2af502ca64 mmc: sdhci: request irq after sdhci_init() is called
Generally request_irq() should be called after hardware has been
initialized into a sane state.  However, sdhci driver currently calls
request_irq() before sdhci_init().  At least, the following kernel panic
seen on i.MX6 is caused by that.  The sdhci controller on i.MX6 may have
noisy glitch on DAT1 line, which will trigger SDIO interrupt handling
once request_irq() is called.  But at this point, the SDIO interrupt
handler host->sdio_irq_thread has not been registered yet.  Thus, we
see the NULL pointer access with wake_up_process(host->sdio_irq_thread)
in mmc_signal_sdio_irq().

Fix the panic by simply reverse the calling sequence between
request_irq() and sdhci_init().

This patch fixes the HC ctrl_2 programming where, in case of
SDR104 and HS200, we have to write 100b in the the UHS Mode
bits. We wrote 101b that is reserved from Arasan Specs.

156e14b126 mmc: sdhci: fix caps2 for HS200
Although the HC supports HS200 (eMMC) the caps2 are always zero; this
means there's no way to use the super speed mode (when init the card).

If the HC support SDR104, for SD3.0, so it also supports HS200 for eMMC
and this patch just sets the MMC_CAP2_HS200 in the host caps2 field.

9668d765ea mmc: sdhci: improve card removal check in sdhci_card_event()
The following error randomly appears on an imx6q board where gpio is
used to implement card-detection when mounting EXT4 rootfs during boot.

mmc1: Card removed during transfer!
mmc1: Resetting controller.

It turns out that the error message comes from the card removal check
in function sdhci_card_event().  While we have a well implemented
function sdhci_do_get_cd() handling all the possible cases of
CD, the current code only checks controller internal CD case.  That
causes problem for other CD cases like gpio on above imx6q board.

Improve the check by using sdhci_do_get_cd() to cover all possible CD
cases, so that above error on the imx6q board gets fixed.

f0710a557c mmc: sdhci: add ability to stay runtime-resumed if the card is powered up
If card power is dependent on SD bus power then the host controller
must not be runtime suspended while the card is powered up.  Add
the ability to stay runtime-resumed in that case and enable it with a new
quirk SDHCI_QUIRK2_CARD_ON_NEEDS_BUS_ON.

1. mmc_rescan will call get_cd to know whether the card is present
before mmc_rescan_try_freq to avoid useless trials during
card removal or start host is called when card is not present.

2. get_cd needs to be checked to resolve slow card removal issue.

type of the second parameter of sdhci_do_start_signal_voltage_switch(),
from "struct mmc_ios *ios" to "int signal_voltage" which causes the
following build warning:

a4f8f257ed mmc: sdhci: check voltage range only on regulators aware of voltage value
Some regulators don't report any voltage values, so checking supported
voltage range results in disabling all SDHCI_CAN_VDD_* flags and
registration failure. This patch finally provides a correct fix for the
registration of SDHCI driver with all possible voltage regulators:
dummy, fixed and regulated without using regulator_count_voltages()
hacks.

52983382c7 mmc: sdhci: enhance preset value function
4d55c5a1 ("mmc: sdhci: enable preset value after uhs initialization")
added preset value support and enabled it by default during sd card init.

Below are the enhancements introduced by this patch:

1. In current code, preset value is enabled after setting clock finished,
which means the clock is manually set by driver firstly and then suddenly
switched to preset value at this point. So the first setting is useless
and unnecessary. What's more, the first clock setting may differ from the
preset one.  The better way is enable preset value just after switch to
UHS mode so the preset value can take effect immediately. So move preset
value enable from mmc_sd_init_card to sdhci_set_ios which will be called
during set timing.

2. In current code, preset value is disabled at the beginning of
mmc_attach_sd.  It's too late since low freq (400khz) should be set in
mmc_power_up.  So move preset value disable to sdhci_set_ios which will
be called during power up.

3. host->clock and ios->drv_type should also be updated according to the
preset value if it's enabled. Current code missed this.

4. This patch also introduce a quirk to disable preset value in case
preset value doesn't work.

This patch has been verified on sdhci-pxav3 platform with both preset
enabled and disabled.

The 8bit in the function name is misleading. When set, it will be
used to set the bus width, regardless of whether 8bit or another
bus width is requested, so change the function name to

b0a8dece55 mmc: sdhci: disable interrupt before free_irq
Current code missed disabling interrupts before free irq which is shared.

Notice below comments for function free_irq (kernel/irq/manage.c):
On a shared IRQ the caller must ensure the interrupt is disabled
on the card it drives before calling this function.

Original code has below issue during suspend/resume when multiple SD
hosts share the same IRQ:
1. Assume there are two hosts (host1 for emmc while host2 for sd) share
the same mmc irq.
2. When system suspend, host2 will be suspended before host1.
So the sequence is below:
	step1: irq handler for host2 removed ->
	step2: irq handler for host1 removed and irq disabled ->
	... system suspended ...
	... system resumed ...
	step3: irq enabled and the irq handler for host1 restored ->
	step4: irq handler for host2 restored
3. So there is the buggy time slot that the irq is enabled but the irq
handler for host2 is removed. Then host2 interrupt can be triggered
but can't be handled at that moment.

505a8680b7 mmc: sdhci: query card presence from cd-gpio before asking SDHCI
Call mmc_gpio_get_cd() to query card presence from cd-gpio before
asking SDHCI.  The rationale behind this change is that flag
SDHCI_QUIRK_BROKEN_CARD_DETECTION is designed for SDHCI controller to
tell that SDHCI_PRESENT_STATE is broken, and it should be used for this
case only.  So when cd-gpio is being used, the controller should set
the flag to tell that SDHCI_PRESENT_STATE is not available.

However, the existing code will skip checking cd-gpio as long as flag
SDHCI_QUIRK_BROKEN_CARD_DETECTION is set.  Change the querying order
between cd-gpio and SDHCI to support the rationale above.

a4071fbbb9 mmc: eSDHC: Recover from ADMA errors
A-003500: False ADMA Error might be reported when ADMA is used for
multiple block read command with Stop at Block Gap. If PROCTL[SABGREQ]
is set when the particular block's data is received by the System side
logic before entire block (with CRC) data is received by the SD side
logic, and also if ADMA descriptor line is fetched at the same time,
then DMA engine might report false ADMA error. eSDHC might not be able
to Continue (PROCTL[CREQ]=1) after Stop at Block Gap.
This issue will impact the eSDHC IP VVN2.3.

6a66180a25 mmc: sdhci: add quirk for lack of 1.8v support
The OLPC XO-1.75 laptop includes a SDHCI controller which is 1.8v
capable, and it truthfully reports so in its capabilities. This
alternate voltage is used for driving new "UHS-I" SD cards at their
full speed.

However, what the controller doesn't know is that the motherboard
physically doesn't have a 1.8v supply available.

Add a quirk so that systems such as this one can override disable
1.8v support, adding support for UHS-I cards (by running them at
3.3v).

This avoids a problem where the system would first try to run the
card at 1.8v, fail, and then not be able to fully reset the card
to retry at the normal 3.3v voltage.

This is more appropriate than using the MISSING_CAPS quirk, which
is intended for cases where the SDHCI controller is actually lying

8363c37414 mmc: sdhci: Balance vmmc regulator_enable(), and always enable vqmmc
The vmmc regulator enable in sdhci_add_host is NOT necessary since
it can be enabled during mmc_power_up by function mmc_regulator_set_ocr.
And this extra enable will make regulator_enable/regulator_disable
unbalanced. Consequently, vmmc can't be disabled during mmc_power_off.

Also, if the vqmmc regulator exists, it should be enabled regardless it
support 1.8v or not.

657d59823c mmc: sdhci: fix IS_ERR() checking of regulator_get()
There are two problems here:

The check for vmmc was printing an unnecessary pr_info() when
host->vmmc is NULL.

The intent of the check for vqmmc was to only remove UHS if we have a
regulator that doesn't support the required voltage, but since IS_ERR()
doesn't catch NULL, we were actually removing UHS modes if vqmmc isn't
present at all -- since it isn't present for most users, this breaks
UHS for them.  This patch fixes that UHS regression in 3.7-rc1.

e6c085863f mmc: core: Fixup broken suspend and eMMC4.5 power off notify
This patch fixes up the broken suspend sequence for eMMC with sleep
support. Additionally it reworks the eMMC4.5 Power Off Notification
feature so it fits together with the existing sleep feature.

The CMD0 based re-initialization of the eMMC at resume is re-introduced
to maintain compatiblity for devices using sleep.

A host shall use MMC_CAP2_POWEROFF_NOTIFY to enable the Power Off
Notification feature. We might be able to remove this cap later on,
if we think that Power Off Notification always is preferred over
sleep, even if the host is not able to cut the eMMC VCCQ power.

bec9d4e593 mmc: sdhci: Test cd-gpio instead of SDHCI presence when probing
Previously to this patch, an SDHCI platform that uses a GPIO for
card detection instead of the internal SDHCI_CARD_PRESENT bit on
the presence register would fail to detect a new card.

Some drivers worked around this in various ways: esdhc-imx defines
an IO accessor to fake the presence bit being true, s3c turns on
polling (which stops the SDHCI driver from checking the bit) after
a card's inserted.  But none of this should be necessary; the real
fix is to check whether we're using a GPIO and avoid relying on
the presence bit if so, as this patch implements.

Host has different current capabilities at different voltages, we need
to record these settings seperately. The defined voltages are 1.8/3.0/3.3.
For other voltages, we do not touch current limit setting.

Before we set the current limit for the sd card, find out the host's
operating voltage first and then find out the current capabilities of
the host at that voltage to set the current limit.

eb6d5ae1c9 mmc: sdhci: poll for card even when card is logically unremovable
The Marvell CaFe is now marked as having bad card detection to fix
a problem during system resume.

Now on the OLPC XO-1 we are facing the issue that the card is marked
as logically unremovable (via MMC_UNSAFE_RESUME), which means that
mmc_card_is_removable considers the card non-removable. The existing
code logic decides not to poll for card presence in this case, and
card detection is also disabled because of the quirk being set.
This means that no SD cards are detected when inserted after boot.

Refine the logic to enable card presence polling in the case when
a card is logically unremovable, only avoiding the poll in the case
when the card is physically non-removable (denoted with
MMC_CAP_NONREMOVABLE).

973905feab mmc: sdhci: Introduce new flag SDHCI_USING_RETUNING_TIMER
Add a new flag of SDHCI_USING_RETUNING_TIMER to represent if the host
is using a retuning timer for the card inserted.

This flag is set when the host does tuning the first time for the card
and the host's retuning mode is 1. This flag is used afterwards whenever
needs to decide if the host is currently using a retuning timer.

This flag is cleared when the card is removed in sdhci_reinit.

The set/clear of the flag and the start/stop of the retuning timer is
associated with the card's init/remove time, so there is no need to
touch it when the host is to be removed as at that time the card should
have already been removed.

Some of the host settings are affected by different cards inserted, e.g.
when an UHS-I card is inserted, the SDHCI_NEEDS_RETUING flag might be
set when the tuning timer expired and host's max_blk_count will be
reduced to make sure the data transfer for a command does not exceed 4MiB
to meet the retuning mode 1's requirement.

When the card is removed, we should restore the original setting of the
host since we can't be sure the next card being inserted will still be
an UHS-I card that needs tuning. The original setting include its
max_blk_count and no set of the flag of SDHCI_NEEDS_RETUNING.

For SD hosts using retuning mode 1, when retuning timer expired, it will
need to do retuning in sdhci_request before processing the actual
request. But the retuning command is fixed: cmd19 for SD card and cmd21
for eMMC card, so we can't use the original request's command to do the
tuning.

And since the tuning command depends on the card type attached to the
host, we will need to know the card type to use the correct tuning
command.

Currently only the capability_0 register can be set if
SDHCI_QUIRK_MISSING_CAPS is defined.  This is a problem when
the capability_1 register also needs changing.  Use the quirk
SDHCI_QUIRK_MISSING_CAPS to allow both registers to be set.

Redefining caps[1] is useful when the board design does not
support 1.8v vccq so UHS modes are not available.  The code that
calls sdhci_add_host can then detect this condition and adjust
the caps so the UHS mode will not be attempted on UHS cards.

Commit c79396c191 ("mmc: sdhci: prevent card detection activity
for non-removable cards") disables card detection where the cards
are marked as non-removable.

This makes sense, but the implementation detail of calling
mmc_card_is_removable() causes some problems, because
mmc_card_is_removable() is overloaded with CONFIG_MMC_UNSAFE_RESUME
semantics.

In the OLPC XO case, we need CONFIG_MMC_UNSAFE_RESUME because our root
filesystem is stored on SD, but we also have external SD card slots
where we want automatic card detection.

Refine the check to only apply to hosts marked as MMC_CAP_NONREMOVABLE,
which is defined to mean that the card is *really* nonremovable. This
could be revisited in future if we find a way to improve
CONFIG_MMC_UNSAFE_RESUME semantics.

4188bba0e9 mmc: Prevent 1.8V switch for SD hosts that don't support UHS modes.
The driver should not try to switch to 1.8V when the SD 3.0 host
controller does not have any UHS capabilities bits set (SDR50, DDR50
or SDR104). See page 72 of "SD Specifications Part A2 SD Host
Controller Simplified Specification Version 3.00" under
"1.8V Signaling Enable". Instead of setting SDR12 and SDR25 in the host
capabilities data structure for all V3.0 host controllers, only set them
if SDR104, SDR50 or DDR50 is set in the host capabilities register. This
will prevent the switch to 1.8V later.

When using MSI it is possible that a new MSI is sent while an earlier
MSI is currently handled. In this case SDHCI_INT_STATUS only contains
SDHCI_INT_RESPONSE and the ISR would not be called again. But at the end
of the ISR SDHCI_INT_DATA_END is now also pending which would be ignored.

Fix this by rereading the interrupt flags in the ISR until no interrupt
we care is pending.

mmc core will notify the device by setting the POWER_OFF_NOTIFICATION
byte in the extended csd register with a value of 1 (POWER_ON).

For suspend mode short timeout is used, whereas for the normal poweroff
long timeout is used.

Ths patch allows runtime PM for sdhci-pci, runtime suspending after
inactivity of 50ms and ensuring runtime resume before SDHC registers
are accessed.  During runtime suspend, interrupts are masked.
The host controller state is restored at runtime resume.

For Medfield, the host controller's card detect mechanism is
supplanted by an always-on GPIO which provides for card detect wake-up.

Not sure about other SDHCI based controller, but on Freescale eSDHC,
the SDHCI_INT_CARD_INSERT bits will be immediately set again when it
gets cleared, if a card is inserted. The driver need to mask the irq
to prevent interrupt storm which will freeze the system.  And the
SDHCI_INT_CARD_REMOVE gets the same situation.

The patch fixes the problem based on the initial idea from
Eric Benard.

cf2b5eea1e mmc: sdhci: add support for retuning mode 1
Host Controller v3.00 can support retuning modes 1,2 or 3 depending on
the bits 46-47 of the Capabilities register. Also, the timer count for
retuning is indicated by bits 40-43 of the same register. We initialize
timer_list for retuning the first time we execute tuning procedure. This
condition is indicated by SDHCI_NEEDS_RETUNING not being set. Since
retuning mode 1 sets a limit of 4MB on the maximum data length, we set
max_blk_count appropriately. Once the tuning timer expires, we set
SDHCI_NEEDS_RETUNING flag, and if the flag is set, we execute tuning
procedure before sending the next command. We need to restore mmc_request
structure after executing retuning procedure since host->mrq is used
inside the procedure to send CMD19. We also disable and re-enable this
flag during suspend and resume respectively, as per the spec v3.00.

c3ed387762 mmc: sdhci: add support for programmable clock mode
Host Controller v3.00 supports programmable clock mode as an optional
feature. The support for this mode is indicated by non-zero value in
bits 48-55 of the Capabilities register. If supported, the actual
value of Clock Multiplier is one more than the value provided in the
bit fields. We only set Clock Generator Select (bit 5) and SDCLK
Frequency Select (bits 8-15) of the Clock Control register in case
Preset Value Enable is not set, otherwise these fields are automatically
set by the Host Controller based on the UHS mode selected. Also, since
the maximum and minimum clock frequency in this mode can be
(Base Clock * Clock Mul) and (Base Clock * Clock Mul)/1024 respectively,
f_max and f_min have been recalculated to reflect this change.

4d55c5a13a mmc: sdhci: enable preset value after uhs initialization
According to the Host Controller spec v3.00, setting Preset Value Enable
in the Host Control2 register lets SDCLK Frequency Select, Clock Generator
Select and Driver Strength Select to be set automatically by the Host
Controller based on the UHS-I mode set. This patch enables this feature.
Since Preset Value Enable makes sense only for UHS-I cards, we enable this
feature after successfull UHS-I initialization. We also reset Preset Value
Enable next time before initialization.

b513ea250e mmc: sd: add support for tuning during uhs initialization
Host Controller needs tuning during initialization to operate SDR50
and SDR104 UHS-I cards. Whether SDR50 mode actually needs tuning is
indicated by bit 45 of the Host Controller Capabilities register.
A new command CMD19 has been defined in the Physical Layer spec
v3.01 to request the card to send tuning pattern.

We enable Buffer Read Ready interrupt at the very begining of tuning
procedure, because that is the only interrupt generated by the Host
Controller during tuning. We program the block size to 64 in the
Block Size register. We make sure that DMA Enable and Multi Block
Select in the Transfer Mode register are set to 0 before actually
sending CMD19. The tuning block is sent by the card to the Host
Controller using DAT lines, so we set Data Present Select (bit 5) in
the Command register. The Host Controller is responsible for doing
the verfication of tuning block sent by the card at the hardware
level. After sending CMD19, we wait for Buffer Read Ready interrupt.
In case we don't receive an interrupt after the specified timeout
value, we fall back on fixed sampling clock by setting Execute
Tuning (bit 6) and Sampling Clock Select (bit 7) of Host Control2
register to 0. Before exiting the tuning procedure, we disable Buffer
Read Ready interrupt and re-enable other interrupts.

5371c927bc mmc: sd: set current limit for uhs cards
We decide on the current limit to be set for the card based on the
Capability of Host Controller to provide current at 1.8V signalling,
and the maximum current limit of the card as indicated by CMD6
mode 0. We then set the current limit for the card using CMD6 mode 1.
As per the Physical Layer Spec v3.01, the current limit switch is
only applicable for SDR50, SDR104, and DDR50 bus speed modes. For
other UHS-I modes, we set the default current limit of 200mA.

This patch adds support for setting UHS-I bus speed mode during UHS-I
initialization procedure. Since both the host and card can support
more than one bus speed, we select the highest speed based on both of
their capabilities. First we set the bus speed mode for the card using
CMD6 mode 1, and then we program the host controller to support the
required speed mode. We also set High Speed Enable in case one of the
UHS-I modes is selected. We take care to reset SD clock before setting
UHS mode in the Host Control2 register, and then re-enable it as per
the Host Controller spec v3.00. We then set the clock frequency for
the UHS-I mode selected.

d6d50a15a2 mmc: sd: add support for driver type selection
This patch adds support for setting driver strength during UHS-I
initialization procedure. Since UHS-I cards set S18A (bit 24) in
response to ACMD41, we use this as a base for UHS-I initialization.
We modify the parameter list of mmc_sd_get_cid() so that we can
save the ROCR from ACMD41 to check whether bit 24 is set.

We decide whether the Host Controller supports A, C, or D driver
type depending on the Capabilities register. Driver type B is
suported by default. We then set the appropriate driver type for
the card using CMD6 mode 1. As per Host Controller spec v3.00, we
set driver type for the host only if Preset Value Enable in the
Host Control2 register is not set. SDHCI_HOST_CONTROL has been
renamed to SDHCI_HOST_CONTROL1 to conform to the spec.

f2119df6b7 mmc: sd: add support for signal voltage switch procedure
Host Controller v3.00 adds another Capabilities register. Apart
from other things, this new register indicates whether the Host
Controller supports SDR50, SDR104, and DDR50 UHS-I modes. The spec
doesn't mention about explicit support for SDR12 and SDR25 UHS-I
modes, so the Host Controller v3.00 should support them by default.
Also if the controller supports SDR104 mode, it will also support
SDR50 mode as well. So depending on the host support, we set the
corresponding MMC_CAP_* flags. One more new register. Host Control2
is added in v3.00, which is used during Signal Voltage Switch
procedure described below.

Since as per v3.00 spec, UHS-I supported hosts should set S18R
to 1, we set S18R (bit 24) of OCR before sending ACMD41. We also
need to set XPC (bit 28) of OCR in case the host can supply >150mA.
This support is indicated by the Maximum Current Capabilities
register of the Host Controller.

If the response of ACMD41 has both CCS and S18A set, we start the
signal voltage switch procedure, which if successfull, will switch
the card from 3.3V signalling to 1.8V signalling. Signal voltage
switch procedure adds support for a new command CMD11 in the
Physical Layer Spec v3.01. As part of this procedure, we need to
set 1.8V Signalling Enable (bit 3) of Host Control2 register, which
if remains set after 5ms, means the switch to 1.8V signalling is
successfull. Otherwise, we clear bit 24 of OCR and retry the
initialization sequence. When we remove the card, and insert the
same or another card, we need to make sure that we start with 3.3V
signalling voltage. So we call mmc_set_signal_voltage() with
MMC_SIGNAL_VOLTAGE_330 set so that we are back to 3.3V signalling
voltage before we actually start initializing the card.

82b0e23a29 mmc: sdhci: Fix read-only detection with JMicron 388 chip
On HP laptops with JMicron 388 chip, the write-locked SD card isn't
detected correctly as read-only in many cases.  This is because the
PRESENT_STATE register becomes unsable just after plugging, and it
returns the WRITE_PROTECT bit wrongly at the first read.

This patch fixes the read-only detection by adding a new sdhci quirk

393c1a34dd mmc: sdhci: Add pre and post reset processing for chip specific reset
Marvell pxa controllers have private registers that may need to be
modified before and after a reset is done.

For example, the SD reset operation, RESET_ALL, will reset the private
registers to their default state.  This will cause the clock adjustment
registers that may have been programmed to have incorrect values.

RESET_DATA sometimes needs to be delayed before the reset is done
(depending on SoC) to enable any transactions being handled by the
SDIO card to be completed.  Needed in pre SD 3.0 silicon to handle
clock gating.

Implement hooks to allow this to happen.

4b01681c77 mmc: sdhci: Fix SDHCI_QUIRK_TIMEOUT_USES_SDCLK
Commit 373e6a (mmc: sdhci: R1B command handling + MMC_CAP_ERASE) moved the
handling of SDHCI_QUIRK_TIMEOUT_USES_SDCLK from sdhci_calc_timeout() to
sdhci_add_host(). This causes division by zero errors on at least the S3C
SDHCI controller as the quirk implementation needs host->clock set to work
but host->clock has not been set when sdhci_add_host() is called.

Fix this by backing out that portion of the change, the clock may vary at
runtime anyway. It does occur to me that we may want to move the quirk to
where we set the clock but this seems more invasive and I'm concerned
about undesirable side effects.

f6a03cbf43 mmc: sdhci: work around broken dma boundary behavior
Some SD host controllers (noticed on an integrated JMicron SD reader on an
HP Pavilion dv5-1250eo laptop) don't update the dma address register before
signaling a dma interrupt due to a dma boundary. Update the register
manually to the next boundary (by default 512KiB), at which the transfer
stopped.

As long as each transfer is at most 512KiB in size (guaranteed by a BUG_ON
in sdhci_prepare_data()) and the boundary is kept at the default value,
this fix is needed at most once per transfer. Smaller boundaries are taken
care of by counting the transferred bytes.

8f230f454f mmc: Add support for JMicron 388 SD/MMC controller
JMicron 388 SD/MMC combo controller supports the 1.8V low-voltage for
SD, but MMC doesn't work with the low-voltage, resulting in an error
at probing.

This patch adds the support for multiple voltage mask per device type,
so that SD works with 1.8V while MMC forces 3.3V.  Here new ocr_avail_*
fields for each device are introduced, so that the actual OCR mask is
switched dynamically.

Also, the restriction of low-voltage in core/sd.c is removed when the
bit is allowed explicitly via ocr_avail_sd mask.

This patch was rewritten from scratch based on Aries' original code.

We now:
 * check for a v3 controller before setting 8-bit bus width
 * offer a callback for platform code to switch to 8-bit mode, which
   allows non-v3 controllers to support it
 * rely on mmc->caps |= MMC_CAP_8_BIT_DATA; in platform code to specify
   that the board designers have indeed brought out all the pins for
   8-bit to the slot.

We were previously relying only on whether the *controller* supported
8-bit, which doesn't tell us anything about the pin configuration in
the board design.

This fixes the MMC card regression reported by Maxim Levitsky here:
   http://thread.gmane.org/gmane.linux.kernel.mmc/4336
by no longer assuming that 8-bit works by default.

ce5f036bbb sdhci-s3c: add support for the non standard minimal clock value
S3C SDHCI host controller can change the source for generating mmc clock.
By default host bus clock is used, what causes some problems on machines
with 133MHz bus, because the SDHCI divider cannot be as high get proper
clock value for identification mode.  This is not a problem for the
controller, because it can generate lower frequencies from other clock
sources.  This patch changes sdhci driver to use get_min_clock() call if
it has been provided.

This fixes the flood of the following warnings on Samsung S5PV210 SoCs:
mmc0: Minimum clock frequency too high for identification mode

1a13f8fa76 mmc: remove the "state" argument to mmc_suspend_host()
Even though many mmc host drivers pass a pm_message_t argument to
mmc_suspend_host() that argument isn't used the by MMC core.  As host
drivers are converted to dev_pm_ops they'll have to construct
pm_message_t's (as they won't be passed by the PM subsystem any more) just
to appease the mmc suspend interface.

We might as well just delete the unused paramter.

Some hosts (e.g.  as found in CNS3xxx SOCs) report wrong value in
CLOCK_BASE capability field, and currently there is no way to force the
SDHCI core to use the platform-provided base clock value.

This patch implements CAP_CLOCK_BASE_BROKEN quirk.  When enabled, the
SDHCI core will always use base clock frequency provided by the platform.

5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

9e506f35b1 sdhci: improve sdhci sdhci_set_adma_desc() code
sdhci_set_adma_desc() is using byte-writes to write data in a specified
order into memory.  Change to using __le16 for the two byte and __le32 for
the four byte cases and use the cpu_to_{le16,le32} to do the conversion
before writing.

This will reduce the size of the code and the number of writes as we no
longer need to chop the data up before writing.

27f6cb160b sdhci: increase timeout for internal clock stabilization.
On an OLPC XO-1.5 development board with Via VX855 chipset, the sdhci
controller can take up to 12ms to stabilize its clock, but the current
timeout at which we give up on the controller is 10ms.

The patch increases the timeout delay rather than using a device-specific
quirk -- since we exit the loop when the clock comes up, increasing the
timeout value will only make us mdelay() longer in the errant case of a
device with a clock that is not stabilizing, which it seems worth waiting
a little longer for in general.

81b3980246 sdhci-of: fix high-speed cards recognition
eSDHC fails to recognize some SDHS cards, throwing timeout errors:

  mmc0: error -110 whilst initialising SD card

That's because we calculate timeout value in a wrong way: on eSDHC hosts
the timeout clock is derivied from the SD clock, which is set dynamically.

As David Vrabel suggested, deriving timeout clock from SD clock is a
common scheme, so let's implement DATA_TIMEOUT_USES_SDCLK quirk and use it
for eSDHC hosts.

Also, from now on we don't need esdhc_get_timeout_clock() callback, so
remove it.

a9e58f2573 sdhci: get rid of "frequency too high" flood when using eSDHC
Since commit 8dfd0374be ("MMC core: limit
minimum initialization frequency to 400kHz") MMC core checks for minimum
frequency, and that causes following messages flood when using eSDHC
controllers:

  ...
  mmc0: Minimum clock frequency too high for identification mode
  mmc0: Minimum clock frequency too high for identification mode
  ...

The warnings are legitimate, since if we'd use 133 MHz clocks for standard
SDHCI controllers, we'd not able to scale frequency down to 400 kHz.

But eSDHC controllers have a non-standard SD clock management, so we can
divide clock by 256 * 16, not just 256.

This patch introduces get_min_clock() callback for sdhci core and
implements it for sdhci-of driver, and thus fixes the issue.

5fe23c7f51 sdhci: Add support for hosts that are only capable of 1-bit transfers
Some hosts (hardware configurations, or particular SD/MMC slots) may
not support 4-bit bus. For example, on MPC8569E-MDS boards we can
switch between serial (1-bit only) and nibble (4-bit) modes, thought
we have to disable more peripherals to work in 4-bit mode.

Along with some small core changes, this patch modifies sdhci-of
driver, so that now it looks for "sdhci,1-bit-only" property in the
device-tree, and if specified we enable a proper quirk.

0633f65424 sdhci: Add quirk for forcing maximum block size to 2048 bytes
FSL eSDHC controllers can support maximum block size up to 4096 bytes,
the MBL (Maximum Block Length) field in the capabilities register
extended by one bit, and is set to 0x3.

But the SDHCI core doesn't support blocks of 4096 bytes, and thus
forces blksz to the lowest value -- 512 bytes. With this patch we can
pin up the blksz to the maximum supported block size, i.e. 2048 bytes.

Small udelay is needed to make eSDHC work in PIO mode. Without
the delay reading causes endless interrupt storm, and writing
corrupts data. The first guess would be that we must wait for
some bit in some register, but I didn't find any reliable bits
that change before and after the delay.

FSL eSDHC hosts have incompatible register map to manage the SDCLK.
This patch adds set_clock callback so that drivers could overwrite
set_clock behaviour.

Similar patch[1] was posted by Ben Dooks, though in Ben's version the
callback is named change_clock, plus the patch has some unrelated bits
that makes the patch difficult to reuse.

4240ff0a02 sdhci: Add get_{max,timeout}_clock callbacks
Some controllers do not provide clock information in their capabilities
(in the Samsung case, it is because there are multiple clock sources
available to the controller). Add hooks to allow the system to supply
clock information.

p.s.
In the original Ben's patch there was a bug that makes sdhci_add_host()
return -ENODEV even if callbacks were specified. This is fixed now.

This patch adds SDHCI_QUIRK_BROKEN_CARD_DETECTION quirk. When specified,
sdhci driver will set MMC_CAP_NEEDS_POLL MMC host capability, and won't
enable card insert/remove interrupts.

This is needed for hosts with unreliable card detection, such as FSL
eSDHC. The original eSDHC driver was tring to "debounce" card-detection
IRQs by reading present state and disabling particular interrupts. But
with this debouncing scheme I noticed that sometimes we miss card
insertion/removal events.

7260cf5e12 sdhci: Split card-detection IRQs management from sdhci_init()
Card detection interrupts should be handled separately as they should
not be enabled before mmc_add_host() returns and should be disabled
before calling mmc_remove_host(). The same is for suspend and resume
routines.

sdhci_init() no longer enables card-detection irqs. Instead, two new
functions implemented: sdhci_enable_card_detection() and
sdhci_disable_card_detection().

New sdhci_reinit() call implemented to behave the same way as the old
sdhci_init().

Also, this patch implements and uses few new helpers to manage IRQs in
a more conveinient way, that is:

- sdhci_clear_set_irqs()
- sdhci_unmask_irqs()
- sdhci_mask_irqs()
- SDHCI_INT_ALL_MASK constant

sdhci_enable_sdio_irq() converted to these new helpers, plus the
helpers will be used by the subsequent patches.

Currently the SDHCI driver works with PCI accessors (write{l,b,w} and
read{l,b,w}).

With this patch drivers may change memory accessors, so that we can
support hosts with "weird" IO memory access requirments.

For example, in "FSL eSDHC" SDHCI hardware all registers are 32 bit
width, with big-endian addressing. That is, readb(0x2f) should turn
into readb(0x2c), and readw(0x2c) should be translated to
le16_to_cpu(readw(0x2e)).

7244b85bd1 sdhci: 'scratch' may be used uninitialized
The variable 'scratch' is always initialized before it's used. The
conditional which is responsible for initialization of 'scratch' will
always evaluate 'true' when the first loop iteration occurs, and thus,
it's properly initialized. GCC doesn't see this, of course, so using
the uninitialized_var() macro seems to work for silencing this case.

The card detection delay was added early when the behaviour of the
card interrupt was still very much unknown (i.e. before there was a
public specification). As it is now known that it is a debounced signal,

8d8bb39b9e dma-mapping: add the device argument to dma_mapping_error()
Add per-device dma_mapping_ops support for CONFIG_X86_64 as POWER
architecture does:

This enables us to cleanly fix the Calgary IOMMU issue that some devices
are not behind the IOMMU (http://lkml.org/lkml/2008/5/8/423).

I think that per-device dma_mapping_ops support would be also helpful for
KVM people to support PCI passthrough but Andi thinks that this makes it
difficult to support the PCI passthrough (see the above thread).  So I
CC'ed this to KVM camp.  Comments are appreciated.

A pointer to dma_mapping_ops to struct dev_archdata is added.  If the
pointer is non NULL, DMA operations in asm/dma-mapping.h use it.  If it's
NULL, the system-wide dma_ops pointer is used as before.

If it's useful for KVM people, I plan to implement a mechanism to register
a hook called when a new pci (or dma capable) device is created (it works
with hot plugging).  It enables IOMMUs to set up an appropriate
dma_mapping_ops per device.

The major obstacle is that dma_mapping_error doesn't take a pointer to the
device unlike other DMA operations.  So x86 can't have dma_mapping_ops per
device.  Note all the POWER IOMMUs use the same dma_mapping_error function
so this is not a problem for POWER but x86 IOMMUs use different
dma_mapping_error functions.

The first patch adds the device argument to dma_mapping_error.  The patch
is trivial but large since it touches lots of drivers and dma-mapping.h in
all the architecture.

This patch:

dma_mapping_error() doesn't take a pointer to the device unlike other DMA
operations.  So we can't have dma_mapping_ops per device.

Note that POWER already has dma_mapping_ops per device but all the POWER
IOMMUs use the same dma_mapping_error function.  x86 IOMMUs use device
argument.

Even the newer ENE controllers have bugs in their DMA engine that make
it too dangerous to use. Disable it until someone has figured out under
which conditions it corrupts data.

603ded16a3 olpc: sdhci: add quirk for the Marvell CaFe's interrupt timeout
The CaFe chip has a hardware bug that ends up with us getting a timeout
value that's too small, causing the following sorts of problems:

[   60.525138] mmcblk0: error -110 transferring data
[   60.531477] end_request: I/O error, dev mmcblk0, sector 1484353
[   60.533371] Buffer I/O error on device mmcblk0p2, logical block 181632
[   60.533371] lost page write due to I/O error on mmcblk0p2

Presumably this is an off-by-one error in the hardware.  Incrementing
the timeout count value that we stuff into the TIMEOUT_CONTROL register
gets us a value that works.  This bug was originally discovered by
Pierre Ossman, I believe.

This has been sitting around unloved for way too long..

The Marvell CaFe chip's SD implementation chokes during card insertion
if one attempts to set the voltage and power up in the same
SDHCI_POWER_CONTROL register write.  This adds a quirk that does
that particular dance in two steps.

It also adds an entry to pci_ids.h for the CaFe chip's SD device.

guarantee the SDHCI specification provides and not one we can provide.

On most of the others this probably broke.

Instead of increasing the header file spider web I choose to include
<linux/scatterlist.h> directly into the affectes files.

55556da012 MMC: Fix handling of low-voltage cards
Fix handling of low voltage MMC cards.

The latest MMC and SD specs both agree that support for
low-voltage operations is indicated by bit 7 in the OCR.
The MMC spec states that the low voltage range is
1.65-1.95V while the SD spec leaves the actual voltage
range undefined - meaning that there is still no such
thing as a low voltage SD card.

However, an old Sandisk spec implied that bits 7.0
represented voltages below 2.0V in 1V or 0.5V increments,
and the code was accordingly written with that expectation.

This confusion meant that host drivers attempting to support
the typical low voltage (1.8V) would set the wrong bits in
the host OCR mask (usually bits 5 and/or 6) resulting in the
the low voltage mode never being used.

This change corrects the low voltage range and adds sanity
checks on the reserved bits (0-6) and for SD cards that
claim to support low-voltage operations.

4be34c99a2 MMC: Consolidate voltage definitions
Consolidate the list of available voltages.

Up until now, a separate set of defines has been
used for host->vdd than that used for the OCR
voltage mask values. Having two sets of defines
allows them to get out of sync and the current
sets are already inconsistent with one claiming
to describe ranges and the other specific voltages.

Only the SDHCI driver uses the host->vdd defines and
it is easily fixed to use the OCR defines.
