Current design has the fcloop job struct, used for both initiator and
target processing, allocated as part of the initiator request structure.
On aborts, the initiator side (based on the request) may terminate, yet
the target side wants to continue processing. the target side can't do
that if the initiator side goes away.
Revise fcloop to allocate an independent target side structure when it
starts an io from the initiator.
Added a lock to the request struct as well to synchronize pointer updates
on abort calls.
Modified target downcalls to recognize conditions where initiator has
aborted the io (thus nulled the pointer between job structs), thus
avoid referencing sgl lists which are gone and no longer making upcalls
to the initiator.
In conditions where the targetport is no longer connected, have the
initiator return an access failure rather than simulating a command
completion.

With the advent of the opdone calls changing context, the lldd can no
longer assume that once the op->done call returns for RSP operations
that the request struct is no longer being accessed.
As such, revise the lldd api for a req_release callback that the
transport will call when the job is complete. This will also be used
with abort cases.
Fixed text in api header for change in io complete semantics.
Revised lpfc to support the new req_release api.

Two new feature flags were added to control whether upcalls to the
transport result in context switches or stay in the calling context.
NVMET_FCTGTFEAT_CMD_IN_ISR:
By default, if the flag is not set, the transport assumes the
lldd is in a non-isr context and in the cpu context it should be
for the io queue. As such, the cmd handler is called directly in the
calling context.
If the flag is set, indicating the upcall is an isr context, the
transport mandates a transition to a workqueue. The workqueue assigned
to the queue is used for the context.

This is safer as it doesn't rely on the data being stored in
a single page in an sgl.
It also aids our effort to start phasing out users of sg_page. See [1].
For this we kmalloc some memory, copy to it and free at the end. Note:
we can't allocate this memory on the stack as the kbuild test robot
reports some frame size overflows on i386.

We want our own clearly defined error field for NVMe passthrough commands,
and the request errors field is going away in its current form.
Just store the status and result field in the nvme_request field from
hardirq completion context (using a new helper) and then generate a
Linux errno for the block layer only when we actually need it.
Because we can't overload the status value with a negative error code
for cancelled command we now have a flags filed in struct nvme_request
that contains a bit for this condition.

We've added a considerable amount of fixes for stalls and issues
with the blk-mq scheduling in the 4.11 series since forking
off the for-4.12/block branch. We need to do improvements on

This avoids duplicating the logic four times, and it also allows to keep
some helpers static in core.c or just opencode them.
Note that this loses printing the aborted status on completions in the
PCI driver as that uses a data structure not available any more.

This patch introduces helper function for checking controller
status during admin and io command processing which returns u16
status. As to bring consistency on returning status, other
friend functions also now return u16 status instead of int
to match the spec.
As part of the theseerror log prints in also prints qid on
which command error occured.

If we are attacked with establishments/teradowns we need to

When handling a new recv command, we grab a new rsp resource and
check for the queue state being live. In case the queue is not in
live state, we simply restore the rsp back to the free list. However
in this flow we didn't set rsp->queue yet, so we cannot dereference it.
Instead, make sure to initialize rsp->queue (and other rsp members)
as soon as possible so we won't reference uninitialized variables.

The check to see if ret is non-zero and return this rather than count
is redundant in two occassions. It is redundant because prior to this

The Set Features implementation for Keep Alive Timer was using the wrong
structure when retrieving the KATO value; it was treating the Set
Features command as a Property Set command.
The NVMe spec defines the Keep Alive Timer feature as having one input
in CDW11 (4 bytes at offset 44 in the command) whereas the code was
reading 8 bytes at offset 48.
Since the Linux NVMe over Fabrics host never sets this feature, this
code has presumably never been tested.

Instead of allocating a single unused biovec for discard requests, send
them down without any payload. Instead we allow the driver to add a
"special" payload using a biovec embedded into struct request (unioned
over other fields never used while in the driver), and overloading
the number of segments for this case.
This has a couple of advantages:
- we don't have to allocate the bio_vec
- the amount of special casing for discard requests in the block
layer is significantly reduced
- using this same scheme for other request types is trivial,
which will be important for implementing the new WRITE_ZEROES
op on devices where it actually requires a payload (e.g. SCSI)
- we can get rid of playing games with the request length, as
we'll never touch it and completions will work just fine
- it will allow us to support ranged discard operations in the
future by merging non-contiguous discard bios into a single
request
- last but not least it removes a lot of code
This patch is the common base for my WIP series for ranges discards and to
remove discard_zeroes_data in favor of always using REQ_OP_WRITE_ZEROES,
so it would be good to get it in quickly.

Add FC LLDD loopback driver to test FC host and target transport within
nvme-fabrics
To aid in the development and testing of the lower-level api of the FC
transport, this loopback driver has been created to act as if it were a
FC hba driver supporting both the host interfaces as well as the target
interfaces with the nvme FC transport.

Implements the FC-NVME T11 definition of how nvme fabric capsules are
performed on an FC fabric. Utilizes a lower-layer API to FC host adapters
to send/receive FC-4 LS operations and perform the FCP transactions
necessary to perform and FCP IO request for NVME.
The T11 definitions for FC-4 Link Services are implemented which create
NVMeOF connections. Implements the hooks with nvmet layer to pass NVME
commands to it for processing and posting of data/response base to the
host via the different connections.

Currently, core.c sets command_id only on rd/wr commands, leaving it to
the transport to set it again to ensure the request had a command id.
Move location of set in core so applies to all commands.

When removing a namespace we delete it from the subsystem namespaces
list with list_del_init which allows us to know if it is enabled or
not.
The problem is that list_del_init initialize the list next and does
not respect the RCU list-traversal we do on the IO path for locating
a namespace. Instead we need to use list_del_rcu which is allowed to
run concurrently with the _rcu list-traversal primitives (keeps list
next intact) and guarantees concurrent nvmet_find_naespace forward
progress.
By changing that, we cannot rely on ns->dev_link for knowing if the
namspace is enabled, so add enabled indicator entry to nvmet_ns for
that.

Add support for handling write zeroes command on target.
Call into __blkdev_issue_zeroout, which the block layer expands into the
best suitable variant of zeroing the LBAs. Allow write zeroes operation
to deallocate the LBAs when calling __blkdev_issue_zeroout.

Documentation/filesystems/configfs/configfs.txt says:
"When unlink(2) is called on the symbolic link, the source item is
notified via the ->drop_link() method. Like the ->drop_item() method,
this is a void function and cannot return failure."
The ->drop_item() is indeed a void function, the ->drop_link() is
actually not. This, together with the fact that the value of ->drop_link()
is silently ignored suggests, that it is the ->drop_link() return
type that should be corrected and changed to void.
This patch changes drop_link() signature and all its users.

Some drivers often use external bvec table, so introduce
this helper for this case. It is always safe to access the
bio->bi_io_vec in this way for this case.
After converting to this usage, it will becomes a bit easier
to evaluate the remaining direct access to bio->bi_io_vec,
so it can help to prepare for the following multipage bvec
support.

draining the qp right after disconnect might not suffice because
the nvmet sq is not fully drained (in nvmet_sq_destroy) and we might
see completions after the drain. Instead, drain right before the
qp destroy which comes after the sq destruction and we can be sure
that no posts come after the drain.

In case we accepted a queue connection and it failed, we might not
remove the queue from the list until we unload and clean it up.
We should delete it from the queue list on the relevant handler.

In the transport, in case of an interal queue error like
error completion in rdma we trigger a fatal error. However,
multiple queues in the same controller can serr error completions
and we don't want to trigger fatal error work more than once.

When we initiate queue teardown sequence we call rdma_destroy_qp
which clears cm_id->qp, afterwards we call rdma_destroy_id, but
we might see a rdma_cm event in between with a cleared cm_id->qp
so watch out for that and silently ignore the event because this
means that the queue teardown sequence is in progress.

This adds a shared per-request structure for all NVMe I/O. This structure
is embedded as the first member in all NVMe transport drivers request
private data and allows to implement common functionality between the
drivers.
The first use is to replace the current abuse of the SCSI command
passthrough fields in struct request for the NVMe command passthrough,
but it will grow a field more fields to allow implementing things
like common abort handlers in the future.
The passthrough commands are handled by having a pointer to the SQE
(struct nvme_command) in struct nvme_request, and the union of the
possible result fields, which had to be turned from an anonymous
into a named union for that purpose. This avoids having to pass
a reference to a full CQE around and thus makes checking the result
a lot more lightweight.

Import a few updates to nvme.h from nvme-cli. This mostly includes a few
new fields and error codes, but also a few renames that so far are only
used in user space. Also one field is moved from an array of two le64
values to one of 16 u8 values so that we can more easily access it.

We're designed to work with high-end devices where
direct IO makes perfect sense. We noticed that we
context switch by scheduling kblockd instead of going
directly to the device without REQ_SYNC for writes.

Instead of exposing ib_get_dma_mr to ULPs and letting them use it more or
less unchecked, this moves the capability of creating a global rkey into
the RDMA core, where it can be easily audited. It also prints a warning
everytime this feature is used as well.

All drivers use the default, so provide an inline version of it. If we
ever need other queue mapping we can add an optional method back,
although supporting will also require major changes to the queue setup
code.
This provides better code generation, and better debugability as well.

Under extreme conditions this might cause data corruptions. By doing that
we we repost the buffer and then post this buffer for the device to send.
If we happen to use shared receive queues the device might write to the
buffer before it sends it (there is no ordering between send and recv
queues). Without SRQs we probably won't get that if the host doesn't
mis-behave and send more than we allowed it, but relying on that is not
really a good idea.

When configuring a device attached listener, we may
see device removal events. In this case we return a
non-zero return code from the cm event handler which
implicitly destroys the cm_id. It is possible that in
the future the user will remove this listener and by
that trigger a second call to rdma_destroy_id on an
already destroyed cm_id -> BUG.
In addition, when a queue bound (active session) cm_id
generates a DEVICE_REMOVAL event we must guarantee all
resources are cleaned up by the time we return from the
event handler.
Introduce nvmet_rdma_device_removal which addresses
(or at least attempts to) both scenarios.

nvme_uninit_ctrl already does that for us. Note that we
reordered nvme_loop_shutdown_ctrl with nvme_uninit_ctrl
but its safe because we want controller uninit to happen
before we shutdown the transport resources.

In case of error, the function kstrndup() returns NULL pointer
not ERR_PTR(). The IS_ERR() test in the return value check
should be replaced with NULL test.

This patch implements the RDMA transport for the NVMe over Fabrics target,
which allows exporting NVMe over Fabrics functionality over RDMA fabrics
(Infiniband, RoCE, iWARP).
All NVMe logic is in the generic target and this module just provides a
small glue between it and the generic code in the RDMA subsystem.

CONFIG_NVME_TARGET has a correct CONFIG_CONFIGFS_FS dependency, but the
newly added NVME_TARGET_LOOP is missing this, resulting in a link
failure:

This patch implements adds nvme-loop which allows to access local devices
exported as NVMe over Fabrics namespaces. This module can be useful for
easy evaluation, testing and also feature experimentation.
To createa nvme-loop device you need to configure the NVMe target to
export a loop port (see the nvmetcli documentaton for that) and then
connect to it using
nvme connect-all -t loop
which requires the very latest nvme-cli version with Fabrics support.

This patch introduces a implementation of NVMe subsystems,
controllers and discovery service which allows to export
NVMe namespaces across fabrics such as Ethernet, FC etc.
The implementation conforms to the NVMe 1.2.1 specification
and interoperates with NVMe over fabrics host implementations.
Configuration works using configfs, and is best performed using
the nvmetcli tool from http://git.infradead.org/users/hch/nvmetcli.git,
which also has a detailed explanation of the required steps in the
README file.
