beb801ac51 RDMA: Move enum ib_cq_creation_flags to uapi headers
The flags field the enum is used with comes directly from the uapi
so it belongs in the uapi headers for clarity and so userspace can
use it.

852f692759 IB/mlx4: Fix incorrectly releasing steerable UD QPs when have only ETH ports
Allocating steerable UD QPs depends on having at least one IB port,
while releasing those QPs does not.

As a result, when there are only ETH ports, the IB (RoCE) driver
requests releasing a qp range whose base qp is zero, with
qp count zero.

When SR-IOV is enabled, and the VF driver is running on a VM over
a hypervisor which treats such qp release calls as errors
(rather than NOPs), we see lines in the VM message log like:

 mlx4_core 0002:00:02.0: Failed to release qp range base:0 cnt:0

Fix this by adding a check for a zero count in mlx4_release_qp_range()
(which thus treats releasing 0 qps as a nop), and eliminating the
check for device managed flow steering when releasing steerable UD QPs.
(Freeing ib_uc_qpns_bitmap unconditionally is also OK, since it
remains NULL when steerable UD QPs are not allocated).

5a371cf87e IB/mlx4: Fix mlx4_ib_alloc_mr error flow
ibmr.device is being set only after ib_alloc_mr() is successfully complete.
Therefore, in case imlx4_mr_enable() returns with error, the error flow
unwinder calls to mlx4_free_priv_pages(), which uses ibmr.device.

Such usage causes to NULL dereference oops and to fix it, the IB device
should be set in the mr struct earlier stage (e.g. prior to calling
mlx4_free_priv_pages()).

07d84f7b6a IB/mlx4: Add support to RSS hash for inner headers
Support RSS hash for inner headers according to a new flag,
MLX4_IB_RX_HASH_INNER provided by the vendor channel.

In case the flag is set, RSS hash will be done on the inner headers of
VXLAN packets (which are encapsulated).
Non-encapsulated packets will be hashed according to the outer headers.

Smatch complains about this code:

    drivers/infiniband/hw/mlx4/qp.c:1827 _mlx4_set_path()
    error: buffer overflow 'dev->dev->caps.gid_table_len' 3 <= 255

The mlx4_ib_gid_index_to_real_index() does check that "port" is within
bounds, but we don't check the return value for errors.  It seems simple
enough to add a check for that.

4d02ebd9bb IB/mlx4: Fix RSS hash fields restrictions
Mistakenly the driver didn't allow RSS hash fields combinations which
involve both IPv4 and IPv6 protocols. This bug caused to failures for
user's use cases for RSS.

Consequently, this patch fixes this bug and allows any combination that
the HW can support.

Additionally, the patch fixes the driver to return an error in case the
user provides an unsupported mask for RSS hash fields.

5f22a1d87c IB/mlx4: Increase maximal message size under UD QP
Maximal message should be used as a limit to the max message payload allowed,
without the headers. The ConnectX-3 check is done against this value includes
the headers. When the payload is 4K this will cause the NIC to drop packets.

Increase maximal message to 8K as workaround, this shouldn't change current
behaviour because we continue to set the MTU to 4k.

To reproduce;
set MTU to 4296 on the corresponding interface, for example:
ifconfig eth0 mtu 4296 (both server and client)

On server:
ib_send_bw -c UD -d mlx4_0 -s 4096 -n 1000000 -i1 -m 4096

On client:
ib_send_bw -d mlx4_0 -c UD <server_ip> -s 4096 -n 1000000 -i 1 -m 4096

Optimize the device performance by assigning multiple physical pages,
which are contiguous, to a single MTT. As a result, the number of MTTs
is reduced and in turn save cache misses of MTTs.

108809a057 IB/mlx4: Fix RSS's QPC attributes assignments
In the modify QP handler the base_qpn_udp field in the RSS QPC is
overwrite later by irrelevant value assignment. Hence, ingress packets
which gets to the RSS QP will be steered then to a garbage QPN.

The patch fixes this by skipping the above assignment when a RSS QP is
modified, also, the RSS context's attributes assignments are relocated
just before the context is posted to avoid future issues like this.

Additionally, this patch takes the opportunity to change the code to be
disciplined to the device's manual and assigns the RSS QP context just at
RESET to INIT transition.

c0348eb069 IB: Let ib_core resolve destination mac address
Since IB/core resolves the destination mac address for user and kernel
consumers, avoid resolving in multiple provider drivers.

Only ib_core resolves DMAC now, therefore resolve_eth_dmac is removed as

62ede77799 Add OPA extended LID support

9abb0d1bbd RDMA: Simplify get firmware interface
There is a need to forward FW version to user space
application through RDMA netlink. In order to make it safe, there
is need to declare nla_policy and limit the size of FW string.

The new define IB_FW_VERSION_NAME_MAX will limit the size of
FW version string. That define was chosen to be equal to
ETHTOOL_FWVERS_LEN, because many drivers anyway are limited
by that value indirectly.

The introduction of this define allows us to remove the string size
from get_fw_str function signature.

3078f5f1bd IB/mlx4: Add support for RSS QP
Add support to work with a RSS QP by using an indirection table object
upon QP creation. Other related QP verbs (e.g. modify/destroy/query) were
updated as well for that QP mode.

Notes:
- The RX hash properties are supplied as driver private data.
- The RSS QP port is used on the associated WQs in its indirection
  table. Applying different ports during WQ life time is not allowed.
- The expected RSS QP flow is: create, modify(RST->INIT),
  modify(RST->RTR), destroy.

400b1ebcfe IB/mlx4: Add support for WQ related verbs
Support create/modify/destroy WQ related verbs.

The base IB object to enable RSS functionality is a WQ (i.e. ib_wq).
This patch implements the related WQ verbs as of create, modify and
destroy.

In downstream patches the WQ will be used as part of an indirection
table (i.e. ib_rwq_ind_table) to enable RSS QP creation.

Notes:
ConnectX-3 hardware requires consecutive WQNs list as receive descriptor
queues for the RSS QP. Hence, the driver manages consecutive ranges lists
per context which the user must respect.
Destroying the WQ does not return its WQN back to its range for
reusing. However, destroying all WQs from the same range releases the
range and in turn releases its WQNs for reusing.

Since the WQ object is not a natural object in the hardware, the driver
implements the WQ by the hardware QP.

As such, the WQ inherits its port from its RSS QP parent upon its
RST->INIT transition and by that time its state is applied to the
hardware.

f330187016 (IB, net)/mlx4: Add resource utilization support
Adding visibility of resource usage of QPs, CQs and counters used by
virtual functions. This feature will be used to give the PF administrator
more data while debugging VF status. Usage info was added to ALLOC_RES
command, to notify the PF if the resource which is being reserved or
allocated for the VF will be used by kernel driver or by user verbs.

Updated reservation and allocation functions of QP, CQ and counter with
additional usage parameter.

ea30b966f7 IB/mlx4: Add inline-receive support
When inline-receive is enabled, the HCA may write received
data into the receive WQE.

Inline-receive is enabled by setting its matching bit in
the QP context and each single-packet message with payload
not exceeding the receive WQE size will be delivered to
the WQE.

The completion report will indicate that the payload was placed to the WQE.

It includes:
1) Return maximum supported size of inline-receive by the hardware
in query_device vendor's data part.
2) Enable the feature when requested by the vendor data input.

4542e3c79a IB/mlx4: Fix CM REQ retries in paravirt mode
CM REQs cannot be successfully retried, because a new pv_cm_id is
created for each request, without checking if one already exists.

By checking if an id exists before creating one, the bug is fixed.

This bug can be provoked by running an RDMA CM user-land application,
but inserting a five seconds delay before the rdma_accept() call on
the passive side. This delay is larger than the default CMA timeout,
and triggers a retry from the active side. The retried REQ will use
another pv_cm_id (the cm_id on the wire). This confuses the CM
protocol and two REJs are sent from the passive side.

The caller to the driver marks GFP_NOIO allocations with help
of memalloc_noio-* calls now. This makes redundant to pass down
to the driver gfp flags, which can be GFP_KERNEL only.

The patch removes the gfp flags argument and updates all driver paths.

eed7624552 RDMA/mlx4: Fix MAD tunneling when SRIOV is enabled
The cited patch added a type field to structures ib_ah and rdma_ah_attr.

Function mlx4_ib_query_ah() builds an rdma_ah_attr structure from the
data in an mlx4_ib_ah structure (which contains both an ib_ah structure
and an address vector).

For mlx4_ib_query_ah() to work properly, the type field in the contained
ib_ah structure must be set correctly.

In the outgoing MAD tunneling flow, procedure mlx4_ib_multiplex_mad()
paravirtualizes a MAD received from a slave and sends the processed
mad out over the wire. During this processing, it populates an
mlx4_ib_ah structure and calls mlx4_ib_query_ah().

The cited commit overlooked setting the type field in the contained
ib_ah structure before invoking mlx4_ib_query_ah(). As a result, the
type field remained uninitialized, and the rdma_ah_attr structure was
incorrectly built. This resulted in improperly built MADs being sent out
over the wire.

This patch properly initializes the type field in the contained ib_ah
structure before calling mlx4_ib_query_ah(). The rdma_ah_attr structure
is then generated correctly.
