d450542e3c block, bfq: increase weight-raising duration for interactive apps
The maximum possible duration of the weight-raising period for
interactive applications is limited to 13 seconds, as this is the time
needed to load the largest application that we considered when tuning
weight raising. Unfortunately, in such an evaluation, we did not
consider the case of very slow virtual machines.

For example, on a QEMU/KVM virtual machine
- running in a slow PC;
- with a virtual disk stacked on a slow low-end 5400rpm HDD;
- serving a heavy I/O workload, such as the sequential reading of
several files;
mplayer takes 23 seconds to start, if constantly weight-raised.

To address this issue, this commit conservatively sets the upper limit
for weight-raising duration to 25 seconds.

e24f1c245f block, bfq: remove slow-system class
BFQ computes the duration of weight raising for interactive
applications automatically, using some reference parameters. In
particular, BFQ uses the best durations (see comments in the code for
how these durations have been assessed) for two classes of systems:
slow and fast ones. Examples of slow systems are old phones or systems
using micro HDDs. Fast systems are all the remaining ones. Using these
parameters, BFQ computes the actual duration of the weight raising,
for the system at hand, as a function of the relative speed of the
system w.r.t. the speed of a reference system, belonging to the same
class of systems as the system at hand.

This slow vs fast differentiation proved to be useful in the past, but
happens to have little meaning with current hardware. Even worse, it
does cause problems in virtual systems, where the speed of the system
can vary frequently, and so widely to just confuse the class-detection
mechanism, and, as we have verified experimentally, to cause BFQ to
compute non-sensical weight-raising durations.

This commit addresses this issue by removing the slow class and the
class-detection mechanism.

4029eef1be block, bfq: add description of weight-raising heuristics
A description of how weight raising works is missing in BFQ
sources. In addition, the code for handling weight raising is
scattered across a few functions. This makes it rather hard to
understand the mechanism and its rationale. This commits adds such a
description at the beginning of the main source file.

aa43457799 block: add ioprio_check_cap function
Aio per command iopriority support introduces a second interface between
userland and the kernel capable of passing iopriority. The aio interface also
needs the ability to verify that the submitting context has sufficient
privileges to submit IOPRIO_RT commands. This patch creates the
ioprio_check_cap function to be used by the ioprio_set system call and also by
the aio interface.

ac857e0d54 block, bfq: remove the removal of 'next' rq in bfq_requests_merged
Since bfq_finish_request() is always called on the request 'next',
after bfq_requests_merged() is finished, and bfq_finish_request()
removes 'next' from its bfq_queue if needed, it isn't necessary to do
such a removal in advance in bfq_merged_requests().

This commit removes such a useless 'next' removal.

8abfa4d6fd block, bfq: remove wrong check in bfq_requests_merged
The request rq passed to the function bfq_requests_merged is always in
a bfq_queue, so the check !RB_EMPTY_NODE(&rq->rb_node) at the
beginning of bfq_requests_merged always succeeds, and the control
flow systematically skips to the end of the function.  This implies
that the body of the function is never executed, i.e., the
repositioning of rq is never performed.

On the opposite end, a control is missing in the body of the function:
'next' must be removed only if it is inside a bfq_queue.

This commit removes the wrong check on rq, and adds the missing check
on 'next'. In addition, this commit adds comments on
bfq_requests_merged.

a12bffebc0 block, bfq: remove wrong lock in bfq_requests_merged
In bfq_requests_merged(), there is a deadlock because the lock on
bfqq->bfqd->lock is held by the calling function, but the code of
this function tries to grab the lock again.

This deadlock is currently hidden by another bug (fixed by next commit
for this source file), which causes the body of bfq_requests_merged()
to be never executed.

This commit removes the deadlock by removing the lock/unlock pair.

a6088845c2 block: kyber: make kyber more friendly with merging
Currently, kyber is very unfriendly with merging. kyber depends
on ctx rq_list to do merging, however, most of time, it will not
leave any requests in ctx rq_list. This is because even if tokens
of one domain is used up, kyber will try to dispatch requests
from other domain and flush the rq_list there.

To improve this, we setup kyber_ctx_queue (kcq) which is similar
with ctx, but it has rq_lists for different domain and build same
mapping between kcq and khd as the ctx & hctx. Then we could merge,
insert and dispatch for different domains separately. At the same
time, only flush the rq_list of kcq when get domain token successfully.
Then if one domain token is used up, the requests could be left in
the rq_list of that domain and maybe merged with following io.

Following is my test result on machine with 8 cores and NVMe card
INTEL SSDPEKKR128G7

5de815a7ee block: remove parent device reference from struct bsg_class_device
Bsg holding a reference to the parent device may result in a crash if a
bsg file handle is closed after the parent device driver has unloaded.

Holding a reference is not really needed: the parent device must exist
between bsg_register_queue and bsg_unregister_queue.  Before the device
goes away the caller does blk_cleanup_queue so that all in-flight
requests to the device are gone and all new requests cannot pass beyond
the queue.  The queue itself is a refcounted object and it will stay
alive with a bsg file.

12f5b93145 blk-mq: Remove generation seqeunce
This patch simplifies the timeout handling by relying on the request
reference counting to ensure the iterator is operating on an inflight
and truly timed out request. Since the reference counting prevents the
tag from being reallocated, the block layer no longer needs to prevent
drivers from completing their requests while the timeout handler is
operating on it: a driver completing a request is allowed to proceed to
the next state without additional syncronization with the block layer.

This also removes any need for generation sequence numbers since the
request lifetime is prevented from being reallocated as a new sequence
while timeout handling is operating on it.

To enables this a refcount is added to struct request so that request
users can be sure they're operating on the same request without it
changing while they're processing it.  The request's tag won't be
released for reuse until both the timeout handler and the completion
are done with it.

ad103e7983 blk-mq: Fix timeout and state order
The block layer had been setting the state to in-flight prior to updating
the timer. This is the wrong order since the timeout handler could observe
the in-flight state with the older timeout, believing the request had
expired when in fact it is just getting started.

e6fc464987 blk-mq: avoid starving tag allocation after allocating process migrates
When the allocation process is scheduled back and the mapped hw queue is
changed, fake one extra wake up on previous queue for compensating wake
up miss, so other allocations on the previous queue won't be starved.

This patch fixes one request allocation hang issue, which can be
triggered easily in case of very low nr_request.

The race is as follows:

1) 2 hw queues, nr_requests are 2, and wake_batch is one

2) there are 3 waiters on hw queue 0

3) two in-flight requests in hw queue 0 are completed, and only two
   waiters of 3 are waken up because of wake_batch, but both the two
   waiters can be scheduled to another CPU and cause to switch to hw
   queue 1

4) then the 3rd waiter will wait for ever, since no in-flight request
   is in hw queue 0 any more.

5) this patch fixes it by the fake wakeup when waiter is scheduled to
   another hw queue

d416c92c5d blk-mq: clear hctx->dispatch_from when mappings change
When the number of hardware queues is changed, the drivers will call
blk_mq_update_nr_hw_queues() to remap hardware queues. This changes
the ctx mappings, but the current code doesn't clear the
->dispatch_from hint. This can result in dispatch_from pointing to
a ctx that isn't mapped to the hctx anymore.

0ba99ca483 block: Add warning for bi_next not NULL in bio_endio()
Recently found a bug where a driver left bi_next not NULL and then
called bio_endio(), and then the submitter of the bio used
bio_copy_data() which was treating src and dst as lists of bios.

Fixed that bug by splitting out bio_list_copy_data(), but in case other
things are depending on bi_next in weird ways, add a warning to help
avoid more bugs like that in the future.

Found a bug (with ASAN) where we were passing a bio to bio_copy_data()
with bi_next not NULL, when it should have been - a driver had left
bi_next set to something after calling bio_endio().

Since the normal case is only copying single bios, split out
bio_list_copy_data() to avoid more bugs like this in the future.

18e5a57d79 block, bfq: postpone rq preparation to insert or merge
When invoked for an I/O request rq, the prepare_request hook of bfq
increments reference counters in the destination bfq_queue for rq. In
this respect, after this hook has been invoked, rq may still be
transformed into a request with no icq attached, i.e., for bfq, a
request not associated with any bfq_queue. No further hook is invoked
to signal this tranformation to bfq (in general, to the destination
elevator for rq). This leads bfq into an inconsistent state, because
bfq has no chance to correctly lower these counters back. This
inconsistency may in its turn cause incorrect scheduling and hangs. It
certainly causes memory leaks, by making it impossible for bfq to free
the involved bfq_queue.

On the bright side, no transformation can still happen for rq after rq
has been inserted into bfq, or merged with another, already inserted,
request. Exploiting this fact, this commit addresses the above issue
by delaying the preparation of an I/O request to when the request is
inserted or merged.

This change also gives a performance bonus: a lock-contention point
gets removed. To prepare a request, bfq needs to hold its scheduler
lock. After postponing request preparation to insertion or merging, no
lock needs to be grabbed any longer in the prepare_request hook, while
the lock already taken to perform insertion or merging is used to
preparare the request as well.

522a777566 block: consolidate struct request timestamp fields
Currently, struct request has four timestamp fields:

- A start time, set at get_request time, in jiffies, used for iostats
- An I/O start time, set at start_request time, in ktime nanoseconds,
  used for blk-stats (i.e., wbt, kyber, hybrid polling)
- Another start time and another I/O start time, used for cfq and bfq

These can all be consolidated into one start time and one I/O start
time, both in ktime nanoseconds, shaving off up to 16 bytes from struct
request depending on the kernel config.

544ccc8dc9 block: get rid of struct blk_issue_stat
struct blk_issue_stat squashes three things into one u64:

- The time the driver started working on a request
- The original size of the request (for the io.low controller)
- Flags for writeback throttling

It turns out that on x86_64, we have a 4 byte hole in struct request
which we can fill with the non-timestamp fields from blk_issue_stat,
simplifying things quite a bit.

50864670b3 block: Shorten interrupt disabled regions
Commit 9c40cef2b7 ("sched: Move blk_schedule_flush_plug() out of
__schedule()") moved the blk_schedule_flush_plug() call out of the
interrupt/preempt disabled region in the scheduler. This allows to replace
local_irq_save/restore(flags) by local_irq_disable/enable() in
blk_flush_plug_list().

But it makes more sense to disable interrupts explicitly when the request
queue is locked end reenable them when the request to is unlocked. This
shortens the interrupt disabled section which is important when the plug
list contains requests for more than one queue. The comment which claims
that disabling interrupts around the loop is misleading as the called
functions can reenable interrupts unconditionally anyway and obfuscates the
scope badly:

f3a1075e5f block: don't disable interrupts during kmap_atomic()
bounce_copy_vec() disables interrupts around kmap_atomic(). This is a
leftover from the old kmap_atomic() implementation which relied on fixed
mapping slots, so the caller had to make sure that the same slot could not
be reused from an interrupting context.

kmap_atomic() was changed to dynamic slots long ago and commit 1ec9c5ddc1
("include/linux/highmem.h: remove the second argument of k[un]map_atomic()")
removed the slot assignements, but the callers were not checked for now
redundant interrupt disabling.

Remove the conditional interrupt disable.

6131837b1d blk-mq: count allocated but not started requests in iostats inflight
In the legacy block case, we increment the counter right after we
allocate the request, not when the driver handles it. In both the legacy
and blk-mq cases, part_inc_in_flight() is called from
blk_account_io_start() right after we've allocated the request. blk-mq
only considers requests started requests as inflight, but this is
inconsistent with the legacy definition and the intention in the code.
This removes the started condition and instead counts all allocated
requests.

f4560231ec blk-mq: start request gstate with gen 1
rq->gstate and rq->aborted_gstate both are zero before rqs are
allocated. If we have a small timeout, when the timer fires,
there could be rqs that are never allocated, and also there could
be rq that has been allocated but not initialized and started. At
the moment, the rq->gstate and rq->aborted_gstate both are 0, thus
the blk_mq_terminate_expired will identify the rq is timed out and
invoke .timeout early.

For scsi, this will cause scsi_times_out to be invoked before the
scsi_cmnd is not initialized, scsi_cmnd->device is still NULL at
the moment, then we will get crash.

1dc3039bc8 block: do not use interruptible wait anywhere
When blk_queue_enter() waits for a queue to unfreeze, or unset the
PREEMPT_ONLY flag, do not allow it to be interrupted by a signal.

The PREEMPT_ONLY flag was introduced later in commit 3a0a529971
("block, scsi: Make SCSI quiesce and resume work reliably").  Note the SCSI
device is resumed asynchronously, i.e. after un-freezing userspace tasks.

So that commit exposed the bug as a regression in v4.15.  A mysterious
SIGBUS (or -EIO) sometimes happened during the time the device was being
resumed.  Most frequently, there was no kernel log message, and we saw Xorg
or Xwayland killed by SIGBUS.[1]

Without this fix, I get an IO error in this test:

The interruptible wait was added to blk_queue_enter in
commit 3ef28e83ab ("block: generic request_queue reference counting").
Before then, the interruptible wait was only in blk-mq, but I don't think
it could ever have been correct.

2434af79c8 blk-mq: Revert "blk-mq: reimplement blk_mq_hw_queue_mapped"
This reverts commit 127276c6ce.

When all CPUs of one hw queue become offline, there still may have IOs
not completed from this hctx. But blk_mq_hw_queue_mapped() is called in
blk_mq_queue_tag_busy_iter(), which is used for iterating request in timeout
handler, timeout event will be missed on the inactive hctx, then request may
never be completed.

Also the replementation of blk_mq_hw_queue_mapped() doesn't match the helper's
name any more, and it should have been named as blk_mq_hw_queue_active().

Even other callers need further verification about this reimplemenation.

So revert this patch now, and we can improve hw queue activate/inactivate event
after adequent researching and test.

37c7c6c76d blk-mq: remove code for dealing with remapping queue
Firstly, from commit 4b855ad371 ("blk-mq: Create hctx for each present CPU),
blk-mq doesn't remap queue any more after CPU topo is changed.

Secondly, set->nr_hw_queues can't be bigger than nr_cpu_ids, and now we map
all possible CPUs to hw queues, so at least one CPU is mapped to each hctx.

So queue mapping has became static and fixed just like percpu variable, and
we don't need to handle queue remapping any more.

efea8450c3 blk-mq: don't check queue mapped in __blk_mq_delay_run_hw_queue()
There are several reasons for removing the check:

1) blk_mq_hw_queue_mapped() returns true always now since each hctx
may be mapped by one CPU at least

2) when there isn't any online CPU mapped to this hctx, there won't
be any IO queued to this CPU, blk_mq_run_hw_queue() only runs queue
if there is IO queued to this hctx

3) If __blk_mq_delay_run_hw_queue() is called by blk_mq_delay_run_hw_queue(),
which is run from blk_mq_dispatch_rq_list() or scsi_mq_get_budget(), and
the hctx to be handled has to be mapped.

bffa9909a6 blk-mq: don't keep offline CPUs mapped to hctx 0
From commit 4b855ad371 ("blk-mq: Create hctx for each present CPU),
blk-mq doesn't remap queue after CPU topo is changed, that said when
some of these offline CPUs become online, they are still mapped to
hctx 0, then hctx 0 may become the bottleneck of IO dispatch and
completion.

This patch sets up the mapping from the beginning, and aligns to
queue mapping for PCI device (blk_mq_pci_map_queues()).

This patch orders getting budget and driver tag by making sure to acquire
driver tag after budget is got, this way can help to avoid the following
race:

1) before dispatch request from scheduler queue, get one budget first, then
dequeue a request, call it request A.

2) in another IO path for dispatching request B which is from hctx->dispatch,
driver tag is got, then try to get budget in blk_mq_dispatch_rq_list(),
unfortunately the budget is held by request A.

3) meantime blk_mq_dispatch_rq_list() is called for dispatching request
A, and try to get driver tag first, unfortunately no driver tag is
available because the driver tag is held by request B

4) both two IO pathes can't move on, and IO stall is caused.

This issue can be observed when running dbench on USB storage.

This patch fixes this issue by always getting budget before getting
driver tag.

bc6d65e6dc blk-mq: Directly schedule q->timeout_work when aborting a request
Request abortion is performed by overriding deadline to now and
scheduling timeout handling immediately.  For the latter part, the
code was using mod_timer(timeout, 0) which can't guarantee that the
timer runs afterwards.  Let's schedule the underlying work item
directly instead.

This fixes the hangs during probing reported by Sitsofe but it isn't
yet clear to me how the failure can happen reliably if it's just the
above described race condition.

bc56e2cafa block, bfq: lower-bound the estimated peak rate to 1
If a storage device handled by BFQ happens to be slower than 7.5 KB/s
for a certain amount of time (in the order of a second), then the
estimated peak rate of the device, maintained in BFQ, becomes equal to
0. The reason is the limited precision with which the rate is
represented (details on the range of representable values in the
comments introduced by this commit). This leads to a division-by-zero
error where the estimated peak rate is used as divisor. Such a type of
failure has been reported in [1].

This commit addresses this issue by:
1. Lower-bounding the estimated peak rate to 1
2. Adding and improving comments on the range of rates representable

818e0fa293 block: Change a rcu_read_{lock,unlock}_sched() pair into rcu_read_{lock,unlock}()
scsi_device_quiesce() uses synchronize_rcu() to guarantee that the
effect of blk_set_preempt_only() will be visible for percpu_ref_tryget()
calls that occur after the queue unfreeze by using the approach
explained in https://lwn.net/Articles/573497/. The rcu read lock and
unlock calls in blk_queue_enter() form a pair with the synchronize_rcu()
call in scsi_device_quiesce(). Both scsi_device_quiesce() and
blk_queue_enter() must either use regular RCU or RCU-sched.
Since neither the RCU-protected code in blk_queue_enter() nor
blk_queue_usage_counter_release() sleeps, regular RCU protection
is sufficient. Note: scsi_device_quiesce() does not have to be
modified since it already uses synchronize_rcu().

We've triggered a WARNING in blk_throtl_bio() when throttling writeback
io, which complains blkg->refcnt is already 0 when calling blkg_get(),
and then kernel crashes with invalid page request.
After investigating this issue, we've found it is caused by a race
between blkcg_bio_issue_check() and cgroup_rmdir(), which is described
below:

Since rcu can only prevent blkg from releasing when it is being used,
the blkg->refcnt can be decreased to 0 during blkg_destroy() and schedule
blkg release.
Then trying to blkg_get() in blk_throtl_bio() will complains the WARNING.
And then the corresponding blkg_put() will schedule blkg release again,
which result in double free.
This race is introduced by commit ae11889636 ("blkcg: consolidate blkg
creation in blkcg_bio_issue_check()"). Before this commit, it will
lookup first and then try to lookup/create again with queue_lock. Since
revive this logic is a bit drastic, so fix it by only offlining pd during
blkcg_css_offline(), and move the rest destruction (especially
blkg_put()) into blkcg_css_free(), which should be the right way as
discussed.

17cb960f29 bsg: split handling of SCSI CDBs vs transport requeues
The current BSG design tries to shoe-horn the transport-specific
passthrough commands into the overall framework for SCSI passthrough
requests.  This has a couple problems:

 - each passthrough queue has to set the QUEUE_FLAG_SCSI_PASSTHROUGH flag
   despite not dealing with SCSI commands at all.  Because of that these
   queues could also incorrectly accept SCSI commands from in-kernel
   users or through the legacy SCSI_IOCTL_SEND_COMMAND ioctl.
 - the real SCSI bsg queues also incorrectly accept bsg requests of the
   BSG_SUB_PROTOCOL_SCSI_TRANSPORT type
 - the bsg transport code is almost unredable because it tries to reuse
   different SCSI concepts for its own purpose.

This patch instead adds a new bsg_ops structure to handle the two cases
differently, and thus solves all of the above problems.  Another side
effect is that the bsg-lib queues also don't need to embedd a
struct scsi_request anymore.

f3bc78d2d4 mq-deadline: Make sure to always unlock zones
In case of a failed write request (all retries failed) and when using
libata, the SCSI error handler calls scsi_finish_command(). In the
case of blk-mq this means that scsi_mq_done() does not get called,
that blk_mq_complete_request() does not get called and also that the
mq-deadline .completed_request() method is not called. This results in
the target zone of the failed write request being left in a locked
state, preventing that any new write requests are issued to the same
zone.

Fix this by replacing the .completed_request() method with the
.finish_request() method as this method is always called whether or
not a request completes successfully. Since the .finish_request()
method is only called by the blk-mq core if a .prepare_request()
method exists, add a dummy .prepare_request() method.

    queue lock, here it is the default *internal lock*;
(5) *race window*, now queue_lock is overridden with *driver specified
    lock*;
(6) now unlock *driver specified lock*, not the locked *internal lock*,
    unlock balance breaks.

The changes in this patch are as follows:
- Move the .queue_lock initialization from blk_init_queue_node() into
  blk_alloc_queue_node().
- Only override the .queue_lock pointer for legacy queues because it
  is not useful for blk-mq queues to override this pointer.
- For all all block drivers that initialize .queue_lock explicitly,
  change the blk_alloc_queue() call in the driver into a
  blk_alloc_queue_node() call and remove the explicit .queue_lock
  initialization. Additionally, initialize the spin lock that will
  be used as queue lock earlier if necessary.

e9a99a6388 block: clear ctx pending bit under ctx lock
When we insert a request, we set the software queue pending bit while
holding the software queue lock. However, we clear it outside of the
lock, so it's possible that a concurrent insert could reset the bit
after we clear it but before we empty the request list. Afterwards, the
bit would still be set but the software queue wouldn't have any requests
in it, leading us to do a spurious run in the future. This is mostly a
benign/theoretical issue, but it makes the following change easier to
justify.

The problem here is that block device opens can see various intermediate
states while gendisk is shutting down and then being recreated.

We fix the problem by introducing new lookup_sem in gendisk that
synchronizes gendisk deletion with get_gendisk() and furthermore by
making sure that get_gendisk() does not return gendisk that is being (or
has been) deleted. This makes sure that once we ever manage to look up
newly created bdev inode, we are also guaranteed that following
get_gendisk() will either return failure (and we fail open) or it
returns gendisk for the new device and following bdget_disk() will
return new bdev inode (i.e., blkdev_open() follows the path as if it is
completely run after new device is created).

105976f517 blk-mq: don't call io sched's .requeue_request when requeueing rq to ->dispatch
__blk_mq_requeue_request() covers two cases:

- one is that the requeued request is added to hctx->dispatch, such as
blk_mq_dispatch_rq_list()

- another case is that the request is requeued to io scheduler, such as
blk_mq_requeue_request().

We should call io sched's .requeue_request callback only for the 2nd
case.

67b4110f8c blk: optimization for classic polling
This removes the dependency on interrupts to wake up task. Set task
state as TASK_RUNNING, if need_resched() returns true,
while polling for IO completion.
Earlier, polling task used to sleep, relying on interrupt to wake it up.
This made some IO take very long when interrupt-coalescing is enabled in
NVMe.

The next patch from Al will sort out the final differences, and we
should be all done.

a787739061 block, bfq: add requeue-request hook
Commit 'a6a252e64914 ("blk-mq-sched: decide how to handle flush rq via
RQF_FLUSH_SEQ")' makes all non-flush re-prepared requests for a device
be re-inserted into the active I/O scheduler for that device. As a
consequence, I/O schedulers may get the same request inserted again,
even several times, without a finish_request invoked on that request
before each re-insertion.

This fact is the cause of the failure reported in [1]. For an I/O
scheduler, every re-insertion of the same re-prepared request is
equivalent to the insertion of a new request. For schedulers like
mq-deadline or kyber, this fact causes no harm. In contrast, it
confuses a stateful scheduler like BFQ, which keeps state for an I/O
request, until the finish_request hook is invoked on the request. In
particular, BFQ may get stuck, waiting forever for the number of
request dispatches, of the same request, to be balanced by an equal
number of request completions (while there will be one completion for
that request). In this state, BFQ may refuse to serve I/O requests
from other bfq_queues. The hang reported in [1] then follows.

However, the above re-prepared requests undergo a requeue, thus the
requeue_request hook of the active elevator is invoked for these
requests, if set. This commit then addresses the above issue by
properly implementing the hook requeue_request in BFQ.

30abb3a67f block: Add should_fail_bio() for bpf error injection
The classic error injection mechanism, should_fail_request() does not
support use cases where more information is required (from the entire
struct bio, for example).

To that end, this patch introduces should_fail_bio(), which calls
should_fail_request() under the hood but provides a convenient
place for kprobes to hook into if they require the entire struct bio.
This patch also replaces some existing calls to should_fail_request()
with should_fail_bio() with no degradation in performance.

5235553d82 blk-wbt: account flush requests correctly
Mikulas reported a workload that saw bad performance, and figured
out what it was due to various other types of requests being
accounted as reads. Flush requests, for instance. Due to the
high latency of those, we heavily throttle the writes to keep
the latencies in balance. But they really should be accounted
as writes.

Fix this by checking the exact type of the request. If it's a
read, account as a read, if it's a write or a flush, account
as a write. Any other request we disregard. Previously everything
would have been mistakenly accounted as reads.

Decoding the assembly, the request claims to have 0xffff segments,
while nvme counts two. This turns out to be because we don't check
for a data carrying request on the mq scheduler path, and since
blk_phys_contig_segment() returns true for a non-data request,
we decrement the initial segment count of 0 and end up with
0xffff in the unsigned short.

There are a few issues here:

1) We should initialize the segment count for a discard to 1.
2) The discard merging is currently using the data limits for
   segments and sectors.

Fix this up by having attempt_merge() correctly identify the
request, and by initializing the segment count correctly
for discards.

This can only be triggered with mq-deadline on discard capable
devices right now, which isn't a common configuration.

86ff7c2a80 blk-mq: introduce BLK_STS_DEV_RESOURCE
This status is returned from driver to block layer if device related
resource is unavailable, but driver can guarantee that IO dispatch
will be triggered in future when the resource is available.

Convert some drivers to return BLK_STS_DEV_RESOURCE.  Also, if driver
returns BLK_STS_RESOURCE and SCHED_RESTART is set, rerun queue after
a delay (BLK_MQ_DELAY_QUEUE) to avoid IO stalls.  BLK_MQ_DELAY_QUEUE is
3 ms because both scsi-mq and nvmefc are using that magic value.

If a driver can make sure there is in-flight IO, it is safe to return
BLK_STS_DEV_RESOURCE because:

1) If all in-flight IOs complete before examining SCHED_RESTART in
blk_mq_dispatch_rq_list(), SCHED_RESTART must be cleared, so queue
is run immediately in this case by blk_mq_dispatch_rq_list();

2) if there is any in-flight IO after/when examining SCHED_RESTART
in blk_mq_dispatch_rq_list():
- if SCHED_RESTART isn't set, queue is run immediately as handled in 1)
- otherwise, this request will be dispatched after any in-flight IO is
  completed via blk_mq_sched_restart()

3) if SCHED_RESTART is set concurently in context because of
BLK_STS_RESOURCE, blk_mq_delay_run_hw_queue() will cover the above two
cases and make sure IO hang can be avoided.

One invariant is that queue will be rerun if SCHED_RESTART is set.

  Some of the obvious brainos found in process are fixed (e.g. POLLIN
  misspelled as POLL_IN). At that point the amount of sparse warnings is
  low and most of them are for genuine bugs - e.g. ->poll() instance
  deciding to return -EINVAL instead of a bitmap. I hadn't touched those
  in this series - it's large enough as it is.

  Another problem it has caught was eventpoll() ABI mess; select.c and
  eventpoll.c assumed that corresponding POLL### and EPOLL### were
  equal. That's true for some, but not all of them - EPOLL### are
  arch-independent, but POLL### are not.

  The last commit in this series separates userland POLL### values from
  the (now arch-independent) kernel-side ones, converting between them
  in the few places where they are copied to/from userland. AFAICS, this
  is the least disruptive fix preserving poll(2) ABI and making epoll()
  work on all architectures.

  As it is, it's simply broken on sparc - try to give it EPOLLWRNORM and
  it will trigger only on what would've triggered EPOLLWRBAND on other
  architectures. EPOLLWRBAND and EPOLLRDHUP, OTOH, are never triggered
  at all on sparc. With this patch they should work consistently on all
  architectures"

Attributes that only implement .seq_ops are read-only, any write to
them should be rejected. But currently kernel would crash when
writing to such debugfs entries, e.g.

chmod +w /sys/kernel/debug/block/<dev>/requeue_list
echo 0 > /sys/kernel/debug/block/<dev>/requeue_list
chmod -w /sys/kernel/debug/block/<dev>/requeue_list

Fix it by returning -EPERM in blk_mq_debugfs_write() when writing to
such attributes.

b889bf66d0 blk-throttle: track read and write request individually
In mixed read/write workload on SSD, write latency is much lower than
read. But now we only track and record read latency and then use it as
threshold base for both read and write io latency accounting. As a
result, write io latency will always be considered as good and
bad_bio_cnt is much smaller than 20% of bio_cnt. That is to mean, the
tg to be checked will be treated as idle most of the time and still let
others dispatch more ios, even it is truly running under low limit and
wants its low limit to be guaranteed, which is not we expected in fact.
So track read and write request individually, which can bring more
precise latency control for low limit idle detection.

Regular block device writes go through blkdev_write_iter(), which does
bdev_read_only(), while zeroout/discard/etc requests are never checked,
both userspace- and kernel-triggered.  Add a generic catch-all check to
generic_make_request_checks() to actually enforce ioctl(BLKROSET) and
set_disk_ro(), which is used by quite a few drivers for things like
snapshots, read-only backing files/images, etc.

8a8747dc01 block, bfq: limit sectors served with interactive weight raising
To maximise responsiveness, BFQ raises the weight, and performs device
idling, for bfq_queues associated with processes deemed as
interactive. In particular, weight raising has a maximum duration,
equal to the time needed to start a large application. If a
weight-raised process goes on doing I/O beyond this maximum duration,
it loses weight-raising.

This mechanism is evidently vulnerable to the following false
positives: I/O-bound applications that will go on doing I/O for much
longer than the duration of weight-raising. These applications have
basically no benefit from being weight-raised at the beginning of
their I/O. On the opposite end, while being weight-raised, these
applications
a) unjustly steal throughput to applications that may truly need
low latency;
b) make BFQ uselessly perform device idling; device idling results
in loss of device throughput with most flash-based storage, and may
increase latencies when used purposelessly.

This commit adds a countermeasure to reduce both the above
problems. To introduce this countermeasure, we provide the following
extra piece of information (full details in the comments added by this
commit). During the start-up of the large application used as a
reference to set the duration of weight-raising, involved processes
transfer at most ~110K sectors each. Accordingly, a process initially
deemed as interactive has no right to be weight-raised any longer,
once transferred 110K sectors or more.

Basing on this consideration, this commit early-ends weight-raising
for a bfq_queue if the latter happens to have received an amount of
service at least equal to 110K sectors (actually, a little bit more,
to keep a safety margin). I/O-bound applications that reach a high
throughput, such as file copy, get to this threshold much before the
allowed weight-raising period finishes. Thus this early ending of
weight-raising reduces the amount of time during which these
applications cause the problems described above.

a52a69ea89 block, bfq: limit tags for writes and async I/O
Asynchronous I/O can easily starve synchronous I/O (both sync reads
and sync writes), by consuming all request tags. Similarly, storms of
synchronous writes, such as those that sync(2) may trigger, can starve
synchronous reads. In their turn, these two problems may also cause
BFQ to loose control on latency for interactive and soft real-time
applications. For example, on a PLEXTOR PX-256M5S SSD, LibreOffice
Writer takes 0.6 seconds to start if the device is idle, but it takes
more than 45 seconds (!) if there are sequential writes in the
background.

This commit addresses this issue by limiting the maximum percentage of
tags that asynchronous I/O requests and synchronous write requests can
consume. In particular, this commit grants a higher threshold to
synchronous writes, to prevent the latter from being starved by
asynchronous I/O.

According to the above test, LibreOffice Writer now starts in about
1.2 seconds on average, regardless of the background workload, and
apart from some rare outlier. To check this improvement, run, e.g.,
sudo ./comm_startup_lat.sh bfq 5 5 seq 10 "lowriter --terminate_after_init"
for the comm_startup_lat benchmark in the S suite [1].

396eaf21ee blk-mq: improve DM's blk-mq IO merging via blk_insert_cloned_request feedback
blk_insert_cloned_request() is called in the fast path of a dm-rq driver
(e.g. blk-mq request-based DM mpath).  blk_insert_cloned_request() uses
blk_mq_request_bypass_insert() to directly append the request to the
blk-mq hctx->dispatch_list of the underlying queue.

1) This way isn't efficient enough because the hctx spinlock is always
used.

2) With blk_insert_cloned_request(), we completely bypass underlying
queue's elevator and depend on the upper-level dm-rq driver's elevator
to schedule IO.  But dm-rq currently can't get the underlying queue's
dispatch feedback at all.  Without knowing whether a request was issued
or not (e.g. due to underlying queue being busy) the dm-rq elevator will
not be able to provide effective IO merging (as a side-effect of dm-rq
currently blindly destaging a request from its elevator only to requeue
it after a delay, which kills any opportunity for merging).  This
obviously causes very bad sequential IO performance.

Fix this by updating blk_insert_cloned_request() to use
blk_mq_request_direct_issue().  blk_mq_request_direct_issue() allows a
request to be issued directly to the underlying queue and returns the
dispatch feedback (blk_status_t).  If blk_mq_request_direct_issue()
returns BLK_SYS_RESOURCE the dm-rq driver will now use DM_MAPIO_REQUEUE
to _not_ destage the request.  Whereby preserving the opportunity to
merge IO.

With this, request-based DM's blk-mq sequential IO performance is vastly
improved (as much as 3X in mpath/virtio-scsi testing).

7bed45954b blk-mq: make sure hctx->next_cpu is set correctly
When hctx->next_cpu is set from possible online CPUs, there is one
race in which hctx->next_cpu may be set as >= nr_cpu_ids, and finally
break workqueue.

The race can be triggered in the following two sitations:

1) when one CPU is becoming DEAD, blk_mq_hctx_notify_dead() is called
to dispatch requests from the DEAD cpu context, but at that
time, this DEAD CPU has been cleared from 'cpu_online_mask', so all
CPUs in hctx->cpumask may become offline, and cause hctx->next_cpu set
a bad value.

2) blk_mq_delay_run_hw_queue() is called from CPU B, and found the queue
should be run on the other CPU A, then CPU A may become offline at the
same time and all CPUs in hctx->cpumask become offline.

This patch deals with this issue by re-selecting next CPU, and making
sure it is set correctly.

During stress tests by syzkaller on the sg driver the block layer
infrequently returns EINVAL. Closer inspection shows the block
layer was trying to return ENOMEM (which is much more
understandable) but for some reason overroad that useful error.

Patch below does not show this (unchanged) line:
   ret =__blk_rq_map_user_iov(rq, map_data, &i, gfp_mask, copy);
That 'ret' was being overridden when that function failed.

fa70d2e2c4 block: allow gendisk's request_queue registration to be deferred
Since I can remember DM has forced the block layer to allow the
allocation and initialization of the request_queue to be distinct
operations.  Reason for this is block/genhd.c:add_disk() has requires
that the request_queue (and associated bdi) be tied to the gendisk
before add_disk() is called -- because add_disk() also deals with
exposing the request_queue via blk_register_queue().

DM's dynamic creation of arbitrary device types (and associated
request_queue types) requires the DM device's gendisk be available so
that DM table loads can establish a master/slave relationship with
subordinate devices that are referenced by loaded DM tables -- using
bd_link_disk_holder().  But until these DM tables, and their associated
subordinate devices, are known DM cannot know what type of request_queue
it needs -- nor what its queue_limits should be.

This chicken and egg scenario has created all manner of problems for DM
and, at times, the block layer.

Summary of changes:

- Add device_add_disk_no_queue_reg() and add_disk_no_queue_reg() variant
  that drivers may use to add a disk without also calling
  blk_register_queue().  Driver must call blk_register_queue() once its
  request_queue is fully initialized.

- Return early from blk_unregister_queue() if QUEUE_FLAG_REGISTERED
  is not set.  It won't be set if driver used add_disk_no_queue_reg()
  but driver encounters an error and must del_gendisk() before calling
  blk_register_queue().

- Export blk_register_queue().

These changes allow DM to use add_disk_no_queue_reg() to anchor its
gendisk as the "master" for master/slave relationships DM must establish
with subordinate devices referenced in DM tables that get loaded.  Once
all "slave" devices for a DM device are known its request_queue can be
properly initialized and then advertised via sysfs -- important
improvement being that no request_queue resource initialization
performed by blk_register_queue() is missed for DM devices anymore.

667257e8b2 block: properly protect the 'queue' kobj in blk_unregister_queue
The original commit e9a823fb34 (block: fix warning when I/O elevator
is changed as request_queue is being removed) is pretty conflated.
"conflated" because the resource being protected by q->sysfs_lock isn't
the queue_flags (it is the 'queue' kobj).

q->sysfs_lock serializes __elevator_change() (via elv_iosched_store)
from racing with blk_unregister_queue():
1) By holding q->sysfs_lock first, __elevator_change() can complete
before a racing blk_unregister_queue().
2) Conversely, __elevator_change() is testing for QUEUE_FLAG_REGISTERED
in case elv_iosched_store() loses the race with blk_unregister_queue(),
it needs a way to know the 'queue' kobj isn't there.

Expand the scope of blk_unregister_queue()'s q->sysfs_lock use so it is
held until after the 'queue' kobj is removed.

To do so blk_mq_unregister_dev() must not also take q->sysfs_lock.  So
rename __blk_mq_unregister_dev() to blk_mq_unregister_dev().

Also, blk_unregister_queue() should use q->queue_lock to protect against
any concurrent writes to q->queue_flags -- even though chances are the
queue is being cleaned up so no concurrent writes are likely.

now hctx can be mapped to possible CPUs, this patch applies this fact
to simplify queue mapping & schedule so that we don't need to handle
CPU hotplug for dealing with physical CPU plug & unplug. With this
simplication, we can work well on physical CPU plug & unplug, which
is a normal use case for VM at least.

Make sure we allocate blk_mq_ctx structures for all possible CPUs, and
set hctx->numa_node for possible CPUs which are mapped to this hctx. And
only choose the online CPUs for schedule.

0478fe6868 block: silently forbid sending any ioctl to a partition
After the first few months, the message has not led to many bug reports.
It's been almost five years now, and in practice the main source of
it seems to be MTIOCGET that someone is using to detect tape devices.
While we could whitelist it just like CDROM_GET_CAPABILITY, this patch
just removes the message altogether.

The patch also removes the "safe but not very useful" ioctl whitelist,
as suggested by Christoph.  I doubt anything is using most of those
ioctls _in general_, let alone on a partition.

7c3fb70f03 block: rearrange a few request fields for better cache layout
Move completion related items (like the call single data) near the
end of the struct, instead of mixing them in with the initial
queueing related fields.

Move queuelist below the bio structures. Then we have all
queueing related bits in the first cache line.

This yields a 1.5-2% increase in IOPS for a null_blk test, both for
sync and for high thread count access. Sync test goes form 975K to
992K, 32-thread case from 20.8M to 21.2M IOPS.

358f70da49 blk-mq: make blk_abort_request() trigger timeout path
With issue/complete and timeout paths now using the generation number
and state based synchronization, blk_abort_request() is the only one
which depends on REQ_ATOM_COMPLETE for arbitrating completion.

There's no reason for blk_abort_request() to be a completely separate
path.  This patch makes blk_abort_request() piggyback on the timeout
path instead of trying to terminate the request directly.

This removes the last dependency on REQ_ATOM_COMPLETE in blk-mq.

Note that this makes blk_abort_request() asynchronous - it initiates
abortion but the actual termination will happen after a short while,
even when the caller owns the request.  AFAICS, SCSI and ATA should be
fine with that and I think mtip32xx and dasd should be safe but not
completely sure.  It'd be great if people who know the drivers take a
look.

1d9bd5161b blk-mq: replace timeout synchronization with a RCU and generation based scheme
Currently, blk-mq timeout path synchronizes against the usual
issue/completion path using a complex scheme involving atomic
bitflags, REQ_ATOM_*, memory barriers and subtle memory coherence
rules.  Unfortunately, it contains quite a few holes.

There's a complex dancing around REQ_ATOM_STARTED and
REQ_ATOM_COMPLETE between issue/completion and timeout paths; however,
they don't have a synchronization point across request recycle
instances and it isn't clear what the barriers add.
blk_mq_check_expired() can easily read STARTED from N-2'th iteration,
deadline from N-1'th, blk_mark_rq_complete() against Nth instance.

In fact, it's pretty easy to make blk_mq_check_expired() terminate a
later instance of a request.  If we induce 5 sec delay before
time_after_eq() test in blk_mq_check_expired(), shorten the timeout to
2s, and issue back-to-back large IOs, blk-mq starts timing out
requests spuriously pretty quickly.  Nothing actually timed out.  It
just made the call on a recycle instance of a request and then
terminated a later instance long after the original instance finished.
The scenario isn't theoretical either.

This patch replaces the broken synchronization mechanism with a RCU
and generation number based one.

1. Each request has a u64 generation + state value, which can be
   updated only by the request owner.  Whenever a request becomes
   in-flight, the generation number gets bumped up too.  This provides
   the basis for the timeout path to distinguish different recycle
   instances of the request.

   Also, marking a request in-flight and setting its deadline are
   protected with a seqcount so that the timeout path can fetch both
   values coherently.

2. The timeout path fetches the generation, state and deadline.  If
   the verdict is timeout, it records the generation into a dedicated
   request abortion field and does RCU wait.

3. The completion path is also protected by RCU (from the previous
   patch) and checks whether the current generation number and state
   match the abortion field.  If so, it skips completion.

4. The timeout path, after RCU wait, scans requests again and
   terminates the ones whose generation and state still match the ones
   requested for abortion.

   By now, the timeout path knows that either the generation number
   and state changed if it lost the race or the completion will yield
   to it and can safely timeout the request.

While it's more lines of code, it's conceptually simpler, doesn't
depend on direct use of subtle memory ordering or coherence, and
hopefully doesn't terminate the wrong instance.

While this change makes REQ_ATOM_COMPLETE synchronization unnecessary
between issue/complete and timeout paths, REQ_ATOM_COMPLETE isn't
removed yet as it's still used in other places.  Future patches will
move all state tracking to the new mechanism and remove all bitops in
the hot paths.

Note that this patch adds a comment explaining a race condition in
BLK_EH_RESET_TIMER path.  The race has always been there and this
patch doesn't change it.  It's just documenting the existing race.

52257ffbfc block, bfq: put async queues for root bfq groups too
For each pair [device for which bfq is selected as I/O scheduler,
group in blkio/io], bfq maintains a corresponding bfq group. Each such
bfq group contains a set of async queues, with each async queue
created on demand, i.e., when some I/O request arrives for it.  On
creation, an async queue gets an extra reference, to make sure that
the queue is not freed as long as its bfq group exists.  Accordingly,
to allow the queue to be freed after the group exited, this extra
reference must released on group exit.

The above holds also for a bfq root group, i.e., for the bfq group
corresponding to the root blkio/io root for a given device. Yet, by
mistake, the references to the existing async queues of a root group
are not released when the latter exits. This causes a memory leak when
the instance of bfq for a given device exits. In a similar vein,
bfqg_stats_xfer_dead is not executed for a root group.

This commit fixes bfq_pd_offline so that the latter executes the above
missing operations for a root group too.

fb350e0ad9 blk-mq: fix race between updating nr_hw_queues and switching io sched
In both elevator_switch_mq() and blk_mq_update_nr_hw_queues(), sched tags
can be allocated, and q->nr_hw_queue is used, and race is inevitable, for
example: blk_mq_init_sched() may trigger use-after-free on hctx, which is
freed in blk_mq_realloc_hw_ctxs() when nr_hw_queues is decreased.

This patch fixes the race be holding q->sysfs_lock.

7d4901a90d blk-mq: avoid to map CPU into stale hw queue
blk_mq_pci_map_queues() may not map one CPU into any hw queue, but its
previous map isn't cleared yet, and may point to one stale hw queue
index.

This patch fixes the following issue by clearing the mapping table before
setting it up in blk_mq_pci_map_queues().

24f5a90f0d blk-mq: quiesce queue during switching io sched and updating nr_requests
Dispatch may still be in-progress after queue is frozen, so we have to
quiesce queue before switching IO scheduler and updating nr_requests.

Also when switching io schedulers, blk_mq_run_hw_queue() may still be
called somewhere(such as from nvme_reset_work()), and io scheduler's
per-hctx data may not be setup yet, so cause oops even inside
blk_mq_hctx_has_pending(), such as it can be run just between:

        ret = e->ops.mq.init_sched(q, e);
AND
        ret = e->ops.mq.init_hctx(hctx, i)

inside blk_mq_init_sched().

This reverts commit 7a148c2fcff8330(block: don't call blk_mq_quiesce_queue()
after queue is frozen) basically, and makes sure blk_mq_hctx_has_pending
won't be called if queue is quiesced.

c2856ae2f3 blk-mq: quiesce queue before freeing queue
After queue is frozen, dispatch still may happen, for example:

1) requests are submitted from several contexts
2) requests from all these contexts are inserted to queue, but may dispatch
to LLD in one of these paths, but other paths sill need to move on even all
these requests are completed(that means blk_mq_freeze_queue_wait() returns
at that time)
3) dispatch after queue freezing still moves on and causes use-after-free,
because request queue is freed

This patch quiesces queue after it is frozen, and makes sure all
in-progress dispatch are completed.

This patch fixes the following kernel crash when running heavy IOs vs.
deleting device:

ca11f209a4 mq-deadline: make it clear that __dd_dispatch_request() works on all hw queues
Don't pass in the hardware queue to __dd_dispatch_request(), since it
leads the reader to believe that we are returning a request for that
specific hardware queue. That's not how mq-deadline works, the state
for determining which request to serve next is shared across all
hardware queues for a device.
