bac65d9d87 powerpc updates for 4.14
Nothing really major this release, despite quite a lot of activity. Just lots of
 things all over the place.
 
 Some things of note include:
 
  - Access via perf to a new type of PMU (IMC) on Power9, which can count both
    core events as well as nest unit events (Memory controller etc).
 
  - Optimisations to the radix MMU TLB flushing, mostly to avoid unnecessary Page
    Walk Cache (PWC) flushes when the structure of the tree is not changing.
 
  - Reworks/cleanups of do_page_fault() to modernise it and bring it closer to
    other architectures where possible.
 
  - Rework of our page table walking so that THP updates only need to send IPIs
    to CPUs where the affected mm has run, rather than all CPUs.
 
  - The size of our vmalloc area is increased to 56T on 64-bit hash MMU systems.
    This avoids problems with the percpu allocator on systems with very sparse
    NUMA layouts.
 
  - STRICT_KERNEL_RWX support on PPC32.
 
  - A new sched domain topology for Power9, to capture the fact that pairs of
    cores may share an L2 cache.
 
  - Power9 support for VAS, which is a new mechanism for accessing coprocessors,
    and initial support for using it with the NX compression accelerator.
 
  - Major work on the instruction emulation support, adding support for many new
    instructions, and reworking it so it can be used to implement the emulation
    needed to fixup alignment faults.
 
  - Support for guests under PowerVM to use the Power9 XIVE interrupt controller.
 
 And probably that many things again that are almost as interesting, but I had to
 keep the list short. Plus the usual fixes and cleanups as always.

Here is the big tty/serial driver update for 4.14-rc1.
 
 Well, not all that big, just a number of small serial driver fixes, and
 a new serial driver.  Also in here are some much needed goldfish tty
 driver (emulator) fixes to try to get that codebase under control.
 
 All of these have been in linux-next for a while with no reported

Lguest seems to be rather unused these days. It has seen only patches
ensuring it still builds the last two years and its official state is
"Odd Fixes".

Remove it in order to be able to clean up the paravirt code.

9227f04314 powerpc/udbg: Reduce the footgun potential of EARLY_DEBUG_LPAR(_HVSI)
For debugging very early boot problems we have CONFIG_PPC_EARLY_DEBUG,
which allows configuring the kernel such that it unconditionally writes
to a particular type of console, regardless of whether that console
exists or not. This is useful sometimes when the kernel crashes before
it can even determine what platform it's on, and therefore what consoles
exist.

However if you boot a kernel built this way on a different platform, it
will generally crash because it writes to a console that doesn't exist.

A particularly nasty instance of this is if you enable the hypervisor
console early debug, and then boot that kernel on bare metal. The result
is that the kernel calls "the hypervisor" very early in boot, but the
kernel *is* the hypervisor, so we jump to the system call handler and
start executing all sorts of code that isn't ready to be run. This may
lead to a machine check or check stop depending on how lucky you are.

Luckily there is an easy way to avoid this particular case. We simply
read the MSR before installing the hooks, and if we see MSR_HV is set
then we are the hypervisor and we definitely should not use the
hypervisor console.

Here is the big driver core update for 4.13-rc1.
 
 The large majority of this is a lot of cleanup of old fields in the
 driver core structures and their remaining usages in random drivers.
 All of those fixes have been reviewed by the various subsystem
 maintainers.  There's also some small firmware updates in here, a new
 kobject uevent api interface that makes userspace interaction easier,
 and a few other minor things.
 
 All of these have been in linux-next for a long while with no reported
 issues.

 issue with the arm-soc tree in the include/linux/platform_data/atmel.h
 file.  Stephen has sent out a fixup for it, so it shouldn't be that
 difficult to merge.

20d58cb642 s390/hvc_iucv: fix broken Kconfig select statement
Select statements in Kconfig do not necessarily enable all required
dependencies and can lead to broken configs. This is also the case
for the "select IUCV" statement within HVC_IUCV:

warning: (HVC_IUCV) selects IUCV which has unmet direct
         dependencies (NET && S390)

Just add the missing "depends on NET" to avoid broken configs.

19fa6e601b tty/hvc_console: fix console lock ordering with spinlock
hvc_remove() takes a spin lock first then acquires the console
semaphore. This situation can easily lead to a deadlock scenario
where we call scheduler with spin lock held.

6d35b131ca tty: Remove extra include in HVC console tty framework

bbc3dfe880 tty/hvc: Use IRQF_SHARED for OPAL hvc consoles
Commit 2def86a720 ("hvc: Convert to using interrupts instead of opal
events") enabled the use of interrupts in the hvc_driver for OPAL
platforms. However on machines with more than one hvc console, any
console after the first will fail to register an interrupt handler in
notifier_add_irq() since all consoles share the same IRQ number but do
not set the IRQF_SHARED flag:

  genirq: Flags mismatch irq 31. 00000000 (hvc_console) vs. 00000000 (hvc_console)
  hvc_open: request_irq failed with rc -16.

This error propagates up to hvc_open() and the console is closed, but
OPAL will still generate interrupts that are not handled, leading to
rcu_sched stall warnings.

Set IRQF_SHARED when calling request_irq(), allowing additional consoles
to start properly. This is only set for consoles handled by
hvc_opal_probe(), leaving other types unaffected.

5de738b3fb hvc_xen: fix xenboot for DomUs
The xenboot early console has been partially broken for DomU for a long
time: the output would only go to the hypervisor via hypercall
(HYPERVISOR_console_io), while it wouldn't actually go to the DomU
console. The reason is that domU_write_console would return early as no
xencons structs are configured for it.

Add an appropriate xencons struct for xenboot from the xenboot setup
callback.

16e506efc0 hvc_xen: add earlycon support
Introduce EARLYCON support in hvc_xen, useful for early debugging on arm
and arm64, where xen early_printk is not available.

It is different from xenboot_write_console on x86 in two ways:

- it does not return if !xen_pv_domain(), not only because ARM guests
  are xen_hvm_domain(), but also because we want to capture all the
  early boot messages, before xen support is discovered
- it does not try to print to the domU console at all, because xen
  support will only be discovered at a later point

e3c5fc4d47 drivers/tty: make hvc/hvc_vio.c explicitly non-modular
The Kconfig currently controlling compilation of this code is:

drivers/tty/hvc/Kconfig:config HVC_CONSOLE
drivers/tty/hvc/Kconfig:        bool "pSeries Hypervisor Virtual Console support"

...meaning that it currently is not being built as a module by anyone.

Lets remove the modular code that is essentially orphaned, so that
when reading the driver there is no doubt it is builtin-only.

Since module_init translates to device_initcall in the non-modular
case, the init ordering remains unchanged with this commit.

We explicitly disallow a driver unbind, since that doesn't have a
sensible use case anyway, and it allows us to drop the ".remove"
code for non-modular drivers.

We don't replace module.h with init.h since the file already has that.

Also note that MODULE_DEVICE_TABLE is a no-op for non-modular code.

79c1faa451 tty: Remove tty_wait_until_sent_from_close()
tty_wait_until_sent_from_close() drops the tty lock while waiting
for the tty driver to finish sending previously accepted data (ie.,
data remaining in its write buffer and transmit fifo).

tty_wait_until_sent_from_close() was added by commit a57a7bf3fc
("TTY: define tty_wait_until_sent_from_close") to prevent the entire
tty subsystem from being unable to open new ttys while waiting for
one tty to close while output drained.

However, since commit 0911261d4c ("tty: Don't take tty_mutex for tty
count changes"), holding a tty lock while closing does not prevent other
ttys from being opened/closed/hung up, but only prevents lifetime event
changes for the tty under lock.

Holding the tty lock while waiting for output to drain does prevent
parallel non-blocking opens (O_NONBLOCK) from advancing or returning
while the tty lock is held. However, all parallel opens _already_
block even if the tty lock is dropped while closing and the parallel
open advances. Blocking in open has been in mainline since at least 2.6.29
(see tty_port_block_til_ready(); note the test for O_NONBLOCK is _after_
the wait while ASYNC_CLOSING).

IOW, before this patch a non-blocking open will sleep anyway for the
_entire_ duration of a parallel hardware shutdown, and when it wakes, the
error return will cause a release of its tty, and it will restart with
a fresh attempt to open. Similarly with a blocking open that is already
waiting; when it's woken, the hardware shutdown has already completed
to ASYNC_INITIALIZED is not set, which forces a release and restart as
well.

So, holding the tty lock across the _entire_ close (which is what this
patch does), even while waiting for output to drain, is equivalent to
the current outcome wrt parallel opens.

HVM_PARAM_CONSOLE_PFN is used to retrieved the console PFN for HVM
guest. It returns a PFN (aka GFN) and not a MFN.

than doing a special case for each of the them.

0df4f266b3 xen: Use correctly the Xen memory terminologies
Based on include/xen/mm.h [1], Linux is mistakenly using MFN when GFN
is meant, I suspect this is because the first support for Xen was for
PV. This resulted in some misimplementation of helpers on ARM and
confused developers about the expected behavior.

For instance, with pfn_to_mfn, we expect to get an MFN based on the name.
Although, if we look at the implementation on x86, it's returning a GFN.

For clarity and avoid new confusion, replace any reference to mfn with
gfn in any helpers used by PV drivers. The x86 code will still keep some
reference of pfn_to_mfn which may be used by all kind of guests
No changes as been made in the hypercall field, even
though they may be invalid, in order to keep the same as the defintion
in xen repo.

Note that page_to_mfn has been renamed to xen_page_to_gfn to avoid a
name to close to the KVM function gfn_to_page.

Take also the opportunity to simplify simple construction such
as pfn_to_mfn(page_to_pfn(page)) into xen_page_to_gfn. More complex clean up
will come in follow-up patches.

These issues are raised in little endian mode because the data exchanged in
memory between the kernel and the hypervisor has to be in big endian
format. This exhibits as errors such as:

  irq: (null) didn't like hwirq-0x1000a00 to VIRQ16 mapping (rc=-22)
  hvsi_console_init: couldn't create irq mapping for 0x1000a00

The data structures already have endian annotations, and sparse is
generating numerous warnings based on those. This commit fixes all of
them.

Most code already uses consts for the struct kernel_param_ops,
sweep the kernel for the last offending stragglers. Other than
include/linux/moduleparam.h and kernel/params.c all other changes
were generated with the following Coccinelle SmPL patch. Merge
conflicts between trees can be handled with Coccinelle.

In the future git could get Coccinelle merge support to deal with
patch --> fail --> grammar --> Coccinelle --> new patch conflicts
automatically for us on patches where the grammar is available and
the patch is of high confidence. Consider this a feature request.

2def86a720 hvc: Convert to using interrupts instead of opal events
Convert the opal hvc driver to use the new irqchip to register for
opal events. As older firmware versions may not have device tree

77bb3dfdc0 xen/events: don't bind non-percpu VIRQs with percpu chip
A non-percpu VIRQ (e.g., VIRQ_CONSOLE) may be freed on a different
VCPU than it is bound to.  This can result in a race between
handle_percpu_irq() and removing the action in __free_irq() because
handle_percpu_irq() does not take desc->lock.  The interrupt handler
sees a NULL action and oopses.

Only use the percpu chip/handler for per-CPU VIRQs (like VIRQ_TIMER).
