b2c3e38a54 ARM: redo TTBR setup code for LPAE
Re-engineer the LPAE TTBR setup code.  Rather than passing some shifted
address in order to fit in a CPU register, pass either a full physical
address (in the case of r4, r5 for TTBR0) or a PFN (for TTBR1).

This removes the ARCH_PGD_SHIFT hack, and the last dangerous user of
cpu_set_ttbr() in the secondary CPU startup code path (which was there
to re-set TTBR1 to the appropriate high physical address space on
Keystone2.)

c0b759d87e ARM: keystone2: rename init_meminfo to pv_fixup
The init_meminfo() method is not about initialising meminfo - it's about
fixing up the physical to virtual translation so that we use a different
physical address space, possibly above the 4GB physical address space.
Therefore, the name "init_meminfo()" is confusing.

Rename it to pv_fixup() instead.

c8ca2b4b29 ARM: keystone2: move update of the phys-to-virt constants into generic code
Make the init_meminfo function return the offset to be applied to the
phys-to-virt translation constants.  This allows us to move the update
into generic code, along with the requirements for this update.

This avoids platforms having to know the details of the phys-to-virt
translation support.

b01aec9b2c EDAC: Cleanup atomic_scrub mess
So first of all, this atomic_scrub() function's naming is bad. It looks
like an atomic_t helper. Change it to edac_atomic_scrub().

The bigger problem is that this function is arch-specific and every new
arch which doesn't necessarily need that functionality still needs to
define it, otherwise EDAC doesn't compile.

So instead of doing that and including arch-specific headers, have each
arch define an EDAC_ATOMIC_SCRUB symbol which can be used in edac_mc.c
for ifdeffery. Much cleaner.

And we already are doing this with another symbol - EDAC_SUPPORT. This
is also much cleaner than having CONFIG_EDAC enumerate all the arches
which need/have EDAC support and drivers.

This way I can kill the useless edac.h header in tile too.

5882bfef63 arm,arm64/xen: move Xen initialization earlier
Currently, Xen is initialized/discovered in an initcall. This doesn't
allow us to support earlyprintk or choosing the preferred console when
running on Xen.

The current function xen_guest_init is now split in 2 parts:
    - xen_early_init: Check if there is a Xen node in the device tree
    and setup domain type
    - xen_guest_init: Retrieve the information from the device node and
    initialize Xen (grant table, shared page...)

The former is called in setup_arch, while the latter is an initcall.

cc88116da0 arm: perf: treat PMUs as CPU affine
In multi-cluster systems, the PMUs can be different across clusters, and
so our logical PMU may not be able to schedule events on all CPUs.

This patch adds a cpumask to encode which CPUs a PMU driver supports
controlling events for, and limits the driver to scheduling events on
those CPUs, and enabling and disabling the physical PMUs on those CPUs.
The cpumask is built based on the interrupt-affinity property, and in
the absence of such a property a homogenous system is assumed.

b3eac0265b arm: perf: Fix callchain parse error with kernel tracepoint events
For ARM, when tracing with tracepoint events, the IP and cpsr are set
to 0, preventing the perf code parsing the callchain and resolving the
symbols correctly.

The fix is to implement perf_arch_fetch_caller_regs for ARM, which fills
several necessary registers used for callchain unwinding, including pc,sp,
fp and cpsr.

7d7d7a413c ARM: vfp: Add vfp_disable for problematic platforms
Some platforms might not be able to fully utilize VFP when e.g: one CPU
out of two in a SMP complex lacks a VFP unit. Adding code to migrate
task to the CPU which has a VFP unit would be cumbersome and not
performant, instead, just add the ability to disable VFP.

The !CONFIG_SMP implementation of futex_atomic_op_inuser() seems to rely
on disabled preemption to guarantee mutual exclusion.

From commit e589ed23dd ("[ARM] 5218/1: arm: improved futex support"):

    "For UP it's enough to disable preemption to ensure mutual exclusion..."

From the code itself:

    "!SMP, we can work around lack of atomic ops by disabling preemption"

Let's make this explicit, to prepare for pagefault_disable() not
touching preemption anymore.

14327c6628 ARM: replace BSYM() with badr assembly macro
BSYM() was invented to allow us to work around a problem with the
assembler, where local symbols resolved by the assembler for the 'adr'
instruction did not take account of their ISA.

Since we don't want BSYM() used elsewhere, replace BSYM() with a new
macro 'badr', which is like the 'adr' pseudo-op, but with the BSYM()
mechanics integrated into it.  This ensures that the BSYM()-ification
is only used in conjunction with 'adr'.

7d485f647c ARM: 8220/1: allow modules outside of bl range
Loading modules far away from the kernel in memory is problematic
because the 'bl' instruction only has limited reach, and modules are not
built with PLTs. Instead of using the -mlong-calls option (which affects
all compiler emitted bl instructions, but not the ones in assembler),
this patch allocates some additional space at module load time, and
populates it with PLT like veneers when encountering relocations that
are out of range.

This should work with all relocations against symbols exported by the
kernel, including those resulting from GCC generated implicit function
calls for ftrace etc.

The module memory size increases by about 5% on average, regardless of
whether any PLT entries were actually needed. However, due to the page
based rounding that occurs when allocating module memory, the average
memory footprint increase is negligible.

5bb5d66d89 ARM: 8341/1: io: Unpessimize relaxed io accessors
commit 195bbcac2e ("ARM: 7500/1: io: avoid writeback addressing
modes for __raw_ accessors") disables writeback addressing modes for
raw i/o. However, the "+Q" output constraint forces the compiler to
disable load hoist optimizations (because the output constraint informs the
compiler of memory stores which the compiler assumes may alias other memory).

Since the relaxed accessors only guarantee ordering wrt i/o accesses to the
same device and not to main memory, there's never a possibility of an accessor
invalidating a hoisted load (because only non-i/o loads would have been hoisted).

The effect is especially noticable with complex address inputs in loops.

7895f73169 ARM: MCPM: remove residency argument from mcpm_cpu_suspend()
This is currently unused.

If a suspend must be limited to CPU level only by preventing the last man
from triggering a cluster level suspend then this should be determined
according to many other criteria the MCPM layer is currently not aware of.
It is unlikely that mcpm_cpu_suspend() would be the proper conduit for
that information anyway.

7cc8b991cd ARM: MCPM: make internal helpers private to the core code
This concerns the following helpers:

	__mcpm_cpu_going_down()
	__mcpm_cpu_down()
	__mcpm_outbound_enter_critical()
	__mcpm_outbound_leave_critical()
	__mcpm_cluster_state()

They are and should only be used by the core code now.  Therefore their
declarations are removed from mcpm.h and their definitions are made
static, hence the need to move them before their users which accounts
for the bulk of this patch.

This left the mcpm_sync_struct definition at an odd location, therefore

8746515d7f xen: Add __GFP_DMA flag when xen_swiotlb_init gets free pages on ARM
Make sure that xen_swiotlb_init allocates buffers that are DMA capable
when at least one memblock is available below 4G. Otherwise we assume
that all devices on the SoC can cope with >4G addresses. We do this on
ARM and ARM64, where dom0 is mapped 1:1, so pfn == mfn in this case.

No functional changes on x86.

1424532b21 ARM: 8347/1: dma-mapping: fix off-by-one check in arm_setup_iommu_dma_ops
Patch 22b3c181c6 ("arm: dma-mapping: limit
IOMMU mapping size") added a check for IO address space size. However
this patch broke IOMMU initialization for typical platforms initialized
from device tree, which get the default IO address space size of 4GiB.
This value doesn't fit into size_t and fails a check introduced by that
commit resulting in failed dma-mapping/iommu initialization. This patch
fixes this issue by adding proper support for full 4GiB address space
size.

204db6ed17 mm: fold arch_randomize_brk into ARCH_HAS_ELF_RANDOMIZE
The arch_randomize_brk() function is used on several architectures,
even those that don't support ET_DYN ASLR. To avoid bulky extern/#define
tricks, consolidate the support under CONFIG_ARCH_HAS_ELF_RANDOMIZE for
the architectures that support it, while still handling CONFIG_COMPAT_BRK.

55dd0df781 jump_label: Allow asm/jump_label.h to be included in assembly
Wrap asm/jump_label.h for all archs with #ifndef __ASSEMBLY__.
Since these are kernel only headers, we don't need #ifdef
__KERNEL__ so can simplify things a bit.

If an architecture wants to use jump labels in assembly, it
will still need to define a macro to create the __jump_table
entries (see ARCH_STATIC_BRANCH in the powerpc asm/jump_label.h
for an example).

d3a875444a ARM: MCPM: move the algorithmic complexity to the core code
All backends are reimplementing a variation of the same CPU reference
count handling. They are also responsible for driving the MCPM special
low-level locking. This is needless duplication, involving algorithmic
requirements that are not necessarily obvious to the uninitiated.
And from past code review experience, those were all initially
implemented badly.

After 3 years, it is time to refactor as much common code to the core
MCPM facility to make the backends as simple as possible.  To avoid a
flag day, the new scheme is introduced in parallel to the existing
backend interface.  When all backends are converted over, the
compatibility interface could be removed.

The new MCPM backend interface implements simpler methods addressing
very platform specific tasks performed under lock protection while
keeping the algorithmic complexity and race avoidance local to the
core code.

fee3fd4fd2 ARM: 8338/1: kexec: Relax SMP validation to improve DT compatibility
When trying to kexec into a new kernel on a platform where multiple CPU
cores are present, but no SMP bringup code is available yet, the
kexec_load system call fails with:

    kexec_load failed: Invalid argument

The SMP test added to machine_kexec_prepare() in commit 2103f6cba6
("ARM: 7807/1: kexec: validate CPU hotplug support") wants to prohibit
kexec on SMP platforms where it cannot disable secondary CPUs.
However, this test is too strict: if the secondary CPUs couldn't be
enabled in the first place, there's no need to disable them later at
kexec time.  Hence skip the test in the absence of SMP bringup code.

This allows to add all CPU cores to the DTS from the beginning, without
having to implement SMP bringup first, improving DT compatibility.

950324ab81 KVM: arm/arm64: rework MMIO abort handling to use KVM MMIO bus
Currently we have struct kvm_exit_mmio for encapsulating MMIO abort
data to be passed on from syndrome decoding all the way down to the
VGIC register handlers. Now as we switch the MMIO handling to be
routed through the KVM MMIO bus, it does not make sense anymore to
use that structure already from the beginning. So we keep the data in
local variables until we put them into the kvm_io_bus framework.
Then we fill kvm_exit_mmio in the VGIC only, making it a VGIC private
structure. On that way we replace the data buffer in that structure
with a pointer pointing to a single location in a local variable, so
we get rid of some copying on the way.
With all of the virtual GIC emulation code now being registered with
the kvm_io_bus, we can remove all of the old MMIO handling code and
its dispatching functionality.

I didn't bother to rename kvm_exit_mmio (to vgic_mmio or something),
because that touches a lot of code lines without any good reason.

8defb3367f ARM: 8320/1: fix integer overflow in ELF_ET_DYN_BASE
Usually ELF_ET_DYN_BASE is 2/3 of TASK_SIZE. With 3G/1G user/kernel
split this is not so, because 2*TASK_SIZE overflows 32 bits,
so the actual value of ELF_ET_DYN_BASE is:
	(2 * TASK_SIZE / 3) = 0x2a000000

When ASLR is disabled PIE binaries will load at ELF_ET_DYN_BASE address.
On 32bit platforms AddressSanitzer uses addresses [0x20000000 - 0x40000000]
for shadow memory [1]. So ASan doesn't work for PIE binaries when ASLR disabled
as it fails to map shadow memory.
Also after Kees's 'split ET_DYN ASLR from mmap ASLR' patchset PIE binaries
has a high chance of loading somewhere in between [0x2a000000 - 0x40000000]
even if ASLR enabled. This makes ASan with PIE absolutely incompatible.

Fix overflow by dividing TASK_SIZE prior to multiplying.
After this patch ELF_ET_DYN_BASE equals to (for CONFIG_VMSPLIT_3G=y):
	(TASK_SIZE / 3 * 2) = 0x7f555554

Define AT_SYSINFO_EHDR, and, if CONFIG_VDSO=y, report the vdso shared
object address via the ELF auxiliary vector.

Note - this adds the AT_SYSINFO_EHDR in a new user-visible header
asm/auxvec.h; this is consistent with other architectures.

772742a6c7 arm-cci: Get rid of secure transactions for PMU driver
Avoid secure transactions while probing the CCI PMU. The
existing code makes use of the Peripheral ID2 (PID2) register
to determine the revision of the CCI400, which requires a
secure transaction. This puts a limitation on the usage of the
driver on systems running non-secure Linux(e.g, ARM64).

Updated the device-tree binding for cci pmu node to add the explicit
revision number for the compatible field.

The supported strings are :
	arm,cci-400-pmu,r0
	arm,cci-400-pmu,r1
	arm,cci-400-pmu - DEPRECATED. See NOTE below

NOTE: If the revision is not mentioned, we need to probe the cci revision,
which could be fatal on a platform running non-secure. We need a reliable way
to know if we can poke the CCI registers at runtime on ARM32. We depend on
'mcpm_is_available()' when it is available. mcpm_is_available() returns true
only when there is a registered driver for mcpm. Otherwise, we assume that we
don't have secure access, and skips probing the revision number(ARM64 case).

The MCPM should figure out if it is safe to access the CCI. Unfortunately
there isn't a reliable way to indicate the same via dtb. This patch doesn't
address/change the current situation. It only deals with the CCI-PMU, leaving
the assumptions about the secure access as it has been, prior to this patch.

9fd85eb502 ARM: pmu: add support for interrupt-affinity property
Historically, the PMU devicetree bindings have expected SPIs to be
listed in order of *logical* CPU number. This is problematic for
bootloaders, especially when the boot CPU (logical ID 0) isn't listed
first in the devicetree.

This patch adds a new optional property, interrupt-affinity, to the
PMU node which allows the interrupt affinity to be described using
a list of phandled to CPU nodes, with each entry in the list
corresponding to the SPI at the same index in the interrupts property.

449e056c76 ARM: cpuidle: Add a cpuidle ops structure to be used for DT
The current state of the different cpuidle drivers is the different PM
operations are passed via the platform_data using the platform driver
paradigm.

This approach allowed to split the low level PM code from the arch specific
and the generic cpuidle code.

Unfortunately there are complaints about this approach as, in the context of the
single kernel image, we have multiple drivers loaded in memory for nothing and
the platform driver is not adequate for cpuidle.

This patch provides a common interface via cpuidle ops for all new cpuidle
driver and a definition for the device tree.

It will allow with the next patches to a have a common definition with ARM64
and share the same cpuidle driver.

The code is optimized to use the __init section intensively in order to reduce
the memory footprint after the driver is initialized and unify the function
names with ARM64.

e4c5a68510 arm64: KVM: use ID map with increased VA range if required
This patch modifies the HYP init code so it can deal with system
RAM residing at an offset which exceeds the reach of VA_BITS.

Like for EL1, this involves configuring an additional level of
translation for the ID map. However, in case of EL2, this implies
that all translations use the extra level, as we cannot seamlessly
switch between translation tables with different numbers of
translation levels.

So add an extra translation table at the root level. Since the
ID map and the runtime HYP map are guaranteed not to overlap, they
can share this root level, and we can essentially merge these two
tables into one.

35307b9a5f arm/arm64: KVM: Implement Stage-2 page aging
Until now, KVM/arm didn't care much for page aging (who was swapping
anyway?), and simply provided empty hooks to the core KVM code. With
server-type systems now being available, things are quite different.

This patch implements very simple support for page aging, by clearing
the Access flag in the Stage-2 page tables. On access fault, the current
fault handling will write the PTE or PMD again, putting the Access flag
back on.

It should be possible to implement a much faster handling for Access
faults, but that's left for a later patch.

With this in place, performance in VMs is degraded much more gracefully.

04b8dc85bf arm64: KVM: Do not use pgd_index to index stage-2 pgd
The kernel's pgd_index macro is designed to index a normal, page
sized array. KVM is a bit diffferent, as we can use concatenated
pages to have a bigger address space (for example 40bit IPA with
4kB pages gives us an 8kB PGD.

In the above case, the use of pgd_index will always return an index
inside the first 4kB, which makes a guest that has memory above
0x8000000000 rather unhappy, as it spins forever in a page fault,
whist the host happilly corrupts the lower pgd.

The obvious fix is to get our own kvm_pgd_index that does the right
thing(tm).

Tested on X-Gene with a hacked kvmtool that put memory at a stupidly
high address.

A possible approach for this is to split the compound page using
split_page() at allocation time, and change the teardown path to
free one page at a time.  It turns out that alloc_pages_exact() and
free_pages_exact() does exactly that.

While we're at it, the PGD allocation code is reworked to reduce
duplication.

This has been tested on an X-Gene platform with a 4kB/48bit-VA host
kernel, and kvmtool hacked to place memory in the second page of
the hardware PGD (PUD for the host kernel). Also regression-tested
on a Cubietruck (Cortex-A7).

f56141e3e2 all arches, signal: move restart_block to struct task_struct
If an attacker can cause a controlled kernel stack overflow, overwriting
the restart block is a very juicy exploit target.  This is because the
restart_block is held in the same memory allocation as the kernel stack.

Moving the restart block to struct task_struct prevents this exploit by
making the restart_block harder to locate.

Note that there are other fields in thread_info that are also easy
targets, at least on some architectures.

It's also a decent simplification, since the restart code is more or less
identical on all architectures.

8aa76875dc arm: define __PAGETABLE_PMD_FOLDED for !LPAE
ARM uses custom implementation of PMD folding in 2-level page table case.
Generic code expects to see __PAGETABLE_PMD_FOLDED to be defined if PMD is
folded, but ARM doesn't do this.  Let's fix it.

Defining __PAGETABLE_PMD_FOLDED will drop out unused __pmd_alloc().  It
also fixes problems with recently-introduced pmd accounting on ARM without
LPAE.

That laconic "some MM" is mainly the removal of remap_file_pages(),
which is a big simplification of the VM, and which gets rid of a *lot*
of random cruft and special cases because we no longer support the
non-linear mappings that it used.

From a user interface perspective, nothing has changed, because the
remap_file_pages() syscall still exists, it's just done by emulating the
old behavior by creating a lot of individual small mappings instead of
one non-linear one.

The emulation is slower than the old "native" non-linear mappings, but
nobody really uses or cares about remap_file_pages(), and simplifying
the VM is a big advantage.

ada63d4074 ARM: 8300/1: teach __asmeq that r11 == fp and r12 == ip
The __asmeq macro is used inside inline asm statements to ensure that
register asm variables that explicitly specify a register are mapped
correctly onto those registers when used in inline asm input and output
constraints. However, the string based matching fails to take into
account that 'fp' is often referred to as 'r11' and 'ip' is often
referred to as 'r12', (e.g., by clang), causing false negatives.

Fix this by making __asmeq consider the ("fp","r11"), ("r11","fp"),
("ip","r12") and ("r12","ip") cases specifically.

f781951299 kvm: add halt_poll_ns module parameter
This patch introduces a new module parameter for the KVM module; when it
is present, KVM attempts a bit of polling on every HLT before scheduling
itself out via kvm_vcpu_block.

This parameter helps a lot for latency-bound workloads---in particular
I tested it with O_DSYNC writes with a battery-backed disk in the host.
In this case, writes are fast (because the data doesn't have to go all
the way to the platters) but they cannot be merged by either the host or
the guest.  KVM's performance here is usually around 30% of bare metal,
or 50% if you use cache=directsync or cache=writethrough (these
parameters avoid that the guest sends pointless flush requests, and
at the same time they are not slow because of the battery-backed cache).
The bad performance happens because on every halt the host CPU decides
to halt itself too.  When the interrupt comes, the vCPU thread is then
migrated to a new physical CPU, and in general the latency is horrible
because the vCPU thread has to be scheduled back in.

With this patch performance reaches 60-65% of bare metal and, more
important, 99% of what you get if you use idle=poll in the guest.  This
means that the tunable gets rid of this particular bottleneck, and more
work can be done to improve performance in the kernel or QEMU.

Of course there is some price to pay; every time an otherwise idle vCPUs
is interrupted by an interrupt, it will poll unnecessarily and thus
impose a little load on the host.  The above results were obtained with
a mostly random value of the parameter (500000), and the load was around
1.5-2.5% CPU usage on one of the host's core for each idle guest vCPU.

The patch also adds a new stat, /sys/kernel/debug/kvm/halt_successful_poll,
that can be used to tune the parameter.  It counts how many HLT
instructions received an interrupt during the polling period; each
successful poll avoids that Linux schedules the VCPU thread out and back
in, and may also avoid a likely trip to C1 and back for the physical CPU.

While the VM is idle, a Linux 4 VCPU VM halts around 10 times per second.
Of these halts, almost all are failed polls.  During the benchmark,
instead, basically all halts end within the polling period, except a more
or less constant stream of 50 per second coming from vCPUs that are not
running the benchmark.  The wasted time is thus very low.  Things may
be slightly different for Windows VMs, which have a ~10 ms timer tick.

The effect is also visible on Marcelo's recently-introduced latency
test for the TSC deadline timer.  Though of course a non-RT kernel has
awful latency bounds, the latency of the timer is around 8000-10000 clock
cycles compared to 20000-120000 without setting halt_poll_ns.  For the TSC
deadline timer, thus, the effect is both a smaller average latency and
a smaller variance.

In drivers/net/vxlan.c, vxlan_find_vni() added a 'flags' parameter
in 'net-next' whereas in 'net' there was a bug fix to pass in the
correct network namespace pointer in calls to this function.

0d3e4d4fad arm/arm64: KVM: Use kernel mapping to perform invalidation on page fault
When handling a fault in stage-2, we need to resync I$ and D$, just
to be sure we don't leave any old cache line behind.

That's very good, except that we do so using the *user* address.
Under heavy load (swapping like crazy), we may end up in a situation
where the page gets mapped in stage-2 while being unmapped from
userspace by another CPU.

At that point, the DC/IC instructions can generate a fault, which
we handle with kvm->mmu_lock held. The box quickly deadlocks, user
is unhappy.

Instead, perform this invalidation through the kernel mapping,
which is guaranteed to be present. The box is much happier, and so
am I.

Let's assume a guest has created an uncached mapping, and written
to that page. Let's also assume that the host uses a cache-coherent
IO subsystem. Let's finally assume that the host is under memory
pressure and starts to swap things out.

Before this "uncached" page is evicted, we need to make sure
we invalidate potential speculated, clean cache lines that are
sitting there, or the IO subsystem is going to swap out the
cached view, loosing the data that has been written directly
into memory.

3c1e716508 arm/arm64: KVM: Use set/way op trapping to track the state of the caches
Trying to emulate the behaviour of set/way cache ops is fairly
pointless, as there are too many ways we can end-up missing stuff.
Also, there is some system caches out there that simply ignore
set/way operations.

So instead of trying to implement them, let's convert it to VA ops,
and use them as a way to re-enable the trapping of VM ops. That way,
we can detect the point when the MMU/caches are turned off, and do
a full VM flush (which is what the guest was trying to do anyway).

This allows a 32bit zImage to boot on the APM thingy, and will
probably help bootloaders in general.

For a GICv2 there is always only one (v)CPU involved: the one that
does the access. On a GICv3 the access to a CPU redistributor is
memory-mapped, but not banked, so the (v)CPU affected is determined by
looking at the MMIO address region being accessed.
To allow passing the affected CPU into the accessors later, extend
struct kvm_exit_mmio to add an opaque private pointer parameter.
The current GICv2 emulation just does not use it.

Currently the maximum number of vCPUs supported is a global value
limited by the used GIC model. GICv3 will lift this limit, but we
still need to observe it for guests using GICv2.
So the maximum number of vCPUs is per-VM value, depending on the
GIC model the guest uses.
Store and check the value in struct kvm_arch, but keep it down to
8 for now.

4429fc64b9 arm/arm64: KVM: rework MPIDR assignment and add accessors
The virtual MPIDR registers (containing topology information) for the
guest are currently mapped linearily to the vcpu_id. Improve this
mapping for arm64 by using three levels to not artificially limit the
number of vCPUs.
To help this, change and rename the kvm_vcpu_get_mpidr() function to
mask off the non-affinity bits in the MPIDR register.
Also add an accessor to later allow easier access to a vCPU with a
given MPIDR. Use this new accessor in the PSCI emulation.
