It appears as though the addition of the PID namespace did not update
the output code for /proc/*/sched, which resulted in it providing PIDs
that were not self-consistent with the /proc mount. This additionally
made it trivial to detect whether a process was inside &init_pid_ns from
userspace, making container detection trivial:

This leads to situations such as:
% unshare -pmf
% mount -t proc proc /proc
% head -n1 /proc/1/sched
head (10047, #threads: 1)
Fix this by just using task_pid_nr_ns for the output of /proc/*/sched.
All of the other uses of task_pid_nr in kernel/sched/debug.c are from a
sysctl context and thus don't need to be namespaced.

The <linux/sched/debug.h> file is a self-contained header and users of
it either don't need <linux/sched.h> - or have already included it.
This reduces the size of the header dependency graph.

Collect the various scheduler and task state debugging APIs scattered
around <linux/sched.h> into the new <linux/sched/debug.h> header.
In particular the show_regs() and show_stack() prototype affects many files,
update them.

We are going to split <linux/sched/debug.h> out of <linux/sched.h>, which
will have to be picked up from other headers and a couple of .c files.
Create a trivial placeholder <linux/sched/debug.h> file that just
maps to <linux/sched.h> to make this patch obviously correct and
bisectable.
Include the new header in the files that are going to need it.
