementing.  It
really wanted to know that it is the one that has the last reference, as
a way to batch freeing resources a bit more efficiently.

Note: we only let refcount go to 0 on device release.

This works well but we now access the ref counter twice so there's a
race: all users might see a high count and decide to defer freeing
resources.
In the end no one initiates freeing resources until the last reference
is gone (which is on VM shotdown so might happen after a looooong time).

Let's do what we probably should have done straight away:
switch from kref to plain atomic, documenting the
semantics, return the refcount value atomically after decrement,
then use that to avoid the deadlock.

6f6b5d1ec5 percpu_ida: Make percpu_ida_alloc + callers accept task state bitmask
This patch changes percpu_ida_alloc() + callers to accept task state
bitmask for prepare_to_wait() for code like target/iscsi that needs
it for interruptible sleep, that is provided in a subsequent patch.

It now expects TASK_UNINTERRUPTIBLE when the caller is able to sleep
waiting for a new tag, or TASK_RUNNING when the caller cannot sleep,
and is forced to return a negative value when no tags are available.

60a01f558a vhost/scsi: Fix incorrect usage of get_user_pages_fast write parameter
This patch addresses a long-standing bug where the get_user_pages_fast()
write parameter used for setting the underlying page table entry permission
bits was incorrectly set to write=1 for data_direction=DMA_TO_DEVICE, and
passed into get_user_pages_fast() via vhost_scsi_map_iov_to_sgl().

However, this parameter is intended to signal WRITEs to pinned userspace
PTEs for the virtio-scsi DMA_FROM_DEVICE -> READ payload case, and *not*
for the virtio-scsi DMA_TO_DEVICE -> WRITE payload case.

This bug would manifest itself as random process segmentation faults on
KVM host after repeated vhost starts + stops and/or with lots of vhost
endpoints + LUNs.

595cb75498 vhost/scsi: use vmalloc for order-10 allocation
As vhost scsi device struct is large, if the device is
created on a busy system, kzalloc() might fail, so this patch does a
fallback to vzalloc().

As vmalloc() adds overhead on data-path, add __GFP_REPEAT
to kzalloc() flags to do this fallback only when really needed.

scatterlist + user-space page pointer memory using se_sess->sess_cmd_map

f7c6be404d vhost_net: correctly limit the max pending buffers
As Michael point out, We used to limit the max pending DMAs to get better cache
utilization. But it was not done correctly since it was one done when there's no
new buffers submitted from guest. Guest can easily exceeds the limitation by
keeping sending packets.

So this patch moves the check into main loop. Tests shows about 5%-10%
improvement on per cpu throughput for guest tx.

c92112aed3 vhost_net: use vhost_add_used_and_signal_n() in vhost_zerocopy_signal_used()
We tend to batch the used adding and signaling in vhost_zerocopy_callback()
which may result more than 100 used buffers to be updated in
vhost_zerocopy_signal_used() in some cases. So switch to use
vhost_add_used_and_signal_n() to avoid multiple calls to
vhost_add_used_and_signal(). Which means much less times of used index
updating and memory barriers.

2% performance improvement were seen on netperf TCP_RR test.

c38e39c378 vhost-net: fix use-after-free in vhost_net_flush
vhost_net_ubuf_put_and_wait has a confusing name:
it will actually also free it's argument.
Thus since commit 1280c27f8e
    "vhost-net: flush outstanding DMAs on memory change"
vhost_net_flush tries to use the argument after passing it
to vhost_net_ubuf_put_and_wait, this results
in use after free.
To fix, don't free the argument in vhost_net_ubuf_put_and_wait,
add an new API for callers that want to free ubufs.

This patch coverts vhost/scsi to se_cmd->cmd_kref TARGET_SCF_ACK_KREF
usage, instead of assuming that vhost_scsi_free_cmd() is always called
before TCM processing is completed in the response fast path.

This includes adding vhost_scsi_check_stop_free() -> target_put_sess_cmd()
to perform the second se_cmd->cmd_kref put, and moving vhost_scsi_free_cmd()
resource release into tcm_vhost_release_cmd() that is invoked once the last
se_cmd->cmd_kref put occurs.

6c131d0c58 vhost/scsi: Drop unnecessary wait_for_tasks=true usage with transport_generic_free_cmd
This patch changes vhost_scsi_free_cmd() to call transport_generic_free_cmd()
with wait_for_tasks=false in order to avoid the extra se_cmd->t_state_lock
access for the wait_for_tasks=true case.

This is unnecessary because vhost_scsi_free_cmd() is only ever called by
vhost_scsi_complete_cmd_work() after TCM completion handoff, and by
vhost_scsi_handle_vq() exception code before TCM submission handoff, so
there is never a case where se_cmd is still active from TCM's perspective
when transport_generic_free_cmd() is called.

When we decide not use zero-copy, msg.control should be set to NULL otherwise

RESET_OWNER ioctl would leave the fd in a bad state if
memory allocation failed: device is stopped
but owner is not reset. Make state changes
after allocating memory, such that a failed

f2f0173d6a tcm_vhost: Wait for pending requests in vhost_scsi_flush()
Unlike tcm_vhost_evt requests, tcm_vhost_cmd requests are passed to the
target core system, we can not make sure all the pending requests will
be finished by flushing the virt queue.

In this patch, we do refcount for every tcm_vhost_cmd requests to make
vhost_scsi_flush() wait for all the pending requests issued before the
flush operation to be finished.

This is useful when we call vhost_scsi_clear_endpoint() to stop
tcm_vhost. No new requests will be passed to target core system because
we clear the endpoint by setting vs_tpg to NULL. And we wait for all the
old requests. These guarantee no requests will be leaked and existing
requests will be completed.

This is useful for any device who wants device specific fields per vq.
For example, tcm_vhost wants a per vq field to track requests which are
in flight on the vq. Also, on top of this we can add patches to move
things like ubufs from vhost.h out to net.c.

This patch adds hotplug and hotunplug support to tcm_vhost.

LUN in guest.

The e{uid,gid} --> {uid,gid} credentials fix conflicted with the

The be2net driver had a bug fix in 'net' that overlapped with the VLAN
interface changes by Patrick McHardy in net-next.

An IGB conflict existed because in 'net' the build_skb() support was
reverted, and in 'net-next' there was a comment style fix within that
code.

Several batman-adv conflicts were resolved by making sure that all
calls to batadv_is_my_mac() are changed to have a new bat_priv first
argument.

Eric Dumazet's TS ECR fix in TCP in 'net' conflicted with the F-RTO
rewrite in 'net-next', mostly overlapping changes.

Thanks to Stephen Rothwell and Antonio Quartulli for help with several
of these merge resolutions.

70181d5120 vhost_net: remove tx polling state
After commit 2b8b328b61 (vhost_net: handle polling
errors when setting backend), we in fact track the polling state through
poll->wqh, so there's no need to duplicate the work with an extra
vhost_net_polling_state. So this patch removes this and make the code simpler.

This patch also removes the all tx starting/stopping code in tx path according
to Michael's suggestion.

Netperf test shows almost the same result in stream test, but gets improvements
on TCP_RR tests (both zerocopy or copy) especially on low load cases.

Tested between multiqueue kvm guest and external host with two direct
connected 82599s.

dfd5d5692c tcm_vhost: Initialize vq->last_used_idx when set endpoint
This patch fixes guest hang when booting seabios and guest.

  [    0.576238] scsi0 : Virtio SCSI HBA
  [    0.616754] virtio_scsi virtio1: request:id 0 is not a head!

vq->last_used_idx is initialized only when /dev/vhost-scsi is
opened or closed.

   vhost_scsi_open -> vhost_dev_init() -> vhost_vq_reset()
   vhost_scsi_release() -> vhost_dev_cleanup -> vhost_vq_reset()

So, when guest talks to tcm_vhost after seabios does, vq->last_used_idx
still contains the old valule for seabios. This confuses guest.

Fix this by calling vhost_init_used() to init vq->last_used_idx when
we set endpoint.

4f7f46d32c tcm_vhost: Use vq->private_data to indicate if the endpoint is setup
Currently, vs->vs_endpoint is used indicate if the endpoint is setup or
not. It is set or cleared in vhost_scsi_set_endpoint() or
vhost_scsi_clear_endpoint() under the vs->dev.mutex lock. However, when
we check it in vhost_scsi_handle_vq(), we ignored the lock.

Instead of using the vs->vs_endpoint and the vs->dev.mutex lock to
indicate the status of the endpoint, we use per virtqueue
vq->private_data to indicate it. In this way, we can only take the
vq->mutex lock which is per queue and make the concurrent multiqueue
process having less lock contention. Further, in the read side of
vq->private_data, we can even do not take the lock if it is accessed in
the vhost worker thread, because it is protected by "vhost rcu".

This patch adds a VHOST_SCSI_FEATURES mask minus VIRTIO_RING_F_EVENT_IDX
so that vhost-scsi-pci userspace will strip this feature bit once
GET_FEATURES reports it as being unsupported on the host.

This is to avoid a bug where ->handle_kicks() are missed when EVENT_IDX
is enabled by default in userspace code.

f87d0fbb57 vringh: host-side implementation of virtio rings.
Getting use of virtio rings correct is tricky, and a recent patch saw
an implementation of in-kernel rings (as separate from userspace).

This abstracts the business of dealing with the virtio ring layout
from the access (userspace or direct); to do this, we use function
pointers, which gcc inlines correctly.

46aa92d1ba vhost/net: fix heads usage of ubuf_info
ubuf info allocator uses guest controlled head as an index,
so a malicious guest could put the same head entry in the ring twice,
and we will get two callbacks on the same value.
To fix use upend_idx which is guaranteed to be unique.

1b7f390eb3 tcm_vhost: Multi-queue support
This adds virtio-scsi multi-queue support to tcm_vhost. In order to use
multi-queue, guest side multi-queue support is need. It can
be found here:

Currently, only one thread is created by vhost core code for each
vhost_scsi instance. Even if there are multi-queues, all the handling of
guest kick (vhost_scsi_handle_kick) are processed in one thread. This is
not optimal. Luckily, most of the work is offloaded to the tcm_vhost
workqueue.

Some initial perf numbers:
1 queue,  4 targets, 1 lun per target
4K request size, 50% randread + 50% randwrite: 127K/127k IOPS

4 queues, 4 targets, 1 lun per target
4K request size, 50% randread + 50% randwrite: 181K/181k IOPS

67e18cf9ab tcm_vhost: Multi-target support
In order to take advantages of Paolo's multi-queue virito-scsi, we need
multi-target support in tcm_vhost first. Otherwise all the requests go
to one queue and other queues are idle.

This patch makes:

1. All the targets under the wwpn is seen and can be used by guest.
2. No need to pass the tpgt number in struct vhost_scsi_target to
   tcm_vhost.ko. Only wwpn is needed.
3. We can always pass max_target = 255 to guest now, since we abort the
   request who's target id does not exist.

- Simplfy lock in vhost_scsi_set_endpoint
- Return -EEXIST when does not match

2b8b328b61 vhost_net: handle polling errors when setting backend
Currently, the polling errors were ignored, which can lead following issues:

- vhost remove itself unconditionally from waitqueue when stopping the poll,
  this may crash the kernel since the previous attempt of starting may fail to
  add itself to the waitqueue
- userspace may think the backend were successfully set even when the polling
  failed.

Solve this by:

- check poll->wqh before trying to remove from waitqueue
- report polling errors in vhost_poll_start(), tx_poll_start(), the return value
  will be checked and returned when userspace want to set the backend

After this fix, there still could be a polling failure after backend is set, it
will addressed by the next patch.

f9611c43ab vhost-net: enable zerocopy tx by default
Zero copy TX has been around for a while now.
We seem to be down to eliminating theoretical bugs
and performance tuning at this point:
it's probably time to enable it by default so that
most users get the benefit.

Keep the flag around meanwhile so users can experiment
with disabling this if they experience regressions.
I expect that we will remove it in the future.

of managing heads which isn't necessary: we
could simly update used ring directly
same as with zerocopy disabled.

Things seem to run a bit faster if we detect

vring changes already do a flush internally where appropriate, so we do
not need a second flush.

It's currently not very expensive but a follow-up patch makes flush more
heavy-weight, so remove the extra flush here to avoid regressing
performance if call or kick fds are changed on data path.

These packet counters are used to drive the zercopy
selection heuristic so nothing too bad happens if they are off a bit -

eaae8132ef vhost-net: select tx zero copy dynamically
Even when vhost-net is in zero-copy transmit mode,
net core might still decide to copy the skb later
which is somewhat slower than a copy in user
context: data copy overhead is added to the cost of
page pin/unpin. The result is that enabling tx zero copy
option leads to higher CPU utilization for guest to guest
and guest to host traffic.

To fix this, suppress zero copy tx after a given number of
packets triggered late data copy. Re-enable periodically
to detect workload changes.

e19d6763cc skb: report completion status for zero copy skbs
Even if skb is marked for zero copy, net core might still decide
to copy it later which is somewhat slower than a copy in user context:
besides copying the data we need to pin/unpin the pages.

Add a parameter reporting such cases through zero copy callback:
if this happens a lot, device can take this into account
and switch to copying in user context.

This patch updates all users but ignores the passed value for now:
it will be used by follow-up patches.

9f0abc1554 tcm_vhost: Convert I/O path to use target_submit_cmd_map_sgls
This patch converts tcm_vhost to use target_submit_cmd_map_sgls() for
I/O submission and mapping of pre-allocated SGL memory from incoming
virtio-scsi SGL memory -> se_cmd descriptors.

This includes removing the original open-coded fabric uses of target
core callers to support transport_generic_map_mem_to_cmd() between
target_setup_cmd_from_cdb() and transport_handle_cdb_direct() logic.

It also includes adding a handful of new tcm_vhost_cmnd member +
assignments in vhost_scsi_allocate_cmd() used from cmwq process
context I/O submission within tcm_vhost_submission_work()

9c58b7ddd7 target: Simplify fabric sense data length handling
Every fabric driver has to supply a se_tfo->set_fabric_sense_len()
method, just so iSCSI can return an offset of 2.  However, every fabric
driver is already allocating a sense buffer and passing it into the
target core, either via transport_init_se_cmd() or target_submit_cmd().

So instead of having iSCSI pass the start of its sense buffer into the
core and then later tell the core to skip the first 2 bytes, it seems
easier for iSCSI just to do the offset of 2 when it passes the sense
buffer into the core.  Then we can drop the se_tfo->set_fabric_sense_len()
everywhere, and just add a couple of lines of code to iSCSI to set the
sense data length to the beginning of the buffer right before it sends
it over the network.

(nab: Remove .set_fabric_sense_len usage from tcm_qla2xxx_npiv_ops +
      change transport_get_sense_buffer to follow v3.6-rc6 code w/o
      ->set_fabric_sense_len usage)

6de7145ca3 tcm_vhost: Fix vhost_scsi_target structure alignment
Here TRANSPORT_IQN_LEN is 224, which is a multiple of 4.
Since vhost_tpgt is 2 bytes and abi_version is 4, the total size would
be 230.  But gcc needs struct size be aligned to first field size, which
is 4 bytes, so it pads the structure by extra 2 bytes to the total of
232.

This padding is very undesirable in an ABI:
- it can not be initialized easily
- it can not be checked easily
- it can leak information between kernel and userspace

Simplest solution is probably just to make the padding
explicit.

163049aefd vhost: make vhost work queue visible
The vhost work queue allows processing to be done in vhost worker thread
context, which uses the owner process mm.  Access to the vring and guest
memory is typically only possible from vhost worker context so it is
useful to allow work to be queued directly by users.

Currently vhost_net only uses the poll wrappers which do not expose the
work queue functions.  However, for tcm_vhost (vhost_scsi) it will be
necessary to queue custom work.

d7ffde35e3 vhost: use USER_DS in vhost_worker thread
On some architectures address spaces are set up in a way that this is
not necessary to work properly but on some others (like s390) it is.
Make sure we operate on the user address space to allow copy_xxx_user()
from the vhost_worker() thread by setting it explicitly before calling
use_mm() and revert it after unuse_mm().

dbf34207c6 vhost_net: re-poll only on EAGAIN or ENOBUFS
Currently, we restart tx polling unconditionally when sendmsg()
fails. This would cause unnecessary wakeups of vhost wokers and waste
cpu utlization when evil userspace(guest driver) is able to hit EFAULT or
EINVAL.

The polling is only needed when the socket send buffer were exceeded or not
enough memory. So fix this by restarting polling only when sendmsg() returns
EAGAIN/ENOBUFS.

When we want to disable vhost_net backend while there's a tx work, a possible
NULL pointer defernece may happen we we try to deference the vq->bufs after
vhost_net_set_backend() assign a NULL to it.

As suggested by Michael, fix this by checking the vq->bufs instead of
vhost_sock_zcopy().

itself, not the arg value as the field name and the function signature

d550dda192 vhost: don't forget to schedule()
This is a tiny, but important, patch to vhost.

Vhost's worker thread only called schedule() when it had no work to do, and
it wanted to go to sleep. But if there's always work to do, e.g., the guest
is running a network-intensive program like netperf with small message sizes,
schedule() was *never* called. This had several negative implications (on
non-preemptive kernels):

 1. Passing time was not properly accounted to the "vhost" process (ps and
    top would wrongly show it using zero CPU time).

 2. Sometimes error messages about RCU timeouts would be printed, if the
    core running the vhost thread didn't schedule() for a very long time.

 3. Worst of all, a vhost thread would "hog" the core. If several vhost
    threads need to share the same core, typically one would get most of the
    CPU time (and its associated guest most of the performance), while the
    others hardly get any work done.

The trivial solution is to add

	if (need_resched())
		schedule();

After doing every piece of work. This will not do the heavy schedule() all
the time, just when the timer interrupt decided a reschedule is warranted
(so need_resched returns true).

7c7c7f01cc vhost-net: add module alias (v2.1)
By adding some module aliases, programs (or users) won't have to explicitly
call modprobe. Vhost-net will always be available if built into the kernel.
It does require assigning a permanent minor number for depmod to work.

  - add missing entry in documentation for loop-control

On backend change, we flushed out outstanding skbs
but forgot to update the used ring, so that
done entries were left in the ubuf_info ring.
As a result we lose heads or complete incorrect ones,
crashing the guest or leaking memory.

This adds experimental zero copy support in vhost-net,
disabled by default. To enable, set
experimental_zcopytx module option to 1.

This patch maintains the outstanding userspace buffers in the
sequence it is delivered to vhost. The outstanding userspace buffers
will be marked as done once the lower device buffers DMA has finished.
This is monitored through last reference of kfree_skb callback. Two
buffer indices are used for this purpose.

The vhost-net device passes the userspace buffers info to lower device
skb through message control. DMA done status check and guest
notification are handled by handle_tx: in the worst case is all buffers
in the vq are in pending/done status, so we need to notify guest to
release DMA done buffers first before we get any new buffers from the
vq.

One known problem is that if the guest stops submitting
buffers, buffers might never get used until some
further action, e.g. device reset. This does not
seem to affect linux guests.

Codes duplication were found between the handling of mergeable and big
buffers, so this patch tries to unify them. This could be easily done
by adding a quota to the get_rx_bufs() which is used to limit the
number of buffers it returns (for mergeable buffer, the quota is
simply UIO_MAXIOV, for big buffers, the quota is just 1), and then the
previous handle_rx_mergeable() could be resued also for big buffers.

5e18247b02 vhost: rcu annotation fixup
When built with rcu checks enabled, vhost triggers
bogus warnings as vhost features are read without
dev->mutex sometimes, and private pointer is read
with our kind of rcu where work serves as a
read side critical section.

Fixing it properly is not trivial.
Disable the warnings by stubbing out the checks for now.

To detect that a sequence number is done, we are doing math on unsigned
integers so the result is unsigned too. Not what was intended for the <=
comparison. The result is user stuck forever in flush call.
Convert to int to fix this.
