Finally remove the two level TOC and build with -mcmodel=medium.
Unfortunately we can't build modules with -mcmodel=medium due to
the tricks the kernel module loader plays with percpu data:
# -mcmodel=medium breaks modules because it uses 32bit offsets from
# the TOC pointer to create pointers where possible. Pointers into the
# percpu data area are created by this method.
#
# The kernel module loader relocates the percpu data section from the
# original location (starting with 0xd...) to somewhere in the base
# kernel percpu data space (starting with 0xc...). We need a full
# 64bit relocation for this to work, hence -mcmodel=large.
On older kernels we fall back to the two level TOC (-mminimal-toc)

the PowerPC scom code still assigns it to a non-void field in
debugfs_blob_wrapper. The /right/ solution might be to change the
debugfs_blob_wrapper->data to also be const, but that is a bit
risky. Instead, cast the value to (void*). It is a bit ugly, but it
is the safest change until it can be investigated where
debugfs_blob_wrapper can be modified.

The original code uses 'Programming Interface' field to judge if PCIE is
EP or RC mode, however, some latest silicons do not support this
functionality. According to PCIE specification, 'Header Type' offset 0x0e

to judge PCIE mode. Because FSL PCI controller does not support
'Header Type', patch still uses 'Programming Interface' to identify PCI

From "Uwe Kleine-Knig" <u.kleine-koenig@pengutronix.de>:
this is the 2nd version of this series whose goal is to make struct
of_device_id.data const. Conceptually a driver must not modify the data
contained there so making it const is the right thing.
v1 of this series was sent with Message-id:
1342182734-321-1-git-send-email-y. Changes since then are:
- powerpc fixes
- several new consts that were found by Arnd that are possible after
patch 19.
Arnd suggested to take this series via arm-soc late for 3.6 in one go
because patch 19 depends on the former patches but is a precondition to
the latter and it fixes a few warnings. So getting it in via the
respective maintainer trees would need a much bigger coordination
effort. That means I prefer getting Acks over you taking the patch.
Vinod Koul already took
dmaengine: at_hdmac: add a few const qualifiers
that is in next-20120723 as 7fd63ccdad now. Vinod, I don't follow your
pull requests, but assuming you didn't let it already pull for 3.6 I
suggest you drop it from your queue and I just take your Ack.
This series was build tested for arm (all defconfigs) and powerpc (all
defconfigs and an allyesconfig) and grep didn't find more issues. As
before it introduces a warning in drivers/regulator/twl-regulator.c.
This driver does modify its .of_match_table when a device is bound which
doesn't fits the concept of independant devices. Arnd noticed another
new warning in drivers/scsi/qlogicpti.c that isn't that easy to resolve,
because the pointer to (now) const data is passed as first argument to
scsi_host_alloc. To fix that properly struct Scsi_Host.hostt needs to
get a const, too. Alternatively I could introduce a cast removing the
const, but I don't like that.

We unified the Freescale pci/pcie initialization by changing the fsl_pci
to a platform driver. In previous PCI code architecture the initialization
routine is called at board_setup_arch stage. Now the initialization is done
in probe function which is architectural better. Also It's convenient for
adding PM support for PCI controller in later patch.
Now we registered pci controllers as platform devices. So we combine two
initialization code as one platform driver.

All SOC device error interrupts are muxed and delivered to the core
as a single MPIC error interrupt. Currently all the device drivers
requiring access to device errors have to register for the MPIC error
interrupt as a shared interrupt.
With this patch we add interrupt demuxing capability in the mpic driver,
allowing device drivers to register for their individual error interrupts.
This is achieved by handling error interrupts in a cascaded fashion.
MPIC error interrupt is handled by the "error_int_handler", which
subsequently demuxes it using the EISR and delivers it to the respective
drivers.
The error interrupt capability is dependent on the MPIC EIMR register,
which was introduced in FSL MPIC version 4.1 (P4080 rev2). So, error
interrupt demuxing capability is dependent on the MPIC version and can
be used for versions >= 4.1.

Remove the dependency on PCI initialization for SWIOTLB initialization.
So that PCI can be initialized at proper time.
SWIOTLB is partly determined by PCI inbound/outbound map which is assigned
in PCI initialization. But swiotlb_init() should be done at the stage of
mem_init() which is much earlier than PCI initialization. So we reserve the
memory for SWIOTLB first and free it if not necessary.

We have been observing hangs, both of KVM guest vcpu tasks and more
generally, where a process that is woken doesn't properly wake up and
continue to run, but instead sticks in TASK_WAKING state. This
happens because the update of rq->wake_list in ttwu_queue_remote()
is not ordered with the update of ipi_message in
smp_muxed_ipi_message_pass(), and the reading of rq->wake_list in
scheduler_ipi() is not ordered with the reading of ipi_message in
smp_ipi_demux(). Thus it is possible for the IPI receiver not to see
the updated rq->wake_list and therefore conclude that there is nothing
for it to do.
In order to make sure that anything done before smp_send_reschedule()
is ordered before anything done in the resulting call to scheduler_ipi(),
this adds barriers in smp_muxed_message_pass() and smp_ipi_demux().
The barrier in smp_muxed_message_pass() is a full barrier to ensure that
there is a full ordering between the smp_send_reschedule() caller and
scheduler_ipi(). In smp_ipi_demux(), we use xchg() rather than
xchg_local() because xchg() includes release and acquire barriers.
Using xchg() rather than xchg_local() makes sense given that
ipi_message is not just accessed locally.
This moves the barrier between setting the message and calling the
cause_ipi() function into the individual cause_ipi implementations.
Most of them -- those that used outb, out_8 or similar -- already had
a full barrier because out_8 etc. include a sync before the MMIO
store. This adds an explicit barrier in the two remaining cases.
These changes made no measurable difference to the speed of IPIs as
measured using a simple ping-pong latency test across two CPUs on
different cores of a POWER7 machine.
The analysis of the reason why processes were not waking up properly
is due to Milton Miller.

Round of refactoring and enhancements to irq_domain infrastructure. This
series starts the process of simplifying irqdomain. The ultimate goal is
to merge LEGACY, LINEAR and TREE mappings into a single system, but had
to back off from that after some last minute bugs. Instead it mainly
reorganizes the code and ensures that the reverse map gets populated
when the irq is mapped instead of the first time it is looked up.
Merging of the irq_domain types is deferred to v3.7
In other news, this series adds helpers for creating static mappings on
a linear or tree mapping.

"Round of refactoring and enhancements to irq_domain infrastructure.
This series starts the process of simplifying irqdomain. The ultimate
goal is to merge LEGACY, LINEAR and TREE mappings into a single
system, but had to back off from that after some last minute bugs.
Instead it mainly reorganizes the code and ensures that the reverse
map gets populated when the irq is mapped instead of the first time it
is looked up.
Merging of the irq_domain types is deferred to v3.7
In other news, this series adds helpers for creating static mappings
on a linear or tree mapping."

The sram_offset parameter represents a physical address and should be of

The current mpic code creates a linear revmap just big enough for all
the sources, which happens to miss the IPIs and timers on some machines.
This will in turn break when the irqdomain code loses the fallback of
doing a linear search when the revmap fails (and really slows down IPIs
otherwise).
This happens for example on the U4 based Apple machines such as the
dual core PowerMac G5s.

As an alternative incremental starting point to Jia Hongtao's patchset,
get the FSL PCI init out of the board files, but do not yet convert to a
platform driver.
Rather than having each board supply a magic register offset for
determining the "primary" bus, we look for which PCI host bridge
contains an ISA node within its subtree. If there is no ISA node,
normally that would mean there is no primary bus, but until certain
bugs are fixed we arbitrarily designate a primary in this case.
Conversion to a platform driver and related improvements can happen
after this, as the ordering issues are sorted out.

The switch from using irq_map to irq_alloc_desc*() for managing irq
number allocations introduced new bugs in some of the powerpc
interrupt code. Several functions rely on the value of NR_IRQS to
determine the maximum irq number that could get allocated. However,
with sparse_irq and using irq_alloc_desc*() the maximum possible irq
number is now specified with 'nr_irqs' which may be a number larger
than NR_IRQS. This has caused breakage on powermac when
CONFIG_NR_IRQS is set to 32.
This patch removes most of the direct references to NR_IRQS in the
powerpc code and replaces them with either a nr_irqs reference or by
using the common for_each_irq_desc() macro. The powerpc-specific
for_each_irq() macro is removed at the same time.
Also, the Cell axon_msi driver is refactored to remove the global
build assumption on the size of NR_IRQS and instead add a limit to the
maximum irq number when calling irq_domain_add_nomap().

The mpc8xx driver uses a reference to NR_IRQS that is buggy. It uses
NR_IRQs for the array size of the ppc_cached_irq_mask bitmap, but
NR_IRQs could be smaller than the number of hardware irqs that
ppc_cached_irq_mask tracks.
Also, while fixing that problem, it became apparent that the interrupt
controller only supports 32 interrupt numbers, but it is written as if
it supports multiple register banks which is more complicated.
This patch pulls out the buggy reference to NR_IRQs and fixes the size
of the ppc_cached_irq_mask to match the number of HW irqs. It also
drops the now-unnecessary code since ppc_cached_irq_mask is no longer
an array.

Pull build fixes for less mainstream architectures from Paul Gortmaker:
"These are fixes for frv(1), blackfin(2), powerpc(1) and xtensa(4).
Fortunately the touches are nearly all specific to files just used by
the arch in question. The two touches to shared/common files
[kernel/irq/debug.h and drivers/pci/Makefile] are trivial to assess as
no risk to anyone.
Half of them relate to xtensa directly. It was only when I fixed the
last xtensa issue that I realized that the arch has been broken for a
significant time, and isn't a specific v3.4 regression. So if you
wanted, we could leave xtensa lying bleeding in the street for a
couple more weeks and queue those for 3.5. But given they are no risk
to anyone outside of xtensa, I figured to just leave them in.
If you are OK with taking the xtensa fixes, then please pull to get:
- one last implicit include uncovered by system.h that is in a file
specific to just one powerpc defconfig. (I'd sync'd with BenH).
- fix an oversight in the PCI makefile where shared code wasn't being
compiled for ARCH=frv
- fix a missing include for GPIO in blackfin framebuffer.
- audit and tag endif in blackfin ezkit board file, in order to find
and fix the misplaced endif masking a block of code.
- fix irq/debug.h choice of temporary macro names to be more internal
so they don't conflict with names used by xtensa.
- fix a reference to an undeclared local var in xtensa's signal.c
- fix an implicit bug.h usage in xtensa's asm/io.h uncovered by my
removing bug.h from kernel.h
- fix xtensa to properly indicate it is using asm-generic/hardirq.h
in order to resolve the link error - undefined ack_bad_irq
The xtensa still fails final link as my latest binutils does something
evil when ld forward-relocates unlikely() blocks, but in theory people
who have older/valid toolchains could now use the thing."

Pull powerpc merge from Benjamin Herrenschmidt:
"Here's the powerpc batch for this merge window. It is going to be a
bit more nasty than usual as in touching things outside of
arch/powerpc mostly due to the big iSeriesectomy :-) We finally got
rid of the bugger (legacy iSeries support) which was a PITA to
maintain and that nobody really used anymore.
Here are some of the highlights:
- Legacy iSeries is gone. Thanks Stephen ! There's still some bits
and pieces remaining if you do a grep -ir series arch/powerpc but
they are harmless and will be removed in the next few weeks
hopefully.
- The 'fadump' functionality (Firmware Assisted Dump) replaces the
previous (equivalent) "pHyp assisted dump"... it's a rewrite of a
mechanism to get the hypervisor to do crash dumps on pSeries, the
new implementation hopefully being much more reliable. Thanks
Mahesh Salgaonkar.
- The "EEH" code (pSeries PCI error handling & recovery) got a big
spring cleaning, motivated by the need to be able to implement a
new backend for it on top of some new different type of firwmare.
The work isn't complete yet, but a good chunk of the cleanups is
there. Note that this adds a field to struct device_node which is
not very nice and which Grant objects to. I will have a patch soon
that moves that to a powerpc private data structure (hopefully
before rc1) and we'll improve things further later on (hopefully
getting rid of the need for that pointer completely). Thanks Gavin
Shan.
- I dug into our exception & interrupt handling code to improve the
way we do lazy interrupt handling (and make it work properly with
"edge" triggered interrupt sources), and while at it found & fixed
a wagon of issues in those areas, including adding support for page
fault retry & fatal signals on page faults.
- Your usual random batch of small fixes & updates, including a bunch
of new embedded boards, both Freescale and APM based ones, etc..."
I fixed up some conflicts with the generalized irq-domain changes from

This branch takes the PowerPC irq_host infrastructure (reverse mapping
from Linux IRQ numbers to hardware irq numbering), generalizes it,
renames it to irq_domain, and makes it available to all architectures.
Originally the plan has been to create an all-new irq_domain
implementation which addresses some of the powerpc shortcomings such
as not handling 1:1 mappings well, but doing that proved to be far
more difficult and invasive than generalizing the working code and
refactoring it in-place. So, this branch rips out the 'new'
irq_domain and replaces it with the modified powerpc version (in a
fully bisectable way of course). It converts all users over to the
new API and makes irq_domain selectable on any architecture.
No architecture is forced to enable irq_domain, but the infrastructure
is required for doing OpenFirmware style irq translations. It will
even work on SPARC even though SPARC has it's own mechanism for
translating irqs at boot time. MIPS, microblaze, embedded x86 and c6x
are converted too.
The resulting irq_domain code is probably still too verbose and can be
optimized more, but that can be done incrementally and is a task for
follow-on patches.

Some MPIC implementations contain one or more blocks of message registers
that are used to send messages between cores via IPIs. A simple API has
been added to access (get/put, read, write, etc ...) these message registers.
The available message registers are initially discovered via nodes in the
device tree. A separate commit contains a binding for the message register
nodes.

When using a multi-ISU MPIC, we can interrupts up to
isu_size * MPIC_MAX_ISU, not just isu_size, so allocate
the right size reverse map.
Without this, the code will constantly fallback to
a linear search.
Signed-off-by: Benjamin Herrenschmidt <benh@kernel.crashing.org>
There are two separate flags controlling whether or not the MPIC is
reset during initialization, which is completely unnecessary, and only
one of them can be specified in the device tree.
Also, most platforms in-tree right now do actually want to reset the
MPIC during initialization anyways, which means lots of duplicate code
passing the MPIC_WANTS_RESET flag.
Fix all of the callers which currently do not pass the MPIC_WANTS_RESET
flag to pass the MPIC_NO_RESET flag, then remove the MPIC_WANTS_RESET
flag and make the code reset the MPIC by default.

The FreeScale PowerQUICC-III-compatible (mpc85xx/mpc86xx) MPICs do not
correctly report the number of hardware interrupt sources, so software
needs to override the detected value with "256".
To avoid needing to write custom board-specific code to detect that
scenario, allow it to be easily overridden in the device-tree.

The mpic->irq_count variable is only used as a software error-checking
limit to determine whether or not an IRQ number is valid. In board code
which does not manually specify an IRQ count to mpic_alloc(), i.e. 0, it
is automatically detected from the number of ISUs and the ISU size.
In practice, all hardware ends up with irq_count == num_sources, so all
of the runtime checks on mpic->irq_count should just check the value of
mpic->num_sources instead.
When platform hardware does not correctly report the number of IRQs,
which only happens on the MPC85xx/MPC86xx, the MPIC_BROKEN_FRR_NIRQS
flag is used to override the detected value of num_sources with the
manual irq_count parameter. Since there's no need to manually specify
the number of IRQs except in this case, the extra flag can be eliminated
and the test changed to "irq_count != 0".

The Freescale MPIC (and perhaps others in the future) is incapable of
routing non-IPI interrupts to more than once CPU at a time. Currently
all of the Freescale boards msut pass the MPIC_SINGLE_DEST_CPU flag to
mpic_alloc(), but that information should really be present in the
device-tree.
Older board code can't rely on the device-tree having the property set,
but newer platforms won't need it manually specified in the code.

The mpic_alloc() function takes a "flags" parameter and assigns it into
the mpic->flags variable fairly early on, but several later pieces of
code detect various device-tree properties and save them into the
"mpic->flags" variable (EG: "big-endian" => MPIC_BIG_ENDIAN).
Unfortunately, a number of codepaths (including several which test the
flag MPIC_BIG_ENDIAN!) test "flags" instead of "mpic->flags", and get
wrong answers as a result.
Consolidate the device-tree flag tests early in mpic_alloc() and change
all of the checks after "mpic->flags" is init'ed to use "mpic->flags".

Add support for a legacy mapping where irq = (hwirq - first_hwirq + first_irq)
so that a controller driver can allocate a fixed range of irq_descs and use
a simple calculation to translate back and forth between linux and hw irq
numbers. This is needed to use an irq_domain with many of the ARM interrupt
controller drivers that manage their own irq_desc allocations. Ultimately
the goal is to migrate those drivers to use the linear revmap, but doing it
this way allows each driver to be converted separately which makes the
migration path easier.
This patch generalizes the IRQ_DOMAIN_MAP_LEGACY method to use
(first_irq-first_hwirq) as the offset between hwirq and linux irq number,
and adds checks to make sure that the hwirq number does not exceed range
assigned to the controller.

Each revmap type has different arguments for setting up the revmap.
This patch splits up the generator functions so that each revmap type
can do its own setup and the user doesn't need to keep track of how
each revmap type handles the arguments.
This patch also adds a host_data argument to the generators. There are
cases where the host_data pointer will be needed before the function returns.
ie. the legacy map calls the .map callback for each irq before returning.
v2: - Add void *host_data argument to irq_domain_add_*() functions

broke the resource fixup for FSL boards.

This patch drops the powerpc-specific irq_host structures and uses the common
irq_domain strucutres defined in linux/irqdomain.h. It also fixes all
the users to use the new structure names.
Renaming irq_host to irq_domain has been discussed for a long time, and this
patch is a step in the process of generalizing the powerpc virq code to be
usable by all architecture.
An astute reader will notice that this patch actually removes the irq_host
structure instead of renaming it. This is because the irq_domain structure
already exists in include/linux/irqdomain.h and has the needed data members.

Add support for vmpic-msi nodes to the fsl_msi driver. The MSI is

There is an issue on FSL-BookE 64-bit devices (P5020) in which PCIe
devices that are capable of doing 64-bit DMAs (like an Intel e1000) do
not function and crash the kernel if we have >4G of memory in the system.
The reason is that the existing code only sets up one inbound window for
access to system memory across PCIe. That window is limited to a 32-bit
address space. So on systems we'll end up utilizing SWIOTLB for dma
mappings. However SWIOTLB dma ops implement dma_alloc_coherent() as
dma_direct_alloc_coherent(). Thus we can end up with dma addresses that
are not accessible because of the inbound window limitation.
We could possibly set the SWIOTLB alloc_coherent op to
swiotlb_alloc_coherent() however that does not address the issue since
the swiotlb_alloc_coherent() will behave almost identical to
dma_direct_alloc_coherent() since the devices coherent_dma_mask will be
greater than any address allocated by swiotlb_alloc_coherent() and thus
we'll never bounce buffer it into a range that would be dma-able.
The easiest and best solution is to just make it so that a 64-bit
capable device is able to DMA to any internal system address.
We accomplish this by opening up a second inbound window that maps all
of memory above the internal SoC address width so we can set it up to
access all of the internal SoC address space if needed.
We than fixup the dma_ops and dma_offset for PCIe devices with a dma
mask greater than the maximum internal SoC address.

This moves the 'cpu sysdev_class' over to a regular 'cpu' subsystem
and converts the devices to regular devices. The sysdev drivers are
implemented as subsystem interfaces now.
After all sysdev classes are ported to regular driver core entities, the
sysdev implementation will be entirely removed from the kernel.
Userspace relies on events and generic sysfs subsystem infrastructure
from sysdev devices, which are made available with this conversion.

The Cell and PowerMac platforms use virtually identical cascaded-IRQ
setup code, so just merge it into the core. Ideally this code would
trigger automatically when an MPIC device-node specifies an "interrupts"
property, perhaps even enabling MPIC_SECONDARY along the way.
Unfortunately, Benjamin Herrenschmidt has had bad experiences in the
past with the quality of Apple PowerMac device-trees, so to be safe we
will only try to parse out an IRQ if the MPIC_SECONDARY flag is set by
the caller.

On Freescale parts with multiple MSI controllers, the controllers are
combined into one "pool" of interrupts. Whenever a device requests an MSI
interrupt, the next available interrupt from the pool is selected,
regardless of which MSI controller the interrupt is from. This works
because each PCI bus has an ATMU to all of CCSR, so any PCI device can
access any MSI interrupt register.
The fsl,msi property is used to specify that a given PCI bus should only
use a specific MSI device. This is necessary, for example, with the
Freescale hypervisor, because the MSI devices are assigned to specific
partitions.
Ideally, we'd like to be able to assign MSI devices to PCI busses within
the MSI or PCI layers. However, there does not appear to be a mechanism
to do that. Whenever the MSI layer wants to allocate an MSI interrupt to
a PCI device, it just calls arch_setup_msi_irqs(). It would be nice if we
could register an MSI device with a specific PCI bus.
So instead we remember the phandles of each MSI device, and we use that to
limit our search for an available interrupt. Whenever we are asked to
allocate a new interrupt for a PCI device, we check the fsl,msi property
of the PCI bus for that device. If it exists, then as we are looping over
all MSI devices, we skip the ones that don't have a matching phandle.

The "struct rio_mport" contains a member of master port I/O memory
resource structure "struct resource iores". This resource will be read
from device tree and be used for rapidio R/W transaction memory space.
Rapidio requests the port I/O memory resource under the root resource
"iomem_resource".
struct rio_mport *port;
port = kzalloc(sizeof(struct rio_mport), GFP_KERNEL);
request_resource(&iomem_resource, &port->iores);
When port failed to initialize, allocated "rio_mport" structure memory
will be freed, and the port I/O memory resource structure pointer
"&port->iores" will be invalid. If other requests resource under
"iomem_resource", "&port->iores" node may be operated in the child
resources list and this will cause the system to crash.
So the requested port I/O memory resource should be released before
freeing allocated "rio_mport" structure.

Since we are heading to fix things up and clean module.h from the

There is one place in the MPIC driver that assumes that the cores are numbered
from 0 to n-1. However, this is not true if the CPUs are not numbered
sequentially. This can happen on a eight-core SOC where cores two and three
are removed in the device tree. So instead of blindly looping, we iterate
over the discovered CPUs and use the SMP ID as the index.
This means that we no longer ask the MPIC how many CPUs there are, so
we also delete mpic->num_cpus.
We also catch if the number of CPUs in the SOC exceeds the number that the
MPIC supports. This should never happen, of course, but it's good to be
sure.

This lifts the restriction that book3s_hv guests can only run one
hardware thread per core, and allows them to use up to 4 threads
per core on POWER7. The host still has to run single-threaded.
This capability is advertised to qemu through a new KVM_CAP_PPC_SMT
capability. The return value of the ioctl querying this capability
is the number of vcpus per virtual CPU core (vcore), currently 4.
To use this, the host kernel should be booted with all threads
active, and then all the secondary threads should be offlined.
This will put the secondary threads into nap mode. KVM will then
wake them from nap mode and use them for running guest code (while
they are still offline). To wake the secondary threads, we send
them an IPI using a new xics_wake_cpu() function, implemented in
arch/powerpc/sysdev/xics/icp-native.c. In other words, at this stage
we assume that the platform has a XICS interrupt controller and
we are using icp-native.c to drive it. Since the woken thread will
need to acknowledge and clear the IPI, we also export the base
physical address of the XICS registers using kvmppc_set_xics_phys()
for use in the low-level KVM book3s code.
When a vcpu is created, it is assigned to a virtual CPU core.
The vcore number is obtained by dividing the vcpu number by the
number of threads per core in the host. This number is exported
to userspace via the KVM_CAP_PPC_SMT capability. If qemu wishes
to run the guest in single-threaded mode, it should make all vcpu
numbers be multiples of the number of threads per core.
We distinguish three states of a vcpu: runnable (i.e., ready to execute
the guest), blocked (that is, idle), and busy in host. We currently
implement a policy that the vcore can run only when all its threads
are runnable or blocked. This way, if a vcpu needs to execute elsewhere
in the kernel or in qemu, it can do so without being starved of CPU
by the other vcpus.
When a vcore starts to run, it executes in the context of one of the
vcpu threads. The other vcpu threads all go to sleep and stay asleep
until something happens requiring the vcpu thread to return to qemu,
or to wake up to run the vcore (this can happen when another vcpu
thread goes from busy in host state to blocked).
It can happen that a vcpu goes from blocked to runnable state (e.g.
because of an interrupt), and the vcore it belongs to is already
running. In that case it can start to run immediately as long as
the none of the vcpus in the vcore have started to exit the guest.
We send the next free thread in the vcore an IPI to get it to start
to execute the guest. It synchronizes with the other threads via
the vcore->entry_exit_count field to make sure that it doesn't go
into the guest if the other vcpus are exiting by the time that it
is ready to actually enter the guest.
Note that there is no fixed relationship between the hardware thread
number and the vcpu number. Hardware threads are assigned to vcpus
as they become runnable, so we will always use the lower-numbered
hardware threads in preference to higher-numbered threads if not all
the vcpus in the vcore are runnable, regardless of which vcpus are
runnable.

The Freescale ePAPR reference hypervisor provides interrupt controller
services via a hypercall interface, instead of emulating the MPIC
controller. This is called the VMPIC.
The ePAPR "virtual interrupt controller" provides interrupt controller
services for external interrupts. External interrupts received by a
partition can come from two sources:
- Hardware interrupts - hardware interrupts come from external
interrupt lines or on-chip I/O devices.
- Virtual interrupts - virtual interrupts are generated by the hypervisor
as part of some hypervisor service or hypervisor-created virtual device.
Both types of interrupts are processed using the same programming model and
same set of hypercalls.

Some irq_host implementations are using virq_to_host to check if
they are the irq_host for a virtual irq. To allow us to make space
versus time tradeoffs, replace this usage with an assertive
virq_is_host that confirms or denies the irq is associated with the
given irq_host.

handler_data should be reserved for flow handlers on the dependent
irq, not consumed by the parent irq code that is part of the irq_chip
code. The msi_data pointer was already set in msidesc->irqhost->hostdata
and being copied to irq_data->chipdata in the msidesc->irqhost->map()
method called via create_irq_mapping, so we can obtain the pointer
from there and free the instance it in teardown_msi_irqs.
Also remove the unnecessary cast of irq_get_handler_data in the
cascade handler, which is the demux flow handler of the parent
msi interrupt. (This is the expected usage for handler_data).

The msi platform device driver was abusing dev.platform_data for its
platform_driver_data. Use the correct pointer for storage.
Platform_data is supposed to be for platforms to communicate to drivers
parameters that are not otherwise discoverable. Its lifetime matches
the platform_device not the platform device driver. It is generally
not needed for drivers that only support systems with device trees.

It was never called because the host is always IRQ_HOST_MAP_LEGACY.
And what it purported to do was mask the interrupt (which will already
have happend if we shutdown the interrupt), then synchronise_irq and
clear the chip pointer, both of which will have been be done by the
caller were we to call unmap on a legacy irq.

Compile the new smp ipi mux and demux code only if a platform
will make use of it. The new config is selected as required.
The new cause_ipi smp op is only available conditionally to point out
configs where the select is required; this makes setting the op an
immediate fail instead of a deferred unresolved symbol at link.
This also creates a new config for power surge powermac upgrade support
that can be disabled in expert mode but is default on.
I also removed the depends / default y on CONFIG_XICS since it is selected
by PSERIES.

Consolidate the mux and demux of ipi messages into smp.c and call
a new smp_ops callback to actually trigger the ipi.
The powerpc architecture code is optimised for having 4 distinct
ipi triggers, which are mapped to 4 distinct messages (ipi many, ipi
single, scheduler ipi, and enter debugger). However, several interrupt
controllers only provide a single software triggered interrupt that
can be delivered to each cpu. To resolve this limitation, each smp_ops
implementation created a per-cpu variable that is manipulated with atomic
bitops. Since these lines will be contended they are optimialy marked as
shared_aligned and take a full cache line for each cpu. Distro kernels
may have 2 or 3 of these in their config, each taking per-cpu space
even though at most one will be in use.
This consolidation removes smp_message_recv and replaces the single call
actions cases with direct calls from the common message recognition loop.
The complicated debugger ipi case with its muxed crash handling code is
moved to debug_ipi_action which is now called from the demux code (instead
of the multi-message action calling smp_message_recv).
I put a call to reschedule_action to increase the likelyhood of correctly
merging the anticipated scheduler_ipi() hook coming from the scheduler
tree; that single required call can be inlined later.
The actual message decode is a copy of the old pseries xics code with its
memory barriers and cache line spacing, augmented with a per-cpu unsigned
long based on the book-e doorbell code. The optional data is set via a
callback from the implementation and is passed to the new cause-ipi hook
along with the logical cpu number. While currently only the doorbell
implemntation uses this data it should be almost zero cost to retrieve and
pass it -- it adds a single register load for the argument from the same
cache line to which we just completed a store and the register is dead
on return from the call. I extended the data element from unsigned int
to unsigned long in case some other code wanted to associate a pointer.
The doorbell check_self is replaced by a call to smp_muxed_ipi_resend,
conditioned on the CPU_DBELL feature. The ifdef guard could be relaxed
to CONFIG_SMP but I left it with BOOKE for now.
Also, the doorbell interrupt vector for book-e was not calling irq_enter
and irq_exit, which throws off cpu accounting and causes code to not
realize it is running in interrupt context. Add the missing calls.

Commit b826291c, "drivercore/dt: add a match table pointer to struct
device" added an of_match pointer to struct device to cache the
of_match_table entry discovered at driver match time. This was unsafe
because matching is not an atomic operation with probing a driver. If
two or more drivers are attempted to be matched to a driver at the
same time, then the cached matching entry pointer could get
overwritten.
This patch reverts the of_match cache pointer and reworks all users to
call of_match_device() directly instead.

This is a significant rework of the XICS driver, too significant to
conveniently break it up into a series of smaller patches to be honest.
The driver is moved to a more generic location to allow new platforms
to use it, and is broken up into separate ICP and ICS "backends". For
now we have the native and "hypervisor" ICP backends and one common
RTAS ICS backend.
The driver supports one ICP backend instanciation, and many ICS ones,
in order to accomodate future platforms with multiple possibly different
interrupt "sources" mechanisms.

This property, defined in the Open PIC binding, tells the kernel not to use
the reset bit in the global configuration register. Additionally, its
presence mandates that only sources which are actually used (i.e. appear in
the device tree) should have their VECPRI bits initialized.
Although, "pic-no-reset" can be used for the same use cases that
"protected-sources" is covering, the "protected-sources" implementation was
left completely intact. This is a more pragmatic approach as there are
already several existing systems which use protected sources. If
"pic-no-reset" *and* "protected-sources" are both used, however, then
"pic-no-reset" takes precedence in terms of the init behavior and the
sanity checks done by protected sources will still take place.
