laid the ground work to support per resource waiting locking.
This patch adds that with a lock unique to each sdma engine and pio
sendcontext and makes necessary changes for verbs, PSM, and vnic to use
the new locks.
This is particularly beneficial for smaller messages that will exhaust
resources at a faster rate.

quiesce_qp() waits until all outstanding packets have been freed.
This wait should be momentary. During a link down event, the cleanup
handling does not ensure that all packets caught by the link down are
flushed properly.
This is caused by the fact that the freeze path and the link down
event is handled the same. This is not correct. The freeze path
waits until the HFI is unfrozen and then restarts PIO. A link down
is not a freeze event. The link down path cannot restart the PIO
until link is restored. If the PIO path is restarted before the link
comes up, the application (QP) using the PIO path will hang (until
link is restored).
Fix by separating the linkdown path from the freeze path and use the
link down path for link down events.
Close a race condition sc_disable() by acquiring both the progress
and release locks.
Close a race condition in sc_stop() by moving the setting of the flag
bits under the alloc lock.

If a packet stream uses an UnsupportedVL (virtual lane), the send
engine will not send the packet, and it will not indicate that an
error has occurred. This will cause the packet stream to block.
HFI has 8 virtual lanes available for packet streams. Each lane can
be enabled or disabled using the UnsupportedVL mask. If a lane is
disabled, adding a packet to the send context must be disallowed.
The current mask for determining unsupported VLs defaults to 0 (allow
all). This is incorrect. Only the VLs that are defined should be
allowed.
Determine which VLs are disabled (mtu == 0), and set the appropriate
unsupported bit in the mask. The correct mask will allow the send
engine to error on the invalid VL, and error recovery will work
correctly.

Move some s_flags defines out of rdmavt and into hfi1 because they are
hfi1 specific and therefore should remain in the driver instead of

User send context integrity bits are cleared before the context is
disabled. If the send context is still processing data, any packets
that need those integrity bits will cause an error and halt the send
context.
During the disable handling, the driver waits for the context to drain.
If the context is halted, the driver will eventually timeout because
the context won't drain and then incorrectly bounce the link.
Reorder the bit clearing and the context disable.
Examine the software state and send context status as well as the
egress status to determine if a send context is in the halted state.
Promote the check macros to static functions for consistency with the
new check and to follow kernel style.
Remove an unused define that refers to the egress timeout.

Kzalloc_node API doesn't check for overflows in size multiplication.
While kcalloc API check for overflows in size multiplication
but these implementations are not NUMA-aware.
This conversion allowed for correcting an allocation used in the hot
path to be on the local NUMA and ensure us overflow free multiplication
for the size of a memory allocation.

Please do not apply this to mainline directly, instead please re-run the
coccinelle script shown below and apply its output.
For several reasons, it is desirable to use {READ,WRITE}_ONCE() in
preference to ACCESS_ONCE(), and new code is expected to use one of the
former. So far, there's been no reason to change most existing uses of
ACCESS_ONCE(), as these aren't harmful, and changing them results in
churn.
However, for some features, the read/write distinction is critical to
correct operation. To distinguish these cases, separate read/write
accessors must be used. This patch migrates (most) remaining
ACCESS_ONCE() instances to {READ,WRITE}_ONCE(), using the following

When an egress resource(SDMA descriptors, pio credits) is not available,
a sending thread will be put on the resource's wait queue. When the
resource becomes available again, up to a fixed number of sending threads
can be awakened sequentially and removed from the wait queue, depending
on the number of waiting threads and the number of free resources. Since
each awakened sending thread will send as many packets as possible, it
is highly likely that the first sending thread will consume all the
egress resources. Subsequently, it will be put back to the end of the wait
queue. Depending on the timing when the later sending threads wake up,
they may not be able to send any packet and be again put back to the end
of the wait queue sequentially, right behind the first sending thread.
This starvation cycle continues until some sending threads exceed their
retry limit and consequently fail.
This patch fixes the issue by two simple approaches:
(1) Any starved sending thread will be put to the head of the wait queue
while a served sending thread will be put to the tail;
(2) The most starved sending thread will be served first.

HFI1 HW specific support for VNIC functionality.
Dynamically allocate a set of contexts for VNIC when the first vnic
port is instantiated. Allocate VNIC contexts from user contexts pool
and return them back to the same pool while freeing up. Set aside
enough MSI-X interrupts for VNIC contexts and assign them when the
contexts are allocated. On the receive side, use an RSM rule to
spread TCP/UDP streams among VNIC contexts.

In sc_buffer_alloc(), the sc->alloc_lock is released
before calling sc_release_update(), and it is reacquired
after the function call. This causes CPU lock trading.
Fix it by not dropping the lock before calling
sc_release_update().

Both pio_buf and send_context structs have oversized
fields and have cachelines that can be optimized.
Reduce oversized fields for both structs.
Make sure pio_buf struct fits within a cacheline.
Move read-only fields to their own cacheline in
send_context struct.
All of this will avoid cacheline trading as the ring
progresses and pio buffers/send contexts are used.

Profiling suggests that the read_seqbegin() in
the txreq put logic is colliding with other uses
of the iowait lock.
The packet at a time use of this lock dictates a unique
lock to avoid reader/writer collisions when the number
of vTxWait events is low.
In order to support a unique lock the iowait struct embedded
in the QP is extended to remember the lock that protects the queue
head.
The QP destroy removes that QP from any wait list. It doesn't
need to know the head because of the linked list API, but it does
need to know the lock required to protect the head.
This also opens up the wait logic to have unique per resources locks
which needs to be in future refinement.

The dma_XXX API functions return bus addresses which are
physical addresses when IOMMU is disabled. Buffer
mapping to user-space is done via remap_pfn_range() with PFN
based on bus address instead of physical. This results in
wrong pages being mapped to user-space when IOMMU is enabled.

Checking the return value of the memory allocation call in
init_pervl_scs() was missed. Recently the kmalloc() was changed to
kzalloc() which identified the problem.
While fixing this issue 2 other bugs were noticed. First, the array
being allocated is accessed in the nomem path which can be reached before
it is allocated. Second, kernel_send_context was not released on error.
Fix both of these by creating a more common memory unwind label structure.

While handling buffer control MAD, partially initialized
dd->kernel_send_context area may cause potential dereference
of uninitialized pointers. Fix by using kzalloc_node()
instead of kmalloc_node().

The current value of 500us for the packet egress timeout is too small
which causes the host to declare failure on draining packets too early
and unnecessarily bounces the link. Increase this to 50ms taking into
account the switch packet discard timer default and the worst case
per-VL package drainage rate.

The credit return threshold adjustment on mtu change algorithm does not
take into account all the kernel send contexts that are assigned per VL.
Use the pio send context map to adjust the credit return thresholds for
all the allocated and assigned kernel send contexts based on the MTU
adjustment per VL.
The pio send context map can be changed dynamically based on the actual
number of operational vls which is set by the fabric manager. When this
happens update the credit return threshold values for all the remapped
kernel send contexts.

The pio map initialization function is off by 1 causing the last
kernel send context that is allocated to not get mapped into the
pio map which leads to the last kernel send context not being used
by any of the qps.
The send context reserved for VL15 is taken care of by setting the
scontext variable that is used as the index into the kernel send
context array to 1 and does not need to be accounted for in the
kernel send context counting loop as it is currently done.
Fix the kernel send context counting loop to account for all the
allocated send contexts and map all of them to the different VLs.

The TODO list for the hfi1 driver was completed during 4.6. In addition
other objections raised (which are far beyond what was in the TODO list)
have been addressed as well. It is now time to remove the driver from
staging and into the drivers/infiniband sub-tree.
