c421530bf8 scsi: aacraid: Reorder Adapter status check
The driver currently checks the SELF_TEST_FAILED first and then
KERNEL_PANIC next. Under error conditions(boot code failure) both
SELF_TEST_FAILED and KERNEL_PANIC can be set at the same time.

The driver has the capability to reset the controller on an KERNEL_PANIC,
but not on SELF_TEST_FAILED.

Fixed by first checking KERNEL_PANIC and then the others.

146aa1786d scsi: aacraid: Skip IOP reset on controller panic(SMART Family)
When the SMART family of controller panic (KERNEL_PANIC) , they do not
honor IOP resets. So better to skip it and directly perform a IWBR reset.

11da1b7c48 scsi: aacraid: Decrease adapter health check interval
Currently driver checks the health status of the adapter once every 24
hours. When that happens the driver becomes dependent on the kernel to
figure out if the  adapter is misbehaving. This might take some time
(when the adapter is idle). The driver currently has support to
restart/recover the controller when it fails, and decreasing the time
interval will help.

Fixed by decreasing check interval from 24 hours to 1 minute

a2d0321dd5 scsi: aacraid: Reload offlined drives after controller reset
During the IOP reset stress testing, it was found that the drives can be
marked offline when the adapter controller crashes and IO's are running
in parallel. When the controller  does come back from the reset, the drive
that is marked offline is not exposed.

Fixed by removing and adding drives that are marked offline. In addition
invoke a scsi host bus rescan to capture any additional configuration
changes.

849ac6a591 scsi: aacraid: Skip wellness sync on controller failure
aac_command_thread checks on the health of controller periodically,
using aac_check_health. If the status is an error state KERNEL_PANIC or
anything else. The driver will attempt to restart the adapter, but the
response is not checked in aac_command_thread. This allows the periodic
sync to go thru and lead the driver to a hung state.

Fixed by terminating the periodic loop(intended per original design),
if the controller is not restored to a healthy state.

a7e2c64284 scsi: aacraid: Fix sync fibs time out on controller reset
After controller shutdown, all sync fibs time out due to not knowing
about the switch to INT-x mode

Fixed by replacing aac_src_access_devreg() to aac_set_intx_mode() call.

aac_fib_map_free frees misaligned fib dma memory, additionally it does not
free up the whole memory.

Fixed by changing the  code to free up the correct and full memory
allocation.

a0c6143e95 scsi: aacraid: Prevent E3 lockup when deleting units
Arrconf management utility at times sends fibs with AdapterProcessed set
in its fibs. This causes the controller to panic and lockup.

Fixed by failing the commands that have AdapterProcessed set in its flag.

16ae9dd35d scsi: aacraid: Fix for excessive prints on EEH
This issue showed up on a kdump debug(single CPU on powerkvm), when EEH
errors rendered the adapter unusable. The driver correctly detected the
issue and attempted to restart the controller, in doing so the driver
attempted to read the status registers of the controller. This triggered
additional eeh errors which continued for a good 6 minutes.

Fixed by returning without waiting when EEH error is reported.

f3ef4a74dc scsi: aacraid: Use correct channel number for raw srb
The channel being used for raw srb commands is retrieved from the utility
sent fibs and is converted into physical channel id. The driver does not
need to to do this since the management utility sends the correct channel
id in the first place and in addition the driver sets inaccurate
information in the cmd sent to the firmware and gets an invalid response.

Fixed by using channel id from srb command.

8af8e1c22f scsi: aacraid: Fix INTx/MSI-x issue with older controllers
commit 78cbccd3bd ("aacraid: Fix for KDUMP driver hang")

caused a problem on older controllers which do not support MSI-x (namely
ASR3405,ASR3805). This patch conditionalizes the previous patch to
controllers which support MSI-x

Shifting a dma_addr_t right by 32 bits causes a compile-time warning
when that type is only 32 bit wide:

drivers/scsi/aacraid/src.c:414:29: error: right shift count >= width of type [-Werror=shift-count-overflow]

This changes the driver to use the predefined macros consistently,
including one correct but open-coded upper_32_bits() instance.

7629146977 scsi: aacraid: rcode is unsigned and should be signed int
aac_fib_send can return -ve error returns and hence rcode should be
signed. Currently the rcode >= 0 check is always true and -ve errors are
not being checked.

c4e2fbca37 scsi: aacraid: Reworked scsi command submission path
Moved the READ and WRITE switch cases to the top. Added a  default
case to the switch case and replaced duplicate scsi result value with a
macro.

The idea is that since most of scsi commands we care about performance
wise are read or write, we need to process them first.

Internally the compiler (GCC) converts a switch case into either a jump
table or a bunch of if else conditions, so placing the often used read,
write cases at the top is an effort in optimization.

fa00c437ee aacraid: Check size values after double-fetch from user
In aacraid's ioctl_send_fib() we do two fetches from userspace, one the
get the fib header's size and one for the fib itself. Later we use the
size field from the second fetch to further process the fib. If for some
reason the size from the second fetch is different than from the first
fix, we may encounter an out-of- bounds access in aac_fib_send(). We
also check the sender size to insure it is not out of bounds. This was
reported in https://bugzilla.kernel.org/show_bug.cgi?id=116751 and was
assigned CVE-2016-6480.

78cbccd3bd aacraid: Fix for KDUMP driver hang
When KDUMP is triggered the driver first talks to the firmware in INTX
mode, but the adapter firmware is still in MSIX mode. Therefore the first
driver command hangs since the driver is waiting for an INTX response and
firmware gives a MSIX response. If when the OS is installed on a RAID
drive created by the adapter KDUMP will hang since the driver does not
receive a response in sync mode.

Fixed by: Change the firmware to INTX mode if it is in MSIX mode before
sending the first sync command.

Currently driver completes double completed or spurious interrupted fibs.
This  is not necessary and causes the SCSI mid layer to issue aborts and
resets, since completing a fib  prematurely might trigger a race condition
resulting in the driver not calling the scsi_done callback.

Fixed by removing the call to fib complete.

fc4bf75ea3 aacraid: Fix for aac_command_thread hang
Typically under error conditions, it is possible for aac_command_thread()
to miss the wakeup from kthread_stop() and go back to sleep, causing it
to hang aac_shutdown.

In the observed scenario, the adapter is not functioning correctly and so
aac_fib_send() never completes (or time-outs depending on how it was
called). Shortly after aac_command_thread() starts it performs
aac_fib_send(SendHostTime) which hangs. When aac_probe_one
/aac_get_adapter_info send time outs, kthread_stop is called which breaks
the command thread out of it's hang.

The code will still go back to sleep in schedule_timeout() without
checking kthread_should_stop() so it causes aac_probe_one to hang until
the schedule_timeout() which is 30 minutes.

Fixed by: Adding another kthread_should_stop() before schedule_timeout()

07beca2be2 aacraid: Relinquish CPU during timeout wait
aac_fib_send has a special function case for initial commands during
driver initialization using wait < 0(pseudo sync mode). In this case,
the command does not sleep but rather spins checking for timeout.This
loop is calls cpu_relax() in an attempt to allow other processes/threads
to use the CPU, but this function does not relinquish the CPU and so the
command will hog the processor. This was observed in a KDUMP
"crashkernel" and that prevented the "command thread" (which is
responsible for completing the command from being timed out) from
starting because it could not get the CPU.

Fixed by replacing "cpu_relax()" call with "schedule()"

During EEH PCI hotplug activity kernel unloads and loads the driver,
causing character device to be unregistered(aac_remove_one).When the
driver is loaded back using aac_probe_one the character device needs
to be registered again for the AIF management tools to work.

Fixed by adding code to register character device in aac_probe_one if
it is unregistered in aac_remove_one.

aac_mutex was used to create protect the ioctl path for only the compat
path, it would be make more sense to place mutex in aac_do_ioctl, which
is the main ioctl function call that handles all ioctl commands.

Created new mutex ioctl_mutex in struct aac_dev to protect switch case
in aac_do_ioctl and removed aac_mutex from aac_cfg_ioctl and
aac_compat_do_ioctl

ecc479e00d aacraid: Set correct msix count for EEH recovery
During EEH recovery number of online CPU's might change thereby changing
the number of MSIx vectors. Since each fib is allocated to a vector,
changes in the number of vectors causes fib to be sent thru invalid
vectors.In addition the correct number of MSIx vectors is not updated in
the INIT struct sent to the controller, when it is reinitialized.

Fixed by reassigning vectors to fibs based on the updated number of MSIx
vectors and updating the INIT structure before sending to controller.

f88fa79a61 aacraid: Fix memory leak in aac_fib_map_free
aac_fib_map_free() calls pci_free_consistent() without checking that
dev->hw_fib_va is not NULL and dev->max_fib_size is not zero.If they are
indeed NULL/0, this will result in a hang as pci_free_consistent() will
attempt to invalidate cache for the entire 64-bit address space
(which would take a very long time).

Fixed by adding a check to make sure that dev->hw_fib_va and
dev->max_fib_size are not NULL and 0 respectively.

3f4ce057d5 aacraid: Fix RRQ overload
The driver utilizes an array of atomic variables to keep track of IO
submissions to each vector. To submit an IO multiple threads iterate
through the array to find a vector which has empty slots to send an
IO. The reading and updating of the variable is not atomic, causing race
conditions when a thread uses a full vector to submit an IO.

Fixed by mapping each FIB to a vector, the submission path then uses
said vector to submit IO thereby removing the possibly of a race
condition.The vector assignment is started from 1 since vector 0 is
reserved for the use of AIF management FIBS.If the number of MSIx
vectors is 1 (MSI or INTx mode) then all the fibs are allocated to
vector 0.

6bf3b630d0 aacraid: SCSI blk tag support
The method to allocate and free FIB's in the present code utilizes
spinlocks. Multiple IO's have to wait on the spinlock to acquire or free
fibs creating a performance bottleneck.

An alternative solution would be to use block layer tags to keep track
of the fibs allocated and freed. To this end aac_fib_alloc_tag was
created to utilize the blk layer tags to plug into the Fib pool.These
functions are used exclusively in the IO path. 8 fibs are reserved for
the use of AIF management software and utilize the previous spinlock
based implementations.

f9c4259678 aacraid: IOCTL fix
After getting the platform shutdown command "VM_CloseAll" response from the
firmware, driver was getting configuration IOCTL request from the upper layers
and it sends down to firmware. This causes firmware assert issue.

This patch fixes the firmware assert issue. During the shutdown, if driver
gets commands from the upper layer, driver sends error code to the upper
layers.

This patch fixes the IOP_RESET issue. Sending IOP_RESET command need to wait
for only 10 sec instead of 5 minutes in case of firmware does not response
IOP_RESET command.  Disable interrupt before setup interrupt routine to
prevent spurious interrupts.

c8b09f6fb6 scsi: don't set tagging state from scsi_adjust_queue_depth
Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth.  For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.

Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway.  The one other case looks
broken, but I've kept the behavior as-is for now.

Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.

Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

64bdcbc449 scsi: add use_cmd_list flag
Add a use_cmd_list flag in struct Scsi_Host to request keeping track of
all outstanding commands per device.

Default behaviour is not to keep track of cmd_list per sdev, as this may
introduce lock contention. (overhead is more on multi-node NUMA.), and
only enable it on the two drivers that need it.
