This patch converts a handful of iscsi_session statistics to type
atomic_long_t, instead of using iscsi_session->session_stats_lock
when incrementing these values.
More importantly, go ahead and drop the spinlock usage within
iscsit_setup_scsi_cmd(), iscsit_check_dataout_hdr(),
iscsit_send_datain(), and iscsit_build_rsp_pdu() fast-path code.

This patch fixes a bug in delayed Task Aborted Status (TAS) handling,
where transport_send_task_abort() was not returning for the case
when the se_tfo->write_pending() callback indicated that last fabric
specific WRITE PDU had not yet been received.
It also adds an explicit cmd->scsi_status = SAM_STAT_TASK_ABORTED
assignment within transport_check_aborted_status() to avoid the case
where se_tfo->queue_status() is called when the SAM_STAT_TASK_ABORTED
assignment + ->queue_status() in transport_send_task_abort() does not
occur once SCF_SENT_DELAYED_TAS has been set.

This patch adds a check to reject text commands with F_BIT=0 ||
C_BIT=1, as multi PDU text command sequences are currently
unsupported.
This avoids the case where a text command received with F_BIT=0,
was generating a text response with F_BIT=1 which is a protocol
error according to RFC-3720 Section 10.11.1.

This patch fixes a >= v3.10 regression bug with mutex_trylock() usage
within iscsit_increment_maxcmdsn(), that was originally added to allow
for a special case where ->cmdsn_mutex was already held from the
iscsit_execute_cmd() exception path for ib_isert.
When !mutex_trylock() was occuring under contention during normal RX/TX
process context codepaths, the bug was manifesting itself as the following
protocol error:
Received CmdSN: 0x000fcbb7 is greater than MaxCmdSN: 0x000fcbb6, protocol error.
Received CmdSN: 0x000fcbb8 is greater than MaxCmdSN: 0x000fcbb6, protocol error.
This patch simply avoids the direct ib_isert callback in lio_queue_status()
for the special iscsi_execute_cmd() exception cases, that allows the problematic
mutex_trylock() usage in iscsit_increment_maxcmdsn() to go away.

In addition to block size (already implemented), passing through
alignment offset, logical-to-phys block exponent, I/O granularity and
optimal I/O length will allow initiators to properly handle layout on
LUNs with 4K block sizes.
Tested with various weird values via scsi_debug module.
One thing to look at with this patch is the new block limits values --
instead of granularity 1 optimal 8192, Lio will now be returning whatever
the block device says, which may affect performance.

Now with percpu refcounting for se_lun in place, go ahead and drop
the legacy per se_cmd accounting for se_lun shutdown.
This includes __transport_clear_lun_from_sessions(), the associated

unused se_cmd structure members and ->transport_state bits.

This patch adds percpu refcounting for se_lun access that allows the
association of an se_lun + se_cmd in transport_lookup_cmd_lun() to
occur without an extra list_head for tracking outstanding I/O during
se_lun shutdown.
This effectively changes se_lun shutdown logic to wait for outstanding
I/O percpu references to complete in transport_lun_remove_cmd() using
se_lun->lun_ref_comp, instead of explicitly draining the per se_lun
command list and waiting for individual se_cmd descriptor processing
to complete.

This patch changes iscsit_check_dataout_hdr() to no longer generate
REJECTs for zero-length DataOUTs, and instead simply ignore these
requests.
This follows RFC-3720, Section 10.7.7. DataSegmentLength
"This is the data payload length of a SCSI Data-In or SCSI Data-Out PDU.
The sending of 0 length data segments should be avoided, but initiators
and targets MUST be able to properly receive 0 length data segments."

This patch adds an explicit check + failure for XCOPY I/O to source +
destination devices with a non-matching block_size.
This limitiation is currently due to the fact that the scatterlist
memory allocated for the XCOPY READ operation is passed zero-copy
to the XCOPY WRITE operation.

This patch adds the missing xcopy_pt_cmd->sense_buffer[] required for
correctly handling CHECK_CONDITION exceptions within the locally
generated XCOPY I/O path.
Also update target_xcopy_read_source() + target_xcopy_setup_pt_cmd()

This patch rejects EXTENDED_COPY when the emulate_3pc attribute has
been explicitly disabled for the receiving device.
It also adds a similar check in target_xcopy_locate_se_dev_e4() to
ignore these devices when doing a search based upon the identifier
WWN provided by EXTENDED_COPY parameter list target descriptors.

This patch changes target_do_xcopy() to allow processing of non-zero
ListIDs in EXTENDED_COPY parameter list data, instead of returning
CHECK_CONDITION status.
As the copy offload implementation reports SNLID=1 (Supports No ListID)
in OPERATING PARAMETERS, any ListID value presented by the client is
currently ignored.
Also, properly extract list_id_usage for informational purposes.

This patch bumps the default number of tags allocated per session by
iscsi-target via transport_alloc_session_tags() -> percpu_ida_init()
by another (tag_num / 2).
This is done to take into account the tags waiting to be acknowledged
and released in iscsit_ack_from_expstatsn(), but who's number are not
directly limited by the CmdSN Window queue_depth being enforced by
the target.
Using a larger value here is also useful to prevent percpu_ida_alloc()
from having to steal tags from other CPUs when no tags are available
on the local CPU, while waiting for unacknowledged tags to be released.

This patch converts iscsit_ack_from_expstatsn() to populate a local
ack_list of commands, and call iscsit_free_cmd() directly from RX
thread context, instead of using iscsit_add_cmd_to_immediate_queue()
to queue the acknowledged commands to be released from TX thread
context.
It is helpful to release the acknowledge commands as quickly as
possible, along with the associated percpu_ida tags, in order to
prevent percpu_ida_alloc() from having to steal tags from other
CPUs while waiting for iscsit_free_cmd() to happen from TX thread
context.

This patch changes transport_generic_free_cmd() to only wait_for_tasks
when shutdown=true is passed to iscsit_free_cmd().
With the advent of >= v3.10 iscsi-target code using se_cmd->cmd_kref,
the extra wait_for_tasks with shutdown=false is unnecessary, and may
end up causing an extra context switch when releasing WRITEs.

This patch addresses a bug for backends such as IBLOCK that perform
asynchronous completion via transport_complete_cmd(), that will call
target_complete_failure_work() -> transport_generic_request_failure(),
upon exception status and invoke cmd->transport_complete_callback()
-> compare_and_write_callback() incorrectly during the failure case.
It adds a check for a non zero se_cmd->scsi_status within the first
invocation of compare_and_write_callback(), and will jump to out plus
up se_device->caw_sem before exiting the callback.

This patch addresses a bug when compare_and_write_callback() invoked from
target_complete_ok_work() hits an failure from __target_execute_cmd() ->
cmd->execute_cmd(), that ends up calling transport_generic_request_failure()
-> compare_and_write_post(), thus causing SCF_COMPARE_AND_WRITE_POST to
incorrectly be set.
The result of this bug is that target_complete_ok_work() no longer hits
the if (!rc && !(cmd->se_cmd_flags & SCF_COMPARE_AND_WRITE_POST) check
that forces an immediate return, and instead double completes the se_cmd
in question, triggering an OOPs in the process.
This patch changes compare_and_write_post() to only set this bit when a
failure has not already occured to ensure the immediate return from within
target_complete_ok_work(), and thus allow transport_generic_request_failure()
to handle the sending of the CHECK_CONDITION exception status.

This patch resets se_cmd->data_length for COMPARE_AND_WRITE emulation
within sbc_compare_and_write() to NoLB * block_size in order to address
a bug with FILEIO backends where a I/O failure will occur when data_length
does not match the I/O size being actually dispatched for the individual
per block READs + WRITEs.
This is done late enough in sbc_compare_and_write() after the memory
allocations have occured in transport_generic_new_cmd() to not cause
any unwanted side-effects.

This patch converts tcm_fc to use transport_init_session_tags()
pre-allocation logic for struct ft_cmd descriptors using per-cpu
session tag pooling in order to effectively avoid memory allocation
+ release for each received I/O.
It adds percpu_ida_alloc() in ft_recv_cmd() to obtain an tag and
locate ft_cmd from se_sess->sess_cmd_map[], and percpu_ida_free()
in ft_free_cmd() to release the tag based upon se_cmd->map_tag id.
It also uses a TCM_FC_DEFAULT_TAGS value of 512, that puts the
per se_sess->sess_cmd_map allocation at ~360K on 64-bit.

This patch changes transport_alloc_session_tags() to fall back to
use vzalloc when kzalloc fails for big tag_num that end up generating
larger order allocations.
Also use is_vmalloc_addr() in transport_alloc_session_tags() failure
path, and normal transport_free_session() path to determine when
vfree() needs to be called instead of kfree().

This patch bumps the default TCP listen backlog within iscsit_setup_np()
from 5 to 256, in order to reduce the overall latency caused by a small
backlog with 100's of simultaneous login attempts directed to the same
single network portal.
Also add a ISCSIT_TCP_BACKLOG macro in iscsi_target_core.h.

This patch bumps the default TA_DEFAULT_CMDSN_DEPTH from 16 -> 64,
which is closer to a sane default for larger pipes @ 10 Gb/sec
with traditional iSCSI, or @ 40/56 Gb/sec Ethernet/Infiniband with
iSCSI Extentions for RDMA.
There is really no downside to increasing this default value for
1 Gb/sec.

This patch removes an unnecessary wait_for_completion within
iscsi_get_thread_set(), that would wait for 1 second before
trying to obtain an inactive struct iscsi_thread_set from
iscsi_get_ts_from_inactive_list().
Since iscsi_allocate_thread_sets() will already be adding the
newly allocated iscsi_thread_set to the inactive list directly,
there is no need to wait here.

This patch removes the iscsi_thread_set->[rx,tx]_post_start_comp that
was originally used synchronize startup between rx and tx threads within
a single thread_set.
Instead, use a single ->ts_activate_sem in iscsi_activate_thread_set()
to wait for both processes to awake in the RX/TX pre handlers.
Also, go ahead and refactor thread_set deallocate code into a common
iscsi_deallocate_thread_one(), and update iscsi_deallocate_thread_sets()
and iscsi_deallocate_extra_thread_sets() use this code

This patch adds support for EXTENDED_COPY emulation from SPC-3, that
enables full copy offload target support within both a single virtual
backend device, and across multiple virtual backend devices. It also
functions independent of target fabric, and supports copy offload
across multiple target fabric ports.
This implemenation supports both EXTENDED_COPY PUSH and PULL models
of operation, so the actual CDB may be received on either source or
desination logical unit.
For Target Descriptors, it currently supports the NAA IEEE Registered
Extended designator (type 0xe4), which allows the reference of target
ports to occur independent of fabric type using EVPD 0x83 WWNs.
For Segment Descriptors, it currently supports copy from block to
block (0x02) mode.
It also honors any present SCSI reservations of the destination target
port. Note that only Supports No List Identifier (SNLID=1) mode is
supported.
Also included is basic RECEIVE_COPY_RESULTS with service action type
OPERATING PARAMETERS (0x03) required for SNLID=1 operation.

This patch adds a extra check for SCF_COMPARE_AND_WRITE within
transport_generic_request_failure() to invoke the callback for
compare_and_write_callback() or compare_and_write_done(), in
order to release se_dev->caw_mutex from the generic failure
path.
It also adds to checks within compare_and_write_callback() to
avoid processing when transport_generic_request_failure() occurs
early enough that cmd->t_data_sg or cmd->t_bidi_data_sg have not
been setup yet, nor se_dev->caw_mutex obtained from within
sbc_compare_and_write().

This patch changes target_complete_ok_work() to fall through
after calling the se_cmd->transport_complete_callback() ->
compare_and_write_post() callback, by keying off the existance
of SCF_COMPARE_AND_WRITE_POST.
This is necessary because once SCF_COMPARE_AND_WRITE_POST has
been set by compare_and_write_post(), the SCSI response needs
to be sent via TFO->queue_status().

This patch adds support for COMPARE_AND_WRITE emulation on a per block
basis. This logic is used as an atomic test and set primative currently
used by VMWare ESX VAAI for performing array side locking of individual
VMFS extent ownership.
This includes the COMPARE_AND_WRITE CDB parsing within sbc_parse_cdb(),
and does the majority of the work within the compare_and_write_callback()
to perform the verify instance user data comparision, and subsequent
write instance user data I/O submission upon a successfull comparision.
The synchronization is enforced by se_device->caw_sem, that is obtained
before the initial READ I/O submission in sbc_compare_and_write(). The
mutex is then released upon MISCOMPARE in compare_and_write_callback(),
or upon WRITE instance user-data completion in compare_and_write_post().
The implementation currently assumes a single logical block (NoLB=1).

After COMPARE_AND_WRITE completes it's comparision, the WRITE
payload SGLs head expect to be updated to point from the verify
instance of user data, to the write instance of user data.
So for this special case, add transport_reset_sgl_orig() usage
within transport_free_pages() and add se_cmd->t_data_[sg,nents]_orig
members to save the original assignments.

COMPARE_AND_WRITE expects to be able to send down a DMA_FROM_DEVICE
to obtain the necessary READ payload for comparision against the
first half of the WRITE payload containing the verify user data.
Currently virtual backends expect to internally reference SGLs,
SGL nents, and data_direction, so change IBLOCK, FILEIO and RD
sbc_ops->execute_rw() to accept this values as function parameters.
Also add default sbc_execute_rw() handler for the typical case for
cmd->execute_rw() submission using cmd->t_data_sg, cmd->t_data_nents,
and cmd->data_direction).

This patch changes iscsi-target to use transport_alloc_session_tags()
pre-allocation logic for per-cpu session tag pooling with internal
ida_alloc() + ida_free() calls based upon the saved se_cmd->map_tag id.
This includes tag pool setup based upon per NodeACL queue_depth after
locating se_node_acl in iscsi_target_locate_portal().
Also update iscsit_allocate_cmd() and iscsit_release_cmd() to use
percpu_ida_alloc() and percpu_ida_free() respectively.

This command converts iscsi/isert-target to use allocations based on
iscsit_transport->priv_size within iscsit_allocate_cmd(), instead of
using an embedded isert_cmd->iscsi_cmd.
This includes removing iscsit_transport->alloc_cmd() usage, along
with updating isert-target code to use iscsit_priv_cmd().
Also, remove left-over iscsit_transport->release_cmd() usage for
direct calls to iscsit_release_cmd(), and drop the now unused
lio_cmd_cache and isert_cmd_cache.

This patch adds a sock->sk_state_change() -> iscsi_target_sk_state_change()
callback in order to handle transient TCP failures during the login process,
where sock->sk_data_ready() -> iscsi_target_sk_data_ready() may not be
called to release connection resources, and relinquish tpg->np_login_lock
via iscsit_deaccess_np()
It performs the sk->sk_state check using iscsi_target_sk_state_check() to
look for TCP_CLOSE_WAIT + TCP_CLOSE, and invokes schedule_delayed_work() ->
iscsi_target_do_cleanup() to perform the remaining cleanup from process
context.
It adds an explicit sk_state_check to iscsi_target_do_login() in order
to determine a state failure when iscsi_target_sk_state_change() may
not be able to proceed before LOGIN_FLAGS_READY=1 is set.
Also use sk->sk_sndtimeo -> sk->sk_rcvtimeo settings during login to
iscsi_target_set_sock_callbacks(), and revert back post login to use
MAX_SCHEDULE_TIMEOUT in iscsi_target_restore_sock_callbacks().

This patch adds support for login negotiation multi-plexing in
iscsi-target code.
This involves handling the first login request PDU + payload and
login response PDU + payload within __iscsi_target_login_thread()
process context, and then changing struct sock->sk_data_ready()
so that all subsequent exchanges are handled by workqueue process
context, to allow other incoming login requests to be received
in parallel by __iscsi_target_login_thread().
Upon login negotiation completion (or failure), ->sk_data_ready()
is replaced with the original kernel sockets handler saved in
iscsi_conn->orig_data_ready.

This patch addresses a v3.11 specific regression where se_cmd->state_list
was being leaked during a fabric WRITE failure, when the fabric releases
an associated se_cmd descriptor before I/O submission occurs, and normal
fast path callbacks have a chance to call target_remove_from_state_list().
It was manifesting with Poison overwritten messages with iscsi-target
once an ImmediateData payload CRC32C failure occured.
This bug was originally introduced during v3.11-rc1 with the following

This patch addresses a potential NULL pointer dereference regression in
iscsit_setup_nop_out() code, specifically for two cases when a solicited
NOPOUT triggers a ISCSI_REASON_PROTOCOL_ERROR reject to be generated.
This is because iscsi_cmd is expected to be NULL for solicited NOPOUT
case before iscsit_process_nop_out() locates the descriptor via TTT
using iscsit_find_cmd_from_ttt().

This patch fixes a bug in __iscsi_target_login_thread() where an explicit
network portal thread reset ends up leaking the iscsit_transport module
reference, along with the associated iscsi_conn allocation.
This manifests itself with iser-target where a NP reset causes the extra
iscsit_transport reference to be taken in iscsit_conn_set_transport()
during the reset, which prevents the ib_isert module from being unloaded
after the NP thread shutdown has finished.

This patch addresses a regression bug within ImmediateData=Yes failure
handling that ends up triggering an OOPs within >= v3.10 iscsi-target
code.
The problem occurs when iscsit_process_scsi_cmd() does the call to
target_put_sess_cmd(), and once again in iscsit_get_immediate_data()
that is triggered during two different cases:
- When iscsit_sequence_cmd() returns CMDSN_LOWER_THAN_EXP, for which
the descriptor state will already have been set to ISTATE_REMOVE
by iscsit_sequence_cmd(), and
- When iscsi_cmd->sense_reason is set, for which iscsit_execute_cmd()
will have already called transport_send_check_condition_and_sense()
to queue the exception response.
It changes iscsit_process_scsi_cmd() to drop the early call, and makes
iscsit_get_immediate_data() call target_put_sess_cmd() from a single
location after dumping the immediate data for the failed command.

Odd little issue, found that if you create an IPv6 portal bound to the
IN6ADDR_ANY wildcard address it will accept IPv4 connections (as long as
bindv6only isn't set globally) but respond to SendTargets requests with
an IPv4-mapped IPv6 address.
Example over loopback:
In targetcli create a wildcard IPv6 portal
/iscsi/iqn.../portals/> create ::
Which should create a portal [::]:3260
Initiate SendTargets discovery to the portal using an IPv4 address
# iscsiadm -m discovery -t st -p 127.0.0.1
The response formats TargetAddress as [::ffff:127.0.0.1]:3260,1
This still works and uses v4 on the network between two v6 sockets, but
only if the initiator supports IPv6 with v4-mapped addresses.
This change detects v4-mapped address on v6 sockets for the wildcard
case, and instead formats the TargetAddress response as an IPv4 address.
In order to not further complicate iscsit_build_sendtargets_response,
I've actually simplified it by moving the bracket wrapping of IPv6
address into iscsit_accept_np where local_ip and login_ip strings are
set. That also simplifies iscsi_stat_tgt_attr_show_attr_fail_intr_addr.
Side effect of the string format change is that
lio_target_nacl_show_info will now print login_ip bracket wrapped for
IPv6 connections, as will a few debug prints.

because strict_strtoul() and strict_strtoull() are obsolete. Thus,

This patch fixes spc_emulate_inquiry_std() to add trailing ASCII
spaces for INQUIRY vendor + model fields following SPC-4 text:
"ASCII data fields described as being left-aligned shall have any
unused bytes at the end of the field (i.e., highest offset) and
the unused bytes shall be filled with ASCII space characters (20h)."
This addresses a problem with Falconstor NSS multipathing.

Three have been checked for but were never set. Remove the dead code.
Also renumbers the remaining ones to a) get rid of the holes after the
removal and b) avoid a collision between TMR_FUNCTION_COMPLETE==0 and
the uninitialized case. If we failed to set a code, we should rather
fall into the default case then return success.

The SBC-2 specification of READ CAPACITY(10) has PMI and LOGICAL BLOCK
ADDRESS fields in the CDB; in SBC-3 these fields are simply listed as
obsolete. However, SBC-2 also has the language
If the PMI bit is set to zero and the LOGICAL BLOCK ADDRESS field
is not set to zero, the device server shall terminate the command
with CHECK CONDITION status with the sense key set to ILLEGAL
REQUEST and the additional sense code set to INVALID FIELD IN CDB.
and in fact at least the Windows SCSI compliance test checks this
behavior. Since no one following SBC-3 is going to set these fields,
we might as well include the check from SBC-2 and pass this test.

This patch adds tracepoints to the target code for commands being
received and being completed, which is quite useful for debugging
interactions with initiators. For example, one can do something like the

This patch addresses a bug where RDMA_CM_EVENT_DISCONNECTED may occur
before the connection shutdown has been completed by rx/tx threads,
that causes isert_free_conn() to wait indefinately on ->conn_wait.
This patch allows isert_disconnect_work code to invoke rdma_disconnect
when isert_disconnect_work() process context is started by client
session reset before isert_free_conn() code has been reached.
It also adds isert_conn->conn_mutex protection for ->state within
isert_disconnect_work(), isert_cq_comp_err() and isert_free_conn()
code, along with isert_check_state() for wait_event usage.

This patch adds target_get_sess_cmd reference counting for
iscsit_handle_task_mgt_cmd(), and adds a target_put_sess_cmd()
for the failure case.
It also fixes a bug where ISCSI_OP_SCSI_TMFUNC type commands

This patch moves ISCSI_OP_REJECT failures into iscsit_sequence_cmd()

This patch changes iscsit_add_reject() + iscsit_add_reject_from_cmd()
usage to not sleep on iscsi_cmd->reject_comp to address a free-after-use
usage bug in v3.10 with iser-target code.
It saves ->reject_reason for use within iscsit_build_reject() so the
correct value for both transport cases. It also drops the legacy
fail_conn parameter usage throughput iscsi-target code and adds
two iscsit_add_reject_cmd() and iscsit_reject_cmd helper functions,
along with various small cleanups.

This patch adds a auth configfs group context following existing
explict NodeACL and discovery auth within:
/sys/kernel/config/target/iscsi/$TARGETNAME/$TPGT/auth/
This patch allows these attributes to be used for CHAP authentication
an TPG is configured in demo-mode (generate_node_acl=1).
Note this authentication information takes precedence over NodeACL
authentication when struct se_node_acl->dynamic_node_acl is present.

This includes splitting existing iscsit_handle_nop_out() into

iscsi socket calls.
Next update iser-target code to use new callers and add FIXME for
the handling iscsi_nopout payloads. Also fix reject response handling
in iscsit_setup_nop_out() to use proper iscsit_add_reject_from_cmd().

Writing 0 when iser was not previously enabled, so succeed but do
nothing so that user-space code doesn't need a try: catch block
when ib_isert logic is not available.

In modern iscsi-target code, the setup and I/O submission is done within a
single process context, so there is no need to acquire se_cmd->t_state_lock while
checking SCF_SUPPORTED_SAM_OPCODE for determining when unsolicited data-out
should be dumped.

This patch drops the se_cmd->t_state_lock access around SCF_SUPPORTED_SAM_OPCODE
assignment within target_setup_cmd_from_cdb().
Original v4.0 target code required this as fabrics would be checking for
this values in different process contexts for setup and I/O submission.
Given that modern v4.1 target code performs setup and I/O submission
from the same process context, this t_state_lock access is no longer
required.

This patch makes target_execute_cmd() set CMD_T_BUSY|CMD_T_SENT while
holding se_cmd->t_state_lock, in order to avoid the extra aquire/release
in __target_execute_cmd().
It also clears these bits in case of a target_handle_task_attr()
failure.

This patch drops an unnecessary acquire/release of se_cmd->t_state_lock within
transport_lun_remove_cmd() when checking CMD_T_DEV_ACTIVE for invoking
target_remove_from_state_list().
For all fast path completion cases, transport_lun_remove_cmd() is always
called ahead of transport_cmd_check_stop(), and since transport_cmd_check_stop()
is calling target_remove_from_state_list() when remove_from_lists=true,
the t_state_lock usage in transport_lun_remove_cmd() can safely be removed.

This patch adds a new transport_cmd_check_stop() parameter for signaling
when TRANSPORT_WRITE_PENDING needs to be set.
This allows transport_generic_new_cmd() to avoid the extra lock acquire/release
of ->t_state_lock in the fast path for DMA_TO_DEVICE operations ahead of
transport_cmd_check_stop() + se_tfo->write_pending().

Instead of taking the buffer and length, update_and_write_aptpl() will
allocate the buffer as needed, and then free it. Instead, the function
takes an 'aptpl' boolean parameter.
This enables us to remove memory alloc/frees from struct
t10_pr_registration and other spots.
There is a slight loss of functionality because each callsite doesn't get
its own pr_debug any more, but this info can be cleaned via ftrace if
necessary and I think the shorter code is worth it.

We use this function exclusively in debug prints. Instead of returning
0 or 1 if isid is present, just set buf to "" if it isn't there. This
saves callers from having to check the return value.

core_scsi3_enulate_pro_register took an 'ignore_key' parameter that
really distinguished between REGISTER and REGISTER_AND_IGNORE_EXISTING_KEY
registration types, which was a little confusing. Same situation for
PREEMPT and PREEMPT_AND_ABORT. Use enums to add a little more
descriptiveness to the code.

Go ahead and propigate up the ->cmd_kref put return value from
target_put_sess_cmd() -> transport_release_cmd() -> transport_put_cmd()
-> transport_generic_free_cmd().
This is useful for certain fabrics when determining the active I/O
shutdown case with SCF_ACK_KREF where a final target_put_sess_cmd()
is still required by the caller.

If a key was larger than 64 bytes, as checked by iscsi_check_key(), the
error response packet, generated by iscsi_add_notunderstood_response(),
would still attempt to copy the entire key into the packet, overflowing
the structure on the heap.
Remote preauthentication kernel memory corruption was possible if a
target was configured and listening on the network.

This patch fixes a bug where FILEIO was incorrectly reporting the number
of logical blocks (+ 1) when using non struct block_device export mode.
It changes fd_get_blocks() to follow all other backend ->get_blocks() cases,
and reduces the calculated dev_size by one dev->dev_attrib.block_size
number of bytes, and also fixes initial fd_block_size assignment at
fd_configure_device() time introduced in commit 0fd97ccf4.

Switch back to pre commit 1c7b13fe65 list splicing logic for active I/O
shutdown with tcm_qla2xxx + ib_srpt fabrics.
The original commit was done under the incorrect assumption that it's safe to
walk se_sess->sess_cmd_list unprotected in target_wait_for_sess_cmds() after
sess->sess_tearing_down = 1 has been set by target_sess_cmd_list_set_waiting()
during session shutdown.
So instead of adding sess->sess_cmd_lock protection around sess->sess_cmd_list
during target_wait_for_sess_cmds(), switch back to sess->sess_wait_list to
allow wait_for_completion() + TFO->release_cmd() to occur without having to
walk ->sess_cmd_list after the list_splice.
Also add a check to exit if target_sess_cmd_list_set_waiting() has already
been called, and add a WARN_ON to check for any fabric bug where new se_cmds
are added to sess->sess_cmd_list after sess->sess_tearing_down = 1 has already
been set.

It is possible for one thread to to take se_sess->sess_cmd_lock in
core_tmr_abort_task() before taking a reference count on
se_cmd->cmd_kref, while another thread in target_put_sess_cmd() drops
se_cmd->cmd_kref before taking se_sess->sess_cmd_lock.
This introduces kref_put_spinlock_irqsave() and uses it in
target_put_sess_cmd() to close the race window.

This patch fixes a regression bug introduced in v3.9-rc1 where if the
underlying struct block_device for a IBLOCK backend is configured with
WCE=1 + DPOFUA=1 settings, the rw = WRITE assignment no longer occurs
in iblock_execute_rw(), and rw = 0 is passed to iblock_submit_bios()
in effect causing a READ bio operation to occur.

This patch adds a rd_nullio parameter that allows RAMDISK_MCP backends
to function in NULLIO mode, where all se_cmd I/O is immediately completed
in rd_execute_rw() without actually performing the SGL memory copy.
This is useful for performance testing when the ramdisk SGL memory copy
begins to eat lots of cycles during heavy small block workloads, so allow
this bit to be enabled when necessary on a per rd_dev basis.

Fix two issues in OOO commands processing done at iscsit_attach_ooo_cmdsn.
Handle command serial numbers wrap around by using iscsi_sna_lt and not regular comparisson.
The routine iterates until it finds an entry whose serial number is greater than the serial number of
the new one, thus the new entry should be inserted before that entry and not after.

This patch adds a new network portal attribute for iser, that lives
under existing iscsi-target configfs layout at:
/sys/kernel/config/target/iscsi/$TARGETNAME/$TPGT/np/$PORTAL/iser
When lio_target_np_store_iser() is enabled, iscsit_tpg_add_network_portal()
will attempt to start an rdma_cma network portal for iser-target, only if
the external ib_isert module transport has been loaded.
When disabled, iscsit_tpg_del_network_portal() will cease iser login service
on the network portal, and release any external ib_isert module reference.

This patch converts struct iscsi_cmd memory allocation + free to use
->iscsit_alloc_cmd() iscsit_transport API caller, and export
iscsit_allocate_cmd() symbols
Also add iscsi_cmd->release_cmd() to be used seperately from
iscsit_transport for connection/session shutdown.

This patch adds RDMAExtensions, InitiatorRecvDataSegmentLength and
TargetRecvDataSegmentLength parameters keys necessary for iser-target
login to occur.
This includes setting the necessary parameters during login path
code within iscsi_login_zero_tsih_s2(), and currently PAGE_SIZE
aligning the target's advertised MRDSL for immediate data and
unsolicited data-out incoming payloads.

Add basic struct iscsit_transport API template to allow iscsi-target for
running with external transport modules using existing iscsi_target_core.h
code.
For all external modules, this calls try_module_get() and module_put()
to obtain + release an external iscsit_transport module reference count.
Also include the iscsi-target symbols necessary in iscsi_transport.h to
allow external transport modules to function.

As the comment sais, this allows Solaris initiators to survive
intermittent errors. The comment from someone reading the Solaris
sources seem to imply that multipathing would be broken without this
patch as well.

If the FILEIO backend is normal file, the emulation uses fallocate to
punch hole to reclaim the free space used by the file. If the FILEIO

This patch adds support for emulation of WRITE_SAME w/ UNMAP=1 within
fd_execute_write_same_unmap() backend code.
If the FILEIO backend is normal file, the emulation uses fallocate to

This patch fixes a bug where a handful of informational / control CDBs
that should be allowed during ALUA access state Standby/Offline/Transition
where incorrectly returning CHECK_CONDITION + ASCQ_04H_ALUA_TG_PT_*.
This includes INQUIRY + REPORT_LUNS, which would end up preventing LUN
registration when LUN scanning occured during these ALUA access states.

This patch fixes a regression introduced in v3.8-rc1 code where a failed
target_check_reservation() check in target_setup_cmd_from_cdb() was causing
an incorrect SAM_STAT_GOOD status to be returned during a WRITE operation
performed by an unregistered / unreserved iscsi initiator port.
This regression is only effecting iscsi-target due to a special case check
for TCM_RESERVATION_CONFLICT within iscsi_target_erl1.c:iscsit_execute_cmd(),
and was still correctly disallowing WRITE commands from backend submission
for unregistered / unreserved initiator ports, while returning the incorrect
SAM_STAT_GOOD status due to the missing SAM_STAT_RESERVATION_CONFLICT
assignment.
This regression was first introduced with:

Go ahead and re-add the missing SAM_STAT_RESERVATION_CONFLICT assignment
during a target_check_reservation() failure, so that iscsi-target code
sends the correct SCSI status.
All other fabrics using target_submit_cmd_*() with a RESERVATION_CONFLICT
call to transport_generic_request_failure() are not effected by this bug.

We can only have one page of data in each sg element, so we can not
cross a page boundary. Fail this case.
The 'while (len > 0 && data_len > 0) {}' loop is not necessary. The loop
can only be executed once.

This patch bumps the default FILEIO backend FD_MAX_SECTORS value from
1024 -> 2048 in order to allow block_size=512 to handle 1M sized I/Os.
The current default rejects I/Os larger than 512K in sbc_parse_cdb():

'se_tpg->tpg_lun_list' is malloced in core_tpg_register() and should be freed
before leaving from the error handling cases, otherwise it will cause memory
leak.
'se_tpg' is malloced out of this function, and will be freed if we return error, so
remove free for 'se_tpg'.

I'm not sure why, but the hlist for each entry iterators were conceived
list_for_each_entry(pos, head, member)
The hlist ones were greedy and wanted an extra parameter:
hlist_for_each_entry(tpos, pos, head, member)
Why did they need an extra pos parameter? I'm not quite sure. Not only
they don't really need it, it also prevents the iterator from looking
exactly like the list iterator, which is unfortunate.
Besides the semantic patch, there was some manual work required:
- Fix up the actual hlist iterators in linux/list.h
- Fix up the declaration of other iterators based on the hlist ones.
- A very small amount of places were using the 'node' parameter, this
was modified to use 'obj->member' instead.
- Coccinelle didn't handle the hlist_for_each_entry_safe iterator
properly, so those had to be fixed up manually.

This patch addresses a v3.5+ regression in iscsi-target where TX thread
process context -> handle_response_queue() execution is allowed to run
unbounded while servicing constant outgoing flow of ISTATE_SEND_DATAIN
response state.
This ends up preventing memory release of StatSN acknowledged commands
in a timely manner when under heavy large block streaming DATAIN
workloads.

This patch adds support for emulation of WRITE_SAME w/ UNMAP=0 within
fd_execute_write_same() backend code.
The emulation uses vfs_writev() to submit a locally populated buffer
from the received WRITE_SAME scatterlist block for duplication, and by
default enforces a limit of max_write_same_len=0x1000 (8192) sectors up
to the limit of 1024 iovec entries for the single call to vfs_writev().
It also sets max_write_same_len to the operational default at setup ->
fd_configure_device() time.
Tested with 512, 1k, 2k, and 4k block_sizes.

This patch enforces individual network portal export on a once per TargetName
basis, thus preventing a network portal from being exported multiple times
across multiple TargetPortalGroups in a single TargetName instance.
This is done in iscsit_tpg_check_network_portal() by walking tiqn->tiqn_tpg_list
and tpg->tpg_gnp_list using iscsit_check_np_match() looking for an existing
network portal mapping from iscsit_tpg_add_network_portal() context, but only
when no pre-existing tpg_np_parent pointer is present.

This patch adds missing bounds checking for the configfs provided
mapped_lun value during target_fabric_make_mappedlun() setup ahead
of se_lun_acl initialization.
This addresses a potential OOPs when using a mapped_lun value that
exceeds the hardcoded TRANSPORT_MAX_LUNS_PER_TPG-1 value within
se_node_acl->device_list[].

This patch fixes a bug in core_tpg_check_initiator_node_acl() ->
core_tpg_get_initiator_node_acl() where a dynamically created
se_node_acl generated during session login would be skipped during
subsequent lookup due to the '!acl->dynamic_node_acl' check, causing
a new se_node_acl to be created with a duplicate ->initiatorname.
This would occur when a fabric endpoint was configured with
TFO->tpg_check_demo_mode()=1 + TPF->tpg_check_demo_mode_cache()=1
preventing the release of an existing se_node_acl during se_session
shutdown.
Also, drop the unnecessary usage of core_tpg_get_initiator_node_acl()
within core_dev_init_initiator_node_lun_acl() that originally
required the extra '!acl->dynamic_node_acl' check, and just pass
the configfs provided se_node_acl pointer instead.

An empty parameter list (length == 0) is not an error, so succeed MODE
SELECT in this case. If the parameter list length is too small,
return the correct sense code of PARAMETER LIST LENGTH ERROR.

The PARAMETER LIST LENGTH field specifies the length in bytes of the
UNMAP parameter list that is available to be transferred from the
Data-Out Buffer. If the parameter list length is greater than zero
and less than 0008h (i.e., eight), then the device server shall
terminate the command with CHECK CONDITION status with the sense key
set to ILLEGAL REQUEST and the additional sense code set to
PARAMETER LIST LENGTH ERROR. A PARAMETER LIST LENGTH set to zero
specifies that no data shall be sent.
so our sense code for too-short descriptors was wrong, and we were
incorrectly failing commands that didn't transfer any descriptors.
While we're at it, also handle the UNMAP check:
If the ANCHOR bit is set to one, and the ANC_SUP bit in the Logical
Block Provisioning VPD page (see 6.6.4) is set to zero, then the
device server shall terminate the command with CHECK CONDITION
status with the sense key set to ILLEGAL REQUEST and the additional
sense code set to INVALID FIELD IN CDB.

The loop in rd_execute_rw() will never terminate if the
sg element has a zero size. Or it'll spill over into
outer space if the sg element is larger than the available
space.
So we need to add some safety catches here.

This patch changes LIO to use the configfs backend device name as the
model if you echo '1' to an individual device's emulate_model_alias attribute.
This is a valid operation only on devices with an export count of 0.

The temporary buffer was only 32 characters but ->last_intr_fail_ip_addr
is a 48 character buffer. We don't need to use a temporary buffer at
all, we can just print directly to "page".

"lstat->last_intr_fail_ip_addr" is an array inside the "lstat" struct.
It's never NULL so we always print "ipv6\n" here. The test should be
"if (lstat->last_intr_fail_ip_family == AF_INET6)".
We don't need the temporary buffer either. We could print directly into
"page".

This patch allows IBLOCK to check block hints in request_queue->flush_flags
when reporting current backend device WriteCacheEnabled status to a remote
SCSI initiator port.
This is done via a se_subsystem_api->get_write_cache() call instead of a
backend se_device creation time flag, as we expect REQ_FLUSH bits to possibly
change from an underlying blk_queue_flush() by the SCSI disk driver, or
internal raw struct block_device driver usage.
Also go ahead and update iblock_execute_rw() bio I/O path code to use
REQ_FLUSH + REQ_FUA hints when determining WRITE_FUA usage, and make SPC
emulation code use a spc_check_dev_wce() helper to handle both types of
cases for virtual backend subsystem drivers.

My static checker complains because we use sprintf() to print some
unsigned ints into 10 byte buffers. In theory unsigned ints can take 10
characters and we need another for the terminator.

Sequential scan of rd_dev->sg_table_array in rd_get_sg_table is
a serious I/O performance bottleneck for large rd LUNs. Fix this
by computing the sg_table index directly from page offset because
all sg_tables (except the last one) have the same number of pages.
Tested with 90 GiB rd_mcp LUN, where the patch improved maximal
random R/W IOPS by more than 100-150%, depending on actual
hardware and SAN setup.

We do the same thing no matter which way the test goes, so just remove
the test and do what we're going to do.
The debug messages printed the wrong value of CMD_T_ACTIVE and don't
seem particularly useful, remove them too.

This patch fixes a possible divide by zero bug when the fabric_max_sectors
device attribute is written and backend se_device failed to be successfully
configured -> enabled.
Go ahead and use block_size=512 within se_dev_set_fabric_max_sectors()
in the event of a target_configure_device() failure case, as no valid
dev->dev_attrib.block_size value will have been setup yet.

When transport_lookup_tmr_lun() fails and we return a task management
response from target_complete_tmr_failure(), we need to call
transport_cmd_check_stop_to_fabric() to release the last ref to the
cmd after calling se_tfo->queue_tm_rsp(), or else we will never remove
the failed TMR from the session command list (and we'll end up waiting
forever when trying to tear down the session).

If a backend IO takes a really long then an initiator might abort a
command, and then when it gives up on the abort, send a LUN reset too,
all before we process any of the original command or the abort. (The
abort will wait for the backend IO to complete too)
When the backend IO final completes (or fails), the abort handling
will proceed and queue up a "return aborted status" operation. Then,
while that's still pending, the LUN reset might find the original
command still on the LUN's list of commands and try to return aborted
status again, which leads to a use-after free when the first
se_tfo->queue_status call frees the command and then the second
se_tfo->queue_status call runs.
Fix this by removing a command from the LUN state_list when we first
are about to queue aborted status; we shouldn't do anything
LUN-related after we've started returning status, so this seems like
the correct thing to do.

The following sequence happens for write commands (or any other
commands with a data out phase):
- The transport calls target_submit_cmd(), which sets CMD_T_ACTIVE in
cmd->transport_state and sets cmd->t_state to TRANSPORT_NEW_CMD.
- Things go on transport_generic_new_cmd(), which notices that the
command needs to transfer data, so it sets cmd->t_state to
TRANSPORT_WRITE_PENDING and calls transport_cmd_check_stop().
- transport_cmd_check_stop() clears CMD_T_ACTIVE in cmd->transport_state
and returns in the normal case.
- Then we continue on to call ->se_tfo->write_pending().
- The data comes back from the initiator, and the transport calls
target_execute_cmd(), which sets cmd->t_state to TRANSPORT_PROCESSING
and calls into the backend to actually write the data.
At this point, the backend might take a long time to complete the
command, since it has to do real IO. If an abort request comes in for
this command at this point, it will not wait for the command to finish
since CMD_T_ACTIVE is not set. Then when the command does finally
finish, we blow up with use-after-free.
Avoid this by setting CMD_T_ACTIVE in target_execute_cmd() so that
transport_wait_for_tasks() waits for the command to finish executing.
This matches the behavior from before commit 1389533ef9 ("target:
remove transport_generic_handle_data"), when data was signaled via
transport_generic_handle_data(), which set CMD_T_ACTIVE because it
called transport_add_cmd_to_queue().

Sebastian found a long-standing issue wrt to allocation of full
config_group instead of pointers for config_group->default_group[]
setup in a number of areas, which ends up saving memory with big
configurations. He also managed to fix another long-standing BUG wrt
to broken 32-bit highmem support within the FILEIO backend driver.

Initiator access config could be easier. The way other storage vendors
have addressed this is to support initiator groups: the admin adds
initiator WWNs to the group, and then LUN permissions can be granted for
the entire group at once.
Instead of changing ktarget's configfs interface, this patch keeps
the configfs interface per-initiator-wwn and just adds a 'tag' field
for each. This should be enough for user tools like targetcli to group
initiator ACLs and sync their configurations.
acl_tag is not used internally, but needs to be kept in configfs so that
all user tools can avoid dependencies on each other.
Code tested to work, although userspace pieces still to be implemented.

The lockdep warning below is in theory correct but it will be in really weird
rare situation that ends up that deadlock since the tcm fc session is hashed
based the rport id. Nonetheless, the complaining below is about rcu callback
that does the transport_deregister_session() is happening in softirq, where
transport_register_session() that happens earlier is not. This triggers the
lockdep warning below. So, just fix this to make lockdep happy by disabling
the soft irq before calling transport_register_session() in ft_prli.
BTW, this was found in FCoE VN2VN over two VMs, couple of create and destroy
would get this triggered.

If the TPG memory is allocated successfully, but we fail further along
in the function, a dangling pointer to freed memory is left in the TPort
structure. This is mostly harmless, but does prevent re-trying the
operation without first removing the TPort altogether.

There is no need to memcpy() a 32-bit integer. The data pointer is
guaranteed to be quadlet aligned by the FireWire stack so we can replace
the memcpy() with an assignment.

The kmalloc() + strlen() + memcpy() block is what kstrdup() does as
well. While here I also removed the "to NULL assignment" of pointers
which are fed to kfree or thrown away anyway.

This patch changes vectored file I/O to use kmap + kunmap when mapping
incoming SGL memory -> struct iovec in order to properly support 32-bit
highmem configurations. This is because an extra bounce buffer may be
required when processing scatterlist pages allocated with GFP_KERNEL.

target_fabric_port_link() and target_fabric_mappedlun_link() code to ensure

We already expect TFO->get_blocks() to return sector_t for zero value case
when doing WRITE_SAME to the end of the backend device, so go ahead and return
sector_t from spc_get_write_same_sectors() to handle this case properly.
Also, update the single iblock_execute_write_same() caller of this code.

Only slots 0-5 are used so 6 slots should be enough. I don't see anyone

- If we stop processing an already-aborted command in
target_execute_cmd(), then we need to complete t_transport_stop_comp
to wake up the the TMR handling thread, or else it will end up
waiting forever.
- If we've a already sent an "aborted" status for a command in
transport_check_aborted_status() then we should bail out of
transport_send_task_abort() to avoid freeing the command twice.

This patch adds support for emulation of WRITE_SAME w/ UNMAP=0 within
iblock_execute_write_same() backend code.
The emulation uses a bio_add_page() call for each sector, and by default
enforces a limit of max_write_same_len=0xFFFF (65536) sectors following
what scsi_debug reports per default for MAXIMUM WRITE SAME LENGTH.
It also sets max_write_same_len to the operational default at setup ->
iblock_configure_device() time.

This patch adds a new max_write_same_len device attribute for use with
WRITE_SAME w/ UNMAP=0 backend emulation. This can be useful for
lowering the default backend value (IBLOCK uses 0xFFFF).
Also, update block limits VPD emulation code in spc_emulate_evpd_b0() to
report MAXIMUM WRITE SAME LENGTH, and enforce max_write_same_len during
sbc_parse() -> sbc_setup_write_same() CDB sanity checking for all emulated
WRITE_SAME w/ UNMAP=0 cases.

with WRITE_SAME w/ UNMAP=1, and performs the ->execute_cmd() setup based
this bit within sbc_setup_write_same() code.

There are some cases, for example when the initiator sends an
out-of-bounds ErrorRecoveryLevel value, where the iSCSI target
terminates the connection without sending back any error. Audit the
login path and add appropriate iscsit_tx_login_rsp() calls to make
sure this doesn't happen.

This patch fixes a bug in the hanlding of initiator provided ExpStatSN and
individual iscsi_cmd->stat_sn comparision during iscsi_conn->stat_sn
wrap-around within iscsit_ack_from_expstatsn() code.
This bug would manifest itself as iscsi_cmd descriptors not being Acked
by a lower ExpStatSn, causing them to be leaked until an iSCSI connection
or session reinstatement event occurs to release all commands.
Also fix up two other uses of incorrect CmdSN SNA comparison to use wrapper
usage from include/scsi/iscsi_proto.h.

We need to disable BHs when taking sess_idr_lock because the
iscsit_handle_time2retain_timeout() timer function takes
se_tpg->session_lock, and iscsit_close_session() nests sess_idr_lock
inside se_tpg->session_lock. So if the timer can run inside
sess_idr_lock, we have a potential AB-BA deadlock.
Fix this by disabling BHs when taking sess_idr_lock. This was found
because of a lockdep warning, but it looks like a real (if highly
theoretical) deadlock. In any case avoiding lockdep spew so that we can
find other issues is a worthy cause.

This patch fixes a possible case in transport_generic_new_cmd() where a
failure from TFO->write_pending() from a fabric module return something
other than -EAGAIN or -ENOMEM would cause a failed WRITE to silently
succeed.
Go ahead and return TCM_LOGICAL_UNIT_COMMUNICATION_FAILURE for this
special case instead of only just making noise with WARN_ON().

Pass the sense reason as an explicit return value from the I/O submission
path instead of storing it in struct se_cmd and using negative return
values. This cleans up a lot of the code pathes, and with the sparse
annotations for the new sense_reason_t type allows for much better
error checking.

This patch fixes a new off-by-one bug in the hardcoded starting offset of
spc_emulate_modesense() code that causes BLOCK DESCRIPTOR to be incorrectly
written within the MEDIUM TYPE buffer area of the mode parameter header.
According to spc4r30, Section 7.5.4, BLOCK DESCRIPTOR for MODE_SENSE_10
starts at byte 3, and BLOCK_DESCRIPTOR for MODE_SENSE (6) starts at byte 2.

Convert spc_emulate_modesense() to use a table of mode pages, rather
than a switch statement. This makes it possible to add more pages
sanely -- in particular we no longer need to make sure we keep the
0x3f (return all mode pages) case in sync.
While we're touching this code, make our MODE SENSE emulation a bit
better in a couple of ways:
- When the initiator passes PC == 1 asking for changeable values,
return all 0s to show we don't support setting anything.
- Return a block descriptor for disk devices.

Now that the reservations and ALUA code have been cleaned up there is no need
for the get_device_rev method, as we only need the standards revision in the
inquiry data, where we can hardcode it.

We do not support host-level reservations for the pscsi backend, and all
virtual backends are newere than SCSI-2, so just make the combined
SPC-3 + SCSI-2 support the only supported variant and kill the switches
for the different implementations, given that this code handles the no-op
version just fine.

Simplify the code a lot by killing the superflous struct se_subsystem_dev.
Instead se_device is allocated early on by the backend driver, which allocates
it as part of its own per-device structure, borrowing the scheme that is for
example used for inode allocation.

The sleeping code in iscsi_target_tx_thread() is susceptible to the classic
missed wakeup race:
- TX thread finishes handle_immediate_queue() and handle_response_queue(),
thinks both queues are empty.
- Another thread adds a queue entry and does wake_up_process(), which does
nothing because the TX thread is still awake.
- TX thread does schedule_timeout() and sleeps forever.
In practice this can kill an iSCSI connection if for example an initiator
does single-threaded writes and the target misses the wakeup window when
queueing an R2T; in this case the connection will be stuck until the
initiator loses patience and does some task management operation (or kills
the connection entirely).
Fix this by converting to wait_event_interruptible(), which does not
suffer from this sort of race.

The expression (max_sectors * block_size) might overflow a u32
(indeed, since iblock sets max_hw_sectors to UINT_MAX, it is
guaranteed to overflow and end up with a much-too-small result in many
common cases). Fix this by doing an equivalent calculation that
doesn't require multiplication.
While we're touching this code, avoid splitting a printk format across
two lines and use pr_info(...) instead of printk(KERN_INFO ...).

If the call to core_dev_release_virtual_lun0() fails, then nothing
sets ret to anything other than 0, so even though everything is
torn down and freed, target_core_init_configfs() will seem to succeed
and the module will be loaded. Fix this by passing the return value
on up the chain.

This patch fixes a regression in spc_emulate_inquiry() code where the
local scope bounce buffer was no longer getting it's memory zeroed,
causing various problems with SCSI initiators that depend upon areas
of INQUIRY EVPD=0x83 payload having been zeroed.
This bug was introduced with the following v3.7-rc1 patch + CC'ed
stable commit:

With kernel 3.6 some obsolete SCSI-2 commands including SEEK_10 have
have been removed by commit 1fd032ee10
"target: move code for CDB emulation".
There are still clients out there which use these old SCSI-2 commands.
This mainly happens when running VMs with legacy guest systems,
connected via SCSI command pass-through to iSCSI targets. Make them
happy and return status GOOD.
Many real SCSI disks or external iSCSI storage devices still support
these old commands. So let's make LIO backward compatible as well.
This patch adds support for the previously removed SEEK_10 and
additionally the SEEK_6 and REZERO_UNIT commands.

Fabric drivers currently expect to internally release se_cmd in the event
of a TMR failure during target_submit_tmr(), which means the immediate call
to transport_generic_free_cmd() after TFO->queue_tm_rsp() from within
target_complete_tmr_failure() workqueue context is wrong.
This is done as some fabrics expect TMR operations to be acknowledged
before releasing the descriptor, so the assumption that core is releasing
se_cmd associated TMR memory is incorrect. This fixes a OOPs where
transport_generic_free_cmd() was being called more than once.
This bug was originally observed with tcm_qla2xxx fabric ports.

Pull block IO update from Jens Axboe:
"Core block IO bits for 3.7. Not a huge round this time, it contains:
- First series from Kent cleaning up and generalizing bio allocation
and freeing.
- WRITE_SAME support from Martin.
- Mikulas patches to prevent O_DIRECT crashes when someone changes
the block size of a device.
- Make bio_split() work on data-less bio's (like trim/discards).
- A few other minor fixups."
Fixed up silent semantic mis-merge as per Mikulas Patocka and Andrew
Morton. It is due to the VM no longer using a prio-tree (see commit
6b2dbba8b6: "mm: replace vma prio_tree with an interval tree").
So make set_blocksize() use mapping_mapped() instead of open-coding the
internal VM knowledge that has changed.

This patch increases the default for nopin_timeout to 15 seconds (wait
between sending a new NopIN ping) and nopin_response_timeout to 30 seconds
(wait for NopOUT response before failing the connection) in order to avoid
false positives by iSCSI Initiators who are not always able (under load) to
respond to NopIN echo PING requests within the current 5 second window.
False positives have been observed recently using Open-iSCSI code on v3.3.x
with heavy large-block READ workloads over small MTU 1 Gb/sec ports, and
increasing these values to more reasonable defaults significantly reduces
the possibility of false positive NopIN response timeout events under
this specific workload.
Historically these have been set low to initiate connection recovery as
soon as possible if we don't hear a ping back, but for modern v3.x code
on 1 -> 10 Gb/sec ports these new defaults make alot more sense.

This patch carries forward a work-around from tcm_loop to target
core code to explicitly clear control CDB READ paylods in order to
avoid bugs in scsi-generic user-space code for INQUIRY that do not
explicitly zero CDB payload memory.

This patch converts tcm_loop to use target_submit_cmd_map_sgls() for
I/O submission and mapping of pre-allocated SGL memory from incoming
scsi_cmnd -> se_cmd descriptors.
This includes removing the original open-coded fabric uses of target
core callers to support transport_generic_map_mem_to_cmd() between
target_setup_cmd_from_cdb() and transport_handle_cdb_direct() logic.

This patch adds a new target_submit_cmd_map_sgls() to pass pre-allocated
SGL memory using transport_generic_map_mem_to_cmd() logic into the generic
target submit I/O codepath.
It also adds a target_submit_cmd() wrapper around target_submit_cmd_map_sgls()
for existing fabric code that already assumes internal target-core SGL memory
allocation.

We've had reports in the past about this specific case, so it's time to
go ahead and explicitly set cache_dynamic_acls=1 for generate_node_acls=1
(TPG demo-mode) operation.
During normal generate_node_acls=0 operation with explicit NodeACLs ->
se_node_acl memory is persistent to the configfs group located at
/sys/kernel/config/target/$TARGETNAME/$TPGT/acls/$INITIATORNAME, so in
the generate_node_acls=1 case we want the reservation logic to reference
existing per initiator IQN se_node_acl memory (not to generate a new
se_node_acl), so go ahead and always set cache_dynamic_acls=1 when
TPG demo-mode is enabled.
