Pull "Disintegrate and delete asm/system.h" from David Howells:
 "Here are a bunch of patches to disintegrate asm/system.h into a set of
  separate bits to relieve the problem of circular inclusion
  dependencies.

  I've built all the working defconfigs from all the arches that I can
  and made sure that they don't break.

  The reason for these patches is that I recently encountered a circular
  dependency problem that came about when I produced some patches to
  optimise get_order() by rewriting it to use ilog2().

  This uses bitops - and on the SH arch asm/bitops.h drags in
  asm-generic/get_order.h by a circuituous route involving asm/system.h.

  The main difficulty seems to be asm/system.h.  It holds a number of
  low level bits with no/few dependencies that are commonly used (eg.
  memory barriers) and a number of bits with more dependencies that
  aren't used in many places (eg.  switch_to()).

  These patches break asm/system.h up into the following core pieces:

    (1) asm/barrier.h

        Move memory barriers here.  This already done for MIPS and Alpha.

    (2) asm/switch_to.h

        Move switch_to() and related stuff here.

    (3) asm/exec.h

        Move arch_align_stack() here.  Other process execution related bits
        could perhaps go here from asm/processor.h.

    (4) asm/cmpxchg.h

        Move xchg() and cmpxchg() here as they're full word atomic ops and
        frequently used by atomic_xchg() and atomic_cmpxchg().

    (5) asm/bug.h

        Move die() and related bits.

    (6) asm/auxvec.h

        Move AT_VECTOR_SIZE_ARCH here.

  Other arch headers are created as needed on a per-arch basis."

Fixed up some conflicts from other header file cleanups and moving code
around that has happened in the meantime, so David's testing is somewhat
weakened by that.  We'll find out anything that got broken and fix it..

69e1aaddd6 Ext4 commits for 3.3 merge window; mostly cleanups and bug fixes
The changes to export dirty_writeback_interval are from Artem's s_dirt
 cleanup patch series.  The same is true of the change to remove the
 s_dirt helper functions which never got used by anyone in-tree.  I've
 run these changes by Al Viro, and am carrying them so that Artem can
 more easily fix up the rest of the file systems during the next merge
 window.  (Originally we had hopped to remove the use of s_dirt from
 ext4 during this merge window, but his patches had some bugs, so I
 ultimately ended dropping them from the ext4 tree.)

3339578f05 jbd2: cleanup journal tail after transaction commit
Normally, we have to issue a cache flush before we can update journal tail in
journal superblock, effectively wiping out old transactions from the journal.
So use the fact that during transaction commit we issue cache flush anyway and
opportunistically push journal tail as far as we can. Since update of journal
superblock is still costly (we have to use WRITE_FUA), we update log tail only
if we can free significant amount of space.

932bb305ba jbd2: remove bh_state lock from checkpointing code
All accesses to checkpointing entries in journal_head are protected
by j_list_lock. Thus __jbd2_journal_remove_checkpoint() doesn't really
need bh_state lock.

Also the only part of journal head that the rest of checkpointing code
needs to check is jh->b_transaction which is safe to read under
j_list_lock.

So we can safely remove bh_state lock from all of checkpointing code which
makes it considerably prettier.

96c866782b jbd2: fix BH_JWrite setting in checkpointing code
BH_JWrite bit should be set when buffer is written to the journal. So
checkpointing shouldn't set this bit when writing out buffer. This didn't
cause any observable bug since BH_JWrite bit is used only for debugging
purposes but it's good to have this consistent.

79feb521a4 jbd2: issue cache flush after checkpointing even with internal journal
When we reach jbd2_cleanup_journal_tail(), there is no guarantee that
checkpointed buffers are on a stable storage - especially if buffers were
written out by jbd2_log_do_checkpoint(), they are likely to be only in disk's
caches. Thus when we update journal superblock effectively removing old
transaction from journal, this write of superblock can get to stable storage
before those checkpointed buffers which can result in filesystem corruption
after a crash. Thus we must unconditionally issue a cache flush before we
update journal superblock in these cases.

A similar problem can also occur if journal superblock is written only in
disk's caches, other transaction starts reusing space of the transaction
cleaned from the log and power failure happens. Subsequent journal replay would
still try to replay the old transaction but some of it's blocks may be already
overwritten by the new transaction. For this reason we must use WRITE_FUA when
updating log tail and we must first write new log tail to disk and update
in-memory information only after that.

With the latest and greatest changes to the freezer, I started seeing
panics that were caused by jbd2 running post-process freezing and
hitting the canary BUG_ON for non-TuxOnIce I/O submission. I've traced
this back to a lack of set_freezable calls in both jbd and jbd2. Since
they're clearly meant to be frozen (there are tests for freezing()), I
submit the following patch to add the missing calls.

a78bb11d7a jbd2: protect all log tail updates with j_checkpoint_mutex
There are some log tail updates that are not protected by j_checkpoint_mutex.
Some of these are harmless because they happen during startup or shutdown but
updates in jbd2_journal_commit_transaction() and jbd2_journal_flush() can
really race with other log tail updates (e.g. someone doing
jbd2_journal_flush() with someone running jbd2_cleanup_journal_tail()). So
protect all log tail updates with j_checkpoint_mutex.

24bcc89c7e jbd2: split updating of journal superblock and marking journal empty
There are three case of updating journal superblock. In the first case, we want
to mark journal as empty (setting s_sequence to 0), in the second case we want
to update log tail, in the third case we want to update s_errno. Split these
cases into separate functions. It makes the code slightly more straightforward
and later patches will make the distinction even more important.

There is normally only a handful of these active at any one time, but
putting them in a separate slab cache makes debugging memory
corruption problems easier.  Manish Katiyar also wanted this make it
easier to test memory failure scenarios in the jbd2 layer.

15291164b2 jbd2: clear BH_Delay & BH_Unwritten in journal_unmap_buffer
journal_unmap_buffer()'s zap_buffer: code clears a lot of buffer head
state ala discard_buffer(), but does not touch _Delay or _Unwritten as
discard_buffer() does.

This can be problematic in some areas of the ext4 code which assume
that if they have found a buffer marked unwritten or delay, then it's
a live one.  Perhaps those spots should check whether it is mapped
as well, but if jbd2 is going to tear down a buffer, let's really
tear it down completely.

Without this I get some fsx failures on sub-page-block filesystems
up until v3.2, at which point 4e96b2dbbf
and 189e868fa8 make the failures go
away, because buried within that large change is some more flag
clearing.  I still think it's worth doing in jbd2, since
->invalidatepage leads here directly, and it's the right place
to clear away these flags.

1ba37268cd jbd2: clear revoked flag on buffers before a new transaction started
Currently, we clear revoked flag only when a block is reused.  However,
this can tigger a false journal error.  Consider a situation when a block
is used as a meta block and is deleted(revoked) in ordered mode, then the
block is allocated as a data block to a file.  At this moment, user changes
the file's journal mode from ordered to journaled and truncates the file.
The block will be considered re-revoked by journal because it has revoked
flag still pending from the last transaction and an assertion triggers.

We fix the problem by keeping the revoked status more uptodate - we clear
revoked flag when switching revoke tables to reflect there is no revoked
buffers in current transaction any more.

a0acae0e88 freezer: unexport refrigerator() and update try_to_freeze() slightly
There is no reason to export two functions for entering the
refrigerator.  Calling refrigerator() instead of try_to_freeze()
doesn't save anything noticeable or removes any race condition.

* Rename refrigerator() to __refrigerator() and make it return bool
  indicating whether it scheduled out for freezing.

* Update try_to_freeze() to return bool and relay the return value of
  __refrigerator() if freezing().

* Convert all refrigerator() users to try_to_freeze().

* Update documentation accordingly.

* While at it, add might_sleep() to try_to_freeze().

8762202dd0 jbd/jbd2: validate sb->s_first in journal_get_superblock()
I hit a J_ASSERT(blocknr != 0) failure in cleanup_journal_tail() when
mounting a fsfuzzed ext3 image. It turns out that the corrupted ext3
image has s_first = 0 in journal superblock, and the 0 is passed to
journal->j_head in journal_reset(), then to blocknr in
cleanup_journal_tail(), in the end the J_ASSERT failed.

So validate s_first after reading journal superblock from disk in
journal_get_superblock() to ensure s_first is valid.

The following script could reproduce it:

9ea7a0df63 jbd2: add debugging information to jbd2_journal_dirty_metadata()
Add debugging information in case jbd2_journal_dirty_metadata() is
called with a buffer_head which didn't have
jbd2_journal_get_write_access() called on it, or if the journal_head
has the wrong transaction in it.  In addition, return an error code.
This won't change anything for ocfs2, which will BUG_ON() the non-zero
exit code.

For ext4, the caller of this function is ext4_handle_dirty_metadata(),
and on seeing a non-zero return code, will call __ext4_journal_stop(),
which will print the function and line number of the (buggy) calling
function and abort the journal.  This will allow us to recover instead
of bug halting, which is better from a robustness and reliability
point of view.

In journal checkpoint, we write the buffer and wait for its finish.
But in cfq, the async queue has a very low priority, and in our test,
if there are too many sync queues and every queue is filled up with
requests, the write request will be delayed for quite a long time and
all the tasks which are waiting for journal space will end with errors like:

jbd2_journal_remove_journal_head() can oops when trying to access
journal_head returned by bh2jh(). This is caused for example by the

jbd2_journal_put_journal_head() in TASK2 sees that b_jcount == 0 and
buffer is not part of any transaction and thus frees journal_head
before TASK1 gets to doing so. Note that even buffer_head can be
released by try_to_free_buffers() after
jbd2_journal_put_journal_head() which adds even larger opportunity for
oops (but I didn't see this happen in reality).

Fix the problem by making transactions hold their own journal_head
reference (in b_jcount). That way we don't have to remove journal_head
explicitely via jbd2_journal_remove_journal_head() and instead just
remove journal_head when b_jcount drops to zero. The result of this is
that [__]jbd2_journal_refile_buffer(),
[__]jbd2_journal_unfile_buffer(), and
__jdb2_journal_remove_checkpoint() can free journal_head which needs
modification of a few callers. Also we have to be careful because once
journal_head is removed, buffer_head might be freed as well. So we
have to get our own buffer_head reference where it matters.

In data=ordered mode, it's theoretically possible (however rare) that
an inode is filed to transaction's t_inode_list and a flusher thread
writes all the data and inode is reclaimed before the transaction
starts to commit.  In such a case, we could erroneously omit sending a
flush to file system device when it is different from the journal
device (because data can still be in disk cache only).

Fix the problem by setting a flag in a transaction when some inode is added
to it and then send disk flush in the commit code when the flag is set.

28e35e42fb jbd2: Fix the wrong calculation of t_max_wait in update_t_max_wait
t_max_wait is added in commit 8e85fb3f to indicate how long we
were waiting for new transaction to start. In commit 6d0bf005,
it is moved to another function named update_t_max_wait to
avoid a build warning. But the wrong thing is that the original
'ts' is initialized in the start of function start_this_handle
and we can calculate t_max_wait in the right way. while with
this change, ts is initialized within the function and t_max_wait
can never be calculated right.

This patch moves the initialization of ts to the original beginning
of start_this_handle and pass it to function update_t_max_wait so
that it can be calculated right and the build warning is avoided also.

229309caeb jbd2: Fix forever sleeping process in do_get_write_access()
In do_get_write_access() we wait on BH_Unshadow bit for buffer to get
from shadow state. The waking code in journal_commit_transaction() has
a bug because it does not issue a memory barrier after the buffer is
moved from the shadow state and before wake_up_bit() is called. Thus a
waitqueue check can happen before the buffer is actually moved from
the shadow state and waiting process may never be woken. Fix the
problem by issuing proper barrier.

deeeaf13b2 jbd2: fix fsync() tid wraparound bug
If an application program does not make any changes to the indirect
blocks or extent tree, i_datasync_tid will not get updated.  If there
are enough commits (i.e., 2**31) such that tid_geq()'s calculations
wrap, and there isn't a currently active transaction at the time of
the fdatasync() call, this can end up triggering a BUG_ON in
fs/jbd2/commit.c:

	J_ASSERT(journal->j_running_transaction != NULL);

It's pretty rare that this can happen, since it requires the use of
fdatasync() plus *very* frequent and excessive use of fsync().  But
with the right workload, it can.

We fix this by replacing the use of tid_geq() with an equality test,
since there's only one valid transaction id that we is valid for us to
wait until it is commited: namely, the currently running transaction
(if it exists).

6cba611e60 jbd2: fix potential memory leak on transaction commit
There is potential memory leak of journal head in function
jbd2_journal_commit_transaction. The problem is that JBD2 will not
reclaim the journal head of commit record if error occurs or journal
is abotred.

I use the following script to reproduce this issue, on a RHEL6
system. I found it very easy to reproduce with async commit enabled.

mount /dev/sdb /mnt -o journal_checksum,journal_async_commit
touch /mnt/xxx
echo offline > /sys/block/sdb/device/state
sync
umount /mnt
rmmod ext4
rmmod jbd2

Removal of the jbd2 module will make slab complaining that
"cache `jbd2_journal_head': can't free all objects".

'write_op' was still used, even though it was always WRITE_SYNC now.
Add plugging around the cases where it submits IO, and flush them

721a9602e6 block: kill off REQ_UNPLUG
With the plugging now being explicitly controlled by the
submitter, callers need not pass down unplugging hints
to the block layer. If they want to unplug, it's because they
manually plugged on their own - in which case, they should just
unplug at will.

e447183180 jbd2: call __jbd2_log_start_commit with j_state_lock write locked
On an SMP ARM system running ext4, I've received a report that the
first J_ASSERT in jbd2_journal_commit_transaction has been triggering:

	J_ASSERT(journal->j_running_transaction != NULL);

While investigating possible causes for this problem, I noticed that
__jbd2_log_start_commit() is getting called with j_state_lock only
read-locked, in spite of the fact that it's possible for it might
j_commit_request.  Fix this by grabbing the necessary information so
we can test to see if we need to start a new transaction before
dropping the read lock, and then calling jbd2_log_start_commit() which
will grab the write lock.

8aefcd557d ext4: dynamically allocate the jbd2_inode in ext4_inode_info as necessary
Replace the jbd2_inode structure (which is 48 bytes) with a pointer
and only allocate the jbd2_inode when it is needed --- that is, when
the file system has a journal present and the inode has been opened
for writing.  This allows us to further slim down the ext4_inode_info
structure.

39e3ac2599 jbd2: Fix I/O hang in jbd2_journal_release_jbd_inode
This fixes a hang seen in jbd2_journal_release_jbd_inode
on a lot of Power 6 systems running with ext4. When we get
in the hung state, all I/O to the disk in question gets blocked
where we stay indefinitely. Looking at the task list, I can see
we are stuck in jbd2_journal_release_jbd_inode waiting on a
wake up. I added some debug code to detect this scenario and
dump additional data if we were stuck in jbd2_journal_release_jbd_inode
for longer than 30 minutes. When it hit, I was able to see that
i_flags was 0, suggesting we missed the wake up.

This patch changes i_flags to be an unsigned long, uses bit operators
to access it, and adds barriers around the accesses. Prior to applying
this patch, we were regularly hitting this hang on numerous systems
in our test environment. After applying the patch, the hangs no longer
occur.

749ef9f842 cfq: improve fsync performance for small files
Fsync performance for small files achieved by cfq on high-end disks is
lower than what deadline can achieve, due to idling introduced between
the sync write happening in process context and the journal commit.

Moreover, when competing with a sequential reader, a process writing
small files and fsync-ing them is starved.

This patch fixes the two problems by:
- marking journal commits as WRITE_SYNC, so that they get the REQ_NOIDLE
  flag set,
- force all queues that have REQ_NOIDLE requests to be put in the noidle
  tree.

Having the queue associated to the fsync-ing process and the one associated
 to journal commits in the noidle tree allows:
- switching between them without idling,
- fairness vs. competing idling queues, since they will be serviced only
  after the noidle tree expires its slice.

dd3932eddf block: remove BLKDEV_IFL_WAIT
All the blkdev_issue_* helpers can only sanely be used for synchronous
caller.  To issue cache flushes or barriers asynchronously the caller needs
to set up a bio by itself with a completion callback to move the asynchronous
state machine ahead.  So drop the BLKDEV_IFL_WAIT flag that is always
specified when calling blkdev_issue_* and also remove the now unused flags
argument to blkdev_issue_flush and blkdev_issue_zeroout.  For
blkdev_issue_discard we need to keep it for the secure discard flag, which
gains a more descriptive name and loses the bitops vs flag confusion.

1113e1b504 JBD2: Allow feature checks before journal recovery
Before we start accessing a huge (> 16 TiB) OCFS2 volume, we need to
confirm that its journal supports 64-bit offsets.  In particular, we
need to check the journal's feature bits before recovering the journal.

This is not possible with JBD2 at present, because the journal
superblock (where the feature bits reside) is not loaded from disk until
the journal is recovered.

This patch loads the journal superblock in
jbd2_journal_check_used_features() if it has not already been loaded,
allowing us to check the feature bits before journal recovery.

9cb569d601 remove SWRITE* I/O types
These flags aren't real I/O types, but tell ll_rw_block to always
lock the buffer instead of giving up on a failed trylock.

Instead add a new write_dirty_buffer helper that implements this semantic
and use it from the existing SWRITE* callers.  Note that the ll_rw_block
code had a bug where it didn't promote WRITE_SYNC_PLUG properly, which
this patch fixes.

In the ufs code clean up the helper that used to call ll_rw_block
to mirror sync_dirty_buffer, which is the function it implements for
compound buffers.

__GFP_NOFAIL is going away, so add our own retry loop.  Also add
jbd2__journal_start() and jbd2__journal_restart() which take a gfp
mask, so that file systems can optionally (re)start transaction
handles using GFP_KERNEL.  If they do this, then they need to be
prepared to handle receiving an PTR_ERR(-ENOMEM) error, and be ready
to reflect that error up to userspace.

OCFS2 uses t_commit trigger to compute and store checksum of the just
committed blocks. When a buffer has b_frozen_data, checksum is computed
for it instead of b_data but this can result in an old checksum being
written to the filesystem in the following scenario:

1) transaction1 is opened
2) handle1 is opened
3) journal_access(handle1, bh)
    - This sets jh->b_transaction to transaction1
4) modify(bh)
5) journal_dirty(handle1, bh)
6) handle1 is closed
7) start committing transaction1, opening transaction2
8) handle2 is opened
9) journal_access(handle2, bh)
    - This copies off b_frozen_data to make it safe for transaction1 to commit.
      jh->b_next_transaction is set to transaction2.
10) jbd2_journal_write_metadata() checksums b_frozen_data
11) the journal correctly writes b_frozen_data to the disk journal
12) handle2 is closed
    - There was no dirty call for the bh on handle2, so it is never queued for
      any more journal operation
13) Checkpointing finally happens, and it just spools the bh via normal buffer
writeback.  This will write b_data, which was never triggered on and thus
contains a wrong (old) checksum.

This patch fixes the problem by calling the trigger at the moment data is
frozen for journal commit - i.e., either when b_frozen_data is created by
do_get_write_access or just before we write a buffer to the log if
b_frozen_data does not exist. We also rename the trigger to t_frozen as
that better describes when it is called.

c35a56a090 jbd2: Improve scalability by not taking j_state_lock in jbd2_journal_stop()
One of the most contended locks in the jbd2 layer is j_state_lock when
running dbench.  This is especially true if using the real-time kernel
with its "sleeping spinlocks" patch that replaces spinlocks with
priority inheriting mutexes --- but it also shows up on large SMP
benchmarks.

5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

d2eecb0393 ext4: Use slab allocator for sub-page sized allocations
Now that the SLUB seems to be fixed so that it respects the requested
alignment, use kmem_cache_alloc() to allocator if the block size of
the buffer heads to be allocated is less than the page size.
Previously, we were using 16k page on a Power system for each buffer,
even when the file system was using 1k or 4k block size.

This is a bit complicated because we are trying to optimize when we
send barriers to the fs data disk.  We could just throw in an extra
barrier to the data disk whenever we send a barrier to the journal
disk, but that's not always strictly necessary.

We only need to send a barrier during a commit when there are data
blocks which are must be written out due to an inode written in
ordered mode, or if fsync() depends on the commit to force data blocks
to disk.  Finally, before we drop transactions from the beginning of
the journal during a checkpoint operation, we need to guarantee that
any blocks that were flushed out to the data disk are firmly on the
rust platter before we drop the transaction from the journal.

e6a47428de jbd2: don't wipe the journal on a failed journal checksum
If there is a failed journal checksum, don't reset the journal.  This
allows for userspace programs to decide how to recover from this
situation.  It may be that ignoring the journal checksum failure might
be a better way of recovering the file system.  Once we add per-block
checksums, we can definitely do better.  Until then, a system
administrator can try backing up the file system image (or taking a
snapshot) and and trying to determine experimentally whether ignoring
the checksum failure or aborting the journal replay results in less
data loss.

0e3d2a6313 ext4: Fix async commit mode to be safe by using a barrier
Previously the journal_async_commit mount option was equivalent to
using barrier=0 (and just as unsafe).  This patch fixes it so that we
eliminate the barrier before the commit block (by not using ordered
mode), and explicitly issuing an empty barrier bio after writing the
commit block.  Because of the journal checksum, it is safe to do this;
if the journal blocks are not all written before a power failure, the
checksum in the commit block will prevent the last transaction from
being replayed.

Using the fs_mark benchmark, using journal_async_commit shows a 50%
improvement:

The function jbd2_journal_write_metadata_buffer() calls
jbd_unlock_bh_state(bh_in) too early; this could potentially allow
another thread to call get_write_access on the buffer head, modify the
data, and dirty it, and allowing the wrong data to be written into the
journal.  Fortunately, if we lose this race, the only time this will
actually cause filesystem corruption is if there is a system crash or
other unclean shutdown of the system before the next commit can take
place.

f91d1d0417 jbd2: Fix a race between checkpointing code and journal_get_write_access()
The following race can happen:

 CPU1                          CPU2
                               checkpointing code checks the buffer, adds
                                 it to an array for writeback
 do_get_write_access()
 ...
 lock_buffer()
 unlock_buffer()
                               flush_batch() submits the buffer for IO
 __jbd2_journal_file_buffer()

So a buffer under writeout is returned from
do_get_write_access(). Since the filesystem code relies on the fact
that journaled buffers cannot be written out, it does not take the
buffer lock and so it can modify buffer while it is under
writeout. That can lead to a filesystem corruption if we crash at the
right moment.

We fix the problem by clearing the buffer dirty bit under buffer_lock
even if the buffer is on BJ_None list. Actually, we clear the dirty
bit regardless the list the buffer is in and warn about the fact if
the buffer is already journalled.

4194b1eaf1 jbd2: use WRITE_SYNC_PLUG instead of WRITE_SYNC
When you are going to be submitting several sync writes, we want to
give the IO scheduler a chance to merge some of them. Instead of
using the implicitly unplugging WRITE_SYNC variant, use WRITE_SYNC_PLUG
and rely on sync_buffer() doing the unplug when someone does a
wait_on_buffer()/lock_buffer().

7f5aa21508 jbd2: Avoid possible NULL dereference in jbd2_journal_begin_ordered_truncate()
If we race with commit code setting i_transaction to NULL, we could
possibly dereference it.  Proper locking requires the journal pointer
(to access journal->j_list_lock), which we don't have.  So we have to
change the prototype of the function so that filesystem passes us the
journal pointer.  Also add a more detailed comment about why the
function jbd2_journal_begin_ordered_truncate() does what it does and
how it should be used.

c88ccea314 jbd2: Fix return value of jbd2_journal_start_commit()
The function jbd2_journal_start_commit() returns 1 if either a
transaction is committing or the function has queued a transaction
commit. But it returns 0 if we raced with somebody queueing the
transaction commit as well. This resulted in ext4_sync_fs() not
functioning correctly (description from Arthur Jones): 

   In the case of a data=ordered umount with pending long symlinks
   which are delayed due to a long list of other I/O on the backing
   block device, this causes the buffer associated with the long
   symlinks to not be moved to the inode dirty list in the second
   phase of fsync_super.  Then, before they can be dirtied again,
   kjournald exits, seeing the UMOUNT flag and the dirty pages are
   never written to the backing block device, causing long symlink
   corruption and exposing new or previously freed block data to
   userspace.

This can be reproduced with a script created by Eric Sandeen
<sandeen@redhat.com>:

4b905671d2 jbd2: Fix oops in jbd2_journal_init_inode() on corrupted fs
On 32-bit system with CONFIG_LBD getblk can fail because provided
block number is too big.  Add error checks so we fail gracefully if
getblk() returns NULL (which can also happen on memory allocation
failures).

e06c8227fd jbd2: Add buffer triggers
Filesystems often to do compute intensive operation on some
metadata.  If this operation is repeated many times, it can be very
expensive.  It would be much nicer if the operation could be performed
once before a buffer goes to disk.

This adds triggers to jbd2 buffer heads.  Just before writing a metadata
buffer to the journal, jbd2 will optionally call a commit trigger associated
with the buffer.  If the journal is aborted, an abort trigger will be
called on any dirty buffers as they are dropped from pending
transactions.

ocfs2 will use this feature.

Initially I tried to come up with a more generic trigger that could be
used for non-buffer-related events like transaction completion.  It
doesn't tie nicely, because the information a buffer trigger needs
(specific to a journal_head) isn't the same as what a transaction
trigger needs (specific to a tranaction_t or perhaps journal_t).  So I
implemented a buffer set, with the understanding that
journal/transaction wide triggers should be implemented separately.

There is only one trigger set allowed per buffer.  I can't think of any
reason to attach more than one set.  Contrast this with a journal or
transaction in which multiple places may want to watch the entire
transaction separately.

The trigger sets are considered static allocation from the jbd2
perspective.  ocfs2 will just have one trigger set per block type,
setting the same set on every bh of the same type.

e07f7183a4 jbd2: improve jbd2 fsync batching
This patch removes the static sleep time in favor of a more self
optimizing approach where we measure the average amount of time it
takes to commit a transaction to disk and the ammount of time a
transaction has been running.  If somebody does a sync write or an
fsync() traditionally we would sleep for 1 jiffies, which depending on
the value of HZ could be a significant amount of time compared to how
long it takes to commit a transaction to the underlying storage.  With
this patch instead of sleeping for a jiffie, we check to see if the
amount of time this transaction has been running is less than the
average commit time, and if it is we sleep for the delta using
schedule_hrtimeout to give us a higher precision sleep time.  This
greatly benefits high end storage where you could end up sleeping for
longer than it takes to commit the transaction and therefore sitting
idle instead of allowing the transaction to be committed by keeping
the sleep time to a minimum so you are sure to always be doing
something.

fd98496f46 jbd2: Add barrier not supported test to journal_wait_on_commit_record
Xen doesn't report that barriers are not supported until buffer I/O is
reported as completed, instead of when the buffer I/O is submitted.
Add a check and a fallback codepath to journal_wait_on_commit_record()
to detect this case, so that attempts to mount ext4 filesystems on
LVM/devicemapper devices on Xen guests don't blow up with an "Aborting
journal on device XXX"; "Remounting filesystem read-only" error.

8c3f25d895 jbd2: don't give up looking for space so easily in __jbd2_log_wait_for_space
Commit 23f8b79e introducd a regression because it assumed that if
there were no transactions ready to be checkpointed, that no progress
could be made on making space available in the journal, and so the
journal should be aborted.  This assumption is false; it could be the
case that simply calling jbd2_cleanup_journal_tail() will recover the
necessary space, or, for small journals, the currently committing
transaction could be responsible for chewing up the required space in
the log, so we need to wait for the currently committing transaction
to finish before trying to force a checkpoint operation.

The multiblock allocator needs to be able to release blocks (and issue
a blkdev discard request) when the transaction which freed those
blocks is committed.  Previously this was done via a polling mechanism
when blocks are allocated or freed.  A much better way of doing things
is to create a jbd2 callback function and attaching the list of blocks
to be freed directly to the transaction structure.

5bf5683a33 ext4: add an option to control error handling on file data
If the journal doesn't abort when it gets an IO error in file data
blocks, the file data corruption will spread silently.  Because
most of applications and commands do buffered writes without fsync(),
they don't notice the IO error.  It's scary for mission critical
systems.  On the other hand, if the journal aborts whenever it gets
an IO error in file data blocks, the system will easily become
inoperable.  So this patch introduces a filesystem option to
determine whether it aborts the journal or just call printk() when
it gets an IO error in file data.

If you mount an ext4 fs with data_err=abort option, it aborts on file
data write error.  If you mount it with data_err=ignore, it doesn't
abort, just call printk().  data_err=ignore is the default.

7ad7445f60 jbd2: don't dirty original metadata buffer on abort
Currently, original metadata buffers are dirtied when they are
unfiled whether the journal has aborted or not.  Eventually these
buffers will be written-back to the filesystem by pdflush.  This
means some metadata buffers are written to the filesystem without
journaling if the journal aborts.  So if both journal abort and
system crash happen at the same time, the filesystem would become
inconsistent state.  Additionally, replaying journaled metadata
can overwrite the latest metadata on the filesystem partly.
Because, if the journal gets aborted, journaled metadata are
preserved and replayed during the next mount not to lose
uncheckpointed metadata.  This would also break the consistency
of the filesystem.

This patch prevents original metadata buffers from being dirtied
on abort by clearing BH_JBDDirty flag from those buffers.  Thus,
no metadata buffers are written to the filesystem without journaling.

44519faf22 jbd2: fix error handling for checkpoint io
When a checkpointing IO fails, current JBD2 code doesn't check the
error and continue journaling.  This means latest metadata can be
lost from both the journal and filesystem.

This patch leaves the failed metadata blocks in the journal space
and aborts journaling in the case of jbd2_log_do_checkpoint().
To achieve this, we need to do:

1. don't remove the failed buffer from the checkpoint list where in
   the case of __try_to_free_cp_buf() because it may be released or
   overwritten by a later transaction
2. jbd2_log_do_checkpoint() is the last chance, remove the failed
   buffer from the checkpoint list and abort the journal
3. when checkpointing fails, don't update the journal super block to
   prevent the journaled contents from being cleaned.  For safety,
   don't update j_tail and j_tail_sequence either
4. when checkpointing fails, notify this error to the ext4 layer so
   that ext4 don't clear the needs_recovery flag, otherwise the
   journaled contents are ignored and cleaned in the recovery phase
5. if the recovery fails, keep the needs_recovery flag
6. prevent jbd2_cleanup_journal_tail() from being called between
   __jbd2_journal_drop_transaction() and jbd2_journal_abort()
   (a possible race issue between jbd2_log_do_checkpoint()s called by
   jbd2_journal_flush() and __jbd2_log_wait_for_space())

If we failed to write metadata buffers to the journal space and
succeeded to write the commit record, stale data can be written
back to the filesystem as metadata in the recovery phase.

To avoid this, when we failed to write out metadata buffers,
abort the journal before writing the commit record.

We can also avoid this kind of corruption by using the journal
checksum feature because it can detect invalid metadata blocks in the
journal and avoid them from being replayed.  So we don't need to care
about asynchronous commit record writeout with a checksum.

23f8b79eae jbd2: abort instead of waiting for nonexistent transaction
The __jbd2_log_wait_for_space function sits in a loop checkpointing
transactions until there is sufficient space free in the journal. 
However, if there are no transactions to be processed (e.g.  because the
free space calculation is wrong due to a corrupted filesystem) it will
never progress.

Check for space being required when no transactions are outstanding and
abort the journal instead of endlessly looping.

05496769e5 jbd2: clean up how the journal device name is printed
Calculate the journal device name once and stash it away in the
journal_s structure.  This avoids needing to call bdevname()
everywhere and reduces stack usage by not needing to allocate an
on-stack buffer.  In addition, we eliminate the '/' that can appear in
device names (e.g. "cciss/c0d0p9" --- see kernel bugzilla #11321) that
can cause problems when creating proc directory names, and include the
inode number to support ocfs2 which creates multiple journals with
different inode numbers.

e9e34f4e8f jbd2: don't abort if flushing file data failed
In ordered mode, the current jbd2 aborts the journal if a file data buffer
has an error.  But this behavior is unintended, and we found that it has
been adopted accidentally.

This patch undoes it and just calls printk() instead of aborting the
journal.  Unlike a similar patch for ext3/jbd, file data buffers are
written via generic_writepages().  But we also need to set AS_EIO
into their mappings because wait_on_page_writeback_range() clears
AS_EIO before a user process sees it.

cd1aac3292 ext4: Add ordered mode support for delalloc
This provides a new ordered mode implementation which gets rid of using
buffer heads to enforce the ordering between metadata change with the
related data chage.  Instead, in the new ordering mode, it keeps track
of all of the inodes touched by each transaction on a list, and when
that transaction is committed, it flushes all of the dirty pages for
those inodes.  In addition, the new ordered mode reverses the lock
ordering of the page lock and transaction lock, which provides easier
support for delayed allocation.

This patch adds necessary framework into JBD2 to be able to track inodes
with each transaction and write-out their dirty data during transaction
commit time.

This new ordered mode brings all sorts of advantages such as possibility 
to get rid of journal heads and buffer heads for data buffers in ordered 
mode, better ordering of writes on transaction commit, simplification of
 some JBD code, no more anonymous pages when truncate of data being 
committed happens.  Also with this new ordered mode, delayed allocation 
on ordered mode is much simpler.

530576bbf3 jbd2: fix race between jbd2_journal_try_to_free_buffers() and jbd2 commit transaction
journal_try_to_free_buffers() could race with jbd commit transaction
when the later is holding the buffer reference while waiting for the
data buffer to flush to disk. If the caller of
journal_try_to_free_buffers() request tries hard to release the buffers,
it will treat the failure as error and return back to the caller. We
have seen the directo IO failed due to this race.  Some of the caller of
releasepage() also expecting the buffer to be dropped when passed with
GFP_KERNEL mask to the releasepage()->journal_try_to_free_buffers().

With this patch, if the caller is passing the GFP_KERNEL to indicating
this call could wait, in case of try_to_free_buffers() failed, let's
waiting for journal_commit_transaction() to finish commit the current
committing transaction , then try to free those buffers again with
journal locked.

8a9362eb40 jbd2: replace potentially false assertion with if block
If an error occurs during jbd2 cache initialisation it is possible for the
journal_head_cache to be NULL when jbd2_journal_destroy_journal_head_cache is
called.  Replace the J_ASSERT with an if block to handle the situation
correctly.

Note that even with this fix things will break badly if jbd2 is statically
compiled in and cache initialisation fails.

code into functions is tidier, simplifies the logic in initialisation in

1dfc3220d9 jbd2: fix possible journal overflow issues
There are several cases where the running transaction can get buffers
added to its BJ_Metadata list which it never dirtied, which makes its
t_nr_buffers counter end up larger than its t_outstanding_credits
counter.

This will cause issues when starting new transactions as while we are
logging buffers we decrement t_outstanding_buffers, so when
t_outstanding_buffers goes negative, we will report that we need less
space in the journal than we actually need, so transactions will be
started even though there may not be enough room for them.  In the worst
case scenario (which admittedly is almost impossible to reproduce) this
will result in the journal running out of space.

The fix is to only refile buffers from the committing transaction to the
running transactions BJ_Modified list when b_modified is set on that
journal, which is the only way to be sure if the running transaction has
modified that buffer.

This patch also fixes an accounting error in journal_forget, it is
possible that we can call journal_forget on a buffer without having
modified it, only gotten write access to it, so instead of freeing a
credit, we only do so if the buffer was modified.  The assert will help
catch if this problem occurs.  Without these two patches I could hit
this assert within minutes of running postmark, with them this issue no
longer arises.

9fc7c63a1d jbd2: fix the way the b_modified flag is cleared
Currently at the start of a journal commit we loop through all of the buffers
on the committing transaction and clear the b_modified flag (the flag that is
set when a transaction modifies the buffer) under the j_list_lock.

The problem is that everywhere else this flag is modified only under the jbd2
lock buffer flag, so it will race with a running transaction who could
potentially set it, and have it unset by the committing transaction.

This is also a big waste, you can have several thousands of buffers that you
are clearing the modified flag on when you may not need to.  This patch
removes this code and instead clears the b_modified flag upon entering
do_get_write_access/journal_get_create_access, so if that transaction does
indeed use the buffer then it will be accounted for properly, and if it does
not then we know we didn't use it.

That will be important for the next patch in this series.  Tested thoroughly
by myself using postmark/iozone/bonnie++.

d00256766a jbd2: correctly unescape journal data blocks
Fix a long-standing typo (predating git) that will cause data corruption if a
journal data block needs unescaping.  At the moment the wrong buffer head's
data is being unescaped.

To test this case mount a filesystem with data=journal, start creating and
deleting a bunch of files containing only JBD2_MAGIC_NUMBER (0xc03b3998), then
pull the plug on the device.  Without this patch the files will contain zeros
instead of the correct data after recovery.

c4e35e07af JBD2: Clear buffer_ordered flag for barried IO request on success
In JBD2 jbd2_journal_write_commit_record(), clear the buffer_ordered
flag for the bh after barried IO has succeed. This prevents later, if
the same buffer head were submitted to the underlying device, which has
been reconfigured to not support barrier request, the JBD2 commit code
could treat it as a normal IO (without barrier).

This is a port from JBD/ext3 fix from Neil Brown.

More details from Neil:

Some devices - notably dm and md - can change their behaviour in
response to BIO_RW_BARRIER requests.  They might start out accepting
such requests but on reconfiguration, they find out that they cannot
any more. JBD2 deal with this by always testing if BIO_RW_BARRIER
requests fail with EOPNOTSUPP, and retrying the write
requests without the barrier (probably after waiting for any pending
writes to complete).

However there is a bug in the handling this in JBD2 for ext4 .

When ext4/JBD2 to submit a BIO_RW_BARRIER request,
it sets the buffer_ordered flag on the buffer head.
If the request completes successfully, the flag STAYS SET.

Other code might then write the same buffer_head after the device has
been reconfigured to not accept barriers.  This write will then fail,
but the "other code" is not ready to handle EOPNOTSUPP errors and the
error will be treated as fatal.

b048d84626 jbd2: Add error check to journal_wait_on_commit_record to avoid oops
The buffer head pointer passed to journal_wait_on_commit_record() could
be NULL if the previous journal_submit_commit_record() failed or journal
has already aborted.

Looking at the jbd2 debug messages, before the oops happened, the jbd2
is aborted due to trying to access the next log block beyond the end
of device. This might be caused by using a corrupted image.

We need to check the error returns from journal_submit_commit_record()
and avoid calling journal_wait_on_commit_record() in the failure case.

This addresses Kernel Bugzilla #9849

95c354fe9f spinlock: lockbreak cleanup
The break_lock data structure and code for spinlocks is quite nasty.
Not only does it double the size of a spinlock but it changes locking to
a potentially less optimal trylock.

Put all of that under CONFIG_GENERIC_LOCKBREAK, and introduce a
__raw_spin_is_contended that uses the lock data itself to determine whether
there are waiters on the lock, to be used if CONFIG_GENERIC_LOCKBREAK is
not set.

Rename need_lockbreak to spin_needbreak, make it use spin_is_contended to
decouple it from the spinlock implementation, and make it typesafe (rwlocks
do not have any need_lockbreak sites -- why do they even get bloated up
with that break_lock then?).

very first time, which is likely to be rounded up to somewhere closer
to 6 seconds)

818d276ceb ext4: Add the journal checksum feature
The journal checksum feature adds two new flags i.e
JBD2_FEATURE_INCOMPAT_ASYNC_COMMIT and JBD2_FEATURE_COMPAT_CHECKSUM.

JBD2_FEATURE_CHECKSUM flag indicates that the commit block contains the
checksum for the blocks described by the descriptor blocks.
Due to checksums, writing of the commit record no longer needs to be
synchronous. Now commit record can be sent to disk without waiting for
descriptor blocks to be written to disk. This behavior is controlled
using JBD2_FEATURE_ASYNC_COMMIT flag. Older kernels/e2fsck should not be
able to recover the journal with _ASYNC_COMMIT hence it is made
incompat.
The commit header has been extended to hold the checksum along with the
type of the checksum.

For recovery in pass scan checksums are verified to ensure the sanity
and completeness(in case of _ASYNC_COMMIT) of every transaction.

The patch below updates the jbd stats patch to 2.6.20/jbd2.
The initial patch was posted by Alex Tomas in December 2005

It provides statistics via procfs such as transaction lifetime and size.

Sometimes, investigating performance problems, i find useful to have
stats from jbd about transaction's lifetime, size, etc. here is a
patch for review and inclusion probably.

f5a7a6b0d9 jbd2: Fix assertion failure in fs/jbd2/checkpoint.c
Before we start committing a transaction, we call
__journal_clean_checkpoint_list() to cleanup transaction's written-back
buffers.

If this call happens to remove all of them (and there were already some
buffers), __journal_remove_checkpoint() will decide to free the transaction
because it isn't (yet) a committing transaction and soon we fail some
assertion - the transaction really isn't ready to be freed :).

We change the check in __journal_remove_checkpoint() to free only a
transaction in T_FINISHED state.  The locking there is subtle though (as
everywhere in JBD ;().  We use j_list_lock to protect the check and a
subsequent call to __journal_drop_transaction() and do the same in the end
of journal_commit_transaction() which is the only place where a transaction
can get to T_FINISHED state.

Probably I'm too paranoid here and such locking is not really necessary -
checkpoint lists are processed only from log_do_checkpoint() where a
transaction must be already committed to be processed or from
__journal_clean_checkpoint_list() where kjournald itself calls it and thus
transaction cannot change state either.  Better be safe if something
changes in future...

a7fa2baf8e jbd2: fix commit code to properly abort journal
We should really call journal_abort() and not __journal_abort_hard() in
case of errors.  The latter call does not record the error in the journal
superblock and thus filesystem won't be marked as with errors later (and
user could happily mount it without any warning).

20c2df83d2 mm: Remove slab destructors from kmem_cache_create().
Slab destructors were no longer supported after Christoph's
c59def9f22 change. They've been
BUGs for both slab and slub, and slob never supported them
either.

This rips out support for the dtor pointer from kmem_cache_create()
completely and fixes up every single callsite in the kernel (there were
about 224, not including the slab allocator definitions themselves,
or the documentation references).

the "jbd2: Move jbd2-debug file to debugfs" patch

changed the jbd2_journal_enable_debug from int type to u8, makes the
jbd_debug comparision is always true when the debugging level is 0. Thus
the compile warning occurs.

Thought about changing the jbd2_journal_enable_debug data type back to
int, but can't, because the jbd2-debug is moved to debug fs, where
calling debugfs_create_u8() to create the debugfs entry needs the value
to be u8 type.

Even if we changed the data type back to int, the code is still buggy,
kernel should not print jbd2 debug message if the
jbd2_journal_enable_debug is set to 0. But this is not the case.

The fix is change the level of debugging to 1. The same should fixed in
ext3/JBD, but currently ext3 jbd-debug via /proc fs is broken, so we
probably should fix it all together.

0f49d5d019 jbd2: Move jbd2-debug file to debugfs
The jbd2-debug file used to be located in /proc/sys/fs/jbd2-debug, but it
incorrectly used create_proc_entry() instead of the sysctl routines, and
no proc entry was ever created.

Instead of fixing this we might as well move the jbd2-debug file to
debugfs which would be the preferred location for this kind of tunable.
The new location is now /sys/kernel/debug/jbd2/jbd2-debug.

If the thread failed to create the subsequent wait_event will hang forever.

This is likely to happen if kernel hits max_threads limit.

Will be critical for virtualization systems that limit the number of tasks
and kernel memory usage within the container.

and kthread_stop()).

126039256c [PATCH] jbd2: wait for already submitted t_sync_datalist buffer to complete
In the current jbd code, if a buffer on BJ_SyncData list is dirty and not
locked, the buffer is refiled to BJ_Locked list, submitted to the IO and
waited for IO completion.

But the fsstress test showed the case that when a buffer was already
submitted to the IO just before the buffer_dirty(bh) check, the buffer was
not waited for IO completion.

Following patch solves this problem.  If it is assumed that a buffer is
submitted to the IO before the buffer_dirty(bh) check and still being
written to disk, this buffer is refiled to BJ_Locked list.

9b57988db9 [PATCH] jbd2: journal_dirty_data re-check for unmapped buffers
When running several fsx's and other filesystem stress tests, we found
cases where an unmapped buffer was still being sent to submit_bh by the
ext3 dirty data journaling code.

I saw this happen in two ways, both related to another thread doing a
truncate which would unmap the buffer in question.

Either we would get into journal_dirty_data with a bh which was already
unmapped (although journal_dirty_data_fn had checked for this earlier, the
state was not locked at that point), or it would get unmapped in the middle
of journal_dirty_data when we dropped locks to call sync_dirty_buffer.

By re-checking for mapped state after we've acquired the bh state lock, we
should avoid these races.  If we find a buffer which is no longer mapped,
we essentially ignore it, because journal_unmap_buffer has already decided
that this buffer can go away.

I've also added tracepoints in these two cases, and made a couple other
tracepoint changes that I found useful in debugging this.

A disk generated some I/O error, after it, I hitted
J_ASSERT(transaction->t_updates > 0) in journal_stop().

It seems to happened on ext3_truncate() path from stack trace. Then,
maybe the following case may trigger J_ASSERT(transaction->t_updates > 0).

ext3_truncate()
    -> ext3_free_branches()
        -> ext3_journal_test_restart()
	    -> ext3_journal_restart()
                -> journal_restart()
                transaction->t_updates--;
                /* another process aborted journal */
                    -> start_this_handle()
		    returns -EROFS without transaction->t_updates++;

    -> ext3_journal_stop()
        -> journal_stop()
	J_ASSERT(transaction->t_updates > 0)

If journal was aborted in middle of journal_restart(), ext3_truncate()
may trigger J_ASSERT().
