df41017eaf ia64: remove support for machvecs
The only thing remaining of the machvecs is a few checks if we are
running on an SGI UV system.  Replace those with the existing
is_uv_system() check that has been rewritten to simply check the
OEM ID directly.

That leaves us with a generic kernel that is as fast as the previous
DIG/ZX1/UV kernels, but can support all hardware.  Support for UV
and the HP SBA IOMMU is now optional based on new config options.

05933aac7b ia64: remove now unused machvec indirections
With the SGI SN2 machvec removal most of the indirections are unused
now, so remove them.  This includes the entire removal of the mmio
read*/write* macros as the generic ones are identical to the
asm-generic/io.h version.

db9a0975a2 docs: ia64: convert to ReST
Rename the ia64 documentation files to ReST, add an
index for them and adjust in order to produce a nice html
output via the Sphinx build system.

There are two upper case file names. Rename them to
lower case, as we're working to avoid upper case file
names at Documentation.

At its new index.rst, let's add a :orphan: while this is not linked to
the main index.rst file, in order to avoid build warnings.

457c899653 treewide: Add SPDX license identifier for missed files
Add SPDX license identifiers to all files which:

 - Have no license information of any form

 - Have EXPORT_.*_SYMBOL_GPL inside which was used in the
   initial scan/conversion to ignore the file

These files fall under the project license, GPL v2 only. The resulting SPDX
license identifier is:

  GPL-2.0-only

c6d4381220 dma-mapping: make the get_required_mask method available unconditionally
This save some duplication for ia64, and makes the interface more
general.  In the long run we want each dma_map_ops instance to fill this
out, but this will take a little more prep work.

5a1e0baa34 PCI/MSI: Don't set up INTx if MSI or MSI-X is enabled
If MSI or MSI-X is enabled, the device uses that.  It uses INTx only if
both MSI and MSI-X are disabled (see PCIe r4.0, sec 7.7.1.2), so if a
device already has MSI or MSI-X enabled, there's no need to set up its
legacy INTx interrupt.

bba6f6fc68 ("[PATCH] MSI-X: fix resume crash") changed the cris, frv,
x86, and ia64 arches to skip INTx setup when MSI is enabled.  The cris and
frv arches have since been removed.

16cf0ebc35 ("x86/PCI: Do not use interrupt links for devices using
MSI-X") extended that by changing x86 to also skip INTx setup when MSI-X is
enabled.

Change ia64 to skip INTx setup when either MSI or MSI-X is enabled by
applying the logic from 16cf0ebc35 to ia64 as well as x86.

ecf677c8dc PCI: Add a generic weak pcibios_align_resource()
Multiple architectures define this as a trivial function, and I'm adding
another one as part of the RISC-V port.  Add a __weak version of
pcibios_align_resource() and delete the now-obselete ones in a handful of
ports.

The only functional change should be that a handful of ports used to export
pcibios_fixup_bus().  Only some architectures export this, so I just
dropped it.

fcdb10d6b1 ia64: Remove redundant checks for WC in pci_mmap_page_range()
For a PCI MMIO BAR, phys_mem_access_prot() should always return UC or WC.
And while a mixture of cached and uncached mappings is forbidden, we were
already mixing WC and UC, which is OK. Just do as we're asked.

237865f195 PCI: Revert "PCI: Call pci_read_bridge_bases() from core instead of arch code"
Revert dff22d2054 ("PCI: Call pci_read_bridge_bases() from core instead
of arch code").

Reading PCI bridge windows is not arch-specific in itself, but there is PCI
core code that doesn't work correctly if we read them too early.  For
example, Hannes found this case on an ARM Freescale i.mx6 board:

  pci_bus 0000:00: root bus resource [mem 0x01000000-0x01efffff]
  pci 0000:00:00.0: PCI bridge to [bus 01-ff]
  pci 0000:00:00.0: BAR 8: no space for [mem size 0x01000000] (mem window)
  pci 0000:01:00.0: BAR 2: failed to assign [mem size 0x00200000]
  pci 0000:01:00.0: BAR 1: failed to assign [mem size 0x00004000]
  pci 0000:01:00.0: BAR 0: failed to assign [mem size 0x00000100]

The 00:00.0 mem window needs to be at least 3MB: the 01:00.0 device needs
0x204100 of space, and mem windows are megabyte-aligned.

Bus sizing can increase a bridge window size, but never *decrease* it (see
d65245c329 ("PCI: don't shrink bridge resources")).  Prior to
dff22d2054, ARM didn't read bridge windows at all, so the "original size"
was zero, and we assigned a 3MB window.

After dff22d2054, we read the bridge windows before sizing the bus.  The
firmware programmed a 16MB window (size 0x01000000) in 00:00.0, and since
we never decrease the size, we kept 16MB even though we only needed 3MB.
But 16MB doesn't fit in the host bridge aperture, so we failed to assign
space for the window and the downstream devices.

I think this is a defect in the PCI core: we shouldn't rely on the firmware
to assign sensible windows.

Ray reported a similar problem, also on ARM, with Broadcom iProc.

Issues like this are too hard to fix right now, so revert dff22d2054.

dff22d2054 PCI: Call pci_read_bridge_bases() from core instead of arch code
When we scan a PCI bus, we read PCI-PCI bridge window registers with
pci_read_bridge_bases() so we can validate the resource hierarchy.  Most
architectures call pci_read_bridge_bases() from pcibios_fixup_bus(), but
PCI-PCI bridges are not arch-specific, so this doesn't need to be in
arch-specific code.

Call pci_read_bridge_bases() directly from the PCI core instead of from
arch code.

For alpha and mips, we now call pci_read_bridge_bases() always; previously
we only called it if PCI_PROBE_ONLY was set.

dc4fdaf0e4 PCI / ACPI: Do not set ACPI companions for host bridges with parents
Commit 97badf873a (device property: Make it possible to use
secondary firmware nodes) uncovered a bug in the x86 (and ia64) PCI
host bridge initialization code that assumes bridge->bus->sysdata
to always point to a struct pci_sysdata object which need not be
the case (in particular, the Xen PCI frontend driver sets it to point
to a different data type).  If it is not the case, an incorrect
pointer (or a piece of data that is not a pointer at all) will be
passed to ACPI_COMPANION_SET() and that may cause interesting
breakage to happen going forward.

To work around this problem use the observation that the ACPI
host bridge initialization always passes NULL as parent to
pci_create_root_bus(), so if pcibios_root_bridge_prepare() sees
a non-NULL parent of the bridge, it should not attempt to set
an ACPI companion for it, because that means that
pci_create_root_bus() has been called by someone else.

9fbbda5c8e ia64/PCI: Treat all host bridge Address Space Descriptors (even consumers) as windows
Prior to c770cb4cb5 ("PCI: Mark invalid BARs as unassigned"), if we tried
to claim a PCI BAR but could not find an upstream bridge window that
matched it, we complained but still allowed the device to be enabled.

c770cb4cb5 broke devices that previously worked (mptsas and igb in the
case Tony reported, but it could be any devices) because it marks those
BARs as IORESOURCE_UNSET, which makes pci_enable_device() complain and
return failure:

  igb 0000:81:00.0: can't enable device: BAR 0 [mem size 0x00020000] not assigned
  igb: probe of 0000:81:00.0 failed with error -22

The underlying cause is an ACPI Address Space Descriptor for a PCI host
bridge window that is marked as "consumer".  This is a firmware defect:
resources that are produced on the downstream side of a bridge should be
marked "producer".  But rejecting these BARs that we previously allowed is
a functionality regression, and firmware has not used the producer/consumer
bit consistently, so we can't rely on it anyway.

Stop checking the producer/consumer bit, and assume all bridge Address
Space Descriptors are for bridge windows.

Note that this change does not affect I/O Port or Fixed Location I/O Port
Descriptors, which are commonly used for the [io 0x0cf8-0x0cff] config
access range.  That range is a "consumer" range and should not be treated
as a window.

a45de93eb1 ACPICA: Resources: Provide common part for struct acpi_resource_address structures.
struct acpi_resource_address and struct acpi_resource_extended_address64 share substracts
just at different offsets. To unify the parsing functions, OSPMs like Linux
need a new ACPI_ADDRESS64_ATTRIBUTE as their substructs, so they can
extract the shared data.

This patch also synchronizes the structure changes to the Linux kernel.
The usages are searched by matching the following keywords:
1. acpi_resource_address
2. acpi_resource_extended_address
3. ACPI_RESOURCE_TYPE_ADDRESS
4. ACPI_RESOURCE_TYPE_EXTENDED_ADDRESS

ce821ef033 ia64/PCI: Clip bridge windows to fit in upstream windows
Every PCI-PCI bridge window should fit inside an upstream bridge window
because orphaned address space is unreachable from the primary side of the
upstream bridge.  If we inherit invalid bridge windows that overlap an
upstream window from firmware, clip them to fit and update the bridge
accordingly.

b1e9cee7f6 ia64: Remove acpi_get_pxm() usage
The IOMMU, LSAPIC, IOSAPIC, and PCI host bridge code doesn't care about
_PXM values directly; it only needs to know what NUMA node the hardware is
on.

This uses acpi_get_node() directly and removes the _PXM stuff.

7b1998116b ACPI / driver core: Store an ACPI device pointer in struct acpi_dev_node
Modify struct acpi_dev_node to contain a pointer to struct acpi_device
associated with the given device object (that is, its ACPI companion
device) instead of an ACPI handle corresponding to it.  Introduce two
new macros for manipulating that pointer in a CONFIG_ACPI-safe way,
ACPI_COMPANION() and ACPI_COMPANION_SET(), and rework the
ACPI_HANDLE() macro to take the above changes into account.
Drop the ACPI_HANDLE_SET() macro entirely and rework its users to
use ACPI_COMPANION_SET() instead.  For some of them who used to
pass the result of acpi_get_child() directly to ACPI_HANDLE_SET()
introduce a helper routine acpi_preset_companion() doing an
equivalent thing.

The main motivation for doing this is that there are things
represented by struct acpi_device objects that don't have valid
ACPI handles (so called fixed ACPI hardware features, such as
power and sleep buttons) and we would like to create platform
device objects for them and "glue" them to their ACPI companions
in the usual way (which currently is impossible due to the
lack of valid ACPI handles).  However, there are more reasons
why it may be useful.

First, struct acpi_device pointers allow of much better type checking
than void pointers which are ACPI handles, so it should be more
difficult to write buggy code using modified struct acpi_dev_node
and the new macros.  Second, the change should help to reduce (over
time) the number of places in which the result of ACPI_HANDLE() is
passed to acpi_bus_get_device() in order to obtain a pointer to the
struct acpi_device associated with the given "physical" device,
because now that pointer is returned by ACPI_COMPANION() directly.
Finally, the change should make it easier to write generic code that
will build both for CONFIG_ACPI set and unset without adding explicit
compiler directives to it.

5cd7595dea PCI/IA64: embed pci hostbridge resources into pci_root_info
Currently, pcibios_resource_to_bus() and pcibios_bus_to_resource()
functions use pci_host_bridge to translate bus side address from/to
cpu side address. The pci_window in pci_controller never be used again.
So we remove pci_window in pci_controller and embed hostbridge resource
into pci_root_info. Bjorn suggested to implement hostbridge resources
release in IA64 like in X86, this patch is to prepare for that.

6c0cc950ae ACPI / PCI: Set root bridge ACPI handle in advance
The ACPI handles of PCI root bridges need to be known to
acpi_bind_one(), so that it can create the appropriate
"firmware_node" and "physical_node" files for them, but currently
the way it gets to know those handles is not exactly straightforward
(to put it lightly).

This is how it works, roughly:

  1. acpi_bus_scan() finds the handle of a PCI root bridge,
     creates a struct acpi_device object for it and passes that
     object to acpi_pci_root_add().

  2. acpi_pci_root_add() creates a struct acpi_pci_root object,
     populates its "device" field with its argument's address
     (device->handle is the ACPI handle found in step 1).

  3. The struct acpi_pci_root object created in step 2 is passed
     to pci_acpi_scan_root() and used to get resources that are
     passed to pci_create_root_bus().

  4. pci_create_root_bus() creates a struct pci_host_bridge object
     and passes its "dev" member to device_register().

  5. platform_notify(), which for systems with ACPI is set to
     acpi_platform_notify(), is called.

So far, so good.  Now it starts to be "interesting".

  6. acpi_find_bridge_device() is used to find the ACPI handle of
     the given device (which is the PCI root bridge) and executes
     acpi_pci_find_root_bridge(), among other things, for the
     given device object.

  7. acpi_pci_find_root_bridge() uses the name (sic!) of the given
     device object to extract the segment and bus numbers of the PCI
     root bridge and passes them to acpi_get_pci_rootbridge_handle().

  8. acpi_get_pci_rootbridge_handle() browses the list of ACPI PCI
     root bridges and finds the one that matches the given segment
     and bus numbers.  Its handle is then used to initialize the
     ACPI handle of the PCI root bridge's device object by
     acpi_bind_one().  However, this is *exactly* the ACPI handle we
     started with in step 1.

Needless to say, this is quite embarassing, but it may be avoided
thanks to commit f3fd0c8 (ACPI: Allow ACPI handles of devices to be
initialized in advance), which makes it possible to initialize the
ACPI handle of a device before passing it to device_register().

Accordingly, add a new __weak routine, pcibios_root_bridge_prepare(),
defaulting to an empty implementation that can be replaced by the
interested architecutres (x86 and ia64 at the moment) with functions
that will set the root bridge's ACPI handle before its dev member is
passed to device_register().  Make both x86 and ia64 provide such
implementations of pcibios_root_bridge_prepare() and remove
acpi_pci_find_root_bridge() and acpi_get_pci_rootbridge_handle() that
aren't necessary any more.

Included is a fix for breakage on systems with non-ACPI PCI host
bridges from Bjorn Helgaas.

e30f99222f ia64/PCI: convert to pci_create_root_bus() for correct root bus resources
Convert from pci_create_bus() to pci_create_root_bus().  This way the root
bus resources are correct immediately.  This fixes the problem of "early"
and "header" quirks seeing incorrect root bus resources.

We can't use pci_scan_root_bus() because, like x86, ACPI hotplug currently
requires pci_bus_add_devices() in a separate host bridge .start() method.

91e86df1a0 PCI: IA64: convert pcibios_set_master() to a non-inlined function
This patch converts IA64's architecture-specific 'pcibios_set_master()'
routine to a non-inlined function.  This will allow follow on
patches to create a generic 'pcibios_set_master()' function using the
'__weak' attribute which can be used by all architectures as a default
which, if necessary, can then be over-ridden by architecture-
specific code.

Converting 'pci_bios_set_master()' to a non-inlined function will allow
IA64's 'pcibios_set_master()' implementation to remain architecture-
specific after the generic version is introduced and thus, not change
current behavior.

No functional change.

57283776b2 ACPI: pci_root: pass acpi_pci_root to arch-specific scan
The acpi_pci_root structure contains all the individual items (acpi_device,
domain, bus number) we pass to pci_acpi_scan_root(), so just pass the
single acpi_pci_root pointer directly.

This will make it easier to add _CBA support later.  For _CBA, we need the
entire downstream bus range, not just the base bus number.  We have that in
the acpi_pci_root structure, so passing the pointer makes it available to
the arch-specific code.
