81d184199e net/mlx4_core: Do not access comm channel if it has not yet been initialized
In the Hypervisor, there are several FW commands which are invoked
before the comm channel is initialized (in mlx4_multi_func_init).
These include MOD_STAT_CONFIG, QUERY_DEV_CAP, INIT_HCA, and others.

If any of these commands fails, say with a timeout, the Hypervisor
driver enters the internal error reset flow. In this flow, the driver
attempts to notify all slaves via the comm channel that an internal error
has occurred.

Since the comm channel has not yet been initialized (i.e., mapped via
ioremap), this will cause dereferencing a NULL pointer.

To fix this, do not access the comm channel in the internal error flow
if it has not yet been initialized.

b42959dc35 net/mlx4: Add VF vlan protocol 802.1ad support
Move the vf to VST 802.1ad mode (mlx4 VST QinQ mode) by setting vf vlan
protocol to 802.1ad.
VST 802.1ad mode in mlx4, is used for STAG strip/insertion by PF, while
the CTAG is set by the VF.
Read current vlan protocol as part of the vf configuration state.

Upon setting vf vlan protocol to 802.1ad, we use a mechanism of handshake
to verify that both the vf and the pf driver version support it.
The handshake uses the command QUERY_FUNC_CAP:
- The vf sets a pre-defined support bit in input modifier.
- A pf that supports the feature sends the request to the vf through a
  pre-defined field in the output mailbox.
- In case vf does not support the feature, the pf will fail the control
  command (in this case, IP link tool command to set the vf vlan
  protocol to 802.1ad).

No change in VST 802.1Q mode.

Add vport attribute vlan protocol.
Init vport vlan protocol by default to 802.1Q.
Add update QP support for VF vlan protocol 802.1ad.
Add func capability vlan_offload_disable to disable all
vlan HW acceleration on VF while the VF is set to VF vlan protocol
802.1ad mode.
No change in VF vlan protocol 802.1Q (VST) mode.

a7e1f04905 net/mlx4_core: Fix deadlock when switching between polling and event fw commands
When switching from polling-based fw commands to event-based fw
commands, there is a race condition which could cause a fw command
in another task to hang: that task will keep waiting for the polling
sempahore, but may never be able to acquire it. This is due to
mlx4_cmd_use_events, which "down"s the sempahore back to 0.

During driver initialization, this is not a problem, since no other
tasks which invoke FW commands are active.

However, there is a problem if the driver switches to polling mode
and then back to event mode during normal operation.

The "test_interrupts" feature does exactly that.
Running "ethtool -t <eth device> offline" causes the PF driver to
temporarily switch to polling mode, and then back to event mode.
(Note that for VF drivers, such switching is not performed).

Fix this by adding a read-write semaphore for protection when
switching between modes.

0c5ddb51e8 net/mlx4_en: initialize cmd.context_lock spinlock earlier
Maciej Żenczykowski reported lockdep warning a spinlock
was not registered before being held in mlx4_cmd_wake_completions()

cmd.context_lock initialization is not at the right place.

1) mlx4_cmd_use_events() can be called multiple times.
   Calling spin_lock_init() on a live spinlock can lead
   to hangs.

2) mlx4_cmd_wake_completions() can be called while lock
   has not been initialized.
   Lockdep complains, and current logic is not race prone.

It seems better to move the initialization earlier in
mlx4_load_one()

fa51b247d6 net/mlx4: fix some error handling in mlx4_multi_func_init()
The while loop after err_slaves should use post-decrement; otherwise
we'll fail to do the kfrees for i==0, and will run into out-of-bounds
accesses if the setup above failed already at i==0.

[I'm not sure why one even bothers populating the ->vlan_filter array:
mlx4.h isn't #included by anything outside
drivers/net/ethernet/mellanox/mlx4/, and "git grep -C2 -w vlan_filter
drivers/net/ethernet/mellanox/mlx4/" seems to suggest that the
vlan_filter elements aren't used at all.]

c02b05011f net/mlx4: Copy/set only sizeof struct mlx4_eqe bytes
When doing memcpy/memset of EQEs, we should use sizeof struct
mlx4_eqe as the base size and not caps.eqe_size which could be bigger.

If caps.eqe_size is bigger than the struct mlx4_eqe then we corrupt
data in the master context.

When using a 64 byte stride, the memcpy copied over 63 bytes to the
slave_eq structure.  This resulted in copying over the entire eqe of
interest, including its ownership bit -- and also 31 bytes of garbage
into the next WQE in the slave EQ -- which did NOT include the ownership
bit (and therefore had no impact).

However, once the stride is increased to 128, we are overwriting the
ownership bits of *three* eqes in the slave_eq struct.  This results
in an incorrect ownership bit for those eqes, which causes the eq to
seem to be full. The issue therefore surfaced only once 128-byte EQEs
started being used in SRIOV and (overarchitectures that have 128/256
byte cache-lines such as PPC) - e.g after commit 77507aa249
"net/mlx4_core: Enable CQE/EQE stride support".

9f5b031770 net/mlx4_core: Relieve cpu load average on the port sending flow
When a port is not attached, the FW requires a longer than usual time to
execute the SENSE_PORT command. In the command flow, the
wait_for_completion_timeout call used in mlx4_cmd_wait puts the kernel
thread into the uninterruptible state during this time. This, in turn,
due to the computation method, causes the CPU load average to increase.

Fix this by using wait_for_completion_interruptible_timeout() for the
SENSE_PORT command, which puts the thread in the interruptible state.
In this state, the thread does not contribute to the CPU load average.

Treat the interrupted case as if the SENSE_PORT command returned
port_type = NONE.

f4ecf29fd7 mlx4_core: Fix fallback from MSI-X to INTx
The test in mlx4_load_one() to remove MLX4_FLAG_MSI_X expects mlx4_NOP() to
fail with -EBUSY. It is also necessary to avoid the reset since the device
is not fully reinitialized before calling mlx4_start_hca() a second time.

Note that this will also affect mlx4_test_interrupts(), the only other user
of MLX4_CMD_NOP.

Remove the limitation that disallows configuring single ported VFs
in the presence of IB ports, after addressing the issues that
prevented that to work.

7c35ef4525 net/mlx4_core: Enhance the MAD_IFC wrapper to convert VF port to physical
Single port VFs always provide port = 1 (even if the actual physical
port used is port 2). As such, we need to convert the port provided
by the VF to the physical port before calling into the firmware.

It turns out that the Linux mlx4 VF RoCE driver maintains a copy of
the GID table and hence this change became critical only for single
ported IB VFs, but it could be needed for other RoCE VF drivers too.

e9a7ff3a71 net/mlx4_core: Return the admin alias GUID upon host view request
Return the admin alias GUID value upon a GET request via HOST. We do this so
that the GUID value requested by the admin is returned even if the SM has not
yet approved this GUID (e.g. the SM is down).

Note that this does not create a problem, since the virtual port will remain
down until the SM does ACK the requested GUID value.

cda373f484 net/mlx4_en: Enable TX rate limit per VF
Support granular QoS per VF, by implementing the ndo_set_vf_rate.

Enforce a rate limit per VF when called, and enabled only for VFs in
VST mode with user priority supported by the device.

We don't enforce VFs to be in VST mode at the moment of configuration,
but rather save the given rate limit and enforce it when the VF is
moved to VST with user priority which is supported (currently 0).

VST<->VGT or VST qos value state changes are disallowed when a rate
limit is configured. Minimum BW share is not supported yet.

08068cd568 net/mlx4: Added qos_vport QP configuration in VST mode
Granular QoS per VF feature introduce a new QP field, qos_vport.

PF administrator can connect VF QPs to a certain QoS Vport, to
inherit its proporties. Connecting QPs to the default QoS Vport
(defined as 0) is always allowed, even when there are no allocated VPPs.
At this point, only the default vport is connected to QPs.

666672d480 net/mlx4: Allocate VPPs for each port on PF init
Initialization of granular Qos per VF mechanism.

Query the port availible VPPs and allocates those on all supported
priorities in an equal share. Allocation is done only in SRIOV mode,
when the feature is supported by the device and port type is Ethernet.

Allocation currently is done only on the default priority 0.

1c29146d38 net/mlx4: Add mlx4_SET_VPORT_QOS implementation
Add the SET_VPORT_QOS device command, which is ntended for virtual
granular QoS configuration per VF in SRIOV mode. The SET_VPORT_QOS
command sets and queries QoS parameters of a VPort. Each priority
allowed for a VPort is assigned with a share of the BW, and a BW
limitation. QoS parameters can be modified at any time, but must be
initialized before any QP is associated with the VPort.

7e95bb99a8 net/mlx4: Add mlx4_ALLOCATE_VPP implementation
Implements device ALLOCATE_VPP command, to be used for granular QoS
configuration of VFs by the PF device. Defines and queries the amount
of VPPs assigned to each port, and the amount of VPPs assigned to each
priority of each port. Once the total VPPs are split between the priorities
of a port, they may be assigned with a share of the BW or a rate limit.

Split into two functions (get/set) whoch are supplied with
mlx4_alloc_vpp_context and physical port number.

4abccb6157 net/mlx4: Aesthetic code changes in multi_func_init
Previous vf_oper and vf_admin code created very long lines, making it hard
to read the code. Added relevant in-struct pointers to reduce code
complexity and avoid code lines spread over 80 lines. Same logic is preserved.

The TCP conflicts were overlapping changes.  In 'net' we added a
READ_ONCE() to the socket cached RX route read, whilst in 'net-next'

With USB, it's a case of the same bug fix first going into net-next
and then I cherry picked it back into net.

bffb023ad2 net/mlx4_core: Fix GEN_EQE accessing uninitialixed mutex
We occasionally see in procedure mlx4_GEN_EQE that the driver tries
to grab an uninitialized mutex.

This can occur in only one of two ways:
1. We are trying to generate an async event on an uninitialized slave.
2. We are trying to generate an async event on an illegal slave number
   ( < 0 or > persist->num_vfs) or an inactive slave.

To deal with #1: move the mutex initialization from specific slave init
sequence in procedure mlx_master_do_cmd to mlx4_multi_func_init() (so that
the mutex is always initialized for all slaves).

To deal with #2: check in procedure mlx4_GEN_EQE that the slave number
provided is in the proper range and that the slave is active.

A table of size dev->caps.pkey_table_len[port]*sizeof(*table)
was allocated, but get_full_pkey_table() assumes that the number
of entries in the table is a multiplication of 32 (which isn't always
correct).

The driver incorrectly assigned an out-mailbox to this command,
and used an opcode modifier = 0, which is a reserved value (it
should use opcode modifier = 1).

0cd9302734 net/mlx4_core: Reset flow activation upon SRIOV fatal command cases
When SRIOV commands are executed over the comm-channel and get
a fatal error (e.g. timeout, closing command failure) the VF enters
into error state and reset flow is activated.

To be able to recognize whether the failure was on a closing command, the
operational code for the given VHCR command is used. Once the device entered
into an error state we prevent redundant error messages from being printed.

55ad359225 net/mlx4_core: Enable device recovery flow with SRIOV
In SRIOV, both the PF and the VF may attempt device recovery whenever they
assume that the device is not functioning.  When the PF driver resets the
device, the VF should detect this and attempt to reinitialize itself.

The VF must be able to reset itself under all circumstances, even
if the PF is not responsive.

The VF shall reset itself in the following cases:

1. Commands are not processed within reasonable time over the communication channel.
This is done considering device state and the correct return code based on
the command as was done in the native mode, done in the next patch.

2. The VF driver receives an internal error event reported by the PF on the
communication channel. This occurs when the PF driver resets the device or
when VF is out of sync with the PF.

Add 'VF reset' capability, which allows the VF to reinitialize itself even when the
PF is not responsive.

As PF and VF may run their reset flow simulantanisly, there are several cases
that are handled:
- Prevent freeing VF resources upon FLR, when PF is in its unloading stage.
- Prevent PF getting VF commands before it has finished initializing its resources.
- Upon VF startup, check that comm-channel is online before sending
  commands to the PF and getting timed-out.

f5aef5aa35 net/mlx4_core: Activate reset flow upon fatal command cases
We activate reset flow upon command fatal errors, when the device enters an
erroneous state, and must be reset.

The cases below are assumed to be fatal: FW command timed-out, an error from FW
on closing commands, pci is offline when posting/pending a command.

In those cases we place the device into an error state: chip is reset, pending
commands are awakened and completed immediately. Subsequent commands will
return immediately.

The return code in the above cases will depend on the command. Commands which
free and close resources will return success (because the chip was reset, so
callers may safely free their kernel resources). Other commands will return -EIO.

Since the device's state was marked as error, the catas poller will
detect this and restart the device's software stack (as is done when a FW
internal error is directly detected). The device state is protected by a
persistent mutex lives on its mlx4_dev, as such no need any more for the
hcr_mutex which is removed.

ffc39f6d6f net/mlx4_core: Refactor mlx4_cmd_init and mlx4_cmd_cleanup
Refactoring mlx4_cmd_init and mlx4_cmd_cleanup such that partial init
and cleanup are possible. After this refactoring, calling mlx4_cmd_init
several times is safe.

This is necessary in the VF init flow when mlx4_init_hca returns -EACCESS,
we need to issue cleanup and re-attempt to call it with the slave flag.

      contributors, is the addition of multi-send for transmit.  This is
      the result of discussions back in Chicago, and the hard work of

      Now, when the ->ndo_start_xmit() method of a driver sees
      skb->xmit_more as true, it can choose to defer the doorbell
      telling the driver to start processing the new TX queue entires.

      skb->xmit_more means that the generic networking is guaranteed to
      call the driver immediately with another SKB to send.

      There is logic added to the qdisc layer to dequeue multiple
      packets at a time, and the handling mis-predicted offloads in
      software is now done with no locks held.

      Finally, pktgen is extended to have a "burst" parameter that can
      be used to test a multi-send implementation.

      Several drivers have xmit_more support: i40e, igb, ixgbe, mlx4,
      virtio_net

      Adding support is almost trivial, so export more drivers to
      support this optimization soon.

1daa4303b4 net/mlx4_core: Deprecate error message at ConnectX-2 cards startup to debug
ConnectX2 HCAs have max_mtu=4k and max_vl=8 vls. However, if you specify
a 4K mtu, the max_vl supported for 4K is 4 vls. The driver at startup
attempts to set a 4K mtu using the max_vl value obtained from QUERY_PORT.

Since the max_vl value is 8 vls (which is supported up to 2K mtu size),
the first attempt to set the mtl/vl port value will fail, generating
the following error message in the log:

  mlx4_core 0000:06:00.0: command 0xc failed: fw status = 0x40

The driver then tries again, using mtu=4k, vls=4, and this succeeds.

Since we do not want to have this error message always displayed at driver
start when there are ConnectX2 HCAs on the host, we deprecate the error
message for this specific command/input_modifier/opcode_modifier/fw-status
to be debug.

a91c772fa0 net/mlx4: Correctly configure single ported VFs from the host
Single port VFs are seen PCI wise on both ports of the PF (we don't have
single port PFs with ConnectX). With this in mind, it's possible for
virtualization tools to try and configure a single ported VF through
the "wrong" PF port.

To handle that, we use the PF driver mapping of single port VFs to NIC
ports and adjust the port value before calling into the low level
code that does the actual VF configuration

114840c3d2 mlx4_core: Add support for secure-host and SMP firewall
Secure-host is the general term for the capability of a device
to protect itself and the subnet from malicious host software.

This is achieved by:
1. Not allowing un-trusted entities to access device configuration
   registers, directly (through pci_cr or pci_conf) and indirectly
   (through MADs).

2. Hiding M_Key from untrusted entities.

3. Preventing the modification of GUID0 by un-trusted entities

4. Not allowing drivers on untrusted hosts to receive nor to transmit
   packets over QP0 (SMP Firewall).

The secure-host capability depends on firmware handling all QP0
packets, and not passing these packets up to the driver. Any information
required by the driver for proper operation (e.g., SM lid) is passed
via events generated by the firmware while processing QP0 MADs.

Driver support mainly requires using the MAD_DEMUX FW command at startup,
where the feature is enabled/disabled through a procedure described in
the Mellanox HCA tools package.
