Since unifying ringbuffer/execlist submission to use
engine->pin_context, we ensure that the intel_ring is available before
we start constructing the request. We can therefore move the assignment
of the request->ring to the central i915_gem_request_alloc() and not
require it in every engine->request_alloc() callback. Another small step
towards simplification (of the core, but at a cost of handling error
pointers in less important callers of engine->pin_context).

When test GVTg as below scenario:
VM boot --> failsafe --> kill qemu --> VM boot.
Qemu report error at the second boot:
ERROR: PCI region size must be pow2 type=0x0, size=0x1fa1000
Qemu need access PCI_ROM_ADDRESS reg to determine the size of expansion
PCI rom. The mechanism just like the BAR reg (write-read) and we should
return the size 0 since we have no rom. If we reject the write to
PCI_ROM_ADDRESS, Qemu cannot get the correct size of rom.
Essentially, GVTg failsafe mode should not break PCI function. So we
exclude cfg space from failsafe mode. This can fix above issue.

This patch introduces two functions for activating/de-activating vGPU in
mdev ops.
A racing condition was found between virtual vblank emulation and KVGMT
mdev release path. V-blank emulation will emulate and inject V-blank
interrupt for every active vGPU with holding gvt->lock, while in mdev
release path, it will directly release hypervisor handle without changing
vGPU status or taking gvt->lock, so a kernel oops is encountered when
vblank emulation is injecting a interrupt with a invalid hypervisor
handle. (Reported by Terrence)
To solve this problem, we factor out vGPU activation/de-activation from
vGPU creation/destruction path and let KVMGT mdev release ops de-activate
the vGPU before release hypervisor handle. Once a vGPU is de-activated,
GVT-g will not emulate v-blank for it or touch the hypervisor handle.

GVTg has introduced the context status notifier to schedule the GVTg
workload. At that time, the notifier is bound to GVTg context only,
so GVTg is not aware of host workloads.
Now we are going to improve GVTg's guest workload scheduler policy,
and add Guc emulation support for new Gen graphics. Both these two
features require acknowledgment for all contexts running on hardware.
(But will not alter host workload.) So here try to make some change.
The change is simple:
1. Move the context status notifier head from i915_gem_context to
intel_engine_cs. Which means there is a notifier head per engine
instead of per context. Execlist driver still call notifier for
each context sched-in/out events of current engine.
2. At GVTg side, it binds a notifier_block for each physical engine
at GVTg initialization period. Then GVTg can hear all context
status events.
In this patch, GVTg do nothing for host context event, but later
will add a function there. But in any case, the notifier callback is
a noop if this is no active vGPU.
Since intel_gvt_init() is called at early initialization stage and
require the status notifier head has been initiated, I initiate it in
intel_engine_setup().
v2: remove a redundant newline. (chris)

The kvmgt code keeps a pointer to the struct kvm associated with the
device, but doesn't actually hold a reference to it. If we do unclean
shutdown testing (ie. killing the user process), then we can see the
kvm association to the device unset, which causes kvmgt to trigger a
device release via a work queue. Naturally we cannot guarantee that
the cached struct kvm pointer is still valid at this point without
holding a reference. The observed failure in this case is a stuck
cpu trying to acquire the spinlock from the invalid reference, but
other failure modes are clearly possible. Hold a reference to avoid
this.

When handling guest request, GVT needs to populate/update shadow_ctx
with guest context. This behavior needs to make sure the shadow_ctx

Since intel_gvt_init() is called at early initialization stage and
require the status notifier head has been initiated, I initiate it in
intel_engine_setup().
v2: remove a redundant newline. (chris)

Per PCI specification, Configuration Register has different types (RO,
RW, RW1C, Rsvd). For RO Register bits are read-only and cannot be
altered by software. For RW1C Register bits indicate status when read.
A Set bit indicates a status event which is Cleared by writing a 1b.
Writing a 0b to RW1C bits has no effect. Reserved Register is for future
implementations, and they are read-only and must return zero when read.
Current vGPU configuration write emulation just copy the value as it is.
So we haven't emulated RO, RW1C and Rsvd Registers correctly. This patch
is following the Spec to correct emulation logic. We add a function
vgpu_cfg_mem_write to wrap the access to vGPU configuration memory.
The write function uses a RW Register bitmap to avoid RO bits be
overwritten, and emulate RW1C behavior for the particular status Register.

the request can be replayed after a GPU reset. With this mechanism,
gvt should wait until the GVT request seqno passed before complete
the current workload. So that there should be a context switch interrupt
come before gvt free the workload. In this way, workload lifecylce
matches with the i915 request lifecycle. The workload can only be freed

We need to split type name vs. actual mdev resource and create fixed

With this we'd like to fix vGPU types for _1, _2, _4 and _8 now, each
type has fixed defined resource size. Available mdev instances that could

send_display_send_uevent() sends two environment variable, and the
first one GVT_DISPLAY_READY is set including a new line at the end of
the string; that is obviously superfluous and wrong -- at least, it
*looks* so when you only read the code.
However, it doesn't appear in the actual output by a (supposedly
unexpected) trick. The code uses snprintf() and truncates the string
in size 20 bytes. This makes the string as GVT_DISPLAY_READY=0 or
...=1 including the trailing NUL-letter. That is, the '\n' found in
the format string is always cut off as a result.
Although the code gives the correct result, it is confusing. This
patch addresses it, just removing the superfluous '\n' from the format
string for avoiding further confusion. If the argument "ready" were
not a bool, the size 20 should be corrected as well. But it's a
bool, so we can leave the magic number 20 as is for now.
FWIW, the bug was spotted by a new GCC7 warning:

This removes the usage of intel_ring_emit in favour of
directly writing to the ring buffer.
intel_ring_emit was preventing the compiler for optimising
fetch and increment of the current ring buffer pointer and
therefore generating very verbose code for every write.
It had no useful purpose since all ringbuffer operations
are started and ended with intel_ring_begin and
intel_ring_advance respectively, with no bail out in the
middle possible, so it is fine to increment the tail in
intel_ring_begin and let the code manage the pointer
itself.
Useless instruction removal amounts to approximately
two and half kilobytes of saved text on my build.

MFN usually refers to "Machine Frame Number" in virtulization world.
Currently GVT-g populates the shadow PPGTT/GGTT page table with MFN
according to the translation between MFN and Guest PFN.
When host IOMMU is enabled, GPU DMA transactions go through the IOMMU,
GPU needs an IOVA<->MFN mapping to walk the shadow page table in host
main memory.
This patch will map a page in IOMMU page table before using it in shadow
page table and release the map when a shadow page is going to be freed.

Our function tests found several issues related to reusing vGPU
instance. They are qemu reboot failure, guest tdr after reboot, host
hang when reboot guest. All these issues are caused by dirty status
inherited from last VM.
This patch fix all these issues by resetting a virtual GPU before VM
use it. The reset logical is put into a low level function
_intel_gvt_reset_vgpu(), which supports Device Model Level Reset, Full
GT Reset and Per-Engine Reset.
vGPU Device Model Level Reset (DMLR) simulates the PCI reset to reset
the whole vGPU to default state as when it is created, including GTT,
execlist, scratch pages, cfg space, mmio space, pvinfo page, scheduler
and fence registers. The ultimate goal of vGPU DMLR is that reuse a
vGPU instance by different virtual machines. When we reassign a vGPU
to a virtual machine we must issue such reset first.
Full GT Reset and Per-Engine GT Reset are soft reset flow for GPU engines
(Render, Blitter, Video, Video Enhancement). It is defined by GPU Spec.
Unlike the FLR, GT reset only reset particular resource of a vGPU per
the reset request. Guest driver can issue a GT reset by programming
the virtual GDRST register to reset specific virtual GPU engine or all
engines.
Since vGPU DMLR and GT reset can share some code so we implement both
these two into one single function intel_gvt_reset_vgpu_locked(). The
parameter dmlr is to identify if we will do FLR or GT reset. The
parameter engine_mask is to specific the engines that need to be
resetted. If value ALL_ENGINES is given for engine_mask, it means
the caller requests a full gt reset that we will reset all virtual
GPU engines.

The current prototype of new_mmio_info() uses void* for parameters read
and write, which are functions with precise calling conventions
(argument types and return type). Write down these conventions in
new_mmio_info() definition.
This has been reported by the following warnings when clang is used to
build the kernel:
drivers/gpu/drm/i915/gvt/handlers.c:124:21: error: pointer type
mismatch ('void *' and 'int (*)(struct intel_vgpu *, unsigned int,

info->write = write ? write : intel_vgpu_default_mmio_write;

Previously to read/write a GPA, we at first try to pin the GFN it belongs
to, then translate the pinned PFN to a kernel HVA, then read/write it.
This is however not necessary. A GFN should be pinned IFF it would be
accessed by peripheral devices (DMA), not by CPU. This patch changes
the read/write method to KVM API, which will leverage userspace HVA
and copy_{from|to}_usr instead.

KVMGT is the MPT implementation based on VFIO/KVM. It provides
a kvmgt_mpt ops to gvt for vGPU access mediation, e.g. to
mediate and emulate the MMIO accesses, to inject interrupts
to vGPU user, to intercept the GTT writing and replace it with
DMA-able address, to write-protect guest PPGTT table for
shadowing synchronization, etc. This patch provides the MPT
implementation for GVT, not yet functional due to theabsence
of mdev.
It's built as kvmgt.ko, depends on vfio.ko, kvm.ko and mdev.ko,
and being required by i915.ko. To not introduce hard dependency
in i915.ko, we used indirect symbol reference. But that means
users have to include kvmgt.ko into init ramdisk if their
i915.ko is included.

There are currently 4 methods in intel_gvt_io_emulation_ops
to emulate CFG/MMIO reading/writing for intel vGPU. A possibly
better scope is: add 3 more methods for vgpu create/destroy/reset
respectively, and rename the ops to 'intel_gvt_ops', then pass
it to the MPT module (say the future kvmgt) to use: they are
all methods for external usage.

Hypervisors are different, the MPT ops is a only superset of
all possibly supported hypervisors. There might be other way
out of the MPT to achieve same target. e.g. vfio-based kvmgt
won't provide map_gfn_to_mfn method to establish guest EPT
mapping for aperture, since it will be done in QEMU/KVM, MMIO
is also trapped elsewhere, etc.

Our low-level wait routine has evolved from our generic wait interface
that handled unlocked, RPS boosting, waits with time tracking. If we
push our GEM fence tracking to use reservation_objects (required for
handling multiple timelines), we lose the ability to pass the required
information down to i915_wait_request(). However, if we push the extra
functionality from i915_wait_request() to the individual callsites
(i915_gem_object_wait_rendering and i915_gem_wait_ioctl) that make use
of those extras, we can both simplify our low level wait and prepare for
extending the GEM interface for use of reservation_objects.
