Add a function to get the maximum address that can be hotplug added.
This is needed to calculate the size of the tce table needed to cover
all memory in 1:1 mode.

Convert NUMA code to new cpumask API. We shift the node to cpumask
setup code until after we complete bootmem allocation so we can
dynamically allocate the cpumasks.

Impact: build fix for powerpc and sparc
Today's linux-next build (powerpc allyesconfig) failed like this:

Caused by commit 082edb7bf4 ("numa,
cpumask: move numa_node_id default implementation to topology.h") from
the cpus4096 tree which removed the include of linux/topology.h from
linux/mmzone.h.
Same for sparc64 defconfig.

mkdir arch/powerpc/include/asm
git mv include/asm-powerpc/* arch/powerpc/include/asm
Followed by a few documentation/comment fixups and a couple of places
where <asm-powepc/...> was being used explicitly. Of the latter only
one was outside the arch code and it is a driver only built for powerpc.
