51bbf9bee3 PCI: hotplug: Demidlayer registration with the core
When a hotplug driver calls pci_hp_register(), all steps necessary for
registration are carried out in one go, including creation of a kobject
and addition to sysfs.  That's a problem for pciehp once it's converted
to enable/disable the slot exclusively from the IRQ thread:  The thread
needs to be spawned after creation of the kobject (because it uses the
kobject's name), but before addition to sysfs (because it will handle
enable/disable requests submitted via sysfs).

pci_hp_deregister() does offer a ->release callback that's invoked
after deletion from sysfs and before destruction of the kobject.  But
because pci_hp_register() doesn't offer a counterpart, hotplug drivers'
->probe and ->remove code becomes asymmetric, which is error prone
as recently discovered use-after-free bugs in pciehp's ->remove hook
have shown.

In a sense, this appears to be a case of the midlayer antipattern:

    so tempting to put in a midlayer should instead be provided as

The presence of midlayer traits in the PCI hotplug core might be ascribed
to its age:  When it was introduced in February 2002, the blessings of a
library approach might not have been well known:

For comparison, the driver core does offer split functions for creating
a kobject (device_initialize()) and addition to sysfs (device_add()) as
an alternative to carrying out everything at once (device_register()).

The odd ->release callback in the PCI hotplug core was added in 2003:

Clearly, a library approach would not force every hotplug driver to
implement a ->release callback, but rather allow the driver to remove
the sysfs files, release its data structures and finally destroy the
kobject.  Alternatively, a driver may choose to remove everything with
pci_hp_deregister(), then release its data structures.

To this end, offer drivers pci_hp_initialize() and pci_hp_add() as a
split-up version of pci_hp_register().  Likewise, offer pci_hp_del()
and pci_hp_destroy() as a split-up version of pci_hp_deregister().

Eliminate the ->release callback and move its code into each driver's
teardown routine.

Declare pci_hp_deregister() void, in keeping with the usual kernel
pattern that enablement can fail, but disablement cannot.  It only
returned an error if the caller passed in a NULL pointer or a slot which
has never or is no longer registered or is sharing its name with another
slot.  Those would be bugs, so WARN about them.  Few hotplug drivers
actually checked the return value and those that did only printed a
useless error message to dmesg.  Remove that.

For most drivers the conversion was straightforward since it doesn't
matter whether the code in the ->release callback is executed before or
after destruction of the kobject.  But in the case of ibmphp, it was
unclear to me whether setting slot_cur->ctrl and slot_cur->bus_on to
NULL needs to happen before the kobject is destroyed, so I erred on
the side of caution and ensured that the order stays the same.  Another
nontrivial case is pnv_php, I've found the list and kref logic difficult
to understand, however my impression was that it is safe to delete the
list element and drop the references until after the kobject is
destroyed.

e1e86ee06c PCI: Add SPDX GPL-2.0 to replace COPYING boilerplate
Add SPDX GPL-2.0 to all PCI files that referred to the kernel default
"COPYING" file, which specifies GPL version 2.

Remove the boilerplate language referring to the GPL and "COPYING", relying
on the assertion in b24413180f ("License cleanup: add SPDX GPL-2.0
license identifier to files with no license") that the SPDX identifier may
be used instead of the full boilerplate text.

e044d8f92f ACPI: Introduce acpi_unload_parent_table() usages in Linux kernel
ACPICA has implemented acpi_unload_parent_table() which can exactly replace
the acpi_get_id()/acpi_unload_table_id() implemented in Linux kernel.  The
acpi_unload_parent_table() has been unit tested in ACPICA simulation
environment.

This patch can also help to reduce the source code differences between
Linux and ACPICA.

7b1998116b ACPI / driver core: Store an ACPI device pointer in struct acpi_dev_node
Modify struct acpi_dev_node to contain a pointer to struct acpi_device
associated with the given device object (that is, its ACPI companion
device) instead of an ACPI handle corresponding to it.  Introduce two
new macros for manipulating that pointer in a CONFIG_ACPI-safe way,
ACPI_COMPANION() and ACPI_COMPANION_SET(), and rework the
ACPI_HANDLE() macro to take the above changes into account.
Drop the ACPI_HANDLE_SET() macro entirely and rework its users to
use ACPI_COMPANION_SET() instead.  For some of them who used to
pass the result of acpi_get_child() directly to ACPI_HANDLE_SET()
introduce a helper routine acpi_preset_companion() doing an
equivalent thing.

The main motivation for doing this is that there are things
represented by struct acpi_device objects that don't have valid
ACPI handles (so called fixed ACPI hardware features, such as
power and sleep buttons) and we would like to create platform
device objects for them and "glue" them to their ACPI companions
in the usual way (which currently is impossible due to the
lack of valid ACPI handles).  However, there are more reasons
why it may be useful.

First, struct acpi_device pointers allow of much better type checking
than void pointers which are ACPI handles, so it should be more
difficult to write buggy code using modified struct acpi_dev_node
and the new macros.  Second, the change should help to reduce (over
time) the number of places in which the result of ACPI_HANDLE() is
passed to acpi_bus_get_device() in order to obtain a pointer to the
struct acpi_device associated with the given "physical" device,
because now that pointer is returned by ACPI_COMPANION() directly.
Finally, the change should make it easier to write generic code that
will build both for CONFIG_ACPI set and unset without adding explicit
compiler directives to it.

3757b94802 ACPI / hotplug: Fix concurrency issues and memory leaks
This changeset is aimed at fixing a few different but related
problems in the ACPI hotplug infrastructure.

First of all, since notify handlers may be run in parallel with
acpi_bus_scan(), acpi_bus_trim() and acpi_bus_hot_remove_device()
and some of them are installed for ACPI handles that have no struct
acpi_device objects attached (i.e. before those objects are created),
those notify handlers have to take acpi_scan_lock to prevent races
from taking place (e.g. a struct acpi_device is found to be present
for the given ACPI handle, but right after that it is removed by
acpi_bus_trim() running in parallel to the given notify handler).
Moreover, since some of them call acpi_bus_scan() and
acpi_bus_trim(), this leads to the conclusion that acpi_scan_lock
should be acquired by the callers of these two funtions rather by
these functions themselves.

For these reasons, make all notify handlers that can handle device
addition and eject events take acpi_scan_lock and remove the
acpi_scan_lock locking from acpi_bus_scan() and acpi_bus_trim().
Accordingly, update all of their users to make sure that they
are always called under acpi_scan_lock.

Furthermore, since eject operations are carried out asynchronously
with respect to the notify events that trigger them, with the help
of acpi_bus_hot_remove_device(), even if notify handlers take the
ACPI scan lock, it still is possible that, for example,
acpi_bus_trim() will run between acpi_bus_hot_remove_device() and
the notify handler that scheduled its execution and that
acpi_bus_trim() will remove the device node passed to
acpi_bus_hot_remove_device() for ejection.  In that case, the struct
acpi_device object obtained by acpi_bus_hot_remove_device() will be
invalid and not-so-funny things will ensue.  To protect agaist that,
make the users of acpi_bus_hot_remove_device() run get_device() on
ACPI device node objects that are about to be passed to it and make
acpi_bus_hot_remove_device() run put_device() on them and check if
their ACPI handles are not NULL (make acpi_device_unregister() clear
the device nodes' ACPI handles for that check to work).

Finally, observe that acpi_os_hotplug_execute() actually can fail,
in which case its caller ought to free memory allocated for the
context object to prevent leaks from happening.  It also needs to
run put_device() on the device node that it ran get_device() on
previously in that case.  Modify the code accordingly.

ab6380ef01 PCI: sgihp: Iterate over all devices in slot, not functions 0-7
Iterate through devices in a slot by using the upstream bridge's
"bus->devices" list instead of assuming they are functions 0-7.  It's
possible there are several slots on the same pci_bus, so restrict it to
only devices matching this slot's device number.

ARI (which allows functions 0-255) is a PCIe-only feature, and this is
a PCI hotplug driver, so we shouldn't find anything other than functions
0-7, but it's better to iterate the same way as other hotplug drivers.

The only difference between acpi_bus_scan() and acpi_bus_add() is the
invocation of acpi_update_all_gpes() in the latter which in fact is
unnecessary, because acpi_update_all_gpes() has already been called
by acpi_scan_init() and the way it is implemented guarantees the next
invocations of it to do nothing.

For this reason, drop acpi_bus_add() and make all its callers use
acpi_bus_scan() directly instead of it.  Additionally, rearrange the
code in acpi_scan_init() slightly to improve the visibility of the
acpi_update_all_gpes() call in there.

0cd6ac52b3 ACPI: Make acpi_bus_scan() and acpi_bus_add() take only one argument
The callers of acpi_bus_add() usually assume that if it has
succeeded, then a struct acpi_device object has been attached to
the handle passed as the first argument.  Unfortunately, however,
this assumption is wrong, because acpi_bus_scan(), and acpi_bus_add()
too as a result, may return a pointer to a different struct
acpi_device object on success (it may be an object corresponding to
one of the descendant ACPI nodes in the namespace scope below that
handle).

For this reason, the callers of acpi_bus_add() who care about
whether or not a struct acpi_device object has been created for
its first argument need to check that using acpi_bus_get_device()
anyway, so the second argument of acpi_bus_add() is not really
useful for them.  The same observation applies to acpi_bus_scan()
executed directly from acpi_scan_init().

Therefore modify the relevant callers of acpi_bus_add() to check the
existence of the struct acpi_device in question with the help of
acpi_bus_get_device() and drop the no longer necessary second
argument of acpi_bus_add().  Accordingly, modify acpi_scan_init() to
use acpi_bus_get_device() to get acpi_root and drop the no longer
needed second argument of acpi_bus_scan().

636458de36 ACPI: Remove the arguments of acpi_bus_add() that are not used
Notice that acpi_bus_add() uses only 2 of its 4 arguments and
redefine its header to match the body.  Update all of its callers as
necessary and observe that this leads to quite a number of removed
lines of code (Linus will like that).

Add a kerneldoc comment documenting acpi_bus_add() and wonder how
its callers make wrong assumptions about the second argument (make
note to self to take care of that later).

02f57c67a8 ACPI: Remove acpi_start_single_object() and acpi_bus_start()
The ACPI PCI root bridge driver was the only ACPI driver implementing
the .start() callback, which isn't used by any ACPI drivers any more
now.

For this reason, acpi_start_single_object() has no purpose any more,
so remove it and all references to it.  Also remove
acpi_bus_start_device(), whose only purpose was to call
acpi_start_single_object().

Moreover, since after the removal of acpi_bus_start_device() the
only purpose of acpi_bus_start() remains to call
acpi_update_all_gpes(), move that into acpi_bus_add() and drop
acpi_bus_start() too, remove its header from acpi_bus.h and
update all of its former users accordingly.

This change was previously proposed in a different from by
Yinghai Lu.

5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

85234ce86d PCI: SGI Hotplug: stop managing bss_hotplug_slot->name
We no longer need to manage our version of hotplug_slot->name
since the PCI and hotplug core manage it on our behalf.

Update the sn_hp_slot_private_alloc() interface to fill in
the correct name for us, as that function already has all
the parameters needed to determine the name.

1359f2701b PCI Hotplug core: add 'name' param pci_hp_register interface
Update pci_hp_register() to take a const char *name parameter.

The motivation for this is to clean up the individual hotplug
drivers so that each one does not have to manage its own name.
The PCI core should be the place where we manage the name.

We update the interface and all callsites first, in a
"no functional change" manner, and clean up the drivers later.

acpi_evaluate_integer only supports 64-bit integers on 64-bit platforms.

Detect all physical PCI slots as described by ACPI, and create entries in
/sys/bus/pci/slots/.

Not all physical slots are hotpluggable, and the acpiphp module does not
detect them.  Now we know the physical PCI geography of our system, without
caring about hotplug.

f46753c5e3 PCI: introduce pci_slot
Currently, /sys/bus/pci/slots/ only exposes hotplug attributes when a
hotplug driver is loaded, but PCI slots have attributes such as address,
speed, width, etc.  that are not related to hotplug at all.

Introduce pci_slot as the primary data structure and kobject model.
Hotplug attributes described in hotplug_slot become a secondary
structure associated with the pci_slot.

This patch only creates the infrastructure that allows the separation of
PCI slot attributes and hotplug attributes.  In this patch, the PCI
hotplug core remains the only user of this infrastructure, and thus,
/sys/bus/pci/slots/ will still only become populated when a hotplug
driver is loaded.

A later patch in this series will add a second user of this new
infrastructure and demonstrate splitting the task of exposing pci_slot
attributes from hotplug_slot attributes.

  - Make pci_slot the primary sysfs entity. hotplug_slot becomes a
    subsidiary structure.
    o pci_create_slot() creates and registers a slot with the PCI core
    o pci_slot_add_hotplug() gives it hotplug capability

  - Change the prototype of pci_hp_register() to take the bus and
    slot number (on parent bus) as parameters.

  - Remove all the ->get_address methods since this functionality is
    now handled by pci_slot directly.

3e643e77a9 Altix: Add ACPI SSDT PCI device support (hotplug)
Support for dynamic loading and unloading of ACPI SSDT tables upon slot
hotplugs and unplugs.

On SN platforms, we now represent every populated root bus slot with a single
ACPI SSDT table containing info for every device and PPB attached to the slot.
 These SSDTs are generated by the prom at initial boot and hotplug time.  The
info in these SSDT tables is used by the SN kernel IO "fixup" code (which is
called at boot and hotplug time).

On hotplugs (i.e.  enable_slot()), if running with an ACPI capable prom,
attempt to obtain a new ACPI SSDT table for the slot being hotplugged.  If
successful, add the table to the ACPI namespace (acpi_load_table()) and then
walk the new devices and add them to the ACPI infrastructure (acpi_bus_add()).

On hot unplugs (i.e.  disable_slot()), if running with an ACPI capable prom,
attempt to remove the SSDT table associated with the slot from the ACPI
namespace (acpi_unload_table_id()) and infastructure (acpi_bus_trim()).

1d2450a4a6 [PATCH] PCI Hotplug: SGI hotplug driver fixes
These fixes were suggested by pcihpd-discuss, but were dropped in the
initial checkin of the code.  These fixes include cleaning up the
hotplug driver sysfs filename, and some minor code cleanups.  The driver
also requires at least PROM 4.30, not 4.20.

6f354b014b [IA64] hotplug/ia64: SN Hotplug Driver - SN Hotplug Driver code
This patch is the SGI hotplug driver and additional changes required for
the driver.  These modifications include changes to the SN io_init.c code
for memory management, the inclusion of new SAL calls to enable and disable
PCI slots, and a hotplug-style driver.
