On a set-top-box based on the Broadcom 7405 SoC (MIPS), the Abilis as102 driver
causes a kernel oops while trying to map the URB stream buffers DMA addresses:

On other boxes based on older SoCs (7401) this doesn't happen, so it looks like
a bug in the kernel specific to MIPS SMP. This issue has been reproduced on
several kernel versions from 2.6.18 to 3.1.0.
Since the base DMA address and the offsets are known, it is possible to map
the DMA addresses of the URB buffers directly in the driver. This workaround
fixes the problem and has been tested on both MIPS and x86 CPUs with success.
By the way, with this fix the driver works perfectly fine on the set-top-box:
both UHF and VHF frequencies are tuned without problems, and zapping is quite
fast. SNR and signal strength reports seems to work fine, too.
The only remaining problem (on both the PC and the set-top-box) is that after
a soft reboot the device is not recognized again by the kernel. It requires
a power cycle (or a manual unplug/replug) to be recognized again. So probably
the device state is not reset properly at shut-down.

On MIPS/ARM set-top-boxes, as well as old x86 PCs, memory allocation failures
in the em28xx driver are common, due to memory fragmentation over time, that
makes impossible to allocate large chunks of coherent memory.
A typical system with 256/512 MB of RAM fails after just 1 day of uptime (see
the old thread for detailed reports and crashlogs).
In fact, the em28xx driver allocates memory for USB isoc transfers at runtime,
as opposite to the dvb-usb drivers that allocates the USB buffers when the
device is initialized, and frees them when the device is disconnected.
Moreover, in digital mode the USB isoc transfer buffers are freed, allocated
and cleared every time the user selects a new channel, wasting time and
resources.
This patch solves both problems by allocating DVB isoc transfer buffers in
em28xx_usb_probe(), and freeing them in em28xx_usb_disconnect().
In fact, the buffers size and number depend only on the max USB packet size
that is parsed from the USB descriptors in em28xx_usb_probe(), so it can
never change for a given device.
This approach makes no sense in analog mode (as the buffer size depends on
the alternate mode selected at runtime), the patch creates two separate sets
of buffers for digital and analog modes.
For digital-only devices, USB buffers are created when the device is probed
and freed when the device is disconnected.
For analog-only devices, nothing changes: isoc buffers are created at runtime.
For hybrid devices, two sets of buffers are maintained: the digital-mode
buffers are created when the device is probed, and freed when the device is
disconnected; analog-mode buffers are created/destroyed at runtime as before.
So, in analog mode, digital and analog buffers coexists at the same time: this
can be justified by the fact that digital mode is by far more commonly used
nowadays, so it makes sense to optimize the driver for this use case scenario.
The patch has been tested in the last few days on a x86 PC and a MIPS
set-top-box, with the PCTV 290e (digital only) and the Terratec Hybrid XS
(hybrid device). With the latter, I switched several times between analog and
digital mode (Kaffeine/TvTime) with no issue at all.
I unplugged/plugged the devices several times with no problem.
Also, after over 3 days of normal usage in the MPIS set-top-box, the PCTV 290e
was still up and running.

The signal strength value returned is not quite correct, it decreases
when I increase the gain of my antenna, and vice versa. It also
doesn't span over the whole 0x0000-0xffff range. Compute a value which
at least increases when signal strength increases, and spans the whole
allowed range.
In practice I get 67% with my antenna fully amplified and 51% with
no amplification. This is close enough to what I get on my other
DVB-T adapter with the same antenna.

Adapt to new controls (subsampling).
For encoding, the destination format now needs to be set to V4L2_PIX_FMT_JPEG
and the subsampling (4:2:2 or 4:2:0) needs to be set using the respective
control (V4L2_CID_JPEG_CHROMA_SUBSAMPLING). Required buffer size for
destination image during encoding is no longer deduced from the format (which
generally implied overestimation), but needs to be given from userspace in
sizeimage.
Not strictly related to the added controls, this patch also fixes setting the
subsampling of the destination image for decoding, depending on the destination
format.

A global delay parameter has the side effect of being overwritten with 0 if a
single ZL2004 or ZL6105 is instantiated. If other chips supported by the same
driver are in the system, this will result in access errors for those chips.
To solve the problem, keep a per-instance copy of the delay parameter, and do
not change the original parameter.

We've been getting occasional oops running a 32-bit kernel on a certain
system in our RHEL test hw. It appears that we fail to get sufficent ioremap
space for the framebuffer, and this leads to an oops.
This patch should fix the oops and leave a message in the logs we can
check for.
A future fix would probably to resize the console to a size that we can
ioremap.

With runtime PM, if the ethernet cable is disconnected, the device is
transitioned to D3 state to conserve energy. If the system is shutdown
in this state, any register accesses in rtl_shutdown are dropped on
the floor. As the device was programmed by .runtime_suspend() to wake
on link changes, it is thus brought back up as soon as the link recovers.
Resuming every suspended device through the driver core would slow things
down and it is not clear how many devices really need it now.

Production GMA3600/3650 hardware turns out to be subtly different to the
development platforms. This combined with a minor driver bug is causing
the kernel to hang on these platforms.
This patch does the following
- turn down a couple of messages that were meant to be debug and are
causing much confusion
- ensure the hotplug interrupt is disabled on Cedartrail systems.
- fix a bug where gtt roll mode called psbfb_sync, which tries to sync
the 2D engine. On other devices it is harmless as the 2D engine is
present but not in use when in gtt roll mode, on Cedartrail it causes
a hang
Without these changes 3.3-rc hangs on boot on Cedartrail based systems.

Rather than returning a single bit for each button in the group, an
array returns an index in each field that corresponds to the pressed
button (like keyboard scan codes). An out-of range value in and array
field is considered no controls asserted.
Apparently, "and" above is a typo and should be "an".
This fixes at least Waltop tablet pen clicks - otherwise BTN_TOUCH is never
released.
The relevant part of Waltop tablet report descriptors is this:
0x09, 0x42, /* Usage (Tip Switch), */
0x09, 0x44, /* Usage (Barrel Switch), */
0x09, 0x46, /* Usage (Tablet Pick), */
0x15, 0x01, /* Logical Minimum (1), */
0x25, 0x03, /* Logical Maximum (3), */
0x75, 0x04, /* Report Size (4), */
0x95, 0x01, /* Report Count (1), */
0x80, /* Input, */

Even if cards supports 1.8V I/O voltage those should anyway be
initialized at 3.3V I/O according to (e)MMC, SD and SDIO specs.
Some eMMC and embedded SDIO devices are able to be initialized
at 1.8V as well, but it is better to be safe.
Do note that initialization in this context means that the card
has been completely powered off, otherwise the card will remain
at the last I/O voltage level that were negotitiated.
Due to the above being taken care of the suspend/resume issues
for UHS-I SD-cards has been fixed.

On i.MX53 we have to write a special SDHCI_CMD_ABORTCMD to the
SDHCI_TRANSFER_MODE register during a MMC_STOP_TRANSMISSION
command. This works for SD cards. However, with MMC cards
the MMC_SET_BLOCK_COUNT command is used instead, but this
needs the same handling. Fix MMC cards by testing for the
MMC_SET_BLOCK_COUNT command aswell. Tested on a custom i.MX53
board with a Transcend MMC+ card and eMMC.
The kernel started used MMC_SET_BLOCK_COUNT in 3.0, so this
is a regression for these boards introduced in 3.0; it should
go to 3.0/3.1/3.2-stable.

A recent commit "mmc: core: Use delayed work in clock gating framework"
(597dd9d79c) introduced a default 200ms delay before clock gating
actually takes place. This means that every time an MMC interface
becomes idle it first stays on for 200ms before gating its clock. This
leads to increased power consumption and is therefore a clear regression.
This patch restores the original behaviour by setting the default delay
to 0. Users prioritising throughput over power efficiency can still
modify the delay via sysfs.

The F75387 supports automatic fan control using either PWM duty cycle or
RPM speed values. Make the driver detect the latter mode, and expose the
different modes in sysfs as per pwm_enable, so that the user can switch
between them.
The interpretation of the pwm_enable attribute for the F75387 is adjusted
to be a superset of those values used for similar Fintek chips which do
not support automatic duty mode, with 2 mapping to automatic speed mode,
and moving automatic duty mode to the new value 4.
Toggling the duty mode via pwm_enable is currently denied for the F75387,
as the chip then simply reinterprets the fan configuration register values
according to the new mode, switching between RPM and PWM units, which
makes this a dangerous operation.
This patch introduces a new pwm mode into the driver. This is necessary
because the new mode (automatic pwm mode, 4) may already be enabled by the
BIOS, and the driver should not break existing functionality. This was seen
on at least one board.

commit e562966dba added support for S4 to
the balloon driver. The freeze function did nothing to free the pages,
since reclaiming the pages from the host to immediately give them back
(if S4 was successful) seemed wasteful. Also, if S4 wasn't successful,
the guest would have to re-fill the balloon. On restore, the pages were
supposed to be marked freed and the free page counters were incremented
to reflect the balloon was totally deflated.
However, this wasn't done right. The pages that were earlier taken away
from the guest during a balloon inflation operation were just shown as
used pages after a successful restore from S4. Just a fancy way of
leaking lots of memory.
Instead of trying that, just leak the balloon on freeze and fill it on
restore/thaw paths. This works properly now. The optimisation to not
leak can be added later on after a bit of refactoring of the code.

ovl->enable/disable are meant to be synchronous so that they can handle
the configuration of fifo sizes. The current kernel doesn't configure
fifo sizes yet, and so the code doesn't need to block to function (from
omapdss driver's perspective).
However, for the users of omapdss a non-blocking ovl->disable is
confusing, because they don't know when the memory area is not used
any more.
Furthermore, when the fifo size configuration is added in the next merge
window, the change from non-blocking to blocking could cause side
effects to the users of omapdss. So by making the functions block
already will keep them behaving in the same manner.
And, while not the main purpose of this patch, this will also remove the
compile warning:

tg3_get_stats64() takes tp->lock when dealing with non-serdes bcm5700
and bcm5701 devices. However, functions that call tg3_halt() have
already acquired tp->lock. When tg3_get_stats64() is called in
tg3_halt(), deadlock will occur.
This patch fixes the problem by separating the stat gathering code into
a new tg3_get_nstats() function. tg3_get_stats64() is recoded to call
this function and take tp->lock. The code that takes tp->lock in
tg3_calc_crc_errors() has been removed. Function signatures have been
cleaned up too.

On Access Point mode, when transmitting a packet, if the destination
station is in powersave mode, we abort transmitting the packet to the
device queue, but we do not reclaim the allocated memory. Given enough
packets, we can go in a state where there is no packet on the device
queue, but we think the device has no memory left, so no packet gets
transmitted, connections breaks and the AP stops working.
This undo the allocation done in the TX path when the station is in
power-save mode.

All the fixes are for the OMAP IOMMU driver. The first patch is the
biggest one. It fixes the calls of the function omap_find_iovm_area() in
the omap-iommu-debug module which expects a 'struct device' parameter
since commit fabdbca instead of an omap_iommu handle. The
omap-iommu-debug code still passed the handle to the function which
caused a crash.
The second patch fixes a NULL pointer dereference in the OMAP code and
the third patch makes sure that the omap-iommu is initialized before the
omap-isp driver, which relies on the iommu. The last patch is only a
workaround until defered probing is implemented.

This is a revert of 6aa56062ea.
This was originally introduced to workaround reads of the ringbuffer
registers returning 0 on SandyBridge causing hangs due to ringbuffer
overflow. The root cause here was reads through the GT powerwell require
the forcewake dance, something we only learnt of later. Now it appears
that reading the reported head position from the HWS is returning
garbage, leading once again to hangs.
For example, on q35 the autoreported head reports:
[ 217.975608] head now 00010000, actual 00010000
[ 436.725613] head now 00200000, actual 00200000

[ 553.273489] head now 00610000, actual 00610018
which appears reasonably sane. In contrast, if we look at snb:
[ 141.970680] head now 00e10000, actual 00008238
[ 141.974062] head now 02734000, actual 000083c8

22) Missing pci_iounmap() in ATM Solos driver. Fix from Julia Lawall.
23) b44_pci_exit() should not have __exit tag since it's invoked from
non-__exit code. From Nikola Pajkovsky.
24) The conversion of the neighbour hash tables over to RCU added a
race, fixed here by adding the necessary reread of tbl->nht, fix
from Michel Machado.
25) When we added VF (virtual function) attributes for network device
dumps, this potentially bloats up the size of the dump of one
network device such that the dump size is too large for the buffer
allocated by properly written netlink applications.
In particular, if you add 255 VFs to a network device, parts of
GLIBC stop working.
To fix this, we add an attribute that is used to turn on these
extended portions of the network device dump. Sophisticaed
applications like 'ip' that want to see this stuff will be changed
to set the attribute, whereas things like GLIBC that don't care
about VFs simply will not, and therefore won't be busted by the
mere presence of VFs on a network device.
Thanks to the tireless work of Greg Rose on this fix.

hwmon: (ads1015) Fix file leak in probe function
hwmon: (max6639) Fix PPR register initialization to set both channels
hwmon: (max6639) Fix FAN_FROM_REG calculation
It used to be that minors where 8 bit. But now they
are actually 20 bit. So the fix is simplicity itself.
I've tested with 300 devices and all user-mode utils
work just fine. I have also mechanically added 10,000
to the ida (so devices are /dev/osd10000, /dev/osd10001 ...)
and was able to mkfs an exofs filesystem and access osds
from user-mode.
All the open-osd user-mode code uses the same library
to access devices through their symbolic names in
/dev/osdX so I'd say it's pretty safe. (Well tested)
This patch is very important because some of the systems
that will be deploying the 3.2 pnfs-objects code are larger
than 64 OSDs and will stop to work properly when reaching
that number.

When pre-allocating skbs for received packets, we set ip_summed =
CHECKSUM_UNNCESSARY. We used to change it back to CHECKSUM_NONE when
the received packet had an incorrect checksum or unhandled protocol.
Commit bc8acf2c8c ('drivers/net: avoid
some skb->ip_summed initializations') mistakenly replaced the latter
assignment with a DEBUG-only assertion that ip_summed ==
CHECKSUM_NONE. This assertion is always false, but it seems no-one
has exercised this code path in a DEBUG build.
Fix this by moving our assignment of CHECKSUM_UNNECESSARY into
efx_rx_packet_gro().

This patch fixes a (mostly cosmetic) bug introduced by the patch
'ppp: Use SKB queue abstraction interfaces in fragment processing'
found here: http://www.spinics.net/lists/netdev/msg153312.html
The above patch rewrote and moved the code responsible for cleaning
up discarded fragments but the new code does not catch every case
where this is necessary. This results in some discarded fragments
remaining in the queue, and triggering a 'bad seq' error on the
subsequent call to ppp_mp_reconstruct. Fragments are discarded
whenever other fragments of the same frame have been lost.
This can generate a lot of unwanted and misleading log messages.
This patch also adds additional detail to the debug logging to
make it clearer which fragments were lost and which other fragments
were discarded as a result of losses. (Run pppd with 'kdebug 1'
option to enable debug logging.)

Adapt omap-iommu-debug to the latest omap-iommu API changes, which
were introduced by commit fabdbca "iommu/omap: eliminate the public
omap_find_iommu_device() method".
In a nutshell, iommu users are not expected to provide the omap_iommu
handle anymore - instead, iommus are attached using their user's device
handle.
omap-iommu-debug is a hybrid beast though: it invokes both public and
private omap iommu API, so fix it as necessary (otherwise a crash
is imminent).
Note: omap-iommu-debug is a bit disturbing, as it fiddles with internal
omap iommu data and requires exposing API which is otherwise not needed.
It should better be more tightly coupled with omap-iommu, to prevent
further bit rot and avoid exposing redundant API. Naturally that's out
of scope for the -rc cycle, so for now just fix the obvious.

This patch fixes a regression that was introduced by
commit 0a5f384677
davinci_emac: Add Carrier Link OK check in Davinci RX Handler
Said commit adds a check whether the carrier link is ok. If the link is
not ok, the skb is freed and no new dma descriptor added to the rx dma
channel. This causes trouble during initialization when the carrier
status has not yet been updated. If a lot of packets are received while
netif_carrier_ok returns false, all dma descriptors are freed and the
rx dma transfer is stopped.
The bug occurs when the board is connected to a network with lots of
traffic and the ifconfig down/up is done, e.g., when reconfiguring
the interface with DHCP.
The bug can be reproduced by flood pinging the davinci board while doing
ifconfig eth0 down
ifconfig eth0 up
on the board.
After that, the rx path stops working and the overrun value reported
by ifconfig is counting up.

For DSS clock domain to transition from idle to active state. It's necessary
to enable the optional clock DSS_FCLK before we enable the module using the
MODULEMODE bits in the clock domain's CM_DSS_DSS_CLKCTRL register.
This sequence was not followed correctly for the 'dss_hdmi' hwmod and it led
to DSS clock domain not getting out of idle when pm_runtime_get_sync() was
called for hdmi's platform device.
Since the clock domain failed to change it's state to active, the hwmod code
disables any clocks it had enabled before for this hwmod. This led to the clock
'dss_48mhz_clk' gettind disabled.
When hdmi's runtime_resume() op is called, the call to dss_runtime_get()
correctly enables the DSS clock domain this time. However, the clock
'dss_48mhz_clk' is needed for HDMI's PHY to function correctly. Since it was
disabled previously, the driver fails when it tries to enable HDMI's PHY.
Fix this for now by ensuring that dss_runtime_get() is called before we call
pm_runtime_get_sync() for hdmi's platform device. A correct fix for later would

In the current kernel implementation, the Logitech Harmony 900 remote
control is matched to the cdc_ether driver through the generic
USB_CDC_SUBCLASS_MDLM entry. However, this device appears to be of the
pseudo-MDLM (Belcarra) type, rather than the standard one. This patch
blacklists the Harmony 900 from the cdc_ether driver and whitelists it for

This provides unified readq()/writeq() helper functions for 32-bit
drivers.
For some cases, readq/writeq without atomicity is harmful, and order of
io access has to be specified explicitly. So in this patch, new two
header files which contain non-atomic readq/writeq are added.
- <asm-generic/io-64-nonatomic-lo-hi.h> provides non-atomic readq/
writeq with the order of lower address -> higher address
- <asm-generic/io-64-nonatomic-hi-lo.h> provides non-atomic readq/
writeq with reversed order
This allows us to remove some readq()s that were added drivers when the
default non-atomic ones were removed in commit dbee8a0aff ("x86:
remove 32-bit versions of readq()/writeq()")
The drivers which need readq/writeq but can do with the non-atomic ones
must add the line:
#include <asm-generic/io-64-nonatomic-lo-hi.h> /* or hi-lo.h */
But this will be nop in 64-bit environments, and no other #ifdefs are
required. So I believe that this patch can solve the problem of
1. driver-specific readq/writeq
2. atomicity and order of io access
This patch is tested with building allyesconfig and allmodconfig as
ARCH=x86 and ARCH=i386 on top of tip/master.

This patch (as1521b) fixes the interaction between usb-storage's
scanning thread and the freezer. The current implementation has a
race: If the device is unplugged shortly after being plugged in and
just as a system sleep begins, the scanning thread may get frozen
before the khubd task. Khubd won't be able to freeze until the
disconnect processing is complete, and the disconnect processing can't
proceed until the scanning thread finishes, so the sleep transition
will fail.
The implementation in the 3.2 kernel suffers from an additional
problem. There the scanning thread calls set_freezable_with_signal(),
and the signals sent by the freezer will mess up the thread's I/O
delays, which are all interruptible.
The solution to both problems is the same: Replace the kernel thread
used for scanning with a delayed-work routine on the system freezable
work queue. Freezable work queues have the nice property that you can
cancel a work item even while the work queue is frozen, and no signals
are needed.
The 3.2 version of this patch solves the problem in Bugzilla #42730.

The xHCI 0.96 spec says that HS bulk and control endpoint NAK rate must
be encoded as an exponent of two number of microframes. The endpoint
descriptor has the NAK rate encoded in number of microframes. We were
just copying the value from the endpoint descriptor into the endpoint
context interval field, which was not correct. This lead to the VIA
host rejecting the add of a bulk OUT endpoint from any USB 2.0 mass
storage device.
The fix is to use the correct encoding. Refactor the code to convert
number of frames to an exponential number of microframes, and make sure
we convert the number of microframes in HS bulk and control endpoints to
an exponent.

The superspeed device attached to a USB 3.0 hub(such as VIA's)
doesn't respond the address device command after resume. The
root cause is the superspeed hub will miss the Hub Depth value
that is used as an offset into the route string to locate the
bits it uses to determine the downstream port number after
reset, and all packets can't be routed to the device attached
to the superspeed hub.
Hub driver sends a Set Hub Depth request to the superspeed hub
except for USB 3.0 root hub when the hub is initialized and
doesn't send the request again after reset due to the resume
process. So moving the code that sends the Set Hub Depth request
to the superspeed hub from hub_configure() to hub_activate()
is to cover those situations include initialization and reset.
The patch should be backported to kernels as old as 2.6.39.

On some systems with an Intel Panther Point xHCI host controller, the
BIOS disables the xHCI PCI device during boot, and switches the xHCI
ports over to EHCI. This allows the BIOS to access USB devices without
having xHCI support.
The downside is that the xHCI BIOS handoff mechanism will fail because
memory mapped I/O is not enabled for the disabled PCI device.
Jesse Barnes says this is expected behavior. The PCI core will enable
BARs before quirks run, but it will leave it in an undefined state, and
it may not have memory mapped I/O enabled.
Make the generic USB quirk handler call pci_enable_device() to re-enable
MMIO, and call pci_disable_device() once the host-specific BIOS handoff
is finished. This will balance the ref counts in the PCI core. When
the PCI probe function is called, usb_hcd_pci_probe() will call
pci_enable_device() again.
This should be back ported to kernels as old as 2.6.31. That was the
first kernel with xHCI support, and no one has complained about BIOS
handoffs failing due to memory mapped I/O being disabled on other hosts
(EHCI, UHCI, or OHCI).

When trying to remove a key, we always send key
flags just setting the key type, not including
the multicast flag and the key ID. As a result,
whenever any key was removed, the unicast key 0
would be removed, causing a complete connection
loss after the second rekey (the first doesn't
cause a key removal). Fix the key removal code
to include the key ID and multicast flag, thus
removing the correct key.
