Protect d_subdirs and d_child with d_lock, except in filesystems that aren't
using dcache_lock for these anyway (eg. using i_mutex).
Note: if we change the locking rule in future so that ->d_child protection is
provided only with ->d_parent->d_lock, it may allow us to reduce some locking.
But it would be an exception to an otherwise regular locking scheme, so we'd
have to see some good results. Probably not worthwhile.

Current code doesn't really enable the usb clocks so if they're disabled
when booting linux, the kernel/machine will hang as soon as someone is trying
to read a usb register

A race exists when initializing ueagle-atm devices where the generic atm
device may not yet be created before the driver attempts to initialize
it's PHY signal state, which checks whether the atm device has been
created or not. This often causes the sysfs 'carrier' attribute to be
'1' even though no signal has actually been found.

UPDATE_ATM_SIGNAL checks whether the ATM device has been created and if
not, will not update the PHY signal state. Because of the race that
does not always happen in time, and the PHY signal state remains
ATM_PHY_SIG_FOUND even though no signal exists.
To fix the race, just create the kthread during initialization, and only
after initialization is complete, start the thread that reboots the
device and initializes PHY state.

flush_scheduled_work() is being deprecated. Directly flush or cancel
work items instead.
* u_ether, isp1301_omap, speedtch conversions are straight-forward.
* ochi-hcd should only flush when quirk_nec() is true as otherwise the
work wouldn't have been initialized.

The device power.status field is too complicated for its purpose
(storing the information about whether or not the device is in the
"active" state from the PM core's point of view), so replace it with
a bit field and modify all of its users accordingly.

Needed to update to apply fixes for which the old branch was too
outdated.
So far the USBLED driver only supports Delcom's "USB Visual Signal
Indicator" (http://www.delcomproducts.com/products_USBLMP.asp). The
driver generates virtual files "red", "green", and "blue" under the
device's /sys/ directory, where color values can be read from and
written to.
This patch adds support for Dream Cheeky's "DL100B Webmail Notifier"
(http://www.dreamcheeky.com/webmail-notifier -- available from several
shops, such as http://www.conrad.at/ce/de/product/777048/USB-WEBMAIL).
This device isn't as pretty as Delcom's, but it's *far* cheaper, and
its 3 LEDs can be set in 32 brightness steps each. The grey envelope
contour can easily be removed, leaving a rather neutral white box (with
a few small holes), which is useful for generic signalling purposes.
Of course, the small circuit board can easily be put into a prettier
case.
The DL100B device pretends to be a HID, but the HID descriptor shows
that it's not overly useful as such (see below). The patch therefore
removes the "HID-ness" (hid-core.c, hid-ids.h), and adds the necessary
commands to usbled.c. The protocol info comes from the developer's
manual that Dream Cheeky kindly provided (815DeveloperManual.pdf).

In order to read/write to the i.MX OTG viewport register it is necessary to setup
the PORTSCx register first.
By default i.MX OTG port is configured for USB serial PHY. In order to use a ULPI PHY
the PORTSCx register needs to be configured properly.
commit 724c852 (USB: ehci/mxc: compile fix) placed the PORTSC setup after the OTG
viewport is accessed and this causes ULPI read/write to fail.
Revert the PORTSC setup order.
Tested on a MX31PDK board with a ISP1504 transceiver:

CI13xxx UDC driver doesn't call complete() callback for requests
with flag no_interrupt set. Thus gadget drivers (like g_ether) are
never notifed about successfully (or not) transmitted requests. As
a result in case of g_ether and queued request with no_interrupt=1
fields g_ether is never notifed about sent packets and TX stalls.
Solution: treat no_interrupt flag like all other UDC drivers do and
call complete() callback for all requests.

There is one nasty scenario when CI13xxx driver fails:
a) two or more rx requests are queued (g_ether does that)
b) rx request completed, interrupt fires and ci13xxx dequeues rq
c) request complete() callback gets called and in turn it calls ep_queue()
c1) in ep_queue() request gets added to the TAIL of the rx queue list
d) ep gets primed with rq from (b)
e) interrupt fires
f) request gets popped from queue head for hw dequeue
G) requets from queue head wasn't enqueued
g1) isr_tr_complete_low() doesn't
enqueue more requests and it doesn't prime EP,
rx traffic stalls
Solution:
a) enque queued requests ASAP, i.e. before calling complete() callback.
b) don't HW enqueue and prime endpoint with recently added request and
use the oldest request in the queue.
Fixed issues:
a) ep_queue() may return an error code despite request was successfully
added to the queue (if _hardware_enqueue() fails)
b) Added requests are always processed in LIFO order, even if they are
added in complete() callback
c) Finally more than two and more queued requests are processed consistently,
even if they were added in complete() callback
The fix was successfully tested on MIPS based SoC with 4KEc CPU core and
CI13612 USB core. Board successfully boots with NFS root using g_ether
on ci13xxx udc.

The ATM subsystem was incorrectly creating the 'device' link for ATM
nodes in sysfs. This led to incorrect device/parent relationships
exposed by sysfs and udev. Instead of rolling the 'device' link by hand
in the generic ATM code, pass each ATM driver's bus device down to the
sysfs code and let sysfs do this stuff correctly.

This patch fixes an issue where the driver does not handle out data in
setup transactions.
The per endpoint cached status register is cleared in the
pch_udc_svc_control_out function. When there is out data available the
function pch_udc_svc_data_out is called which tries to pick it up the
status, which now is cleared to 0. When the status is 0, the function
doesn't start reading the data from the FIFO.
There is a second bug in all this, pch_udc_svc_data_out takes the
endpoint number (0 for EP0), while pch_udc_svc_control_out passes the
endpoint index (1 for EP0). Effectively pch_udc_svc_data_out picks up
the wrong internal ep structure.
This patch makes sure to put back the cached status and pass the
endpoint number rather than index when calling pch_udc_svc_data_out.

MSM SoC has chipidea USB controller. So use ci13xxx_udc core.
This driver depends on transceiver driver for clock control,
PHY initialization, VBUS detection. Register for notify_event
callback to perform MSM specific quirks after controller is reset
and stopped.

Introduces ci13xxx_udc_driver struct for bus glue drivers to hint
ci13xxx_udc core about their special requirements. The flags include
avoiding hardware register access when controller is not in peripheral
mode, enabling pull-up upon VBUS, disabling streaming mode and dependency
on transceiver driver.
Initialize gadget_ops in udc_probe so that transceiver can notify VBUS
presence even when no gadget driver is bounded.
A notify_event callback is embedded in the same struct. This patch implements
two events called CONTROLLER_RESET_EVENT and CONTROLLER_STOPPED_EVENT to
notify the bus glue driver after resetting and stopping the controller for
performing SoC specific quirks.

dma_pool_alloc() require sleeping context when called with GFP_KERNEL
argument. Hence release the spin lock before calling dma_pool_alloc().
usb_ep_alloc_request can also be called with non-atomic GFP flags. Hence
get rid off spin lock while allocation request memory.
Use GFP_ATOMIC flag for allocating request for ep0 in interrupt handler.

Implement runtime and system pm ops to put hardware into low power
mode (LPM). As part of LPM, USB clocks are turned off, PHY is put
into suspend state and PHY comparators are turned off if VBUS/Id
notifications are not required from PHY.

This patch adds support for EHCI compliant HSUSB Host controller found
on MSM chips. The root hub has a single port and TT is built into it.

This driver implements PHY initialization, clock management, ULPI IO ops
and simple OTG state machine to kick host/peripheral based on Id/VBUS
line status. VBUS/Id lines are tied to a reference voltage on some boards.
Hence provide debugfs interface to select host/peripheral mode.

When ASPM PM Feature is enabled on UMI link, devices that use ISOC stream of
data transfer may be exposed to longer latency causing less than optimal per-
formance of the device. The longer latencies are normal and are due to link
wake time coming out of low power state which happens frequently to save
power when the link is not active.
The following code will make exception for certain features of ASPM to be by
passed and keep the logic normal state only when the ISOC device is connected
and active. This change will allow the device to run at optimal performance
yet minimize the impact on overall power savings.

Commit 06c3859 (usb: gadget/imx-udc: remove usage of deprecated symbol
USBD_INT0) was a bit precipitant because the name used instead didn't
match the usual naming scheme for irqs on arm/imx. I renamed the irq to
the right name in e083000 (ARM: imx: dynamically allocate imx_udc
device) when 06c3859 didn't hit Linus' tree, so I missed to add a
compat #define.
This patch allows compiling imx_udc.c with and without e083000.

OMAP4430 supports UTMI and ULPI types of transceiver interface.
In UTMI mode: The PHY is embedded within OMAP4430. The transceiver functionality
is split between the twl6030 PMIC chip and OMAP4430. The VBUS, ID pin
sensing and OTG SRP generation part is integrated in TWL6030 and UTMI PHY
functionality is embedded within the OMAP4430.
There is no direct interactions between the MUSB controller and TWL6030
chip to communicate the session-valid, session-end and ID-GND events.
It has to be done through a software by setting/resetting bits in
one of the control module register of OMAP4430 which in turn toggles
the appropriate signals to MUSB controller.
musb driver is register for blocking notifications from the transceiver
driver to get the event notifications for connect/disconnect and ID-GND.
Based on these events call the transceiver init/shutdown function to
configure the transceiver to toggle the VBUS valid, session end and ID_GND
signals to musb and power on/off the internal PHY.
For ID_GND event notifications, toggle the ID_GND signal and then wait for
musb to be configured as "A" device, and then call the transceiver function
to set the VBUS.
In OTG mode and musb as a host, When the Micro A connector used, VBUS is turned on
and session bit set. When the device is connected, enumeration goes through.
When the device disconnected from the other end of the connector(ID is still grounded),
link will detect the disconnect and end the session. When the device is connected back,
there are no events generated in the TWL6030-usb, and link is already down.
So the device is not detected. Removed the session bit disable code which
will recognize the connect of the device.
Limitation: In OTG host mode, if device is connected during boot, it does not get
detected. If disconnect and connect it back or connect after boot only it works.
Fix for this, I will submit seperate patch later.

Adding the twl6030-usb transceiver support for OMAP4 musb driver.
OMAP4 supports 2 types of transceiver interface.
1. UTMI: The PHY is embedded within OMAP4. The transceiver functionality
is split between the twl6030 PMIC chip and OMAP4430. The VBUS, ID pin

functionality is embedded within the OMAP4430.

2. ULPI interface is provided for off-chip transceivers.

deprecated and useless.

Fix two bugs with the port array setup.
The first bug will only show up with broken xHCI hosts with Extended
Capabilities registers that have duplicate port speed entries for the same
port. The idea with the original code was to set the port_array entry to
-1 if the duplicate port speed entry said the port was a different speed
than the original port speed entry. That would mean that later, the port
would not be exposed to the USB core. Unfortunately, I forgot a continue
statement, and the port_array entry would just be overwritten in the next
line.
The second bug would happen if there are conflicting port speed registers
(so that some entry in port_array is -1), or one of the hardware port
registers was not described in the port speed registers (so that some
entry in port_array is 0). The code that sets up the usb2_ports array
would accidentally claim those ports. That wouldn't really cause any
user-visible issues, but it is a bug.
This patch should go into the stable trees that have the port array and
USB 3.0 port disabling prevention patches.

only in this tree at the time.

preparing to a big refactor on musb code. We need
to be able to compile in all glue layers (or at
least all ARM-based ones) together and have a
working binary.
While preparing for that, we move every glue
layer to export only one symbol, which is
a struct musb_platform_ops, and make all
other functions static.
Later patches will come to allow for compiling
all glue layers together and have a working
binary.

This will be passed to musb_core by platform glue

for several HW glue layers.
Later patches will come using this structure and

Soon resource data will get automatically
populated from a set of autogenerated data
from TI's hardware database for the OMAP
platform.
Such database, might not have resources at
the expected order by the current drivers.
While we could hack in some exceptions to
that tool to generate resources in a specific
order, it seems less fragile to use the
resource name instead. That way, no matter
what order the resources are generated, the
driver still work.
Modified the OMAP, Blackfin and Davinci
architecture files to add the name of the IRQs
in the resource structures and musb driver to
use the platform_get_irq_byname() api to get
the device and dma irq numbers instead of using
the index.

This patch add USB client support Marvell PXA9xx/PXA168 chips. The USB
controller in PXA9xx/PXA168 is a High-Speed OTG controller. The available
endpoints is different between PXA9xx and PXA168.
NOTE:
It is the first version of Marvell PXA9xx/PXA168 USB controller driver.
The support for OTG mode will be added in later patch.
PXA9xx and PXA168 has integrated UTMI PHY in the chips. The initialization
for the PHY is a little different between PXA9xx and PXA168.

Several of the EHCI glue drivers either predate or were merged in the
same timeframe as API changes at the USB core level, resulting in some
missing endpoint_reset and clear_tt_buffer_complete callbacks.
This fixes up all of ehci-atmel, mxc, w90x900, and xilinx-of to tie in
the new helpers, which brings them in line with everyone else.

Function twl4030_usb_remove can cause unbalanced regulator disables in
twl4030_phy_power if the cable is not connected. Regulator enable/disable
calls are in balance only if the twl4030_phy_resume was called prior the
twl4030_usb_remove, that is, the cable was connected.
Fix this by checking the 'asleep' variable in twl4030_usb_remove since that
variable is used to check state in other functions.

Some devices (ex ZTE 2726) simply don't respond at all when data is sent
to some of their USB interfaces. The data gets stuck in the TTYs queue
and sits there until close(2), which them blocks because closing_wait
defaults to 30 seconds (even though the fd is O_NONBLOCK). This is
rarely desired. Implement the standard mechanism to adjust closing_wait
and let applications handle it how they want to.

Usually the usbmon returns the amount of data specified in
urb->transfer_buffer_length for output submissions and urb->actual_length
for input callbacks. However, for Isochronous input transfers, this is
not enough, since the returned data buffer may contain "holes".
One easy way to fix this is to use urb->transfer_buffer_length,
but this often transfers a whole lot of unused data, so we find
how much was actually used instead.

Update the ehci-omap glue layer to support the controller in the
OMAP4. Major differences from OMAP3 is that the OMAP4 has per-port
clocking, and supports ULPI output clocking mode. The old input
clocking mode is not supported.
Also, there are only 2 externally available ports as against 3
in the OMAP3. The third port is internally tied off and should
not be used.

Introduce the CONFIG_USB_EHCI_HCD_OMAP option to select

to usbhost_fs_fck, to better reflect the clocks' functionalities.
In OMAP4, the frequencies for the corresponding clocks are not
necessarily the same as with OMAP3, however the functionalities
are.

This patch (as1437) fixes a bug in the usb-serial autosuspend
handling. Since the usb-serial core now has autosuspend support, it
must set the .supports_autosuspend member in every serial driver it
registers. Otherwise the usb_autopm_get_interface() call won't work.
This fixes Bugzilla #23012.

Default llseek operation behavior was changed by the patch named
"vfs: make no_llseek the default" after the yurex driver had been merged,
so the llseek to yurex is now ignored.
This patch add llseek fop with default_llseek to yurex driver
to catch up to the change.

The CNS3XXX SOC has include USB EHCI and OHCI compatible controllers.
This patch adds the necessary glue logic to allow ehci-hcd and ohci-hcd
drivers to work on CNS3XXX
The EHCI and OHCI controllers share a common clock control and reset
bit, therefore additional check for the timming of enabling and disabling
is required. The USB bit of PLL Power Down Control is also shared by OTG,
24MHzUART clock, Crypto clock, PCIe reference clock, and Clock Scale
Generator. Therefore we only ensure it is enabled, while not disabling it.

the fact we don't stash the hcd pointer in the driver data, so we provide
our own shutdown handler.

The Inventra DMA engine used with the MUSB controller in many
SoCs cannot use DMA for control transfers on EP0, but can use
DMA for all other transfers.
The USB core maps urbs for DMA if hcd->self.uses_dma is true.
(hcd->self.uses_dma is true for MUSB as well).
Split the uses_dma flag into two - one that says if the
controller needs to use PIO for control transfers, and
another which says if the controller uses DMA (for all
other transfers).
Also, populate this flag for all MUSB by default.
(Tested on OMAP3 and OMAP4 boards, with EHCI and MUSB HCDs
simultaneously in use).

If RXCSR_AUTOCLEAR flag is not cleard before PIO reading, one packet
may be recieved by musb fifo, but no chance to notify
software, so cause packet loss, follows the detailed process:
- PIO read one packet
- musb fifo auto clear the MUSB_RXCSR_RXPKTRDY
- musb continue to recieve the next packet, and MUSB_RXCSR_RXPKTRDY
is set
- software clear the MUSB_RXCSR_RXPKTRDY, so there is no chance for
musb to notify software that the 2nd recieved packet.
The patch does fix the g_ether issue below:
- use fifo_mode 3 to enable double buffer
- 'ping -s 1024 IP_OF_BEAGLE_XM'
- one usb packet of 512 byte is lost, so ping failed,
which can be observed by wireshark
note:
Beagle xm takes musb rtl1.8 and may fallback to pio mode
for unaligned buffer.

Buffer is mapped to dma when dma channel is
allocated. If, for some reason, dma channel
programming fails, musb code will fallback
to PIO mode to transfer that request. In
that case, we need to unmap the buffer
back to CPU.
MUSB RTL1.8 and above cannot handle buffers
which are not 32bit aligned. That happens to
every request sent by g_ether gadget
driver. Since the buffer sent was unaligned,
we need to fallback to PIO.
Because of that, g_ether was failing due
to missing buffer unmapping.
With this patch and [1] g_ether works fine
with all MUSB revisions.
Verified with OMAP3630 board, which has
MUSB RTL1.8 using g_ether and g_zero.

Disabling SuperSpeed ports is a Very Bad Thing (TM). It disables
SuperSpeed terminations, which means that devices will never connect at
SuperSpeed on that port. For USB 2.0/1.1 ports, disabling the port meant
that the USB core could always get a connect status change later. That's
not true with USB 3.0 ports.
Do not let the USB core disable SuperSpeed ports. We can't rely on the
device speed in the port status registers, since that isn't valid until
there's a USB device connected to the port. Instead, we use the port
speed array that's created from the Extended Capabilities registers.

An xHCI host controller contains USB 2.0 and USB 3.0 ports, which can
occur in any order in the PORTSC registers. We cannot read the port speed
bits in the PORTSC registers at init time to determine the port speed,
since those bits are only valid when a USB device is plugged into the
port.
Instead, we read the "Supported Protocol Capability" registers in the xHC
Extended Capabilities space. Those describe the protocol, port offset in
the PORTSC registers, and port count. We use those registers to create
two arrays of pointers to the PORTSC registers, one for USB 3.0 ports, and
another for USB 2.0 ports. A third array keeps track of the port protocol
major revision, and is indexed with the internal xHCI port number.
This commit is a bit big, but it should be queued for stable because the "Don't
let the USB core disable SuperSpeed ports" patch depends on it. There is no
other way to determine which ports are SuperSpeed ports without this patch.

We have been having problems with the USB-IF Gold Tree tests when plugging
and unplugging devices from the tree. I have seen that the reset-device
and configure-endpoint commands, which are invoked from
xhci_discover_or_reset_device() and xhci_configure_endpoint(), will sometimes
time out.
After much debugging, I determined that the commands themselves do not actually
time out, but rather their completion events do not get delivered to the right
place.
This happens when the command ring has just wrapped around, and it's enqueue
pointer is left pointing to the link TRB. xhci_discover_or_reset_device() and
xhci_configure_endpoint() use the enqueue pointer directly as their command
TRB pointer, without checking whether it's pointing to the link TRB.
When the completion event arrives, if the command TRB is pointing to the link
TRB, the check against the command ring dequeue pointer in
handle_cmd_in_cmd_wait_list() fails, so the completion inside the command does
not get signaled.
The patch below fixes the timeout problem for me.
This should be queued for the 2.6.35 and 2.6.36 stable trees.

This patch (as1434) cleans up the uses of usb_mark_last_busy() in
usbcore. The function will be called when a device is resumed and
whenever a usage count is decremented. A call that was missing from
the hub driver is added: A hub is used whenever one of its ports gets
suspended (this prevents hubs from suspending immediately after their
last child).
In addition, the call to disable autosuspend support for new devices
by default is moved from usb_detect_quirks() (where it doesn't really
belong) into usb_new_device() along with all the other runtime-PM
initializations. Finally, an extra pm_runtime_get_noresume() is added
to prevent new devices from autosuspending while they are being
registered.

This patch (as1428) converts USB over to the new runtime-PM core
autosuspend framework. One slightly awkward aspect of the conversion
is that USB devices will now have two suspend-delay attributes: the
old power/autosuspend file and the new power/autosuspend_delay_ms
file. One expresses the delay time in seconds and the other in
milliseconds, but otherwise they do the same thing. The old attribute
can be deprecated and then removed eventually.

Call pm_runtime_no_callbacks to set no_callbacks flag for USB
interfaces. Since interfaces cannot be power-managed separately from
their parent devices, there's no reason for the runtime-PM core to

This patch adds the USB device driver of EG20T(Topcliff) PCH.
EG20T PCH is the platform controller hub that is going to be used in
Intel's upcoming general embedded platform. All IO peripherals in
EG20T PCH are actually devices sitting on AMBA bus.
EG20T PCH has USB device I/F. Using this I/F, it is able to access system
devices connected to USB device.

This patch (as1435) fixes an obscure and unlikely race in ehci-hcd.
When an async URB is unlinked, the corresponding QH is removed from
the async list. If the QH's endpoint is then disabled while the URB
is being given back, ehci_endpoint_disable() won't find the QH on the
async list, causing it to believe that the QH has been lost. This
will lead to a memory leak at best and quite possibly to an oops.
The solution is to trust usbcore not to lose track of endpoints. If
the QH isn't on the async list then it doesn't need to be taken off
the list, but the driver should still wait for the QH to become IDLE
before disabling it.
In theory this fixes Bugzilla #20182. In fact the race is so rare
that it's not possible to tell whether the bug is still present.
However, adding delays and making other changes to force the race
seems to show that the patch works.

On AMD SB700/SB800/Hudson-2/3 platforms, USB EHCI controller may read/write
to memory space not allocated to USB controller if there is longer than
normal latency on DMA read encountered. In this condition the exposure will
be encountered only if the driver has following format of Periodic Frame
List link pointer structure:
For any idle periodic schedule, the Frame List link pointers that have the
T-bit set to 1 intending to terminate the use of frame list link pointer
as a physical memory pointer.
Idle periodic schedule Frame List Link pointer shoule be in the following
format to avoid the issue:
Frame list link pointer should be always contains a valid pointer to a
inactive QHead with T-bit set to 0.

Move the mid-layer's ->queuecommand() invocation from being locked
with the host lock to being unlocked to facilitate speeding up the
critical path for drivers who don't need this lock taken anyway.
The patch below presents a simple SCSI host lock push-down as an
equivalent transformation. No locking or other behavior should change
with this patch. All existing bugs and locking orders are preserved.
Additionally, add one parameter to queuecommand,
struct Scsi_Host *
and remove one parameter from queuecommand,
void (*done)(struct scsi_cmnd *)
Scsi_Host* is a convenient pointer that most host drivers need anyway,
and 'done' is redundant to struct scsi_cmnd->scsi_done.
Minimal code disturbance was attempted with this change. Most drivers
needed only two one-line modifications for their host lock push-down.

Andiry's xHCI bus suspend patch introduced the possibly of a host
controller replaying old commands on the command ring, if the host
successfully restores the registers after a resume.
After a resume from suspend, the xHCI driver must restore the registers,
including the command ring pointer. I had suggested that Andiry set the
command ring pointer to the current command ring dequeue pointer, so that
the driver wouldn't have to zero the command ring.
Unfortunately, setting the command ring pointer to the current dequeue
pointer won't work because the register assumes the pointer is 64-byte
aligned, and TRBs on the command ring are 16-byte aligned. The lower
seven bits will always be masked off, leading to the written pointer being
up to 3 TRBs behind the intended pointer.
Here's a log excerpt. On init, the xHCI driver places a vendor-specific
command on the command ring:

(I can't see the results of this bug because the xHCI restore always fails
on this box, and the xHCI driver re-allocates everything.)
The fix is to zero the command ring and put the software and hardware
enqueue and dequeue pointer back to the beginning of the ring. We do this
before the system suspends, to be paranoid and prevent the BIOS from
starting the host without clearing the command ring pointer, which might
cause the host to muck with stale memory. (The pointer isn't required to
be in the suspend power well, but it could be.) The command ring pointer
is set again after the host resumes.

bdev read-only status can be queried using bdev_read_only() and may
change while the device is being opened. Enforce it by checking it
from blkdev_get() after open succeeds.
This makes bdev_read_only() check in open_bdev_exclusive() and
fsg_lun_open() unnecessary. Drop them.

USB2.0 spec 9.6.6 says: For all endpoints, bit 10..0 specify the maximum
packet size(in bytes).
So the wMaxPacketSize mask should be 0x7ff rather than 0x3ff.
This patch should be queued for the stable tree. The bug in
xhci_endpoint_init() was present as far back as 2.6.31, and the bug in
xhci_get_max_esit_payload() was present when the function was introduced
in 2.6.34.

If the xHCI host controller shares an interrupt line with another device,
the xHCI driver needs to check if the interrupt was generated by its
hardware. Unfortunately, the user will see a ton of "Spurious interrupt."
lines if the other hardware interrupts often. Lawrence found his dmesg
output cluttered with this output when the xHCI host shared an interrupt
with his i915 hardware.
Remove the warning, as sharing an interrupt is a normal thing.
This should be applied to the 2.6.36 stable tree.

OK, the USB gadget serial driver actually has a couple of problems. On
gs_open(), it always allocates and queues an additional QUEUE_SIZE (16)
worth of requests, so with a loop like this:
i=1 ; while echo $i > /dev/ttyGS0 ; do let i++ ; done
eventually we run into OOM (Out of Memory).
Technically, it is not a leak as everything gets freed up when the USB
connection is broken, but not on gs_close().
With a USB device/gadget controller driver that has limited resources
(e.g., Marvell has a this MAX_XDS_FOR_TR_CALLS of 64 for transmit and
receive), so even after 4
stty -F /dev/ttyGS0
we cannot transmit anymore. We can still receive (not necessarily
reliably) as now we have 16 * 4 = 64 descriptors/buffers ready, but the
device is otherwise not usable.

When huawei datacard with PID 0x14AC is insterted into Linux system, the
present kernel will load the "option" driver to all the interfaces. But
actually, some interfaces run as other function and do not need "option"
driver.
In this path, we modify the id_tables, when the PID is 0x14ac ,VID is
0x12d1, Only when the interface's Class is 0xff,Subclass is 0xff, Pro is
0xff, it does need "option" driver.

Commit 126512e3f2 added support for FSL's USB
controller on powerpc. In this commit the Open Firmware code was selected
and compiled unconditionally.
This breaks on ARM systems from FSL which use the same driver (.i.e. the i.MX
series), because ARM don't have OF support (yet). This patch fixes the problem
by only selecting the OF code on systems with Open Firmware support.

This commit fixes some issues with File-backed Storage Gadget
error recovery when registering LUN's devices.
First of all, when device_register() fails the device still
needs to be put. However, because lun_release() decreases
fsg->ref reference counter the counter must be incremented
beforehand.
Second of all, after any of the device_create_file()s fails,
device_unregister() is called which in turn (indirectly) calls
lun_release() which decrements fsg->ref. So, again, the
reference counter must be incremented beforehand.
Lastly, if the first or the second device_create_file()
succeeds, the files are never removed. To fix it,
device_remove_file() needs to be called. This is done by
simply marking LUN as registered prior to creating files so
that fsg_unbind() can handle removing files.

REQ_HARDBARRIER is dead now, so remove the leftovers. What's left
at this point is:
- various checks inside the block layer.
- sanity checks in bio based drivers.
- now unused bio_empty_barrier helper.
- Xen blockfront use of BLKIF_OP_WRITE_BARRIER - it's dead for a while,
but Xen really needs to sort out it's barrier situaton.
- setting of ordered tags in uas - dead code copied from old scsi
drivers.
- scsi different retry for barriers - it's dead and should have been
removed when flushes were converted to FS requests.
- blktrace handling of barriers - removed. Someone who knows blktrace
better should add support for REQ_FLUSH and REQ_FUA, though.

The Inventra DMA engine in version 1.8 and later of the MUSB
controller cannot handle DMA addresses that are not aligned
to a 4 byte boundary. It ends up ignoring the last two bits
programmed in the DMA_ADDR register. This is a deliberate
design change in the controller and is documented in the
programming guide.
Earlier versions of the controller could handle these
accesses just fine.
Fail dma_channel_program if we see an unaligned address when
using the newer controllers, so that the caller can carry out
the transfer using PIO mode.
(Current callers already have this backup path in place).

Split the USB MMR init steps out into a helper func that both the platform
init and the resume code may call.
Then while suspending, the gpio_vrsel will change from high to low which
will generate a wakeup event and resume the system immediately, so we need
to manually drive it low before we sleep.

Commit 9f445cb29918dc488b7a9a92ef018599cce33df7[USB: musb: disable
double buffering for older RTL versions] tries to disable double
buffer mode by writing endpoint hw max packet size to TXMAP/RXMAP.
First the approach can break full speed and cause overflow problems.
We should always set those registers with the actual max packet size
from endpoint descriptor.
Second, the problem describe by commit 9f445cb299
was caused by musb gadget driver; nothing to do with RTL revision as
originaly suspected.
The real fix to the problem is to always use actual max packet
size from endpoint descriptor to config TXMAP/RXMAP registers.

Some actions like musb_platform_exit are only performed on module
removal and not on shutdown, which results in PHY being left enabled
on reboot at least. This is sometimes causing strange failures after
reboot (observed on OMAP3 pandora board), when DEVCTL does not report
VBUS state correctly due to unknown reasons (possibly because of
communication issues between musb IP and PHY). Running
musb_platform_exit before reset seems to resolve that issue.
Move some exit code from musb_remove() to musb_shutdown() so that it
is performed on both module removal and shutdown/reset. Also convert
the host check so that it doesn't need #ifdef.

1, In Rx double buffer case, FIFO may have two packets, so
rxstate should be called to unload fifo if RXPKTRDY is set
even the current request has not been completed.
2, Commit 633ba7876b96ec339ef685357e2f7c60b5a8ce85
introduces autoclear to support double buffer in dma mode 0,
so remove clearing RXPKTRDY manually for dma mode 0.
3, Commit c7af6b29ffeffbeb28caf39e5b2ce29b11807c7d may break
dma mode 1 for non-doublebuffer endpoint, fix it.
With this patch, either usbtest #5 or g_file_storage(writing
file to device in usb host) or g_ether have been tested OK in
double buffer case(using fifo mode 3). Also, this patch has
been verified that single buffer case can't be broken.

Crash is triggered by commit e6484930d7 ("net: allocate tx queues in
register_netdevice"), which moved tx netqueue creation into register_netdev.
So now calling netif_stop_queue() before register_netdev causes an oops.
Move netif_stop_queue() after net device registration to fix crash.

The OCTEON II SOC has USB EHCI and OHCI controllers connected directly
to the internal I/O bus. This patch adds the necessary 'glue' logic
to allow ehci-hcd and ohci-hcd drivers to work on OCTEON II.
The OCTEON normally runs big-endian, and the ehci/ohci internal
registers have host endianness, so we need to select
USB_EHCI_BIG_ENDIAN_MMIO.
The ehci and ohci blocks share a common clocking and PHY
infrastructure. Initialization of the host controller and PHY clocks
is common between the two and is factored out into the
octeon2-common.c file.
Setting of USB_ARCH_HAS_OHCI and USB_ARCH_HAS_EHCI is done in
arch/mips/Kconfig in a following patch.

Crash is triggered by commit e6484930d7 ("net: allocate tx queues in
register_netdevice"), which moved tx netqueue creation into register_netdev.
So now calling netif_stop_queue() before register_netdev causes an oops.
Move netif_stop_queue() after net device registration to fix crash.

Instead of always assigning an increasing inode number in new_inode
move the call to assign it into those callers that actually need it.
For now callers that need it is estimated conservatively, that is
the call is added to all filesystems that do not assign an i_ino
by themselves. For a few more filesystems we can avoid assigning
any inode number given that they aren't user visible, and for others
it could be done lazily when an inode number is actually needed,
but that's left for later patches.

Fix regression introduced by commit
f26788da3b (USB: serial: refactor generic
close) which broke driver close().
This driver uses non-standard semantics for the read urb which makes the
generic close function fail to kill it (the read urb is actually an
interrupt urb and therefore bulk_in size is zero).

AM35x has musb interface and uses CPPI4.1 DMA engine.
Current patch supports only PIO mode. DMA support can be
added later once basic CPPI4.1 DMA patch is accepted.
Also added USB_MUSB_AM35X which is required to differentiate musb ips
between OMAP3x and AM35x. This config would be used to for below
purposes,
- Select am35x.c instead of omap2430.c for compilation
at drivers/usb/musb directory. Please note there are
significant differneces in these two files as musb ip
in quite different on AM35x.
Please note that in multi omap configuration only omap2430.c
file will get compiled and we would require to select only
AM35x based board config to compile am35x.c
- Select workaround codes applicable for AM35x musb issues.
one such workaround is for bytewise read issue on AM35x.

A few devices (such as the RCA VR5220 voice recorder) are so
non-compliant with the USB spec that they have invalid maxpacket sizes
for endpoint 0. Nevertheless, as long as we can safely use them, we
may as well do so.
This patch (as1432) softens our acceptance criterion by allowing
high-speed devices to have ep0-maxpacket sizes other than 64. A
warning is printed in the system log when this happens, and the
existing error message is clarified.

This patch implements the PCI suspend/resume.
Please refer to xHCI spec for doing the suspend/resume operation.
For S3, CSS/SRS in USBCMD is used to save/restore the internal state.
However, an error maybe occurs while restoring the internal state.
In this case, it means that HC internal state is wrong and HC will be
re-initialized.

This patch implements xHCI bus suspend/resume function hook.
In the patch it goes through all the ports and suspend/resume
the ports if needed.
If any port is in remote wakeup, abort bus suspend as what ehci/ohci do.

This commit implements port remote wakeup.
When a port is in U3 state and resume signaling is detected from a device,
the port transitions to the Resume state, and the xHC generates a Port Status
Change Event.
For USB3 port, software write a '0' to the PLS field to complete the resume
signaling. For USB2 port, the resume should be signaling for at least 20ms,
irq handler set a timer for port remote wakeup, and then finishes process in
hub_control GetPortStatus.
Some codes are borrowed from EHCI code.

Add software trigger USB device suspend resume function hook.
Do port suspend & resume in terms of xHCI spec.
Port Suspend:
Stop all endpoints via Stop Endpoint Command with Suspend (SP) flag set.
Place individual ports into suspend mode by writing '3' for Port Link State
(PLS) field into PORTSC register. This can only be done when the port is in
Enabled state. When writing, the Port Link State Write Strobe (LWS) bit shall
be set to '1'.
Allocate an xhci_command and stash it in xhci_virt_device to wait completion for
the last Stop Endpoint Command. Use the Suspend bit in TRB to indicate the Stop
Endpoint Command is for port suspend. Based on Sarah's suggestion.
Port Resume:
Write '0' in PLS field, device will transition to running state.
Ring an endpoints' doorbell to restart it.

When the system suspends and a host controller's power is lost, the USB
core attempts to revive any USB devices that had the persist_enabled flag
set. For non-SuperSpeed devices, it will disable the port, and then set
the udev->reset_resume flag. This will cause the USB core to reset the
device, verify the device descriptors to make sure it's the same device,
and re-install any non-default configurations or alternate interface
settings.
However, we can't disable SuperSpeed root hub ports because that turns off
SuperSpeed terminations, which will inhibit any devices connecting at USB
3.0 speeds. (Plus external hubs don't allow SuperSpeed ports to be
disabled.)

power loss was discovered, and xhci_discover_or_reset_device() has not
been called), and all URBs to the device will fail.
If the device driver responds by resetting the device, everything will
continue smoothly. However, if lsusb is used before the device driver
resets the device (or there is no driver), then all lsusb descriptor
fetches will fail.
The quick fix is to pretend the port is disabled in hub_activate(), by
clearing the local variable. But I'm not sure what other parts of the hub
driver need to be changed because they have assumptions about when ports
will be disabled.

xHCI driver uses hardware assigned device address. This may cause device
address conflict in certain cases.
Use kernel assigned address for devices under xHCI. Store the xHC assigned

Rename xhci_reset_device() to xhci_discover_or_reset_device().
If xhci_discover_or_reset_device() is called to reset a device which does
not exist or does not match the udev, it calls xhci_alloc_dev() to
re-allocate the device.
This would prevent the reset device failure, possibly due to the xHC restore
error during S3/S4 resume.

Add a pointer to udev in struct xhci_virt_device. When allocate a new
virt_device, make the pointer point to the corresponding udev.
Modify xhci_check_args(), check if virt_dev->udev matches the target udev,
to make sure command is issued to the right device.

The bulk-read callback had two bugs:
a) The bulk-in packet's leading two zeros were returned (and the two last
bytes truncated)
b) The wrong URB was transmitted for the second (and later) read requests,
causing further reads to return the entire packet (including leading
zeros)

If the iowarrior devices in this case statement support more than 8 bytes
per report, it is possible to write past the end of a kernel heap allocation.
This will probably never be possible, but change the allocation to be more
defensive anyway.

This patch for FTDI USB serial driver ads new VID/PIDs used on various
devices manufactured by Papouch (http://www.papouch.com). These devices
have their own VID/PID, although they're using standard FTDI chip. In
ftdi_sio.c, I also made small cleanup to have declarations for all
Papouch devices together.

This commit changes storage_common.h, file_storage.c and

scsi/scsi.h file instead of redefining the commands in
storage_common.c.

At otg device mode, the otg host resume should do no-op during
system resume, otherwise, the otg device will be treated as a
host for enumeration.
So, the otg host driver returns -ESHUTDOWN if it detects the
current usb mode is device mode. The host driver has to return
-ESHUTDOWN, otherwise, the usb_hc_died will be called.

Appotech ax3003 (the larger brother of the ax203) based devices are even
more buggy then the ax203. They will go of into lala land when ever they
see a READ_DISC_INFO scsi command. So add a new US_FL which tells the
scsi sr driver to not issue any READ_DISC_INFO scsi commands.

This is based on an initial patch by Rahul Ruikar.
The goku_remove() function can be called before device_register() so it
can call device_unregister() improperly. Also if the call to
device_register() fails we need to call put_device().
As I was changing the error handling in goku_probe(), I noticed that
the label was "done" but actually if the function succeeds we return
earlier. I renamed the error path to "err" instead of "done."

The hardware can only do DMA to 4 byte aligned addresses.
When this requirement is not met use PIO or a bounce buffer.
PIO is used when the buffer is small enough to directly
use the hardware data memory (2*maxpacket).
A bounce buffer is used for larger transfers.

Release the hardware resources and reset the internal HCD state
associated with an isochronous endpoint when the last URB queued
for it completes.
Previously this was only done in then endpoint_disable() method
causing usbtest 15 and 16 to hang when run twice in succession
without a disconnect.

This patch (as1430) fixes a bug in usbcore. When a device
configuration change occurs or a device is removed, the endpoints for
the old config should be completely disabled. However it turns out
they aren't; this is because usb_unbind_interface() calls
usb_enable_interface() or usb_set_interface() to put interfaces back
in altsetting 0, which re-enables the interfaces' endpoints.
As a result, when a device goes through a config change or is
unconfigured, the ep_in[] and ep_out[] arrays may be left holding old
pointers to usb_host_endpoint structures. If the device is
deauthorized these structures get freed, and the stale pointers cause
errors when the the device is eventually unplugged.
The solution is to disable the endpoints after unbinding the
interfaces instead of before. This isn't as large a change as it
sounds, since usb_unbind_interface() disables all the interface's
endpoints anyway before calling the driver's disconnect routine,
unless the driver claims to support "soft" unbind.
This fixes Bugzilla #19192. Thanks to "Tom" Lei Ming for diagnosing
the underlying cause of the problem.

Since commit 461972d8a4 (musb_core: don't call
musb_platform_exit() twice), unloading the driver module results in a WARNING
"kobject: '(null)' (c73de788): is not initialized, yet kobject_put() is being
called." (or even kernel oops) on e.g. DaVincis, though only in the OTG mode.
There exists dubious and unbalanced put_device() call in musb_free() which
takes place only in the OTG mode. As this commit caused musb_platform_exit()
to be called (and so unregister the NOP transceiver) before this put_device()
call, this function references already freed memory.
On the other hand, all the glue layers miss the otg_put_transceiver() call,
complementary to the otg_get_transceiver() call that they do. So, I think
the solution is to get rid of the strange put_device() call, and instead
call otg_put_transceiver() in the glue layers...

Extends FSL EHCI platform driver glue layer to support
MPC5121 USB controllers. MPC5121 Rev 2.0 silicon EHCI
registers are in big endian format. The appropriate flags
are set using the information in the platform data structure.
MPC83xx system interface registers are not available on
MPC512x, so the access to these registers is isolated in
MPC512x case. Furthermore the USB controller clocks
must be enabled before 512x register accesses which is
done by providing platform specific init callback.
The MPC512x internal USB PHY doesn't provide supply voltage.
For boards using different power switches allow specifying
DRVVBUS and PWR_FAULT signal polarity of the MPC5121 internal
PHY using "fsl,invert-drvvbus" and "fsl,invert-pwr-fault"
properties in the device tree USB nodes. Adds documentation
for this new device tree bindings.

Replace FSL USB platform code by simple platform driver for
creation of FSL USB platform devices.
The driver creates platform devices based on the information
from USB nodes in the flat device tree. This is the replacement
for old arch fsl_soc usb code removed by this patch. The driver
uses usual of-style binding, available EHCI-HCD and UDC
drivers can be bound to the created devices. The new of-style
driver additionaly instantiates USB OTG platform device, as the
appropriate USB OTG driver will be added soon.

g_file_storage functionality by enabling AUTOCLEAR on
all cases without caring for all gadget drivers.
This patch will only enable AUTOCLEAR if our endpoint's
FIFO was configured with double buffering support. Note
this is not a complete fix, double buffered case still
doesn't work always, but that hasn't been working for
quite some time. Other than reverting the entire commit
and breaking testusb with double buffered case again,
I decided it was better to fix the single buffered case
and spend more time fixing double buffered case properly.

The USB stack maps the buffer for DMA if the controller supports DMA.
MUSB controller can perform DMA as well as PIO transfers.
The buffer needs to be unmapped before CPU can perform
PIO data transfers.
Export unmap_urb_for_dma() so that drivers can perform
the DMA unmapping in a sane way.

Enabling CONFIG_USB_MUSB_DEBUG option causes -DDEBUG to be added to gcc's
command line, however the DBG() macro doesn't depend on DEBUG, so that the
debugging messages get printed regardless of the option, and I don't think
that this was intended. Get rid of otherwise unused xprintk() macro and make
DBG() macro directly call pr_debug() which only results in the actual code
generated if DEBUG is defined.
This change makes musb_hdrc.o ~30% less in size with CONFIG_USB_MUSB_DEBUG
disabled (in host mode).

RTS and DTR should not be modified based on CRTSCTS when calling
set_termios.
Modem control lines are raised at port open by the tty layer and should stay
raised regardless of whether hardware flow control is enabled or not.
This is in conformance with the way serial ports work today and many
applications depend on this behaviour to be able to talk to hardware
implementing hardware flow control (without the applications actually using
it).
Hardware which expects different behaviour on these lines can always
use TIOCMSET/TIOCMBI[SC] after port open to change them.

This patch, adds to the option driver the Onda Communication
(http://www.ondacommunication.com) vendor id, and the MT825UP modem
device id.
Note that many variants of this same device are being release here in
Italy (at least one or two per telephony operator).
These devices are perfectly equivalent except for some predefined
settings (which can be changed of course).
It should be noted that most ONDA devices are allready supported (they
used other vendor's ids in the past). The patch seems working fine here,
and the rest of the driver seems uninfluenced.

The un-registration of OHCI driver was not done in the ohci_hcd_mod_exit
function. This was affecting rmmod command not to work for OMAP3
platforms. The platform driver un-registration for OMAP3 platforms is
perfomed while removing the OHCI module from kernel.

There are lots of ZTE USB id's currently not covered by usb/serial. Adds them,
to allow those devices to work properly on Linux.
While here, put the USB ID's for 0x2002/0x2003 at the sorted order.
This patch is based on zte.c file found on MF645.
PS.: The ZTE driver is commenting the USB ID for 0x0053. It also adds, commented,
an USB ID for 0x0026.
Not sure why, but I think that 0053 is used by their devices in storage mode only.
So, I opted to keep the comment on this patch.

In today linux-next I got a compile error on usb/host/isp1362-hcd:

The problem is when the CONFIG_USB_OTG option is enabled.
ohci variable is never declared and there isn't any CONFIG_USB_OTG dependent code
besides the portion defined in isp1362_hub_control.
So I think that maybe USB OTG support is not needed/supported.
This patch removes the CONFIG_USB_OTG dependent block so the driver can compile cleanly.

The composite gadget will OOPS if the host sends a control request
targetted to an interface of an un-configured composite device. This patch
prevents this.
The OOPS was observed during WHQL USB CV tests. With this patch, the device
STALLs as per requirement.
Failing test case: From host do the following. I used libusb-1.0

This patch (as1417) fixes a problem affecting some (or all) nVidia
chipsets. When the computer is shut down, the OHCI controllers
continue to power the USB buses and evidently they drive a Reset
signal out all their ports. This prevents attached devices from going
to low power. Mouse LEDs stay on, for example, which is disconcerting
for users and a drain on laptop batteries.
The fix involves leaving each OHCI controller in the OPERATIONAL state
during system shutdown rather than putting it in the RESET state.
Although this nominally means the controller is running, in fact it's
not doing very much since all the schedules are all disabled. However
there is ongoing DMA to the Host Controller Communications Area, so
the patch also disables the bus-master capability of all PCI USB
controllers after the shutdown routine runs.
The fix is applied only to nVidia-based PCI OHCI controllers, so it
shouldn't cause problems on systems using other hardware. As an added
safety measure, in case the kernel encounters one of these running
controllers during boot, the patch changes quirk_usb_handoff_ohci()
(which runs early on during PCI discovery) to reset the controller
before anything bad can happen.

The protocol code is set 00 in IAD and it's set to 01 in ACM control
interface descriptor in f_acm.c file. Due to this, windows is unable to
install the modem(ACM) driver based on class-subclass-protocol matching.
This patch corrects the protocol code in ACM IAD to the same as in
acm_control_interface_desc protocol code.

tdi_reset is already taking care of setting host mode for tdi devices.
Don't duplicate code in platform driver.
Make ehci_halt a nop if the controller is not in host mode (otherwise it
will fail), and let's ehci_reset do the tdi_reset.
We need to move hcd->has_tt flags before ehci_halt, in order ehci_halt
knows we are a tdi device.
Before the setup routine was doing :
- put controller in host mode
- ehci_halt
- ehci_init
- hcd->has_tt = 1;
- ehci_reset
Now we do :
- hcd->has_tt = 1;
- ehci_halt
- ehci_init
- ehci_reset
PS : now we handle correctly the device -> host transition.

This patch (as1413) changes g_file_storage to avoid generating a bogus
automatic serial-number string descriptor. If the user doesn't provide
a valid serial number via a module parameter then a warning is logged
and the gadget won't have any serial string descriptor at all.

To accomplish this the function to register a gadget driver takes the bind
function as a second argument. To make things clearer rename the function
to resemble platform_driver_probe.
This fixes many section mismatches like

This patch removes string registration from the Mass Storage
Gadget. With recent changes to the composite framework, all
that we need is handled by the composite layer. This means
composite registers a string ID for manufacturer and product.
This also adds the "needs_serial" so that composite layer will
issue a warning if user space fails to provide the iSerialNumber
module parameter.

The iManufatcurer, iProduct and iSerialNumber composite module
parameters were only used when the gadget driver registers
strings for manufacturer, product and serial number. If the
gadget never bothered to set corresponding fields in USB device
descriptors those module parameters are ignored.
This commit makes the parameters work even if the strings ID
have not been assigned. It also changes the way IDs are
overridden -- what IDs are overridden is now saved in
usb_composite_dev structure -- which makes it unnecessary to
modify the string tables the way previous code did.
The commit also adds a iProduct and iManufatcurer fields to the
usb_composite_device structure. If they are set, appropriate
strings are reserved and added to device descriptor. This makes
it unnecessary for gadget drivers to maintain code for setting
those. If iProduct is not set it defaults to
usb_composite_device::name; if iManufatcurer is not set
a default "<system> <release> with <gadget-name>" is used.
The last thing is that if needs_serial field of
usb_composite_device is set and user failed to provided
iSerialNumber parameter a warning is issued.

This patch removes some of the string registration from the
FunctionFS Gadget as composite layer can handle the
iManufacturer and iProduct for us.
It also removes some of the module parameters which were
redundant as well as changes the name of others to better much
the module parameter of the composite layer.
Other then that, it also fixes formatting of multiline comments
to match the coding style.

Add new driver to access the SAM-BA boot application of Atmel AT91SAM
devices.
The SAM-BA firmware cannot handle merged write requests so we cannot use
the generic write implementation (which uses the port write fifo).
Tested with the SAM-BA 2.10 tools and an Atmel at91sam9260-ek.

This adds support for the USB transceiver driver in the Langwell chipset used
on the Intel MID platforms. It folds up the original patch set which includes
basic support for the device, PHY low power mode (Please notice that there is
a limitation, after we drive VBus down, 2ms delay is required from SCU FW to
sync up OTGSC register with USBCFG register), software timers (the hardware
timers do not work in low power mode), HNP, SRP.

Dan Rosenberg noted that various drivers return the struct with uncleared
fields. Instead of spending forever trying to stomp all the drivers that
get it wrong (and every new driver) do the job in one place.
This first patch adds the needed operations and hooks them up, including
the needed USB midlayer and serial core plumbing.

All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.
The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.
New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time. Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.
The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.

Only OMAP2+ platforms have the System Control Module (SCM) IP block.
In the past, we've kept the SCM header file in plat-omap. This has
led to abuse - device drivers including it; includes being added that
create implicit dependencies on OMAP2+ builds; etc.
In response, move the SCM headers into mach-omap2/.
As part of this, remove the direct SCM access from the OMAP UDC
driver. It was clearly broken. The UDC code needs an indepth review for
use on OMAP2+ chips.

The BKL is only used in fill_super, which is protected by the superblocks

This patch (as1429) updates the Kconfig help text for
CONFIG_USB_SUSPEND. The power/level file is now deprecated; we should
tell people to use power/control instead.

Commit 46034dca51 (USB: musb_gadget_ep0: stop
abusing musb_gadget_set_halt()) forgot to restart a queued request after
clearing the endpoint halt feature. This results in a couple of USB resets
while enumerating the file-backed storage gadget due to CSW packet not being
sent for the MODE SENSE(10) command.

This patch fixes the issue which was observed while transfering
a large file ( > 20MB) over USB (OMAP MUSB controller acts as USB host)
to an attached USB thumb drive.
It was found that CDB field of CBW packet was set to 0x0. This was
due to missing a barrier before DMA engine starts transfer.
This buffer is allocated using dma_alloc_coherent which gives
non-cacheble but bufferable memory and hence needed a write
memory barrier to flush the write buffer.

This patch fixes one bugs of OUT transfer in double buffer case:
-the current code only enable autoclear for dma mode 1, and not
for dma mode 0
Without this patch, test #5 of usbtest can't be passed if we
configure musb as g_zero and use fifo mode 3 to enable double
buffer mode.
With this patch and the following patch(fix dma length),
on my beagle B5, test#5(queued bulk out) may go beyond
18Mbyte/s(seems dma mode 0 is quicker in double buffer case)
if musb is configured as g_zero and fifo mode 3 is taken, follows
the test command:

Also I have tested this patch can't make g_ether broken.

This patch fixes one infinite hang of bulk IN transfer in double buffer
case, the hang can be observed easily by test #6 of usbtest if musb is
configured as g_zero and fifo mode 3 is taken to enable double fifo.
In fact, the patch only removes the check for non-empty fifo before
loading data from new request into fifo since the check is not correct:
-in double buffer case, fifo may accommodate more than one packet,
even though it has contained one packet already and is non-empty
-since last DMA is completed before calling musb_g_tx, it is sure
that fifo may accommodate at least one packet
Without applying the patch, new requst enqueued from .complte may not
have a chance to be loaded into fifo, then will never be completed and
cause infinite hangs.
With the patch, on my beagle B5, test#6(queued bulk in) can be passed and
test result may go beyond 33Mbyte/s if musb is configured as g_zero and
fifo mode 3 is taken, follows the test command:

Recent changes in the usbhid layer exposed a bug in usbcore. If
CONFIG_USB_DYNAMIC_MINORS is enabled then an interface may be assigned
a minor number of 0. However interfaces that aren't registered as USB
class devices also have their minor number set to 0, during
initialization. As a result usb_find_interface() may return the
wrong interface, leading to a crash.
This patch (as1418) fixes the problem by initializing every
interface's minor number to -1. It also cleans up the
usb_register_dev() function, which besides being somewhat awkwardly
written, does not unwind completely on all its error paths.

The TIOCGICOUNT device ioctl in both mos7720.c and mos7840.c allows
unprivileged users to read uninitialized stack memory, because the
"reserved" member of the serial_icounter_struct struct declared on the
stack is not altered or zeroed before being copied back to the user.
This patch takes care of it.

is put forward to power down phy if no usb cable is connected,
but does introduce the two issues below:
1), phy is not into work state if usb cable is connected
with PC during poweron, so musb device mode is not usable
in such case, follows the reasons:
-twl4030_phy_resume is not called, so
regulators are not enabled
i2c access are not enabled
usb mode not configurated
2), The kernel warings[1] of regulators 'unbalanced disables'
is caused if poweron without usb cable connected
with PC or b-device.
This patch fixes the two issues above:
-power down phy only if no usb cable is connected with PC
and b-device
-do phy initialization(via __twl4030_phy_resume) if usb cable
is connected with PC(vbus event) or another b-device(ID event) in
twl4030_usb_probe.
This patch also doesn't put VUSB3V1 LDO into active mode in
twl4030_usb_ldo_init until VBUS/ID change detected, so we can
save more power consumption than before.
This patch is verified OK on Beagle board either connected with
usb cable or not when poweron.

seq_files use the private_data field of a file struct for storing a seq_file structure,
data should be stored in seq_file's own private field (e.g. file->private_data->private)
Otherwise seq_release() will free the private data when the file is closed.

The _remove() routine is flagged __init_or_module, despite only being
used in a __devexit context. As the rest of the driver is already
balanced out with __devinit, switch to __devexit and __devexit_p()
wrapping.

Ignore ADSL routers, which can have the same vendor and product IDs
as ADSL modems but should be handled by the cx82310_eth driver.
This intentionally ignores device IDs that aren't currently handled
by cx82310_eth. There may be other device IDs that perhaps shouldn't
be claimed by cxacru.

Certain USB devices, such as the Nokia X6 mobile phone, don't expose any
endpoint descriptors on some of their interfaces. If the ACM driver is forced
to probe all interfaces on a device the a NULL pointer dereference will occur
when the ACM driver attempts to use the endpoint of the alternative settings.
One way to get the ACM driver to probe all the interfaces is by using the
/sys/bus/usb/drivers/cdc_acm/new_id interface.
This patch checks that the endpoint pointer for the current alternate settings
is non-NULL before using it.

The command endpoint is either a bulk or interrupt endpoint, but using
the wrong type of transfer causes an error if CONFIG_USB_DEBUG is
enabled after commit f661c6f8c6, which
checks for this mismatch.
Detect which type of endpoint it is and use a bulk/int URB as
appropriate. There are other function calls specifying a bulk pipe,
but usb_clear_halt doesn't use the pipe type (only the endpoint) and
usb_bulk_msg auto-detects interrupt transfers.

S60 phones from Nokia and Samsung expose two ACM channels. The first is a modem
with a standard AT-command interface, which is picked up correctly by CDC-ACM.
The second ACM port is marked as having a vendor-specific protocol. This means
that the ACM driver will not claim the second channel by default.
This adds support for the second ACM channel for the following devices:

When using the remove sysfs file, the device configuration is set to -1
(unconfigured). This eventually unbind drivers with the bandwidth_mutex
held. Some drivers may call functions that hold said mutex, like
usb_reset_device. This is the case for rtl8187, for example. This will
lead to the same process holding the mutex twice, which deadlocks.
Besides, according to Alan Stern:
"The deadlock problem probably could be handled somehow, but there's a
separate issue: Until the usb_disable_device call finishes unbinding
the drivers, the drivers are free to continue using their allocated
bandwidth. We musn't change the bandwidth allocations until after the
unbinding is done. So this patch is indeed necessary."
Unbinding the driver before holding the bandwidth_mutex solves the
problem. If any operation after that fails, drivers are not bound again.
But that would be a problem anyway that the user may solve resetting the
device configuration to one that works, just like he would need to do in
most other failure cases.

Unfortunately some of the hardware PID belonging to auto-install CDROM
(AICD) of Novatel modems found their way into the option module. This
causes the AICD to be treated as a modem in stead of a disk. Since the
modem ports do not appear until after the AICD is ejected, this
essentially disables the modem. After a couple of minutes the AICD
should auto-eject, but it is just too long a wait. The frequency of the
failure seems to depend on both the hardware and the linux distribution.
Here is a patch that fixes this up, and also adds a couple of new PID,
offering some explanations and removing some incomplete and unnecessary
comments.

This patch removes the following section mismatch warning,
by moving the function rndis_init() from .init.text to .text.

The iounmap(ehci->ohci_hcctrl_reg); should be the first thing we do
because the ioremap() was the last thing we did. Also if we hit any of
the goto statements in the original code then it would have led to a
NULL dereference of "ehci". This bug was introduced in: 796bcae736
"USB: powerpc: Workaround for the PPC440EPX USBH_23 errata [take 3]"
I modified the few lines in front a little so that my code didn't
obscure the return success code path.

Add ftdi product ID for Lenz LI-USB, a model train interface. This
was NOT tested against 2.6.35, but a similar patch was tested with the
CentOS 2.6.18-194.11.1.el5 kernel. It wasn't clear to me what
ordering is being used in ftdi_sio.c, so I inserted the ID after another
model train entry(SPROG_II).

This patch fixes a isoc transfer bug reported by Sander Eikelenboom.
When ep->skip is set, endpoint ring dequeue pointer should be updated
when processed every missed td. Although ring dequeue pointer will also
be updated when ep->skip is clear, leave it intact during missed tds
processing may cause two issues:
1). If the very next valid transfer following missed tds is a short
transfer, its actual_length will be miscalculated;
2). If there are too many missed tds during transfer, new inserted tds
may found the transfer ring full and urb enqueue fails.

The code to increment the TRB pointer has a slight ambiguity that could
lead to a bug on different compilers. The ANSI C specification does not
specify the precedence of the assignment operator over the postfix
operator. gcc 4.4 produced the correct code (increment the pointer and
assign the value), but a MIPS compiler that one of John's clients used
assigned the old (unincremented) value.
Remove the unnecessary assignment to make all compilers produce the
correct assembly.

The reset state of twl4030-usb is not sleeping, it starts
up awaken and we need to disable it if we have booted
with a disconnected cable to avoid over consumption on
the default state.
To avoid problems later, we read the current state of the
transceiver from the PHY_PWR_CTRL register. The bootloader
can, anyways, put the device to sleep before us.
Tested on a custom OMAP board.
