3c88c692c2 x86/stackframe/32: Provide consistent pt_regs
Currently pt_regs on x86_32 has an oddity in that kernel regs
(!user_mode(regs)) are short two entries (esp/ss). This means that any
code trying to use them (typically: regs->sp) needs to jump through
some unfortunate hoops.

Change the entry code to fix this up and create a full pt_regs frame.

This then simplifies various trampolines in ftrace and kprobes, the
stack unwinder, ptrace, kdump and kgdb.

Much thanks to Josh for help with the cleanups!

1fc654cf6e x86/paravirt: Standardize 'insn_buff' variable names
We currently have 6 (!) separate naming variants to name temporary instruction
buffers that are used for code patching:

 - insnbuf
 - insnbuff
 - insn_buff
 - insn_buffer
 - ibuf
 - ibuffer

These are used as local variables, percpu fields and function parameters.

Standardize all the names to a single variant: 'insn_buff'.

43a1b0cb4c kprobes/x86: Fix instruction patching corruption when copying more than one RIP-relative instruction
After copy_optimized_instructions() copies several instructions
to the working buffer it tries to fix up the real RIP address, but it
adjusts the RIP-relative instruction with an incorrect RIP address
for the 2nd and subsequent instructions due to a bug in the logic.

This will break the kernel pretty badly (with likely outcomes such as
a kernel freeze, a crash, or worse) because probed instructions can refer
to the wrong data.

For example putting kprobes on cpumask_next() typically hits this bug.

cpumask_next() is normally like below if CONFIG_CPUMASK_OFFSTACK=y
(in this case nr_cpumask_bits is an alias of nr_cpu_ids):

If we put a kprobe on it and it gets jump-optimized, it gets
patched by the kprobes code like this:

This shows that the first two MOV instructions were copied to a
trampoline buffer at 0xffffffffa000207a.

Here is the disassembled result of the trampoline, skipping
the optprobe template instructions:

This dump shows that the second MOV accesses *(nr_cpu_ids+3) instead of
the original *nr_cpu_ids. This leads to a kernel freeze because
cpumask_next() always returns 0 and for_each_cpu() never ends.

Fix this by adding 'len' correctly to the real RIP address while
copying.

2e62024c26 kprobes/x86: Use preempt_enable() in optimized_callback()
The following commit:

  a19b2e3d78 ("kprobes/x86: Remove IRQ disabling from ftrace-based/optimized kprobes”)

removed local_irq_save/restore() from optimized_callback(), the handler
might be interrupted by the rescheduling interrupt and might be
rescheduled - so we must not use the preempt_enable_no_resched() macro.

Use preempt_enable() instead, to not lose preemption events.

2bbda764d7 kprobes/x86: Do not disable preempt on int3 path
Since int3 and debug exception(for singlestep) are run with
IRQ disabled and while running single stepping we drop IF
from regs->flags, that path must not be preemptible. So we
can remove the preempt disable/enable calls from that path.

c86a32c09f kprobes/x86: Disable optimizing on the function jumps to indirect thunk
Since indirect jump instructions will be replaced by jump
to __x86_indirect_thunk_*, those jmp instruction must be
treated as an indirect jump. Since optprobe prohibits to
optimize probes in the function which uses an indirect jump,
it also needs to find out the function which jump to
__x86_indirect_thunk_* and disable optimization.

Add a check that the jump target address is between the
__indirect_thunk_start/end when optimizing kprobe.

a19b2e3d78 kprobes/x86: Remove IRQ disabling from ftrace-based/optimized kprobes
Kkprobes don't need to disable IRQs if they are called from the
ftrace/jump trampoline code, because Documentation/kprobes.txt says:

  -----
  Probe handlers are run with preemption disabled.  Depending on the
  architecture and optimization state, handlers may also run with
  interrupts disabled (e.g., kretprobe handlers and optimized kprobe
  handlers run without interrupt disabled on x86/x86-64).
  -----

So let's remove IRQ disabling from those handlers.

63fef14fc9 kprobes/x86: Make insn buffer always ROX and use text_poke()
Make insn buffer always ROX and use text_poke() to write
the copied instructions instead of set_memory_*().
This makes instruction buffer stronger against other
kernel subsystems because there is no window time
to modify the buffer.

d9f5f32a7d kprobes/x86: Do not jump-optimize kprobes on irq entry code
Since the kernel segment registers are not prepared at the
entry of irq-entry code, if a kprobe on such code is
jump-optimized, accessing per-CPU variables may cause a
kernel panic.

However, if the kprobe is not optimized, it triggers an int3
exception and sets segment registers correctly.

With this patch we check the probe-address and if it is in the
irq-entry code, it prohibits optimizing such kprobes.

This means we can continue probing such interrupt handlers by kprobes
but it is not optimized anymore.

c207aee480 objtool, x86: Add several functions and files to the objtool whitelist
In preparation for an objtool rewrite which will have broader checks,
whitelist functions and files which cause problems because they do
unusual things with the stack.

These whitelists serve as a TODO list for which functions and files
don't yet have undwarf unwinder coverage.  Eventually most of the
whitelists can be removed in favor of manual CFI hint annotations or
objtool improvements.

a8d11cd071 kprobes/x86: Consolidate insn decoder users for copying code
Consolidate x86 instruction decoder users on the path of
copying original code for kprobes.

Kprobes decodes the same instruction a maximum of 3 times when
preparing the instruction buffer:

 - The first time for getting the length of the instruction,
 - the 2nd for adjusting displacement,
 - and the 3rd for checking whether the instruction is boostable or not.

For each time, the actual decoding target address is slightly
different (1st is original address or recovered instruction buffer,
2nd and 3rd are pointing to the copied buffer), but all have
the same instruction.

Thus, this patch also changes the target address to the copied
buffer at first and reuses the decoded "insn" for displacement
adjusting and checking boostability.

d0381c81c2 kprobes/x86: Set kprobes pages read-only
Set the pages which is used for kprobes' singlestep buffer
and optprobe's trampoline instruction buffer to readonly.
This can prevent unexpected (or unintended) instruction
modification.

This also passes rodata_test as below.

Without this patch, rodata_test shows a warning:

  WARNING: CPU: 0 PID: 1 at arch/x86/mm/dump_pagetables.c:235 note_page+0x7a9/0xa20
  x86/mm: Found insecure W+X mapping at address ffffffffa0000000/0xffffffffa0000000

With this fix, no W+X pages are found:

  x86/mm: Checked W+X mappings: passed, no W+X pages found.
  rodata_test: all tests were successful

75013fb16f kprobes/x86: Fix kernel panic when certain exception-handling addresses are probed
Fix to the exception table entry check by using probed address
instead of the address of copied instruction.

This bug may cause unexpected kernel panic if user probe an address
where an exception can happen which should be fixup by __ex_table
(e.g. copy_from_user.)

Unless user puts a kprobe on such address, this doesn't
cause any problem.

This bug has been introduced years ago, by commit:

  464846888d ("x86/kprobes: Fix a bug which can modify kernel code permanently").

2a6730c8b6 kprobes/x86: Check for invalid ftrace location in __recover_probed_insn()
__recover_probed_insn() should always be called from an address
where an instructions starts. The check for ftrace_location()
might help to discover a potential inconsistency.

This patch adds WARN_ON() when the inconsistency is detected.
Also it adds handling of the situation when the original code
can not get recovered.

6ba48ff46f x86: Remove arbitrary instruction size limit in instruction decoder
The current x86 instruction decoder steps along through the
instruction stream but always ensures that it never steps farther
than the largest possible instruction size (MAX_INSN_SIZE).

The MPX code is now going to be doing some decoding of userspace
instructions.  We copy those from userspace in to the kernel and
they're obviously completely untrusted coming from userspace.  In
addition to the constraint that instructions can only be so long,
we also have to be aware of how long the buffer is that came in
from userspace.  This _looks_ to be similar to what the perf and
kprobes is doing, but it's unclear to me whether they are
affected.

The whole reason we need this is that it is perfectly valid to be
executing an instruction within MAX_INSN_SIZE bytes of an
unreadable page. We should be able to gracefully handle short
reads in those cases.

This adds support to the decoder to record how long the buffer
being decoded is and to refuse to "validate" the instruction if
we would have gone over the end of the buffer to decode it.

The kprobes code probably needs to be looked at here a bit more
carefully.  This patch still respects the MAX_INSN_SIZE limit
there but the kprobes code does look like it might be able to
be a bit more strict than it currently is.

This patch frees the 'optinsn' slot when we get a range check error,
to prevent memory leaks.

9326638cbe kprobes, x86: Use NOKPROBE_SYMBOL() instead of __kprobes annotation
Use NOKPROBE_SYMBOL macro for protecting functions
from kprobes instead of __kprobes annotation under
arch/x86.

This applies nokprobe_inline annotation for some cases,
because NOKPROBE_SYMBOL() will inhibit inlining by
referring the symbol address.

This just folds a bunch of previous NOKPROBE_SYMBOL()
cleanup patches for x86 to one patch.

a7b0133ea9 kprobes/x86: Use text_poke_bp() instead of text_poke_smp*()
Use text_poke_bp() for optimizing kprobes instead of
text_poke_smp*(). Since the number of kprobes is usually not so
large (<100) and text_poke_bp() is much lighter than
text_poke_smp() [which uses stop_machine()], this just stops
using batch processing.
