The ttusb2_msg function uses on-stack variables to submit commands to
dvb_usb_generic. This eventually gets to the DMA api layer and will throw a
traceback if the debugging options are set.
This allocates the temporary buffer variables with kzalloc instead.

dvb-usb-it9137-01.fw does not work with version 2 chips

Leaving this code enabled breaks the build on some architectures,
and we shouldn't have any floating point math in the kernel, anyway.
These macros need to be re-written, but it's harmless to simply
return zero for now.

There are numerous broken references to Documentation files (in other
Documentation files, in comments, etc.). These broken references are
caused by typo's in the references, and by renames or removals of the
Documentation files. Some broken references are simply odd.
Fix these broken references, sometimes by dropping the irrelevant text
they were part of.
