0e98719b0e nvme: simplify the API for getting log pages

59a8f43b63 lightnvm: limit get chunk meta request size
For devices that does not specify a limit on its transfer size, the
get_chk_meta command may send down a single I/O retrieving the full
chunk metadata table. Resulting in large 2-4MB I/O requests. Instead,
split up the I/Os to a maximum of 256KB and issue them separately to
reduce memory requirements.

242e461fb6 lightnvm: Remove redundant rq->__data_len initialization
Since both blk_old_get_request() and blk_mq_alloc_request() initialize
rq->__data_len to zero, it is not necessary to initialize that member
in nvme_nvm_alloc_request(). Hence remove the rq->__data_len
initialization from nvme_nvm_alloc_request().

For the sysfs functions, the function names are embedded into their
error strings. If the function name later changes, the string may
not be updated accordingly. Update the strings to use __func__
to avoid this.

a294c19945 lightnvm: implement get log report chunk helpers
The 2.0 spec provides a report chunk log page that can be retrieved
using the stangard nvme get log page. This replaces the dedicated
get/put bad block table in 1.2.

This patch implements the helper functions to allow targets retrieve the
chunk metadata using get log page. It makes nvme_get_log_ext available
outside of nvme core so that we can use it form lightnvm.

e46f4e4822 lightnvm: simplify geometry structure
Currently, the device geometry is stored redundantly in the nvm_id and
nvm_geo structures at a device level. Moreover, when instantiating
targets on a specific number of LUNs, these structures are replicated
and manually modified to fit the instance channel and LUN partitioning.

Instead, create a generic geometry around nvm_geo, which can be used by
(i) the underlying device to describe the geometry of the whole device,
and (ii) instances to describe their geometry independently.

96257a8a7f nvme: lightnvm: add late setup of block size and metadata
The nvme driver sets up the size of the nvme namespace in two steps.
First it initializes the device with standard logical block and
metadata sizes, and then sets the correct logical block and metadata
size. Due to the OCSSD 2.0 specification relies on the namespace to
expose these sizes for correct initialization, let it be updated
appropriately on the LightNVM side as well.

ed754e5dee nvme: track shared namespaces
Introduce a new struct nvme_ns_head that holds information about an actual
namespace, unlike struct nvme_ns, which only holds the per-controller
namespace information.  For private namespaces there is a 1:1 relation of
the two, but for shared namespaces this lets us discover all the paths to
it.  For now only the identifiers are moved to the new structure, but most
of the information in struct nvme_ns should eventually move over.

To allow lockless path lookup the list of nvme_ns structures per
nvme_ns_head is protected by SRCU, which requires freeing the nvme_ns
structure through call_srcu.

Implement a generic path for sending sync I/O on LightNVM. This allows
to reuse the standard synchronous path trough blk_execute_rq(), instead

608cc4b14a nvme: fix lightnvm check
nvme_nvm_ns_supported assumes every device is a pci_dev, which leads to
reading an incorrect field, or possible even a dereference of unallocated
memory for fabrics controllers.

Fix this by introducing a quirk for lighnvm capable devices instead.

74d46992e0 block: replace bi_bdev with a gendisk pointer and partitions index
This way we don't need a block_device structure to submit I/O.  The
block_device has different life time rules from the gendisk and
request_queue and is usually only available when the block device node
is open.  Other callers need to explicitly create one (e.g. the lightnvm
passthrough code, or the new nvme multipathing code).

For the actual I/O path all that we need is the gendisk, which exists
once per block device.  But given that the block layer also does
partition remapping we additionally need a partition index, which is
used for said remapping in generic_make_request.

Note that all the block drivers generally want request_queue or
sometimes the gendisk, so this removes a layer of indirection all
over the stack.

2a842acab1 block: introduce new block status code type
Currently we use nornal Linux errno values in the block layer, and while
we accept any error a few have overloaded magic meanings.  This patch
instead introduces a new  blk_status_t value that holds block layer specific
status codes and explicitly explains their meaning.  Helpers to convert from
and to the previous special meanings are provided for now, but I suspect
we want to get rid of them in the long run - those drivers that have a
errno input (e.g. networking) usually get errnos that don't know about
the special block layer overloads, and similarly returning them to userspace
will usually return somethings that strictly speaking isn't correct
for file system operations, but that's left as an exercise for later.

For now the set of errors is a very limited set that closely corresponds
to the previous overloaded errno values, but there is some low hanging
fruite to improve it.

blk_status_t (ab)uses the sparse __bitwise annotations to allow for sparse
typechecking, so that we can easily catch places passing the wrong values.

With gcc 4.1.2:

Indeed, since commit 2e13f33a24 ("lightnvm: create cmd before
allocating request"), the request is passed to nvme_nvm_rqtocmd() before
it is allocated.

Fortunately, as of commit 91276162de ("lightnvm: refactor end_io
functions for sync"), nvme_nvm_rqtocmd () no longer uses the passed
request, so this warning is a false positive.

Drop the unused parameter to clean up the code and kill the warning.

Currently it's used by the lighnvm passthrough ioctl, but we'd like to make
it private in preparation of block layer specific error code.  Lighnvm already
returns the real NVMe status anyway, so I think we can just limit it to
returning -EIO for any status set.

This will need a careful audit from the lightnvm folks, though.

27fa9bc545 nvme: split nvme status from block req->errors
We want our own clearly defined error field for NVMe passthrough commands,
and the request errors field is going away in its current form.

Just store the status and result field in the nvme_request field from
hardirq completion context (using a new helper) and then generate a
Linux errno for the block layer only when we actually need it.

Because we can't overload the status value with a negative error code
for cancelled command we now have a flags filed in struct nvme_request
that contains a bit for this condition.

9460e28022 lightnvm: Use blk_init_request_from_bio() instead of open-coding it
This patch changes the behavior of the lightnvm driver as follows:
* REQ_FAILFAST_MASK is set for read-ahead requests.
* If no I/O priority has been set in the bio, the I/O priority is
  copied from the I/O context.
* The rq_disk member is initialized if bio->bi_bdev != NULL.
* The bio sector offset is copied into req->__sector instead of
  retaining the value -1 set by blk_mq_alloc_request().
* req->errors is initialized to zero.

The asserts in _nvme_nvm_check_size are not compiled due to the function
not begin called. Make sure that it is called, and also fix the wrong
sizes of asserts for nvme_nvm_addr_format, and nvme_nvm_bb_tbl, which
checked for number of bits instead of bytes.

17912c49ed lightnvm: submit erases using the I/O path
Until now erases have been submitted as synchronous commands through a
dedicated erase function. In order to enable targets implementing
asynchronous erases, refactor the erase path so that it uses the normal
async I/O submission functions. If a target requires sync I/O, it can
implement it internally. Also, adapt rrpc to use the new erase path.

2849a7becb nvme/lightnvm: Prevent small buffer overflow in nvme_nvm_identify
There are two closely named structs in lightnvm:
struct nvme_nvm_addr_format and
struct nvme_addr_format.

The first struct has 4 reserved bytes at the end, the second does not.
(gdb) p sizeof(struct nvme_nvm_addr_format)
$1 = 16
(gdb) p sizeof(struct nvm_addr_format)
$2 = 12

In the nvme_nvm_identify function we memcpy from the larger struct to the
smaller struct. We incorrectly pass the length of the larger struct
and overflow by 4 bytes, lets not do that.

06894efea7 lightnvm: use end_io callback instead of instance
When the lightnvm core had the "gennvm" layer between the device and the
target, there was a need for the core to be able to figure out which
target it should send an end_io callback to. Leading to a "double"
end_io, first for the media manager instance, and then for the target
instance. Now that core and gennvm is merged, there is no longer a need
for this, and a single end_io callback will do.

84d4add793 lightnvm: add ioctls for vector I/Os
Enable user-space to issue vector I/O commands through ioctls. To issue
a vector I/O, the ppa list with addresses is also required and must be
mapped for the controller to access.

For each ioctl, the result and status bits are returned as well, such

19bd6fe73c lightnvm: reduce number of nvm_id groups to one
The number of configuration groups has been limited to one in current
code, even if there is support for up to four. With the introduction
of the open-channel SSD 1.3 specification, only a single
group is exposed onwards. Reflect this in the nvm_id structure.

ade69e2432 lightnvm: merge gennvm with core
For the first iteration of Open-Channel SSDs, it was anticipated that
there could be various media managers on top of an open-channel SSD,
such to allow vendors to plug in their own host-side FTLs, without the
media manager in between.

Now that an Open-Channel SSD is exposed as a traditional block device,
there is no longer a need for this. Therefore lets merge the gennvm code
with core and simplify the stack.

8e53624d44 lightnvm: eliminate nvm_lun abstraction in mm
In order to naturally support multi-target instances on an Open-Channel
SSD, targets should own the LUNs they get blocks from and manage
provisioning internally. This is done in several steps.

Since targets own the LUNs the are instantiated on top of and manage the
free block list internally, there is no need for a LUN abstraction in
the media manager. LUNs are intrinsically managed as in the physical
layout (ch:0,lun:0, ..., ch:0,lun:n, ch:1,lun:0, ch:1,lun:n, ...,
ch:m,lun:0, ch:m,lun:n) and given to the targets based on the target
creation ioctl. This simplifies LUN management and clears the path for a
partition manager to sit directly underneath LightNVM targets.
